La rinuncia al confronto critico con l’esistente con le sue specificità e connotazioni identitarie in conseguenza di una massificazione pervasiva dominata dalle culture mediatiche globalizzate. La rinuncia in architettura alla regola che, come affermava Gregotti, «è comunque fondamento anche dell’eccezione ma non ostentata o dell’eccezione come regola», e infine la rinuncia «dell’opera architettonica intesa come metafora di eternità» in conseguenza della continua esaltazione della provvisorietà, della continua riciclabilità, di un continuo flusso instabile assunto come unico aspetto valoriale. Credo che le parole di Gregotti mettano in luce una chiave di lettura interpretativa fondamentale che ci aiuta a comprendere come le forme emergenti del paesaggio antropogeografico, oggi più che mai nell’intera dimensione esistenziale dell’abitare la terra, sono espressione dello stridente convivere duale e antinomico del silenzio della natura, naturalis o artificialis che sia, e della sopraffazione del frastuono generato dai costanti processi modificativi del tempo recente. Tali forme devono essere sempre re-interpretate come gli elementi principali dei fattori identitari degli stessi luoghi.

Bayesian Statistics has been increasingly popular in the last five decades. Besides having decision theoretic foundation, the Bayesian approach found popularity thanks to the simple and intuitive framework that it offers to model phenomena, allowing the statistician to include prior knowledge in the inference procedure. Each unknown quantity in the model is endowed with a prior distribution which encodes our prior belief, and Bayes' rule functions as a probabilistic tool to update the prior belief after observing the data. The result of the inference scheme is the posterior distribution, which, being a probability distribution on the unknown parameters, automatically provides a principled way to quantify the uncertainty of our estimation and make prediction. The choice of the prior distribution can be crucial for the performance of the Bayesian model. In particular, restricting the prior distribution to a finite-dimensional space can lead to a model which may not fit the data properly. Very frequently in applications, in order to capture complex patterns of the data, statistical models include infinite-dimensional parameters, and in such cases the model is referred to as nonparametric. The first part of this thesis is dedicated to Bayesian Nonparametric modelling. In particular the focus will be on modelling two discrete random structures: random partitions and random feature allocations. These two objects are the building blocks for several models used for clustering, feature modeling, network modeling, regression, density estimation. They allow to describe dataset of increasing complexity, such as collections of labelled observations with an increasing number of labels, and observations showing features with the number of features growing over time. A recurrent assumption in Bayesian modelling is the exchangeability of the data. Despite being a reasonable and convenient assumption, it leads to strict asymptotic behaviour of the model. In clustering application for instance, exchangeability in random partition models implies that the size of each cluster grows linearly with the number of observations. Analogously, in feature modelling, exchangeable feature allocation models have the number of objects sharing the same feature grow linearly with the number of objects observed. Such restrictive asymptotic properties are intrinsic in exchangeable models and are not appropriate for some applications, therefore it is necessary to go beyond exchangeability in order to design priors with a broader range of asymptotic properties. Moreover, we show that the proposed priors retain good properties already achieved by exchangeable models, such as the power-law property, which is ubiquitous in real-world applications. On the one hand Bayesian modelling gives freedom by allowing to choose the prior distribution which best reflects our prior belief. On the other hand, especially in nonparametric settings, the design of such priors could be challenging and one would rather opt for standard and objective priors to use in practice. One way to validate and compare priors consists in a frequentist analysis of the posterior distribution. Consistency of the posterior distribution for example, ensures that the effect of the prior knowledge vanishes as an increasing amount of data is collected, while posterior contraction rates can be used to compare different priors. In the second part of the thesis we study asymptotic properties for different priors in Functional Data Analysis. The latter has gained an increasing attention in the last decades thanks to the raise of data collected over fine grids, making it reasonable to model them as functions. While Functional Data Analysis has been thoroughly studied in the frequentist literature and widely used in Bayesian modelling, asymptotic analysis of Bayesian methods are rather scarce. We will focus on two models: Functional Linear Regression and Functional Single-Index Model, both in the setting of functional predictor and scalar response. In particular we will show that for two classes of priors used for Bayesian Functional Linear Regression, minimax posterior contraction rates can be achieved. Moreover, posterior contraction rates are derived for a class of priors for the Bayesian Functional Single-Index Models.

Background Abdominal wall hernia is a common surgical condition. Patients may present in an emergency with bowel obstruction, incarceration or strangulation. Small bowel obstruction (SBO) is a serious surgical condition associated with significant morbidity. The aim of this study was to describe current management and outcomes of patients with obstructed hernia in the UK as identified in the National Audit of Small Bowel Obstruction (NASBO). Methods NASBO collated data on adults treated for SBO at 131 UK hospitals between January and March 2017. Those with obstruction due to abdominal wall hernia were included in this study. Demographics, co‐morbidity, imaging, operative treatment, and in‐hospital outcomes were recorded. Modelling for factors associated with mortality and complications was undertaken using Cox proportional hazards and multivariable regression modelling. Results NASBO included 2341 patients, of whom 415 (17·7 per cent) had SBO due to hernia. Surgery was performed in 312 (75·2 per cent) of the 415 patients; small bowel resection was required in 198 (63·5 per cent) of these operations. Non‐operative management was reported in 35 (54 per cent) of 65 patients with a parastomal hernia and in 34 (32·1 per cent) of 106 patients with an incisional hernia. The in‐hospital mortality rate was 9·4 per cent (39 of 415), and was highest in patients with a groin hernia (11·1 per cent, 17 of 153). Complications were common, including lower respiratory tract infection in 16·3 per cent of patients with a groin hernia. Increased age was associated with an increased risk of death (hazard ratio 1·05, 95 per cent c.i. 1·01 to 1·10; P = 0·009) and complications (odds ratio 1·05, 95 per cent c.i. 1·02 to 1·09; P = 0·001). Conclusion NASBO has highlighted poor outcomes for patients with SBO due to hernia, highlighting the need for quality improvement initiatives in this group., This study shows that small bowel obstruction due to abdominal wall hernia is common, typically involves a co‐morbid group of patients and has poor outcomes, including high mortality rates. - Abstract
OBJECTIVES: Healthcare systems worldwide have been overburdened by the coronavirus disease 2019 (COVID-19) outbreak. Accordingly, hospitals had to implement strategies to profoundly reshape both non-COVID-19 medical care and surgical activities. Knowledge about the impact of the COVID-19 pandemic on cardiac surgery practice is pivotal. The goal of the present study was to describe the changes in cardiac surgery practices during the health emergency at the national level. METHODS: A 26-question web-enabled survey including all adult cardiac surgery units in Italy was conducted to assess how their clinical practice changed during the national lockdown. Data were compared to data from the corresponding period in 2019. RESULTS: All but 2 centres (94.9%) adopted specific protocols to screen patients and personnel. A significant reduction in the number of dedicated cardiac intensive care unit beds (-35.4%) and operating rooms (-29.2%), along with healthcare personnel reallocation to COVID departments (nurses-15.4%, anaesthesiologists-7.7%), was noted. Overall adult cardiac surgery volumes were dramatically reduced (1734 procedures vs 3447; P 0.001), with a significant drop in elective procedures [580 (33.4%) vs 2420 (70.2%)]. CONCLUSIONS: This national survey found major changes in cardiac surgery practice as a response to the COVID-19 pandemic. This experience should lead to the development of permanent systems-based plans to face possible future pandemics. These data may effectively help policy decision-making in prioritizing healthcare resource reallocation during the ongoing pandemic and once the healthcare emergency is over. © 2021 The Author(s). - Abstract
Background: \ud Small bowel obstruction is a common surgical emergency, and is associated with high levels of morbidity and mortality across the world. The literature provides little information on the conservatively managed group. The aim of this study was to describe the burden of small bowel obstruction in the UK.\ud \ud Methods: \ud This prospective cohort study was conducted in 131 acute hospitals in the UK between January and April 2017, delivered by trainee research collaboratives. Adult patients with a diagnosis of mechanical small bowel obstruction were included. The primary outcome was in‐hospital mortality. Secondary outcomes included complications, unplanned intensive care admission and readmission within 30 days of discharge. Practice measures, including use of radiological investigations, water soluble contrast, operative and nutritional interventions, were collected.\ud \ud Results: \ud Of 2341 patients identified, 693 (29·6 per cent) underwent immediate surgery (within 24 h of admission), 500 (21·4 per cent) had delayed surgery after initial conservative management, and 1148 (49·0 per cent) were managed non‐operatively. The mortality rate was 6·6 per cent (6·4 per cent for non‐operative management, 6·8 per cent for immediate surgery, 6·8 per cent for delayed surgery; P = 0·911). The major complication rate was 14·4 per cent overall, affecting 19·0 per cent in the immediate surgery, 23·6 per cent in the delayed surgery and 7·7 per cent in the non‐operative management groups (P < 0·001). Cox regression found hernia or malignant aetiology and malnutrition to be associated with higher rates of death. Malignant aetiology, operative intervention, acute kidney injury and malnutrition were associated with increased risk of major complication.\ud \ud Conclusion: \ud Small bowel obstruction represents a significant healthcare burden. Patient‐level factors such as timing of surgery, acute kidney injury and nutritional status are factors that might be modified to improve outcomes.

L'articolo costruisce un quadro sinottico che raffronta l’Arsenale palermitano ad analoghe architetture realizzate in contesti geografici di influenza politico-militare spagnola, in ambito italiano e del Mediterraneo in genere. L’attività cantieristica navale in Sicilia, e la conseguente costruzione dei luoghi deputati a tale scopo, è attestabile sin dall’età greca e fenicio-punica. Parte della flotta romana fu certamente costruita nei navalia della parte orien- tale dell’Isola, nei principali arsenali del tempo come quelli di Messana e Siracusa, dove era reperibile con facilità la materia prima necessaria per la costruzione delle imbarcazioni: gli alberi dei boschi dei Nebrodi, il piombo dei Monti Peloritani, la pece del Ragusano. La città di Messina, in particolare, vantava tradizioni marinare antichissime grazie ad un porto che per secoli ne aveva influenzato lo sviluppo eco- nomico, commerciale e politico. Il saggio pone in evidenza come l'Arsenale di Palermo, in confronto agli arsenali delle principali città del Mediterraneo, presenta delle specificità tipologiche e un accentuato carattere architettonico che lo rende un episodio abbastanza singolare. Dal punto di vista planimetrico la sua organizzazione interna, con il sistema delle campate formanti le quattro navate contornate dai magazzini, appare bloccata e non suscettibile di organici ampliamenti; che del resto sarebbero stati impediti dalla presenza, a breve distanza, di altre costruzioni preesistenti o inopportunamente edificate dopo il 1620. La realizzazione (anche se parziale) del primo livello per un uso comunque distinto e separato da quello dell’arsenale, gli conferiva una anomala facies da palatium non riscontrabile in altri casi. Le sue dimensioni, pari a circa 5500 mq, erano peraltro piuttosto modeste se confrontate con quelle dei principali arsenali italiani ed europei agli inizi del Seicento, che raggiungevano, in genere, ben oltre i diecimila metri quadrati, e presentavano una complessa articolazione di strutture edilizie.

Il saggio costituisce una riflessione sulle personali esperienze didattiche, svolte nell’ambito dei laboratori di progettazione architettonica del terzo anno, ma anche del laboratorio di laurea, del Corso di Studi in Architettura magistrale a ciclo unico quinquennale, dell’Università degli Studi di Palermo, partendo da una premessa che con frequenza ha costituito il prologo agli stessi corsi e si riassume nel motto ciceroniano Quid Tum? Corsi dedicati in prevalenza al tema delle architetture e degli spazi esequiali ma anche al recupero di aree oggi al margine di insediamenti urbani, segnate da stratificazioni e da ancestrali processi di antropizzazione: dal sistema dei borghi rurali dell’entroterra palermitano alle architetture rupestri del quartiere Rabato e del parco dell’Addolorata ad Agrigento, dalle Latomie di Marsala, alle cave di Cusa e dei monti Sicani a quelle di Mazzara e di Favignana. Luoghi individuati, essenzialmente come campo di sperimentazione progettuale dei laboratori finalizzati a utilizzare la particolare dimensione fisica di specifiche aree come materia formativa e strutturante della stessa idea di progetto.

Il contributo affronta il tema del rapporto tra architettura e natura attraverso il ruolo strutturante che il paesaggio antropizzato riveste per quel tipo di progetto che basa la propria essenza costitutiva sulla dimensione fisica e spirituale dei luoghi; in tal senso, il paesaggio è inteso quale finalità principale dell’architettura. Ad esplicitazione dei concetti espressi, si prendono in esame gli esiti della ricerca progettuale condotta dall’autore nell’ambito del proprio laboratorio di progettazione architettonica, destinato allo sviluppo delle tesi di laurea, incentrato sul tema L’architettura nella roccia e operante intorno ai concetti dello scavo, della sottrazione, del progetto del vuoto e della generazione di architetture silenti. The paper deals with the relationship between archi- tecture and nature through the man-made landscape structural role in a particular project, centered on the physical and spiritual dimension of the places. That’s how the landscape is intended as the main purpose of architecture. To explain the conveyed notions, we will examine the results of the design research carried out by the author for his laboratory of architectural design, for his dissertation on Architettura nella roccia about the excavating, subtraction and emptiness design concepts together with the generation of silent architectures.

OBJECTIVES: To evaluate the prevalence of diarrhea and its association with drug use in elderly outpatients. METHODS: The study was carried out by 133 general practitioners (GPs) who referred to 24 geriatric units in Italy. The demographic data, disability, gastrointestinal symptoms, and current medications were evaluated using a structured interview, including the evaluation of the activities of daily living (ADL), the instrumental activities of daily living (IADL), and the gastrointestinal symptoms rating scale (GSRS). RESULTS: The study included 5,387 elderly subjects who regularly completed the structured interview. In total, 488 patients (9.1% of the whole population, 210 men and 278 women, mean age 75.6 6.2 yr, range 65–100 yr) reported diarrhea, that is, items 11 and 12 of the GSRS, during the 7-day period before the interview. The prevalence of diarrhea significantly increased with older age (P= 0.025), the severity of ADL (P < 0.0001) and IADL disability (P < 0.0001), and the number of drugs taken (P= 0.0002). A multivariate analysis demonstrated that the presence of diarrhea was significantly associated with the use of antibiotics (odds ratio [OR] 4.58, 95% confidence interval [CI] 1.95–10.73), proton pump inhibitors (OR 2.97, 95% CI 2.03–4.35), allopurinol (OR 2.19, 95% CI 1.26–3.81), psycholeptics (OR 1.82, 95% CI 1.26–2.61), selective serotonin reuptake inhibitors (OR 1.71, 95% CI 1.01–2.89), and angiotensin II receptor blockers (OR 1.46, 95% CI 1.08–1.99), also accounting for sex, age, and the use of antidiarrheal agents and drugs for functional gastrointestinal disorders. CONCLUSION: Diarrhea is a common problem in elderly outpatients. Its prevalence increases with old age, the severity of disability, and the number of drugs. Monitoring the presence of diarrhea and its complications in elderly patients who need treatments with drugs significantly associated with diarrhea may be clinically useful.

1. Scheda Allestimento del Museo Civico nel convento dei Padri Cappuccini a Bisacquino, Palermo. Il progetto di allestimento museale s’inserisce nel quadro specifico proprio del museo della cultura materiale. La qualità architettonica complessiva del convento dei frati cappuccini pone la necessità di armonizzare l’allestimento museale con il carattere specifico degli ambienti costitutivi il complesso claustrale, individuati, nel progetto di restauro dell’edificio, come spazi espositivi. Proprio l’intendimento dell’allestire “come arte del porgere; ovvero mostrare con misura e porgere con intelligenza” ha indotto verso scelte progettuali declinate in ragione del percorso narrativo del museo e nel contempo in grado di esaltare le peculiari qualità degli spazi esistenti. Al fine di costituire diverse situazioni spaziali interpretative delle specificità geometrico- configurative e dimensionali delle singole sale, le forme degli espositori variano di ambiente in ambiente raggiungendo tal volta l’effetto di veri e propri apparati scenici. In tal modo ogni spazio si presenta come frammento di un “paesaggio d’interni” che si ricompone nell’unitarietà dell’allestimento e della serrata sequenza degli ambienti ad infilata del convento. 2. Scheda Allestimento del Museo "La Fabbrica di Guglielmo" nell'Abbazia benedettina di Santa Maria La Nuova a Monreale Il complesso monumentale Guglielmo II con il suo inestricabile sistema di gallerie e sale, con i suoi punti di affaccio privilegiato verso scenari paesaggistici e architettonici straordinari, contiene, nel suo intimo, il senso profondo del tema museale. La memoria stessa, soprattutto quella dell’antico cenobio benedettino offuscata dai recenti lavori di restauro, costituisce il tema cui il progetto tenta di dare forma, ospitando in successione concatenata di apparati allestitivi e scenografici, repertori iconografici, proiezioni, effetti luminosi, sonori ed olfattivi, ricostruzioni virtuali, voci fuori campo carichi del senso della storia che li rende elementi significanti, capaci di dialogare, facendoli risuonare, con i ricordi, i documenti, gli echi di atmosfere antiche che in questi specifici luoghi si conservano. Allestendo si tessono dunque relazioni con uno spazio che accoglie, include, ospita; relazioni che comunque in ognuno degli ambiti architettonici dell’edificio interessati non tendono a celare, occultare, velare il “contenitore”, ma piuttosto tentano di leggerlo, commentarlo, evidenziarlo al fine di emblematizzarlo.

The aim of our study was to investigate possible interaction of IL-17, TRAIL, and TNF-α in the modulation of osteoblast homeostasis in vitro, using human differentiated osteoblastic Saos-2 cells as in vitro model.The effects of these cytokines on osteoblastic cell viability were assessed, by MTT assay, alone or in combination, at different times and concentrations. The effects of IL-17 and TNF-α on the regulatory system of osteoclast activity RANK/RANKL/ OPG were evaluated by Western blot and ELISA techniques in cell culture media. Quantitative expression of RANKL, OPG and pro-inflammatory factors were analysed at the mRNA level by quantitative real time RT-PCR.Effects of IL-17, TNF-α and TRAIL on osteoblastic cell viability indicated that IL-17 alone, or in combination with TNF-α did not alter Saos-2 cell viability. On the other hand, TRAIL, as expected, exhibited time- and concentration-dependent cytotoxicity. The expression both RANKL and OPG were increased at the mRNA level and protein release by IL-17 and TNF-α, either alone or in combination. The analysis of IL-17 and TNF-α on pro-inflammatory molecules mRNA expression, such as CXC family chemokines CXCL-1 and CXCL-5, COX-2 and IL-6 demonstrated an increase in these pro-inflammatory cytokines with cooperative effects of the combination.Overall, these results suggest that IL-17, TRAIL and TNF-α sustain bone tissue inflammation associated with decrease of calcified component. To do so, they act redundantly each other, to amplify the inflammatory response in the bone. In conclusion, unravelling novel molecular targets within the bone-cytokine network represents a platform for innovative treatment of bone diseases due to immunological diseases such as psoriatic arthritis.

Il testo descrive i contenuti del coordinamento dei Laboratori di Progettazione architettonica del 3° anno tenuti presso il Corso di laurea in Architettura dell'Università di Palermo. Nell’ambito di tale coordinamento, lo sforzo prodotto per approfondire la ricerca sul tema dell’accoglienza ha generato grande tensione progettuale nei docenti e, soprattutto, negli studenti. Certamente essi hanno saputo cogliere la grande attualità e il portato etico e antropologico delle questioni legate al fenomeno dell’immigrazione. Hanno, inoltre, saputo interpretare in maniera straordinaria lo spirito della coralità che proprio il coordinamento ha suggerito, come ben evidenziato nella pubblicazione dei risultati didattici del coordinamento.

Background/Objective: This study aims at the evaluation of the efficacy and safety of a combination therapy based on pidobenzone 4% and fractional CO2 laser or cryotherapy in the treatment of solar lentigines and the prevention of eventual posttreatment hyperchromia. Methods: Efficacy was clinically evaluated by grading the pigmentation level with the Skin Tone Color Scale (STCS), and by grading patients' impression through a Visual Analog Scale (VAS). Results: Our study shows that the associated treatment was safe and that it improves the therapeutic results on solar lentigines and prevents postiatrogenic hyperpigmentation compared with physical therapy alone. Conclusion: The combination of cryotherapy and pidobenzone 4% has been found to be the most useful treatment.
