Objective: Celiac disease is a proximal small bowel disease developing due to persistent intolerance in people who are genetically susceptible to gluten and other gluten-like grain proteins in cereals such as mainly wheat, barley, rye, and oats. Treatment is a lifelong gluten-free diet. Strict adherence to this treatment is important for the prognosis of the disease. Since it is quite costly to follow-up of the disease, there is need to parameters that are easy to apply to reflect diet compliance and antibody level and do not require additional cost. In this study we aimed to determine whether hemogram parameters and albumin level can be used to evaluate the compliance of gluten-free diet in Celiac patients. Methods: Fifty-seven of 133 Celiac patients whose disease was confirmed with anti-tissue transglutaminase-IgA (anti-tTG-IgA) levels were included to the study. Neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), mean platelet volume (MPV), platelet distribution width (PDW), erythrocyte distribution width (RDW) and albumin levels were compared in the periods when the anti-tTG-IgA levels of the patients were positive and negative. The relationship of these results with gluten-free diet compliance was evaluated. Results: The mean age of the patients was 38.96 (19-66). RDW value, at anti-tTG-IgA positive the period was significantly higher compared to the period when it was negative (p=0.005). PDW (p=0.02) and albumin (p=0.035) values were significantly low. Although the PLR (p=0.074) value was found to be higher, this difference was not statistically significant. There was no difference in NLR (p=0.69) and MPV (p=0.12) values. Conclusion: PDW, RDW, and albumin levels are more cost-effective, and can be used as an auxiliary parameter to evaluate dietary adherence and antibody levels in Celiac patients' follow-up. Introduction: Introduction: hypertension and diabetes are chronic disorders associated with an increased risk of cardiovascular disease. Objectives: to evaluate the effect of 52 % low-sodium salt applied to the Chinese-modified DASH (CM-DASH) diet on risk of atherosclerotic cardiovascular disease (ASCVD) in patients with hypertension and type-2 diabetes. Methods: the low-sodium salt group (LSSG) took 5 g/day of 52 % low-sodium salt plus CM-DASH diet for 8 weeks, while the normal-sodium salt group (NSSG) took the same dose of normal-sodium salt plus CM-DASH diet for 8 weeks. Blood tests, 24-hour urine tests, anthropometric measurements, and 10-year risk of ASCVD prediction were assessed. Results: compared with baseline, both LSSG and NSSG showed a significant reduction in 10-year risk of ASCVD, but we did not find any statistically significant differences in 10-year risk of ASCVD between LSSG and NSSG. Conclusions: our study shows that salt limits and DASH diets reduce the risk of cardiovascular disease whereas low-sodium salt containing 52 % sodium chloride did not significantly lower the risk of cardiovascular disease when compared to regular salt. Due to the limitations of the research, additional studies will be necessary to confirm our findings. [ABSTRACT FROM AUTHOR]- Published

With an ageing population, healthy ageing becomes more important. Healthy nutrition is part of this process and can be supported in many ways. The PROMISS trial studies the effect of increasing protein intake in older adults on their physical functioning. Within this trial, a sub-study was performed, researching the added effect of using a diet-tracking app enhanced with persuasive and (optional) gamification techniques. The goal was to see how older adult participants received such technology within their diet program. There were 48 participants included in this sub-study, of which 36 completed the study period of 6 months. Our results on adherence and user evaluation show that a dedicated app used within the PROMISS trial is a feasible way to engage older adults in diet tracking. On average, participants used the app 83% of the days, during a period of on average 133 days. User-friendliness was evaluated with an average score of 4.86 (out of 7), and experienced effectiveness was evaluated with an average score of 4.57 (out of 7). However, no effect of the technology on protein intake was found. The added gamification elements did not have a different effect compared with the version without those elements. However, some participants did like the added gamification elements, and it can thus be nice to add them as additional features for participants that like them. This article also studies whether personal characteristics correlate with any of the other results. Although some significant results were found, this does not give a clear view on which types of participants like or benefit from this technology.

Abstract Background The aim of the study was to assess both patients’ and their parents’ knowledge of phenylketonuria (PKU) treatment and compliance with PKU diet. Methods The study included 173 PKU patients aged 10–19 and 110 parents of PKU children who were enrolled in the study on the basis of questionnaire data. The study also included 45 patients aged ≥20. Results Our study demonstrated that only 45% (n = 74) of PKU patients knew daily Phe intake recommendations, 27% of patients (n = 41) knew the Phe content in a minimum of three out of four researched food products. Patients’ knowledge concerning Phe intake (p = 0.0181) and the knowledge of selected food products (p = 0.041819) improved with age. We did not establish such a correlation in the group of PKU children’s parents. Approximately 31% of patients and 22% of parents reported helplessness, which increased with the child’s age, associated with the necessity to adhere to the diet; 30% of patients reported feeling ashamed of the fact that they could not eat all food products. Regardless of age, children were more likely than parents to report helplessness (p = 0.032005). Among patients, 41.40% declared that they would wish to select products unassisted but their parents did not permit them to do so. The question of whether parents teach children self-reliance in meal preparation was answered affirmatively by 98% of parents and only 81% of children (p = 0.0001). Conclusion Our data demonstrated that parents’ and children’s knowledge concerning treatment recommendations and food products does not have a direct impact on attitude to the PKU diet. Limiting children’s independence in meal selection, growing helplessness in the face of dietary adherence and shame resulting from the necessity to follow a different diet observed in PKU families are responsible for shaping and perpetuating a consistently negative attitude to the diet. The care of PKU paediatric patients requires consistent, long-term family and individual therapy which may counteract the effects of learned helplessness. In regard to the educational effort, a good parent-child relationship as well as the teaching of behaviours motivating patients to comply with the diet are of great importance.

FLUID control , *HEMODIALYSIS patients , *PATIENT education , *PATIENT compliance , *ACTIVITIES of daily living - Abstract
Introduction: Some hemodialysis patients may experience problems in completing activities of daily living and adhering to diet and fluid restrictions due to a decrease in self‐care power and a loss of competence. Methods: The study was conducted with 78 people assigned to the intervention (N = 38) and control groups (N = 40). Data were collected using the sociodemographic characteristics questionnaire, dialysis diet and fluid nonadherence questionnaire (DDFQ), and fluid control in hemodialysis patients scale (FCHPS). The participants in the intervention group were given the "Nutrition Education Booklet for Dialysis Patients". The participants in the intervention group were trained through four education sessions across 4 months, and the measurement tools were administered to them. The participants in the control group were interviewed twice, once at the onset of the study and once 2 months later and the measurement tools were administered to them. Findings: In the intervention group, a decrease was observed in the pre‐ and postdialysis interdialytic weight gain, ultrafiltration (UF) volume, and blood pressure values of the patients after the training. There was a statistically significant decrease in the mean scores for the frequency and degree of nonadherence to diet restriction, and for the frequency and degree of nonadherence to fluid restriction in the participants in the intervention group compared to the participants in the control group (P < 0.05). There was a statistically significant increase in the mean scores obtained from the FCHPS and its subscales by the participants in the intervention group compared to the participants in the control group (P < 0.05). Conclusion: The training given to the hemodialysis patients positively contributed to their adherence to diet and fluid restrictions. The patients' adherence to diet and fluid restriction increased. Background: Diabetes mellitus (DM) was a condition of insufficient insulin because the pancreas was damaged or insulin could not be used effectively. DM was related to diet. Food for DM patients was important to monitor so the blood sugar would be controlled. Preliminary studies showed that 60% of patients had low levels of dietary compliance. This study aimed to look at the relationship of family support with hospital diet compliance in type 2 DM patients and see the relation between the knowledge levels with hospital diet compliance in type 2 DM patients in Surabaya Haji Hospital. Materials dan Methods: This research was an analytical study with a cross sectional approach. The research variables consisted of family support, the knowledge levels and hospital diet compliance. Respondents were type 2 diabetes mellitus patients in inpatient Hajj Surabaya Hospital with inclusion criteria: 1) without complications of type 2 diabetes mellitus patients; 2) able to communicate well; and 3) willing to become respondents by signing informed consent. Samples were obtained through simple random sampling. Data were analyzed using frequency distribution, cross tabulation, and chi-square statistical test to determine the effect of variables. Result: The results showed that almost all respondents with high family support had high dietary compliance (81.3%), the chi-square test results show that ρ value is 0.004 <α 0.05. That is, there is a relationship between family support and adherence to hospital diet in patients with type 2 diabetes in Surabaya Haji Hospital. The results showed that there were no DM sufferers who had good knowledge about DM patient diet. 75% of respondents with a low level of diet compliance had a low knowledge level, the chi-square test results showed that ρ value was 0.013 < α (0.05), which meant there was a relation between the knowledge level with hospital diet compliance in type 2 DM patients. Conclusion: Family support and knowledge levels about diet regulation would affect the compliance level of the hospital diet to respondents so the health education was needed for families who look after the hospital. Background: The aim of the study was to assess both patients' and their parents' knowledge of phenylketonuria (PKU) treatment and compliance with PKU diet. Methods: The study included 173 PKU patients aged 10-19 and 110 parents of PKU children who were enrolled in the study on the basis of questionnaire data. The study also included 45 patients aged ≥20. Results: Our study demonstrated that only 45% ( n = 74) of PKU patients knew daily Phe intake recommendations, 27% of patients ( n = 41) knew the Phe content in a minimum of three out of four researched food products. Patients' knowledge concerning Phe intake ( p = 0.0181) and the knowledge of selected food products ( p = 0.041819) improved with age. We did not establish such a correlation in the group of PKU children's parents. Approximately 31% of patients and 22% of parents reported helplessness, which increased with the child's age, associated with the necessity to adhere to the diet; 30% of patients reported feeling ashamed of the fact that they could not eat all food products. Regardless of age, children were more likely than parents to report helplessness ( p = 0.032005). Among patients, 41.40% declared that they would wish to select products unassisted but their parents did not permit them to do so. The question of whether parents teach children self-reliance in meal preparation was answered affirmatively by 98% of parents and only 81% of children ( p = 0.0001). Conclusion: Our data demonstrated that parents' and children's knowledge concerning treatment recommendations and food products does not have a direct impact on attitude to the PKU diet. Limiting children's independence in meal selection, growing helplessness in the face of dietary adherence and shame resulting from the necessity to follow a different diet observed in PKU families are responsible for shaping and perpetuating a consistently negative attitude to the diet. The care of PKU paediatric patients requires consistent, long-term family and individual therapy which may counteract the effects of learned helplessness. In regard to the educational effort, a good parent-child relationship as well as the teaching of behaviours motivating patients to comply with the diet are of great importance. [ABSTRACT FROM AUTHOR]

Introduction: hypertension and diabetes are chronic disorders associated with an increased risk of cardiovascular disease.

Background: The ketogenic diet (KD) is a very low-carbohydrate, high-fat and adequate-protein diet with no calorie limit that induces a metabolic condition called "physiological ketosis". It was first introduced to treat epilepsy in the 1920s and has become quite popular recently as weight-loss and performance-enhancing diet. Its therapeutic use in a range of diseases is under investigation. During KD interventions people are supposed to monitor compliance with the dietary regimen by daily urine testing for ketosis. However, there are no studies investigating the best time for testing. Findings: Twelve healthy subjects (37 ± 11 years; BMI = 23.0 ± 2.5 kg/m²) were instructed to, during the sixth week of a KD and with stable ketosis, measure their urine (8x) and blood (18x) ketone concentration at regular intervals during a 24-h period. According to their 1-day food record, the subjects consumed on average a diet with 74.3 ± 4.0 %, 19.5 ± 3. 5 %, and 6.2 ± 2.0 % of total energy intake from fat, protein and carbohydrate, respectively. The lowest blood β-hydroxybutyrate (BHB) (0.33 ± 0.17 mmol/l) and urine acetoacetate (AA) (0.46 ± 0.54 mmol/l) concentrations were measured at 10:00, respectively. The highest BHB (0.70 ± 0.62 mmol/l) and AA concentrations were noted at 03:00, respectively. Via urine testing the highest levels of ketosis were found at 22:00 and 03:00 and the highest detection rates (>90 %) for ketosis were at 07:00, 22:00 and 03:00, respectively. Conclusions: These results indicate that ketonuria in subjects with stable ketosis is highest and can be most reliably detected in the early morning and post-dinner urine. Recommendations can be given regarding precise time of the day for measuring ketone bodies in urine in future studies with KDs. [ABSTRACT FROM AUTHOR]

Type 2 Diabetes Mellitus (DM) is one of chronic diseases resulting from decreased insulin production and is characterized by an increase in glucose in the blood. Dietary adherence is one of the effective managements that can control blood sugar levels. This management requires supporting factors and one of them is motivation. This research aimed to describe the motivation and dietary compliance of patients with type 2 DM at the working area of Puskesmas Padalarang. This quantitative descriptive research used a cross-sectional approach. The samples were selected using proportional random sampling techniques. This study used the Treatment Self-Regulation Questionnaire (TSRQ) with the validity score 0.4821 and a diet adherence questionnaire with the validity score 0.4732. The results showed that the internal motivation of respondents was a good internal motivation with 33.3% and 66.7% had low motivation. Only 2.2% had good external motivation and almost all respondents had a low external motivation with 97.8%. Meanwhile, there were only 3.3% with a good level of adherence, 94.4% with a sufficient level of adherence and 2.2% had a poor level of diet adherence. Therefore, it is expected for Puskesmas to implement more motivational interviewing therapy to increase motivation and diet adherence in type 2 DM patients.

Nowadays, gamification is applied in many areas, including healthy lifestyle promotion. In earlier work, a system has been proposed to stimulate diet compliance and adherence of participants of a trial within the PROMISS-project. In this paper, we describe the design of a gamified version of this system. The goal of the gamification is to further stimulate diet compliance and adherence to the system, but also to increase the knowledge about the diet and make the use of the system more fun. To do so, we implemented gamification elements (profile page, achievements, mini-games, and a reward garden) to address multiple behaviour change techniques. Based on a small evaluation, the system has been improved so that it can be used by participants of the PROMISS trial. Background: People with diabetes mellitus are increasing in number. Controlling blood sugar levels is an action that must be taken to get normal and stable blood sugar levels. Self-control takes precedence over treatment. Compliance with diet is one of the important things in controlling blood sugar levels. Raflesia Room as one of the internal medicine inpatients in dr. SoedomoTrenggalek, received inpatient care for the majority of diabetes mellitus patients. Purpose : The purpose of this study was to analyze the relationship between diet adherence and blood sugar levels in people with diabetes mellitus. Methods : The research design used descriptive analytic with cross sectional approach. The population in this study were patients with diabetes mellitus in the Raflesia room of Dr. SoedomoTrenggalek Hospital in January 2017 with 30 patients. With a simple random sampling technique so that 29 samples were obtained. Result : The results showed that almost all respondents, namely 25 (86%) respondents, had abnormal blood sugar levels. Almost all respondents, 23 (79%) did not comply with the diet. Chi square test results obtained 0.004 which means there is a relationship between dietary compliance with blood sugar levels of diabetes mellitus patients in Raflesia room Dr. Soedomo Hospital Trenggalek in 2017. Conclusion : Old age is an age that is susceptible to a decrease in body function, especially hormonal causes of decreased pancreatic function resulting in a decrease in insulin sensitivity which causes high blood sugar levels. The level of education has an important role in diet compliance where patients are required to understand 3J "Types, Amounts and Schedules" so that the lower level of education causes lower diet compliance. Disobedience to diets causes sufferers to consume irregular foods which cause a pile of glucose and blood so that hyperglycemia occurs.

[ABSTRACT FROM AUTHOR]

CELIAC disease , *GLUTEN-free diet - Abstract
Background: In coeliac disease (CD) there is a permanent gluten intolerance requiring life-long adherence to a strict gluten-free diet (GFD). An inadequate diet increases the risk for long-term complications. Coeliac patients often have great difficulty in maintaining a strictly GFD. We aimed to study whether young adults with CD diagnosed before the age of 4 years have a better dietary compliance than patients diagnosed later in life. Method: Twenty-nine adults with CD diagnosed in childhood were studied. They had had CD for 17-24 (mean 20) years. Their compliance to GFD was assessed using a questionnaire and serological markers (IgA and IgG anti-endomysium antibodies and IgA anti-tissue transglutaminase antibodies). Results: At least 80% of the coeliac patients who had been diagnosed before the age of 4 years complied with the GFD compared to 36% of the CD patients older than 4 years at diagnosis ( P < 0.05). Conclusion: This is the first study to show that patients with CD diagnosed before 4 years of age keep to a GFD significantly better than patients diagnosed after 4 years. It is thus important to diagnose childhood CD as early as possible in order to minimize the risk for reduced well-being and other potentially serious complications in coeliac individuals on an inadequate diet. Diagnosis of histamine intolerance (HIT) has been based on low serum diamine oxidase (DAO) values, functional gastrointestinal disorders and improvement of symptoms with a histamine-reduced diet (HRD). In a retrospective analysis of outpatients' charts we identified 101 patients with HIT. After a median of 13 months, a questionnaire was distributed to the patients so that they could be classified into four diet-compliance groups. Calculated with all 101 patients we found an increase of serum DAO values due to a HRD. In the 63 patients that completed the questionnaire, we found that 50 patients had improvement of symptoms or no continuing symptoms. A significant increase of serum DAO levels was found in the patients with strict and occasional diet compliance. Therefore, we demonstrate that a HRD is not only improving symptoms in HIT, but is causing an increase in serum DAO values that correlates with the degree of diet compliance.

Diabetes mellitus (DM) is one of the biggest global health emergencies in the 21st century. Diabetes mellitus is a genetic or hereditary disease that is difficult to cure but can be controlled with a 3 J diet program that is the number, type and schedule. Patient adherence to the diet program is one of the obstacles in patients with type 2 diabetes mellitus. Compliance is caused by many factors, one of which is self-motivation and support of health workers. The purpose of this study was to determine whether there is a relationship between self-motivation and support of health workers with diet compliance with DM type 2 sufferers in the Work Area of the Community Health Center of Bangkinang City. This research is a quantitative study with cross sectional design. The sample in this study were some patients with type 2 diabetes mellitus in the work area of the City Health Center Bangkinang, amounting to 86 people with a purposive sampling technique. Data processing using chi-square test. The results showed that there was a significant relationship between self-motivation (p value = 0,000) and support of health workers (p value = 0.020) with diet compliance of DM type 2 sufferers in the Bangkinang City Health Center Work Area. The results of this study are expected to be used by people with type 2 diabetes are expected to be able to comply with the correct diet according to diet 3j, namely the type, amount and schedule recommended by health workers, patients are expected to carry out healthy living behaviors.

Introduction: Some hemodialysis patients may experience problems in completing activities of daily living and adhering to diet and fluid restrictions due to a decrease in self-care power and a loss of competence. The participants in the intervention group were given the “Nutrition Education Booklet for Dialysis Patients”. The participants in the intervention group were trained through four education sessions across 4 months, and the measurement tools were administered to them. The participants in the control group were interviewed twice, once at the onset of the study and once 2 months later and the measurement tools were administered to them. Findings: In the intervention group, a decrease was observed in the pre- and postdialysis interdialytic weight gain, ultrafiltration (UF) volume, and blood pressure values of the patients after the training. © 2019 International Society for Hemodialysis

Gluten-free diet (GFD) is the only treatment for celiac disease (CD). There is a general consensus that strict GFD adherence in CD patients leads to full clinical and histological remission accompanied by improvement in quality of life and reduced long-term complications. Despite the importance of monitoring the GFD, there are no clear guidelines for assessing the outcome or for exploring its adherence. Available methods are insufficiently accurate to identify occasional gluten exposure that may cause intestinal mucosal damage. Serological tests are highly sensitive and specific for diagnosis, but do not predict recovery and are not useful for follow-up. The use of serial endoscopies, it is invasive and impractical for frequent monitoring, and dietary interview can be subjective. Therefore, the detection of gluten immunogenic peptides (GIP) in feces and urine have been proposed as new non-invasive biomarkers to detect gluten intake and verify GFD compliance in CD patients. These simple immunoassays in human samples could overcome some key unresolved scientific and clinical problems in CD management. It is a significant advance that opens up new possibilities for the clinicians to evaluate the CD treatment, GFD compliance, and improvement in the quality of life of CD patients.

- Abstract
We aimed to assess the perceived impact of the lockdown, imposed to control the spreading of COVID-19, on the adherence of Italian celiac disease (CD) subjects to the gluten-free diet by a web-based survey. A total of 1983 responses were analyzed, 1614 (81.4%) by CD adults and 369 (18.6%) by parents/caregivers of CD children/adolescents. The compliance with the GFD was unchanged for 69% of the adults and 70% of the children, and improved for 29% of both. The factors increasing the probability to report stricter compliance were the presence of CD symptoms in the last year before the lockdown (odds ratio (OR) 1.98, 95% confidence interval (CI) 1.46–2.26), a partial usual adherence to gluten-free diet (GFD) (OR 1.91, 95% CI 1.2–3.06), and having tried recipes with naturally gluten-free ingredients more than usual (OR 1.58, 95% CI 1.28–1.96) for adults; the presence of CD symptoms in the last year (OR 2.05, 95% CI 1.21–3.47), still positive CD antibodies (OR 1.89, 95% CI 1.14–3.13), and other family members with CD (OR 2.24, 95% CI 1.3–3.85) for children/adolescents. Therefore, the lockdown led to a reported improved adherence to the GFD in one-third of the respondents, in particular in those with previous worse disease control, offering the opportunity to avoid sources of contamination/transgression and increase the use of naturally gluten-free products. PurposeTreatment of coeliac disease (CD) is essentially dietary and requires permanent changes in dietary habits. Gluten‐free diet compliance affects every aspect of an individual's quality of life. This paper aims to analyse the difficulties associated with the food practices tried and reported by CD patients and their health and quality of life.Design/methodology/approachA questionnaire with open, closed and multiple choice questions adapted from an already validated instrument was administered to a sample of 105 CD patients. The inclusion criteria were patients diagnosed with CD living in Distrito Federal (DF) who agreed to participate in the study.FindingsThe greatest concerns of CD patients were eating out (44.23 per cent), having to read food labels (50.00 per cent), believing that foods are not safe (52.88 per cent) and not having dietary alternatives (56.44 per cent). Some (39.42 per cent) patients have no difficulty following the diet, 42.3 per cent have some difficulty and 18.27 per cent find it very difficult to follow the diet.Originality/valueThis study discloses some of the social difficulties faced by CD patients, such as eating with relatives and friends, and discusses the impact of these challenges on their quality of life. The data show that information can help CD patients to maintain their quality of life.

Triheptanoin-enriched diets have been successfully used in the experimental treatment of various metabolic disorders. Maximal therapeutic effect is achieved in the context of a ketogenic diet where triheptanoin oil provides 30–40% of the daily caloric intake. However, pre-clinical studies using triheptanoin-rich diets are hindered by the difficulty of administering to laboratory animals as a solid foodstuff. In the present study, we successfully synthesized triheptanoin to the highest standards of purity from glycerol and heptanoic acid, using sulfonated charcoal as a catalyst. Triheptanoin oil was then formulated as a solid, stable and palatable preparation using a ketogenic base and a combination of four commercially available formulation agents: hydrophilic fumed silica, hydrophobic fumed silica, microcrystalline cellulose, and talc. Diet compliance and safety was tested on C57Bl/6 mice over a 15-week period, comparing overall status and body weight change. Practical applications: This work provides a complete description of (i) an efficient and cost-effective synthesis of triheptanoin and (ii) its formulation as a solid, stable, and palatable ketogenic diet (triheptanoin-rich; 39% of the caloric intake) for rodents. Triheptanoin-rich diets will be helpful on pre-clinical experiments testing the therapeutic efficacy of triheptanoin in different rodent models of human diseases. In addition, using the same solidification procedure, other oils could be incorporated into rodent ketogenic diet to study their dosage and long-term effects on mammal health and development. This approach could be extremely valuable as ketogenic diet is widely used clinically for epilepsy treatment.

Screening of 1,042,000 Greek newborns for PKU using the Guthrie test, revealed a mean incidence 1:10,420 for Hyperphenylalaninaemia (1:24,233 classic PKU and 1:18,280 benign variants). The mean Phe concentration of the first sample (3-15 days) was 14.1 mg/dl (6-40) for PKU while, for variants, it was 7.7 mg/dl (4-16). Using 4 mg/dl as the limit under which no repeat test was requested, no case of PKU has been missed. On the whole, 100 newborns with persistent Hphe have been detected: 43 proved to be classical PKU and 57 benign variants. After excluding BH4 deficiency, Phe blood level has been kept below 10 mg/dl in all Hphe babies during the first 2 years of life, irrespective of the type of Hphe. All treated Hphe babies have developed normally. Discrimination between PKU and variants was achieved in the 6-18 months of life with protein loading tests. Treated PKU children had the lowest Phe concentrations during the first year of life (mean 5.1 mg/dl). From then on, a steady yearly increase was observed. Their mean IQ in the Stanford-Binet L-M dropped from 107.2 in the second year to 95.8 in the sixth year. A high negative correlation is not clearly shown, probably due to the small number of cases. PKU clinic (supportive and counselling group therapy) along with individual consultations have proved to be helpful, as has the promotion of diet compliance. The need to individualize each of Hphe before deciding upon ways of treatment is stressed and a protocol to be followed for each Hphe is proposed.

Currently, there is no universal agreement on galactosaemia screening, fundamentally because of the risk-benefit uncertainties. We conducted two exhaustive systematic searches in the main electronic databases (PubMed, Embase, Cochrane, etc.) to recover relevant information about the disease and screening test/s in order to support decision making in Spain. All of the 45 studies identified that covered disease issues were retrospective case series or cross-sectional analysis (level-4 evidence). Studies consistently found that the majority of patients presented characteristic symptomatology before diagnosis. Long term disabilities were not significantly correlated with age of diagnosis, onset of dietary restriction or strict diet compliance. The five studies that provided accuracy data used different cut-off points and verification tests, and thus differed in their definitions of a positive case (level-3b evidence). The estimated sensitivity was 100 % and the specificity 99.9 %. The false-positive rate ranged from 0.0005 % to 0.25 %, and the PPV from 0 % to 64.3 %. The comparative clinical effectiveness in relation to not screening or implementation of other programs is unknown. In summary, existing evidence remains insufficient to establish the appropriateness of newborn screening for galactosaemia screening, although health benefits could be expected if early diagnosis and treatment is achieved. If screening is implemented in Spain, it would be important that a pilot programme be implemented to assess false positive rate and ensure that early diagnosis is not delayed.

A gluten-free diet (GFD) is the safest treatment modality in patient with coeliac disease (CD) and other gluten-related disorders. Contamination and diet compliance are important factors behind persistent symptoms in patients with gluten related-disorders, in particular CD. How much gluten can be tolerated, how safe are the current gluten-free (GF) products, what are the benefits and side effects of GFD? Recent studies published in Nutrients on gluten-free products’ quality, availability, safety, as well as challenges related to a GFD are discussed.
