Etik prensipler temelde “İnsan Hakları ” ilkeleri altında yapılandırılırlar ve etik felsefesi görüşüaltında, Tıp Bilimi açısından “doğru nedir” ve “ne yapmalıyım” sorularına özgün koşullarda veyaolgu/hastaya durumsal olarak cevap bulmaya çalışmaktır. “Hastalığı değil, hastayı tedavi et ”prensibi empati/duygudaşlık yaklaşımı hedeflenmelidir.

Objective: While routine administration of rhesus (Rh) immunoglobulin has significantly reduced the incidence of Rh alloimmunization, maternal alloimmunization to other red cell antigens remains a contributor to perinatal morbidity and mortality. Although the Kell antigen is seen on the red cells of only 9% of the general population, attention to Kell antibodies continues to increase. Case Report: A case of fetal hydrops was sonographically detected at 30 weeks of gestation. Antenatal tests to evaluate the fetus’s condition clearly showed that the level of hemolytic disease was critical and the baby was delivered prematurely due to fetal distress. The combination of anemia, reticulocytopenia, hydrops fetalis, and a positive indirect Coombs test suggested Kell isoimmunization. The baby was successfully treated with exchange transfusion of Kellnegative packed red cells, and was discharged on postnatal d 30. Conclusion: The presented case of hydrops fetalis was due Kell alloimmunization that was detected during the postnatal period, and thus we plan to discuss the perinatal approach to Kell immunization.

The aim of this study is to examine the effect of the medical home model (MHM) on the developmental status of very low birth weight (VLBW) infants at 12- 18 months corrected age in a tertiary health center in Turkey. Material- Method: A total of 199 infants who were born with birth weight ≤ 1500 grams and/or gestational week ≤ 32 weeks at Zekai Tahir Burak (ZTB) Maternity Teaching Hospital Ankara and received medical and developmental follow-up at ZTB hospital within the medical home model were enrolled. Their developmental status was evaluated at corrected 12- 18 months with Bayley II. Results: Of the study group, 49% was female and 51% was male. Mean birth weight was 1238 ± 319 grams and 25,1% (n=50) was ≤ 1000 grams. Mean gestational age was 29, 2±2,2 weeks and 38,2% was ≤ 28 weeks. About 50% was multiplets. Bayley Mental Development Index scores (MDI) and Psychomotor Development Index scores (PDI) were < 70 in 7% and 19,6% of the total group, respectively and 7% was diagnosed as cerebral palsy (CP). Bayley MDI and PDI scores were < 70 in 12% and 32% of the infants born ≤ 1000 grams and 12% had CP. Conclusion: The low rate of babies with unfavorable developmental status in the study group suggests that MHM may help the early recognition and intervention of major developmental problems in high risk babies and decreases later problems. Long term studies are needed to determine the effect of MHM on minor developmental problems of the VLBW infants. Yüksek riskli bu bebeklerin izleminin tüm koruyucu ve tedavi edici hizmetlerin aynı ekip tarafından, aynı “çatı altında” verilmesini sağlayan “Bütüncül İzlem modeli (BİM)” ile yapılması önerilmektedir. Amaç: Bu çalışmada 1 yıllık dönemde T.C.Sağlık Bakanlığı Dr. Zekai Tahir Burak Kadın Sağlığı Eğitim ve Araştırma Hastanesinde doğan, yenidoğan kliniğinde yatan ve taburculuk sonrasında aynı merkezin Gelişimsel Pediatri ünitesinde izlenen çok düşük doğum ağırlıklı bebeklerin düzeltilmiş 12-18 aylarındaki gelişimsel sonuçlarının belirlenmesi amaçlandı. Materyal - Metod: Doğum ağırlığı ≤ 1500 gram ve/veya gestasyonel haftası ≤ 32 hafta olan, taburcu edilen ve izlemleri tamamlanmış olan toplam 199 bebek çalışmaya alındı. Bebekler taburculuk sonrasında BİM kapsamında, belirli aralıklarla aynı gelişimsel pediatri ünitesinde bir gelişimsel pediatrist ve bir çocuk gelişimcisi tarafından değerlendirildi ve uygun gelişimsel destekler verildi. Düzeltilmiş 12-18 aylarında bebeklerin nörolojik muayeneleri ve “Bayley Bebekler için Gelişimsel Değerlendirme Ölçeği II” ile gelişimsel değerlendirmeleri yapıldı. Bulgular: Çalışma grubunda yer alan 199 bebeğin %49'u kız ve %51'i erkekti. Bebeklerin ortalama doğum ağırlığı 1238 ±319 gram ve %25,1'inin (n=50) doğum ağırlığı ≤ 1000 gram idi. Bebeklerin ortalama doğum haftası 29,2±2,2 hafta ve %38,2'sinin ≤ 28 hafta idi. Bebeklerin yaklaşık %50'si çoğul gebelikti. Yapılan gelişimsel değerlendirmede bebeklerin %7'sinin bilişsel ölçek puanı < 70, %19,6'sının hareket ölçek puanı < 70 olarak saptandı. Bebeklerin %7'si serebral palsi tanısı aldı. Doğum ağırlığı 1000 gram ve altında olan toplam 50 bebeğin %12'sinin bilişsel ölçek puanı < 70 ve %32'sinin hareket ölçek puanı < 70 olarak belirlendi. Bebeklerin %12,0'si serebral palsi tanısı aldı. Sonuç: Çalışma grubumuzdaki bebeklerimizin 12-18 aydaki gelişimsel sonuçlarının iyi olmasının başarılı yenidoğan yoğun bakım koşulları, BİM kapsamında iyi bir izlem, yeterli gelişimsel destek ve sorunların erken fark edilip yönlendirme yapılmasına bağlı olabileceği düşünülmektedir. Ancak minör gelişimsel sorunların belirlenmesi açısından uzun dönem izleme ihtiyaç vardır. Anahtar kelimeler: Prematüre; Sağlık Hizmeti; Bütüncül İzlem; Çocuk.

Erken yenidoğan dönemde hemolitik aneminin en sık nedeni kan grup uyuşmazlıklarıdır. Son dört dekatta anti-D gamaglobulin kullanmının yaygınlaşması ile Rh uyuşmazlıkları azalmış ve minör kan grubu uyuşmazlıklarının yenidoğan hemolitik anemi etiyolojisindeki oranı giderek artmıştır. Bu nedenle major kan grubu uygunsuzluğu bulunmayan ve periferik yaymada hemoliz bulguları olan yenidoğanlarda minör kan grup uyuşmazlığının düşünülmesi önemlidir. Minör kan grubu uyuşmazlığına bağlı olgularda subklinik hemoliz bulgularından aktif hemoliz ve kan değişimi gerektiren yenidoğan sarılığına kadar değişkenlik gösteren tablolar oluşabilir. Bu yazıda erken yenidoğan hatta fetal dönemde ciddi anemi semptomlarını gösteren ve intravenöz immünoglobulin ile hemolizi kontrol altına alınan bir olgu sunulmaktadır., Blood group incompatibility is the most common cause of hemolytic anemia in the early newborn period. The widespread use of anti-D gammaglobulin in the last four decades has decreased the rate of Rh incompatibility and increased the rate of minor blood group incompatibility in the etiology of newborn hemolytic anemia. It is important to consider minor group incompatibility in newborn babies with hemolysis in peripheral blood smears but without major blood group incompatibility. The clinical picture with minor blood group incompatibility may vary from subclinical hemolysis to active hemolysis and newborn jaundice that necessitates transfusion. We present a case with symptoms of severe anemia in the early newborn and even in the fetal period, in whom hemolysis was controlled with intravenous immunoglobulin

Amaç: Son zamanlarda sadece anne sütüyle beslenen yenidoğanlarda, yüksek morbidite ve mortaliteye neden olabilen hipernatremik dehidratasyon olgularında belirgin artış gözlenmektedir. Bu çalışmada hipernatremik dehidratasyonu olan yenidoğan bebeklerin demografik özelliklerini ve epidemiyolojik risk faktörlerini ortaya çıkarmak ve dehidratasyon olgularının sayısını azaltacak uygulamaları belirlemek amaçlanmıştır. Gereç ve Yöntemler: Çalışmamızda Ocak 2009- Haziran 2012 tarihleri arasında hastanemiz yenidoğan polikliniğine başvuran serum sodyum konsantrasyonu ≥150 mEq/L olan 87 term yenidoğan bebek geriye yönelik değerlendirildi Bulgular: Bebeklerin 49’u (%56) erkek, 38’i (%44) kız ve ortalama doğum ağırlıkları 3424±395 gram’dı. Olguların ortalama başvuru yaşları 4.3±2.8 gün ve ortalama kilo kayıpları %12.2±4.2 olarak bulundu. Normal spontan vajinal yol ile doğum ve primipar annelerin oranı sırası ile %51.7 ve % 75.8’di. Olguların %96.5’i sadece anne sütü ile beslenmişti. Bebeklerin en sık başvuru şikayetleri ateş (%24.7), sarılık (%23.5) ve zayıf emme (%10.6) iken %37.6’inde başvuru anında yakınma bulunmamaktaydı. Çalışmamız süresince bir hastada intrakranial, bir hastada ise ekstremitede tromboz tespit edildi. Başvuru sırasında ortalama serum sodyum, üre ve kreatinin düzeyleri sırası ile 155.2±6.2 mEq/L, 89.1±73.9 mg/ dl ve 0.89±0.65 mg/dl olarak tespit edildi. Hastaların %66.7’sine intravenöz mayi desteği verildi. Olguların hastanede yatış süresi ortalama 2.6±2.3 gündü. Sonuç: Emzirme yetersizliğine bağlı olarak gelişen hipernatremik dehidratasyon yenidoğan bebeklerde halen ciddi bir problem olarak görünmektedir. Yenidoğan bebeklerin hayatın ilk günlerinde rutin olarak değerlendirilmesi hipernatremik dehidratsyonun erken tanı ve tedavisi için önemlidir. Ateş, sarılık ve emmede zayıflık olan bebeklerde hipernatremikdehidratasyon düşünülmelidir. Ayrıca gerekli olgularda sodyum ölçümün yapılması yararlı olabilir., Objective: The incidence of hypernatremic dehydration because of the lactation failure in newborns, causing higher rates of morbidity and mortality, has increased in recent years. The aim of the study was to identify the preventive approaches for hypernatremic dehydration by determining demographic characteristics and epidemiologic risk factors in newborns with hypernatremic dehydration.Material and Methods: A total of 87 term neonates with a serum sodium level ≥150 mEq/L that presented at our clinic between January 2009 and June 2012 were evaluated retrospectively.Results: There were 49 (56%) male and 38 (44%) female infants. Mean birth weight was 3424±395 grams. Mean age and mean percentage of weight loss at admission were 4.3±2.8 days and 12.2±4.2 % respectively. The percentages of spontaneous vaginal delivery and primiparous mothers were 51.7% and 75.8% respectively. Breast milk had been used exclusively in 97%. The most common complaints were fever (24.7%), jaundice (23.5%), and poor absorption (10.6%), while 37.6% did not have symptoms at the time of hospital admission. One infant received a diagnosis of intracranial venous thrombosis and another infant lower extremity venous thrombosis over the study period. The mean sodium, urea and creatinine levels at admission were 155.2±6.2 mEq/L, 89.1±73.9 mg/dl and 0.89±0.65 mg/dl respectively. Intravenous fluid resuscitation was required in 67% of the infants. Mean duration of hospitalization was 2.6±2.3 days.Conclusion: Hypernatremic dehydration from lactation failure remains a serious problem in newborns. Routine newborn examination in the first few days of life is important for the early diagnosis and prevention of this condition. Hypernatremic dehydration should be considered in infants with fever, jaundice and poor sucking. Moreover, determination of the Na level should be helpful when required

Giriş ve Amaç: Bu çalışmada dört yıl içerisinde hastanemizde doğan bebekler içinde immün olmayan hidrops fetalis tanısı alan olguların özelliklerini değerlendirdik.Gereç ve Yöntemler: Retrospektif olarak planlanan ve etik kurul onayı alınan çalışmaya Ocak 2008 ile Aralık 2011 tarihleri arasında hastanemizde doğan, immün olmayan hidrops fetalis tanısı alan olgular dahil edildi.Bulgular: Çalışma süresince 39 olgu immün olmayan hidrops fetalis tanısı aldı. Olgulardan 22’si (%56.4) antenatal dönemde tanı almıştı ve bunların 8’inin (%20.5) nedeni prenatal dönemde saptandı. En sık saptanan bulgu tüm hastalarda görülen yaygın cilt ödemi (%100) iken, neonatal asit (%84.6) ikinci sıklıkta saptandı. Neden olarak iki olguda konjenital kalp hastalığı, iki olguda supraventriküler taşikardi, iki olguda konjenital kistik adenomatoid malformasyon, iki olguda şilotoraks, yedi olguda ikizden ikize transfüzyon sendromu, üç olguda anemi saptanırken, beş olgunun dismorfik olduğu, bir olguda kistik higromaya ve bir olguda ise intrauterin perfore mekonyum ileusuna bağlı immün olmayan hidrops fetalis geliştiği düşünüldü. Geri kalan 14 (%35.9) olguda ise neden bulunamadı. Mortalite oranı %76.9 (30/39) olup, 16’sı (%41) ilk 24 saat içinde kaybedildi.Sonuç: İmmün olmayan hidrops fetalis olgularında mortalite oranı halen çok yüksektir. Çalışmamızda antenatal dönemde en sık rastlanılan bulgular cilt ödemi ve neonatal asitti. Nedenler arasında farklı organ ve sistemlerin hastalıkları yer almaktadır. Bu yüzden tanı için çok sayıda test ve incelemenin kısa sürede yapılması gerekebilmektedir., Objective: We evaluated the characteristics of babies with nonimmune hydrops fetalis born at our hospital during a four-year period.Material and Methods: All newborns with a prenatal or early neonatal diagnosis of nonimmune hydrops fetalis based on clinical history, physical and laboratory examination during the period between January 2008 and December 2011 were evaluated retrospectively.results: A total of 39 newborns with hydrops fetalis were included in this study. Twenty two infants (56.4%) were diagnosed in the antenatal period, and the etiological factor was evident in the prenatal period in 8 infants (20.5%). The most common finding was skin edema in all patients (100%), while neonatal ascites (84.6%) was the second most common sign. Findings related with nonimmune hydrops fetalis were as follows: congenital heart disease (2/39), arrythmia (2/39), congenital cystic adenomatoid malformation (2/39), chylothorax (2/39), cystic hygroma (1/39), twin-totwin transfusion syndrome (7/39), anemia (3/39), genetic/chromosomal abnormalities (5/39), and perforated intrauterine meconium ileus (1/39). No obvious cause was identified in 35.8% (14/39) of cases and these were classified as idiopathic. The mortality rate was 76.9%, and 16 of them (41%) died within the first 24 hours of life.conclusion: The mortality rate is still very high in nonimmune hydrops fetalis. The most common signs were skin edema and neonatal ascites in the antenatal period. Diseases of organs and systems were frequent among the causes of nonimmune hydrops fetalis. Comprehensive diagnostic testing and examinations should therefore be performed as soon as possible

Yenidoğan bakım kalitesinin artması çok küçük prematüre bebeklerin yaşatılmasına olanak sağlamış olmakla birlikte bu bebeklerde “Yeni Bronkopulmoner Displazi” (BPD) olarak tanımlanan klinik, radyolojik ve histopatolojik bulguları daha hafif seyirli olan BPD nin yeni bir şekli ortaya çıkmıştır. Yeni BPD şekli, esas olarak akciğer gelişiminde duraksama ve pulmoner fonksiyonların bozulması ile ilişkilidir. Akciğer gelişiminin kanaliküler ve sakküler fazında doğan prematüre yenidoğanlarda çeşitli risk faktörlerinin etkisiyle alveol ve damar gelişimi sekteye uğramakta böylece alveolar ve kapiller hipoplazi ortaya çıkmaktadır. Surfaktan yetersizliği, yenidoğanın ventilasyonu, oksijen tedavisi, reaktif oksijen ürünleri, akciğer ödemi, fetal ve neonatal infeksiyonlar ve akciğer damar gelişiminde bozulma ile bazı antenatal etmenler akciğer hasarlanmasından sorumlu tutulmaktadır. Bu yazıda prematüre yenidoğanlardaki akciğer hasar mekanizmaları, yeni BPD tanımı, klasik BPD tablosundan farklılıkları, BPD den korunmaya yönelik güncel yaklaşımlar incelenmiştir., Improvements in antenatal and neonatal care have resulted in an increase in the survival of very preterm infants, whereas new form of Bronchopulmonary displasia (BPD) that has mild clinical course, radiological, and histopathological findings has occurred in these babies.. This new form of BPD is mainly related with a disruption of lung organogenesis and impaired pulmonary function. In premature infants born at the intersection of the canalicular and saccular phases of lung development, alveolar and capiller development are inhibited following exposure to the noxious effects of different risk factors. As a result, alveolar and capiller hypoplasia occur in these premature babies. These risk factors include surfactant deficiency, neonatal ventilation, oxygen therapy, reactive oxygen species, lung oedema, fetal and neonatal infections, disruption of vasculogenesis, and some antenatal factors that dysregulate subsequent lung development. In this article, mechanisms of lung injury in the preterm infants, the definition of new BPD, the difference between ‘‘old’’ and ‘’new’’ BPD, and the current approaches to prevent BPD have been reviewed

Patent duktus arteriozus çok düşük doğum ağırlıklı prematüre bebeklerde soldan sağa şanta bağlı olarak konjestif kalp yetmezliği, nekrotizan enterokolit, bronkopulmoner displazi, intrakraniyal kanama ve ölüme neden olabilmektedir. Çok düşük doğum ağırlıklı prematüre bebeklerde patent duktus arteriozusun cerrahi ligasyonu için en uygun zamanın ne olduğu tartışma konusu olmaya devam etmektedir. Optimum tedavi yaklaşımına rağmen şiddetli duktal şanta bağlı intestinal, renal ve pulmoner problem yaşayan 3 hastamız oldu. Bir hasta medikal tedaviye dirençliydi. İki hastaya kontrendikasyon nedeniyle medikal tedavi verilemedi. Duktus arteriosus açıklığı olan çok düşük doğum ağırlıklı bebeklerde medikal tedaviye yanıtsızlık veya tedavinin kontrendike olduğu durumlarda erken cerrahi yaklaşım akılda tutulmalıdır., Patent ductus arteriosus (PDA) causing large left-to-right shunts may lead to congestive heart failure, necrotizing enterocolitis, bronchopulmonary dysplasia, intracranial hemorrhage, and death in extremely-low-birthweight premature infants. Optimal timefor surgical ligation of PDA in extremely-low-birth-weight premature infants remains controversial.Herein, we report 3 patients who had intestinal, renal, and pulmonary complications due to severe PDA. One patient’s ductus was refractory to medical treatment while the remaining2 patients could not receive medical treatment because of contraindications. We suggest that early surgical ligation should be considered in extremely-low-birth-weight premature infants with PDA refractory to medical treatment or in those who have contraindications for medical treatment

Giriş: Purpura fulminans nadir görülen bir klinik tablodur. Yenidoğan döneminde sıklıkla konjenital veya kazanılmış protein C veya S eksikliğinden kaynaklanır. Sepsis ilişkili yaygın damar içi pıhtılaşma da bir diğer risk faktörüdür.Yöntem: Üçüncü basamak bir yenidoğan yoğun bakım ünitesinde Ocak 2008 ile Mayıs 2010 tarihleri arasında purpura fulminans tanısı ile izlenmiş olguların dosyaları geriye dönük olarak incelendi. Olguların faktör 5 Leiden, protrombin G20210A ve metilentetrahidrofolat redüktaz mutasyonu sonuçları, plazma protein C, S, antitrombin 3, antifosfolipid antikor ve homosistein düzeyleri ve kültür sonuçları kaydedildi.Bulgular: Toplam yedi olgunun verileri değerlendirildi. Olguların tamamının tanı anında plazma protein C, S ve antitrombin 3 düzeyi düşüktü. Metilentetrahidrofolat redüktazın homozigot mutasyonu bir olguda, heterozigot mutasyonu ise bir diğer olguda saptandı. Homosistein düzeyi metilentetrahidrofolat redüktaz mutasyonu olan olgularda hafif yüksek; diğerlerinde ise normaldi. Bir olguda tedavi sonrasında protein C, S ve antitrombin 3 düzeyi tekrarlanabildi ve sonuçlar normal bulundu. Dört olgunun kan kültüründe etken, izole edildi. İzole edilen mikroorganizmalar Pantoea agglomerans, Candida albicans, Klebsiella pneumonia ve Staphylococcus hominis’di.Sonuç: Bildiğimiz kadarıyla serimiz bugüne kadar yayınlanmış en geniş purpura fulminans tanılı yenidoğan olgu serisidir ve Pantoea agglomerans, Candida albicans ve Klebsiella pneumonia yenidoğanlarda purpura fulminans etkenleri olarak ilk kez bu seri ile gösterilmiştir. Purpura fulminans tanısı ile izlenen ve etiyolojide sepsis ilişkili yaygın damar içi pıhtılaşma düşünülen yenidoğan olgularda bu nadir patojenler de akılda tutularak tedavi ve takipler planlanmalıdır., Introduction: Purpura fulminans is a rare clinical entity. In the neonatal period it is commonly caused by inherited or acquired protein C or S deficiency. Disseminated intravascular coagulation associated with septicemia in newborns may be another risk factor for purpura fulminans. Methods: Charts of the patients followed up with the diagnosis of purpura fulminans at a tertiary level neonatal intensive care unit between January 2008 and May 2010 were retrospectively reviewed. Factor 5 Leiden, prothrombin 20210 A and methylenetetrahydrofolate reductase mutations, levels of plasma protein C, S, antithrombin III, antiphospholipid antibody and homocystein, and culture results were recorded in all the patients. Results: Results of a total of seven patients were evaluated. Plasma protein C, S and antithrombin III levels were found decreased at the time of diagnosis in all the cases. One of the cases had methylenetetrahydrofolate reductase homozygous mutation, and another had methylenetetrahydrofolate reductase heterozygous mutation. Homocystein levels were slightly elevated in cases with methylenetetrahydrofolate reductase mutation, and normal in the others cases. Plasma protein C, S and antithrombin 3 levels were studied in only one case after treatment and were normal. A pathogen was identified in blood culture in four out of the seven cases. The isolated microorganisms were Pantoea agglomerans, Candida albicans, Klebsiella pneumonia and Staphylococcus hominis.Conclusion: Pantoea agglomerans, Candida albicans, and Klebsiella pneumonia were determined as the causative agents of purpura fulminans in this series of newborns, which, to our knowledge, is the largest one reported till now. These pathogens should be remembered in infants who are followed up with the diagnosis of purpura fulminans and suspected to have disseminated intravascular coagulation associated with sepsis

Giriş ve Amaç: Yenidoğan ünitelerinde son yıllarda görülen gelişmelerle birlikte preterm bebeklerde mortalite azalmıştır. Bu azalma özellikle aşırı düşük doğum ağırlığı (ADDA) olan bebek grubunda belirgindir. Bu grubun yoğun bakım kısa dönem sonuçları, yoğun bakım izlem ve tedavi özellikleri, eşlik eden morbidite sıklığı ve şiddeti diğer hasta gruplarından belirgin derecede farklıdır. Bu hasta grubunun verilerinin ortaya konması hasta tedavi ve izlem protokollerinin oluşturulması açısından önem arz etmektedir. Ülkemizin en büyük yenidoğan yoğun bakım servisine sahip olan ünitemizin aşırı düşük doğum ağırlıklı hasta grubunun verilerinin ortaya konmasını amaçladık. Gereç ve Yöntemler: Bu retrospektif çalışmada, hastanemiz yenidoğan kliniğinde Ocak 2009-Temmuz 2010 tarihleri arasında izlenen, ADDA prematüre bebekler alınmıştır. Perinatal demografik özellikler ile yoğun bakım izlem özellikleri, kısa dönem izlem sonuçları ile eşlik eden morbiditeler kaydedilmiştir. Bulgular: Yüz elli dört ADDA prematüre bebek çalışmaya alındı. Kırkbir hasta (%26.6) izlem sırasında kaybedildi. Yoğun bakım izlemleri yapılan 113 ADDA prematüre bebeğin ortalama gebelik haftası 26.9±1.8 hafta, ortalama doğum ağırlığı 853±60 gram, ortalama anne yaşı 27.7±5.6 yıl idi. Hastaların izlemlerinde %53.9 patent duktus arteriozus (PDA), %9.7 nekrotizan enterokolit (NEK), %21.4 ciddi İntrakraniyal Kanama [(İKK);evre 3,4)], şant operasyonu gereken hidrosefali ise %3.5 olarak gözlendi. Tedavi gerektiren prematüre retinopatisi (ROP) %23 gözlendi. Bronkopulmoner displazi(BPD) ise %30.3 oranında gözlendi. Sonuçlar: Çalışmamızda ortaya konan ADDA prematüre bebeklere yönelik ülkemizden yayınlanan bu ilk geniş serili veriler bu grupta mortalitenin düşük seyretmesinin yanında yüksek oranda eşlik eden morbiditelere yönelik tedavi ve takip politikalarının düzenlenmesi gerekliliğini ortaya koymuştur., Indroduction and Objective: Mortality of preterm infants has been decreased with recent developments in the neonatal intensive care units (NICU). This decrease is prominent especially in the extremely low birthweight (ELBW) preterm infants group. Intensive care unit short-term outcomes, features of the intensive care follow-up, the treatment, incidence and severity of comorbidity differ in this group than the others. Results of the data for this patient group is important to set the patient treatment and follow-up protocols. We aimed to present the data of ELBW preterm infants who are discharged from our NICU which is the largest one in Turkey.Material and Methods: In this retrospective study, ELBW preterm infants who were followed-up at our Neonatology outpatient clinic of our hospital between January-2009 and July 2010 areincluded. Perinatal demographic features, intensive care follow-up features, short-term outcomes and comorbidities are recorded.Results: A hundred and fifty-four ELBW preterm infants were included to the study. Among 113 survived ELBW preterm infants in the NICU mean gestational week, mean birth weight and mean maternal age were 26.9±1.8 weeks, 853±60 g and 27.7±5.6 years, respectively. During the hospitalization, patent ductus arteriosus (PDA) was observed in 53.9%, necrotizing enterocolitis (NEC) in 9.7%, pneumothorax in 5.5%, pulmonary hemorrhage in 9.8%, severe intracranial hemorrhage (stage 3 and 4) 21.4% and hydrocephalia requiring shunt operation was in 3.5% of the neonates. Retinopathy of premature (ROP) requiring treatment was observed in 23%. Bronchopulmonary dysplasia (BDP) was found to be 30.3% of the patients whereas only eight patients (8%) required home-oxygen treatment.Conclusion: This first wide serie data presented in our country in ELBW premature neonates indicates that special treatment and follow-up policies should be set for the related comorbidities in the era of low mortality

Neuroblastoma is the most frequent extracranial solid tumor in childhood, but it is seldom diagnosed prenatally. It usually presented with adrenal masses. Presentation of other localization is extremely rare. We report a case of cervical neuroblastoma identified at 20 weeks of gestational age. This is the third case diagnosed antenatally on neck region in the literature. Additionally, it is the first case that extended to the brain. We also discussed the literature for cervical neuroblastoma detected prenatally.

Nager tipi akrofasiyal disostoz, kraniyofasiyal bölge ve ekstremiteleri ilgilendiren deformitelerin olduğu, diğer sistemlere ait anomalilerin eşlik edebildiği nadir görülen bir genetik bozukluktur. Şimdiye kadar yaklaşık 80 vaka bildirilmiştir. Vakamızda belirgin mikrognati , malar ve maksiller hipoplazi, yarık damak, düşük kulak, antevert burun delikleri, sol kulakta işitme azlığı, baş parmak, radius yokluğu, kısa kol, tibia hipoplazisi, ayak bileği deformitesi mevcuttu. Hasta altıncı ayında beslenme güçlüğüne bağlı malnütrisyon ve enfeksiyon nedeniyle kaybedildi. Vakayı, diğer kraniyofasiyal (mandibulofasiyal) ve akrofasiyal disostoz sendromlarından ayırt edici özeliklerini vurgulamak, sendromda alt ekstremite anomalileri nadir olmasına rağmen vakamızda farklı bir klinik prezentasyon bulunduğu için sunmaya değer bulduk., Nager type acrofacial dysostosis is a rare genetic disorder which is seen with the deformities of craniofacial region and extremities,which may accompany with sistemic disorders. To date approximately 80 cases have been reported. Our case had prominent micrognathia, malar and maxillary hypoplasia, cleft palate, low set ears, antevert nares, hearing loss in the left ear, the absence of thumb and radius, short arm, hypoplasia of the tibia and ankle deformity. The patient was died because of the malnutrition due to feeding difficulties and infection at the sixth month. We herein present this case to emphasize the distinguishing features of this syndrome from other syndromes of craniofacial (mandibulofacial) and acrofacial dysostosis as our case had a different clinical presentation since lower extremity anomalies are rarely seen in Nager type acrofacial dysostosis

Yenidoğanda hemostatik sistemdeki fizyolojik özelliklere ek olarak kalıtsal veya kazanılmış protrombotik risk faktörlerinin varlığı tromboza eğilime neden olmaktadır. Prematüre ve hasta yenidoğanların yaşama oranının artması nedeniyle yoğun bakım tedavisi ve izlemi ile ilişkili komplikasyonların görülme sıklığı da artmıştır. Trombotik komplikasyonlar asemptomatik olabileceği gibi organ kaybına neden olan bir klinik tablo ile de sonuçlanabilir. Bu nedenle, yenidoğan döneminde trombozun erken dönemde tanınması ve uygun tedavinin planlanması önemlidir.
