This study does not propose to elucidate how adiponectin secretion is regulated, but how its adiponectin concentration is an indicator of heart disease. About adiponectin, it is not known whether it is functionally an enzyme, or very likely a cytokine/chemokine/hormone, secreted by fat cells/adipocytes in the abdomen. Abdominal fat secretes 67 hormones, and all of which cause disease. For example, adiponectin generates diabetes and ischaemic heart disease via dyslipidemia. Based on clinical symptoms, electrocardiographic and echocardiographic parameters, a group of 208 patients with diastolic cardiac dysfunction with or without preserved systolic function, developed on a background of painful chronic ischaemic heart disease, stable angina on exertion, was constituted. The serum levels of adiponectin, total cholesterol, LDL cholesterol, HDL cholesterol and triglycerides were measured. Using the identified values, it was appreciated whether adiponectin correlates with the type of any of the two conditions, so that it can be recognised as a diagnostic and risk stratification marker. The myeloperoxidase levels of 208 patients with ischemic heart disease hospitalized for congestive heart failure were tested at admission, and discharge after 14 days. The calculations for the serum average concentration/lot/date, were made, subsequent, using the values of the tested parameters regarding MPO (admission and discharge) the mean concentration/lot was made, regardless of time of determination. The calculated average values analyses of the three MPO serum parameters were constantly elevated and permanently matched the myocardial contractility deficiency class. The integrated data of the study, allows us to propose the MPO’s circulating concentration as a diagnosis and risk assessment biomarker in the etiology of myocardial ischemia for chronic cardiac failure. In a group of 208 patients with chronic ischaemic heart disease, the variation of A2-associated-LDL phosphatase (Lp-PLA2) serum concentration values was analysed in dynamics at a two-week interval. The conclusion of the study is that the values of serum concentration of Lp-PLA2 can be accepted as a biomarker with diagnostic specificity for chronic ischaemic heart disease, a parameter of real utility in medical practice, both in situations where the patient, although clinically reporting the existence of angina pectoris, does not show specific changes on an EKG, and for the assessment of the response to personalised therapy. Lab tests revealed positive inflammatory tests and coagulopathy. Bilateral giant hemorrhagic Baker's cyst was diagnosed by Doppler ultrasonography, CT and MRI. The positive diagnosis of rheumatoid arthritis with acute onset, causing severe inflammation of synovial knee joints, giant hemorrhagic cysts and disseminated intravascular coagulation was challenging. Spectroscopy results of aluminophosphate glasses doped with transition metal (TM) ions (Cr and/or Mn) are reported. Li2O-BaO-Al203-La2O3-P205 (CrO3, MnO2), Li2O-BaO-Al2O3 La2O3-P2O5(CrO3-Mn02) and Li20-Na20-BaO-Al2O3- La2O3-P205 (CrO3-MnO2) glasses have been prepared. Ultraviolet-vis, Raman and photoluminescence spectroscopies have been used to investigate the structural and optical properties of the doped glasses. Optical transmission in the ultraviolet-vis range was used to study the oxidation states of the TM ions and the redox equilibria of the transition ion pairs in the aluminophosphate glasses as well as their coordination with oxygen ion ligands (02-). Aluminophosphate glasses doped with Mn-Cr ions show absorption peaks at 650 and 680 nm, which are characteristic of Cr3+ ions. The absorption peak at about 450 nm reveals the presence of both Cr3+ and Mn2+ ions. Raman peaks at about 700 and 1200 cm-1, obtained by 325 and 514.5 nm laser excitation, were assigned to the P-O-P symmetrical stretching mode and to the P02 symmetrical stretching mode, respectively. The fluorescence peak obtained by 514.5 nm laser excitation is localised at about 600 nm for Mn-doped samples and is due to Mn2+ ions in octahedral coordination. Glass samples containing Cr and Cr-Mn ion pairs exhibit fluorescence emission at about 800 nm due to Cr3+ ions in octahedral and tetrahedral symmetry; no Mn fluorescence was seen in these glasses. The effect of the mixed alkali ions on the spectra of these glass doped with transition ion pairs was also investigated. An increase in the sodium/lithium content results in a nonlinear shift of the ultraviolet transmission edge and improves the jquorescence intensity of the Cr-Mn-doped glass samples. Introduction: Hemophilia is a hereditary X-linked coagulopathy characterized by a deficiency of FVIII (hemophilia A) or FIX (hemophilia B) that causes hemorrhage depending on the severity of the disease; in cases of severe forms, spontaneous hemorrhage may be life threatening. Localized severe bleeding takes place at the different sites such as articular (ankle, knee, elbow), muscular (iliopsoas, thigh, forearm) and mucous membranes (gums, tongue, rhinorrhea, genitourinary tract). Almost all of the patients with hemophilia (PwH) have a form of arthropathy, most often disabling and deforming, with important static, balance, gripping and walking disorders , marked pain and a significant reduction in quality of life. Material and method: The study includes 85 patients with hemophilia (PwH) from all over the country, patients taking part constantly and periodically in our rehabilitation programs. From June to August 2017, the patients benefited from a rehabilitation treatment along with specific medical education lessons. The patients are between 3 and 50 years of age; 66 patients with hemophilia A, 12 patients with hemophilia B and 7 von Willebrand disease patients. The clinical examination includes goniometric measurements, the number of joints with hemarthrosis/ arthropathy, number of target joints (> 4 joint bleeding in 6 months), HJHS (Hemophilia Joint Health Score), Functional Independence Score in Hemophilia (FISH) score and quality of life evaluation (EQ-5D-a, EQ-5D-Y with EQ-VAS). The patients took part in a complex medical rehabilitation program, under the substitution therapy coordinated by the hematologist, a program consisting of individual and group kinesiotherapy, hydrotherapy, hydro-kinesiotherapy, electrotherapy and massage, the duration of the treatment being between 10 and 20 days. The medical rehabilitation treatment has been associated with psychological counseling sessions and medical education lessons. Results: Following the rehabilitation program, we found a significant improvement in pain relief, increased joint mobility and improved quality of life. We also evaluated musculoskeletal status in these patients, suggesting both the severity and gravity of musculoskeletal impairment and the degree of disability, especially in patients over 18 years of age. Conclusion: Hemophiliac arthropathy is a severe form of joint disease, in most cases invalidating, which significantly reduces the quality of life of these patients. It is important to emphasize the vital role of rehabilitation treatment that PwH should follow, treatment that relieves pain, improves musculoskeletal function, prepares the patient for future arthroplasty, or recovers the joint/limb function after arthroplasty. Rehabilitation in hemophilia begins with diagnosis and continues throughout life, constantly and sustained, coordinated by a multidisciplinary team composed of a hematologist, rehabilitation physician, physical therapist, masseur and psychologist. Over the last few years, prenatal diagnosis of congenital heart diseases experienced an important development. Overall survival of patients with congenital heart disease was improved in the last decade due to the advanced diagnostic methods and progression of cardiovascular surgical procedures in this field. Due to the significant increase of the long-term survival rate, currently, in most developed countries, there are departments dedicated to monitoring the adults with congenital heart malformation due to the particularities and special needs of this type of patients, compared with the regular cardiac pathology of adults. This article proposes a review of existing data at this moment related to the prenatal and postnatal diagnosis and their impact on the evolution regarding the most common cyanotic congenital malformation - Tetralogy of Fallot. In the following pages, we is emphasize the importance of knowing the precise fetal pathology for early determination of medical behavior in the postnatal period for the best care of these children. Great artery transposition, one of the most frequent and severe cyanotic heart malformations, represented the subject of many studies and research up to this moment. Although postnatal period is critical in this pathology, with correct management patients can benefit from complete surgical correction - arterial switch operation with good long term evolution. Prenatal diagnosis of Great Artery Transposition has an important contribution in choosing the appropriate treatment at the right time in postnatal period so that specific complications resulting from delaying the initiation of specific therapies can be avoided. This article proposes a review of existing data at this moment related to the importance of prenatal diagnosis in Great Artery Transposition and underlines how an accurate fetal diagnosis influences the decision of establishing the appropriate treatment in postnatal life for the children with this type of malformation and the complications that can be avoided. Infective endocarditis is rare in children and is rarer on a normal structural heart in an infant without any surgical intervention. Most cases are related to a pre-existing congenital lesion, the most frequent etiology are Gram-positive cocci and the most feared are fungal agents. This report presents a 7-monthold infant with fungal endocarditis on a normal structural heart. The diagnosis was suspected on clinical examination and was confirmed by echocardiography and positive blood cultures. His particular clinical evolution after medical and surgical treatment illustrates a severe disease with poor prognosis which may be a complication of neonatal intensive care procedures.
