Abstract

Background: Decisions about organ donation are stressful for family members of potential organ donors. We sought to comprehensively explore the donation process from interviews conducted with family members of patients admitted to pediatric and adult intensive care units in Canada. Methods: We conducted a qualitative study using semistructured, in-depth interviews with 271 family members asked to make an organ donation decision. We recruited participants from all provinces with an organ donation organization (n = 10), and analyzed themes using a modified grounded theory approach. On the basis of these inter views, suggestions were made by researchers and family members on how to improve the process of organ donation. Results: We identified 3 main themes and 9 subthemes. Families need more comprehensive support around the time of donation, including having access to someone with shared experiences, support during specific moments as needed and better support during critical transitions (e.g., when the donor body goes to the operating room). The theme of better connection to recipient(s) included receiving information about the donation surgery (e.g., which organs were recovered), establishing connection with recipients (e.g., via social networks or letters) and planned encounters. Support after donation, such as updates on organ transplantation, early mental health checks and continued connection to donor organizations, could be improved. We derived 20 suggestions for improving the organ donation process, derived from interviews with family members of pediatric and adult organ donors. Interpretation: We found gaps in family support during end-of-life and donation care. Feelings of abandonment, lack of support and poor-to-little follow-up provide the empirical findings needed for hospitals and organ donor organizations to provide bettersupporttodonorfamilies., In Canada, 2 pathways to deceased organ donation exist. Abstract

Contexte: Ameliorer l'approche des donneurs d'organes potentiels et obtenir leur consentement pourrait favoriser l'autonomie des patients et donner acces a un plus grand nombre d'organes a transplanter. Nous avons voulu identifier les facteurs modifiables qui influent sur le consentement au don d'organes. Methodes: Nous avons procede a une etude de cohorte retrospective regroupant les adultes ([greater than or equal to] 18 ans) consecutivement orientes vers le systeme de don d'organes en Ontario entre avril 2013 et juin 2019. Nous avons analyse les donnees cliniques et demographiques des patients, les donnees relatives a leurs mandataires et les particularites des approches pour le consentement au don. Les parametres de l'etude etaient le consentement au don d'organes et le taux d'approches. Nous avons analyse les liens independants entre le consentement et les facteurs propres aux approches et au systeme. Resultats: Nous avons identifie 34 837 signalements de donneurs d'organes potentiels, dont 6548 (18,8 %) ont fait l'objet d'approches aupres de leurs mandataires en vue d'un consentement. Parmi ces derniers, 3927 (60,0 % des approches) ont mene a un consentement au don d'organes et 1883 patients (48,0 % des consentements) ont effectivement ete donneurs. La raison la plus courante pour laquelle des mandataires n'ont pas ete approches en vue du consentement a un don potentiel etait un retard de signalement de la part de l'equipe soignante (45,2 %). Les facteurs modifiables independants associes au consentement incluaient: approche telephonique (rapport des cotes [RC] ajuste 0,46, intervalle de confiance IC a 95 %o 0,35-0,58) et approche en collaboration avec le medecin et la coordination des dons (RC ajuste 1,26, IC a 95 % 1,01-1,59). Interpretation: Le consentement au don d'organes a ete associe a plusieurs facteurs modifiables. Abstract

Background: Optimizing the approach to and consent of potential organ donors maximizes patient autonomy and the availability of organs for transplants. We set out to identify modifiable factors associated with donation consent. Methods: We conducted a retrospective cohort study of consecutive adults ([greater than or equal to] 18 yr) referred for organ donation in Ontario between April 2013 and June 2019. We analyzed patient clinical data and demographics, data on substitute decision-makers and characteristics of the donation consent approach. Study outcomes were consent for organ donation and approach rate. We evaluated independent associations between consent and approach- and system-level factors. Results: We identified 34 837 referrals for organ donation, of which 6548 (18.8%) substitute decision-makers were approached for consent. Of these, 3927 (60.0% of approaches) consented for organ donation and 1883 (48.0% of consents) patients proceeded to be organ donors. The most common reason substitute decision-makers were not approached for consent in a case with donation potential was a late referral by the health care team (45.2%). Modifiable factors independently associated with consent included a telephone approach for consent (adjusted odds ratio [OR] 0.46, 95% confidence interval [CI] 0.35-0.58) and a collaborative approach by a physician and donation coordinator (adjusted OR 1.26, 95% CI 1.01-1.59). Interpretation: Consent for organ donation was associated with several modifiable factors. Abstract

Background & Purpose: Acute and critical care (ACC) settings are a highly demanding and specific environment for registered nurses (RNs) to provide care in, and the use of coping strategies is key to supporting their work-related well-being. However, currently, there is a lack of comprehensive evidence on how RNs in ACC settings, specifically, cope with work-related stressors. Therefore, this review will summarize the international literature on coping strategies RNs use in ACC settings to deal with work-related stressors. Methods & Procedures: Scoping review using the JBI methodology. Databases to be searched include Medline, Embase, CINAHL, Web of Science, and Cochrane Library. The report will consider references if focused on the coping strategies RNs use to manage work-related issues in ACC settings worldwide. Quantitative, qualitative, experimental, and gray literature will be considered if relevant to our topic. Two independent reviewers will screen, extract, appraise, and analyze the reports. Tables, charts, and diagrams will be used as applicable for data presentation. Expected results/discussion/conclusion: This review will provide comprehensive evidence on coping strategies RNs use in ACC settings to manage work stressors; such evidence will guide quality improvement strategies and tailored interventions aimed at improving working-related well-being among this population.
