Abstract

International audience; Objective:It is recognized that some cardiovascular risk factors (CVRF) are involved in poor ocular health. The deleterious effects are potentially more pronounced among older adults. However, it is unknown whether CVRF areassociated independently or not to Visual Impairment (VI) among African older people while both (CVRF and VI) are verycommon in sub-Saharan Africa. Therefore, we investigated the association between CVRF and near VI amongCongolese older people.Methods: Participants were Congolese older people aged ≥ 65 years included in EPIDEMCA-FU (Epidemiology ofDementia in Central Africa - Follow-up) population-based cohort carried out from 2011 to 2015. Visual acuity wasassessed at 30 cm using a Parinaud chart. Near VI was defined as having a visual acuity < 20/40. A cohort design wasused taking all CVRF and covariates at baseline and visual acuity at 1st follow-up. Associations were investigated usingmultivariate logistic regression.Results: 549 participants were included in our analyses. Participants median age was 72 years [interquartile range: 68-78 years] and 331 (60.3%) were females. In total, 378 had near VI (328 had mild/moderate near VI, 38 had severe VI and12 were blind). Prevalence of near VI was 68.8% [95% Confidence Interval: 64.9% - 72.7%] and higher in males (71.1%;95%CI: 65.1% - 77.1%) than females (67.4%; 95% CI: 62.3% - 72.4%). Of the CVRF explored, we found that having highBMI ≥ 25 kg/m2 (adjusted Odds Ratio = 2.31 [95%CI: 1.25 – 4.28]), diabetes (aOR=2.14 [95%CI: 1.07 – 4.29]) andhypertension (aOR=1.65 [95%CI: 1.03 – 2.66]) were independently associated with near VI. However, after adjustmenton cognitive status, hypertension (full-adjusted OR=1.58 [95%CI: 0.97 – 2.58]) was no longer significantly associated withVI. There was no interaction between these CVRF. We found no significant association for alcohol consumption(p=0.541), smoking status (p=0.88), and indirect marker of poor diet (0.33) in our sample.Conclusion: Prevalence of near VI was high among Congolese older people. High BMI, diabetes, and hypertension wereindependently associated to near VI in this population. Our study suggests that these CVRF could represent targets for VIprevention. However, further studies are required to clarify underling mechanisms because these associations may bemediated by the presence of ocular conditions/eye diseases.

Abstract

International audience; Aim of study: To determine the prevalence of cardiovascular disease and associated risk factors in the population of Gaza strip in Palestine. Methods: A cross sectional stratified cluster sample design was applied in this study. A sample of 2240 participant (1121 males and 1119 females) aged ???25 years participated in the study. For each individual, trained staff administered a questionnaire, where all variables of interest followed WHO???s STEP wise approach to surveillance chronic disease risk factors (STEPS) (WHO, 2001). Sociodemographic data, anthropometric measure (body mass index, blood pressure), and biochemical test (blood sugar and lipids profiles) were measured. Short International Physical Activity (IPAQ) questionnaire form was used. We used SPSS (version 22.0) to analyze the data. Results: The most common condition was coronary artery disease (8.3%), followed by cerebro vascular events (3%). The associated risk factors were obesity (47.8%), hypertension (28.4%), current smoking account for (23.2%), diabetes mellitus (19.1%), high cholesterol level (8.8%), and high triglycerides level (40.2%). Additionally, physical activity was found to be low (48.3%); particularly with increasing age. More than 30% of the population has less than 4 days of consumption of fruit and vegetables per week and 65.9% has less than 2 servings per day. Conclusion: The burden of CVDs and their associated risk factors is considerable in Gaza and represents a major public health concern. Effective strategies in management, education and healthcare centers are required for an accurate management and implementation of preventive measure in this area

We compared the prediction of major amputation by 3 hemodynamic methods. Patients were selected from the Cohorte des Patients ARTériopathes cohort of patients hospitalized for peripheral arterial disease. Patients with CLI were enrolled according to the Trans-Atlantic Inter-Society Consensus Document on Management of Peripheral Arterial Disease II definition and followed up for at least 1 year. Abstract

Background/Aims: Alzheimer disease (AD) is particularly devastating, with no cure, no means of prevention, and no proven way to slow progression. AD is associated with the worsening of cognitive function attributable to a variety of factors of which little is known. Our main objective was to determine factors associated with rapid cognitive decline (RCD) in older AD patients. Methods: We conducted a 12-month, prospective, multi-centre cohort study. Community-living individuals aged ≥65 years with mild-to-moderate AD were included. RCD was defined as the loss of ≥3 points/year in the Mini-Mental State Examination (MMSE) score. Potential individual-level predictors were collected at baseline. Results: A total of 521 individuals were included. The mean age was 80.8 ± 9.0 years and 66.0% were females. The average baseline MMSE score was 20.5 ± 4.5. The incidence of RCD was 40.9% (95% confidence interval [CI], 36.7–45.1). RCD was more common in patients with moderate (53.5%) than mild (22.3%) AD. The factors associated with RCD were: a parental history of dementia (odds ratio [OR], 2.32 [95% CI, 1.24–4.21], p = 0.011), psychotic symptoms (OR, 2.06 [95% CI, 1.22–3.48], p = 0.007), malnutrition (OR, 1.61 [95% CI, 1.06–2.63], p = 0.028), and the female gender (OR, 1.48 [95% CI, 1.03–2.15], p = 0.036). An MMSE score < 20 at treatment onset was also associated with RCD (p < 0.001). Conclusion: The factors associated with RCD were an MMSE score < 20 at treatment onset, female gender, psychotic symptoms, malnutrition, and a family history of dementia. These results may be directly relevant to patients, their families, and their physicians, enabling early anticipation of difficult clinical trajectories and poor functional outcomes.

International audience; En 2012, la Haute Autorité de Santé éditait des recommandations sur « le bon usage des antiplaquettaires ». Chez les sujets asymptomatiques, l’utilisation de l’aspirine était proposée lorsque le risque cardiovasculaire était élevé qu’ils soient diabétiques ou non (grade B) et en cas de sténose carotidienne (grade B). En cas de lésions d’athérosclérose symptomatique, en dehors des atteintes coronaires, les auteurs proposaient la prescription :– après un accident neurologique, soit d’asprine, de clopidogrel ou d’une association aspirine dipyridamole (toutes de grade A) ;– en cas d’artériopathie des membres inférieurs (AOMI), d’asprine en première intention ou de clopidogrel en cas d’intolérance ;– d’une bithérapie asprine-clopidogrel d’une durée d’un mois après mise en place de stent.Depuis lors, la situation a évolué avec l’apparition de nouvelles molécules qui, dans un premier temps, ont d’abord été évaluées dans des indications cardiologiques. Le vorapaxar, représentant d’une nouvelle classe de médicament antagoniste de l’agrégation induite par la thrombine (PAR-1), a reçu un agrément de la FDA chez les sujets avec une AOMI. De nouveaux antagonistes du récepteur P2Y12, en particulier le ticagrelor, sont l’objet d’étude (EUCLID) chez les sujets avec une AOMI symptomatique. Le ticagrelor est un inhibiteur direct, d’action rapide, réversible du récepteur P2Y12. Dans les études à visée coronaire, cette thérapeutique est associée à un moindre taux d’évènement thrombotique que le clopidogrel mais au prix d’un sur-risque hémorragique. D’autres antiplaquettaires sont en cours de développement avec des mécanismes divers à type d’inhibition des récepteurs P2Y12, ou PAR-1, d’inhibition directe ou de la synthèse du thromboxane (térotroban, picotamide, ridogrel) ainsi que du recepteur GPVI. L’utilisation des nouveaux antiplaquettaires orientera probablement vers une personnalisation des traitements afin d’obtenir le meilleur compromis entre les risques hémorragiques et thrombotiques.

Abstract

The object of this study was to compare the prognostic value of different methods of ankle–brachial index (ABI) calculation. From April 1998 to September 2008, we calculated the ABI in 1223 patients before coronary artery bypass grafting. The ABI was calculated according to five different calculation modes of the numerator. The patients were classified into three groups: clinical peripheral artery disease (PAD), subclinical PAD if no clinical history but abnormal ABI (< 0.90 or > 1.40), and no PAD. The primary outcome was total mortality. During a follow-up of 7.6 years (0.1–15.9), 406 patients (33%) died. The prevalence of the subclinical PAD varied from 22% to 29% according to the different modes of ABI calculation. Areas under the ROC curve to predict mortality according to different calculation modes varied from 0.608 ± 0.020 to 0.625 ± 0.020 without significant differences. The optimal ABI threshold to predict mortality varied for every method, ranging from 0.87 to 0.95. In multivariate models, ABI was significantly and independently associated with total mortality (hazard ratio (HR) = 1.46, 95% CI: 1.15–1.85, p = 0.002); however, this association was not significantly different between the various methods (HRs varying from 1.46 to 1.67). The use of the optimal ABI threshold for each calculation mode (rather than standard 0.90) allowed a slight improvement of the model. In conclusion, the ABI prognostic value to predict mortality is independent from its method of calculation.
