Total hip arthroplasty (THA) performed for developmental dysplasia of the hip is a technically difficult procedure with a high complication rate, especially in the presence of completely dislocated hips. This study aimed to evaluate at least 10 years of follow-up results of cementless, ceramic-on-ceramic (CoC) THA performed with transverse subtrochanteric osteotomy in Crowe type IV hips.We retrospectively reviewed 50 patients' 67 hips that underwent CoC, cementless THA with transverse subtrochanteric osteotomy between 2008 and 2011. Clinical and radiological data of the hips were examined. Clinical results were evaluated using the Harris Hip Score and the Western Ontario and McMaster Universities Osteoarthritis Index.The mean Harris Hip Score improved from 22.9 ± 9.9 preoperatively to 94.1 ± 8.1 at the final follow-up (P0.001). The median Western Ontario and McMaster Universities Osteoarthritis Index score improved from 72 (interquartile range: 17) preoperatively to 2 (interquartile range: 17) postoperatively (P0.001). The preoperative mean leg length discrepancy was improved from 4.9 ± 1 cm to 1.5 ± 1 cm in unilateral cases at the last follow-up (P0.001). Revision surgery was required because of nonunion in two patients, prosthetic infection in one patient, and aseptic femoral loosening in the other patient. The overall ten-year survival rate was 94% for femoral stems and 98.5% for acetabular components as per Kaplan-Meier survival analysis.Transverse subtrochanteric shortening osteotomy combined with using cementless acetabular and femoral components with a CoC bearing surface promises successful clinical results and high prosthesis survival in the treatment of Crowe IV hips at long-term follow-up.

Aims To determine the likelihood of achieving a successful closed reduction (CR) of a dislocated hip in developmental dysplasia of the hip (DDH) after failed Pavlik harness treatment We report the rate of avascular necrosis (AVN) and the need for further surgical procedures. Methods Data was obtained from the Northern Ireland DDH database. All children who underwent an attempted closed reduction between 2011 and 2016 were identified. Children with a dislocated hip that failed Pavlik harness treatment were included in the study. Successful closed reduction was defined as a hip that reduced in theatre and remained reduced. Most recent imaging was assessed for the presence of AVN using the Kalamchi and MacEwen classification. Results There were 644 dislocated hips in 543 patients initially treated in Pavlik harness. In all, 67 hips failed Pavlik harness treatment and proceeded to arthrogram (CR) under general anaesthetic at an average age of 180 days. The number of hips that were deemed reduced in theatre was 46 of the 67 (69%). A total of 11 hips re-dislocated and underwent open reduction, giving a true successful CR rate of 52%. For the total cohort of 67 hips that went to theatre for arthrogram and attempted CR, five (7%) developed clinically significant AVN at an average follow-up of four years and one month, while none of the 35 hips whose reduction was truly successful developed clinically significant AVN. Conclusion The likelihood of a successful closed reduction of a dislocated hip in the Northern Ireland population, which has failed Pavlik harness treatment, is 52% with a clinically significant AVN rate of 7%. As such, we continue to advocate closed reduction under general anaesthetic for the hip that has failed Pavlik harness. Cite this article: Bone Jt Open 2021;2(8):594–598.

Purpose Late-diagnosed dislocated hips underwent open reduction, Dega osteotomy, and proximal femoral osteotomy between 1968 and 1988. The objectives of this study are to assess the survival of hips into adulthood, clinical and radiological outcome, patients’ life perspectives and the risk factors of failure. Methods An assessment of 67 hips treated when younger than five years (Results The 40-year survival rates were 73% (95% confidence (CI) 71% to 76%) in the Conclusion Early failure risk factors are older age at the surgical procedure, high AVN grade and bilateral hip involvement. Still, the results facilitate hip reposition whenever technically manageable, even in teenagers. Level of evidence III

AIM To see if three-dimensional (3D) methods could bring new understanding to acetabular changes in shape and orientation in the spastic hip and in which direction(s) acetabular orientation might change, which is crucial for planning appropriate hip correction surgery. METHOD We performed a retrospective study of pelvic computed tomography (CT) examinations in 20 consecutive patients (10 females, 10 males). The mean age of patients was 12 years 9 months (SD 2y; range: 9-16y) at the time of the CT examination. The control group consisted of 18 consecutive pelvic CT examinations (36 acetabula) of deceased individuals (six females, 12 males) aged 4 to 17 years (mean age: 10y 6mo; SD 5y 2mo) whose whole-body CT scans were taken shortly after their death. We compared 3D CT reconstructions of 28 unstable and dislocated hips in children with bilateral cerebral palsy (Gross Motor Function Classification System levels IV and V) with the unaffected side and typically developing controls to assess spatial orientation (inclination, anteversion, and tilt), acetabular volume, and surface area. Additionally, we analysed the multiple factors that may lead to structural and spatial changes of the acetabulum. Purpose Congenital dislocation of the knee and hip is a rare congenital disorder. The specific aim of the study was to evaluate the clinical and radiological outcomes of the children with congenital dislocation of the knee and hip who presented after six months of age. Methods All the consecutive children with congenital dislocation of the knee and hip joints were retrospectively reviewed. We included cases that were treated after six months of age and followed up for a minimum of two years. Twenty-four children with congenital dislocation of the knee and hip (thirteen with ligamentous laxity, eleven children with stiff joints) were included. The knee was dislocated in 45 limbs; the hip was dislocated in 40 instances. The knee joint dislocation was treated with quadricepsplasty in all twenty-four children (45 knees). The hip dislocation (n = 32) was addressed with either closed reduction (n = 8) or open reduction of the hip (n = 24). Eight hip dislocations were not addressed. The outcome of the hip and knee was evaluated. Results The clinical and radiological outcomes were better in children with ligamentous laxity than without laxity. Twenty-two children were community walkers. An orthosis was needed in eight children. The frequency of spontaneous reduction of unreduced dislocation of the hip was noted in three children (5/8 hips). Conclusion Outcome in combined dislocation of knee and hip is good in most cases with surgical interventions. The outcome is better in children with ligamentous laxity. Spontaneous reduction of the dislocated hips might be achieved after gaining knee flexion following knee surgery for congenital the knee in a few cases.

Prosthetic dislocation in total hip arthroplasty (THA) is the most common cause for readmission in the 90 days following surgery. This investigation sought to quantify risk factors for readmission for early prosthetic dislocation within 30 days after primary THA. This study used the National Surgical Quality Improvement Program (NSQIP) database for the years 2012 to 2017. The primary outcome was reoperation or readmission for prosthetic dislocation within 30 days after primary total hip replacement. Secondary outcomes included native NSQIP medical complications. A total of 159,234 patients were included. Of these, 0.25% (n = 399) experienced reoperation or readmission for prosthetic dislocation within 30 days postoperatively. A total of 217 dislocated hips (54.4%) returned to the operating room only once, and 27 hips (6.8%) returned to the operating room twice. The mean day of first reoperation/readmission for dislocation was 13.5 (standard deviation [SD]: 9.0). In multivariable logistic regression, the following factors were significantly associated with early reoperation/readmission for prosthetic dislocation: patient age 80+ years (odds ratio [OR]: 1.51 vs. 50–59), high creatinine (OR: 1.75 vs. normal range), smoking (OR: 1.53), history of severe chronic obstructive pulmonary disease (COPD) (OR: 1.73), general anesthesia (OR: 1.41 vs. spinal), American Society of Anesthesiologists (ASA) class 3–5 (OR: 1.66 vs. 1 or 2), fracture (OR: 2.17), chronic steroid use (OR: 1.54), and operative duration ≥ 2 hours (all p

Developmental dysplasia of the hip (DDH) is a hip joint anomaly that is characterized by a laxity or abnormal positioning of the femoral head with respect to the acetabulum. It is the most common perinatal pathology of the skeleton and includes a spectrum of alterations ranging from flattening or acetabular dysplasia, to a complete dislocation of the femoral head outside the joint that can compromise joint development and stability. The purpose of this presentation is to transmit a systematic study in the evaluation of the newborn's hip, emphasizing a correct physical examination as primordial in the detection of DDH. Also, guide the pediatrician in the proper selection of the complementary diagnostic method to be used according to the age of the patient, in order to optimize detection and reduce the number of dislocated hips in early and late stages as well as the incidence of related pathologies.

Background Closed reduction is generally attempted on children under 18 months of age who present as older infants or who fail nonoperative management for developmental dysplasia of the hip (DDH). However, many of these patients will not achieve concentric reduction by closed means and will ultimately require open reduction (OR). Limited evidence exists to predict which patients can be adequately closed reduced versus those who will need open treatment. Methods Prospectively collected data from 4 select centers of an international multicenter study group sharing a common treatment approach were reviewed for all DDH infants under 18 months old that required operative management. Patients were categorized into 2 groups: those whose hips were successfully closed reduced or those who ultimately required OR. Factors were compared between the closed reduction and OR groups for 2 separate cohorts-those presenting early in life with ultrasound (US) data, and those presenting later with radiographic data. Results A total of 154 patients (166 hips) were included in the analysis. Overall, 87.3% were female. In the early-presenting cohort, purposeful entry multivariate regression revealed that patients with Graf IV hips on US had 3.8 times higher odds of requiring an OR. For the later cohort, hips that were clinically irreducible (ie, negative Ortolani sign) had 3.3 times higher odds of requiring OR. Patients with International Hip Dysplasia Institute (IHDI) grade IV hips had 2.5 times higher odds of needing an OR versus those with grade III hips and 15.4 times higher odds than those with grade II hips. Children with an IHDI grade IV hip and a negative Ortolani sign had 4.4 times higher odds of needing OR. Conclusion Children younger than 18 months with dislocated hips who require OR are more likely to have a high-grade radiographic dislocation (IHDI grade IV), negative preoperative Ortolani sign, and a Graf IV classification on initial US. This information may help surgeons budget the use of operating room time and better counsel parents about intraoperative expectations. Level of evidence Level III-Prognostic.

Objective To evaluate the predictive values of femoral proximal medullary morphology for the use of subtrochanteric osteotomy (STO) in unilateral Crowe IV developmental dysplasia of the hip (DDH). Methods Ninety four patients with unilateral Crowe type IV DDH (59 hips in STO group and 35 hips in the non‐STO group) between April 2008 and June 2019 were enrolled. All patients underwent THA using the Pinnacle acetabular shell, ceramic liner and femoral head, the S‐ROM stem with proximal sleeve. Three parameters on the standard anteroposterior hip radiographs were measured: the widths of medullary canals at 20 mm above the center of lesser trochanter (CLT)，20 mm below the CLT and the isthmus. Canal flare index (CFI), metaphyseal canal flare index (MCFI), diaphyseal canal flare index (DCFI) were calculated. A S‐ROM femoral stem was used in all patients during total hip arthroplasty (THA). Results The CFI and DCFI in the STO group were lower than those in the non‐STO group. However, there was no statistical difference in MCFI between the two groups. The receiver operating characteristic (ROC) curves shown that DCFI had the highest area under the curve (AUC), at 0.885. This was followed by the CFI, which had an AUC of 0.847. The AUC of MCFI was 0.579. The optimal threshold for DCFI was 1.44, which lead to a sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of 0.771, 0.898, 0.869, and 0.818, respectively. For CFI, the optimal threshold was 3.28, resulting in a sensitivity, specificity, PPV, and NPV of 0.829, 0.729, 0.878, and 0.644, respectively. Conclusions The DCFI and CFI may be potent indicators in predicting the use of STO in unilateral Crowe IV DDH. The optimal threshold for CFI and DCFI were 3.28 and 1.44 and had good sensitivity and specificity for predicting the use of STO during THA., The diaphyseal canal flare index and canal flare index may be potent indicators in predicting the use of subtrochanteric osteotomy in unilateral Crowe IV developmental dysplasia of the hip.

Background A well-known complication of having hip alloplasty surgery is dislocation of the prothesis. This affects 2%-4% of the patients, and 75% of the dislocations occur within the first year after surgery. The aim of our study was to gain knowledge about the clinical considerations underlying the choice of anaesthesia for, and treatment of, patients with dislocated hips by specialists in anaesthesiology and orthopaedic surgery. Methods We used semi-structured group interviews of specialists in anaesthesiology and orthopaedic surgery. An interview guide was developed and pilot tested before the group interviews. In total, 25 specialists participated, recruited from two university hospitals in Denmark. Data saturation was reached after seven group interviews. Inductive content analysis was used in the data analysis. Results We identified four overall themes, describing essential considerations made by the specialists: "Adhering to the principle of minimal intervention", "Ensuring patient safety through optimal working conditions", "Organisational considerations" and "Pain induced gastric retention". Conclusion Our study illustrates different clinical considerations made by specialists in anaesthesiology and orthopaedic surgery in relation to the treatment of patients with dislocated hips. Our results hope to promote mutual understanding in joint clinical decision-making in relation to patients with dislocated hips as well as to inspire less-experienced anaesthesiologists and orthopaedic surgeons. Additionally, our results suggest a set of "rules of thumb" for how jointly to decide on the repositioning site of the patient during the procedure.

Timely detection of Developmental Dysplasia of the hip (DDH) in infants and children is crucial as DDH can lead to permanent hip instability if diagnosis is delayed [1]. Existing methods of DDH detection, such as ultrasound and x-rays, are expensive and require trained medical personnel to perform and interpret the tests. Furthermore, x-rays subject patients to ionizing radiation with its attendant cancer risks, especially problematic when repeat studies are performed. In the current study, an acoustic non-invasive approach for DDH detection in a postmortem pig model is presented. We previously designed and tested a similar approach in different simplified benchtop models of the hip joint [2]. A compact system to assess sound transmission through joints was also tested [3] [4]. Moreover, physics-based computational studies were carried out to explain biomechanical factors determining the reduction of dislocated hips [5] [6] [7].

Introduction: This study aimed to evaluate the predictive values of false acetabulum for assessment of whether to use subtrochanteric osteotomy.Materials and Methods: We retrospectively included a total of 182 patients (232 hips) affected by Crowe type IV developmental dysplasia who underwent primary THA with modular cementless stem from April 2008 to May 2019 in our institution. Based on radiographs and operative notes, we found 175 hips were performed with subtrochanteric osteotomy and 57 without subtrochanteric osteotomy, which was named (subtrochanteric osteotomy) STO group and non-STO group, respectively. The predictive values of absence of false acetabulum and distalization of greater trochanter were analyzed using receiver operating characteristic (ROC) curves.Results: ROC curves showed that absence of false acetabulum and distalization of greater trochanter had the AUCs of 0.957 and 0.987, respectively. And there was no statistical significance in the difference of these two AUCs (P=0.392). The optimal threshold for the distalization of greater trochanter was 4.83 cm, which resulted in a sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of 92.6%, 100%, 100%, and 81.4%, respectively. For the absence of false acetabulum, the sensitivity, specificity, PPV, and NPV were 94.9%, 96.5%, 98.8%, and 85.9%, respectively.Conclusions: The absence of false acetabulum may be a potent indicator in predicting the use of subtrochanteric osteotomy in high dislocated hips.

Purpose The treatment of painful and chronic dislocated hip in children with severe cerebral palsy (CP) is particularly demanding and controversial. Numerous surgical techniques have been described, and their outcomes vary a lot. The purpose of the present study is to evaluate a new method, which combines varus derotational subtrochanteric osteotomy (VDSO) and external osteosynthesis: (VDSOEO). Methods Six non-ambulatory children with spastic quadriplegia and chronic dislocated painful hips were treated. The technique involved a small incision on the subtrochanteric site of the osteotomy, followed by retention with a single-sided external osteosynthesis with rotational correction capability [swiveling clamp (SC)] for the reduction of the femur head in the acetabulum, and finally by the osteotomy. Hardware was removed without a second intervention four-six months postoperatively and after the osteotomy was healed. Evaluation of the method was based on clinical, functional, and radiological criteria. Results Four patients achieved improved radiological scores. Two patients demonstrated resubluxation during the period of the osteotomy's healing process. However, no patients experienced pain, and all were able to sit post-surgery, while caregivers reported improved capacity for nursing care. Conclusions It is our strong belief that this approach can improve the quality of life in children with severe CP and painful and chronic dislocated hips. It is a viable and definitely less invasive procedure than classic pelvic or femur osteotomies.

Background: Arthrogryposis multiplex congenita (AMC) is a rare syndrome with multiple joint contractures. There is controversy regarding what the best surgical approach and age is for performing reduction of dislocated hips in AMC. The purpose of this retrospective study was to evaluate the clinical outcome of early open reduction on infant hip dislocation with arthrogryposis multiplex congenita through the modified Smith-Peterson approach, with retained rectus femoris intact. Methods: From 2010 to 2017, we performed this procedure on 28 dislocated hips in 20 infants under 12 months of age with AMC. The clinical and radiology data were retrospectively reviewed. The mean age at surgery was 6.9 ± 5.1 months, with a mean follow-up of 42.4 ± 41.1 months. Background Cases of late diagnosed developmental dysplasia of the hip (DDH) have increased in Australia in recent years despite an ongoing clinical screening program and increasing use of selective ultrasonography screening. Objective The aim of this paper is to discuss an evidence-based approach that involves prevention and early detection of DDH using assessment and surveillance that is suitable for general practitioners (GPs) who see young children in their practices. Discussion Repeated, carefully performed clinical examinations of the paediatric hip from newborn to walking age remain the best method for early detection of DDH. GPs are ideally placed to conduct regular assessments to maximise detection of dislocated hips in early infancy and lower the age at detection for children who develop progressive signs of DDH outside the newborn period.

One of the important pathologic changes in developmental dysplasia of the hip (DDH) is increased acetabular version angle (AA). Reasonable correction for excessive AA is an important step in the treatment of DDH, making accurate AA measurement very crucial. However, the results of different AA measurement methods vary. Thus, this study aimed to compare the difference in AA measurements between 2-dimensional computed tomography (2D-CT) and 3-dimensional computed tomography (3D-CT) in children with DDH and to identify the AA degree in children with DDH to guide treatment.AA was measured by 2D-CT and 3D-CT in 186 children with DDH, and the measurement results were compared with the physical measurement result in the 3D-printed pelvis (3D-PP) model. The 3D-PP was a 1:1 model identical to the human pelvis. All patients were unilaterally affected.The results of AA measurement through 2D-CT, 3D-CT, and 3D-PP of normal hips were 14.0±6.6, 11.9±5.3, and 11.9±3.4 degrees, respectively, whereas those of the dislocated hips were 24.9±8.9, 19.8±5.2, and 19.5±4.3 degrees, respectively. In both the normal and dislocated hip groups, the results between 2D-CT and 3D-CT was significantly different (P0.05), but there was no difference between the results of 3D-CT and 3D-PP (P0.05). The AA of the normal and dislocated hips as measured by 3D-PP was 11.9±3.6 and 19.6±4.3 degrees, respectively, with statistically significant difference (P0.05). In the dislocated hips, a significant positive correlation was found between age and AA (r=0.756, P0.05) and between AA and degree of dislocation (r=0.837, P0.05).3D-CT is more accurate than 2D-CT for AA measurement, and compared with normal hips, AA in dislocated hips increased by ∼7.7 degrees on average. AA increases as age and degree of dislocation increase.Level II.

Congenital hip dislocation (luxation) has an incidence of 0.4 - 0.7% and is regarded as a prearthrotic deformity. Thus, if not being diagnosed and treated at a very early age, extensive surgical measures are inevitable in childhood and early adulthood.In the time between 01/2013 and 02/2019 we performed 28 600 hips sonographies in babies as part of general screening measures at U2 or U3. There were 71 instable, dysplastic or dislocated hips diagnosed that were treated by arthrographic, closed reduction. After a hip spica cast was applied, reposition was controlled by MRI, estimating the acetabular head index (ACI), the head coverage index (HCI) as well as the femoral head's sphericity or by sonography using the Graf method.Overall success rate was 91.6% for primary closed reduction. Patients with primarily irreducible hips were significantly older (p 0.003) than patients with primarily successful reducible hips. Congenital dislocated hips had significantly higher ACIs (p 0.001) and HCIs (p = 0.03) as well as significantly less well rounded femoral heads (sphericity; p 0.001) compared to stable hips.Early diagnosis and treatment of congenital dislocated hips by closed reduction is essential for a sufficient and regular maturation of the hips without further surgical interventions.Die kongenitale Hüftluxation tritt mit einer Inzidenz von 0,4 – 0,7% auf, stellt eine präarthrotische Deformität dar und wird deshalb, wenn nicht frühzeitig im Säuglingsalter erfolgreich therapiert, umfangreiche operative Maßnahmen im Kindes- und frühen Erwachsenenalter erforderlich machen.Im Zeitraum von 01/2013 bis 02/2019 wurden 28 600 Hüftultraschalluntersuchungen an Säuglingen im Rahmen der U2 bzw. U3 durchgeführt. Dabei wurden 71 instabile dysplastische bzw. luxierte Hüften diagnostiziert und anschließend arthrografisch gestützt geschlossen reponiert. Das Repositionsergebnis wurde durch Anlage eines Becken-Bein-Gipses gesichert und im MRT mittels Azetabulum-Kopf-Index (AKI), Sphärizität und Kopf-Überdachungs-Index (KÜX) bzw. mittelfristig mittels Hüftsonografie nach Graf kontrolliert.Die Erfolgsquote geschlossener Reposition und damit Hüftausreifung betrug 91,6% bei der primären Reposition. Patienten mit primär nicht suffizient reponierbarer Hüfte waren signifikant (p = 0,003) älter als Patienten, deren Hüften primär geschlossen reponierbar waren. Kongenital luxierte bzw. instabile Hüften zeigten einen signifikant geringen AKI (p 0,001), KÜX (p = 0,03) und eine signifikante Entrundung (Sphärizität; p 0,001) im Vergleich zu stabilen Hüftgelenken.Die frühe Diagnose und Therapie der kongenitalen Hüftluxation mittels geschlossener Reposition ist entscheidend für eine regelhafte Ausreifung der Hüfte ohne zusätzliche chirurgische Maßnahmen.

Purpose Children with sonographic grade IV hip dysplasia according to Graf and with failed conservative treatment usually need surgical reduction afterwards. Surgical reduction of the hip can lead to severe complications, the occurrence of residual acetabular dysplasia, osteonecrosis, redislocation and other postoperative complications. This paper investigates whether arthroscopic reduction is a promising alternative to open reduction. Methods We retrospectively examined 66 patients (78 hips) who were not older than two years at the first time of surgery. Arthroscopic reduction was performed on 17 children (19 hips) and open reduction on 49 children (59 hips). Patient records were used to determine redislocation, postoperative complication and residual dysplasia. Radiographs were used to determine Tönnis classification for osteonecrosis and pathological acetabular (AC) angle for residual dysplasia. We considered data up to a two-year follow-up. Statistical evaluation was performed with binary logistic regression. Results After arthroscopic reduction, 6% showed osteonecrosis, compared with 20% with open reduction (p = 0.334). Redislocation was not observed after arthroscopic reduction but for 29% after open reduction (p = 0.005). An improvement of femoral head coverage was achieved with residual dysplasia of 23.5% after arthroscopic reduction, compared with 62% after open reduction (p = 0.002). Conclusion The arthroscopic procedure represents a meaningful alternative to the open procedure due to a lower complication rate, a safe setting, a lower rate of residual dysplasia, no observed redislocation and occurrence of osteonecrosis only once in the arthroscopic group of developmental dysplasia of the hip. The arthroscopic procedure should be tested in further studies and in other clinics in order to broaden the empirical base. Level of Evidence Level III (retrospective cohort study)

Purpose Early detection and conservative treatment are essential for a successful outcome in developmental dysplasia of the hip (DDH). The aim of this study was to evaluate the efficacy of Tubingen hip flexion splint treatment on dislocated hips of type D, 3, and 4 according to the Graf classification. Methods A total of 104 dislocated hips in 92 patients were treated with Tubingen splints. Splint treatment was applied to patients with dislocated hips as diagnosed under ultrasonographic evaluation. After four to six weeks of treatment, the hips were re-evaluated according to the Graf classification under ultrasonography. The success of Tubingen splinting was determined as follows: hips initially diagnosed as Graf type 3 and 4 were upgraded to type 2c, 2b, or 1 after treatment, and hips initially diagnosed as Graf type D were upgraded to type 2b or 1 after treatment. Results The mean age at treatment initiation was 11.91 +/- 5.16 (range, 4-32) weeks. There were no statistically significant relationships between success rates and sex, bilateral hip involvement, or initial physical examination findings (p > 0.05). The age at the start of treatment was found to be statistically significant in terms of the success of the splint (p = 0.03). Conclusions For successful treatment with Tubingen splints, the cut-off point of starting initial treatment was defined as the 15th week, with sensitivity of 84.62% and specificity of 62.50%. The success rate was 75% with a successful outcome in 78 hips. In view of these results, Graf type D, 3, and 4 dislocated hips can be successfully treated with Tubingen splints.

Hip pathology is common in arthrogryposis and affects between 30% and 80% of children with the disorder. Hip contractures, hip dysplasia and hip dislocation are common presentations. The pathological changes affecting the hip joint are unique to arthrogryposis and thus their clinical examination and treatment must be tailored both to the disease and the child. Treatment ranges from stretching, therapy and bracing to surgical releases of contracted tissue and open reductions of dislocated hips. Irrespective of the specific hip deformity or treatment type, guiding principles include early intervention as an infant and combined procedures to minimize the time spent immobilized. Children’s hip diseases were not well understood until the advent of X-ray in 1895. Thus childhood surgery prior to that time was limited to draining of infections, closed reductions (a few open) of dislocated hips and other limited procedures. X-ray availability, antibiotics, and stainless steel implants revolutionized thinking about childhood hip surgery. This chapter focuses on the key advances made over the last 100 years in the surgical treatment of congenital (developmental) dislocation of the hip, Perthes disease, and slipped capital femoral epiphysis.

Aim: The aim of the study was to assess the success and possible complications of closed reduction (CR) treatment of developmental hip dysplasia (DHD) in late-diagnosed children and explores its relation to the acetabular index (AI) measurement prior to treatment. Patients and Methods: Twenty-three consecutive patients with dislocated hips, 16 unilateral and 7 bilateral (30 hips), were retrospectively included in the study. They were admitted to the specialist pediatric orthopedic unit of the University Hospital (Tripoli Medical Center) in Tripoli, Libya. There were 21 females and 2 males with an average age at diagnosis of 17 months (range from 14 to 31 months). Their average follow-up period was 3 years (2–5 years), and none of them received treatment prior to diagnosis. All patients received prior inpatient skin traction for at least 2 weeks followed by CR with soft tissue release (adductor tenotomy), hip spica applied and maintained for an average of 3 months. Patients who had a failure of reduction or resubluxation at follow-up went for open reduction and a reconstruction procedure. Results: CR was successful in 27 hips (90%), failed in 3 (10%) other, the average age of the successful reduction group was 20.5 months, while that of the open reduction group, it was 23 months ( P = 0.25). The average AI of the CR group was 39.0°, while that of the open reduction group, it was 42.7° ( P = 0.15); 6.7% of patients with an AI of 40° had a failure of CR of the hip ( P = 0.46). No complications of treatment were recorded at follow-up. Conclusion: Staged CR of DHD in older children in the hands of experienced specialists is still a valid means of their treatment, especially in developing countries with limited resources. There is a relatively higher failure rate of CR, the older the child is and the higher the AI.

Background: The use of a brace has been shown to be an effective treatment for hip dislocation in infants; however, previous studies of such treatment have been single-center or retrospective. The purpose of the current study was to evaluate the success rate for brace use in the treatment of infant hip dislocation in an international, multicenter, prospective cohort, and to identify the variables associated with brace failure. Methods: All dislocations were verified with use of ultrasound or radiography prior to the initiation of treatment, and patients were followed prospectively for a minimum of 18 months. Successful treatment was defined as the use of a brace that resulted in a clinically and radiographically reduced hip, without surgical intervention. The Mann-Whitney test, chi-square analysis, and Fisher exact test were used to identify risk factors for brace failure. A multivariate logistic regression model was used to determine the probability of brace failure according to the risk factors identified. Results: Brace treatment was successful in 162 (79%) of the 204 dislocated hips in this series. Six variables were found to be significant risk factors for failure: developing femoral nerve palsy during brace treatment (p = 0.001), treatment with a static brace (p < 0.001), an initially irreducible hip (p < 0.001), treatment initiated after the age of 7 weeks (p = 0.005), a right hip dislocation (p = 0.006), and a Graf-IV hip (p = 0.02). Hips with no risk factors had a 3% probability of failure, whereas hips with 4 or 5 risk factors had a 100% probability of failure. Conclusions: These data provide valuable information for patient families and their providers regarding the important variables that influence successful brace treatment for dislocated hips in infants. On-farm emergency slaughter (OFES), whereby inspection, stunning, and bleeding occur on the farm before the carcass is transported to a slaughterhouse, is permitted in some jurisdictions as a means to avoid inhumane transportation while salvaging meat from injured animals. However, OFES is controversial and its use for dairy cows has been little studied. Inspection documents for 812 dairy cows were examined to identify how OFES was used for dairy cows in British Columbia, Canada, over 16.5 mo. Producers used OFES for dairy cows aged 1 to 13 yr (median of 4 yr). Leg, hip, nerve, spinal, foot, and hind-end injuries or conditions (in that order) were the most common reasons for OFES, and some cases may have been a consequence of calving. Foot conditions were disproportionately common among cows 5 yr and older, and hind-end conditions were disproportionately common among cows 6 yr and older. Producers used OFES promptly after traumatic injury (within 1 d) for some cows, but OFES was delayed for others, sometimes until cows had been nonambulatory for 2 to 6 d. In some cases, OFES was used for nontraumatic chronic conditions, such as lameness and hind-end weakness, rather than traumatic injuries such as fractures and dislocated hips. Use of OFES appears to conform to the purpose of the program when used promptly after traumatic injuries, but clear guidelines are needed to avoid inappropriate use and delays that may prolong animal suffering.

Conventional stems may be unsuitable for hypoplastic femurs associated with severe dysplasia, meanwhile, custom-made or modular stems in total hip arthroplasty are often complex and expensive. This series included 21 Crowe type IV dysplastic hips in which a non-modular cementless conical stem was implanted with transverse subtrochanteric femoral osteotomy. Follow up averaged 40months. Twenty hips survived with mean Harris hip score improved from 52 to 90. One hip failed for stem loosening. The average leg lengthening was 3.8cm with transient sciatic nerve palsy occurring in three hips. Femoral offset averaged 3.3cm postoperatively. The non-modular conical stem not only obviated the complexities, high medical cost and potential risk at the neck-stem interface associated with stem modularity, but also simplified surgical technique.

Bipolar hemiarthroplasty is frequently used in the treatment of intracapsular hip fractures. Although the dissociation between the bipolar components is a very rare complication, it has very serious results and almost always leads to reoperation with revision of the prosthesis. In this regard, there are very few cases reported in the literature. Dissociation can happen during the hip dislocation, the reduction maneuvers, or spontaneously without any dislocation. Here we report early dissociation between bipolar components in two cases during the attempt of closed reduction maneuvers and three cases with spontaneous dissociation without any trauma. To prevent or minimize this complication; the reduction of dislocated hips must be achieved very carefully under general anesthesia with fluoroscopic control. During the initial operation the surgeons must be sure that the bipolar components are locked to each other and after final reduction, especially in osteoarthritic acetabulums, that the cup position is not in varus position. Self-centering (eccentric) cups have reduced the dissociation rate, but could not completely finish it. Therefore further studies are needed for the development of advanced implant designs. Purpose Developmental dysplasia of the hip (DDH) affects approximately 1 % of live births. Dislocated hips require reduction and stabilisation in a spica cast, and reduction efficacy is assessed radiologically. Numerous measurements are used to ascertain the adequacy of reduction but can be inconsistent in evaluating femoral head position. This study describes the morphology of the developing acetabulum in DDH and validates a novel method to assess adequate reduction of the dysplastic hip following closed or open reduction. Methods A retrospective review was performed of 66 consecutive patients undergoing reduction of hip dislocation over a 2-year period. Three independent reviewers evaluated postoperative CT scans to assess anterior-posterior (AP) displacement and modified Shenton's line. Acetabular morphology was also assessed along with hip congruency using a described novel ‘posterior neck line’. Results Dislocated hips were successfully identified using the posterior neck line with a sensitivity of 0.71 and specificity of 0.88 giving a negative predictive value of 0.97. The interobserver reliability of this technique was higher in comparison against both (AP) displacement and modified Shenton's line. Conclusions We have shown a novel approach in assessing the acetabular morphology of DDH and a novel technique to accurately confirm the reduction of dislocated hips following open or closed reduction.

AimsThere is no consensus about the best method of achieving equal leg lengths at total hip arthroplasty (THA) in patients with Crowe type-IV developmental dysplasia of the hip (DDH). We reviewed our experience of a consecutive series of patients who underwent THA for this indication.Patients and MethodsWe retrospectively reviewed 78 patients (86 THAs) with Crowe type-IV DDH, including 64 women and 14 men, with a minimum follow-up of two years. The mean age at the time of surgery was 52.2 years (34 to 82). We subdivided Crowe type-IV DDH into two major types according to the number of dislocated hips, and further categorised them into three groups according to the occurrence of pelvic obliquity or spinal curvature. Leg length discrepancy (LLD) and functional scores were analysed.ResultsType-I included 53 patients with unilateral dislocation, in which 25 (category A) had no pelvic obliquity or spinal deformity, 19 (category B) had pelvic obliquity with a compensated spinal curvature and nine (category C) had pelvic obliquity and decompensated spinal degenerative changes. Type-II included 25 patients with one dislocated and one dysplastic hip, in which there were eight of category A, 15 of category B and two of category C. Pre-operatively, there were significant differences between the anatomical and functional LLD in type-IB (p = 0.005) and -IC (p < 0.001), but not in type-IA, -IIA or -IIB. Post-operatively, bony LLD increased significantly in types-IB, -IC and -IIB, whereas functional LLD decreased significantly in each type except for IIA. The mean functional LLD decreased from 30.7 mm (standard deviation (sd) 18.5) pre-operatively to 6.2 mm (sd 4.4) post-operatively and the mean anatomical LLD improved from 35.8 mm (sd 19.7) pre-operatively to 12.4 mm (sd 8.3) post-operatively.ConclusionPelvic and spinal changes are common in patients with Crowe type-IV DDH and need to be taken into consideration when planning THA, in order to obtain equal leg lengths post-operatively. Background and purpose As much as one-third of all total hip arthroplasties in patients younger than 60 years may be a consequence of developmental dysplasia of the hip (DDH). Screening and early treatment of neonatal instability of the hip (NIH) reduces the incidence of DDH. We examined the radiographic outcome at 1 year in children undergoing early treatment for NIH. Subjects and methods All children born in Malmo undergo neonatal screening for NIH, and any child with suspicion of instability is referred to our clinic. We reviewed the 1-year radiographs for infants who were referred from April 2002 through December 2007. Measurements of the acetabular index at 1 year were compared between neonatally dislocated, unstable, and stable hips. Results The incidence of NIH was 7 per 1,000 live births. The referral rate was 15 per 1,000. 82% of those treated were girls. Among 2,500 cases of arthrogryposis, 14 families and 17 individuals were identified that had a linear vertical crease (indentation/dimple) along the anterior tibia, usually bilaterally. Approximately half had developmental delay/intellectual disability and the others had normal intellectual development. There are generalized flexion contractures with the legs more severely affected than the arms. The ankles are flexed at birth suggesting a relationship of contraction of the anterior tibial compartment to the in utero development of the anterior tibial crease. Dislocated hips are seen in 60%. Hands open with physical therapy and become quite functional. Those with normal intelligence are ambulatory, often with a crouching stance. Three families had recurrence suggesting a recessive pattern of inheritance. All parents were normal. No affected individuals have had offspring. Several have had CGH arrays, which were normal. The presence of this crease in cases of arthrogryposis appears to be a helpful clinical observation and may even be pathognomonic.

The aim of this study was to report the results of early open reduction of hip dislocations in infants with arthrogryposis multiplex congenita. Seven patients who were under 6 months of age at the time of hip reduction, with a mean follow-up period of 47.5±11.3 months after surgery, were included in this study. Four of seven patients (six of 13 hips) required additional hip surgeries during their follow-up. The short-term results of early open reduction of hips were not promising as most of the patients required additional hip surgeries.

Chronically dislocated hips (6 weeks) are usually the consequence of difficulties accessing appropriate healthcare in a timely fashion after dislocation; this explains why they are more common in developing countries. Due to a lack of research, there is currently no consensus on the best treatment available for patients presenting with this condition. Therefore, it is important to assess the treatments available so as to ensure that doctors adequately manage those presenting with this debilitating condition in the future.To identify the best treatment strategy for chronic hip dislocations based on the treatment outcomes achieved by a free surgical clinic in Phnom Penh, Cambodia.A retrospective analysis of the surgical centre's electronic records was conducted. Patients presenting with hips dislocated for6 weeks were included whilst congenitally dislocated hips were excluded. Treatment outcomes, based on follow up notes, were then assessed. Data abstracted during chart review was analysed using descriptive and comparative statistics.72 patients presented to the clinic with chronic hip dislocations. 42 patients received recorded treatment and 32 were followed up. Among patients with follow-up, 63% experienced 'good' outcomes after treatment. Open reductions, the most common treatment, were successful 65% of the time. The use of preoperative traction increased the success of open reductions by 13%, however, this result was not statistically significant (Open reductions with pre-operative traction seem to be an acceptable treatment in this setting.

Type XXVII collagen is a fibrillar collagen; however, the molecular structure is different from that of other fibrillar collagens: type XXVII collagen forms characteristic nonstriated fibrils, its triple helix domain is shorter than that of other fibrillar collagens and it presents two interruptions, and it lacks the characteristic N-terminal telopeptide-like region and the minor triple helical domain. This suggests that the function of this fibrillar collagen is different from that of other fibrillar collagens. In adults, type XXVII collagen is primarily expressed in cartilage. During development, the expression is particularly high in developing bones, and mutations in mice lead to chondrodysplasia. In humans the only disease associated with certainty with mutations in the type XXVII collagen gene is Steel syndrome, which is characterized by dislocated hips, prominent forehead and a long oval-shaped face, short stature, scoliosis, and other skeletal abnormalities.

Background: A delay in the diagnosis of developmental dislocation of the hip has many long-term consequences. This retrospective study was undertaken in order to establish an incidence of late-presenting developmental dislocation of the hip in Western Australia, and investigate possible causes for missed diagnoses. Method: Data were collected retrospectively from 1 January to 31 December 2010. Theatre records were searched for operative descriptions including the words ‘arthrogram hip’, ‘EUA hip’, ‘closed reduction hip’, ‘open reduction hip’ and ‘spica’. Medical records were checked to establish the demographic details and background history of cases identified. Delayed diagnosis of developmental dysplasia of the hip (DDH) was defined as a dislocated hip requiring operative reduction, diagnosed at age greater than 3 months. Results: Seventeen children with 21 dislocated hips were identified. Age at diagnosis ranged from 6 months to 5 years. Girls accounted for 88.2% (15/17) and the left hip was involved two-thirds of the time (14/21). Bilateral dislocations were found in four children. Conclusion: This study has identified an incidence of late-presenting developmental hip dislocation of approximately three times the previously established rate. Possible reasons for this are explored. Additional retrospective audit is now underway, and changes are already in place to ensure that infants with DDH born in Western Australia are identified and treated as early as possible.

Background We posed 2 questions: what is the long-term result of open reduction surgery in developmental dysplasia of the hip, and is there an age at surgery above which the outcome was too poor to recommend the operation? Methods Between 1955 and 1995, 148 patients with 179 dislocated hips had open reduction surgery for developmental dysplasia of the hip (141 anterior and 38 Ludloff medial approaches). We attempted to locate all 148 patients for the follow-up evaluation. Results Fifty-three patients (36%) with 66 hips (37%) were located and participated in the study. These 66 hips represented 34% of the anterior open reductions and 47% of the Ludloff medial reductions. Twenty-two of the 66 hips had Severin IV or worse outcomes and included 7 with total hip arthroplasties and 2 with hip fusions. Age at surgery was significantly lower for Severin I, II, and III, compared with Severin IV and above (P=0.003, 0.001, 0.003) with outcomes deteriorating substantially after age 3. Approximately half of the hips required further surgery for dysplasia. All hips that sustained osseous necrosis had Severin IV or worse outcomes, and hips that redislocated and required revision surgery only achieved Severin I or II ratings 18% of the time. Nine "normal" hips became dysplastic and 3 had pelvic osteotomies as teenagers. Two other normal hips developed osseous necrosis during treatment of the contralateral hip. Conclusions Results deteriorate as the age at surgery increases. Osseous necrosis and redislocation predict a poor functional and radiographic result. The "normal" hip may develop insidious dysplasia and also may be injured during treatment of the involved hip. Above age 3, some patients may not have sufficient acetabular growth to remodel a surgically reduced hip. Level of evidence Level IV--case series.

Purpose Magnetic resonance imaging (MRI) has been successfully used in the determination of the adequacy of the surgical reduction of congenitally dislocated hips in children. We present the results of a prospective series of patients treated conservatively. MRI was performed in all hips after positioning was deemed adequate on radiographs after spica cast application. The goal of this study was to evaluate the usefulness of MRI in this indication. Methods After the study was approved by our local ethics committee, 31 patients for a total of 36 dislocated hips were included. After the traction period, hip testing was performed and a hip spica cast was applied under general anaesthesia. All children had MRI within 1 week of reduction, without the need for contention or general anaesthesia. Hip reduction was assessed on axial and coronal MRI images. Results The concentric reduction of the hip was confirmed in 30 cases out of 36. In three cases, the dislocation was retrospectively suspected on radiographs and then confirmed. In the three remaining cases, hip dislocation was only diagnosed on MRI. Conclusions MRI screening of congenitally dislocated hips after reduction procedures is a safe and reliable procedure to assess the concentric reduction of the hip. Even in doubtful cases, MRI detected persistent hip dislocations and was conducive to iterative reduction and satisfactory outcome and result.

Hip reconstruction with subtrochanteric valgus extension pelvic support osteotomy and distal femoral osteotomy for lengthening and varus correction is one of the options available for salvage of chronic unstable hips and is also known as Ilizarov hip reconstruction (IHR). This study evaluated the outcomes and complications associated with IHR in skeletally mature young patients. Twelve patients (7 males, 5 females) with a mean age of 23 years underwent IHR for chronically dislocated hips due to various causes. Preoperative clinical and radiological evaluations were used to determine the site of osteotomies and the required angulations. Postoperatively the patients were followed up clinically and radiologically for a minimum of 36 months. Ilizarov fixator was removed when adequate lengthening was achieved and there was radiological evidence of union. Harris Hip Score was used to document hip function preoperatively and at final evaluation. Objectives This study aims to assess the value of certain radiographic features of the pelvis associated with developmental dysplasia of the hip (DDH), in predicting the method of treatment; whether conservative or operative, in patients between 12 and 24 months of age where both options are valid. Methods A retrospective review of the radiographs and the clinical files of children diagnosed with DDH during their second year of life (mean age 18.2 months), including only children who had a follow up of clinically and radiologically for a minimum of 5 years. The initial plain AP pelvic radiograph before commencing any treatment was used to measure 12 different measurements and 2 ratios. Each radiograph was studied individually and blindly, without knowing to which group of patients it belonged. Results A total of 72 dislocated hips in 54 patients were studied. The mean, standard deviation and P. values of age and radiographic measurements were calculated. Patients were divided into 2 groups according to the treatment modality used whether conservative or operative then they were compared. The measurement of the width of the femoral head ossific nucleus was significantly higher in the conservative group. The measurements of lateralization, superior migration of the femoral head and the acetabular angle were found to be significantly higher in the operative group. Conclusion Study results suggest the possibility of using specific radiographic measurements in patients with DDH during the second year of life to determine the most feasible way of management.

Purpose There is no consensus in the literature regarding the diagnosis and treatment of developmental dysplasia of the hip (DDH). We designed a national questionnaire to assess the various opinions and current practice of paediatric orthopaedic surgeons in the Netherlands regarding the diagnosis and treatment of DDH in children less than 1 year old. Methods The questionnaire was sent to all members of the Dutch Paediatric Orthopaedic Society (DPOS). It discusses different methods and criteria used in the diagnosis of DDH, the use of different therapies and the use of different imaging techniques to evaluate the result of treatment. Results With 38 responders, the overall response rate to the survey was 67%. Most surgeons use clinical, radiographic and/or ultrasound examination for the diagnosis. The starting point of treatment is usually on the mild part of the DDH spectrum. The Pavlik harness is most popular in the treatment of dislocated hips, whereas in dysplastic hips, most surgeons use a rigid splint. The duration of treatment has a wide range and evaluation of the effect of treatment is predominantly done by radiography. Conclusions The diagnosis and treatment of DDH in the Netherlands has as much diversity as the literature has recommendations about this subject. The lack of consensus on many aspects of DDH diagnosis and treatment should form the basis for a discussion among Dutch paediatric orthopaedic surgeons. Using the available evidence, it should be possible to formulate a more uniform protocol for the diagnosis and treatment of DDH.

Purpose The Ilizarov hip reconstruction is a well accepted but complication-prone operative salvage procedure in chronically dislocated hips, not least due to the long-term application of external fixation. Although the advantages of fully implantable devices are well known in limb lengthening and are described consistently, until now, external fixation has been used exclusively to perform the Ilizarov hip reconstruction procedure. We present a new technique of Ilizarov hip reconstruction with purely internal implants. Methods A 14-year-old girl with a history of spina bifida presented with a 4-cm-short right leg, a Trendelenburg gait and a complex neurological disease expression. Because of refusal of external fixation by the patient and significantly lower complication rates, an Ilizarov hip reconstruction without external fixation was performed. A locking compression plate was applied to fix the proximal femoral valgus-extension osteotomy and a motorised intramedullary distraction nail was used for the distal, lengthening–varisation osteotomy. Results A healing index of 33 days/cm and full weight bearing after 6 months were noted. At the 1 year follow-up, the patient showed an improvement of the Trendelenburg gait, as well as successful leg equalisation. Satisfaction to a high degree was additionally noted by factors such as reduced pain, the ability to wear workaday clothes and cosmetically appealing scars. No complications were recorded. Conclusion The exclusive use of internal implants for Ilizarov hip reconstruction is a feasible and patient-friendly alternative to traditional methods. Their use, however, may be restricted by geometric preconditions.

Objectives: This study aims to assess the radiographic development of the acetabulum after closed reduction of developmental dysplasia of the hip in patients with different follow-up periods. Methods: The authors undertook a retrospective review of clinical records and radiographs of children who were diagnosed with developmental dysplasia of the hip and underwent closed reduction (mean age at closed reduction: 8.6 months) at King Khalid Civil Hospital, Tabuk, Saudi Arabia. Patients with a follow-up period < 12 months were excluded from the study. Acetabular index angles for anteroposterior pelvic radiograph before treatment and at the final follow-up were used to assess acetabular development. Patients were divided into four groups according to the follow-up period and then the acetabular index was compared. Results: A total of 64 dislocated hips of 40 patients were included in the study. The average follow-up period was 33.9 months (range: 12–82 months). Mean acetabular index before closed reduction was 34.37° (range: 25–46°), whereas the mean acetabular index after closed reduction was 23.8° (range: 10–37°). All groups showed improvement in the acetabular coverage and the acetabular index was significantly higher in groups with a longer follow-up period. Conclusions: This study found that successful closed reduction of congenitally dislocated hips within the recommended age results in better acetabular development (coverage) and this improvement is more evident in patients with a longer follow-up period. Further studies can consolidate these results and help define the criteria for deciding early acetabuloplasty.
