Honer (2)(3), Alasdair M. Barr (1)(3) Abstract: Antipsychotic drugs (APDs), and the 'atypical' APDs in particular, are commonly associated with metabolic side effects in humans. These include glucose dysregulation, insulin resistance, hyperlipidemia, weight gain and hypertension, which put patients at increased risk of cardiometabolic disorders. The underlying biology of APD-induced side effects in humans is poorly understood, and therefore preclinical rodent models are essential for translational research. With numerous recent studies on the topic, there is an emerging consensus that some symptoms, such as glucose dysregulation and insulin resistance, are more reliably observed than others, such as weight gain and hypertension, but, comparison between preclinical studies is complicated by numerous factors, including drug-specific effects and variables such as diet and treatment regimen. In this paper, we provide a major review of this important and growing field of preclinical study, and address crucial issues for future research. Abstract

A new glucose transport model relying upon diffusion and convection across the capillary membrane was developed, and supplemented with tissue space and lymph flow. The rate of glucose utilization (J.sub.util) in the tissue space was described as a saturation function of glucose concentration in the interstitial fluid (C.sub.glu,isf), and was varied by applying a scaling factor f to J.sub.max. With f = 0, the glucose diffusion ceased within ~20 min. While, with increasing f, the diffusion was accelerated through a decrease in C.sub.glu,isf, but the convective flux remained close to resting level. When the glucose supplying capacity of the capillary was measured with a criterion of J.sub.util/J.sub.max = 0.5, the capacity increased in proportion to the number of perfused capillaries. A consistent profile of declining C.sub.glu,isf along the capillary axis was observed at the criterion of 0.5 irrespective of the capillary number. Increasing blood flow scarcely improved the supplying capacity. Abstract

OBJECTIVE--The inability of pancreatic β-cells to appropriately respond to glucose and secrete insulin are primary defects associated with β-cell failure in type 2 diabetes. Mitochondrial dysfunction has been implicated as a key factor in the development of type 2 diabetes; however, a link between mitochondrial dysfunction and defective insulin secretion is unclear. RESEARCH DESIGN AND METHODS--We investigated the changes in islet mitochondrial function and morphology during progression from insulin resistance (3 weeks old), immediately before hyperglycemia (5 weeks old), and after diabetes onset (10 weeks old) in transgenic MKR mice compared with controls. The molecular and protein changes at 10 weeks were determined using microarray and iTRAQ proteomic screens. RESULTS--At 3 weeks, MKR mice were hyperinsulinemic but normoglycemic and β-cells showed negligible mitochondrial or morphological changes. At 5 weeks, MKR islets displayed abrogated hyperpolarization of mitochondrial membrane potential (Δ[Ψ.sub.m]), reduced mitochondrial [Ca.sup.2+] uptake, slightly enlarged mitochondria, and reduced glucose-stimulated insulin secretion. By 10 weeks, MKR mice were hyperglycemic and hyperinsulinemic and β-cells contained swollen mitochondria with disordered cristae. β-Cells displayed impaired stimulus-secretion coupling including reduced hyperpolarization of Δ[Ψ.sub.m], impaired [Ca.sup.2+]-signaling, and reduced glucose-stimulated ATP/ADP and insulin release. Furthermore, decreased cytochrome c oxidase-dependent oxygen consumption and signs of oxidative stress were observed in diabetic islets. Protein profiling of diabetic islets revealed that 36 mitochondrial proteins were differentially expressed, including inner membrane proteins of the electron transport chain. CONCLUSIONS--We provide novel evidence for a critical role of defective mitochondrial oxidative phosphorylation and morphology in the pathology of insulin resistance-induced β-cell failure.
