Abstract

Frontal fibrosing alopecia (FFA) is a recently described inflammatory and scarring type of hair loss affecting almost exclusively women. Despite a dramatic recent increase in incidence the aetiopathogenesis of FFA remains unknown. We undertake genome-wide association studies in females from a UK cohort, comprising 844 cases and 3,760 controls, a Spanish cohort of 172 cases and 385 controls, and perform statistical meta-analysis. We observe genome-wide significant association with FFA at four genomic loci: 2p22.2, 6p21.1, 8q24.22 and 15q2.1. Within the 6p21.1 locus, fine-mapping indicates that the association is driven by the HLA-B*07:02 allele. At 2p22.1, we implicate a putative causal missense variant in CYP1B1, encoding the homonymous xenobiotic- and hormone-processing enzyme. Transcriptomic analysis of affected scalp tissue highlights overrepresentation of transcripts encoding components of innate and adaptive immune response pathways. These findings provide insight into disease pathogenesis and characterise FFA as a genetically predisposed immuno-inflammatory disorder driven by HLA-B*07:02.

Abstract

Summary: Background: A transition from a subtyping to a phenotyping approach in rosacea is underway, allowing individual patient management according to presenting features instead of categorization by predefined subtypes. The ROSacea COnsensus (ROSCO) 2017 recommendations further support this transition and align with guidance from other working groups. Objectives: To update and extend previous global ROSCO recommendations in line with the latest research and continue supporting uptake of the phenotype approach in rosacea through clinical tool development. Methods: Nineteen dermatologists and two ophthalmologists used a modified Delphi approach to reach consensus on statements pertaining to critical aspects of rosacea diagnosis, classification and management. Voting was electronic and blinded. Results: Delphi statements on which the panel achieved consensus of ≥ 75% voting 'Agree' or 'Strongly agree' are presented. The panel recommends discussing disease burden with patients during consultations, using four questions to assist conversations. The primary treatment objective should be achievement of complete clearance, owing to previously established clinical benefits for patients. Cutaneous and ocular features are defined. Treatments have been reassessed in line with recent evidence and the prior treatment algorithm updated. Combination therapy is recommended to benefit patients with multiple features. Ongoing monitoring and dialogue should take place between physician and patients, covering defined factors to maximize outcomes. A prototype clinical tool (Rosacea Tracker) and patient case studies have been developed from consensus statements. Conclusions: The current survey updates previous recommendations as a basis for local guideline development and provides clinical tools to facilitate a phenotype approach in practice and improve rosacea patient management. Abstract

Background: Melanoma incidence among white South Africans is increasing. Among black individuals, melanoma is associated with advanced stage at presentation and significant mortality.Objectives: A study was undertaken to assess the perception of skin cancer risk, knowledge of skin cancer, and understanding of the importance and use of sunscreens among South Africans.Methods: A cross-sectional survey was conducted in general outpatients over four months at a large central hospital in Durban, South Africa.Results: Only half the white respondents reported regular use of a sunscreen. Among black and Indian respondents, the number was substantially lower. Less than 20% of white respondents had ever checked their skin for suspicious moles. Most black patients were not aware that they are at risk of skin cancer, and only 10% were aware of the risk of developing skin cancers on acral sites and nails.Conclusions: There is a worrying lack of knowledge about skin cancer and sun protection behaviours among all South Africans. Given the increase in melanoma incidence and racial disparities in survival rates, it is imperative to target each population with effective, culturally sensitive educational programmes.Keywords: education, melanoma, prevention, screening, skin cancer, South Africa, suns

Skin and skin pigmentation have both personal and societal effects on an individual. In a 2020 study of six dermatology textbooks and two online resources, 10-3% of over 3469 textbook images were in darker skin phototypes. Despite the increase in populations with darker skin phototypes in the USA and Europe, dermatology images in textbooks have overwhelmingly been of lighter skin phototypes.2,3 This presents a particular challenge to dermatology teaching in South Africa, for example, where 70% of the population has skin types V to VI.
