Abstract

In June 2021, an unprecedented extreme heatwave impacted the Pacific Northwest of North America, resulting in more than 1000 excess deaths and affecting infrastructure and wildlife. Predicting such extreme events is key for preparedness and early action, and beyond temperature, it is important to consider biometeorological forecasts, accounting for the effects of the environment on the human body. The performance of ECMWF's (the European Centre for Medium-Range Weather Forecasts) temperature and heat stress (Humidex and the Universal Thermal Climate Index) forecasts during this heatwave is explored, and highlights that several days in advance, an event surpassing the maximum of climatology was predicted with extremely high confidence (100% of ensemble members) – successfully predicting the unprecedented.

Abstract

BACKGROUND: In the past decades, climate change has been impacting human lives and health via extreme weather and climate events and alterations in labour capacity, food security, and the prevalence and geographical distribution of infectious diseases across the globe. Climate change and health indicators (CCHIs) are workable tools designed to capture the complex set of interdependent interactions through which climate change is affecting human health. Since 2015, a novel sub-set of CCHIs, focusing on climate change impacts, exposures, and vulnerability indicators (CCIEVIs) has been developed, refined, and integrated by Working Group 1 of the "Lancet Countdown: Tracking Progress on Health and Climate Change", an international collaboration across disciplines that include climate, geography, epidemiology, occupation health, and economics. DISCUSSION: This research in practice article is a reflective narrative documenting how we have developed CCIEVIs as a discrete set of quantifiable indicators that are updated annually to provide the most recent picture of climate change's impacts on human health. In our experience, the main challenge was to define globally relevant indicators that also have local relevance and as such can support decision making across multiple spatial scales. We found a hazard, exposure, and vulnerability framework to be effective in this regard. We here describe how we used such a framework to define CCIEVIs based on both data availability and the indicators' relevance to climate change and human health. We also report on how CCIEVIs have been improved and added to, detailing the underlying data and methods, and in doing so provide the defining quality criteria for Lancet Countdown CCIEVIs. CONCLUSIONS: Our experience shows that CCIEVIs can effectively contribute to a world-wide monitoring system that aims to track, communicate, and harness evidence on climate-induced health impacts towards effective intervention strategies. Abstract

Air temperature has been the most commonly used exposure metric in assessing relationships between thermal stress and mortality. Lack of the high-quality meteorological station data necessary to adequately characterize the thermal environment has been one of the main limitations for the use of more complex thermal indices. Global climate reanalyses may provide an ideal platform to overcome this limitation and define complex heat and cold stress conditions anywhere in the world. In this study, we explored the potential of the Universal Thermal Climate Index (UTCI) based on ERA5 — the latest global climate reanalysis from the European Centre for Medium-Range Weather Forecasts (ECMWF) — as a health-related tool. Employing a novel ERA5-based thermal comfort dataset ERA5-HEAT, we investigated the relationships between the UTCI and daily mortality data in 21 cities across 9 European countries. We used distributed lag nonlinear models to assess exposure-response relationships between mortality and thermal conditions in individual cities. We then employed meta-regression models to pool the results for each city into four groups according to climate zone. To evaluate the performance of ERA5-based UTCI, we compared its effects on mortality with those for the station-based UTCI data. In order to assess the additional effect of the UTCI, the performance of ERA5-and station-based air temperature (T) was evaluated. Whilst generally similar heat- and cold-effects were observed for the ERA5-and station-based data in most locations, the important role of wind in the UTCI appeared in the results. The largest difference between any two datasets was found in the Southern European group of cities, where the relative risk of mortality at the 1st percentile of daily mean temperature distribution (1.29 and 1.30 according to the ERA5 vs station data, respectively) considerably exceeded the one for the daily mean UTCI (1.19 vs 1.22). Abstract

The Lancet Countdown is an international collaboration established to provide an independent, global monitoring system dedicated to tracking the emerging health profile of the changing climate. The 2020 report presents 43 indicators across five sections: climate change impacts, exposures, and vulnerabilities; adaptation, planning, and resilience for health; mitigation actions and health co-benefits; economics and finance; and public and political engagement. This report represents the findings and consensus of the 35 leading academic institutions and UN agencies that make up The Lancet Countdown, and draws on the expertise of climate scientists, geographers, engineers, experts in energy, food, and transport, economists, social, and political scientists, data scientists, public health professionals, and doctors.

14. Verification of heat stress thresholds for a health-based heatwave definition

Abstract

Heatwaves represent a threat to human health and excess mortality is one of the associated negative effects. A health-based definition for heatwaves is therefore relevant, especially for early warning purposes, and it is here investigated via the Universal Thermal Climate Index (UTCI). The UTCI is a bioclimate index elaborated via an advanced model of human thermo-regulation that estimates the thermal stress induced by air temperature, wind speed, moisture and radiation on the human physiology. Using France as a testbed, the UTCI was computed from meteorological reanalysis data to assess the thermal stress conditions associated to heat-attributable excess mortality in five cities. UTCI values at different climatological percentiles were defined and evaluated in their ability to identify periods of excess mortality (PEMs) over 24 years. Using verification metrics such as the probability of detection (POD), the false alarm ratio (FAR) and the frequency bias (FB), daily minimum and maximum heat stress levels equal or above corresponding UTCI 95th percentiles (15±2°C and 34.5±1.5°C, respectively) for 3 consecutive days are demonstrated to correlate to PEMs with the highest sensitivity and specificity (0.69 ≤ POD ≤ 1, 0.19 ≤ FAR ≤ 0.46, 1 ≤ FB ≤ 1.48) than minimum, maximum and mean heat stress level singularly and other bioclimatological percentiles. This finding confirms the detrimental effect of prolonged, unusually high heat stress at day and night time and suggests the UTCI 95th percentile as a health meaningful threshold for a potential heat health watch warning system.

Abstract

Assistive technology is playing a crucial role in supporting caregivers and patients with neurological disorders to carry out home care services. Nevertheless, the technological advances allowing for the automation of such services often lead to systems composed of several devices that make patients not always comfortable when interacting with them, so limiting their effective use. In order to improve the acceptance level of an assistive system, a robotic system is proposed to act as a smart sensor whose functionalities to control its behavior as well as to provide meaningful observations from the input data are modeled as services. The system is supported by a middleware able to automatically schedule a set of home care services that are personalized for each patient considering the personal daily routine, the cognitive status, and the personality profile. In addition, the proposed system is able to react to dynamic changes in the patient's state by modifying the robot behavior and adapting on the fly the set of proposed home care services.

Abstract

The provision of Service-Based Applications (SBAs) will be driven by market-oriented mechanisms, and the market value of an application will depend not only on its functionality, but also on the value of "Quality of Service" (QoS) parameters affecting its performance. These parameters are not static properties since they may vary according to the provision strategies of providers as well as the demand of users having their own preferences on the application QoS values. In this paper we propose a market-based negotiation mechanism among service providers and a user requesting a QoS-aware SBA. It allows to take into account the variability of service QoS attribute values typical of the future market of services, as well as to dynamically set the length of the negotiation process that is usually very time-consuming especially in the context of SBAs.

22. A Continuation Based Middleware to Experiment with Economy Based Service Scheduling

Abstract

Service-oriented computing is posing new challenges in the management of computational resources. They are considered as a network-available commodity to perfom some tasks and their provision should account both for user- and provider-dependent Quality-of-Service (QoS) requirements. In this scenario, the possibility to manage the execution of services is advisable to be able to control their provision, so that providers can make different decisions on the QoS they provide their services with according to the requirements of new service requests. In this work we present a middleware architecture to implement service providers featuring flexible control of services by explicitly controlling the suspension and resuming of service execution. The middleware uses program "continuations" as the basic mechanism to control service execution at programming level rather than using operating system facilities. In order to test the proposed middleware we developed a service provider implementing a budget-based resource-sharing scheduling algorithm. Different tests show that the provider is able to meet QoS requirements in terms of cost associated to a service request, still assuring the possibility to run all the received requests

In the context of Internet of Services (IoS), Service Based Applications are composed of a number of possibly independent services that are available in a network and provided by many actors under different conditions (like price, time to deliver, and so on). Service provision conditions may change in time depending on provider policies, and as such they cannot be statically advertised together with the service description. In this paper we propose and investigate the possibility to use the chemical computational model to address the problem of finding compositions of services that satisfy time constraints coming from the structure of an abstract workflow against the time availability associated to each service component.

Abstract

The value of commercial Service-Based Applications (SBAs) will depend not only on their functionality, but also on the value of their non-functional properties, known as QoS attributes, that are not tied to a specific functionality, but rather to its delivery features. QoS values may vary according to the provision strategies of providers as well as users' requirements expressed as global constraints on the SBA QoS. Automatic negotiation is a viable approach to drive QoS-aware selection of services for SBAs, but its adoption may result computationally expensive due to the communication overhead among the involved negotiators, so limiting its application to real service-based scenarios. In this paper, an empirical evaluation of the impact of negotiation communication costs occurred when composing services to deliver a QoS-aware SBA is carried out, in order to estimate the advantages and disadvantages of negotiation in a market of services, and to identify negotiation parameters settings for which communication costs can be compensated by an increased probability for the negotiation to succeed.

Abstract

Ambient Assisted Living is playing a crucial role in supporting dementia patients to live in their preferred environ- ment, so limiting the involvement of careers and/or relatives. In order for such systems to become a reality, patients need to feel comfortable when interacting with them, and so an agent-based modular approach is adopted to make it possible to personalize the provision of digital services for each specific patient's needs. Here, we experiment with a robotic based ambient assisted environment to analyze the perceived usability when real patients interact with it in a controlled research laboratory where the system is deployed, by taking into account both their personality traits and cognitive status. The perceived usability is evaluated through a survey with a set of patients filling a questionnaire specifically designed for the experimentation that is based on the Unified Theory of Acceptance and Use Technology (UTAUT). The preliminary obtained results show that the perceived usability of the system is related to some traits of patients' personality, while their cognitive status impacts the provided assistive services.

Abstract

QoS-based service composition enables the development of complex business applications. They are composed of distributed services characterized by QoS attributes representing non-functional characteristics, such as cost, execution time, and reliability. With the proliferation of services on the Internet, more candidates for each component service may be available with different QoS values. Hence, the ones satisfying QoS global constraints required for the application have to be selected. Automated negotiation is adopted to select component services in a dynamic market of services where provided QoS values may vary according to different market strategies. Nevertheless, when dealing with multiple QoS attributes, multiple service providers, and not shared information, it is difficult to guarantee the formal properties of the negotiation outcomes. In the present work, we propose a trade-off negotiation strategy that allows reaching a Pareto-optimal agreement, if it exists. The agreement consists of the QoS values of component services that are the ones selected to provide the complete application. The strategy exploits both the competition, that is due to multiple services providing the same functionality with different QoS values, and the cooperation among the providers of the different component services, that are necessary to meet the required end-to-end QoS constraints.

Abstract

Socially assistive robotics technology is expected to play a crucial role in supporting home patients with neurological disorders. Nevertheless, the adoption of such technology in real home environments is still far to be reached since it presents several challenges mainly related to its acceptance in the everyday life. In this domain, a high degree of personalization is required usually obtained by a customization of a robotic system with respect to specific assistive tasks. On the contrary, socially assistive robots are usually general-purpose platforms requiring a considerable effort for customization. In this work, to limit static and costly customization of robotic systems, a service-oriented approach is adopted to represent and manage assistive tasks to be performed by a social robotic system, allowing to decouple a given functionality from its concrete implementation that can be provided by different devices and with different execution modalities. The service-oriented approach for robotics applications, known as Robot-as-a-Service, is becoming attractive for decoupling the robot hardware from the functionalities it provides.
