Abstract Background Cardiac papillary fibroelastomas are rare, accounting for approximately 10% of all cardiac tumors, with 44% of cases located on the aortic valve and only 15% of cases located on the tricuspid valve. However, the optimal management of papillary fibroelastomas remains varied. Case presentation We present two successful instances of treating heart valve papillary fibroelastomas through minimally invasive surgery. These cases involved heart valve papillary fibroelastomas located in two common sites: the aortic valve on the left heart, which was accessed via an upper hemi-sternotomy, and the tricuspid valve on the right heart, which was accessed via beating heart total thoracoscopy. Conclusion The article consistently demonstrates the effectiveness of a minimally invasive surgical approach in managing heart valve papillary fibroelastomas. This study provides further evidence by presenting two cases of heart valve papillary fibroelastomas - one on the aortic valve and the other on the tricuspid valve - that were successfully treated using this approach, resulting in favorable outcomes.

Metacognitive skills consist of knowledge and regulation metacognition. This study was conducted to investigate the metacognition of undergraduate students and its correlation with students’ academic performance.Methods: The metacognitive skills inventory comprised 52 binary-scale items administered to 202 Vietnam Military Medical University medical students. The entire semester and clinical results were used to measure their academic performance.Results: Medical students’ total metacognitive awareness score was high (median 0.8). The median metacognitive knowledge score was significantly lower than the metacognitive regulation score (0.7 vs 0.8, respectively). The participants with a total metacognition score ≥ 0.8 had significantly higher academic results (full semester exam results of 7.4 and clinical exam of 7.5). The group of participants in the military, having sports habits and usually searching academic documents in English, had a higher proportion of total metacognitive awareness score ≥ 0.8 than the group without these above characteristics (with the percentages of 53.3%, 59%, and 64.3%, respectively; p < 0.05). The number of books read by participants with a total metacognitive awareness score ≥ 0.8 was significantly higher than those with a total metacognitive awareness score < 0.8 (3.5 compared to 2.4 books).Conclusion: Metacognitive awareness of Vietnam Military Medical University medical students was likely to be high. A high score of metacognitive awareness could predict high academic performance. Being a military student, playing sports, reading books, and searching English documents were predictors of better metacognitive awareness.Keywords: metacognition, medical student, metacognitive awareness, academic performance

This research aims to investigate the relationship between maternal serum cortisol and insulin resistance indices and fetal ultrasound characteristics in women with GDM.Methods: A cross-sectional and descriptive study on 144 GDM in Vietnam from January 2015 to December 2020. Serum cortisol was measured using electrochemiluminescence immunoassay at 8 a.m. on the examination day in the vicinity of the 24th gestational week. Fetal ultrasound was performed by an experienced person who was blind to the study.Results: The mean cortisol level in the GDM group was 627.04 nmol/L. Serum cortisol levels positively correlated with abdominal circumference (AC), fasting plasma glucose (FPG), insulin, triglycerides, HOMA2-IR, and Mathew indices (with r of 0.18, 0.22, 0.18, 0.17, 0.18, and 0.22, respectively). Serum cortisol levels negatively correlated with QUICKI and McAuley indices (with r of − 0.19 and − 0.21), respectively. In a univariate linear regression, maternal serum cortisol positively correlated with fetal AC, head circumference (HC), and biparietal diameter (BPD) (with r of 0.21; 0.23; and 0.25, respectively). In a multivariate linear regression analysis, cortisol positively correlated with fetal AC, HC, and BPD after adjusting to maternal McAuley index.Conclusion: Serum cortisol levels in GDM correlated with fasting blood glucose, triglycerides, and insulin resistance. Femoral artery IMT damages were one of the common peripheral artery type 2 diabetes. This study was conducted to determine the association between femoral artery IMT and plasma albumin, hs-CRP levels, and white blood cell count in newly diagnosed patients with type 2 diabetes mellitus (nT2D).Materials and Methods: From January 2015 to May 2020, 306 patients with nT2D were recruited for this cross-sectional descriptive study at Vietnam’s National Endocrinology Hospital. We measured IMT by Doppler ultrasound.Results: There was a statistically significant difference in albumin, hs-CRP levels, hs-CRP-to-albumin ratio, and white blood cell counts between three different IMT groups namely normal IMT, thick IMT, and atherosclerosis (p = 0.003, p = 0.001, p = 0.001 and p = 0.049, respectively). In the multivariate linear regression analysis, white blood cell count, and hs-CRP levels showed a significantly positive correlation to IMT (standardized B and p of 0.17, 0.015 and 0.163, 0.024, respectively), but albumin levels were a significantly negative correlation to IMT (standardized B = – 0.151, p = 0.029). The multivariate logistic regression analysis showed that albumin (OR = 0.79, 95% CI 0.65– 0.90, p = 0.018), hs-CRP (OR = 1.09, 95% CI 1.01– 1.18, p = 0.026), and white blood cell count (OR = 1.36, 95% CI 1.03– 1.81, p = 0.033) had correlation to atherosclerosis of femoral artery.Conclusion: Reduced plasma albumin, elevated hs-CRP, and white blood cell count associated with IMT increased the odds for atherosclerosis of femoral artery among nT2D.Keywords: plasma albumin, high-sensitivity C-reactive protein, white blood cell count, newly diagnosed type 2 diabetes mellitus, intima-media thickness, atherosclerotic plaque, femoral artery

Screening for DPN was based on the United Kingdom screening test. fGLP-1 was measured by enzyme-linked immunosorbent assay.Results: The prevalence of DPN was 26.6%, in which mild grade was 17.3%, moderate grade was 8.2% and severe grade was 1.1% in total. Age (OR = 1.73, 95% CI 1.12– 2.67, p = 0.012), smoking (OR = 1.64, 95% CI 1.03– 2.62, p = 0.037), poor control HbA1c (OR = 2.66, 95% CI 1.23– 5.76, p = 0.01), 24-h urinary albumin (24hUA) (OR = 2.49, 95% CI 1.26– 4.94, p = 0.007), and diabetic retinopathy (OR = 3.17, 95% CI 1.46– 6.89, p = 0.002) significantly increased the risk for DPN. In multivariate logistic regression analysis, hypertension (OR = 2.96, 95% CI 1.16– 7.55, p = 0.023), triglyceride (OR = 1.50, 95% CI 1.11– 2.03, p = 0.009), albumin (OR = 0.85, 95% CI 0.75– 0.95, p = 0.005), and fGLP-1 (OR = 0.79, 95% CI 0.67– 0.93, p = 0.005) correlated with DPN. The fGLP-1 concentrations were reduced significantly in DPN (p < 0.001). In particular, male patients with DPN had a significantly lower fGLP-1 levels than those without DPN (p < 0.001).Conclusion: The prevalence of DPN among nT2D was 26.6%. Abstract Background Minimally invasive mitral valve replacement has become popular across the world. However, annular rupture and patient – prosthetic mismatch (PPM) are still problematic, particularly in the Asian population. To avoid this, a predictor model could be beneficial. Our study aimed to assess the value of mitral valve diameters measured on TTE and CT scan on predicting the actual mitral prostheses. Methods From January 2018 to December 2019, a total number of 96 patients underwent minimally invasive mitral valve replacement. The association between imaging measurements and the outcome was checked by scatter plot and Pearson’s correlation coefficient. Univariable linear regression was used to build the prediction model. Results The three strongest correlations for the whole population are the following features: Mean TTE diameter (0.702), mean diameter on CT lowest plane through the mitral annulus (0.679), and area-derived diameter on CT highest plane through the mitral annulus (0.665). The prosthetic size of the tissue valve group was more correlated to the calculated annulus diameters than that of the mechanical valve group. Tissue valve size predictor models based on these calculated diameters were 16.19 + 0.27 × d (r = 0.744), 12.74 + 0.44 × d (r = 0.756) and 12.79 + 0.38 × d (r = 0.730), respectively. Conclusion Mitral prosthetic size could be predicted based on the mitral diameters measured on TTE and CT scan. The overall correlation coefficient varied from 0.665 (CT Scan) to 0.702 (TTE). These models performed better when applied to bioprosthesis.

We find a bound for the Goldie dimension of hereditary modules in terms of the cardinality of the generator sets of its quasi-injective hull. Several consequences are deduced. In particular, it is shown that every right hereditary module with countably generated quasi-injective hull is noetherian. Neuroimaging using the 7-Tesla (7T) human magnetic resonance (MR) system is rapidly gaining popularity after being approved for clinical use in the European Union and the USA. This trend is the same for functional MR imaging (MRI). The primary advantages of 7T over lower magnetic fields are its higher signal-to-noise and contrast-to-noise ratios, which provide high-resolution acquisitions and better contrast, making it easier to detect lesions and structural changes in brain disorders. Another advantage is the capability to measure a greater number of neurochemicals by virtue of the increased spectral resolution. Many structural and functional studies using 7T have been conducted to visualize details in the white matter and layers of the cortex and hippocampus, the subnucleus or regions of the putamen, the globus pallidus, thalamus and substantia nigra, and in small structures, such as the subthalamic nucleus, habenula, perforating arteries, and the perivascular space, that are difficult to observe at lower magnetic field strengths. The target disorders for 7T neuroimaging range from tumoral diseases to vascular, neurodegenerative, and psychiatric disorders, including Alzheimer’s disease, Parkinson’s disease, multiple sclerosis, epilepsy, major depressive disorder, and schizophrenia. MR spectroscopy has also been used for research because of its increased chemical shift that separates overlapping peaks and resolves neurochemicals more effectively at 7T than a lower magnetic field. This paper presents a narrative review of these topics and an illustrative presentation of images obtained at 7T. We expect 7T neuroimaging to provide a new imaging biomarker of various brain disorders.

23. Understanding growth and physiological responses to slash management, thinning and fertiliser application in short-rotation tropical acacia plantations

In Vietnam acacia plantations are an important resource for the production of pulpwood and sawn timber. However, research is needed to support sustainable and profitable production over successive rotations. This thesis reports on results from three field experiments which were conducted on Acacia auriculiformis and Acacia hybrid species in Vietnam. The experiments explored the growth and physiological responses of acacia to slash management, thinning and fertiliser application to test the hypothesis that constraints in nutrients and/or water and light resources were constraining the growth of acacias, both at establishment, and later in the rotation. The rationale for focusing on resource constraints is that they offer some opportunity for management intervention. The first study explored the impacts of contrasting slash and litter management techniques applied at the start of the second rotation and re-applied at the start of the third rotation with an additional phosphorus fertiliser treatment during the inter-rotation phase of A. auriculiformis plantations. Removal of slash and litter after harvesting the first rotation removed `20.2 Mg ha^-1` biomass, containing `169.6, 13.9, 76.3, and 25.1 kg ha^-1 of N, P, K, and Ca,` respectively, from the site. Greater amounts were removed after the second rotation commensurate with higher productivity and amounts of biomass produced. Growth of trees in the second rotation was significantly higher where slash and litter were retained compared to where they were removed. Soil organic carbon and nitrogen contents were greater (26% and 40% respectively) in treatments with slash and litter retained compared to initial levels before the treatments were applied. In the second rotation, there was no growth response to P fertiliser but extractable soil P declined during this period. In the third rotation there was a positive response to added P fertiliser. Slash and litter retention along with improvements in the genetics of planting stock, weed control and stocking with each rotation resulted in average growth rates increasing from `10.6 m^3 ha^-1 yr^-1` in the first rotation (age 7 yr) to `28.3 m^3 ha^-1 yr^-1` in the second rotation (age 6 yr) and to `33.9 m^3 ha^-1 yr^-1` at age 5 yr in the third rotation. Thus, the results indicated that there is an opportunity to increase and sustain production of A. auriculiformis over at least three rotations by integrated management practices promoting better stocking, planting of genetically improved stock, organic matter and nutrient conservation, P fertiliser addition and judicious weed management. The second experiment investigated how thinning, P fertiliser application and slash retention at age 4 yr interact to affect the physiology and growth of A. auriculiformis trees to age 7 yr. The photosynthetic rate (`A_(max`) in the thinned treatment was significantly higher than in the unthinned treatment one year after thinning. A combined treatment of thinning and phosphorus fertiliser (P) application increased `A_(max)`, but `A_(max)` in the unthinned treatment did not significantly increase when P was supplied. Foliar nitrogen and phosphorus concentrations were greater in thinned than in unthinned treatments. Tree diameter was significantly greater under thinning, but it was not influenced by the application of fertiliser (`50 kg P ha^-1`) and slash and litter retention. The recovery of larger diameter, sawlog sized, timber in thinned treatments was significantly higher than in unthinned treatments. At seven years, the total stand value of wood products in the thinned treatments (including the thinning harvested in year 4) was higher than (5 %) for the unthinned treatments. This experiment supports the practice of mid-rotation thinning of A. auriculiformis in these environments to increase the value of these plantations to acacia growers. Growth and physiological responses of the Acacia hybrid plantation to thinning treatments of different intensity at age two and three years were also examined in a field experiment. Three years, after intensive thinning at age two, stand volume was reduced by 15.8 % but average stem diameter was increased by 16.7 %. The moderate thinning regime resulted in no significant loss in stand volume and an increase in average diameter of 7.5 %. After thinning the LAI of the intensively thinned stand recovered rapidly and there was no significant difference between unthinned and thinned treatments one year after thinning. This was associated with decreased litterfall production. Intensive thinning increased photosynthetic rates of the lower crown by 30.4 % in association with increased phosphorus concentration in leaves of 37.5 %. Tree growth was significantly influenced by season and thinning reduced leaf water stress during the dry season. Thinning of Acacia hybrid at age 2 or 3 yr resulted in higher leaf-level photosynthesis, enhanced water relations, and improved foliar phosphorus relative to unthinned trees. This suggests that intensive thinning at age two years or moderate thinning at age three years are practices that are likely to confer greater benefit to acacia growers. The investment decision should also account for the market value of different log sizes, the costs associated with harvest of these logs, and the risks associated with managing plantations for sawlog production, including longer rotations. In conclusion, the productivity of commercial acacia plantations can be maintained and improved by adopting integrated management practices, especially by retaining slash and litter after harvesting to promote soil conservation during the inter-rotation phase. Applying thinning for short-rotation tropical acacia plantations can reduce intraspecific competition in stands for water and light resources and increase the benefits for acacia growers.

OBJECTIVES: Despite extensive clinical use, limited data are available on optimal loading and maintenance doses of vancomycin in critically ill patients. This study aimed to develop a rational approach for optimised dosage of vancomycin given in a continuous infusion in critically ill patients. METHODS: Vancomycin pharmacokinetic (PK) data (total serum concentrations) were obtained from 55 intensive care unit (ICU) patients (Bach Mai Hospital, Hanoi, Vietnam) receiving a 20 mg/kg loading dose followed by continuous infusion stratified by creatinine clearance (CLCr). Population PK modelling and Monte Carlo simulations were performed using a nonlinear mixed-effects modelling (NONMEM) program for a target of 20-30 mg/L to optimise efficacy and minimise nephrotoxicity. RESULTS: A two-compartment model with first-order elimination best fitted the PK data with central and peripheral volumes of distribution of 1.01 and 2.39 L/kg, respectively (allometric scaling to a 70 kg standard subject). The population total clearance of 3.63 L/h was only explained by renal function in the covariate and final model. The simulations showed that a 25-mg/kg loading dose infused over 90 minutes was optimal to reach the target range. The optimal maintenance dose for low renal function (CLCr < 45 mL/min) was 1000-1500 mg/day. For augmented renal clearance (CLCr > 130 mL/min) the dose should be up to 3500 mg/day or even 4500 mg/day to achieve adequate exposure. These simulated maintenance doses were larger than previously proposed for non-ICU patients. CONCLUSION: Large loading and maintenance doses of vancomycin are generally needed in critically ill patients. Because of high interindividual variability in vancomycin PK, drug monitoring may still be necessary.

- Abstract
To study the heterogeneity of complex tissues by joint profiling of gene expression and its regulation, we require an accurate and high-throughput method. Here we described improved high-throughput combinatorial indexing-based single-nucleus chromatin accessibility and mRNA expression sequencing 2 (SNARE-Seq2) co-assay. This protocol involves fixing and permeabilizing the nucleus followed by tagmentation, chromatin barcode ligation, reverse transcription, pooling and splitting for the next rounds of cell barcode ligation into cDNA and accessible chromatin (AC) on the same nucleus. The captured cDNA and AC are co-amplified before splitting and enrichment into single-nucleus RNA and single-nucleus AC sequencing libraries. The protocol can also be applied to both nuclei and whole cells to capture mRNA in the cytoplasm. This improvement allows us to generate hundreds of thousands of data set of each assay and can be scaled up to half a million cells from a single experiment. The entire procedure can be complete in 3.5 d for generating joint single-nucleus RNA and single-nucleus ATAC sequencing libraries.

After introduction of the first human 7 tesla (7T) system in 1999, 7T MR systems have been employed as one of the most advanced platforms for human MR research for more than 20 years. Currently, two 7T MR models are approved for clinical use in the U.S.A. The approval facilitated introduction of the 7T system, summing up to around 100 worldwide. The approval in Japan is much awaited. As a clinical MR scanner, the 7T MR system is drawing attention in terms of safety. Several large-sized studies on bioeffects have been reported for vertigo, dizziness, motion disturbances, nausea, and others. Such effects might also be found in MR workers and researchers. Frequency and severity of reported bioeffects will be presented and discussed, including their variances. The high resonance frequency and shorter RF wavelength of 7T increase the concern about the safety. Homogeneous RF pulse excitation is difficult even for the brain, and a multi-channel parallel transmit (pTx) system is considered mandatory. However, pTx may create a hot spot, which makes the estimation of specific absorption rate (SAR) to be difficult. The stronger magnetic field of 7T causes a large force of displacement and heating on metallic implants or devices, and the scan of patients with them should not be conducted at 7T. However, there are some opinions that such patients might be scanned even at 7T, if certain criteria aremet. This article provides a brief review on the effect of the static magnetic field on humans (MR subjects, workers, and researchers) and neurons, in addition to scan sound, SAR, and metal implants and devices. Understanding and avoiding adverse effects will contribute to the reduction in safety risks and the prevention of incidents. [ABSTRACT FROM AUTHOR]

Publisher's version (útgefin grein), Sex differences have been observed in multiple facets of cancer epidemiology, treatment and biology, and in most cancers outside the sex organs. Efforts to link these clinical differences to specific molecular features have focused on somatic mutations within the coding regions of the genome. Here we report a pan-cancer analysis of sex differences in whole genomes of 1983 tumours of 28 subtypes as part of the ICGC/TCGA Pan-Cancer Analysis of Whole Genomes (PCAWG) Consortium. We both confirm the results of exome studies, and also uncover previously undescribed sex differences. These include sex-biases in coding and non-coding cancer drivers, mutation prevalence and strikingly, in mutational signatures related to underlying mutational processes. These results underline the pervasiveness of molecular sex differences and strengthen the call for increased consideration of sex in molecular cancer research., We thank all the members of the Boutros lab for insightful discussions. - Abstract
This document provides an overview of the protocols used by the Human Kidney, Urinary Tract, and Lung Mapping Center (KULMAP) for generation of spatially resolved mulitomic cell type organ maps within HuBMAP. This involves sequencing of the transcriptomes and epigenomes of dissociated single cells in a massively parallel manner, which will then inform on a highly multiplexed RNA in situ imaging method (DART-FISH) for spatial mapping of hundreds of molecular targets in the tissue sections, at a subcellular resolution. These spatial molecular maps will serve as scaffolds for computational registration of cell types and the associated transcriptome/chromatin maps to the tissue space.

- Abstract
Nuclei can be readily isolated from frozen tissues with a combination of chemical and physical treatments that can circumvent the non-uniform or incomplete dissociation of solid tissues into single cells. The isolation of nuclei can also circumvent RNA degradation or any introduction of technical artefacts (such as stress responses) that could be triggered during whole cell dissociation methods. Data generated from single-nucleus genomic assays permits discovery of molecular cell types that can be used to define the overall cellular makeup of a tissue or organ, and ultimately will inform upon adult human tissue atlases.

In aquatic environments, assessment of microplastic concentrations is increasing worldwide but environments from developing countries remain under-evaluated. Due to disparities of facilities, financial resources and human resources between countries, protocols of sampling, analysis and observations used in developed countries cannot be fully adapted in developing ones, and required specific adaptations. In Viet Nam, an adapted methodology was developed and commonly adopted by local researchers to implement a microplastic monitoring in sediments and surface waters of 21 environments (rivers, lakes, bays, beaches) of eight cities or provinces. Microplastic concentrations in surface waters varied from 0.35 to 2522 items m-3, with the lowest concentrations recorded in the bays and the highest in the rivers. Fibers dominated over fragments in most environments (from 47% to 97%). The microplastic concentrations were related to the anthropogenic pressure on the environment, pointing out the necessity in a near future to identify the local sources of microplastics.

N and on the unit sphere in ℝN+1 . It is shown that sequences of unisolvent sets can be combined to get other sequences of unisolvent sets such that the existence of the limits is preserved. - Abstract
Wireless edge caching for mobile social networks (MSNs) has emerged as one of the prospective solutions to provide reliable and low-latency communication services for mobile users on social networking. In this chapter, we first give an overview of MSNs, including their development and challenges. We then discuss mobile edge caching (MEC) paradigms to address emerging issues for MSNs, e.g., service delay, users’ experience, and economic efficiency. In addition to the advantages, the development of MEC networks also places some key challenges’ such as hierarchical architecture of MEC networks, proactive caching, privacy, and security issues. The framework can authenticate MSN users based on public-key cryptography and predict their content demands utilizing a matrix factorization method. Based on the prediction, an optimal content caching policy for an MEC node is presented to minimize the average latency of all MSN users under the MEC nodes’ storage capacity constraints. Furthermore, this framework provides an optimal business model to maximize the revenue for MSN service providers based on the demands of the MSN users and the obtained optimal caching policy.

Genetic alterations in signaling pathways that control cell-cycle progression, apoptosis, and cell growth are common hallmarks of cancer, but the extent, mechanisms, and co-occurrence of alterations in these pathways differ between individual tumors and tumor types. Using mutations, copy-number changes, mRNA expression, gene fusions and DNA methylation in 9,125 tumors profiled by The Cancer Genome Atlas (TCGA), we analyzed the mechanisms and patterns of somatic alterations in ten canonical pathways: cell cycle, Hippo, Myc, Notch, Nrf2, PI-3-Kinase/Akt, RTK-RAS, TGFβ signaling, p53 and β-catenin/Wnt. We charted the detailed landscape of pathway alterations in 33 cancer types, stratified into 64 subtypes, and identified patterns of co-occurrence and mutual exclusivity. Eighty-nine percent of tumors had at least one driver alteration in these pathways, and 57% percent of tumors had at least one alteration potentially targetable by currently available drugs. Thirty percent of tumors had multiple targetable alterations, indicating opportunities for combination therapy. An integrated analysis of genetic alterations in 10 signaling pathways in >9,000 tumors profiled by TCGA highlights significant representation of individual and co-occurring actionable alterations in these pathways, suggesting opportunities for targeted and combination therapies.
