Scholars and civil society have argued that competition erodes supplier morality. This paper establishes a robust irrelevance result, whereby intense market competition does not crowd out consequentialist ethics; it thereby issues a strong warning against the wholesale moral condemnation of markets and procompetitive institutions. Intense competition, while not altering the behavior of profitable suppliers, may, however, reduce the standards of highly ethical suppliers or not-for-profits, raising the potential need to protect the latter in the marketplace. Abstract

This paper studies how opioid analgesic sales are empirically related to socioeconomic disparities in France, with a focus on poverty. This analysis is made possible using the OpenHealth database, which provides retail sales data for opioid analgesics available on the French market. We exploit firm-level data for each of the 94 departments in Metropolitan France between 2008 and 2017. We show that increases in the poverty rate are associated with increases in sales: a one percentage point increase in poverty is associated with approximately a five percent increase in mild opioid sales. Our analysis further shows that opioid sales are positively related to the share of middle-aged people and individuals with basic education only, while they are negatively related to population density. The granularity and longitudinal nature of these data allow us to control for a large pool of potential confounding factors. Our results suggest that additional interventions should be more intensively addressed towards the most deprived areas. We conclude that a combination of policies aimed at improving economic prospects and strictly monitoring access to opioid medications would be beneficial for reducing opioid-related harm.

Abstract

This PhD thesis focuses on the socioeconomic determinants of health outcomes and drug consumption, the evaluation of policy levers aimed at limiting inappropriate pharmaceuticals’ use, as well as the role played by pharmaceutical advertising in the healthcare sector. The thesis is broadly divided into two parts. The first part investigates how socioeconomic disparities are empirically related to the spread of an infectious disease, the COVID-19 pandemic (Chapter 1), as well as consumption of potentially addictive medicines, as given by opioid pain-relievers (Chapter 2). The second part analyses the impact of laws restricting access to narcotic medications. The focus here is on Codeine-containing products, classified as mild opioids. I show how the impact of this type of policies varies depending on the socioeconomic environment in which they are applied in Chapter 3. Chapter 4, instead, studies how pharmaceutical companies reallocate their advertising expenses across geographical areas following the implementation of such policies and how this, in turn, affects sales. This research is primarily empirical and policy-oriented, even though one of my projects includes a theoretical component. Abstract

The paper develops a theory of costly communication in which the sender's and receiver's motivations and abilities endogenously determine the communication mode and the transfer of knowledge. Communication is modeled as a problem of moral hazard in teams, in which the sender and receiver select persuasion and message elaboration efforts. The model is shown to provide a rich set of insights concerning (i) the impact of incentive alignment on communication strategies, (ii) the relative influence and the complementarity/substitutability between issue-relevant communication and cues (information that relates to the credibility of the sender rather than to the issue at stake), and (iii) the path dependency of communication. Abstract

As of the second half of September 2021, most European countries are taking decisions about (i) how forcefully to address vaccine hesitancy, and (ii) which nonpharmaceutical interventions (NPIs) to maintain at a time where hospitalisations and deaths have been reduced by vaccination but could pick up with increased societal contacts and worsening weather. On vaccination, availability of vaccines and the logistics of delivery are no longer binding constraints for rich countries for now. The approval of multiple effective covid vaccines in record time represents a big success for our biomedical innovation ecosystem.2 Of course, the fact that, at this point, only 2% of citizens of low-income countries have received at least one vaccine dose is very worrisome and should be addressed at the global level. High-income countries have an overwhelming ethical responsibility, for the common good and also for their self-interest, to decrease the probability of the emergence of new variants against which current vaccines could be ineffective.
