Abstract

Soccer has been categorised as an intermittent sport, where periods of intense exercise are interspersed with periods of active and passive recovery. Performance relies on several factors including the ability to sprint repeatedly, termed as repeated-sprint ability. It is imperative that a scheduled fitness testing programme includes an assessment of this component to ensure maximal performance during match-play. Although there is a plethora of tests available, the aim of this thesis was to develop, validate and evaluate a reactive repeated-sprint test (RRST), and to investigate its use with a variety of age groups and different performance levels. Data was collected during elite soccer matches and high-intensity running distance in the most intense 5- min period was quantified during match-play. The findings led to the development of a soccer-specific RRST, whereby a detailed evaluation was carried out; the validation, reliability and sensitivity was investigated, as well as utilising the RRST to examine maturation trends and correlates with other physical performance tests. The data demonstrate that the RRST is a reliable and valid test that distinguishes between performance across standard, position and seasonal period. It is also suggested that it is sensitive to monitor maturation trends as well as a large correlation with agility. In summary, the present thesis provides new insight into the most intense period and how players can be monitored throughout a season and at different phases of development. It proposes a new fitness test of best practice for soccer clubs; one that can be used as a multi-tool to assess more than one component of fitness (a practical asset due to the busy day-to-day training week of soccer clubs).

J Strength Cond Res 24(9): 2343-2351, 2010--The aims of the study were to (a) determine the high-intensity activity patterns of soccer players at different performance levels and playing positions, (b) investigate temporary and end game fatigue in elite domestic and international soccer matches, and (c) quantify acceleration and maximal running speed profiles of elite soccer players. Elite domestic (n = 100) and international (n = 10) soccer players were analyzed using a multicamera computerized tracking system. No differences were found for high-intensity running distance (2,520 [+ or -] 678 vs. 2,745 [+ or -] 332 m), mean recovery time (67 [+ or -] 15 vs. 71 [+ or -] 26 seconds), or maximal running speed (7.76 [+ or -] 0.31 vs. 7.66 [+ or -] 0.34 m x [s.sup.-1]). The distance covered in high-intensity running irrespective of playing level was 18% lower (p < 0.05) in the last than in the first 15-minute period of the game (391 [+ or -] 117 vs. 478 [+ or -] 141 m). The decline in high-intensity running immediately after the most intense 5-minute period was similar between international (222 [+ or -] 33 vs. 109 [+ or -] 37 m or 51% decline) and elite domestic (243 [+ or -] 81 vs. 114 [+ or -] 51 m or 53% decline) players. Wide midfielders, central midfielders, fullbacks, and attackers covered a greater (p < 0.01) distance in high-intensity running than central defenders (3,243 [+ or -] 625, 2,949 [+ or -] 435, 2,806 [+ or -] 408, 2,618 [+ or -] 745 vs. 2,034 [+ or -] 284 m). Results demonstrate that high-intensity running is reduced during various periods of elite soccer matches, and high-intensity activity profiles and fatigue patterns are similar between international and elite domestic players but vary markedly between playing positions. These data provide valuable information to the fitness coach regarding the high-intensity active profile of elite soccer players that could be used to develop soccer-specific training drills. Individuals living with HIV and those at risk are vulnerable to stress disorders. Corticotropin-releasing hormone (CRH) is critical in neuroendocrine immune regulation. CRH, a neuropeptide, is distributed in the central and peripheral nervous systems and acts principally on CRH receptor type 1 (CRHR1). CRH in the brain modulates neuropsychiatric disorders. CRH and stress modulation of immunity is two-pronged; there is a direct action on hypothalamic–pituitary–adrenal secretion of glucocorticoids and through immune organ sympathetic innervation. CRH is a central and systemic proinflammatory cytokine. Glucocorticoids and their receptors have gene regulatory actions on viral replication and cause central and systemic immune suppression. CRH and stress activation contributes to central nervous system (CNS) viral entry important in HIV-associated neurocognitive disorders and HIV-associated dementia. CNS CRH overproduction short-circuits reward, executive, and emotional control, leading to addiction, cognitive impairment, and psychiatric comorbidity. CRHR1 is an important therapeutic target for medication development. CRHR1 antagonist clinical trials have focused on psychiatric disorders with little attention paid to neuroendocrine immune disorders. Studies of those with HIV and those at risk show that concurrent stress-related disorders contribute to higher morbidity and mortality; stress-related conditions, addiction, immune dysfunction, and comorbid psychiatric illness all increase HIV transmission. Neuropsychiatric disease, chronic inflammation, and substance abuse are endemic, and chronic distress is a pathologic factor. It is being understood that stress and CRH are fundamental to neuroendocrine immunity; therapeutic interventions with existing and novel agents hold promise for restoring homeostasis, reducing morbidity and mortality for those with HIV and possibly reducing future disease transmission. Abstract

Given the limitations of antiretroviral therapy and recent advances in our understanding of HIV persistence during effective treatment, there is a growing recognition that a cure for HIV infection is both needed and feasible. The International AIDS Society convened a group of international experts to develop a scientific strategy for research towards an HIV cure. Several priorities for basic, translational and clinical research were identified. This Opinion article summarizes the group's recommended key goals for the international community.

Abstract

Administration of interleukin (IL)-2 to human immunodeficiency virus (HIV)-infected patients leads to significant increases in CD4 T cell counts. We previously have shown that IL-2 induces increased proliferation and survival of CD4 T cells. Deuterium labeling studies were undertaken to study the relationship between IL-2-induced increases in the CD4 T cell count and the effects of IL-2 on cell proliferation and survival. A strong inverse correlation was noted between the rate of decay of the label in CD4 cells and increases in CD4 cell counts (R =or- 0.67; P.001). This correlation was not seen with the level of proliferating cells. Although the CD4 cell count at baseline and the number of CD4 cells expressing CD25 were also predictive of increases in the CD4 cell count, the rate of decay remained the most statistically significant predictor in multivariate regression models. Thus, an increase in the survival of CD4 T cells appears to be the critical mechanism leading to sustained increases in the CD4 cell counts of HIV-infected patients receiving intermittent IL-2 therapy.

Abstract

Purpose: The aim of this study was to determine the reliability, validity and sensitivity of a reactive repeated-sprint test (RRST).Methods: Elite (n = 72) and sub-elite male (n = 87) and elite female soccer players (n = 12) completed the RRST at set times during a season. Total distance timed was 30 m and the RRST performance measure was the total time (s) across eight repetitions. Competitive match running performance was measured using GPS and high-intensity running quantified (≥ 19.8 km h(-1)).Results: Test-retest coefficient of variation in elite U16 and sub-elite U19 players was 0.71 and 0.84 %, respectively. Elite U18 players' RRST performances were better (P < 0.01) than elite U16, sub-elite U16, U18, U19 and elite senior female players (58.25 ± 1.34 vs 59.97 ± 1.64, 61.42 ± 2.25, 61.66 ± 1.70, 61.02 ± 2.31 and 63.88 ± 1.46 s; ES 0.6-1.9). For elite U18 players, RRST performances for central defenders (59.84 ± 1.35 s) were lower (P < 0.05) than full backs (57.85 ± 0.77 s), but not attackers (58.17 ± 1.73 s) or central and wide midfielders (58.55 ± 1.08 and 58.58 ± 1.89 s; ES 0.7-1.4). Elite U16 players demonstrated lower (P < 0.01) RRST performances during the preparation period versus the start, middle and end of season periods (61.13 ± 1.53 vs 59.51 ± 1.39, 59.25 ± 1.42 and 59.20 ± 1.57 s; ES 1.0-1.1). Very large magnitude correlations (P < 0.01) were observed between RRST performance and high-intensity running in the most intense 5-min period of a match for both elite and sub-elite U18 players (r = -0.71 and -0.74), with the best time of the RRST also correlating with the arrowhead agility test for elite U16 and U18 players (r = 0.84 and 0.75).Conclusion: The data demonstrate that the RRST is a reliable and valid test that distinguishes between performance across standard, position and seasonal period.
