1. 3D printing of anatomical models for surgeons: an investigation on repeatability

Abstract

As part of the trend towards personalised medicine, surgeons are increasingly using 3D printed replicas for preoperative planning. This raises the question of how reliable these models are. This paper examines the repeatability of manufacturing human mandibles. Five polyamide replicas were produced using selective laser sintering and digitised using structured light scanning. Quantitative comparisons were made using Mimics Software. The differences were analysed graphically, using histograms and kernel density estimates. The mean differences ranged between + 0.0274 (SD 0.0671) mm and − 0.0284 (SD 0.0629) mm. The median of absolute differences was 0.0308 mm, i.e. 50% of absolute differences were smaller than 31 $$\upmu \hbox {m}$$ . For the 22,811,168 differences measured, all were between + 1.9836 and − 2.0526 mm. The proportion of absolute differences below 0.10 mm was between 82.09 and 98.84%, and between 94.43 and 99.90% when using a threshold of 0.20 mm. 99.95% of the absolute differences were below 1.00 mm. In conclusion: 3D printed models may not be identical, even when based on the same imaging study and patient; on the other hand, identical replicas can be obtained with a constant production chain; we recommend that four distinguishing criteria should be used in future investigations: qualitative and quantitative accuracy, repeatability and reproducibility.

Abstract

In the case presented herein, healing of the wound was uneventful. Normal sensation returned to the external ear within 3 to 4 months. The obvious residual adverse effect was the discrepancy in the size of the treated ear relative to the opposite ear (Fig 8). The result after 8 months indicated no recurrence of the lesion and the patient was asymptomatic (Figs 9, 10). The somewhat elevated tissue at the superior greater helix may have been prevented had the medial advancement flap been utilized. CNH is an uncommon but devastating lesion to those who are unfortunate enough to develop it. Conservative therapy using pressure-relieving techniques, topical cortisone, or intralesional cortisone injections should be attempted initially. Surgical management of the lesion should be considered when these measures fail to eliminate the condition. Simple excision of the involved helix, elevation of a skin flap, and removal of the underlying involved cartilage, or wedge resection and primary closure, may manage lesions smaller than 1.5 cm. With larger lesions, the Antia-Buch procedure provides an excellent means of ear reconstruction.

6. Potential adverse events of endosseous dental implants penetrating the maxillary sinus: long-term clinical evaluation.

Abstract

Objectives/hypothesis: The aim of this study was to evaluate the nature and incidence of long-term maxillary sinus adverse events related to endosseous implant placement with protrusion into the maxillary sinus., Study Design: Retrospective cohort study., Methods: All patients who underwent placement of endosseous dental implants with clinical evidence of implant penetration into the maxillary sinus with membrane perforation were included in this study. Only patients with a minimum follow-up of 5 years after implant placement were included in this study. Maxillary sinus assessment was both clinical and radiological., Results: Eighty-three implants with sinus membrane perforation in 70 patients met the study's inclusion criteria. Mean age was 65.96 years ± 14.23. Twelve patients had more than one implant penetrating the maxillary sinus, and seven of them had bilateral sinus perforation. Estimated implant penetration was ≤ 3 mm in all cases. The average clinical and radiological follow-up was 9.98 years ± 3.74 (range 60-243 months). At the follow-up appointments, there were no clinical or radiological signs of sinusitis in any patient., Conclusion: This long-term study, spreading over a period of up to 20 years, indicates that no sinus complication was observed following implant penetration into the maxillary sinus. Furthermore, absence of occurrence of such complications is related to the maintenance of successful osseointegration. A contrario, and in the presence of an acute or chronic maxillary sinusitis, the differential diagnosis must always consider other potential odontogenic and nonodontogenic etiologies., (Copyright © 2013 The American Laryngological, Rhinological and Otological Society, Inc.)

Abstract

In this study, we have found that dipeptidylpeptidase IV (DPPIV) plays in vivo an active role in the modulation of the inflammatory response of chronic rhinosinusitis. Human nasal mucosa expresses DPPIV-like immunoreactivity in submucosal seromucus glands, leukocytes, and endothelial cells of blood vessels. DPPIV enzymatic activity in nasal tissue biopsies taken from patients suffering from chronic rhinosinusitis was correlated inversely with the density of inflammatory cells in the nasal mucosa, and the DPPIV activity rose when chronic rhinosinusitis was treated. By using a pig animal model, we have shown that the intranasal administration of recombinant DPPIV decreased the vasodilatation induced by exogenous substance P (SP), a proinflammatory peptide released by sensory nerves. In contrast, an inhibitor of DPPIV enhanced the vasodilatatory effect at low doses of SP. SP5-11 was 100- to 1000-fold less potent than SP as a vasodilator of the nasal mucosa. The vasodilatatory effect of SP was abolished by a NK1 receptor antagonist. In conclusion, these results suggest a new pathophysiological pathway for rhinitis based on clinical observations in humans, indicating the involvement of an enzyme to modulate non-adrenergic and non-cholinergic substrate that occurred during nasal dysfunctions.
