4. Pleural Amebiasis without Hepar Involvement

Background: Pleuropulmonary amebiasis is the most common complication of amoebic liver abscess, occurring in 15% of patients with amoebic liver disease and in 1% of patients with amoebic dysentery. Most commonly occurs by direct extension of a right superior lobe hepatic abscess through the diaphragm to the right lower lobe of the lung with a mortality rate of 5-16%. Primary pleural amebiasis without liver involvement is rare. Case Presentation: Reported an 18-year-old male patient with pleural amebiasis from the results of parasitological examination of pleural fluid found Entamoeba histolytica. History, physical examination and support showed normal liver function. The patient has a history of poor sanitation with a dissertation of unhygienic habits. Patients without liver problems remain at risk for amoebiasis pleura Conclusion: Pleural amebiasis without liver involvement was a rare case. Further investigation was needed in patients with pleural amoebiasis to other organs, such as the liver and digestive tract. Diagnose pleural amebiasis should be the main focus in determining the etiology. Entamoeba histolytica may be involved in pleural inflammation and cause effusion.

BSF larvae were the agents capable of breaking down large amounts of organic waste. These larvae were known to reduce organic waste effectively and produced larval biomass. The substrate type was one of the factors that could affect the larvae's performance. This study aimed to determine the effect of the type of substrate or organic waste on the growth and waste reduction performance of larvae. Larvae were reared for ten days on several organic wastes, including food waste, solid decanter, coconut pulp, and sago pulp. The growth rate, bioconversion rate, waste reduction index, and feed conversion ratio were analyzed. The larvae with the best reduction performance were then analyzed for protein and fat on days 7 and 10. The results showed that BSF larvae in food waste showed more growth than in other wastes. BSF larvae in food waste showed a high waste reduction performance, while BSF larvae in solid decanter showed poor waste reduction performance. The BSF larval biomass in food waste contained high levels of protein and fat (38.94 % and 42.86 %, respectively).

Speaking skill, which involves the ability to communicate orally, demands self-confidence and composure when interacting with others. Furthermore, speaking in the English language poses a significant challenge for many students, often inducing nerves and anxiety. The purpose of this research was to examine the correlation between students' learning anxiety and their speaking skills at the lower secondary school level. To investigate this relationship, a quantitative research approach with a correlation design was adopted. The data collection involved the administration of a questionnaire (FLCAS) to assess students' learning anxiety and an English-speaking test to evaluate their speaking skills. The questionnaire used in this study was adapted from the work of Horwitz et al. (1986) to measure students' learning anxiety. The collected data were analyzed using the SPSS Program version 25. The target population for this English-speaking of all second-grade students, totaling 102 individuals. Through simple random sampling, 41 students were selected as respondents. The analysis of the data yielded results indicating the acceptance of the alternative hypothesis (Hi) and the rejection of the null hypothesis (H0) (0.046 < 0.05). Thus, the findings suggest a significant correlation between students' learning anxiety and their speaking skills. In summary, the research findings support the existence of a relationship between students' learning anxiety and their proficiency in speaking skills. The study contributes to the understanding of the challenges faced by students in developing their speaking abilities, particularly in the context of English language learning.

Abstract Social forestry has been an instrument of government to escalate community welfare. Forest village in Indonesia is one of the schemes recognized in the national social forestry program since 2007. The economic potential of forest village management is now realized in the Government Regulation in Lieu of Law No.2/2022 regarding Job Creation, specifically designed to accelerate the national economy. The ease offered by the JCL changes nothing to the community perspective in benefiting Village forests through a collaborative scheme between Village-owned enterprises and other groups. Thus, it is essential to portray this collaboration’s potency to provide village community welfare further. This study uses the socio-legal method by applying statutory and conceptual approaches supported by interview’s respond from the governments. This study’s result shows that the village-owned enterprises shall collaborate in benefiting the village forest to escalate the village IDM level which also represents the achievement of Village SDGs. Abstrak Perhutanan sosial telah menjadi instrumen Pemerintah untuk meningkatkan kesejahteraan masyarakat. Hutan desa di Indonesia merupakan salah satu skema yang diakui dalam program perhutanan sosial nasional sejak tahun 2007. Potensi ekonomi pengelolaan desa hutan kini diakui dalam Perpu No.2/2022 tentang Cipta Kerja (Perpu Cipta Kerja) yang dirancang khusus untuk mengakselerasi perekonomian nasional. Kemudahan yang ditawarkan dalam peraaturan tersebut tidak mengubah cara pandang masyarakat dalam memanfaatkan hutan Desa melalui skema kerjasama antara Badan Usaha Milik Desa dengan kelompok lain. Oleh karena itu, penting untuk menggambarkan potensi kerjasama ini yang bertujuan untuk lebih memberikan kesejahteraan masyarakat desa. Penelitian ini menggunakan metode yuridis sosiologis yang didukung oleh hasil wawancara pemerintah terkait. Hasil penelitian menunjukkan bahwa BUMDes dapat berkolaborasi dalam memanfaatkan hutan desa untuk meningkatkan level IDM desa yang juga merupakan pencapaian Tujuan Pembangunan Nasional Desa (TPB Desa). Pada akhirnya, ini berkontribusi pada pencapaian Tujuan Pembangunan Berkelanjutan Nasional (TPB Nasional). Background: Tuberculosis (TB) and lung cancer cause significant morbidity and mortality worldwide and pose a global health threat. Each year these two diseases account for more than 1.6 million deaths worldwide. The incidence of both diseases is still high in many developing countries, especially in Asian countries. TB and lung cancer are often confused and misdiagnosed, especially in countries with diagnostic challenges of low TB incidence and risk of missed diagnosis. Case Presentation: The following is a case report of a 53-year-old male patient diagnosed with pulmonary TB accompanied by right lung cancer, and the same respiratory complaints can be had by lung cancer and TB. However, the presence of facial edema (part of the superior vena cava syndrome) causes clinicians to focus more on lung cancer so that the diagnosis of TB is often overlooked. Conclusion: Tuberculosis should be a significant concern, especially in patients with malignancies such as lung cancer and located in TB endemic areas. Delay in diagnosis and or miss diagnosis will affect the patient's outcome.

The Covid-19 pandemic has harmed the sustainability of economic activities in Indonesia. Central Bangka Regency, the Covid-19 pandemic, has also harmed business actors. The purpose of this study was to analyze the mapping of the creative economy sub-sector in Central Bangka Regency and its survival during the Covid-19 pandemic. This study uses a mixed-method analysis approach. Quantitatively, this analysis uses linear regression analysis to identify the influence of government policies on the survival of creative economy businesses during the pandemic. Qualitative analysis is carried out by identifying data on the distribution of the creative economy that still persists during the pandemic and the factors that influence it both internally and externally using the Quantitative Strategic Planning Matrix (QSPM). The results of the analysis obtained are the weighted IFE value of 2.350 and the weighted EFE value of 2.756 which can be plotted in the Internal-External matrix. This shows that the creative economy business in Central Bangka Regency is currently in a period of growth because it previously experienced a reduction in production and distribution during the Covid-19 Pandemic. Meanwhile, based on the results of the QSPM analysis, the main strategy that must be carried out during this pandemic is to strengthen promotions using the digital marketing concept. Meanwhile, based on the results of linear regression analysis, it was found that there was a positive relationship between the survival of the creative economy business during the pandemic and the creative economy management program in the Central Bangka Regency.

The revitalization and preservation of Taman Ujung Soekasada cultural heritage area make this building used as a heritage tourism destination in Karangasem. The purpose of this research is to analyze the uniqueness of Taman Ujung Soekasada cultural heritage as heritage tourism and to find out people's perceptions about the development of the area into a tourist destination in Amlapura City. This study uses a mixed-method that combines two analyzes, namely quantitative and qualitative. Quantitative analysis was obtained through a perceptual survey of one hundred respondents to find out their opinion on the management of Ujung Soekasada Park as a heritage tourism area. Qualitative analysis was carried out by identifying spatial conditions, spatial planning, and building patterns in Ujung Soekasada Park, Amlapura. The physical elements contained in the traditional architecture of Taman Ujung Soekasada have a high value if it can be managed properly to become a tourist destination, especially to provide added value to community economic activities such as increasing micro-businesses, selling local community handicrafts, staging cultural arts and activities. other. Based on the results of the analysis, it is obtained the identification of the perceptions of the visitor community and tourism actors that they strongly agree to use Ujung Soekasada Park as a cultural tourism area by displaying the potential of traditional works of buildings, with a percentage of 86.57% hope that it can encourage tourists to come to Amlapura City, so that it can encourage progress of community economic activities around the tourist center.

Trenggalek is one of the potential regencies in East Java, with many beautiful tourist destinations. Realizing this fact, the government of Trenggalek has set a target to develop 100 tourist villages in Trenggalek by 2024. Tourist villages are now becoming one of the main attractions in Trenggalek. To make the villages more known and visited by tourists, some support is needed, one of which is the involvement of tour guides skilled in using English. Therefore, the community service team from UPN "Veteran" Jawa Timur under the scheme PIKAT (Pemanfaatan Ipteks Bagi Masyarakat = Utilization of Science and Technology for Society) held an English for Tour Guide program in Pandean Village, Dongko Sub-district, Trenggalek. Supported by the Department of Tourism and Culture of Trenggalek. This program has been held since July and is still running. The service program consists of three main activities, namely: (1) preparation, debriefing, and pre-departure; (2) implementation; and (3) evaluation. The tour guides joined and attended the program enthusiastically. They were actively involved in the discussions, sharing sessions, and role-plays. It is expected that this program can be impactful for tour guides in tourist villages in Trenggalek and can be extended in a broader scope in the future. In addition, more tourists are interested in visiting Trenggalek, from East Java and around Indonesia.

The purpose of this research is to analyze the influence of financial ratios (Return on Equity, Current Ratio, Debt to Equity Ratio, Total Assets Turnover) in predicting the probability of default. The samples in this study were 22 companies. Data was collected using library study methods and documentation which were analyzed using the Eviews program. The results of the research obtained are as follows: (1) Return on Equity (ROE) affects negatively the probability of default, (2) Current Ratio (CR) does not affect the probability of default, (3) Debt to Equity Ratio (DER) affects positively the probability of default, (4) Total Assets Turnover (TAT) affects negatively the probability of default. Abstrak dalam Bahasa Indonesia Tujuan dari penelitian ini adalah untuk menganalisis pengaruh antara rasio keuangan (Return on Equity, Current Ratio, Debt to Equity Ratio, Total Assets Turnover) dalam memprediksi kemungkinan gagal bayar (probabilitas default). Jumlah sampel dalam penelitian ini adalah 22 perusahaan. Data dikumpulkan dengan menggunakan metode studi pustaka dan dokumentasi yang kemudian dianalisis menggunakan program Eviews. This study aims to determine the effect of IFRS convergence on earnings management using corporate governance as a moderating variable. The population in this study are manufacturing companies listed on the Indonesia Stock Exchange during the period of 2015-2017. This study used a purposive sampling method to select 38 companies with 114 units of analysis. The results indicate that IFRS convergence do not affect earnings management. The results also show that managerial ownership and auditor quality cannot moderate the influence of IFRS on earnings management. Meanwhile, the number of audit committee meetings can alter the effect of IFRS convergence on earnings management.

Kopi luwak adalah kopi bernilai tinggi dengan aroma dan rasa khas, yang dihasilkan dari biji kopi yang telah difermentasi sebagian dalam sistem pencernaan luwak (Paradoxurus hermaprodithus). Tingginya permintaan kopi ini dan produksi alami yang unik berimplikasi pada praktek pemaksaan dan penyiksaan hewan luwak, serta adanya kontroversi status kehalalan produk. Fermentasi in vitro menggunakan selulase, xilanase, dan protease yang diisolasi dari kotoran luwak tampaknya dapat menjadi alternatif proses untuk menghindari masalah pelecehan hewan, menjamin kehalalan produk serta memastikan keberlanjutan produksi kopi luwak. Kajian ini bertujuan untuk meringkas dan mensintesis peran tiga enzim ini (selulase, xilanase, dan protease) dalam fermentasi in vitro kopi luwak dan pengaruhnya pada karakteristik kimia dan sensori berdasarkan hasil-hasil penelitian terbaru. Hasil kajian menunjukkan bahwa fermentasi in vitro menggunakan selulase, xilanase, dan protease. biji kopi menghasilkan karakteristik kimia dan sensorik yang sama dengan yang dihasilkan melalui proses alami yang melibatkan hewan luwak.

Abstract :This research suggests that increasing economic growth effect on land use Banjarmasin called the increasing needs of economic land, which in turn affects the conversion of agricultural land, swamp and bush into residential land, industry and trade. In Islamic view due to conditions that occur in Banjarmasin is like two poles masalih and mafasid mutual attraction between infltration needs such as paddy felds, swamps and thickets with the residential land needs which important for the beneft entirely. This condition can be minimized by the efforts of economic equality through justice and equitable development as well as economic growth in the Islamic concept. Abstract : Penelitian ini menunjukkan bahwa peningkatan pertumbuhan ekonomi berpengaruh terhadap tata guna lahan kota Banjarmasin yakni meningkatnya kebutuhan lahan ekonomi yang pada akhirnya berdampak pada alih fungsi lahan pertanian, rawa dan semak menjadi lahan pemukiman, industri dan perdagangan. Dalam pandangan Islam kondisi yang terjadi pada kota Banjarmasin ini ibarat dua kutub masalih dan mafasid yang saling tarik menarik antara kebutuhan lahan resapan seperti sawah, rawa dan semak dengan kebutuhan terhadap lahan pemukiman yang seluruhnya penting bagi kemaslahatan. Kondisi ini dapat diminimalisir dengan upaya pemerataan ekonomi melalui keadilan dan pemerataan pembangunan sebagaimana konsep pertumbuhan ekonomi dalam Islam.

Lagophthalmos is one of the most unpleasant and dangerous conditions that affect patients with facial palsy. The lack of ocular protection leads to corneal problems (such as conjunctival infections, acute and chronic keratitis, corneal ulcerations, and blindness). A dynamic reanimation of blinking eyelids is the therapeutic gold standard. However, success is not guaranteed with these dynamic techniques; even if results are good, blinking is usually restored within a year of the initial operation. Procedures that act more rapidly and have a higher success rate are needed. We proposed that lipofilling of the upper eyelid would improve eye closure, exploit the advantages of using autologous fat, and avoid the risks of exposure or migration associated with loading the lid with gold. Between 2012 and September 2018, we did upper eyelid lipofilling procedures for 75 patients with unilateral facial palsy. The main favourable result of lipofilling of the upper lid was the immediate improvement in corneal discomfort. Everybody described a partial to total increase in corneal comfort postoperatively. In the treatment of paralytic lagophthalmos, lipofilling of the upper eyelid produces favourable aesthetic and functional results, ocular health is restored, and the patients’ quality of life is improved.

22. Electrocautery, Harmonic, and Thunderbeat Instruments in Parotid Surgery: A Retrospective Comparative Study.

- Abstract
The aim of this retrospective study has been to compare the surgical outcomes of patients undergoing superficial parotidectomy with three different instruments: bipolar electrocautery, ultrasound, and mixed energy instruments. The clinical records of 102 patients who had undergone superficial parotidectomy for benign tumors between January 2016 and April 2022 were considered. Based on the tool used during the surgery, the patients were divided into three study groups: classic electrocautery hemostasis group (CH group), ultrasonic instrument group (HA group), and combined energy instrument group (TB group). The duration of surgery, the total post-operative drainage volume, and the intra-operative blood loss were significantly higher in the CH group compared to the HA and the TB group, while the differences were not significant between the latter two groups. Facial nerve weakness was detected in 45.9% of the CH group, 12.5% of the HA group, and 21.2% of the TB group. The rate of facial nerve dysfunction in the CH group was significantly higher than in the HA group (0.011). In the patients who experienced post-operative facial nerve dysfunction, the recovery time was significantly shorter in the HA group compared to the CH and the TB group. The HA and TB groups have demonstrated comparable and significantly better surgical outcomes than bipolar electrocautery. Ultrasound instruments have been shown to cause, in comparison with the other techniques, a lower rate of temporary facial nerve dysfunction and, if this is present, lead to a faster spontaneous recovery time.

- Abstract
Objectives: Arteriovenous malformations (AVM) are the most troublesome vascular malformations to deal with. They tend to behave like low-grade malignancies with infiltrative and disruptive growth. Crucially, the clinical course of an AVM that has been improperly managed is usually characterized by a recurrence that is much more aggressive than the original disease. As in oncology, a comprehensive staging system is highly desirable and is to date lacking in the literature. The authors present a new comprehensive staging system., Methods: A multicentric multidisciplinary team of experts in the field of vascular anomalies has created this new staging system. The SECg staging system defines the local extension of the disease (S1-S4), the vascular architecture of the malformation (E1, E2, E3), the severity of the symptoms (C0-C3) and the presence or absence of growth of the AVM (g+, g-)., Results: This staging system allows to address all the aspects of AVMs and, more importantly, to help building an appropriate, individualized treatment plan for affected patients. After being staged an AVM can be defined as (a) healable, (b) healable with predicted sequelae, or (c) unhealable. Then, the SECg system allows to outline (a) absolute indications, (b) relative indications, and (c) no indications for treatment. The purpose of the treatment (radical, palliative) is furthermore taken into consideration., Conclusions: This multicentric, the SECg staging system that this multidisciplinary group of Authors has defined allows for a comprehensive staging of the disease which in turn has enabled to outline an algorithm to properly manage AVMs.

Purpose: We aimed to evaluate the safety and effectiveness of percutaneous sclerotherapy using gelified ethanol in patients with low-flow malformations (LFMs)., Methods: A retrospective study was performed, analyzing treatment and outcome data of 6 patients that presented with 7 LFMs (3 lymphatic and 3 venous). Median diameter of LFMs was 6 cm (interquartile range [IQR], 4.5-8.5 cm). Data regarding pain, functional and/or cosmetic issues were assessed. Diagnosis was performed clinically and confirmed by Doppler ultrasound, while extension of disease was assessed by magnetic resonance imaging (MRI). Percutaneous puncture was performed with 23G needle directly or with ultrasound guidance. All the LFMs were treated with gelified ethanol injection. The median volume injected per treatment session was 4.4 mL., Results: Technical and clinical success were obtained in all cases. No recurrences were recorded during a median follow up of 17 months (IQR, 12-19 months). Among the 6 patients, 5 had complete relief (83%) and one showed improvement of symptoms. The median VAS score was 7 (IQR, 6-7.5) before and 0 (IQR, 0-0) after treatment. All patients had functional and esthetic improvement (100%). Four patients (66.7%) revealed very good acceptance and two patients (33.3%) good acceptance. No major complications or systemic side effects were observed., Conclusion: Gelified ethanol percutaneous sclerotherapy was easy to handle, well-tolerated, safe and effective in the short-term follow-up. Longer follow-up of efficacy is mandatory for further conclusions.

Vascular malformations are often found inside the orbit. Isolated venous malformations (frequently misnamed as cavernous hemangiomas) are the most frequent among these. However, also lymphatic and arteriovenous malformations can affect the orbit. The complex anatomy of the orbit and the fact that its content easily suffers from compartmental syndrome explain why treating orbital vascular malformations can be challenging and technically demanding. In this study, two institutions have retrospectively collected their cases, consisting in a total of 69 vascular malformations of the orbit. Each type of malformation has been evaluated separately in terms of diagnosis, indications for treatment, techniques and outcomes. Moreover, the authors have analyzed in detail venous malformations, identifying three different types, named orbital venous malformation (OVM) 1, 2 and 3. These behave differently from each other, and a prompt differential diagnosis is mandatory to pose correct indications, minimize risks and improve results. Overall, surgery was the technique of choice for OVM1, microcystic lymphatic malformations (LM) and arteriovenous malformations (AVM). A pure transnasal approach with mass removal and reconstruction of the medial wall with polyethylene sheets was chosen for OVM1 (intra- or extraconal) located in the medial or superomedial compartment. Capillary malformations (CMs) and Lymphatic malformations (LMs) are vascular malformations that share a common tendency to cause soft and hard tissue hypertrophy, expressed in the three dimensions of the space. The middle third of the face is the most commonly involved area, with a predominance of the upper lip. To our knowledge, no paper has previously focused on the surgical technique to correct such deformities. We propose and illustrate a surgical protocol, consisting in merging an asymmetric bikini upper lip reduction and a unilateral bull horn resection technique, that allowed to reduce the overall volume of the hypertrophied lip thus restoring lip symmetry. - Abstract
The aim of the study was to assess the mid-term effectiveness and safety of an original technique consisting of reconstructing fractures of the medial wall of the orbit with porous polyethylene implants with an exclusive transnasal approach. Twenty-five patients were treated. Each patient underwent a pre-operative ophthalmologic evaluation and a CT scan. The surgery started with an anteroposterior ethmoidectomy of the fractured side; all the fractured bone fragments were removed and all usual landmarks of healthy bony margins were identified. A Medpor sheet was placed endoscopically to reconstruct the fractured wall. Each patient received an immediate postoperative CT scan, and was evaluated at day 1, 7, 30 and 6 months after surgery clinically and with an endoscopic examination. In all patients, preoperative enophthalmos and/or diplopia were corrected. The CT scans showed excellent reconstruction of the fractured bony walls. The immediate postoperative period was characterized by a very high degree of subjective comfort. No perioperative complications were detected. At the 6 months follow up, all meshes appeared covered by epithelialized mucosa at the endoscopic inspection, and clinical results were stable. Scars or lid complications are always prevented. - Abstract
This paper examines a complex argument such as stress, focusing on those elements of the work environment that play a significant role in the generation of its symptomatology or exercising a protective function against it. Two factors have to be examined in order to understand and predict stress at work: the individuals' behavior and the organizational and environmental context. The scientific-pragmatic approach proposed by Behavior Analysis is examined. This approach emphasizes the need to evaluate the presence of coherent contingencies for each work group and, above all, the typology and the quality of consequences.
