Abstract

An approximate of 72 consecutive HIV patients with chronic hepatitis C who had a simultaneous liver biopsy and liver stiffness measurement by transient elastogarphy for the assessment of the liver fibrosis, is studied. Liver stiffness measurement is a promising non-invasive method for the assessment of fibrosis in HIV-infected patients with chronic HCV infection.

Abstract

A drug called peginterferon alfa-2a combined with ribavirin appears to be more effective for treating hepatitis C than interferon alfa-2b plus ribavirin or peginterferon alfa-2a alone. This was the conclusion of researchers who compared all three drug treatments in a study of 1,121 patients with hepatitis C. Peginterferon alfa-2a was also less likely to cause depression or symptoms similar to influenza.

Abstract

Background. Orthotopic liver transplantation has been used in a large number of patients with primary liver cancer because it increases the possibilities of resection of large tumors. Despite isolated cases of prolonged survival, however, the results of liver transplantation for advanced tumors have been universally disappointing because of high rates of tumor recurrence. In an attempt to reduce the recurrence rate, a pilot study testing a multimodal adjuvant treatment in patients undergoing liver replacement for hepatocellular carcinoma was undertaken. Methods. The treatment consisted of preoperative hepatic arterial chemoembolization (iodized oil, doxorubicin, and gelatin sponge) and radiotherapy (5 Gy in one fraction immediately before surgery), and postoperative systemic chemotherapy with mitoxantrone. Nine patients entered this study. The tumor was solitary in two cases (5 cm and 8 cm) and multifocal in seven cases (2-9 nodules, 3-9 cm). The postoperative TNM stages were II in one case, III in one case, and IVA in seven cases. Results. Chemoembolization and radiotherapy were performed in seven cases each (five patients had both treatments). All patients underwent liver transplantation with conventional immunosuppression. One patient died of heart failure 4 days after surgery. The remaining eight patients received 4 to 10 courses of chemotherapy (mean 9). The main toxicity of chemotherapy was leucopenia. Two patients died of recurrence: one at 7 months and one at 11 months. Six patients are alive, five of them without evidence of disease, with a mean follow-up of 30 months (range 16-45) after liver transplantation. The 3-year actuarial survival is 64%. Conclusions. These results show that an aggressive adjuvant therapy can be used in association with liver transplantation in the treatment of advanced hepatocellular carcinoma without increased mortality and suggest that such a protocol could be effective in preventing tumor recurrence. 48. Prise en charge des personnes infectées par les virus de l'hépatite B ou de l'hépatite C

Abstract

Ce premier "Rapport de recommandations sur la prise en charge des personnes infectées par les virus de l’hépatite B ou de l’hépatite C" est le fruit d’une démarche collective impulsée par la Ministre de la Santé et réalisée sous l’égide de l’Agence nationale de recherches sur le sida et les hépatites virales (ANRS) et de l’Association française pour l’étude du foie (AFEF). Ce rapport a été coordonné par le Pr Daniel Dhumeaux (hépatologue au CHU Henri Mondor de Créteil et président du Programme national de lutte contre les hépatites B et C 2009-2012). Pour produire ses recommandations, il a mobilisé pendant plus d’une année quelque 200 cliniciens, chercheurs, patients et associations de patients. L’élaboration du rapport s’est organisée autour de 22 groupes thématiques d’experts et d’un comité indépendant des experts, fait de personnalités scientifiques et du monde associatif, dont la mission a été d’assurer la validation et la synthèse des textes. Par les mesures qu’il met en avant, ce rapport se propose, selon trois axes : – de relancer la prévention des hépatites B et C, jugée en retard sur plusieurs points (vaccination contre l’hépatite B notamment), – d’articuler les étapes de la prise en charge des patients atteints d’hépatites B et C selon des parcours de soins et des stratégies thérapeutiques spécifiques qui leur sont propres, – de soutenir, à tous les niveaux, les valeurs d’équité dans les soins par des mesures concrètes de lutte contre les inégalités sociales. En émettant ces recommandations, le collectif responsable du rapport a eu clairement conscience de la contrepartie économique des traitements nouveaux, en particulier de l’hépatite C, qui implique des choix et des indications raisonnables. Avec des thérapeutiques dont l’efficacité est désormais proche de 100 %, l’éradication de cette infection peut devenir un objectif réaliste.
