Amaç: Lomber diskektomi sonrasında görülen spondilodiskit oldukça nadir olmakla beraber insidansı birçok farklı yazar tarafından %0,1-18,8 civarında bildirilmiştir. En sık görülen patojen Staphylococcus aureus’tur. Yöntemler: Hastanemizde 2007-2015 yılları arasında tek seviye veya iki seviye lomber disk hernisi nedeniyle opere etmiş olduğumuz 1.154 hastanın medikal kayıtları retrospektif olarak çıkartıldı. Bu hastaların 554’ü kadın, 600’ü erkek hasta idi. Bu hastaların 1.062’sine tek seviye, 91’ine 2 seviye lomber mikrocerrahi ile diskektomi operasyonu uygulandı. Bu hastaların tamamına anestezi esnasında cerrahi antimikrobiyal profilaksi rehberlerinin önerileri doğrultusunda profilaktik tek doz sefazolin sodyum verildi. Hastaların 12’sinde (%1,03) spondilodiskit gelişti. Spondilodiskit gelişmiş hastadaki komorbiditeler, izole edilen patojenler, antibiyotik duyarlılıkları, kullandıkları antibiyotikler ve hastanede kalış süreleri not edildi. Bulgular: On iki hastanın 7’si kadın, 5’i erkekti. Ortalama yaş 45,75±14,16 yıl idi. Bu hastaların 11’i tek seviye, 1 tanesi 2 seviye lomber mikrodiskektomi operasyonu geçirdi. Beş hasta L4-5 ve 8 hasta L5-S1 seviyelerinden diskektomi operasyonu geçirdi. Bu hastaların 3’ünde S. aureus (%25), 4’ünde Staphylococcus epidermidis (%33), 3’ünde Escherichia coli (%25) ve 2 hastada üreme olmadı. Ortalama hastanede kalış süresi 29,45±3,98, spondilodiskit olmamış hastalarda ise 1,99±0,81 olup iki grup birbirinden anlamlı derecede farklıydı (p=0,0001). Sonuç: Çoğu cerrahın antibiyotik profilaksisini postoperatif dönemde veya hastanın yattığı süre içerisinde devam ettirme eğilimi olmasına rağmen, çalışmamızda sadece anestezi indüksiyonu esnasında yapılan tek doz profilaktik antibiyotiğin spondilodiskit oranını tıbbi literatüre göre artırmadığı tespit edilmiştir., Objective: Although spondylodiscitis seen after lumbar discectomy is very rare, its incidence has been reported to be around 0.1-18.8% by many different authors. The most common pathogen is Staphylococcus aureus. Methods: Medical records of 1,154 patients who were operated in our hospital between 2007 and 2015 due to a single or two-level lumbar disc hernia were retrospectively extracted. Of these patients, 554 were female and 600 were male. Discectomy operation was performed in 1,062 of these patients with single-level and 91 with two-level lumbar microdiscectomy. All of these patients were given a prophylactic single dose of cefazolin sodium in accordance with the recommendations of the surgical antimicrobial prophylaxis guidelines during anesthesia. Spondylodiscitis developed in 12 patients (1.03%). Comorbidities in patients who developed spondylodiscitis, isolated pathogens, antibiotic susceptibility, antibiotics used, and hospital stay were noted. Results: Of the 12 patients, 7 were female and 5 were male. Mean age was 45.75±14.16 years. Eleven of these patients underwent single level, one had 2 levels of lumbar microdiscectomy. Five patients underwent discectomy at L4-5 and 8 patients at L5-S1 levels. Three of these patients had S. aureus (25%), 4 had Staphylococcus epidermidis (33%) and 3 had Escherichia coli (25%) and 2 patients had no reproduction. The mean hospital stay was 29.45±3.98, and in patients without spondylodiscitöis it was 1.99±0.81, the two groups were significantly different from each other (p=0.0001). Conclusion: Although most surgeons have a tendency to maintain antibiotic prophylaxis postoperatively or during hospitalization period, our study found that a single dose prophylactic antibiotic administered during anesthesia induction did not increase rate of spondylodiscitis by medical literature.

Objective: The optimal surgical approach for treatment of thoracolumbar junction disc herniations (TLJDH) remains controversial. Anterolateral retroperitoneal, anterior transthoracic, posterolateral, lateral and transforaminal endoscopic approaches are used in current surgical treatment of TLJDH. In a posterolateral transcambin approach, a small piece of bone is removed from the pars interarticularis and facet joints, and decompression is achieved by discectomy without causing instability. The transcambin approach has advantages such as discectomy without advanced neural tissue retraction, short recovery time and less postoperative complications. Methods: The eight patients with 3 T12-L1 and 5 L1-2 TLJDH who were operated on using the posterolateral transcambin approach between 2016-2018 in our clinic were presented retrospectively. The study included patients with extruded or sequestrated disc herniation that had far lateral, foraminal, and paramedian extension, and caused cauda equina syndrome, conus medullaris syndrome, and radiculopathy. A skin incision and fascia opening were done from midline. The exiting root was revealed at laterally of pars interarticularis by subperiosteally stripping of paravertebral muscles and the removal of disc fragments were targeted. Results: Three male and five female patients with a mean age of 45.1 (32-66). Mean operation time was 57.8 minutes. Preoperative VAS values were 8.75±0.51 and postoperative VAS values were 1.25±0.65. According to the MacNab classification, 62.5% excellent, 25% good and 12.5% were fair. There was no neurological injury, cerebrospinal fluid (CSF) fistula, pneumothorax, or operation field hematoma. Conclusion: The transcambin approach was found to be useful in terms of a relatively short operation time, low morbidity probability, and less postoperative pain in patients with TLJDH., Amaç: Torakolomber bileşke disk herniasyonlarının (TLBDH) tedavisi için optimal cerrahi yaklaşım biçimi halen tartışmalıdır. TLBDH’nın güncel cerrahi tedavisinde anterolateral retroperitoneal, anterior transtorasik, posterolateral, lateral ve transforaminal endoskopik yaklaşımlar uygulanmaktadır. Paravertebral adeleler subperiostal sıyrılarak pars lateralinde çıkan kök açığa konulup sekestre veya ekstrüde disk fragmanlarının alınması hedeflendi. Bulgular: Hastaların 3’ü erkek, 5’i kadın ve ortalama yaşı 45,1 (3266) olarak saptandı. Ortalama operasyon süresi 57,8 dakikaydı. Preoperatif VAS değerleri 8,75±0,51, postoperatif VAS değerleri 1,25±0,65 saptandı. MacNab klasifikasyonuna göre postoperatif 6 ay sonraki sonuçları; %62,5‘i mükemmel, %25’i iyi, %12,5’i vasat olarak bulundu. Hastalarda nörolojik hasar, beyin omurilik sıvısı (BOS) fistülü, pnömotoraks ve operasyon lojunda hematom gelişmedi. Sonuç: TLBDH’li hastalarda transkambin yaklaşım, görece kısa operasyon süresi, düşük morbidite olasılığı ve daha az postoperatif ağrıya sebep olması açısından yararlı bir yaklaşım olarak bulunmuştur.

Amaç: Vestibüler şvannoma vakaları, gelişen görüntüleme teknikleri sebebiyle günümüzde daha sık izlenmektedir. Bu çalışmada, İstanbul Üniversitesi İstanbul Tıp Fakültesi Nöroşirurji Anabilim Dalı’nda retrosigmoid kranyektomi tekniğiyle ameliyat edilen hastaların sonuçları retrospektif olarak değerlendirildi. Yöntemler: Kliniğimizde 2012-2017 tarihleri arasında opere edilen vestibuler şvannoma tanılı 18 hastanın radyolojik ve klinik bulguları değerlendirildi. Hastalara olası tedavi yöntemleri detaylı olarak anlatıldıktan sonra hepsinden oral ve yazılı aydınlatılmış onam alındı. Bulgular: Çalışmamızda, 13 kadın ve 5 erkekten oluşan toplam 18 hasta incelendi. Kitle çapı 3cm’nin altında olan 11 hasta; 3cm’nin üstünde olan 7 hasta saptandı. Tüm hastalara retrosigmoid kranyektomi ile kitle rezeksiyonu yapıldı. Hastaların 11’inde total rezeksiyon, 7’sinde subtotal rezeksiyon yapıldı. Total rezeksiyon yapılan hastaların yedisinde (%63,6) yeni gelişen fasyal parezi izlendi. Üçer hastada (%27,2) yeni gelişen glossofarengeal ve abdusens sinir parezisi izlendi. 3cm’den büyük çaplı tümörlerde %42 oranında fasyal sinir korundu. Tüm hastaların içerisinde bir hastada (%5,5) BOS fistülü; bir hastada (%5,5) da hidrosefali gelişti. Sonuç: Vestibüler şvannom olgularında, mikrocerrahi, radyocerrahi ve ‘bekle/takip et’ olmak üzere üç yaklaşım izlenmektedir. Her ne kadar 3cm’nin üstündeki tümörlerde altın standart mikrocerrahi olsa da, 3cm’nin altındaki tümörlerde tedavi planı tartışmalıdır. Yaptığımız bu çalışmada, kitle boyutu 3cm’nin altında olan ve total rezeksiyon yapılan hastaların hepsinde postoperatif fasyal parezi gelişmiştir. Total rezeksiyon yapılan hastalarda gelişen 6. ve 9. kafa çifti parezilerinin, subtotal rezeksiyon yapılan hastalarda daha az sıklıkla görülmüş olması, total-subtotal rezeksiyon kararını etkileyebilecek bir faktördür. Bu nedenle 3cm’den büyük çaplı tümörlerde mevcut nöromonitörizasyon imkanları da göz önünde bulundurularak (total rezeksiyon/subtotal rezeksiyon + radyocerrahi) tedavi planı göz önünde bulundurulmalıdır. In this study, the results of the patients who underwent surgery in the Department of Neurosurgery, Istanbul University Istanbul School of Medicine were retrospectively evaluated. Methods: Radiological and clinical findings of 18 patients with vestibular schwannoma diagnosed in our clinic from 2012 to 2017 were evaluated. After describing the possible treatment methods of the disease in detail, oral and written informed consent was obtained from all of them. Results: In our study, 18 patients, consisting of 13 women and 5 males, were examined. Eleven patients were found with a mass diameter below 3 cm, and seven patients were found with mass diameter above 3 cm. All patients underwent mass resection with retrosigmoid craniotomy. Total resection was performed in 11 patients, and subtotal resection was done in 7 patients. In seven patients with total resection (63.6%), a newly developed facial paresis was observed. The newly developed glossopharyngeal nerve paresis was observed in three patients (27.2%), and also abducens nerve paresis in three patients (27.2%). The facial nerve was preserved at a rate of 42% in tumors larger than 3 cm in size. The CSF fistula developed in one patient (5.5%); hydrocephalus developed in one patient (5.5%). Conclusion: In cases of vestibular schwannoma, microsurgery, radiosurgery, and "wait/follow" approaches are monitored. Although gold is standard microsurgery in tumors sized above 3 cm, the treatment plan for tumors under 3 cm in size is controversial. In this study, postoperative facial paresis was developed in all patients with total resection, which were below the mass size of 3 cm. The fact that the abducens and glossopharyngeal nerve paresis in patients with total resection have been seen less frequently in patients with subtotal resection is a factor that can affect the decision of total-subtotal resection. Therefore, the treatment plan should be taken into consideration (total resection/subtotal resection + radiosurgery) considering the existing neuromonitoring facilities in tumors larger than 3 cm in size.

Amaç: Endoskopik girişimler nöroşirürji uygulamalarında konvansiyel yöntemlerin yerini almaktadır. Bu çalışmanın amacı nöral ve vasküler yapıların pons-serebellum sisternası anatomisinin endoskopik retrosigmoid suboksipital yaklaşımla incelemek ve nöral-vasküler yapılar ile ilgili ölçümler yapmaktır. Yöntemler: Çalışmada 5 adet insan kuru kadavrası kullanılarak, endoskopik retrosigmoid yaklaşımla pons-serebellum sisternası incelendi. Sisterna içindeki sinirlerin etrafındaki vasküler yapılar ile kranyektomi çapının boyutları, petrozal ven, trigerminal ve fasyal-vestibulokoklear sinirlerin duraya olan uzaklıkları, sinirlerin boyları, trigeminal sinirin çapı ve sinirlerin birbirlerine ve petrozal vene olan uzaklıkları ölçüldü. Bulgular: Dura ile petrozal ven, tigeminal sinir ve fasyal-vestibulokoklear sinirler arasındaki mesafe sırasıyla 34-53 mm (ort. 43,5 mm), 46-62 mm (ort. 54,1 mm) ve 37-47 mm (ort. 42 mm) ölçüldü. Trigeminal sinirin çapı 1,27-3,27 mm (ort. 1,8 mm) olarak ölçüldü. Petrozal ven ile trigeminal sinir ve fasyal-vestibülokoklear sinirler arasındaki mesafe sırasıyla 6-13 mm (ort. 9,5 mm) ve 6-12 mm (ort. 8,6 mm), trigeminal sinir ile fasyal ve vestibülokoklear sinirler arasındaki mesafe 9-15 mm (ort. 10,1 mm) ölçüldü. Çalışmada kullanılan beş kadavrada, beş tane trigeminal sinire ve beş tane fasyal-vestibülokoklear sinirlere arter teması izlendi. Sonuç: Yapılan çalışmada, pons-serebellum sisternası ve sisternanın içindeki anatomik yapılar endoskopik olarak detaylı bir şekilde incelenebildi. Trigeminal sinire ve fasyal-vestibülokoklear sinirlere sıklıkla damar basısı olduğu görüldü., Objective: Endoscopic interventions are rapidly replacing conventional methods in neurosurgical applications. The main purpose of this study was to examine the neurovascular structures in the pons-cerebellum cistern in detail through the endoscopic retrosigmoid suboccipital approach and to make morphometric measurements. Methods: In five human dry cadavers, pons-cerebellum cisterna was examined by endoscopic retrosigmoid approach. Relations with the vascular structures surrounding the nerves, the dimensions of the craniectomy diameter, the distances of the petrosal vein and the trigeminal and facial–vestibulocochlear nerves to the dura, the length of the nerves, the diameter of the trigeminal nerves, and the distance between the nerves and petrosal vein were measured. Results: The distances between the dura and the petrosal vein, trigeminal nerve and facial-vestibulocochlear nerves were 34–53 mm (mean, 43.5 mm), 46–62 mm (mean, 54.1 mm), and 37–47 mm (mean, 42 mm), respectively. The diameter of the trigeminal nerve was measured as 1.27–3.27 mm (mean, 1.8 mm). The distances between the petrosal vein and the trigeminal nerve and facial–vestibulocochlear nerves were 6–13 mm (mean, 9.5 mm) and 6–12 mm (mean, 8.6 mm), respectively, and the distance between the trigeminal nerve and facial–vestibulocochlear nerves was 9-15 mm (mean, 10.1 mm). Arterial compression was observed at the five trigeminal nerves and five fascial-vestibulocochlear nerves. Conclusion: In this study, pons-cerebellum cistern and anatomical neurovascular structures in this place were able to examine with endoscope in detail. Vascular compression was frequently observed in trigeminal nerve and facial–vestibulocochlear nerves.

Background Hypoxic-ischemic encephalopathy (HIE) is one of the leading causes of death and long-term neurological impairment in the pediatric population. Despite a limited number of treatments to cure HIE, stem cell therapies appear to be a potential treatment option for brain injury resulting from HIE. Aim To investigate the efficacy and safety of stem cell-based therapies in pediatric patients with HIE. METHODS The study inclusion criteria were determined as the presence of substantial deficit and disability caused by HIE. Wharton’s jelly-derived mesenchymal stem cells (WJ-MSCs) were intrathecally (IT), intramuscularly (IM), and intravenously administered to participants at a dose of 1 × 106/kg for each administration route twice monthly for 2 mo. In different follow-up durations, the effect of WJ-MSCs administration on HIE, the quality of life, prognosis of patients, and side effects were investigated, and patients were evaluated for neurological, cognitive functions, and spasticity using the Wee Functional Independence Measure (Wee FIM) Scale and Modified Ashworth (MA) Scale. Results For all participants (n = 6), the mean duration of exposure to hypoxia was 39.17 + 18.82 min, the mean time interval after HIE was 21.83 ± 26.60 mo, the mean baseline Wee FIM scale score was 13.5 ± 0.55, and the mean baseline MA scale score was 35 ± 9.08. Three patients developed only early complications such as low-grade fever, mild headache associated with IT injection, and muscle pain associated with IM injection, all of which were transient and disappeared within 24 h. The treatment was evaluated to be safe and effective as demonstrated by magnetic resonance imaging examinations, electroencephalographies, laboratory tests, and neurological and functional scores of patients. Patients exhibited significant improvements in all neurological functions through a 12-mo follow-up. The mean Wee FIM scale score of participants increased from 13.5 ± 0.55 to 15.17 ± 1.6 points (mean ± SD) at 1 mo (z = - 1.826, P = 0.068) and to 23.5 ± 3.39 points at 12 mo (z = -2.207, P = 0.027) post-treatment. The percentage of patients who achieved an excellent functional improvement (Wee FIM scale total score = 126) increased from 10.71% (at baseline) to 12.03% at 1 mo and to 18.65% at 12 mo posttreatment. Conclusion Both the triple-route and multiple WJ-MSC implantations were safe and effective in pediatric patients with HIE with significant neurological and functional improvements. The results of this study support conducting further randomized, placebo-controlled studies on this treatment in the pediatric population. WOS:000661465500008 Q2
