1. An impulsive geomagnetic effect from an early-impulsive flare

Abstract

The geomagnetic "solar flare effect" (SFE) results from excess ionization in the Earth's ionosphere, famously first detected at the time of the Carrington flare in 1859. This indirect detection of a flare constituted one of the first cases of "multimessenger astronomy," whereby solar ionizing radiation stimulates ionospheric currents. Well-observed SFEs have few-minute time scales and perturbations of >10 nT, with the greatest events reaching above 100 nT. In previously reported cases the SFE time profiles tend to resemble those of solar soft X-ray emission, which ionizes the D-region; there is also a less-well-studied contribution from Lyman-alpha. We report here a specific case, from flare SOL2024-03-10 (M7.4), in which an impulsive SFE deviated from this pattern. This flare contained an "early impulsive" component of exceptionally hard radiation, extending up to gamma-ray energies above 1 MeV, distinctly before the bulk of the flare soft X-ray emission. We can characterize the spectral distribution of this early-impulsive component in detail, thanks to the modern extensive wavelength coverage. A more typical gradual SFE occurred during the flare's main phase. Abstract

PURPOSE: To report the results of OPAL (ClinicalTrials.gov identifier: NCT03574779) cohort A, a single-arm substudy of niraparib plus dostarlimab and bevacizumab for the treatment of advanced, platinum-resistant ovarian cancer (PROC). METHODS: Participants with PROC who received 1-2 previous lines of therapy were treated with niraparib (200 or 300 mg once daily), dostarlimab (500 mg once every 3 weeks for four 21-day cycles, followed by 1,000 mg once every 6 weeks), and bevacizumab (15 mg/kg once every 3 weeks). The primary end point was investigator-assessed objective response rate (ORR) per RECIST v1.1. Safety was also assessed. Exploratory biomarker end points included evaluation of changes in the tumor molecular profile and microenvironment using baseline and on-treatment tumor samples. RESULTS: Of 41 enrolled participants (median age, 66.0 years [range, 37-83 years]), 9.8% had tumors that were BRCA-mutated, 19.5% were homologous recombination (HR)-deficient, and 17.1% were HR repair (HRR)-mutated. As of the cutoff date, all participants discontinued treatment. The ORR was 17.1% (80% CI, 9.8 to 27.0), including one complete response (2.4%); the disease control rate was 73.2% (80% CI, 62.3 to 82.2). Two participants withdrew before first postbaseline scan because of adverse events (AEs). Grade ≥3 treatment-emergent AEs were reported in 92.7% of participants, with the most common being hypertension (26.8%). Response was not correlated with BRCA, HRR, HR deficiency (HRD), or PD-L1 status. Changes suggesting immune activation were observed in on-treatment samples after triplet therapy. CONCLUSION: Results demonstrated modest activity of niraparib, dostarlimab, and bevacizumab in participants with PROC, many of whom had prognostic factors for poor treatment response. Most participants with response were bevacizumab-naïve. No association was found with HRD, BRCA, or PD-L1 status. AEs were consistent with previous monotherapy reports, except that hypertension was reported more frequently.

Abstract

BACKGROUND: Medulloblastoma is the most common malignant brain tumor in children. Most cases are sporadic, but well characterized germline alterations in APC, ELP1, GPR161, PTCH1, SUFU, and TP53 predispose to medulloblastoma. However, knowledge about pathogenic/likely pathogenic (P/LP) variants that predispose to medulloblastoma vary based on genes evaluated, patient demographics, and pathogenicity definitions. METHODS: Germline exome sequencing was conducted on 160 childhood survivors of medulloblastoma. Analyses focused on rare variants in 239 known cancer susceptibility genes (CSGs). P/LP variants were identified using ClinVar and InterVar. Variants of unknown significance in known medulloblastoma predisposing genes (APC, ELP1, GPR161, PTCH1, SUFU, TP53) were further classified for loss of function variants. We compared the frequency of P/LP variants in cases to that in 1,259 cancer-free adult controls. RESULTS: Twenty cases (12.5%) had a P/LP variant in an autosomal dominant CSG versus 5% in controls (p=1.0 x10-3), and 10 (6.3%) of these were P/LP variants in a known medulloblastoma gene, significantly greater than 0.2% observed in controls (p=1.4x10-8). The CSGs with the most P/LP variants in cases, and significantly higher than controls, were ELP1 (p=3.0x10-4) and SUFU (p=1.4x10-3). CONCLUSION: Approximately one in eight pediatric medulloblastoma survivors had an autosomal dominant P/LP CSG variant. We confirm several known associated genes and identify novel genes that may be important in medulloblastoma.

Abstract

Abstract Background A better understanding of lung cancer etiology and the development of screening biomarkers have important implications for lung cancer prevention. Methods We included 623 matched case–control pairs from the Cancer Prevention Study (CPS) cohorts. Pre-diagnosis blood samples were collected between 1998 and 2001 in the CPS-II Nutrition cohort and 2006 and 2013 in the CPS-3 cohort and were sent for metabolomics profiling simultaneously. Cancer-free controls at the time of case diagnosis were 1:1 matched to cases on date of birth, blood draw date, sex, and race/ethnicity. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated using conditional logistic regression, controlling for confounders. The Benjamini–Hochberg method was used to correct for multiple comparisons. Results Sphingomyelin (d18:0/22:0) (OR: 1.32; 95% CI: 1.15, 1.53, FDR = 0.15) and taurodeoxycholic acid 3-sulfate (OR: 1.33; 95% CI: 1.14, 1.55, FDR = 0.15) were positively associated with lung cancer risk. Participants diagnosed within 3 years of blood draw had a 55% and 48% higher risk of lung cancer per standard deviation increase in natural log-transformed sphingomyelin (d18:0/22:0) and taurodeoxycholic acid 3-sulfate level, while 26% and 28% higher risk for those diagnosed beyond 3 years, compared to matched controls. 13. Advancing Indigenous futures with two-eyed seeing: Strategies for restoration and repair through collaborative research

Abstract

This article builds on the Indigenous research concept of two-eyed seeing, that is, learning from one eye with the strengths of Indigenous knowledges and ways of knowing, and from the other eye with the strengths of Western knowledges and ways of knowing. We do so by drawing on the authors’ multiple standpoints (Karuk Tribal Member, Karuk Enrolled Descendent, and non-Indigenous ally) and experiences building longstanding research collaborations that apply biophysical science, ethnographic methods, and Karuk oral traditions to tribal lands protection. Using Kovach’s conversational methodology, we discuss problems of health and well-being that arise from two-eyed seeing research collaborations affecting Indigenous lands, waters, and resources. We specifically examine interventions for advancing Indigenous leadership in research that intersect with the Karuk Tribe’s ecocultural revitalization initiatives through (1) stewardship of baskets alongside basket-weaving communities (human and nonhuman); (2) family based management of ceremonial trails, and (3) allyship for tribal–academic collaborations. Our analysis emphasizes how the aliveness of Karuk knowledge resists ahistorical essentialism, for example, by engaging with the joy of human/nonhuman relations, ceremonial scale, and solidarity practices. Responding to ongoing challenges with knowledge hierarchies, this work recognizes the importance of mutual acknowledgment of persons across systems for advancing Indigenous research as a multi-vocal initiative with the capacity for restoration and repair.

Abstract

Background The long-term outcomes of COVID-19 hospitalisation in individuals with pre-existing airway diseases are unknown. Methods Adult participants hospitalised for confirmed or clinically suspected COVID-19 and discharged between 5 March 2020 and 31 March 2021 were recruited to the Post-hospitalisation COVID-19 (PHOSP-COVID) study. Participants attended research visits at 5 months and 1 year post discharge. Clinical characteristics, perceived recovery, burden of symptoms and health-related quality of life (HRQoL) of individuals with pre-existing airway disease (i.e., asthma, COPD or bronchiectasis) were compared to the non-airways group. Results A total of 615 out of 2697 (22.8%) participants had a history of pre-existing airway diseases (72.0% diagnosed with asthma, 22.9% COPD and 5.1% bronchiectasis). Abstract

Transient receptor potential (TRP) ion channels are sophisticated signaling machines that detect a wide variety of environmental and physiological signals. Every cell in the body expresses one or more members of the extended TRP channel family, which consists of over 30 subtypes, each likely possessing distinct pharmacological, biophysical, and/or structural attributes. While the function of some TRP subtypes remains enigmatic, those involved in sensory signaling are perhaps best characterized and have served as models for understanding how these excitatory ion channels serve as polymodal signal integrators. With the recent resolution revolution in cryo-electron microscopy, these and other TRP channel subtypes are now yielding their secrets to detailed atomic analysis, which is beginning to reveal structural underpinnings of stimulus detection and gating, ion permeation, and allosteric mechanisms governing signal integration. These insights are providing a framework for designing and evaluating modality-specific pharmacological agents for treating sensory and other TRP channel-associated disorders.

Abstract

Background Pre-existing cardiovascular disease (CVD) or cardiovascular risk factors have been associated with an increased risk of complications following hospitalisation with COVID-19, but their impact on the rate of recovery following discharge is not known.Objectives To determine whether the rate of patient-perceived recovery following hospitalisation with COVID-19 was affected by the presence of CVD or cardiovascular risk factors.Methods In a multicentre prospective cohort study, patients were recruited following discharge from the hospital with COVID-19 undertaking two comprehensive assessments at 5 months and 12 months. Patients were stratified by the presence of either CVD or cardiovascular risk factors prior to hospitalisation with COVID-19 and compared with controls with neither. Full recovery was determined by the response to a patient-perceived evaluation of full recovery from COVID-19 in the context of physical, physiological and cognitive determinants of health.Results From a total population of 2545 patients (38.8% women), 472 (18.5%) and 1355 (53.2%) had CVD or cardiovascular risk factors, respectively. Compared with controls (n=718), patients with CVD and cardiovascular risk factors were older and more likely to have had severe COVID-19. Full recovery was significantly lower at 12 months in patients with CVD (adjusted OR (aOR) 0.62, 95% CI 0.43 to 0.89) and cardiovascular risk factors (aOR 0.66, 95% CI 0.50 to 0.86).Conclusion Patients with CVD or cardiovascular risk factors had a delayed recovery at 12 months following hospitalisation with COVID-19. Targeted interventions to reduce the impact of COVID-19 in patients with cardiovascular disease remain an unmet need.Trail registration number ISRCTN10980107.

Abstract

We report the effects of radiation damage on helium diffusion in zircon using data from molecular dynamics simulations. We observe an increase in activation energy for helium diffusion as a result of radiation damage and increasing structural disorder. The activation energy in a heavily damaged region is smaller than in a completely amorphous system which is correlated with remaining order in the cation sublattices of the damaged structure not present in the fully amorphized system. The reduction of activation energy is related to the disappearance of fast diffusion pathways that present in the crystal. Consistent with the change in activation energy, we observe the accumulation of helium atoms in the damaged structure and discuss the implications of this effect for the formation of helium bubbles and zircon's performance as an encapsulation material for nuclear waste.

Abstract

Moving from an era of dam building to dam removal brings additional perspectives to indigenous water governance and hydrosocial relations in the Klamath River Basin (US). This collaborative research initiative with the Karuk Tribe builds greater understanding of the sociocultural impacts of Klamath dam removal and river restoration through Karuk knowledge. Addressing a knowledge gap around the social dimensions of dam removal, we held focus groups and interviews with Karuk cultural practitioners, tribal leaders, and tribal youth in the six-month period leading up to demolition. Extending beyond a focus on infrastructure removal or single-species restoration, we consider how Indigenous environmental relations and cosmologies are embedded in dam removal and river restoration. Specifically, Karuk knowledge shifts the significance of dam removal by elucidating deeply interconnected ecological, cultural, and ceremonial relations that are co-constituted with the Klamath watershed, thereby recasting dam removal as a holistic eco-cultural revitalization initiative. This reconfigures dam removal goals to include improving community health and well-being, enhancing spiritual elements of river restoration, responding to colonial legacies, and engaging tribal youth. In the Klamath case, restorative justice becomes possible through Karuk participation in river restoration to facilitate the revitalization of reciprocal relations held between Karuk people and the Klamath River—including Karuk eco-cultural and ceremonial practices for restoring balance in the world.

Abstract

Understanding the source of sub-stellar polarimetric observations in the optical and near-infrared is key to characterizing sub-stellar objects and developing potential diagnostics for determining properties of their atmospheres. Differential scattering from a population of aligned, non-spherical dust grains is a potential source of polarization that could be used to determine geometric properties of the dust clouds. This paper addresses the problem of the spheroidal growth of dust grains in electrically activated sub-stellar atmospheres. It presents the novel application of a mechanism whereby non-spherical, elongated dust grains can be grown via plasma deposition as a consequence of the surface electric field effects of charged dust grains. We numerically solve the differential equations governing the spheroidal growth of charged dust grains via plasma deposition as a result of surface electric field effects in order to determine how the dust eccentricity and the dust particle eccentricity distribution function evolve with time. From these results, we determine the effect of spheroidal dust on the observed linear polarization. Numerical solutions show that $e\approx 0.94$ defines a watershed eccentricity, where the eccentricity of grains with an initial eccentricity less than (greater than) this value decreases (increases) and spherical (spheroidal) growth occurs. This produces a characteristic bimodal eccentricity distribution function yielding a fractional change in the observed linear polarization of up to $\approx0.1$ corresponding to dust grains of maximal eccentricity at wavelengths of $\approx1 \mu$m, consistent with the near infrared observational window. Abstract

We report the results of a large-scale modelling study of radiation damage effects in the nuclear waste form zirconolite. We particularly focus on the effects of radiation damage in amorphous zirconolite and have developed a new way to analyse the damaged structure in terms of local coordination statistics. On the basis of this analysis, we find that the amorphous structure responds to radiation damage differently from the crystal. Amorphous zirconolite is found to be "softer" than crystalline zirconolite with a much larger number of atoms becoming displaced and changing coordination during a 70 keV cascade. The local coordination and connectivity analysis shows that the amorphous structure continues to evolve as a result of repeated radiation damage, changes which cannot be identified from globally averaged properties such as pair distribution functions. We also find large density inhomogeneities at the local level which we suggest may play an important role for future developments in nuclear waste storage. Finally, we find a correlation between the changes in enthalpy and local coordination, suggesting that measurements of enthalpy change can be linked quantitatively to structural radiation damage. Our results raise an interesting possibility of whether an evolution of the amorphous structure due to radiation damage can converge to a new equilibrium amorphous regime, posing the fundamental question of what that structure may be.

Abstract

The nature of the amorphous state has been notably difficult to ascertain at the microscopic level. In addition to the fundamental importance of understanding the amorphous state, potential changes to amorphous structures as a result of radiation damage have direct implications for the pressing problem of nuclear waste encapsulation. Here, we develop new methods to identify and quantify the damage produced by high-energy collision cascades that are applicable to amorphous structures and perform large-scale molecular dynamics simulations of high-energy collision cascades in a model zircon system. We find that, whereas the averaged probes of order such as pair distribution function do not indicate structural changes, local coordination analysis shows that the amorphous structure substantially evolves due to radiation damage. Our analysis shows a correlation between the local structural changes and enthalpy. Important implications for the long-term storage of nuclear waste follow from our detection of significant local density inhomogeneities. Although we do not reach the point of convergence where the changes of the amorphous structure saturate, our results imply that the nature of this new converged amorphous state will be of substantial interest in future experimental and modelling work., Comment: 11 pages, 15 figures

Abstract

Antecedentes: La enfermedad cardiovascular es la primera causa de muerte en el mundo y en Colombia. Las estatinas, como grupo farmacológico, son considerados terapia de primera línea, tanto en prevención primaria como secundaria, para reducir desenlaces cardiovasculares mayores. Sin embargo, desde que se reportó el aumento de casos de Diabetes mellitus 2 en población que consume estatinas, entidades como la FDA añadió una advertencia en relación con su prescripción, lo que significó una nueva evaluación de su relación riesgo-beneficio. Objetivo: Determinar la evidencia relacionada entre la incidencia de DM2 y el uso de estatinas, contrastando con la eficacia de estos medicamentos y sopesar su uso actual en la práctica clínica. Métodos: Se realizó una búsqueda en PubMED, SciELO y Elsevier, considerando como criterio de selección estudios publicados entre 2017-2022 y aquellos considerados como “trascendentales”, incluyendo 10 ensayos clínicos, 9 metaanálisis y 1 estudio de cohorte. Resultados: A pesar de tener datos contradictorios, para algunos fármacos en particular hubo mayor fuerza de asociación (ej: Rosuvastatina y Atorvastatina), en una relación dosis dependiente. Sin embargo, las estatinas presentan balance riesgo beneficio a favor, con número necesario a tratar de 18-83 vs número necesario para dañar de 165-263, destacando la poca evidencia en población latina. Conclusiones: La evidencia está a favor del uso de estatinas. El manejo del paciente con DM2, desde la perspectiva integral del riesgo cardiovascular y sin discriminar su etiología, incluye el uso de estatinas.

Abstract

Although many loci have been associated with height in European ancestry populations, very few have been identified in African ancestry individuals. Furthermore, many of the known loci have yet to be generalized to and fine-mapped within a large-scale African ancestry sample. We performed sex-combined and sex-stratified meta-analyses in up to 52,764 individuals with height and genome-wide genotyping data from the African Ancestry Anthropometry Genetics Consortium (AAAGC). We additionally combined our African ancestry meta-analysis results with published European genome-wide association study (GWAS) data. In the African ancestry analyses, we identified three novel loci (SLC4A3, NCOA2, ECD/FAM149B1) in sex-combined results and two loci (CRB1, KLF6) in women only. In the African plus European sex-combined GWAS, we identified an additional three novel loci (RCCD1, G6PC3, CEP95) which were equally driven by AAAGC and European results. Among 39 genome-wide significant signals at known loci, conditioning index SNPs from European studies identified 20 secondary signals. Two of the 20 new secondary signals and none of the 8 novel loci had minor allele frequencies (MAF) < 5%. Of 802 known European height signals, 643 displayed directionally consistent associations with height, of which 205 were nominally significant (p < 0.05) in the African ancestry sex-combined sample. Furthermore, 148 of 241 loci contained ≤20 variants in the credible sets that jointly account for 99% of the posterior probability of driving the associations. In summary, trans-ethnic meta-analyses revealed novel signals and further improved fine-mapping of putative causal variants in loci shared between African and European ancestry populations.

Abstract

From communities rooted in place to transnational coalitions, this special feature applies concepts of collaborative care rooted in Indigenous knowledge systems to the field of environmental governance. We highlight restorative, liberatory practices rooted in caretaking ethics and reciprocal human-nature relations. Our approach also centers decision making by those most connected to a given resource and the sustenance it provides. Despite global extraction, dispossession, and other colonial legacies, these efforts build toward collective action and community self-determination, both through formal policy change and informal practices. Three facets of collaborative care in environmental governance are threaded through the special feature: (1) care in place, (2) care in power, and (3) care in commoning. These themes connect both Indigenous-led and allied scholarship from the United States to the Netherlands, Japan to Madagascar, and Aotearoa to Canada. Though diverse in their interests and challenges, the authors and communities featured in this research build toward collective action and community self-determination in caring for the places that are the source of collective abundance.

Abstract

Coagulation is a vital stage in treating contaminated water using coagulants that can be either synthetic or natural. Currently, water is being treated by water treatment plants that use aluminum- or iron-based salts and synthetic polymers for coagulation. However, these synthetic coagulants have flaws, including the production of large amounts of chemical sludge, considerable effects on the pH of the purified water, relatively expensive to use, and potentially toxic effects on the environment. The potential and constraints of replacing chemical coagulants with natural plant extracts for water treatment are reviewed in this paper. Various natural coagulants previously investigated for the removal of heavy metals, turbidity, pathogens, and other contaminants from surface and synthetic water were analyzed based on dosage rates, active ingredients responsible for coagulation, and extraction methods among other criteria. A comparative analysis was performed in studies in which both conventional chemical coagulants and natural plant extracts were used for water treatment. Various studies have shown that it is possible to replace chemical coagulants with plant-based extracts. It has been demonstrated that it is possible to replace chemical coagulants with natural plant-based extracts because seed extracts such as those of fenugreek can achieve turbidity removal efficiencies as high as 98% as compared to 85% for alum. The use of natural coagulants may be beneficial for water treatment plants because they may produce less sludge than chemical coagulants; this increases environmental sustainability while decreasing the cost of handling sludge. They are less toxic than chemical coagulants and do not pose any adverse threats to the environment; therefore, they are a safer alternative. However, studies are needed on how these plant-based coagulants can be commercialized in the same way that chemical coagulants are readily available.

Abstract

Current approaches to flood management are increasingly insufficient to deal with intensifying flood trends. In this paper, we define and map out the responsibilities and relationships of local, state, and federal governing entities at various levels. We use these relationships to identify gaps in governance needed to address the high financial, human, and infrastructure costs of flooding. This paper offers a description of current flood policies and provides recommendations for innovations in policy solutions to improve governance gaps. We identify three themes from the literature on intergovernmental relations and flood governance: (1) intergovernmental relations (interlinkages and gaps) for flood governance; (2) risks inherent to flood governance (financial, physical, social and individual, and perception of risk); (3) data adequacy and interoperability.

El objetivo de este artículo es analizar la experiencia pedagógica de un grupo de 12 estudiantes de Licenciatura en Educación Infantil pertenecientes a la comunidad indígena Embera Katío. Para lograrlo se realizó un análisis de narrativas sobre la práctica pedagógica y los modos de interpretar, ver y reflexionar el proceso educativo y la función docente. Los resultados muestran una primera reflexión desde el concepto de comunalidad y la apreciación de los saberes ancestrales; la segunda reflexión desde la identidad y las geografías de la infancia. Por último, la transformación de la práctica pedagógica refleja la realidad educativa originada en el territorio, la perspectiva del trabajo colaborativo y la búsqueda de prácticas situadas en un contexto intercultural para la conformación de comunidades de aprendizaje auténticas.

Abstract

Persons of African ancestry (AA) have a twofold higher risk for multiple myeloma (MM) compared with persons of European ancestry (EA). Genome-wide association studies (GWASs) support a genetic contribution to MM etiology in individuals of EA. Little is known about genetic risk factors for MM in individuals of AA. We performed a meta-analysis of 2 GWASs of MM in 1813 cases and 8871 controls and conducted an admixture mapping scan to identify risk alleles. We fine-mapped the 23 known susceptibility loci to find markers that could better capture MM risk in individuals of AA and constructed a polygenic risk score (PRS) to assess the aggregated effect of known MM risk alleles. In GWAS meta-analysis, we identified 2 suggestive novel loci located at 9p24.3 and 9p13.1 at P < 1 × 10-6; however, no genome-wide significant association was noted. In admixture mapping, we observed a genome-wide significant inverse association between local AA at 2p24.1-23.1 and MM risk in AA individuals. Of the 23 known EA risk variants, 20 showed directional consistency, and 9 replicated at P < .05 in AA individuals. In 8 regions, we identified markers that better capture MM risk in persons with AA. AA individuals with a PRS in the top 10% had a 1.82-fold (95% confidence interval, 1.56-2.11) increased MM risk compared with those with average risk (25%-75%). The strongest functional association was between the risk allele for variant rs56219066 at 5q15 and lower ELL2 expression (P = 5.1 × 10-12). Our study shows that common genetic variation contributes to MM risk in individuals with AA.

36. Argument by Numbers: the Normative Impact of Statistical Legal Tech

The introduction of statistical “legal tech” raises questions about the future of law and legal practice. While technologies have always mediated the concept, practice, and texture of law, a qualitative and quantitative shift is taking place: statistical legal tech is being integrated into mainstream legal practice, and particularly that of litigators. These applications — particularly in search and document generation — mediate how practicing lawyers interact with the legal system. By shaping how law is “done”, the applications ultimately come to shape what law is. Where such applications impact on the creative elements of the litigator’s practice, for example via automation bias, they affect their professional and ethical duty to respond appropriately to the unique circumstances of their client’s case — a duty that is central to the Rule of Law. The statistical mediation of legal resources by machine learning applications must therefore be introduced with great care, if we are to avoid the subtle, inadvertent, but ultimately fundamental undermining of the Rule of Law. In this contribution we describe the normative effects of legal tech application design, how they are (in)compatible with law and the Rule of Law as normative orders, particularly with respect to legal texts which we frame as the proper source of “lossless law”, uncompressed by statistical framing. We conclude that reliance on the vigilance of individual lawyers is insufficient to guard against the potentially harmful effects of such systems, given their inscrutability, and suggest that the onus is on the providers of legal technologies to demonstrate the legitimacy of their systems according to the standards inherent in the legal system.The translation and publication of this article is based on the CC BY Attribution-NonCommercial 4.0 International license, under which this article was published in English at https://osf.io/preprints/socarxiv/ts259/. Introduction Chronic low back pain (CLBP) is a global health concern associated with multidimensional/biopsychosocial levels of affectation in developed countries, with holistic management requiring consideration of these factors. There has been minimal research exploring the psychosocial impact of CLBP, and the factors influencing it, in African contexts, with none in Ghana.Objectives To explore the psychosocial impact of CLBP among patients with CLBP in Ghana.Design Qualitative study using individual semistructured face-to-face interviews, underpinned by Straussian grounded theory principles and critical realist philosophy.Participants Thirty patients with CLBP attending physiotherapy at two hospitals in Ghana.Results Five categories: loss of self and roles, emotional distress, fear, stigmatisation and marginalisation, financial burden, and social support and three mechanisms: acquired biomedical/mechanical beliefs from healthcare professionals (HCPs), sociocultural beliefs and the socioeconomic impact of CLBP were derived.Conclusion CLBP adversely affects multidimensional/biopsychosocial aspects of individuals experiencing CLBP in Ghana. This delineates the need for a biopsychosocial approach to care. There is the need for HCPs in Ghana to reassess current CLBP management strategies to address the influence of adverse HCPs biomedical inclinations on patients’ psychosocial consequences. Population-based education strategies and consideration of formal support systems for persons with disabling CLBP may also be beneficial.

Abstract

In contemporary sub-stellar model atmospheres, dust growth occurs through neutral gas-phase surface chemistry. Recently, there has been a growing body of theoretical and observational evidence suggesting that ionisation processes can also occur. As a result, atmospheres are populated by regions composed of plasma, gas and dust, and the consequent influence of plasma processes on dust evolution is enhanced. This paper aims to introduce a new model of dust growth and destruction in sub-stellar atmospheres via plasma deposition and plasma sputtering. Using example sub-stellar atmospheres from Drift-Phoenix, we have compared plasma deposition and sputtering timescales to those from neutral gas-phase surface chemistry to ascertain their regimes of influence. We calculated the plasma sputtering yield and discuss the circumstances where plasma sputtering dominates over deposition. Within the highest dust density cloud regions, plasma deposition and sputtering dominates over neutral gas-phase surface chemistry if the degree of ionisation is $\gtrsim10^{-4}$. Loosely bound grains with surface binding energies of the order of $0.1-1$ eV are susceptible to destruction through plasma sputtering for feasible degrees of ionisation and electron temperatures; whereas, strong crystalline grains with binding energies of the order $10$ eV are resistant to sputtering. Abstract

The transient receptor potential melastatin 8 (TRPM8) ion channel is the primary detector of environmental cold and an important target for treating pathological cold hypersensitivity. Here, we present cryo-electron microscopy structures of TRPM8 in ligand-free, antagonist-bound, or calcium-bound forms, revealing how robust conformational changes give rise to two nonconducting states, closed and desensitized. We describe a malleable ligand-binding pocket that accommodates drugs of diverse chemical structures, and we delineate the ion permeation pathway, including the contribution of lipids to pore architecture. Furthermore, we show that direct calcium binding mediates stimulus-evoked desensitization, clarifying this important mechanism of sensory adaptation. We observe large rearrangements within the S4-S5 linker that reposition the S1-S4 and pore domains relative to the TRP helix, leading us to propose a distinct model for modulation of TRPM8 and possibly other TRP channels.

Abstract

Water governance engages with complex collective action problems that typically involve a wide range of actors across multiple jurisdictions and large geographical areas. Scholars have conceptualised frameworks of collaborative and polycentric governance to reflect more democratic, devolved and diverse arrangements for governing complexity. What has often been overlooked, however, is the sociopolitical context of working with Indigenous nations and the distinct cultural and political perspectives they bring to polycentric water governance. Focusing on the Karuk Tribe in the Klamath Basin (western United States), this case study examines sovereignty and sustainability concerns that arise with collaborative, polycentric water governance initiatives that involve Indigenous nations. First, we leverage environmental justice frameworks to reveal tensions between collaborative, polycentric governance and social justice concerns. Second, using social network analysis, we examine Klamath water quality networks that involve the Karuk Tribe. Our analysis shows that the Karuk Tribe – as represented by five tribal natural resource managers – connected to 244 distinct organisations and 21 coalitions around water quality issues during the 2018/2019 study period. Social networks help us to visualise the labour required of tribal managers working on water quality issues across multiple centres of governance. Third, we develop the concept of networked sovereignty in water governance to consider both the opportunity and the burden that some Indigenous nations are taking on to advance self-determination in this moment of devolved governance – when tribal managers are building relationships with hundreds of agencies and organisations.

Abstract

Summary: Background: The scale of COVID-19 and its well documented long-term sequelae support a need to understand long-term outcomes including frailty. Methods: This prospective cohort study recruited adults who had survived hospitalisation with clinically diagnosed COVID-19 across 35 sites in the UK (PHOSP-COVID). The burden of frailty was objectively measured using Fried's Frailty Phenotype (FFP). The primary outcome was the prevalence of each FFP group—robust (no FFP criteria), pre-frail (one or two FFP criteria) and frail (three or more FFP criteria)—at 5 months and 1 year after discharge from hospital. For inclusion in the primary analysis, participants required complete outcome data for three of the five FFP criteria. Longitudinal changes across frailty domains are reported at 5 months and 1 year post-hospitalisation, along with risk factors for frailty status. Patient-perceived recovery and health-related quality of life (HRQoL) were retrospectively rated for pre-COVID-19 and prospectively rated at the 5 month and 1 year visits. This study is registered with ISRCTN, number ISRCTN10980107. Findings: Between March 5, 2020, and March 31, 2021, 2419 participants were enrolled with FFP data. Mean age was 57.9 (SD 12.6) years, 933 (38.6%) were female, and 429 (17.7%) had received invasive mechanical ventilation. 1785 had measures at both timepoints, of which 240 (13.4%), 1138 (63.8%) and 407 (22.8%) were frail, pre-frail and robust, respectively, at 5 months compared with 123 (6.9%), 1046 (58.6%) and 616 (34.5%) at 1 year. Factors associated with pre-frailty or frailty were invasive mechanical ventilation, older age, female sex, and greater social deprivation. Frail participants had a larger reduction in HRQoL compared with before their COVID-19 illness and were less likely to describe themselves as recovered. Interpretation: Physical frailty and pre-frailty are common following hospitalisation with COVID-19. Improvement in frailty was seen between 5 and 12 months although two-thirds of the population remained pre-frail or frail. This suggests comprehensive assessment and interventions targeting pre-frailty and frailty beyond the initial illness are required. 43. Invitation to Trusteeship Rather than Treat? Higher Education, Human Rights and Student Litigation: A Response to Fulford (2020)

Abstract

This article argues that, as tutors, we are bound not only by the rules of contract law (i.e., to avoid breaching the terms of that which was agreed to), but also by our duty of care, and the principles of human rights law that protect the right to education. We must strive to avoid negligent acts and any potentially harmful practices or policies. Looking to recent litigation, we are bound also to provide useful, meaningful guidance on how learners might best achieve--and subsequently evidence--high levels of intellectual attainment and wider 'learning gains.'. The concepts of fairness and equitable treatment are key, especially where universities have agreed to widen access and improve opportunities. With the protection of vulnerable learners increasingly leaning towards the provisions--and promises--of human rights law, a sort of estoppel-led 'trusteeship' (over shared knowledge and learning processes) can perhaps also be inferred. Abstract

Competition, and its effect on educational environments, has been widely debated. On the one hand, it is argued that competition raises attainment and, on the other, it is said that whilst it may raise attainment for some, it exists at the expense of a supportive school environment. Should philosophy undertaken as a subject in schools, such as P4C, involve any level of competition if there is a chance of it raising performance? Scholars have argued that communities of inquiry within P4C conflict with the notion of competition, using competition as a contrast to cooperation, as competition implies that only certain voices will be heard and, without it, participation is more welcome and inclusive. Perhaps there is already too much competition in schools, in which case philosophy should be the one place students need not worry about competing with their peers and instead focus purely on collaboration. But what if the very skills that competition undermines are rewarded in a competition? While it stands that competition can silence particular voices and conflict with cooperation, I will argue that competition can avoid these outcomes and improve philosophical performance if such competition rewards collaboration and inquiry, therefore encouraging it.

Abstract

[Display omitted] Mepolizumab is an anti-IL-5 mAb treatment for severe eosinophilic asthma that reduces asthma exacerbations. Residual airway inflammation with mepolizumab therapy may lead to persistent exacerbations. Oral corticosteroids remain the main treatment for these residual exacerbations. Our study aimed to explore the corticosteroid responsiveness of airway inflammation after mepolizumab treatment to find potentially treatable inflammatory mechanisms beyond the IL-5 pathway. The MAPLE trial was a multicenter, randomized, double-blind, placebo-controlled, crossover study of 2 weeks of high-dose oral prednisolone treatment at stable state in 27 patients treated with mepolizumab for severe eosinophilic asthma. We analyzed paired sputum (n = 16) and plasma (n = 25) samples from the MAPLE trial using high-throughput Olink proteomics. We analyzed additional sputum proteins using ELISA. In patients receiving mepolizumab, prednisolone significantly downregulated sputum proteins related to type 2 inflammation and chemotaxis including IL-4, IL-5, IL-13, CCL24, CCL26, EDN, CCL17, CCL22, OX40 receptor, FCER2, and the ST2 receptor. Prednisolone also downregulated cell adhesion molecules, prostaglandin synthases, mast cell tryptases, MMP1, MMP12, and neuroimmune mediators. Neutrophilic pathways were upregulated. Type 2 proteins were also downregulated in plasma, combined with IL-12, IFN-γ, and IP-10. IL-10 and amphiregulin were upregulated. At stable state, prednisolone has broad anti-inflammatory effects on top of mepolizumab. These effects are heterogeneous and may be clinically relevant in residual exacerbations. Abstract

Exposure to ambient fine particulate matter (PM2.5) is a major global health concern. Quantitative estimates of attributable mortality are based on disease-specific hazard ratio models that incorporate risk information from multiple PM2.5 sources (outdoor and indoor air pollution from use of solid fuels and secondhand and active smoking), requiring assumptions about equivalent exposure and toxicity. We relax these contentious assumptions by constructing a PM2.5-mortality hazard ratio function based only on cohort studies of outdoor air pollution that covers the global exposure range. We modeled the shape of the association between PM2.5 and nonaccidental mortality using data from 41 cohorts from 16 countries-the Global Exposure Mortality Model (GEMM). We then constructed GEMMs for five specific causes of death examined by the global burden of disease (GBD). The GEMM predicts 8.9 million [95% confidence interval (CI): 7.5-10.3] deaths in 2015, a figure 30% larger than that predicted by the sum of deaths among the five specific causes (6.9; 95% CI: 4.9-8.5) and 120% larger than the risk function used in the GBD (4.0; 95% CI: 3.3-4.8). Differences between the GEMM and GBD risk functions are larger for a 20% reduction in concentrations, with the GEMM predicting 220% higher excess deaths. These results suggest that PM2.5 exposure may be related to additional causes of death than the five considered by the GBD and that incorporation of risk information from other, nonoutdoor, particle sources leads to underestimation of disease burden, especially at higher concentrations.

50. Understanding how patients’ pain beliefs influence chronic low back pain management in Ghana: a grounded theory approach

Introduction Chronic low back pain (CLBP) is associated with negative consequences in high and low/middle-income countries. Pain beliefs are important psychosocial factors that affect the occurrence and progression of CLBP and may be influenced by the sociocultural context and interactions with healthcare professionals (HCPs). The pain beliefs of Ghanaian patients with CLBP are unknown and the factors influencing pain beliefs in African contexts are unclear.Objectives To explore the pain beliefs of Ghanaian patients with CLBP, how they influence CLBP management/coping and to identify the mechanisms influencing them.Design Qualitative study using individual semistructured face-to-face interviews, situated within Straussian grounded theory principles and critical realist philosophy.Participants Thirty patients with CLBP accessing physiotherapy at two teaching hospitals in Ghana.Results Participants suggested dominant biomedical/mechanical beliefs (related to CLBP causes, posture and activity, and the belief of an endpoint/cure for CLBP). Maladaptive beliefs and practices, in particular fear-avoidance beliefs, and dependence on passive management and coping, were common among participants. These beliefs and practices were mostly influenced by HCPs and sociocultural expectations/norms. Although spirituality, pacing activity and prescribed exercises were commonly mentioned by participants, other active strategies and positive beliefs were expressed by a few participants and influenced by patients’ themselves. Limited physiotherapy involvement, knowledge and awareness were also reported by participants, and this appeared to be influenced by the limited physiotherapy visibility in Ghana.Conclusion Participants’ narratives suggested the dominant influence of HCPs and the sociocultural environment on their biomedical/mechanical beliefs. These facilitated maladaptive beliefs and adoption of passive coping and management practices. Therefore, incorporation of more positive beliefs and holistic/active strategies by Ghanaian patients and HCPs may be beneficial. Furthermore, patient empowerment and health literacy opportunities to address unhelpful CLBP/sociocultural beliefs and equip patients with management options for CLBP could be beneficial.
