Abstract

Objective: The aim of our study was to evaluate sonographic endometrial thickness succeeding the estrogen-only stage and on the day when embryo transfer (ET) occurred in artificial frozen embryo transfer (FET) cycles to delve into the effect of endometrial compaction (EC) on clinical pregnancy rate (CPR). Patients and Methods: In the first group endometrial thickness diminished when ET occurred when compared to the end of the estrogen-only phase (n:37). Endometrial thickness increased/did not alter for the second group (n:70). Demographic characteristics were recorded and the following were studied: in vitro fertilization (IVF) treatment indications, hormone levels, total antral follicle count, duration of infertility, embryo quality, embryo-fundus distance, endometrial thickness at the end of estrogen-only phase and on ET day, luteal support, CPR. Abstract

Objectives: To investigate the effect of human chorionic gonadotropin day progesterone (hCG-P) level on pregnancy outcomes in in vitro fertilization (IVF) cycles. Material and methods: This study is an analysis of a cohort of 1318 fresh IVF-embryo transfer cycles, including 579 agonists and 739 antagonists, performed at a single IVF center between 2007 and 2018. For fresh cycles, we performed Receiver Operating Characteristic analysis (ROC) to calculate the threshold value of hCG-P, which affects pregnancy outcomes. We divided patients below and above the determined threshold value into two groups, then, correlation analysis and we performed logistic regression analysis. Results: According to ROC curve analysis of hCG-P, AUC was 0.537 (95% CI: 0.510-0.564, p < 0.05) for LBR, and the threshold value for P was 0.78. The hCG-P threshold value of 0.78 proved to be significant in relation to BMI, type of drug used during induction, the hCG day E2, the total number of oocytes, the number of oocytes and the subsequent pregnancy outcome between the two groups (p < 0.05). However, the model we built, which accounted for hCG-P, total number of oocytes, age, BMI, induction protocol, total dose of gonadotropin used in induction did not prove significant in terms of its effect on LBR. Conclusions: The threshold value of hCG-P that we found to have an effect on LBR was quite low compared with the p value generally recommended in the literature. Therefore, further studies are needed to determine an accurate p-value that reduces success in managing fresh cycles. [ABSTRACT FROM AUTHOR]

Abstract

Introduction. Tubo-ovarian abscess (TOA) is a conglomerated mass of pelvic organs including tube, ovary, and bowel. The most commonly isolated organisms from TOAs are Escherichia coli (E. coli) and Bacteroides species. Case Report. We reported a case of Clostridium septicum (C. septicum) infection from a ruptured TOA with atypical clinical features. Culture of intra-abdominal free fluid obtained during surgery yielded C. septicum. VITEK II (bioMérieux, France) automated system was used for advanced identification of the bacteria. Parenteral clindamycin in combination with an aminoglycoside was used. The patient was discharged 19 days after the surgery and was clinically asymptomatic 6 months after the surgery. Conclusion. The differential diagnosis of TOA caused by C. septicum can be difficult, due to the lack of the symptoms. Tissues infected with C. septicum can become necrotic. A combination of early, adequate antibiotic therapy and surgery is the key point of the treatment.

Abstract

Aim: In this study, we aimed to examine the factors predicting pregnancy by comparing successful in vitro fertilization/intracytoplasmic sperm injection (IVF)/(ICSI) fresh cycles with previous failed ones of the same patients in the same year. Materials and Methods: The study consisted of two groups, failed (n:283) and successful IVF fresh cycles (n:283) that applied one after another within the same year, thusly each woman acted under her own control. IVF treatment indications, antral follicle count, ovulation induction protocol type, initial and total gonadotropin dose, progesterone, and estradiol (E2) levels on the trigger day, the number of oocytes retrieved and mature oocytes, number of Grade 1, Grade 2, Grade 3 embryo, day of embryo transfer (ET), endometrial thickness on trigger and oocyte pick-up (OPU) day, embryo-fundus distance and beta-human chorionic gonadotropin (B-hCG) levels on the 10th day after the ET were measured. The same luteal phase support was administered in both groups. Results: Totally 566 cycles and 283 women were included in the study. There was a significant difference in luteinizing hormone (LH) levels on OPU day and number of grade 1 ET between the two groups, LH levels on OPU day and number of grade 1 ET were higher in successful IVF fresh cycles group (p<0.05). Conclusion: Low serum LH levels on OPU day was associated with decreased pregnancy results. Increased IVF success was observed in grade 1 ET cycles. [ABSTRACT FROM AUTHOR]

38. De novo postmenopausal endometriosis during tibolone treatment: a case report and review of the literature

Abstract

Abstract: Endometriosis is uncommon before puberty and after menopause as it is an estrogen-dependent disease. A case is presented of postmenopausal endometriosis encountered in a patient who had received tibolone (Livial, Organon, Cambridge, UK) 1 year before the diagnosis of the adnexal mass for 3 months for relief from vasomotor symptoms and had the medication stopped because of fibrocystic disease of the breast. Transvaginal ultrasonography showed homogeneous cystic adnexal mass of 36 � 26 mm with no internal echoes in the right ovary. Laparoscopic right salpingooophorectomy was performed and the histopathological examination of the cyst showed an endometriotic cyst. Most of the cases with postmenopausal endometriosis are associated with the use of hormone replacement therapy (HRT). However, tibolone is recommended in hormone replacement therapy of postmenopausal symptomatic women who have a past history of hormone-dependent tumors such as endometriosis. There is restricted data in the literature about tibolone use and recurrence or de novo formation of endometriosis.
