Background: Schistosomiasis remains a public health problem, particularly in sub-Saharan Africa. The disease is intimately connected to poverty and environmental factors. Our research was readily embedded into a multi-country schistosomiasis oversampling study. The aim of the study presented here was to determine the prevalence of Schistosoma mansoni and to investigate the role of water body characteristics and water-related human activities in disease transmission., Methods: In August and September 2022, a cross-sectional study was conducted in the western part of Côte d'Ivoire. Stool and urine samples were collected from 1602 and 1729 children aged 5-14 years, respectively, in 65 villages in the health districts of Biankouma, Ouaninou and Touba. Additionally, data were collected from direct observation of water-related activities at water bodies and interviews conducted with community leaders and health workers. The prevalence and risk factors for Schistosoma infection were assessed using generalised estimating equation models., Results: The prevalence ofS. mansoni and S. haematobium were 27.4% (95% confidence interval [CI] 21.5-34.3%) and 0.1% (95% CI 0.03-0.5%), respectively. Low prevalence of soil-transmitted helminths was observed with 2.4%, 0.4% and 0.2% for hookworm, Trichuris trichiura and Ascaris lumbricoides, respectively. At the health district level, we found S. mansoni prevalence of 34.4% (95% CI 25.0-45.3%), 34.3% (95% CI 24.0-46.2%) and 16.3% (95% CI 9.5-26.6%) for Biankouma, Ouaninou and Touba, respectively. Female and male participants were at a similar risk of infection (29.0% vs. 26.0%, odds ratio [OR]: 1.18, 95% CI 0.92-1.50). Children aged 9-14 years showed a higher prevalence than their younger counterparts aged 5-8 years (34.5% vs. 22.7%, OR: 1.80, 95% CI 1.42-2.27). High infection prevalence was observed in villages where children were washing clothes and dishes at open surface water sites and pursued recreational activities (e.g. swimming and playing in the water). The temperature, total dissolved solids and pH of water samples showed no significant association with S. mansoni infection at the village unit., Conclusions: Human water-related activities such as washing clothes and playing in the water are risk factors for S. mansoni transmission. Hence, preventive chemotherapy should be combined with information, education and communication to avoid or reduce the frequency of water exposure in children as part of a comprehensive package of interventions towards elimination of schistosomiasis as a public health problem., (© 2024. The elimination of schistosomiasis as a public health problem by 2030 is one of the main goals put forth in the World Health Organization's roadmap for neglected tropical diseases. This study aimed to compare different sampling approaches to guide mapping and preventive chemotherapy. A cross-sectional parasitological survey was conducted from August to September 2022 in the health districts of Biankouma, Ouaninou, and Touba in the western part of Côte d'Ivoire. The prevalence and intensity of Schistosoma mansoni infection were assessed in children aged 5-14 years using three sampling approaches. The first approach involved a random selection of 50% of the villages in the health districts. The second approach involved a random selection of half of the villages selected in approach 1, thus constituting 25% of the villages in the health district. The third approach consisted of randomly selecting 15 villages from villages selected by approach 2 in each health district. The overall prevalence of S. mansoni was 23.5% (95% confidence interval (CI): 19.9-27.6%), 21.6% (95% CI: 17.1-26.8%), and 18.3% (95% CI: 11.9-27.1%) with the first, second, and third approach, respectively. The respective geometric mean S. mansoni infection intensity was 117.9 eggs per gram of stool (EPG) (95% CI: 109.3-127.3 EPG), 104.6 EPG (95% CI: 93.8-116.6 EPG), and 94.6 EPG (95% CI 79.5-112.7 EPG). We conclude that, although randomly sampling up to 50% of villages in a health district provides more precise population-based prevalence and intensity measures of S. mansoni , randomly selecting only 15 villages in a district characterized by low heterogeneity provides reasonable estimates and is less costly.

Background: Annual mass drug administration (MDA) using praziquantel is the cornerstone of schistosomiasis morbidity control but is not sufficient to interrupt transmission. We implemented a cluster-randomized trial to compare the effectiveness of 4 different intervention packages to interrupt transmission of Schistosoma haematobium in a seasonal transmission setting of Côte d'Ivoire., Methods: Sixty-four localities with a S. haematobium prevalence in school children aged 13-14 years above 4% were randomly assigned to 1 of 4 intervention arms over a 3-year period: (1) the current standard strategy consisting of annual MDA before peak of transmission, (2) annual MDA after peak of transmission, (3) biannual MDA, and (4) standard MDA combined with snail control. The primary outcome was prevalence and intensity of S. haematobium infection in children aged 9-12 years 1 year after the final intervention, using urine filtration performed by experienced microscopists., Results: By study end, we observed the lowest S. haematobium prevalence in the biannual MDA, compared to the standard treatment arm (0.6% vs 7.5%; odds ratio [OR] = 0.07, 95% confidence interval [CI] = .02 to .24). The prevalence in arms 2 and 4 was about 3.5%, which was not statistically significantly different from the standard strategy (both ORs 0.4, 95% CI = .1 to ~1.8). New cases of infection were still observed in all arms at study end., Conclusions: Biannual MDA was the only regimen that outperformed the standard treatment. All strategies resulted in decreased prevalence of infection; however, none of them was able to interrupt transmission of S. haematobium within a 3-year period., Clinical Trials Registration: ISRCTN10926858., (© The Author(s) 2021. Background: Control of intermediate host snails using molluscicides for the control and/or elimination of schistosomiasis is a strategy in line with WHO recommendations. Niclosamide is the main chemical molluscicide recommended by WHO. However, except the immediate killing of the snail, the extent of the impact of the molluscicide application on the evolution of snail life-history traits, in relation to recolonization of treated sites is not well known. This study aimed to characterize the spatial variation of life-history traits of populations of the freshwater snail Bulinus truncatus, in relation to niclosamide spraying in the field. From 2016 to 2018, we conducted a trial, using niclosamide to control the intermediate host snails for interrupting the seasonal transmission of urinary schistosomiasis in northern and central Côte d'Ivoire. Five villages (sites) were considered, including three test and two control villages. In the test villages, the molluscicide was sprayed in habitats harboring B. truncatus snails three times a year (November, February-March and June). We sampled six B. truncatus populations: two populations from the control villages without any treatment; one collected before treatment and three sampled 2-3 months after treatment of the site with niclosamide. The snail populations were monitored for several life-history traits, including survival, growth, fecundity and hatchability, under laboratory conditions, over one generation (G 1 ). We tested the population, region (North/Centre) and treatment status (treated/untreated) effects on the variation of the measured life-history traits and correlations between pairs of traits were estimated., Results: On the whole, the traits varied among populations. The risk of death was lower in northern populations compared to central ones. The age at first reproduction was reached earlier with a smaller size of snails in northern populations. Values of first reproduction features (size and fecundity) were lower in treated snail populations. The overall growth of untreated populations was higher than that of treated ones. The late fecundity and egg hatching were higher in northern than in central snails. At first reproduction, age was negatively correlated with some fecundity parameters. However, growth was positively associated with fecundity., Conclusions: Our study showed a spatial variation of life-history traits in B. truncatus snails. The mollusciciding seems to have led to the depression of some life-history traits in the snail populations. However, investigations should be carried out over several generations of snails to better clarify the impact of niclosamide on the evolution of the life-history traits., (© 2022. The Author(s).)

Schistosoma mansoni infection is common among school-age children (SAC) in western Côte d'Ivoire. Little is known on the infection rate of preschool-aged children (PSAC) due to epidemiological data deficiency and nonappropriate formulation of the drug. Thus, mass drug administration for schistosomiasis control mainly targets SAC. This study aims to identify the risk factors and spatial distribution of S. mansoni infection among PSAC in Blapleu, endemic foci of S. mansoni . We carried out a cross-sectional study in households with PSAC aged 1-6 years. A structured questionnaire was administered to mothers/guardians to obtain data on sociodemographics and water contact behaviour of children. Point-of-care circulating cathodic antigen (POC-CCA) immunodiagnostic test in urine and Kato-Katz (K-K) method with stool were used for S. mansoni infection diagnosis. Multiple logistic regression analysis was performed to determine the relationship between S. mansoni infection and sociodemographic data. Coordinates recorded by a Global Positioning System of households, water source points, and infected PSAC were used to map the spatial distribution of S. mansoni infection cases. This study was conducted with 350 PSAC aged 1-6 years. The overall infection prevalence of S. mansoni varies from 31.43% with the K-K method to 62.86% with the POC-CCA. PSAC aged 2-6 years were highly infected with S. mansoni than those aged 1-2 years (OR = 14.24, 95% CI: 5.85-34.64). PSAC who did not have access and who do not live close to the infected water source were at a significant lower risk of S. mansoni infection (OR = 0.13, 95% CI: 0.057-0.30). The main purpose of water contact of PSAC was to help their mother for laundry that occurs weekly. In Blapleu, a high risk of S. mansoni infection was observed among PSAC. Background: Preventive chemotherapy using praziquantel is the mainstay for schistosomiasis control. However, there is little evidence on what is supposed to be the most effective school-based treatment strategy to sustain morbidity control. The aim of this study was to compare differences in Schistosoma mansoni prevalence and infection intensity between three different schedules of school-based preventive chemotherapy in an area with moderate prevalence of S. mansoni in Côte d'Ivoire., Methodology: Seventy-five schools were randomly assigned to one of three intervention arms: (i) annual school-based preventive chemotherapy with praziquantel (40 mg/kg) over four years; (ii) praziquantel treatment only in the first two years, followed by two years whithout treatment; and (iii) praziquantel treatment in years 1 and 3 without treatment in-between. Cross-sectional parasitologic surveys were carried out prior to each round of preventive chemotherapy. The difference in S. mansoni prevalence and infection intensity was assessed by multiple Kato-Katz thick smears, among children aged 9-12 years at the time of each survey. First-grade children, aged 5-8 years who had never received praziquantel, were also tested at baseline and at the end of the study., Principal Findings: Overall, 7,410 children aged 9-12 years were examined at baseline and 7,223 at the final survey. The baseline prevalence of S. mansoni was 17.4%, 20.2%, and 25.2% in arms 1, 2, and 3, respectively. In the final year, we observed the lowest prevalence of 10.4% in arm 1, compared to 18.2% in arm 2 and 17.5% in arm 3. The comparison between arms 1 and 2 estimated an odds ratio (OR) of 0.52 but the difference was not statistically significant (95% confidence interval (CI) = 0.23-1.16). Likewise the difference between arms 1 and 3 lacked statistical significance (OR = 0.55, 95% CI = 0.23-1.29). There was no noteworthy difference observed between arms 2 and 3 (OR = 1.06, 95% CI = 0.64-1.75). The lowest S. mansoni fecal egg counts in the final year survey were observed in arm 1 (7.9 eggs per gram of stool (EPG)). However, compared with 11.5 EPG in arm 2 and 15.4 EPG in arm 3, the difference lacked statistical significance. There were 4,812 first-grade children examined at baseline and 4,513 in the final survey. The overall prevalence of S. mansoni in these children slightly decreased in arms 1 (from 4.5% to 3.6%) and 2 (from 4.7% to 4.3%), but increased in arm 3 (from 6.8% to 7.9%). However, there was no significant difference in prevalence and infection intensity observed between study arms., Conclusions/significance: The three treatment schedules investigated led to a reduction in the prevalence and intensity of S. mansoni infection among children aged 9-12 years. Comparing intervention arms at the end of the study, no statistically significant differences were observed between annual treatement and the other two treatment schedules, neither in reduction of prevalence nor intensity of infection. It is important to combine our results with those of three sister trials conducted simultaneously in other African countries, before final recommendations can be drawn., Competing Interests: The authors have declared that no competing interests exist.

In order to assess the impact of different control strategies against Schistosoma haematobium in seasonal transmission foci in Côte d'Ivoire, a three-year cluster randomized trial was implemented. The decrease in S. haematobium prevalence among children aged 9-12 years was the primary outcome. In the first step, an eligibility survey was conducted, subjecting 50 children aged 13-14 years to a single urine filtration. Sixty-four villages with a prevalence of S. haematobium of ≥4% were selected and randomly assigned to four intervention arms consisting of annual mass drug administration (MDA) before (arm 1) and after (arm 2) the peak transmission, biannual treatment with praziquantel before and after the peak transmission season (arm 3), and annual MDA before the peak transmission season, coupled with focal chemical snail control using molluscicides (arm 4). At baseline, we observed a prevalence of 24.8%, 10.1%, 13.9%, and 15.9% in study arms 1, 2, 3, and 4, respectively. One year after the first intervention, the prevalence decreased in all study arms by about two-thirds or more. The prevalence in arm 2 was lower than in arm 1 (3.5% vs. 8.1%), but the difference was not statistically significant (odds ratio (OR) = 0.42, 95% confidence interval (CI) = 0.10-1.80). After adjusting for baseline prevalence, arms 1 and 2 performed roughly similarly (adjusted odds ratio (aOR) = 1.03, 95% CI = 0.34-3.07). The prevalence in arms 3 and 4 (1.9% and 2.2%) were significantly lower compared to arm 1 in the unadjusted and the adjusted models (arm 3 vs. arm 1, OR = 0.22, 95% CI = 0.05-0.95, aOR = 0.19, 95% CI = 0.08-0.48; arm 4 vs. arm 1, OR = 0.26, 95% CI = 0.08-0.85, aOR = 0.23, 95% CI = 0.06-0.87). The initial intervention showed a significant impact on the prevalence of S. haematobium . It will be interesting to monitor the comparative impact of the different intervention arms and to determine whether the interruption of seasonal transmission of S. haematobium can be achieved in this epidemiological setting within three years.

As part of its diverse portfolio, the Schistosomiasis Consortium for Operational Research and Evaluation (SCORE) included two cluster-randomized trials evaluating interventions that could potentially lead to interruption of schistosomiasis transmission (elimination) in areas of Africa with low prevalence and intensity of infection. These studies, conducted in Zanzibar and Côte d'Ivoire, demonstrated that multiyear mass drug administration (MDA) with praziquantel failed to interrupt the transmission of urogenital schistosomiasis, even when provided biannually and/or supplemented by small-scale implementation of additional interventions. Other SCORE activities related to elimination included a feasibility and acceptability assessment of test-treat-track-test-treat (T5) strategies and mathematical modeling. Future evaluations of interventions to eliminate schistosomiasis should recognize the difficulties inherent in conducting randomized controlled trials on elimination and in measuring small changes where baseline prevalence is low. Highly sensitive and specific diagnostic tests for use in very low-prevalence areas for schistosomiasis are not routinely available, which complicates accurate measurement of infection rates and assessment of changes resulting from interventions in these settings. Although not encountered in these two studies, as prevalence and intensity decrease, political and community commitment to population-wide MDA may decrease. Because of this potential problem, SCORE developed and funded the T5 strategy implemented in Egypt, Kenya, and Tanzania. It is likely that focal MDA campaigns, along with more targeted approaches, including a T5 strategy and snail control, will need to be supplemented with the provision of clean water and sanitation and behavior change communications to achieve interruption of schistosome transmission.

The Schistosomiasis Consortium for Operational Research and Evaluation (SCORE) was created in 2008 to answer questions of importance to program managers working to reduce the burden of schistosomiasis in Africa. In the past, intermediate host snail monitoring and control was an important part of integrated schistosomiasis control. However, in Africa, efforts to control snails have declined dramatically over the last 30 years. A resurgence of interest in the control of snails has been prompted by the realization, backed by a World Health Assembly resolution (WHA65.21), that mass drug administration alone may be insufficient to achieve schistosomiasis elimination. SCORE has supported work on snail identification and mapping and investigated how xenomonitoring techniques can aid in the identification of infected snails and thereby identify potential transmission areas. Focal mollusciciding with niclosamide was undertaken in Zanzibar and Côte d'Ivoire as a part of elimination studies. Two studies involving biological control of snails were conducted: one explored the association of freshwater riverine prawns and snail hosts in Côte d'Ivoire and the other assessed the current distribution of Procambarus clarkii , the invasive Louisiana red swamp crayfish, in Kenya and its association with snail hosts and schistosomiasis transmission. SCORE also supported modeling studies on the importance of snail control in achieving elimination and a meta-analysis of the impact of molluscicide-based snail control programs on human schistosomiasis prevalence and incidence. SCORE's snail control studies contributed to increased investment in building capacity, and specimens collected during SCORE research deposited in the Schistosomiasis Collections at the Natural History Museum (SCAN) will provide a valuable resource for the years to come.

Background: Accurate identification of schistosome species infecting intermediate host snails is important for understanding parasite transmission, schistosomiasis control and elimination. Cercariae emerging from infected snails cannot be precisely identified morphologically to the species level. We used molecular tools to clarify the distribution of the Schistosoma haematobium group species infecting bulinid snails in a large part of Côte d'Ivoire and confirmed the presence of interspecific hybrid schistosomes., Methods: Between June 2016 and March 2017, Bulinus snails were sampled in 164 human-water contact sites from 22 villages of the northern and central parts of Côte d'Ivoire. Multi-locus genetic analysis (mitochondrial cox1 and nuclear ITS) was performed on individual schistosome cercariae shed from snails, in the morning and in the afternoon, for species and hybrid identification., Results: Overall, 1923 Bulinus truncatus, 255 Bulinus globosus and 1424 Bulinus forskalii were obtained. Among 2417 Bulinus screened, 25 specimens (18 B. truncatus and seven B. globosus) shed schistosomes, with up to 14% infection prevalence per site and time point. Globally, infection rates per time point ranged between 0.6 and 4%. Schistosoma bovis, S. haematobium and S. bovis × S. haematobium hybrids infected 0.5%, 0.2% and 0.4% of the snails screened, respectively. Schistosoma bovis and hybrids were more prevalent in B. truncatus, whereas S. haematobium and hybrid infections were more prevalent in B. globosus. Schistosoma bovis-infected Bulinus were predominantly found in northern sites, while S. haematobium and hybrid infected snails were mainly found in central parts of Côte d'Ivoire., Conclusions: The data highlight the necessity of using molecular tools to identify and understand which schistosome species are transmitted by specific intermediate host snails. The study deepens our understanding of the epidemiology and transmission dynamics of S. haematobium and S. bovis in Côte d'Ivoire and provides the first conclusive evidence for the transmission of S. haematobium × S. bovis hybrids in this West African country. The current emphasis of schistosomiasis control is placed on preventive chemotherapy using praziquantel. However, reinfection may occur rapidly in the absence of complementary interventions. Recent studies from Senegal suggest that predatory prawns might feed on intermediate host snails and thus impact on schistosomiasis transmission. We designed a study with four repeated cross-sectional surveys pertaining to prawns and snails, coupled with a single cross-sectional parasitological survey among humans. We assessed for potential associations between the presence/density of prawns and snails and correlation with Schistosoma infection in a composite sample of school-aged children and adults. The study was carried out between October 2015 and December 2016 in 24 villages located near the Agnéby and Mé coastal river systems in south-eastern Côte d'Ivoire. At each site, snails and prawns were collected, and in each village, 150 individuals were subjected to stool and urine examination for the diagnosis of Schistosoma mansoni and S. haematobium. We found peaks of relative abundance of intermediate host snails in the villages of the Agnéby River system, while predatory prawns were predominantly recorded in the Mé River system. A negative association was observed between intermediate host snail densities and riverine prawns; however, no pattern was found between this trend in the predator-prey relationship and the prevalence of human schistosomiasis.

Background: Parasitic worms (helminths) are common infections in low- and middle-income countries. For most helminth species, school-aged children are at highest risk of infection and morbidity, such as impaired cognitive and physical development. Preventive chemotherapy is the current mainstay for helminthiases control. Sanitation improvement and hygiene-related education are important complementary strategies, which act by altering children's behaviour. However, little is known about the effect of improved knowledge on the risk of helminth infection. The aim of this study was to assess the potential influence of knowledge that children acquired at home or in school, without any specific health education intervention, on helminth infections., Methods: In May 2014, we conducted a cross-sectional survey in western Côte d'Ivoire. A total of 2498 children, aged 9-12 years, were subjected to three consecutive stool examinations using duplicate Kato-Katz thick smears to determine infections with soil-transmitted helminths and Schistosoma mansoni. Additionally, children were interviewed to assess their knowledge about helminth infections. Four knowledge scores were constructed by factor analysis; one, reflecting general knowledge about helminths and three manifesting helminth species-specific knowledge. The effect of general and specific knowledge on children's helminth infection status was determined using meta-analysis., Results: Children who scored high in the hookworm-specific knowledge were less likely to be infected with hookworm but no association was found for the other helminth species. Moreover, greater general knowledge was not associated with lower odds of being infected with any helminth species. Most of the children interviewed believed that the effect of preventive chemotherapy is permanent, and hence, re-treatment is not necessary., Conclusions: Specific knowledge about different types of helminths might not suffice to induce behavioural change which in turn reduces infection and reinfection with helminths. Health education interventions should strive to strengthen the perception of risk and to clarify the true benefit of preventive chemotherapy.

Background: To achieve a world free of schistosomiasis, the objective is to scale up control and elimination efforts in all endemic countries. Where interruption of transmission is considered feasible, countries are encouraged to implement a comprehensive intervention package, including preventive chemotherapy, information, education and communication (IEC), water, sanitation and hygiene (WASH), and snail control. In northern and central Côte d'Ivoire, transmission of Schistosoma haematobium is seasonal and elimination might be achieved. In a cluster-randomised trial, we will assess different treatment schemes to interrupt S. haematobium transmission and control soil-transmitted helminthiasis over a 3-year period. We will compare the impact of (i) arm A: annual mass drug administration (MDA) with praziquantel and albendazole before the peak schistosomiasis transmission season; (ii) arm B: annual MDA after the peak schistosomiasis transmission season; (iii) arm C: two yearly treatments before and after peak schistosomiasis transmission; and (iv) arm D: annual MDA before peak schistosomiasis transmission, coupled with chemical snail control using niclosamide., Methods/design: The prevalence and intensity of S. haematobium and soil-transmitted helminth infections will be assessed using urine filtration and Kato-Katz thick smears, respectively, in six administrative regions in northern and central parts of Côte d'Ivoire. Once a year, urine and stool samples will be collected and examined from 50 children aged 5-8 years, 100 children aged 9-12 years and 50 adults aged 20-55 years in each of 60 selected villages. Changes in S. haematobium and soil-transmitted helminth prevalence and intensity will be assessed between years and stratified by intervention arm. In the 15 villages randomly assigned to intervention arm D, intermediate host snails will be collected three times per year, before niclosamide is applied to the selected freshwater bodies. The snail abundance and infection rates over time will allow drawing inference on the force of transmission., Discussion: This cluster-randomised intervention trial will elucidate whether in an area with seasonal transmission, the four different treatment schemes can interrupt S. haematobium transmission and control soil-transmitted helminthiasis. Background: The construction and operation of small multipurpose dams in Africa have a history of altering the transmission of water-based diseases, including schistosomiasis. The current study was designed to investigate the abundance and dynamics of schistosomiasis intermediate host snails and Schistosoma infections in humans during the construction and the first years of operation of a small multipurpose dam in Côte d'Ivoire., Methods: The study was carried out in Raffierkro and four neighbouring villages in central Côte d'Ivoire between 2007 and 2012. Snails were collected by two experienced investigators using scoops and forceps for 15 min at each site. Snails were identified at genera and, whenever possible, species level, and subjected to testing for cercarial shedding. Schoolchildren aged 6-15 years were examined once every year for Schistosoma haematobium and S. mansoni infection, using urine filtration and duplication Kato-Katz thick smears, respectively. Additionally, 551 adults were examined for Schistosoma infection before (June 2007) and 359 individuals 2 years after dam construction (June 2009)., Results: Overall, 1 700 snails belonging to nine different genera were collected from 19 sampling sites. Bulinus (potential intermediate host snails of S. haematobium) and Pila were the most common genera, whereas Biomphalaria (potential intermediate host snail of S. mansoni), Lymnaea, Physa and Melanoides were found in two villages. During the first-year sampling period, 65 snails were collected, of which 13 (20%) were schistosomiasis intermediate hosts. In subsequent years, out of 1 635 snails collected, 1 079 (66%) were identified as potential intermediate host for schistosomiasis, but none were shedding cercariae. The prevalence of S. mansoni among adults in the study area was low (0.4% in 2007 and 0.3% in 2009), whereas the prevalence of S. haematobium declined from 13.9% to 2.9% in this two-year period., Conclusions: The low prevalence of schistosomiasis in humans and the absence of infected intermediate host snails during the construction and early phase of operation of a small multipurpose dam suggest that there was no or only very little local transmission. However, the considerable increase in the number of intermediate host snails and their dispersion in irrigation canals call for rigorous surveillance, so that adequate public health measures can be taken in case of early signs of an outbreak.

Background: A deeper understanding of the ecology and small-scale heterogeneity of malaria transmission is essential for the design of effective prevention, control and elimination interventions. The spatial and temporal distribution of malaria vectors was investigated in five villages in close proximity to a hydro-agricultural system in Côte d'Ivoire over the course of construction and the early phase of irrigated rice farming., Methods: The study was carried out in five villages (Raffierkro, N'Douakro, Ahougui, Kpokahankro, Koffikro) near Bouaké, central Côte d'Ivoire, between early 2007 and late 2009. In each village, mosquitoes were collected by human landing catches and identified morphologically at genus and species level, and entomological parameters were determined. Plasmodium infection was assessed by dissection and an enzyme-linked immunosorbent assay., Results: A total of 19,404 mosquitoes belonging to the genus Anopheles were sampled during 328 human-night catches. Before the construction of the hydro-agricultural system, comparable densities of Anopheles gambiae were observed in all villages. In subsequent years, densities in Raffierkro and Ahougui were significantly higher than the other villages [Kruskal-Wallis (KW) test = 31.13, p < 0.001]. The density of Anopheles funestus in the five villages was comparable in the early stage of the project, while a high density was reported in Koffikro at the end (KW test = 11.91, p = 0.018). Transmission of Plasmodium falciparum is perennial in the study area. Over the course of the study, high entomological inoculation rates (EIRs) were found: 219-328 infectious bites per person per year with An. gambiae. For An. funestus considerably lower EIRs were observed (5.7-39.4). Changing patterns of An. gambiae were not correlated with malaria transmission., Conclusion: In this study setting, located in the bioclimatic transition zone of Côte d'Ivoire, rice cultivation was not observed to increase malaria transmission. The entomological parameters recorded until the onset of rice-growing activities in a hydro-agricultural system presented considerable heterogeneity both in space and time; a strong increase of Anopheles mosquitoes was observed in two of the five villages located in close proximity to the dam and irrigated rice fields. Malaria still is a main public health problem in all villages that require adequate control measures.

Abstract From June 2007 to June 2008, entomological, malacological and parasitological investigations were carried out in five villages in close proximity to a small dam of Raf-fierkro in Bouaké, central Côte-d'Ivoire. The objective of the study was to identify vectors and intermediate host snails of parasitic diseases related to water, and to assess the prevalence of malaria, schistosomiasis and soil-transmitted helminthiasis before dam construction. Mosquitoes were caught by human landing catches, snails were collected in water bodies, and microscopic analyses of blood, stool and urine samples were carried out. Two malaria vector species were identified:Anopheles gambiae and Anopheles funestus. The average infective rate of An. gambiae ranged between 3.1 and 4.5 %. The infective rate of An. funestus was 1 % in all sites. The entomological inoculation rate ranged between 343.1 and 427.1 infective bites per person per year (ib/p/y)for An. gambiae and between 14.6 and 40.1 ib/p/y for An. funestus. Three species of were found: Plasmodium falciparum (predominant species), Plasmodium ovale and Plasmodium malariae. There were no intermediate host snails of schistosomiasis with the exception of Biomphalaria pfeifferi,intermediate host snail of Schistosoma mansoni in Ahougui. The prevalence of urinary and intestinal schistosomiasis and soil-transmitted helminthiasis was low. Our studies revealed important transmission of malaria, with the presence of three plasmodial species, whereas schistosomiasis and soil-transmitted helminthiasis were present, but only at low frequencies.
