To reduce treatment burden and optimise patient outcomes in diabetic macular oedema, we present 1-year results from two phase 3 trials of faricimab, a novel angiopoietin-2 and vascular endothelial growth factor-A bispecific antibody.YOSEMITE and RHINE were randomised, double-masked, non-inferiority trials across 353 sites worldwide. Adults with vision loss due to centre-involving diabetic macular oedema were randomly assigned (1:1:1) to intravitreal faricimab 6·0 mg every 8 weeks, faricimab 6·0 mg per personalised treatment interval (PTI), or aflibercept 2·0 mg every 8 weeks up to week 100. PTI dosing intervals were extended, maintained, or reduced (every 4 weeks up to every 16 weeks) based on disease activity at active dosing visits. The primary endpoint was mean change in best-corrected visual acuity at 1 year, averaged over weeks 48, 52, and 56. Efficacy analyses included the intention-to-treat population (non-inferiority margin 4 Early Treatment Diabetic Retinopathy Study [ETDRS] letters); safety analyses included patients with at least one dose of study treatment. These trials are registered with ClinicalTrials.gov (YOSEMITE NCT03622580 and RHINE NCT03622593).3247 patients were screened for eligibility in YOSEMITE (n=1532) and RHINE (n=1715). After exclusions, 940 patients were enrolled into YOSEMITE between Sept 5, 2018, and Sept 19, 2019, and 951 patients were enrolled into RHINE between Oct 9, 2018, and Sept 20, 2019. These 1891 patients were randomly assigned to faricimab every 8 weeks (YOSEMITE n=315, RHINE n=317), faricimab PTI (n=313, n=319), or aflibercept every 8 weeks (n=312, n=315). Non-inferiority for the primary endpoint was achieved with faricimab every 8 weeks (adjusted mean vs aflibercept every 8 weeks in YOSEMITE 10·7 ETDRS letters [97·52% CI 9·4 to 12·0] vs 10·9 ETDRS letters [9·6 to 12·2], difference -0·2 ETDRS letters [-2·0 to 1·6]; RHINE 11·8 ETDRS letters [10·6 to 13·0] vs 10·3 ETDRS letters [9·1 to 11·4] letters, difference 1·5 ETDRS letters [-0·1 to 3·2]) and faricimab PTI (YOSEMITE 11·6 ETDRS letters [10·3 to 12·9], difference 0·7 ETDRS letters [-1·1 to 2·5]; RHINE 10·8 ETDRS letters [9·6 to 11·9], difference 0·5 ETDRS letters [-1·1 to 2·1]). Incidence of ocular adverse events was comparable between faricimab every 8 weeks (YOSEMITE n=98 [31%], RHINE n=137 [43%]), faricimab PTI (n=106 [34%], n=119 [37%]), and aflibercept every 8 weeks (n=102 [33%], n=113 [36%]).Robust vision gains and anatomical improvements with faricimab were achieved with adjustable dosing up to every 16 weeks, demonstrating the potential for faricimab to extend the durability of treatment for patients with diabetic macular oedema.F Hoffmann-La Roche.

Background: There is a lack of consensus on the use of intravitreal corticosteroid therapies in patients with diabetic macular edema (DME) and prior vitrectomized eyes in clinical practice. Methods: Retro-IDEAL was a 3-year retrospective, multicenter study in patients with chronic DME (i.e. DME that persists or recurs despite treatment) treated with ILUVIEN® (0.2 µg daily fluocinolone acetonide intravitreal implant), who had suboptimal outcomes with first-line vascular endothelial growth-factor inhibitors and other DME therapies. Results: A total of 81 eyes (63 patients) were included of which 39 eyes had undergone prior vitrectomy (PV group) while 42 eyes had not undergone prior vitrectomy (NPV). Baseline characteristics were balanced; however, more patients had proliferative diabetic retinopathy in the PV group vs. the NPV group (21.62% vs 9.38%, respectively). Over 36 months, mean visual acuity (VA) increased in both groups with a tendency for more ETDRS letters being gained in the NPV group (+5.33) vs. the PV group (+2.42). By month 36, central retinal thickness was reduced to ⩽300 µm in two-thirds of the eyes in both groups and the mean change from baseline in intraocular pressure was similar in both groups (+0.50 mmHg −0.75 mmHg; NPV and PV group). Conclusions: These long-term data suggest that the 0.2 μg/day FAc implant is effective in both vitrectomized and non-vitrectomized patients, with a manageable safety profile, and improved VA and reduced supplemental therapies for patients with a suboptimal response to first-line DME therapies. Clinicians may consider utilizing the FAc implant earlier in the DME disease process.

Purpose To examine vision-related quality of life in patients with diabetic macular edema (DME) treated with intravitreal aflibercept (EYLEA, Regeneron Pharmaceuticals, Inc, Tarrytown, NY). Design AQUA was a multicenter, open-label, single-arm, phase 4 study. Participants Adults 18 years of age or older with type 1 or 2 diabetes mellitus and DME. Methods Patients received intravitreal aflibercept 2 mg every 8 weeks for 52 weeks, after 5 initial doses every 4 weeks. Main Outcome Measures The primary outcome was the change in 25-item National Eye Institute Visual Function Questionnaire (NEI VFQ-25) total score from baseline to week 52. Secondary outcomes included the change in NEI VFQ-25 near and distant activities subscale scores, best-corrected visual acuity (BCVA; Early Treatment Diabetic Retinopathy Study [ETDRS] letters), and central retinal thickness (CRT) from baseline to week 52. Change in NEI VFQ-25 score at week 52 for better-seeing eyes (BSEs) and worse-seeing eyes (WSEs) also was evaluated. Results A total of 553 patients comprised the full analysis set, and 560 patients comprised the safety analysis set. At baseline, the mean NEI VFQ-25 total score was 70.12, mean BCVA was 61.5 ETDRS letters, and mean CRT was 464.81 μm. A mean of 8.8 injections were administered over 52 weeks. At week 52, the mean improvement from baseline in the NEI VFQ-25 total score was +6.11 (standard deviation [SD], 11.46); the corresponding improvements in near and distant activities were +11.37 (SD, 18.01) and +7.33 (SD, 17.32), respectively. Similarly, improvements in patients whose BSE and WSE were treated were 7.74 (SD, 13.59) and 5.48 (SD, 9.70), respectively. At week 52, mean change in BCVA was +10.0 ETDRS letters (SD, 8.0 ETDRS letters), and mean change in CRT was –175.38 μm (SD, 132.62 μm). Overall, 53.6% of patients reported treatment-emergent adverse events (TEAEs), of whom 26.8% experienced an ocular TEAE in the study eye. The most common serious ocular TEAE was endophthalmitis (0.5% [n = 3]). Five deaths (0.9%) were reported, but were not considered treatment related. Conclusions Intravitreal aflibercept was associated with clinically meaningful improvements in NEI VFQ-25 total score over 52 weeks in patients with DME; these were even more pronounced for near than for distant activities. Adverse events were consistent with the known safety profile of intravitreal aflibercept.

Introduction: The Retro-IDEAL (ILUVIEN Implant for chronic DiabEtic MAcuLar edema) study is a retrospective study designed to assess real-world outcomes achieved with the ILUVIEN® (0.19 mg fluocinolone acetonide (FAc)) in patients with chronic diabetic macular edema (DME) in clinical practices in Germany. Methods: This study was conducted across 16 sites in Germany and involved 81 eyes (63 patients) with persistent or recurrent DME and a prior suboptimal response to a first-line intravitreal therapy (primarily anti-VEGF intravitreal therapies). Results: Patients were followed-up for 30.8 ± 11.3 months (mean ± standard deviation) and had a mean age of 68.0 ± 10.4 years. Best-recorded visual acuity (BRVA) improved by +5.5 letters at month 9 (P ⩽ 0.005, n=56; from a baseline of 49 letters) and this was maintained through to month 30 (P ⩽ 0.05, n = 42). There was a concurrent improvement in central macular thickness with a reduction from 502 µm at baseline to 338 µm at year 1 (P ⩽ 0.0001, n = 43). This effect was sustained to year 3 (i.e. 318 µm; P ⩽ 0.0001, n = 29). Mean intraocular pressure (IOP) remained constant between baseline and year 3 with a peak change of 1.9 mm Hg occurring at year 1. Elevated IOP was observed in a similar percentage of patients prior to (22.2% of cases) and following (27.2%) treatment with the FAc implant. In the majority of cases, these elevations were managed effectively with IOP medications. Conclusions: Despite substantial amounts of prior intravitreal treatments – primarily with anti–vascular endothelial growth factor (VEGF) drugs – this real-world study showed that sustained structural and functional improvements can last for up to 3 years with a single FAc implant.

Introduction Oxygen saturation in retinal vessels can be non-invasively measured by a new method. It is known that oxygen extraction is lowered in diabetic retinopathy. In the cross-sectional study presented here, it has been investigated whether diabetes-specific changes and typical accompanying diseases could influence the measurements in oximetry. Methods During a time span of seven months, 237 diabetics were included who came to our clinic for an intravitreal injection, who met the inclusion criteria, who showed no exclusion criteria and gave written consent. 203 eyes of 203 patients were evaluated. The oxygen saturation (SO2) was measured in digitally stored fundus images taken with a fundus camera (Zeiss 450 FF). In its illumination beam, a filter was inserted with two transmission maxima at 584 nm and 610 nm (bandwidth 10 nm). With the program “Oximetry” (Imedos Systems, Jena, Germany), the venous and the arterial SO2 were measured. The values were stored in a spreadsheet. Diabetes related data, clinical parameters and accompanying diseases were documented in the predefined scheme. Results In the whole patient sample, the venous oxygen saturation (vSO2) was 69 ± 12%, the arterial saturation (aSO2) 99 ± 5% and the arteriovenous difference 31 ± 9%. A statistically significant association could not be found between the measurement values of oximetry and age, duration of diabetes and HbA1c (Kruskal-Wallis ANOVA: p > 0.05). The differences in oximetry values between units of analysis (UOA) e.g. hypertension and non units of analysis (NOA) were analysed. Descriptive testing showed significant differences in vSO2% in the following units of analysis: allergies n = 47: UOA vs. NUOA: 64 vs. 69 (t test: p Conclusion The clinically observed improvement in the retinal state situation after intravitreal injections with VEGF inhibitors may be seen in association with the improved oxygen extraction. It seems reasonable to study whether a switch to sartans in the therapy of hypertension may improve retinal function.

PURPOSE To investigate hair cortisol concentration (HCC), a biochemical correlate of long-term cortisol output patterns, and its relationship to active central serous chorioretinopathy (CSC). METHODS Twenty-six participants were included in this observational pilot study (11 patients with active CSC and 15 healthy controls). Hair cortisol concentrations (HCCs) were determined from 3 cm hair strands collected near the scalp from patients and controls as an index of cumulative cortisol secretion over the 3-month period prior to hair sampling. RESULTS Patients with CSC exhibited higher HCCs (mean value: 20.14, 95% CI: 14.89-27.16 pg/mg) than healthy controls (mean value: 11.06, 95% CI: 8.63-14.22 pg/mg, p = 0.008). Group differences were not affected by relevant covariates (BMI, smoking status, sex). CONCLUSION Patients with active CSC have increased HCC, supporting the fact that cortisol is a major player in CSC pathogenesis.

Zusammenfassung Hintergrund Bei der vitreomakulären Traktion (VMT) besteht eine abnormal starke Adhärenz zwischen Netzhaut und Glaskörperkortex vorrangig um die Fovea. Es ist eine visusbedrohende Erkrankung mit den klinischen Symptomen Metamorphopsien und Visusreduktion. Seit 2013 ist Ocriplasmin (Jetrea®) zur intravitrealen Behandlung von vitreomakulären Traktionen und auch in Zusammenhang mit einem Makulaforamen (≤ 400 μm) in Deutschland zugelassen. Material/Methoden Alle 23 Augen von 21 Patienten mit persistierender vitreomakulärer Traktion mit und ohne Makulaforamen (MF), die bisher an unserer Klinik mit Ocriplasmin intravitreal behandelt wurden, werteten wir retrospektiv aus. Vor und nach der Behandlung erfolgten die Bestimmung des bestkorrigierten Visus (BCVA), eine ophthalmoskopische Vorderabschnitts- und Fundusuntersuchung, sowie eine SD-OCT-Untersuchung. Als therapeutischer Erfolg wurde die komplette Lösung der VMT bzw. der Verschluss des MF gewertet. Ergebnisse Eine komplette Lösung der VMT erfolgte bei 8 Augen (34,8 %), in 15 Augen (65,2 %) persistierte sie. Zwei der 4 MF waren nach der Behandlung verschlossen mit Normalisierung der anatomischen Strukturen der Netzhaut. Der Visus vor der Ocriplasmin-Injektion betrug im Mittel 0,39 ± 0,25 logMAR mit einem für die Gesamtgruppe leichten, jedoch nicht statistisch signifikanten Abfall auf 0,41 ± 0,24 logMAR (p = 0,613) nach 37,3 ± 22,9 Tagen. Die zentrale Netzhautdicke (CRT) betrug vor der Ocriplasmin-Injektion im Mittel 453,3 ± 172,7 µm mit einer leichten, jedoch für die Gesamtgruppe ebenfalls statistisch nicht signifikanten Abnahme auf 412,0 ± 212 µm (p = 0,124). Schlussfolgerung Ocriplasmin stellt eine medikamentöse Behandlungsoption bei VMT mit und ohne Makulaforamen dar. Unsere Ergebnisse unterstützen eine kritische Indikationsstellung, da signifikante Zusammenhänge bez. einer Patientenselektion ausblieben. Tendenziell korrelierte der Misserfolg mit zipfeliger Adhärenz, höherem Patientenalter sowie ausbleibenden Chromatopsien.

Purpose: To evaluate the safety and efficacy of dexamethasone intravitreal implant (Ozurdex, DEX implant) 0.7 and 0.35 mg in the treatment of patients with diabetic macular edema (DME).Design: Two randomized, multicenter, masked, sham-controlled, phase III clinical trials with identical protocols were conducted. Data were pooled for analysis.Participants: Patients (n = 1048) with DME, best-corrected visual acuity (BCVA) of 20/50 to 20/200 Snellen equivalent, and central retinal thickness (CRT) of ≥300 μm by optical coherence tomography.Methods: Patients were randomized in a 1:1:1 ratio to study treatment with DEX implant 0.7 mg, DEX implant 0.35 mg, or sham procedure and followed for 3 years (or 39 months for patients treated at month 36) at ≤40 scheduled visits. Patients who met retreatment eligibility criteria could be retreated no more often than every 6 months.Main Outcome Measures: The predefined primary efficacy endpoint for the United States Food and Drug Administration was achievement of ≥15-letter improvement in BCVA from baseline at study end. Safety measures included adverse events and intraocular pressure (IOP).Results: Mean number of treatments received over 3 years was 4.1, 4.4, and 3.3 with DEX implant 0.7 mg, DEX implant 0.35 mg, and sham, respectively. The percentage of patients with ≥15-letter improvement in BCVA from baseline at study end was greater with DEX implant 0.7 mg (22.2%) and DEX implant 0.35 mg (18.4%) than sham (12.0%; P ≤ 0.018). Mean average reduction in CRT from baseline was greater with DEX implant 0.7 mg (−111.6 μm) and DEX implant 0.35 mg (−107.9 μm) than sham (−41.9 μm; P < 0.001). Rates of cataract-related adverse events in phakic eyes were 67.9%, 64.1%, and 20.4% in the DEX implant 0.7 mg, DEX implant 0.35 mg, and sham groups, respectively. Increases in IOP were usually controlled with medication or no therapy; only 2 patients (0.6%) in the DEX implant 0.7 mg group and 1 (0.3%) in the DEX implant 0.35 mg group required trabeculectomy.Conclusions: The DEX implant 0.7 mg and 0.35 mg met the primary efficacy endpoint for improvement in BCVA. The safety profile was acceptable and consistent with previous reports.

Retinale angiomatose Proliferationen (RAP) stellen eine Sonderform der bei exsudativer bzw. „neovaskularer“ altersbedingter Makuladegeneration (nAMD) auftretenden Neovaskularisationen dar. Haufig sind sie mit einem massiven Visusverlust assoziiert. Intravitreales Ranibizumab ist eine haufig angewendete Therapiemethode. Allerdings ist die Effektivitat geringer im Vergleich zu anderen choroidalen Neovaskularisationen. Wir untersuchten die Effektivitat von Ranibizumab im spaten Stadium RAP III mit retinochoroidalen Anastomosen im Vergleich zum Outcome von anderen CNV-Lasionen. Es handelt sich um eine retrospektive Analyse von allen Ranibizumab-behandelten Patienten mit nAMD. Die Patienten wurden unterteilt in Gruppen abhangig vom Lasionstypus in RAP (klinisch identifiziert und selektiert, bewiesen durch Fluoreszenzangiographie) und in CNV-Lasion (identifiziert durch Fluoreszenzangiographie) mit Einteilung in okkult, minimal und uberwiegend klassisch. Der bestkorrigierte Visus (BCVA) wurde vor (zum Zeitpunkt „Diagnose“) und wahrend (erste, zweite, dritte Injektion) der Behandlung erhoben sowie nach der Upload-Phase (1. Kontrollvisite). Vor der ersten Injektion verschlechterte sich der Visus in allen Gruppen (0,73 zu 0,78 logMAR fur alle CNV-Lasionen; 0,95 zu 1,02 logMAR fur RAP). Wahrend der Upload-Phase kam es in der RAP-Gruppe zu keiner weiteren Visusverschlechterung, aber auch zu keiner Verbesserung (1,02 zu 1,03 logMAR), in der anderen Gruppe kam es jedoch zu einem signifikanten Visusanstieg (0,78 zu 0,67 logMAR). Die Behandlung des spaten Stadiums III bei RAP-Lasionen mit Ranibizumab ist effektiv. Stabilisierung des Visus kann erreicht werden – aber im Unterschied zu anderen CNV-Lasionen keine weitere Verbesserung. Daher sollten Patienten mit RAP-Lasionen so fruh wie moglich identifiziert und behandelt werden.

Hintergrund: Die intravitreale Injektion des anti-VEGF-Inhibitors Ranibizumab gilt seit 2007 als Standardtherapie fur die Behandlung der exsudativen AMD. Trotz konsequenter Behandlung mit Ranibizumab kann nicht bei allen Patienten eine vollstandige Befundberuhigung erreicht werden. Seit 2012 steht mit dem rekombinanten Fusionsprotein Aflibercept eine 2. gleichwertige Therapieoption zur Verfugung. Wir untersuchten, ob Patienten, die scheinbar nicht mehr auf Ranibizumab ansprechen, von einem Wechsel auf Aflibercept profitieren. Methoden: In diese retrospektive Studie wurden 83 Augen von 81 Patienten eingeschlossen, bei denen seit Zulassung von Aflibercept 2012 die Indikation zum Therapiewechsel nach therapierefraktarer Ranibizumab-Behandlung gestellt wurde. Einschlusskriterien waren ein Alter ≥ 50 Jahre und mindestens 10 stattgehabte Ranibizumab-IVI (IVI: intravitreale Injektion) am Studienauge vor Therapiewechsel auf Aflibercept. Ausgeschlossen wurden Patienten, die zusatzlich andere schwerwiegende visuslimitierende Erkrankungen aufwiesen. Primare Endpunkte waren die Visusentwicklung und die Beurteilung der zentralen Netzhautdicke (CMT) im OCT. Sekundare Endpunkte waren der prozentuale Anteil Augen, die nach Aflibercept-IVI keine Aktivitat der choroidalen Neovaskularisation (CNV) mehr zeigten und der Anteil der Augen mit Zeilengewinn/-verlust. Ein 49-jahriger Patient stellte sich mit Sehminderung und relativ afferentem Pupillendefizit rechts vor. In der statischen Perimetrie zeigte sich ein ausgepragter Gesichtsfelddefekt. Unter dem Verdacht auf eine Retrobulbarneuritis erfolgten die stationare Aufnahme und eine Schadel-Orbita-Magnetresonanztomographie. Dabei zeigte sich rechts intraorbital eine dem N. opticus semizirkular anliegende Raumforderung, die sich in der kraniellen Computertomographie (cCT) partiell verkalkt darstellte. Es wurde die Diagnose eines Meningeoms gestellt. Im Verlauf betrug der Visus am rechten Auge bestkorrigiert 1,0, die Gesichtsfelddefekte waren regredient. Therapieoptionen wurden interdisziplinar diskutiert, zunachst sind lediglich Verlaufskontrollen geplant.

Purpose: To retrospectively evaluate the re-injection interval, efficacy and safety of dexamethasone (DEX) intravitreal implant 0.7 mg in the treatment of macular oedema (ME) due to retinal vein occlusion (RVO) in Germany in 2009-2012. Methods: Retrospective, multicentre, anonymised observational study of data collected from the first DEX implant 0.7 mg injection through 3-6 months following the last injection. Data were included if the patient was >18 years old, had a diagnosis of ME secondary to branch or central RVO, and received at least 2 DEX implant 0.7 mg injections during routine practice. Results: Data from 87 patients were analysed. Mean time to re-injection between first and second treatments was 5.03 months in the total RVO population, and 5.46 and 4.52 months for the branch and central RVO subpopulations, respectively. An intraocular pressure increase of >25 mm Hg was recorded in 20% of patients, and 34% of patients began treatment with anti-glaucoma medication, but surgery was not needed for this condition. Conclusions: DEX implant 0.7 mg was found to be well tolerated and effective with repeat treatments in clinical practice.

The purpose of this study was to investigate the effectiveness of re-switch from intravitreal aflibercept to ranibizumab in patients with exudative age-related macular degeneration.This retrospective case series included 17 eyes of 17 patients who had previously switched from ranibizumab to aflibercept and finally back to ranibizumab. Main outcomes were change of visual acuity (VA) and assessment of central macular thickness (CMT). Secondary outcomes included predictive factors which had a beneficial effect as VA and CMT before re-switch, number of previous injections and gender.The mean VA was 0.64 ± 0.36 logMAR before the switch, and 0.87 ± 0.40 logMAR before the re-switch, and gained with a slight but not significantly improvement up to 0.85 ± 0.58 logMAR after the re-switch (p = 0.896). The average CMT before the switch was 448.6 µm ± 181.5. This decreased to 343.8 µm ± 161.3 after the switch (p = 0.614) to 299.1 µm ± 155.8 at switchback (p = 0.133). Overall, 8 patients (47%) had an improvement of vision, whereas in 5 patients (30%) VA deteriorated. Further analysis of predictive factors revealed a mean improvement of VA in male patients after re-switch, while female patients lost VA, with statistical significance between after the switch and after the re-switch to the benefit of male patients (p = 0.016).A re-switch from aflibercept to ranibizumab may enable improvement in morphological parameters and stabilization of VA in patients with exudative age-related macular degeneration who achieved no more benefit from the initial switch.In der vorliegenden Fallserie sollte die Wirksamkeit einer Therapierückumstellung von intravitrealem Aflibercept auf Ranibizumab bei Patienten mit persistierender Aktivität einer altersbedingten Makuladegeneration untersucht werden.Es wurde eine kleine Patientengruppe (17 Augen von 17 Patienten) untersucht, bei der zuerst eine Therapieumstellung von Ranibizumab auf Aflibercept und schließlich eine Therapierückumstellung auf Ranibizumab erfolgte. Als primäre Endpunkte wurden die Visusentwicklung und Veränderung der zentralen Netzhautdicke im SD-OCT festgelegt. Sekundärer Endpunkt war die Analyse prädiktiver Faktoren mit günstigem Effekt auf die Therapierückumstellung. Hierfür wurden das Geschlecht der Patienten, der Ausgangsvisus und die Netzhautdicke vor Therapierückumstellung sowie die jeweilige Anzahl der Vorinjektionen untersucht.Der durchschnittliche Visus vor Therapieumstellung betrug 0,64 ± 0,36 logMAR, vor Therapierückumstellung 0,87 ± 0,40 logMAR mit einer leichten, jedoch nicht signifikanten Verbesserung auf 0,85 ± 0,58 logMAR nach Therapierückumstellung (p = 0,896). Die zentrale Netzhautdicke vor Therapieumstellung betrug 448,6 ± 181,5 µm. Diese verringerte sich auf 343,8 ± 161,3 µm nach Therapieumstellung (p = 0,614) und knapp nicht statistisch signifikant auf 299,1 ± 155,8 µm nach Therapierückumstellung (p = 0,133). Hierbei zeigte sich im Vergleich vor und nach Therapierückumstellung ein signifikanter Unterschied (p = 0,016) zugunsten der Männer.Eine Therapierückumstellung von Aflibercept auf Ranibizumab könnte bei Patienten mit persistierender Aktivität einer altersbedingten Makuladegeneration, welche nicht mehr von der Therapieumstellung profitierten, eine Verbesserung der morphologischen Parameter und Visusstabilisierung ermöglichen.

Increased levels of systemic vascular endothelial growth factors (VEGFs) in patients with diabetes are associated with increased risk of microvessel disease. On the other hand, low VEGF levels after intravitreal antibody application may be associated with acute cardiovascular complications and treatment failure. Individual levels of systemic VEGF vary in a wide range depending on analytical methods and quality of diabetes control. So far only limited information exists on intraindividual fluctuations over longer periods and circadian rhythms. We analysed the intraindividual variance of VEGF-A, VEGF-C and placental growth factor (PLGF) in CTAD (citrate-theophylline-adenine-dipyridamol) plasma as well as VEGF-A in serum over a period of 6 months in patients with stable controlled type 2 diabetes (10 M, 10 F) and age and sex matched subjects with normal glucose tolerance (NGT). Furthermore, circadian levels of VEGFs were controlled hourly from 7:30 a.m. to 7:30 p.m. under standardized metabolic ward conditions. In addition, the relationship to metabolic, hormonal and inflammatory biomarkers was analyzed. VEGF-A, VEGF-C and PLGF remained stable in plasma and VEGF-A in serum over 6 months in both groups. No circadian change was observed in VEGF-A serum and plasma concentrations. A minor decrease of VEGF-C plasma levels was evident after 5 p.m. in both groups and a significant peak of PLGF concentrations occurred after lunch, which was more pronounced in T2DM. In multivariate analysis, only serum VEGF-A correlated to diabetes duration, whereas VEGF-C only correlated to HbA1c and fasting blood glucose. We did not observe significant intraindividual variances for VEGF-A in serum and VEGF-A, VEGF-C and PLGF in CTAD plasma over a period of 6 months. Taken together, a single morning measurement of systemic VEGF levels after 7:30 am appears to be a reliable parameter for the individual risk associated with abnormal VEGF concentrations in blood. Background: Retinal Angiomatous Proliferations (RAP) is a subgroup of exsudative or “wet” Age-related Macular Degeneration (wAMD) with devastating reduction of visual acuity in later stages. Intravitreal Ranibizumab provides a good therapy, but is considered to be not as effective in this class of neovascularization compared to choroidal neovascularization (CNV). We investigated the efficacy of Ranibizumab in late stage III RAP with retino-choroidal anastomosis compared to the outcome of CNV lesions. Methods: Retrospective analysis of the data of all for wAMD with Ranibizumab treated patients. Patients were divided into groups depending on the lesion type into RAP (identified and selected clinically, proven by fluorescein angiography) and CNV types (identified by fluorescein angiography only) named occult, minimally and predominantly classic groups. Best-corrected visual acuity (BCVA) was obtained before (“diagnosis”), during (1st, 2nd and 3rd injection) and after upload (“1st control”). Results: Before first injection the visual acuity decreased in all groups (0.73 to 0.78 logMAR for all CNV, 0.95 to 1.02 logMAR for RAP). During upload there is no further decline in visual acuity but no improvement as well up to the 1st control visit in the RAP group (1.02 to 1.03 logMAR), but a statistically significant increase in all other groups (0.78 to 0.67 logMAR). Conclusion: Clinically identified late stage III RAP lesions with retino-choroidal anastomosis respond worse to treatment with monthly Ranibizumab than all other lesion types regardless of their severity. Treatment results in stabiliziation of visual acuity, but – in contrast to other forms of CNV – no further improvement. Therefore, patients with this special form need to be identified and treated as early as possible.

Der folgende Fall einer 48-jahrigen Patientin berichtet uber eine einseitige akute Visusminderung am rechten Auge. Die Patientin war sonst gesund und bemerkte seit 2 Tagen Schwindel sowie eine reduzierte Sehscharfe. Als Nebenbefund bestand eine Zahnfleischschwellung, weswegen sie sich bereits in zahnarztlicher Behandlung befand. Ophthalmoskopisch zeigte sich am rechten Auge ein makularer Venolenverschluss mit Roth-Flecken. Die sofortige Fokussuche konnte als Ursache der vaskularen Pathologie eine bisher nicht bekannte akute myeloische Leukamie (AML) diagnostisch sichern. Die AML kann sich auf unterschiedliche Art und Weise manifestieren. Die Netzhaut ist in 50% der Falle mit betroffen. Durch ein sekundares Hyperviskositatssyndrom, wie es bei ca. 20% der akuten Leukamien auftritt, kann es zu symptomatischen Zentralvenenverschlussen kommen, oder der Verschluss, wie im vorliegenden Fall, betrifft primar nur eine kleine Venole. Somit kann sich eine AML auch als Erstes durch ophthalmologische Symptome bemerkbar machen. Daher sollte auch bei dezenter Symptomatik immer an eine systemische Grunderkrankung gedacht werden und eine Ursachenabklarung erfolgen. Eine Differenzialblutbilduntersuchung ist dabei unverzichtbar. Unter einer schnell veranlassten, adaquaten Therapie kommt es meist zu einer Befundbesserung. Eine augenarztliche Verlaufskontrolle ist unerlasslich, da auch die einzuleitende Chemotherapie ophthalmologische Nebenwirkungen aufweisen kann.

We present the case of a 52-year-old female patient who presented with bilateral loss of vision over the course of several years. Funduscopy revealed multiple drusen in a "honeycomb"-like configuration at the entire posterior pole and in the peripapillary region. Autofluorescence and fluorescence angiography (FLA) showed multiple hyperfluorescent defects which show no leakage due to staining of the drusen-like lesions. Optical coherence tomography (OCT) revealed a marked irregularity of the photoreceptor-retinal pigment epithelium complex. Electroretinography (ERG) was without pathological findings, while the electrooculography (EOG) was abnormal, reflecting a functional disturbance of the retinal pigment epithelium. In consideration of all clinical findings, our patient suffered from Doyne honeycomb retinal dystrophy (DHRD) which is a rare autosomal dominant inherited retinal disease with full penetrance. Patients with DHRD are usually asymptomatic until the age of 30-40 years. In the later stages of this disease, central vision deteriorates as a result of geographic atrophy or choroidal neovascularization.

We report the case of a 42-year-old patient who presented with unilateral loss of vision in the left eye. The clinical examination revealed retinal detachment and a low intraocular pressure was found in the left eye. Except for an extinct electroretinogram (ERG) and a siderosis bulbi in the left eye no signs of an intraocular body were detectable, either in the ultrasound B-scan or in the clinical examination. Pars plana vitrectomy with silicon oil tamponade was performed and revealed an iron-containing body next to the fovea which was removed without any complications. Despite an initially easily visible retinal detachment, a newly acquired siderosis bulbi in combination with ocular trauma in the clinical history should raise the suspicion of a residual intraocular iron foreign body, until this can be disproven.

The high-density silicone oil (Densiron 68), a mixture of F6H8 with silicone oil, seems to be a therapeutic option, at least in selected patients with complex inferior retinal re-detachment, where standard procedures have already failed. In an interventional case series we used Densiron as a primary endotamponade.Twelve eyes of 12 patients aged 31 years to 85 years with inferior complex rhegmatogenous retinal detachment with secondary proliferative vitreoretinopathy (PVR) grades CP2 to CA8 were included. Surgical techniques (pars plana vitrectomy, membrane peeling, retinotomy, retinectomy, endophotocoagulation, cryocoagulation, endotamponade) did not include a scleral buckling procedure (except one eye). Mean duration of the Densiron endotamponade was 78.3 +/- 29.74 days, with a mean follow up after removal of 400.6 +/- 85.4 days.After Densiron removal, four patients (33.3%) showed a stable reattached retina without further interventions, while, in six patients (50%), recurrent retinal re-detachment appeared during endotamponade, generally within 2 months. One patient (8.3%) developed re-detachment 5 months after Densiron removal. One eye (8.3%) lost light perception due to severe intraretinal fibrosis with chronic hypotonia, despite complete retinal re-attachment. Visual acuity improved from mean logarithm of the minimum angle of resolution (logMAR) of 2.95 +/- 1.21 to 1.87 +/- 1.32 (statistically significant, P = 0.022). Side effects included temporary inflammatory reaction/fibrin accumulation (n = 2/2), moderate-to-severe intraretinal fibrosis (n = 3), elevated intraocular pressure (IOP) (n = 3), emulsification (n = 2), sterile hypopyon (n = 1), vitreous haemorrhage (n = 1) and chronic hypotony (n = 1).Primary anatomical success rate of 33.3% was less encouraging than as expected. Especially, re-detachments within the posterior staphyloma in highly myopic patients were common during Densiron endotamponade. However, the surgical success increased to 75% after reintervention, even without the use of an additional encircling band. The observed adverse effects and the functional outcomes do not contraindicate the use of Densiron as an internal tamponade for a period of 3 months.

BACKGROUND: Until now it was thought that morphological parameters of the eye such as corneal thickness, corneal curvature and axial length do not affect tonometry results. However, the aim of this study was to find out whether there actually is an influence of these parameters on applanation tonometry. PATIENTS AND METHOD: In this prospective study we examined 125 eyes of 125 normal patients with a corneal thickness of 568.8 +/- 43.79 microm, a corneal curvature of 7.72 +/- 0.27 mm and an axial length of 23.62 +/- 2.05 mm. Before performing a phacoemulsification, the anterior chamber was temporarily punctured. With a closed system the intraocular pressure (IOP) was manometrically set at 20, 35 and 50 mmHg using an H (2)O column. The IOP was then measured with a Perkins tonometer. With these patients we compared 102 eyes that had undergone LASIK due to a myopia of 6.3 +/- 2.17 D. Before and 6 months after surgery, IOD, k-values and central corneal thickness of these patients were measured. RESULTS: At all set pressure levels there was a highly significant correlation of measured IOP and corneal thickness. At all set pressure levels the measured IOP significantly depended on corneal thickness (r(2) = 0.78 - 0.83). After LASIK, IOP was reduced from 16.5 +/- 2.1 to 12.9 +/- 1.9 mmHg. There was a significant correlation between IOP and corneal curvature as well as corneal thickness (r(2) = 0.631; P < 0.001). The biomechanical characteristics of the cornea are changed so that the measured IOP has to be corrected by an additional 0.75 mmHg. CONCLUSION: Since corneal thickness does affect Goldmann applanation tonometry we recommend to use the "Dresden Correction Table" (Tab. ) to achieve the real IOP. Pressure measurements after LASIK are inaccurate because of a change in corneal biomechanics, corneal thickness and curvature and they should be corrected as follows: IOP (real) = IOP (measured) + (540 - CCT)/71 + (43 - K-value)/2.7 + 0.75 mmHg.

Background Keratectasia is one of the most severe complications after refractive laser surgery. Usually penetrating keratoplasty is the treatment of choice to achieve an optical rehabilitation in such cases. Patients and methods We report on a female patient who developed keratectasia in both eyes 4 weeks after LASIK. Due to a severe keratectasia 10 months after LASIK, a treatment with riboflavin/UVA cross-linking was performed. Results Due to the induced collagen cross-linking the biomechanical status of the cornea was stabilized and a progression of the keratectasia was prevented. The postoperative refraction and corneal topography have been stable for 18 months. Conclusion Collagen cross-linking leads to a stiffening of the anterior parts of the corneal stroma. The increase of biomechanical stability can stop the progression of a keratectasia after LASIK by means of a simple procedure.

An irregular corneal surface compromises IOP measurement by Goldmann applanation tonometry. In such cases accurate measurement without corneal contact would be desirable. The new eyelid tonometer TGDc-01 measures IOP without corneal contact through the eyelid. The aim of the study was to evaluate the accuracy of the eyelid tonometer compared with Goldmann applanation tonometry (cornea thickness-corrected values) in subjects without corneal alterations.IOP was measured in 199 eyes of 103 subjects without corneal alterations by means of two different methods. Measurements with the transpalpebral tonometer TGDc-01 and the Goldmann applanation tonometer were performed within 5 min in random order.The mean difference between lid tonometry and Goldmann applanation tonometry was 0.71 mmHg, SD +/-2.467 mmHg. In the reliability analysis the intraclass correlation coefficient was 0.8620. Compared with Goldmann applanation tonometry 66.4% of the IOP readings measured by lid tonometry were in an interval of +/-2 mmHg, 81.0% in an interval of +/-3 mmHg. The maximum of deviation was -6 mmHg and +6 mmHg, respectively. The Bland and Altman plots are shown.Lid tonometry correlates sufficiently with Goldmann applanation tonometry, but in more than 10% of the measurements the IOP readings differed by more than 3 mmHg. The eyelid tonometer may be helpful as a screening tool when Goldmann applanation tonometry is not applicable.

BACKGROUND Recently interferon therapy has become increasingly important in the treatment of viral and malignant diseases. Subsequently, a variety of adverse effects of interferon use has been reported. Ocular complications following interferon therapy include retinal ischaemia with cotton wool spots, retinal haemorrhages, capillary nonperfusion, vasospasm, and arteriolar occlusion. CASE REPORT A 37-year old male complained about increasingly blurred vision in his right eye. Because of a chronic type B hepatitis he has been treated with interferon alpha for a total of 35 weeks. Visual acuity was slightly reduced (OD cc 0.9, OS cc 1.0 p). Dilated fundus examination and fluorescein angiography showed an incipient ischaemic retinopathy with perimacular arteriolar alterations in both eyes and single cotton wool spots in his right eye. Perimetry showed a slight reduction of the mean deviation. Amsler-grid did not show any pathology. As the retinopathy was thought to be related to the interferon treatment, it was decided to discontinue this therapy immediately. 2 weeks later visual acuity was improved (OD cc 1.25 p, OS cc 1.0) and dilated fundus examination showed no signs of perimacular ischaemic retinopathy and the initial cotton wool spots had been resolved. CONCLUSION In patients with long-time interferon treatment non-specific visual complaints like blurred vision or dark spots might be symptoms of an early ischaemic retinopathy. Retinal alterations might be reversible if treatment is discontinued. If an ischaemic retinopathy is present it should be considered to abandon interferon therapy, since permanent loss of vision has been described in advanced capillary non-perfusion and arteriolar occlusion. However, the decision to discontinue the interferon therapy should be made interdisciplinary while considering diagnosis and therapy indication.

To investigate the effect(s) of intravitreally injected ranibizumab on retinal vessel diameter in patients with diabetic macular edema.Participants of this prospective study were 14 men and 16 women (30 eyes) aged 60 ± 11 years (mean ± standard deviation), all with clinically significant diabetic macular edema. Treatment comprised 3 intravitreal injections of ranibizumab given at 4-week intervals. Examinations were conducted before the first (baseline), before the second (Month 1), before the third (Month 2) injections, and 3 months after baseline (Month 3). Measured parameters included systemic blood pressure, static retinal vessel analysis (central retinal artery equivalent and central retinal vein equivalent), and dynamic retinal vessel analysis, as measured by the change in vessel diameter in response to flicker stimulation during three measurement cycles. Flicker stimulation was accomplished using a 50-second baseline recording, followed by an online measurement during 20-second flicker stimulation and 80-second online measurements in both arteriolar and venular vessel segments.Static retinal vessel analysis showed a reduction of central retinal artery equivalent from 186.25 ± 51.40 μm (baseline) to 173.20 ± 22.2 μm (Month 1), to 174.30 ± 27.30 μm (Month 2), and to 170.56 ± 22.89 μm (Month 3), none of which was statistically significant (P = 0.23, 0.12, and 0.14, respectively). Central retinal vein equivalent was reduced from 216.21 ± 25.0 μm (baseline) to 214.48 ± 25.4 μm (Month 1), to 214.80 ± 24.30 μm (Month 2), and to 211.41 ± 24.30 μm (Month 3), revealing no statistically significant differences between examination time points (P = 0.54, 0.06, and 0.24, respectively). Dynamic vessel analysis yielded a mean retinal arterial diameter change of +1.47% ± 2.3 (baseline), +1.91% ± 2.5 (Month 1), +1.76% ± 2.2 (Month 2), and +1.66% ± 2.1 (Month 3), none of which showed statistically significant differences (P = 0.32, 0.49, and 0.70, respectively). Mean retinal venous diameter changes were +3.15% ± 1.7 (baseline), +3.7% ± 2.3 (Month 1), +4.0% ± 2.0 (Month 2), and +4.95% ± 1.9 (Month 3), none of which showed statistically significant differences (P = 0.12, 0.17, and 0.14, respectively). Central retinal thickness, as measured by spectral domain optical coherence tomography, decreased significantly from 435.2 ± 131.8 μm (baseline) to 372.3 ± 142.8 μm (Month 3), P = 0.01. Regression analysis of arteriolar and venular diameters indicated that there was no significant correlation between these 2 parameters (r = 0.053; P = 0.835 and r = 0.06; P = 0.817, respectively). Also, no significant correlation was observed between the difference in the central retinal thickness and change in arteriolar or venular dilatation (r = 0.291, P = 0.241 and r = 0.06, P = 0.435, respectively).Intravitreally applied ranibizumab did not significantly affect retinal vessel diameter in patients with diabetic macular edema. Decline in the central foveal thickness after ranibizumab therapy, as measured by spectral domain optical coherence tomography, was not linked to any change in retinal vessel diameter or dilatatory response, neither for arterioles nor venules.

Purpose To evaluate intravitreal aflibercept for treatment of macular edema secondary to central retinal vein occlusion (CRVO). Design Randomized, double-masked, phase 3 study. Methods A total of 177 patients with macular edema secondary to CRVO were randomized to receive 2 mg intravitreal aflibercept (n = 106) or sham (n = 71) every 4 weeks for 20 weeks. From weeks 24 to 48, patients were monitored every 4 weeks; the former group received intravitreal aflibercept as needed (PRN), and the sham group received sham. From weeks 52 to 76, patients were monitored every 8 weeks, and both groups received intravitreal aflibercept PRN. The primary endpoint (proportion of patients who gained ≥15 letters) was at week 24. This study reports exploratory outcomes at week 76. Results The proportion of patients who gained ≥15 letters in the intravitreal aflibercept and sham groups was 60.2% vs 22.1% at week 24 (patients discontinued before week 24 were considered nonresponders; P P P .001). Mean μm change from baseline central retinal thickness was −448.6 vs −169.3 at week 24 ( P P P = .1122). Over 76 weeks, the most common ocular serious adverse event in the intravitreal aflibercept group was macular edema (3.8%). Conclusions The visual and anatomic improvements seen after fixed, monthly dosing at week 24 were largely maintained when treatment intervals were extended. Patients with macular edema following CRVO benefited from early treatment with intravitreal aflibercept.

Levels of vascular endothelial growth factors (VEGF) are regulated in a complex network of adipokines, glucose control, and low grade inflammation together with activated platelets, leucocytes, and endothelial dysfunction. Increased levels of VEGF are associated with enhanced angiogenesis and impaired repair mechanisms of vascular lesions in endorgans. Little is known about the interaction of systemic VEGF levels with quality of diabetes control, biomarkers of inflammation, and diabetic nephropathy. Moreover, it is unclear, whether serum and plasma VEGF levels are similarly suited to reflect risk associated with VEGF. In this case control study, we analyzed these parameters in serum and plasma of age and sex matched controls without diabetes (n=99) and type 2 diabetes (n=302). Serum VEGF-A was significantly increased in patients with T2DM while plasma levels were in the same range as for controls. Individual levels varied in a wide range. Serum levels were 4.9 times higher in controls and 7.3 times higher in T2DM as compared to plasma levels. T2DM was associated with significantly higher levels of hsCRP, ALAT, and albumin/creatinine ratio. When calculated for tertiles of HbA1c, we observed a highly significant increase from tertile one to the upper tertile for serum VEGF-A but not for plasma VEGF-A. Correlation analysis revealed a significant relationship between VEGF-A, HbA1c, inflammation, and diabetic nephropathy. Our results indicate that increased VEGF-A levels in T2DM significantly depend on quality of HbA1c control. Serum levels of VEGF-A, with a strong contribution of platelet derived VEGF, better reflect the glycemic burden than plasma levels of VEGF-A. Mechanistic studies are needed to explore links to inflammation and diabetic nephropathy.

Objective: To evaluate the safety and efficacy of 1 or 2 treatments with dexamethasone intravitreal implant (DEX implant) over 12 months in eyes with macular edema owing to branch or central retinal vein occlusion (BRVO or CRVO). Design: Two identical, multicenter, prospective studies included a randomized, 6-month, double-masked, sham-controlled phase followed by a 6-month open-label extension. Participants: We included 1256 patients with vision loss owing to macular edema associated with BRVO or CRVO. Methods: At baseline, patients received DEX implant 0.7 mg (n = 421), DEX implant 0.35 mg (n = 412), or sham (n = 423) in the study eye. At day 180, patients could receive DEX implant 0.7 mg if best-corrected visual acuity (BCVA) was 250 mu m. Main Outcome Measures: The primary outcome for the open-label extension was safety; BCVA was also evaluated. Results: At day 180, 997 patients received open-label DEX implant. Except for cataract, the incidence of ocular adverse events was similar in patients who received their first or second DEX implant. Over 12 months, cataract progression occurred in 90 of 302 phakic eyes (29.8%) that received 2 DEX implant 0.7 mg injections versus 5 of 88 sham-treated phakic eyes (5.7%); cataract surgery was performed in 4 of 302 (1.3%) and 1 of 88 (1.1%) eyes, respectively. In the group receiving two 0.7-mg DEX implants (n = 341), a >= 10-mmHg intraocular pressure (IOP) increase from baseline was observed in (12.6% after the first treatment, and 15.4% after the second). The IOP increases were usually transient and controlled with medication or observation; an additional 10.3% of patients initiated IOP-lowering medications after the second treatment. A >= 15-letter improvement in BCVA from baseline was achieved by 30% and 32% of patients 60 days after the first and second DEX implant, respectively. Conclusions: Among patients with macular edema owing to BRVO or CRVO, single and repeated treatment with DEX implant had a favorable safety profile over 12 months. In patients who qualified for and received 2 DEX implant injections, the efficacy and safety of the 2 implants were similar with the exception of cataract progression. Objective: To evaluate the safety and efficacy of dexamethasone intravitreal implant (DEX implant; OZURDEX, Allergan, Inc., Irvine, CA) compared with sham in eyes with vision loss due to macular edema (ME) associated with branch retinal vein occlusion (BRVO) or central retinal vein occlusion (CRVO). Design: Two identical, multicenter, masked, randomized, 6-month, sham-controlled clinical trials (each of which included patients with BRVO and patients with CRVO). Participants: A total of 1267 patients with vision loss due to ME associated with BRVO or CRVO. Intervention: A single treatment with DEX implant 0.7 mg (n = 427), DEX implant 0.35 mg (n = 414), or sham (n = 426). Main Outcome Measures: The primary outcome measure for the pooled data from the 2 studies was time to achieve a >= 15-letter improvement in best-corrected visual acuity (BCVA). Secondary end points included BCVA, central retinal thickness, and safety. Results: After a single administration, the time to achieve a >= 15-letter improvement in BCVA was significantly less in both DEX implant groups compared with sham (P= 15-letter improvement in BCVA was significantly higher in both DEX implant groups compared with sham at days 30 to 90 (P= 15-letter loss in BCVA was significantly lower in the DEX implant 0.7-mg group compared with sham at all follow-up visits (P 25 mmHg peaked at 16% at day 60 (both doses) and was not different from sham by day 180. There was no significant between-group difference in the occurrence of cataract or cataract surgery. Conclusions: Dexamethasone intravitreal implant can both reduce the risk of vision loss and improve the speed and incidence of visual improvement in eyes with ME secondary to BRVO or CRVO and may be a useful therapeutic option for eyes with these conditions.

Background: High-density silicone oils are newly developed long-term tamponade agents for the treatment of complicated retinal detachment in the inferior retina. Previous studies describe satisfying anatomical and functional results. In this study we examined the largest cohort so far for a 9-month follow-up and performed a comparison to conventional silicone oil. Methods: Our study documents results and adverse effects after vitreoretinal surgery and endotamponade with Densiron® 68 in 99 cases of complicated retinal detachment. A 9-month follow-up was performed. Data of 21 patients with intraocular conventional silicone oil tamponade in complicated retinal detachment were retrospectively analyzed and served as control. Results: Anatomical success was achieved in 78 of 89 eyes (87.6%) with completed follow-up; visual acuity did not change significantly (from mean preoperative logMAR 1.88 to postoperative logMAR 1.96 (p = 0.9). Compared to control a higher anatomical success but a similar number of adverse effects were observed with heavy silicone oil in vitreous. Nevertheless, patients who received Densiron 68 twice due to redetachment showed a significantly higher rate of intraocular inflammation with the tamponade agent in situ. Conclusion: Our results support the hypothesis of Densiron 68 as potent tamponade agent for complicated retinal detachment in the inferior retinal segments especially in eyes where a previous operation failed. In this study, we evaluated the anatomic and functional outcome and intraocular adverse effects after a 3-month endotamponade with Densiron 68, a mixture of F(6)H(8) with silicone oil, in complex inferior re-detachments.Forty-eight eyes of 48 patients aged 27-82 years with retinal re-detachment due to proliferative vitreoretinopathy (PVR) grades CP1 to CA7 were included. Mean duration of the Densiron endotamponade was 108.7+/-66.9 days, with a mean follow-up after removal of 102.8+/-31.9 days.Twenty-two patients (45.8%) showed stable retinal reattachment after Densiron removal. Fourteen patients (29.2%) developed retinal re-detachment after removal, generally within 1 month and in the upper circumference (n=8). In 11 patients (22.9%) recurrent re-detachment (inferior n=8) appeared during Densiron endotamponade. In one eye (2.1%) treatment was primarily unsuccessful. Visual acuity improved from mean logMAR 1.66+/-1.03 to 1.47+/-0.97 (not statistically significant, P=0.257). Side effects included temporary inflammatory reaction (n=10), fibrin accumulation (n=6), sterile hypopyon (n=2), vitreous hemorrhage (n=6), elevated IOP (n=5), emulsification (n=4) and chronic hypotony (n=4).The anatomical success rate without further interventions of 45.8% (22 of 48 patients) seems unsatisfactory. However, in evaluating the potential of Densiron, it should be considered that all patients in this study had previous surgery with standard procedures, including silicone oil, which had already failed. Intraoperative laser photocoagulation of the periphery of the upper quadrants might reduce the risk of retinal re-detachments.

Recently interferon therapy has become increasingly important in the treatment of viral and malignant diseases. Subsequently, a variety of adverse effects of interferon use has been reported. Ocular complications following interferon therapy include retinal ischaemia with cotton wool spots, retinal haemorrhages, capillary nonperfusion, vasospasm, and arteriolar occlusion.A 37-year old male complained about increasingly blurred vision in his right eye. Because of a chronic type B hepatitis he has been treated with interferon alpha for a total of 35 weeks. Visual acuity was slightly reduced (OD cc 0.9, OS cc 1.0 p). Dilated fundus examination and fluorescein angiography showed an incipient ischaemic retinopathy with perimacular arteriolar alterations in both eyes and single cotton wool spots in his right eye. Perimetry showed a slight reduction of the mean deviation. Amsler-grid did not show any pathology. As the retinopathy was thought to be related to the interferon treatment, it was decided to discontinue this therapy immediately. 2 weeks later visual acuity was improved (OD cc 1.25 p, OS cc 1.0) and dilated fundus examination showed no signs of perimacular ischaemic retinopathy and the initial cotton wool spots had been resolved.In patients with long-time interferon treatment non-specific visual complaints like blurred vision or dark spots might be symptoms of an early ischaemic retinopathy. Retinal alterations might be reversible if treatment is discontinued. If an ischaemic retinopathy is present it should be considered to abandon interferon therapy, since permanent loss of vision has been described in advanced capillary non-perfusion and arteriolar occlusion. However, the decision to discontinue the interferon therapy should be made interdisciplinary while considering diagnosis and therapy indication.
