Background: Pediatric liver transplant (PLT) activity has flourished over time although with limited expansion in the graft pool. The study aims to identify pre‐transplant factors that predict post‐transplant patient and graft survival in the PLT population. Methods: Retrospective review of PLTs at a single tertiary transplant unit from 2000 to 2019. Univariate and multivariate analyses of pre‐transplant factors were performed to identify predictors of patient and graft survival. Results: Two hundred and seventy‐six patients received 320 PLTs. The most common cause of graft loss was hepatic artery thrombosis (n = 13, 29.6%). The most common cause of mortality was sepsis (n = 11, 29.7%). Univariate analysis showed that the following variables had a significant (p <.05) impact on patient survival: recipient age, weight, height, graft type (technical variant graft), transplant category (acute liver failure), the era of transplant, and invasive ventilation. The following variables had a significant (p <.05) impact on graft survival: recipient age, weight, height, transplant category (acute liver failure), and the era of transplant. Multivariate analysis precluded the era of transplant as the only significant factor for patient survival; patients transplanted after 2005 had significantly higher patient survival. No independent factor predicting graft survival was identified. For children transplanted after 2005, the only factor that predicted patient survival was pre‐transplant invasive ventilation. Conclusions: Our study suggests that the learning curve and pre‐transplant invasive ventilation in the recipient have a significant impact on patient survival. The traditional view of worse outcomes of smaller PLT candidates should be changed. [ABSTRACT FROM AUTHOR]

Laparoscopic cholecystectomy is commonly performed, and several factors increase the risk of open conversion, prolonging operating time and hospital stay. Preoperative stratification would improve consent, scheduling and identify appropriate training cases. The aim of this study was to develop a validated risk score for conversion for use in clinical practice.Preoperative patient and disease-related variables were identified from a prospective cholecystectomy database (CholeS) of 8820 patients, divided into main and validation sets. Preoperative predictors of conversion were identified by multivariable binary logistic regression. A risk score was developed and validated using a forward stepwise approach.Some 297 procedures (3.4%) were converted. The risk score was derived from six significant predictors: age (p = 0.005), sex (p 0.001), indication for surgery (p 0.001), ASA (p 0.001), thick-walled gallbladder (p = 0.040) and CBD diameter (p = 0.004). Testing the score on the validation set yielded an AUROC = 0.766 (p 0.001), and a score6 identified patients at high risk of conversion (7.1% vs. 1.2%).This validated risk score allows preoperative identification of patients at six-fold increased risk of conversion to open cholecystectomy.

To review the outcome of transperitoneal laparoscopic dismembered pyeloplasty (LDP) in our institution.From April 2001 to April 2005, 36 men and 34 women underwent LDP for symptomatic and radiologically proved ureteropelvic junction (UPJ) obstruction. A successful outcome was defined clinically by improvement in symptoms and objectively by improvement in the diuretic renography study performed at 3 and 12 months after reconstruction.The mean operative time was 160.5 +/- 45 minutes with a mean blood loss of 77.8 +/- 52.8 mL. There were no conversions to open surgery. Lower-pole vessels were observed in 38 patients (54.3%). In all cases, the ureter was transposed anteriorly. The mean hospital stay was 3.3 +/- 2.1 days. Postoperative complications were recorded in 4 patients (5.7%). One patient developed an anastomotic stricture. A successful outcome was achieved in 68 of the 70 patients (97.1%) at a mean follow-up of 27.6 +/- 13.2 months.Our results further strengthen the evidence that LDP has a reliable medium-term outcome with the added benefit of being minimally invasive.
