CBNs can exhibit distinguished properties such as high mechanical strength, chemical stability, high electrical conductivity, and biocompatibility. These desirable physicochemical properties have triggered their uses in many fields, including biomedical applications. In this review, we specifically focus on applying CBNs as scaffolds in tissue engineering, a therapeutic approach whereby CBNs can act for the regeneration or replacement of damaged tissue. Here, an overview of the structures and properties of different CBNs will first be provided. We will then discuss state-of-the-art advancements of CBNs and hydrogels as scaffolds for regenerating various types of human tissues. Finally, a perspective of future potentials and challenges in this field will be presented. Since this is a very rapidly growing field, we expect that this review will promote interdisciplinary efforts in developing effective tissue regeneration scaffolds for clinical applications.Keywords: carbon, biomaterial, nanotechnology, tissue engineering, scaffold

2. WHICH CLINICAL PARAMETERS HAVE IMPACT ON OUTCOME IN THE EARLY PHASE OF COMPLICATED INTRA-ABDOMINAL INFECTION? - A PROSPECTIVE STUDY.

Abstract

Purpose: There is still no comprehensive analysis with Bulgarian patients investigating the association between perioperative clinical parameters and final outcome in the early phase of complicated intraabdominal infection. Methods: This single-center prospective study was conducted in the Clinic of Surgical Diseases at the University Hospital Stara-Zagora for the period November 2018 - August 2021. Before surgery and on the 3rd postoperative day (POD) we measured axillary temperature (t), systolic blood pressure (SBP), mean arterial pressure (MAP), respiratory rate (RR), heart rate (HR), mental status and systemic inflammatory response syndrome (SIRS) in 62 patients with complicated intra-abdominal infections (cIAIs). Results: Of the 62 patients, nine died (14.5%). Preoperatively, only MAP successfully discriminated survivors from non-survivors (p = 0.027), which was confirmed by the ROC Curve analysis (AUROC = 0.731). Postoperatively, almost all of the clinical parameters except axillary temperature (AUROC = 0.573) showed prognostic ability - SBP (AUROC = 0.779) and MAP (AUROC = 0.864) for prediction of favorable outcome, HR (AUROC = 0.916) and RR (AUROC = 0.935) for prediction of lethal outcome. Conclusion: All investigated clinical parameters, except for axillary temperature, demonstrated the ability to predict the final outcome on the 3rd POD in patients with cIAIs. [ABSTRACT FROM AUTHOR]

Abstract

Background Immunocompromised patients with acute diverticulitis are at increased risk of morbidity and mortality. The aim of this study was to compare clinical presentations, types of treatment, and outcomes between immunocompromised and immunocompetent patients with acute diverticulitis. Methods We compared the data of patients with acute diverticulitis extracted from the Web-based International Registry of Emergency Surgery and Trauma (WIRES-T) from January 2018 to December 2021. First, two groups were identified: medical therapy (A) and surgical therapy (B). Each group was divided into three subgroups: nonimmunocompromised (grade 0), mildly to moderately (grade 1), and severely immunocompromised (grade 2). Results Data from 482 patients were analyzed—229 patients (47.5%) [M:F = 1:1; median age: 60 (24–95) years] in group A and 253 patients (52.5%) [M:F = 1:1; median age: 71 (26–94) years] in group B. There was a significant difference between the two groups in grade distribution: 69.9% versus 38.3% for grade 0, 26.6% versus 51% for grade 1, and 3.5% versus 10.7% for grade 2 (p p = 0.027) was more common in higher grades of immunodeficiency. Patients with grade 2 needed longer hospitalization (p = 0.005). In group B, a similar condition was found in terms of severe sepsis (p = 0.002), quick Sequential Organ Failure Assessment score > 2 (p = 0.0002), and Mannheim Peritonitis Index (p = 0.010). A Hartmann’s procedure is mainly performed in grades 1–2 (p p = 0.047). Mortality was higher in the immunocompromised patients (p = 0.002). Conclusions Immunocompromised patients with acute diverticulitis present with a more severe clinical picture. When surgery is required, immunocompromised patients mainly undergo a Hartmann’s procedure. Postoperative morbidity and mortality are, however, higher in immunocompromised patients, who also require a longer hospital stay.

Abstract

Association of advanced age, neoplastic disease and immunocompromission (IC) may lead to surgical emergencies. Few data exist about this topic. Present study reports the preliminary data from the WIRES-T trial about patients managed for colorectal neoplastic emergencies in immunocompromised patients. The required data were taken from a prospective observational international register. The study was approved by the Ethical Committee with approval n. 17575; ClinicalTrials.gov Identifier: NCT03643718. 839 patients were collected; 753 (80.7%) with mild–moderate IC and 86 (10.3%) with severe. Median age was 71.9 years and 73 years, respectively, in the two groups. The causes of mild–moderate IC were reported such malignancy (753–100%), diabetes (103–13.7%), malnutrition (26–3.5%) and uremia (1–0.1%), while severe IC causes were steroids treatment (14–16.3%); neutropenia (7–8.1%), malignancy on chemotherapy (71–82.6%). Preoperative risk classification were reported as follow: mild–moderate: ASA 1–14 (1.9%); ASA 2–202 (26.8%); ASA 3–341 (45.3%); ASA 4–84 (11.2%); ASA 5–7 (0.9%); severe group: ASA 1-1 patient (1.2%); ASA 2–16 patients (18.6%); ASA 3–41 patients (47.7%); ASA 4–19 patients (22.1%); ASA 5–3 patients (3.5%); lastly, ASA score was unavailable for 105 cases (13.9%) in mild–moderate group and in 6 cases (6.9%) in severe group. All the patients enrolled underwent urgent/emergency surgery Damage control approach with open abdomen was adopted in 18 patients. Mortality was 5.1% and 12.8%, respectively, in mild–moderate and severe groups. Long-term survival data: in mild–moderate disease-free survival (median, IQR) is 28 (10–91) and in severe IC, it is 21 (10–94). Overall survival (median, IQR) is 44 (18–99) and 26 (20–90) in mild–moderate and severe, respectively; the same is for post-progression survival (median, IQR) 29 (16–81) and 28, respectively. Univariate and multivariate analyses showed as the only factor influencing mortality in mild–moderate and severe IC is the ASA score. Colorectal neoplastic emergencies in immunocompromised patients are more frequent in elderly. Sigmoid and right colon are the most involved. Emergency surgery is at higher risk of complication and mortality; however, management in dedicated emergency surgery units is necessary to reduce disease burden and to optimize results by combining oncological and acute care principles. This approach may improve outcomes to obtain clinical advantages for patients like those observed in elective scenario. Lastly, damage control approach seems feasible and safe in selected patients.

5. Irrigation rate and fertilization as factors determining yield in tomatoes - mid-early outdoor production.

Abstract

In the period 2018-2020, an experiment was carried out with tomato variety "Nikolina F1" on leached cinnamon forest soil. The influence of increasing irrigation and fertilizer rates delivered by fertigation on tomato yields was studied. The experiment contained 8 variants at two irrigation rates (1-4 variants - 100% MMC; 5-8 variants - 50% MMC) and increasing levels of fertigation (T1 and T5 - without fertilization; T2 and T6 - N15P8K15; T3 and T7 - N20P12K20; T4 and T8 - N25P16K25). The results show that the formed yields of tomatoes during the studied years are the result of the influence of climatic factors, applied fertilization and irrigation rates. The average yields of tomatoes from the three experimental years range between 2864.0 and 5601.3 kg.da-1 with an average yield of 4363.96 kg.da-1. With average yields for the three experimental years of 2145.3 kg.da-1 for 2018; 5513.63 kg.da-1 for 2019 and 5430.5 kg.da-1 for 2020, as the plants of variant T3 fertilizes with N20P12K20 and full irrigation rate - 100% MMC realized the highest yields, in 2018 with 23.1 %, in 2019 with 37.2% and in 2020 with 21.6% higher than the average yields for the respective years. The three-factor statistical analysis of the average yield data shows that the influence of the year is the strongest - 67.77% of the formed yields are dependent on the experimental year. The degree of influence of the applied fertigation is 19.5%, of the irrigation rate - 1.3%, of fertigation and the year is 7.48%, and the joint influence of fertilization and the irrigation rate is less than 1%. Abstract

This paper presents quantitative comparison of the effect of modified contemporary diesel blends under effective parameters of diesel engine. Comparative tests were made under bench conditions. The values of the power, economy and toxic performance of a diesel engine operating with standard fuel and modified diesel blends have been determined. It is concluded that the operation of a Volkswagen 1.9 diesel engine with a modified contemporary diesel blend results in a reduction in the exhaust gas temperature without altering its economic and environmental performance. Abstract

The use of the technology for color laser marking on chromium-nickel basisin industrial productions is studied from the point of view of the repeatability and stability ofthe produced color markings. The research and research was done during the Erasmuspractice by the authors at the Rēzekne Academy of Technology, Latvia. For this purpose, acolor palette AISI 304 was developed, consisting of fifteen colors. The dependence of theobtained colors on the different parameters of the laser processing is analyzed. The obtainedcolors were then tested using optical, scanning electron and atomic force microscopy, and theconfiguration of the oxide films was determined by Raman spectroscopy. The obtained colorshave a suitable uniformity, brightness and cover almost all spectral areas, and the obtainedcolors have many times better quality than other metal bases. A test for color standardizationand palette repeatability was also performed by estimating the reflection spectra of theformed colors. The color palette demonstrates high repeatability for all colors except onespecific color. In addition, the stability of color markings with respect to environmental,mechanical and chemical resistance was studied. The obtained colors show high resistance tomost environmental conditions; however, exposure to very high temperatures and extremehumidity (100 ° C, 90%) and to low temperatures and extreme humidity (-40 ° C, 90%) resultsin a deterioration in the quality of several colors. The color marks show high hardness andexcellent mechanical resistance to external influences and exceptional resistance to variouschemicals, with the exception of acid solutions and salts.

Abstract

12. Quality of tomato production depending on the applied irrigation rate and fertigation.

Abstract

During the period 2018-2020, on leached Cinnamon forest soil at outdoor irrigation conditions, studies were conducted with determinant tomato variety Nikolina F1 -- medium early production. The influence of 50 and 100% irrigation rates and increasing fertilization by fertigation (N15P8K15; N20P12K20 и N25P16K25) on the quality of the formed production has been established. The results of the three-year study show higher quality indicators in tomatoes with reduced irrigation compared to 100% irrigation rate - 6.0% higher dry matter content, 12.3% more total sugars, 16.99% higher sugar-acid number, as well as an acid content reduced by 8.3%. In terms of dry matter, total sugars and sugar-acid number, the variant fertilized with N20P12K20 is optimal for both irrigation rates. In both irrigation rates, there is a trend for the acid content in the fruits to increase with the fertilizer rate, while in the case of ascorbic acid, there are no pronounced trends over the years. Abstract

Background: Colorectal cancer (CRC) is one of the most common malignancies in developed countries. Research for diagnostic and prognostic biomarkers is increasing. There are more than 750,000 papers indexed in PubMed directly related to biomarkers. A lot of these papers claim to report clinically useful biomarkers. Unfortunately, very few serum biomarkers are in clinical use. Materials and methods: We performed a review for most of the published CRC serum biomarkers in the PubMed, Embase and Web of Science databases. Results: In total, 119 articles were obtained from the databases, of which 89 articles reporting on CRC serum prognostic markers were considered as relevant. In these studies, a total of 200 individual tumor markers were obtained. Conclusions: A general approach to understand how to predict clinical outcomes using risk, diagnostic, and prognostic biomarkers, is needed. In the future, serum biomarkers will drive advances in risk, diagnosis, and prognosis, they will be the targets of powerful molecular therapies, and they will individualize and optimize therapy.

Abstract

Cardiopoietic cells, produced through cardiogenic conditioning of patients' mesenchymal stem cells, have shown preliminary efficacy. The Congestive Heart Failure Cardiopoietic Regenerative Therapy (CHART-1) trial aimed to validate cardiopoiesis-based biotherapy in a larger heart failure cohort. This multinational, randomized, double-blind, sham-controlled study was conducted in 39 hospitals. Patients with symptomatic ischaemic heart failure on guideline-directed therapy (n = 484) were screened; n = 348 underwent bone marrow harvest and mesenchymal stem cell expansion. Those achieving > 24 million mesenchymal stem cells (n = 315) were randomized to cardiopoietic cells delivered endomyocardially with a retention-enhanced catheter (n = 157) or sham procedure (n = 158). Procedures were performed as randomized in 271 patients (n = 120 cardiopoietic cells, n = 151 sham). The primary efficacy endpoint was a Finkelstein–Schoenfeld hierarchical composite (all-cause mortality, worsening heart failure, Minnesota Living with Heart Failure Questionnaire score, 6-min walk distance, left ventricular end-systolic volume, and ejection fraction) at 39 weeks. The primary outcome was neutral (Mann–Whitney estimator 0.54, 95% confidence interval [CI] 0.47–0.61 [value > 0.5 favours cell treatment], P = 0.27). Exploratory analyses suggested a benefit of cell treatment on the primary composite in patients with baseline left ventricular end-diastolic volume 200–370 mL (60% of patients) (Mann–Whitney estimator 0.61, 95% CI 0.52–0.70, P = 0.015). No difference was observed in serious adverse events. One (0.9%) cardiopoietic cell patient and 9 (5.4%) sham patients experienced aborted or sudden cardiac death. The primary endpoint was neutral, with safety demonstrated across the cohort. Further evaluation of cardiopoietic cell therapy in patients with elevated end-diastolic volume is warranted.

Abstract

Pollution intensities measure total emissions per dollar of output for the production sectors of the economy. This paper presents quantitative assessment of the effect of modified gasoil under effective parameters of diesel engine. Comparative tests were made under bench conditions. The values of the power, economy and toxic performance of a diesel engine operating with standard fuel and modified gas oil have been determined. It is concluded that the operation of a Volkswagen 1.9 diesel engine with a modified gas oil results in a reduction in the exhaust gas temperature without altering its economic and environmental performance. Abstract

Current transformers (CTs) are precision devices that scale high currents down to values that can be easily handled by measurement equipment. To support CT applications in revenue metering, a comparison on AC current transformer calibration systems was performed among 15 European national metrology institutes using a precision CT as the travelling device. The first comparison results for the transformation ratios (4, 5, 6, 8, 10) kA/5 A of the travelling CT at nominal burden of 5 VA and 15 VA indicate good agreement between the participating laboratories. The main differences are found for phase displacement, at least partly caused by the instability of the traveling standard.
