Abstract

Bimekizumab è un anticorpo monoclonale umanizzato, recentemente approvato per il trattamento della psoriasi a placche moderato-grave, inibitore selettivo delle isoforme dell’interleuchina-17A e F. Le esperienze real-life riguardanti l’utilizzo del farmaco sono limitate ed il profilo del paziente con riposta più elevata PASI100 cioè super responder SR non è stato analizzato. Presentiamo questo studio multicentrico, retrospettivo volto a disegnare il profilo del paziente che beneficia maggiormente del trattamento con bimekizumab, considerando come SR coloro che raggiungono PASI 100 alla settimana-4 e 16. Sono stati studiati pazienti adulti affetti da psoriasi moderato-grave, trattati con bimekizumab per almeno 16 settimane presso 21 centri dermatologici italiani, secondo regole di appropriatezza AIFA e in accordo con la scheda tecnica del farmaco. Endpoints di efficacia erano PASI75, PASI90 e PASI100 alla settimana 4 e 16, ed in particolare la percentuale di SR ai due tempi. I pazienti che non raggiungevano tale target venivano definiti come non-super responders NSRs. Sono stati studiati 137 pazienti con età media 52,47±15,56 anni, BMI medio 27,43±5,91 e PASI medio al basale di 16,00±9,29. Alla settimana 4 il 72% dei pazienti raggiungeva PASI75, il 50% PASI90, mentre il 43% otteneva PASI100, definendo la popolazione SR alla settimana 4. Alla settimana 16, il 93% dei pazienti raggiungeva PASI75, il 77% PASI90, il 70% dei pazienti risultava essere SR mentre solo il 7% non raggiungeva PASI 75. Sono emerse alcune differenze caratterizzanti pazienti con un più rapido raggiungimento dello stato di SRs alla settimana 4 rispetto alla settimana 16: età ≥46<65, coesistenza di <2 comorbidità e stato naïve a precedenti trattamenti biologici. Considerando alcune caratteristiche clinico-demografiche è stata condotta un’analisi univariata per valutare le differenze tra SR e NSRs. Abstract

We study the impact of wall corrugations in microchannels on the process of capillary filling by means of three broadly used methods - Computational Fluid Dynamics (CFD), Lattice-Boltzmann Equations (LBE) and Molecular Dynamics (MD). The numerical results of these approaches are compared and tested against the Concus-Finn (CF) criterion, which predicts pinning of the contact line at rectangular ridges perpendicular to flow for contact angles theta > 45. While for theta = 30, theta = 40 (no flow) and theta = 60 (flow) all methods are found to produce data consistent with the CF criterion, at theta = 50 the numerical experiments provide different results. Whilst pinning of the liquid front is observed both in the LB and CFD simulations, MD simulations show that molecular fluctuations allow front propagation even above the critical value predicted by the deterministic CF criterion, thereby introducing a sensitivity to the obstacle heigth., Comment: 25 pages, 8 figures, Langmuir in press

Abstract

We report on simulations of capillary filling of high-wetting fluids in nano-channels with and without obstacles. We use atomistic (molecular dynamics) and hydrokinetic (lattice-Boltzmann) approaches which point out clear evidence of the formation of thin precursor films, moving ahead of the main capillary front. The dynamics of the precursor films is found to obey a square-root law as the main capillary front, z^2(t) ~ t, although with a larger prefactor, which we find to take the same value for the different geometries (2D-3D) under inspection. The two methods show a quantitative agreement which indicates that the formation and propagation of thin precursors can be handled at a mesoscopic/hydrokinetic level. This can be considered as a validation of the Lattice-Boltzmann (LB) method and opens the possibility of using hydrokinetic methods to explore space-time scales and complex geometries of direct experimental relevance. Then, LB approach is used to study the fluid behaviour in a nano-channel when the precursor film encounters a square obstacle. Abstract

A recent Italian study examined the immune response to the COVID-19 vaccine among psoriatic patients receiving biological therapy. The study included 346 psoriatic patients and 151 controls who received two doses of the vaccine and were tested for antibodies. The results showed that both groups had a strong antibody response, with no significant difference between patients and controls. Mild adverse events were reported by about one-fourth of participants, and no serious adverse events were reported. The study also found that the type of vaccine administered was associated with antibody levels, with higher levels observed in those vaccinated with Janssen and lower levels in those vaccinated with AstraZeneca. However, the number of subjects who received these vaccines was limited. The study suggests that COVID-19 vaccinations are effective and safe for psoriatic patients undergoing biological treatments. Abstract

Abstract

The dynamics of capillary filling in the presence of chemically coated heterogeneous boundaries is investigated, both theoretically and numerically. In particular, by mapping the equations of front motion onto the dynamics of a dissipative driven oscillator, an analytical criterion for front pinning is derived, under the condition of diluteness of the coating spots. Abstract

We present a systematic study of capillary filling for a binary fluid by using mesoscopic a lattice Boltzmann model describing a diffusive interface moving at a given contact angle with respect to the walls. We compare the numerical results at changing the ratio the typical size of the capillary, H, and the wettability of walls. Numerical results yield quantitative agreement with the Washburn law in all cases, provided the channel lenght is sufficiently larger then the interface width. We also show that in the initial stage of the filling process, transient behaviour induced by inertial effects are under control in our lattice Boltzmann equation and in good agreement with the phenomenology of capillary filling. Abstract

Numerical simulations of two-dimensional capillary filling using the pseudo-potential lattice Boltzmann model for multiphase fluids are presented, with special emphasis on the role of finite-vapour density effects. It is shown that whenever the density of the light-phase exceeds about ten percent of the dense phase, the front motion proceeds through a combined effect of capillary advection and condensation. As a result, under these conditions, the front proceeds at a higher speed as compared to the Washburn prediction. We compare the numerical results at changing the density ratio between liquid and gas phases and the ratio between the typical size of the capillary and the interface width. It is shown that numerical results yield quantitative agreement with the Washburn law when both ratios are large, i.e. as the hydrodynamic limit of a infinitely thin interface is approached. We also show that in the initial stage of the filling process, transient behaviour induced by inertial effects and ``vena contracta'' mechanisms, may induce significant departure from the Washburn law. Both effects are under control in our lattice Boltzmann equation and in good agreement with the phenomenology of capillary filling.

Abstract

Background The susceptibility of patients with chronic plaque psoriasis and the risks or benefits related to the use of biological therapies for COVID-19 are unknown. Few data about prevalence, clinical course and outcomes of COVID-19 among psoriatic patients were reported. The aims of this study were 1) to assess the prevalence and severity of COVID-19 in psoriatic patients treated with biologic agents during the first phase of the emergency (22 February to 22 April 2020) in Italy, and 2) to report the clinical outcomes of patients who have been exposed to individuals with confirmed SARS-CoV-2 infection. Methods Patients with moderate-to-severe chronic plaque psoriasis, aged ≥18 years and undergoing treatment with biologic agents as of 22 February 2020, were eligible to be included in PSO-BIO-COVID study. Demographic and clinical characteristics of patients using any biologic for psoriasis treatment between 22 February and 22 April 2020 were registered. For all confirmed or suspected cases of COVID-19, data about concomitant disease, ongoing therapies, and comorbidities were also reported. Results A total of 12,807 psoriatic patients were included in the PSO-BIO-COVID study. In this cohort twenty-six patients (0.2%) had a swab confirmation of SARS-CoV-2 infection. Eleven patients required hospitalization and two died. 125 of 12807 patients (1.0%) with exposure to a patient with COVID-19 under quarantine or active health surveillance, were reported. Conclusion The incidence of COVID-19 observed in our cohort of psoriatic patients (0.2%) is similar to that seen in the general population (0.31%) in Italy. However, the course of the disease was mild in most patients. Biological therapies may likely lessen "cytokine storm" of COVID-19, which sometimes lead to multiple organ failure, ARDS, and death.

24. How to fight SARS-COV-2 vaccine hesitancy in patients suffering from chronic and immune-mediated skin disease: four general rules

Abstract

All public health ministries have implemented strategies to contain the spread of COVID-19 worldwide. Vaccines against SARS-CoV-2 still represent the most effective weapon to combat the circulation of the virus, in order to decrease the impact of COVID-19 on the general health of the population, to prevent the emergence of new SARS-CoV-2 variants and avoid excessive hospitalization. However, the success of a vaccination campaign largely depends on the penetrance of the message addressed to general population, which takes on an even more strategic value when vaccine candidates suffer from chronic diseases. In this view, patients suffering from immune-mediated skin diseases could represent a “weak link in the vaccine chain.” Our main objective is to focus attention on four main elements in support of vaccination strategy in order to promote the patients’ awareness to be at highest risk of negative consequences in case of SARS-Cov-2 infection, and to build, strengthen and maintain trust in vaccines’ efficacy and safety. © 2021 Taylor & Francis Group, LLC.

Abstract

Saliva is easy to access, non‐invasive and a useful source of information useful for the diagnosis of serval inflammatory and immune‐mediated diseases. Following the advent of genomic technologies and ‐omic research, studies based on saliva testing have rapidly increased and human salivary proteome has been partially characterized. As a proteomic protocol to analyze the whole saliva proteome is not currently available, the most common aim of the proteomic analysis is to discriminate between physiological and pathological conditions. The salivary proteome has been initially investigated in several diseases: oral squamous cell carcinoma and oral leukoplakia, chronic graft‐versus‐host disease, and Sjögren’s syndrome. Otherwise, salivary proteomics studies in the dermatological field are still in the initial phase, thus the aim of this review is to collect the best research evidence on the role of saliva proteomics analysis in immune‐mediated skin diseases to understand the direction of research in this field. The results of PRISMA analysis reported herein suggest that human saliva analysis could provide significant data for the diagnosis and prognosis of several immune‐mediated and inflammatory skin diseases in the next future. © 2021 by the authors. 32. Combined treatment of palmar hyperhidrosis with botulinum toxin type A and oxybutynin chloride: Results of a clinical, multicenter, prospective study

Abstract

Oxybutynin chloride and botulinum toxin type A (BTX-A) have demonstrated to be effective treatments for primary palmar hyperhidrosis (PPH); however, both of them are not completely free from local and/or generalized side effects. Primary aim of this study is to compare efficacy and safety of a sequencing administration of oral oxybutynin chloride after BTX-A injections vs oral oxybutynin chloride in monotherapy in patients with PPH. Secondary aim is to evaluate if the sequencing approach can allow the control of hyperhidrosis with lower dose of oral oxybutynin. Patients enrolled were compared for short- and long-term efficacy and safety of treatments. Effectiveness was evaluated through the Hyperhidrosis Disease Severity Scale (HDSS), and the Dermatology Life Quality Index (DLQI) score; safety was assessed through collection of the adverse events reported by patients both at baseline, at 24 and 52 weeks. Patients receiving sequencing treatment showed significant greater improvement than patients receiving oxybutynin chloride alone at T24 (HDSS P =.0076 and DLQI P =.0139) and T52 (HDSS P =.0387 and DLQI P =.0087). The dose of oxybutynin chloride useful to control hyperhidrosis was lower, and retention rate to the treatment was higher in patients receiving sequencing treatment (P =.001), than patients receiving monotherapy (P =.04). A sequencing therapeutic approach to palmar hyperhidrosis increases both efficacy and safety compared with the use of oral oxybutynin chloride alone, and allows clinicians to keep lower dosage of oxybutynin chloride reducing generalized side effects and increasing the retention rate to the treatment. © 2020 Wiley Periodicals LLC.

After 6 months from discharge, 10/89 (11.24%) patients showed dermatological symptoms: 5/89 (5.61%) showed persistence of telogen effluvium, 1 of which was associated with pruritus, 1/89 (1.12%) presented guttate psoriasis, 1/89 (1.12%) presented purpuric capillaritis resolved in 15 days, 1/89 (1.12%) presented intertrigo. After 6 months from discharge, 6/30 (20.00%) patients showed dermatological symptoms: 5/30 (16.60%) showed persistence of telogen effluvium, 1 of which was associated with pruritus, 1/30 (3.33%) presented guttate psoriasis. Analysis of dermatological symptoms 1 month after hospital discharge (T1) showed that 4/30 (13.33%) patients had presented the following symptoms: 4/30 (13.33%) had telogen effluvium and 1/30 (3.33%) also had skin xerosis. Abstract

After a deep review of the main economic studies and applications about the emerging problem of obesity, this paper focuses on a particular aspect of the issue: the improvement of fruit and vegetable purchasing in retailing sector. First of all it has been analyzed the actual presence of strategies aimed at improving fruit and vegetable consumption in the main retailing companies operating in Italy. Then the study investigates the role of price in consumers’ purchasing choices. High price elasticity of F&V products could suggest the implementation of lower prices to consumers instead of implementing other policy interventions. The methodology is based on qualitative and quantitative research methods. In particular, in depth interviews to retailing sector experts have been run to gather useful insights about retailers’ management of F&V department. Then quantitative analysis on IRI data about F&V consumption in Italy has been conducted to evaluate the influence of price on consumers’ attitude towards F&V. The results obtained provide meaningful insights to formulate marketing strategies and policy interventions.
