Abstract

Aims International studies have examined patients' views concerning the discussion of deactivating an implantable cardioverter defibrillator (ICD). Findings reported that many patients were either not informed about the subject or were informed late in their illness trajectory. This study aimed to explore healthcare professionals' perspectives on discussing ICD deactivation and identify priorities for clinical practice and future research.Methods and results Eleven interviews were conducted, involving heart failure nurses, physicians, and an allied professional. All were responsible for the care of patients with an ICD, from the UK or Sweden. A semi-structured guide was used. All interviews were audio-recorded, transcribed, translated (as applicable), and analysed independently by experienced researchers, using framework analysis. Findings were presented, along with published work at a stakeholder meeting, and a consensus agreement was reached on priorities for clinical practice and future research. Four themes emerged from the exploratory interviews. Healthcare professionals described the discussion about deactivation as challenging, requiring compassion and involvement of family members. They agreed that the topic should be initiated prior to, or shortly after, device implantation. This was reflected in the priorities to improve communication, through the increased availability and implementation of prompts and tools, as well as the provision of tailored information to patients and family members. Stakeholders recognized the future potential of digital technology in device education.Conclusion Discussing deactivation remains challenging. Healthcare professionals recognized the need to initiate the discussion early, with compassion and involvement of family members. 15. Using ERIC on CD/ROM (A Guide to the Basics).

Abstract

This guide provides basic instruction on how to search the ERIC database on the SilverPlatter CD-ROM, which is the computerized equivalent to Current Index to Journals in Education (CIJE) and Resources in Education (RIE). The entire database is available on three discs, from 1966 to the present. Included in this guide is a description of the ERIC network and its scope as well as discussions of when it is most appropriate to search ERIC's print version, ERIC online, or ERIC on CD-ROM. Sample ERIC journal and document entries which are tagged to identify an entry's fields are followed by a list of the 11 basic fields with brief descriptions and examples of how to search the specific fields. A search example which utilizes Boolean logic demonstrates two methods of searching by subject headings/descriptors; the free text search technique is also discussed. An example of how this strategy would look on CD-ROM is provided and a sample entry is included. Other methods which can be used to refine a search (e.g., searching only the major descriptors, limiting by document type, limiting by education level descriptors, or limiting by year) are included. A list of descriptors that correspond to the publication type categories is provided as well as definitions of the mandatory educational level descriptors. The economic downturn of 2007-08 forced many academic libraries in the United States to cancel journals. We surveyed life sciences librarians from ARL libraries to find out about their experiences with journal cancellations during 2008-12. Overall, we discovered that two factors were essential in decision-making: faculty consultations and data. While faculty consultations and data have always been influential in journal cancellations, our survey allowed us to explore what roles these two factors played in the recent period of necessary reductions.

Abstract

Introduction: Cardiac cachexia is a wasting syndrome that may present in heart failure, with clinical identification being challenging. This study explored the difficulties with applying the recommended diagnostic criteria for cardiac cachexia in heart failure patients. Methods: A cross-sectional study was conducted in 200 patients with NYHA III-IV heart failure. Patients were assessed for cachexia based on the Evans et al. (2008) diagnostic criteria: 5% weight loss in ≤ 12 months or BMI < 20 kg/m2 plus 3 of: 1) Decreased muscle strength, 2) Fatigue, 3) Anorexia, 4) Low fat-free mass index, and 5) Abnormal biochemistry (elevated inflammatory markers (CRP & IL-6), anaemia (Hb Results: 30 out of 200 participants (15%) were identified with cachexia. The cachectic group had significantly (p < 0.05) lower BMI, fat-free mass index, muscle strength, red blood cell count, and albumin; and significantly higher C-reactive protein, fatigue, and anorexia issues. Oedema was present in 60.5% of the sample and 85% of patients possessed an average BMI of 29.9 kg/m2 (borderline of obese classification). 10.5% of participants met the BMI cut-off of < 20 kg/m2 from the cachexia diagnostic criteria. Conclusions: The recommended diagnostic criteria identified a 15% prevalence of cachexia in patients with advanced heart failure. However, oedema may overshadow the detection of weight loss and muscle wasting. Moreover, a higher BMI cut-off value for the diagnostic criteria may be required. Future research should identify cachexia biomarkers to improve clinical identification of the syndrome.

The sequential approach had two phases, with phase 1 including 200 patients with NYHA III-IV heart failure assessed for characteristics of cardiac cachexia. Abstract

Background: Cardiac cachexia is a multifactorial wasting syndrome, which is characterised by unintentional weight loss, a reduction in skeletal muscle mass, and reduced quality of life. There is a paucity of qualitative research outlining the impact and experience of cardiac cachexia in advanced heart failure patients and their caregivers. Research in this area may promote a better understanding of the syndrome, enhance current treatment strategies, and highlight priorities for patient care. Purpose: To qualitatively investigate the impact and experience of cardiac cachexia in advanced heart failure patients and their caregivers. Methods: Semi-structured interviews were performed with advanced heart failure (NYHA functional class III-IV) patients meeting diagnostic criteria for cardiac cachexia (number (n) = 8) and their caregivers (n = 5). Interview questions followed a “laddered style approach”, with a focus on determining the experience of cachexia for patients and carers, and its impact on their daily lives. Interviews were digitally recorded and transcribed verbatim. Average interview duration was 43 (15-64) minutes. Data were thematically analysed, using the 6-step approach of Braun & Clarke. Themes were developed and refined by several members of the research team to ensure rigor. Results: Four key themes were developed from the data. 1) “Changed relationship with food and eating”: patients with cachexia referred to eating as something they now ‘make’ themselves do without enjoyment, often just to placate their caregiver. Caregivers noted this change in habit and were concerned about inadequate nutritional intake due to the patient’s lack of interest in food. 2) “Not me in the mirror”: patients struggled with their appearance and had a negative perception of themselves, linked to their weight loss. Caregivers were similarly aware of the physical changes in their loved ones and emotionally impacted. 3) “Lack of understanding regarding cachexia”: despite the patient and caregivers’ concerns, they had a perception that healthcare professionals were just ‘fobbing you off’. Even though weight loss was noted and distressing to patients and caregivers, there was little clinical recognition of it, nor any advice or support from the clinical team regarding management. 4) “Uncertainty regarding the future”: patients and caregivers recognised cachexia as a bad sign, and many expressed concerns about their future health and prognosis.Conclusion: These novel qualitative findings highlight the severe impact of cardiac cachexia on the daily lives of patients and caregivers, and demonstrate their limited understanding of the syndrome. To improve care, professionals need to recognise and discuss cachexia with heart failure patients and caregivers, supporting them to cope with prognostic implications and develop more effective management strategies.

Abstract

Cancer therapeutics-related cardiac dysfunction (CTRCD) has emerged as a major cause of morbidity and mortality in cancer survivors. Effective clinical management of CTRCD is impeded by a lack of sensitive diagnostic and prognostic strategies. Circulating molecular markers could potentially address this need as they are often indicative of cardiac stress before cardiac damage can be detected clinically. A growing understanding of the underlying physiological mechanisms for CTRCD has inspired research efforts to identify novel pathophysiologically relevant biomarkers that may also guide development of cardio-protective therapeutic approaches. The purpose of this review is to evaluate current circulating biomarkers of cardiac stress and their potential role in diagnosis and management of CTRCD. Abstract

Background: While studies have evaluated caregiver outcomes in heart failure, the burden and support needs when caring for someone with advanced heart failure at the end of life have yet to be outlined. Aim: To identify psychosocial factors associated with caregiver burden and evaluate the support needs of caregivers in advanced heart failure. Design: A sequential mixed methods study comprising two phases: (1) postal survey with advanced heart failure patients and their caregivers and (2) interviews with current and bereaved caregivers. Correlation, chi-square, t test, regression and thematic analysis were undertaken on the data. Participants: Advanced heart failure patients (n = 112) and their caregivers (n = 84) were recruited from secondary care settings across the United Kingdom and Ireland. For phase 2 interviews, current caregivers (n = 20) were purposively recruited from phase 1, and bereaved caregivers (n = 10) were purposively recruited via voluntary organisation, social media and email. Results: More than half the caregivers (53%) had levels of distress associated with depression (Zarit Burden score >24). Caregiver depression score, preparedness for caregiving and patients' depression score predicted caregiver burden. Qualitative analysis identified an overarching theme of lack of future care planning and four subthemes: (1) seeking emotional support from someone who understands, (2) want information on prognostication, (3) lack of knowledge on how to and where to get support and (4) require knowledge on what to expect at the end of life. Conclusion: Caregivers have unmet needs and feel unprepared for the future. Background: Whether revascularization by percutaneous coronary intervention (PCI) can improve event-free survival and left ventricular function in patients with severe ischemic left ventricular systolic dysfunction, as compared with optimal medical therapy (i.e., individually adjusted pharmacologic and device therapy for heart failure) alone, is unknown.Methods: We randomly assigned patients with a left ventricular ejection fraction of 35% or less, extensive coronary artery disease amenable to PCI, and demonstrable myocardial viability to a strategy of either PCI plus optimal medical therapy (PCI group) or optimal medical therapy alone (optimal-medical-therapy group). The primary composite outcome was death from any cause or hospitalization for heart failure. Major secondary outcomes were left ventricular ejection fraction at 6 and 12 months and quality-of-life scores.Results: A total of 700 patients underwent randomization - 347 were assigned to the PCI group and 353 to the optimal-medical-therapy group. Over a median of 41 months, a primary-outcome event occurred in 129 patients (37.2%) in the PCI group and in 134 patients (38.0%) in the optimal-medical-therapy group (hazard ratio, 0.99; 95% confidence interval [CI], 0.78 to 1.27; P = 0.96). The left ventricular ejection fraction was similar in the two groups at 6 months (mean difference, -1.6 percentage points; 95% CI, -3.7 to 0.5) and at 12 months (mean difference, 0.9 percentage points; 95% CI, -1.7 to 3.4). Quality-of-life scores at 6 and 12 months appeared to favor the PCI group, but the difference had diminished at 24 months.Conclusions: Among patients with severe ischemic left ventricular systolic dysfunction who received optimal medical therapy, revascularization by PCI did not result in a lower incidence of death from any cause or hospitalization for heart failure.
