Le pied plat flexible affecte 20-25% de la population adulte. Il est caractérisé par un affaissement anormal de l’arche longitudinale médiale en charge ainsi qu’une pronation excessive du pied. Les orthèses plantaires (OPs) représentent la prise en charge conservatrice la plus fréquemment utilisée au regard de cette pathologie. Toutefois, il existe un manque de consensus quant à leur effet bénéfique, notamment à cause de la variété d’OPs (géométrie et matériaux) utilisée. Ces dernières années, le développement des techniques d’impression 3D a permis d’innover et de faciliter la production d’OPs sur-mesure. Les réalisations actuelles ne se sont cependant limitées qu’à des formes monolithiques reposant sur la forme du pied. L’objectif de cette thèse est d'approfondir les connaissances relatives à l’impact des OPs sur la biomécanique du pied plat, afin d’aider au développement et à l’évaluation d’une OP originale imprimée en 3D. À cet égard, trois objectifs spécifiques ont été définis : (1) investiguer l’effet de la forme géométrique des orthèses plantaires chez des personnes ayant des pieds plats flexibles ; (2) quantifier l’effet de la rigidité d’OPs imprimées en 3D et celle de l’addition d’éléments anti-pronateurs novateurs sur la cinématique du pied ainsi que les pressions plantaires ; (3) évaluer l’impact d’OPs sur-mesure imprimées en 3D sur la biomécanique des membres inférieurs chez des personnes ayant des pieds plats. Par la réalisation d’une revue systématique avec méta-analyse, l’effet de la forme géométrique des OPs sur la cinématique et la cinétique du membre inférieur lors de la marche chez des personnes ayant les pieds plats a pu être déterminé. Seules les études ayant clairement décrit les modifications géométriques des OPs utilisées ont été incluses. Elles ont ensuite été divisées en cinq groupes en fonction de leur forme géométrique : avec stabilisateur d’arrière-pied médial, avec stabilisateur d’avant-pied médial, avec combinaison d’un stabilisateur d’arrière-pied et d’avant-pied médial, avec stabilisateur neutre, et avec support d’arche. La revue a ainsi mis en évidence que l’utilisation de stabilisateur médiaux était la modification géométrique la plus efficace pour réduire l’éversion de l’arrière-pied et ainsi contrôler la pronation excessive. Cependant, l'hétérogénéité dans les protocoles expérimentaux contribue à la faible évidence au regard des effets des OPs sur la biomécanique de la marche chez des personnes ayant les pieds plats. Sur la base des observations tirées de notre revue de la littérature, des stabilisateurs d’arrière-pied innovants (neutre avec extension sous l’arche) ont été développés pour être utilisés avec une OP originale imprimée en 3D. Par conséquent, l’objectif de notre deuxième étude était de quantifier l’effet de ces stabilisateurs mais aussi de déterminer l’impact de la rigidité de notre OP sur la cinématique du pied et les pressions plantaires. Pour se faire, 15 hommes en bonne santé et ayant les pieds neutres (pointure 9.5-10 US) ont été recrutés afin de s’affranchir de l’interaction possible avec une pathologie. Ainsi, il a été mis en évidence qu’une augmentation de la rigidité était associée à une réduction plus importante de l’éversion à l’arrière-pied (Différence Moyenne (DM) = -0.83°). Cette dernière a d’autant plus été réduite par l’ajout de stabilisateurs (DM = -1.15° et -2.43°). Au niveau des pressions plantaires, outre le transfert de la charge vers le médio-pied induit par le port des OPs, l’augmentation de la rigidité a contribué à accentuer les pics de pression sous l’arche et l’arrière-pied (DM de +21.6% à +31.7%). Enfin, notre troisième étude avait pour but d’évaluer l’impact d’OPs personnalisées et imprimées en 3D sur la biomécanique des membres inférieurs chez des personnes ayant des pieds plats. Pour ce faire, 19 patients recrutés par l’intermédiaire de podiatres ont reçu deux paires d’OPs sur-mesure, respectivement flexible et rigide, et ont participé à une évaluation biomécanique (cinématique, cinétique, pressions plantaires). L’augmentation de la rigidité n’a eu que peu d’effets sur la cinématique et les efforts articulaires. Elle a cependant été associée à une augmentation des pressions sous l’arche (DM = +34.4% pour la pression moyenne). L’effet de notre stabilisateur a également été quantifié. Il a été associé à une réduction significative de l'éversion à l’arrière-pied (DM = -2.0°), une réduction du moment interne d'inversion à cheville (DM = -0.03 Nm/kg), et à une légère augmentation du moment interne d’abduction au genou (DM ≈ +0.04 Nm/kg). Dans l’ensemble, le présent travail de thèse a permis de mieux saisir les mécanismes d’action des OPs sur la biomécanique des personnes ayant les pieds plats, de guider le développement d’une OP imprimée en 3D et de stabilisateurs d’arrière-pied innovants, et de confirmer que l’ajout d’éléments anti-pronateurs est essentiel afin d’observer un impact bénéfique des OPs sur le contrôle de la pronation excessive., Flatfoot has been reported to affect around 20–25% of the adult population. It is defined by an abnormally low medial longitudinal arch upon weight bearing and an excessive foot pronation. Foot orthoses (FOs) have commonly been used as a conservative treatment to manage this deformity. However, due to the variety of FOs (geometrical designs and materials) that have been used, there is still low evidence of their beneficial effect. In recent years, the advent of 3D printing techniques has facilitated the production of innovative and customized FOs. Yet, current achievements are limited to monolithic form based on the foot shape. The objective of this thesis was to deepen the knowledge relative to FOs’ impact on flatfoot biomechanics, in order to help the development and the assessment of an original 3D printed FO. Three specific objectives were defined for this purpose: (1) investigate the effect of FOs, based on their geometrical design, in individuals with flexible flatfeet; (2) asses the effect of 3D printed FOs stiffness and newly designed anti-pronator components on foot kinematics and plantar pressures; and (3) evaluate the impact of custom 3D printed FOs on lower extremity biomechanics in individuals with flatfeet. Though a systematic and meta-analysis review, the effects FOs geometrical design on lower limb kinematics and kinetics during walking in people with flatfeet has been determined. Only studies that clearly described FOs geometrical design were included. They were then categorized into five groups based on the geometrical design of FOs: with medial rearfoot posting, with medial forefoot posting, with a combination of forefoot and rearfoot posting, with neutral rearfoot posting, and with arch support. The review highlighted that medial postings are the most effective FO feature to reduce the rearfoot eversion and therefore control excessive foot pronation. However, heterogeneity between study protocols contributes to low evidence of beneficial effects of FOs on flatfeet biomechanics during walking. Based on our literature review, innovative rearfoot postings (neutral with an extension under the medial arch) have been developed for an original 3D printed FO. Hence, our second study aimed to determine the effect of these postings as well as the stiffness of our FO on foot kinematics and plantar pressures. To do so, a study involving 15 healthy men with neutral feet (shoe size 9.5-10 US) was carried out. Healthy people were recruited to avoid any interaction with a pathology. The study showed that increasing FOs stiffness was associated to a greater reduction in rearfoot eversion (Mean Difference (MD) = -0.83°). Rearfoot eversion was further decreased when adding the rearfoot postings (MD = -1.15° and -2.43°). Looking at plantar pressures, besides a shift of the loads to the midfoot region while wearing FOs, higher peak pressures under the rearfoot and the medial arch (MD from +21.6% to +31.7%) were observed when increasing the FOs stiffness. Finally, the third study aimed at evaluating the impact of custom 3D printed FOs on lower extremity biomechanics in individuals with flatfeet. Nineteen patients, recruited by experienced podiatrists, were given two pairs of custom 3D printed FOs and participated in a biomechanical analysis (kinematics, kinetics, plantar pressures). Increasing FOs stiffness had little effects on kinematics and joint moments. However, it resulted in higher plantar pressures under the arch (MD = +34.4% for mean pressures). The addition of our rearfoot posting was associated with notable effects; it significantly reduced the eversion angle (MD = -2.0°) and inversion moment at the ankle (DM = -0.03 Nm/kg), and increased slightly the knee abduction moment (MD ≈ +0.04 Nm/kg). Overall, the present thesis has provided a better understanding on how FOs impact the biomechanics of individuals with flatfeet, helped the development of a 3D printed FO as well as innovative rearfoot postings, and confirmed that anti-pronator components are essential to observe a beneficial impact of FOs on the control of excessive foot pronation.

International audience; Background: Foot orthoses are commonly used to correct for foot alterations and especially address excessive foot pronation in individuals with flatfeet. In recent years, 3D printing has taken a key place in orthotic manufacturing processes as it offers more options and can be patient specific. Hence, the purpose of this study was to evaluate whether stiffness of 3D printed foot orthoses and a newly designed rearfoot posting have an effect on lower limb kinematics and kinetics in individuals with flatfeet.Methods: Nineteen patients with flexible flatfeet were provided two pairs of customized 3D printed ¾ length orthotics. Foot orthoses were of different stiffness and could feature a rearfoot posting, consisting of 2-mm carbon fiber plate. Lower limb kinematics and kinetics were computed using a multi-segment foot model. One-way ANOVAs using statistical non-parametric mapping, refined by effect sizes, were performed to determine the magnitude of the effect between conditions.Findings: Foot orthoses stiffness had little effect on midfoot and forefoot biomechanics. Reductions in midfoot eversion and forefoot abduction were observed during short periods of stance with rigid foot orthoses. Adding the posting had notable effects on rearfoot kinematics and on the ankle and knee kinetics in the frontal plane; it significantly reduced the eversion angle and inversion moment at the ankle, and increased the knee abduction moment.Interpretation: Using an anti-pronator component is more effective than increasing foot orthoses stiffness to observe a beneficial impact of foot orthoses on the control of excessive foot pronation in individuals with flatfeet.

Background Foot orthoses (FOs) are one of the most common interventions to restore normal foot mechanics in flatfoot individuals. New technologies have made it possible to deliver customized FOs with complex designs for potentially better functionalities. However, translating the individuals’ biomechanical needs into the design of customized FOs is not yet fully understood. Research question Our objective was to identify whether the deformation of customized FOs is related to foot kinematics and plantar pressure during walking. Methods The kinematics of multi-segment foot and FOs contour were recorded together with plantar pressure in 17 flatfoot individuals while walking with customized FOs. The deformation of FOs surface was predicted from its contour kinematics using an artificial neural network. Plantar pressure map and deformation were divided into five anatomically based regions defined by the corresponding foot segments. Forward stepwise linear mixed models were built for each of the four gait phases to determine the feet-FOs interaction. Results It was observed that some associations existed between foot kinematics and pressure with regional FOs deformation. From heel-strike to foot-flat, longitudinal arch angle was associated with FOs deformation in forefoot. From foot-flat to midstance, rearfoot eversion accounted for variation in the deformation of medial FOs regions, and forefoot abduction for the lateral regions. From midstance to heel-off, rearfoot eversion, longitudinal arch angle, and plantar pressure played significant role in deformation. Finally, from heel-off to toe-off, forefoot adduction affected the deformation of forefoot and midfoot. Significance This study provides guidelines for designing customized FOs. Flatfoot individuals with excessive rearfoot eversion or very flexible medial arches require more support on medial FOs regions, while the ones with excessive forefoot abduction need the support on lateral regions. However, a compromise should be made between the level of support and the level of increase in plantar pressure to avoid stress on foot structures.

FOs’ geometrical design and material properties have been shown to influence their impact on foot biomechanics. New technologies such as 3D printing provide the potential to produce custom shapes and add functionalities to FOs by adding extra-components. Research question The purpose of this study was to determine the effect of 3D printed FOs stiffness and newly design postings on foot kinematics and plantar pressures in healthy people. Methods Two pairs of ¾ length prefabricated 3D printed FOs were administered to 15 healthy participants with normal foot posture. FOs were of different stiffness and were designed so that extra-components, innovative flat postings, could be inserted at the rearfoot. In-shoe multi-segment foot kinematics as well as plantar pressures were recorded while participants walked on a treadmill. One-way ANOVAs using statistical non-parametric mapping were performed to estimate the effect of FOs stiffness and then the addition of postings during the stance phase of walking. Results Increasing FOs stiffness altered frontal and transverse plane foot kinematics, especially by further reducing rearfoot eversion and increasing the rearfoot abduction. Postings had notable effect on rearfoot frontal plane kinematics, by enhancing FOs effect. Looking at plantar pressures, wearing FOs was associated with a shift of the loads from the rearfoot to the midfoot region. Higher peak pressures under the rearfoot and midfoot (up to +31.7 %) were also observed when increasing the stiffness of the FOs. Significance 3D printing techniques offer a wide range of possibilities in terms of material properties and design, providing clinicians the opportunity to administer FOs that could be modulated according to pathologies as well as during the treatment by adding extra-components. Further studies including people presenting musculoskeletal disorders are required.

To compare our experience with N-butyl cyanoacrylate glue as the primary embolic agent versus other embolic agents for transcatheter arterial embolization (TAE) in refractory peptic ulcer bleeding and to identify factors associated with early rebleeding and 30-day mortality. Retrospective study of 148 consecutive patients comparing the clinical success rate in 78 patients managed with Glubran®2 N-butyl cyanoacrylate metacryloxysulfolane (NBCA-MS) alone or with other agents and 70 with other embolic agents only (coils, microspheres, ethylene-vinyl alcohol copolymer, or gelatin sponge) at a university center in 2008–2019. Univariate and multivariate logistic regression analyses were done to identify prognostic factors. The technical success rate was 95.3% and the primary clinical success was 64.5%. The early rebleeding and day-30 mortality rates were 35.4% and 21.3%, respectively. Rebleeding was significantly less common with than without Glubran®2 (OR, 0.47; 95% CI, 0.22–0.99; p = .047) and significantly more common with coils used alone (OR, 20.4; 95% CI, 10.13–50.14; p = .024). The only other factor independently associated with early rebleeding was having two or more comorbidities (OR, 20.14; 95% CI, 10.01–40.52; p = .047). Day-30 mortality was similar in the two treatment groups. A lower initial hemoglobin level was significantly associated with higher day-30 mortality (OR, 10.38; 95% CI, 10.10–10.74; p = .006). Fluoroscopy time was significantly shorter with Glubran®2 (20.8 ± 11.5 min vs. 35.5 ± 23.4 min, p = .002). Both groups (Glubran®2 vs. other agents) had similar rates of overall complications (10.7% vs. 9.1%, respectively, p = .786). Glubran®2 NBCA-MS as the primary agent allowed for faster and better clinical success compared to other embolic agents when used for TAE to safely stop refractory peptic ulcer bleeding. • Choice of embolic agent for arterial embolization of refractory peptic ulcer bleeding is still debated. We compared our experience with N-butyl cyanoacrylate (NBCA) glue vs. other embolic agents. • The use of Glubran®2 NBCA glue in the endovascular management of refractory peptic ulcer bleeding was significantly faster and more effective, and at least as safe compared to other embolic agents. • NBCA glue offers several advantages compared to other embolic agents and provides rapid hemostasis when used for arterial embolization to treat refractory peptic ulcer bleeding. It should be the first-line therapy.

Introduction: Pycnodysostosis, or Toulouse Lautrec's disease, is a rare lysosomal genetic disease with an autosomal recessive transmission, and it is classified as one of the osteo-chondro-dysplasias (ICD-10). Patients with pycnodysostosis manifest with several clinical features, including osteosclerosis, bone fragility, short stature and acro-osteolysis of the distal phalanges. Maxillofacial symptoms mainly associate a lack of maxillary and mandibular growth, open mandibular angle, and significant dental crowding. Observations: This work presents the case of two sisters suffering from pycnodysostosis as part of a family affected by it (mother and maternal aunt also affected). The first patient has received regular monitoring and oral care. The second one suffered from spontaneous mandibular osteomyelitis, which was difficult to treat and cure. Discussion: Oral symptoms of this disease make maintaining oral hygiene difficult and predispose to tooth decay and periodontal disease. The most common complications are mandibular osteomyelitis and pathological fractures. Pycnodysostosis must be differentiated from osteopetrosis and cleidocranial dysplasia. Conclusion: These cases illustrate the difficulty of managing the patients suffering from pycnodysostosis, whose treatment may be difficult.

STUDY QUESTION: Does In vitro maturation (IVM) of immature oocytes affect health, including growth at 2 years of age, in singletons born to mothers with polycystic ovary syndrome (PCOS)? SUMMARY ANSWER: This study of 92 singletons born after IVM in mothers with PCOS showed no significant differences in anthropometry and health outcome parameters in comparison with a cohort of 74 peers born after intracytoplasmic sperm injection (ICSI) and conventional controlled ovarian stimulation (COS) in mothers with PCOS. WHAT IS KNOWN ALREADY: IVM has been used worldwide in women with PCOS. However, the paucity of available data related to children's health following IVM is an important impediment to a more widespread use of the technology. Although previous reports on the neonatal outcome after IVM are generally reassuring, these studies have flaws that hamper the interpretation of outcomes. Moreover, few studies have reported on health outcomes after IVM beyond infancy, and particularly growth data in children born after IVM of immature oocytes from mothers with PCOS are lacking. STUDY DESIGN SIZE DURATION: This single-center cohort study compared anthropometry and health outcomes in 92 singletons born after ICSI of in vitro matured oocytes with 74 singletons born after ICSI without IVM (COS). All participants were born to mothers who were diagnosed with PCOS phenotype A, B, C or D and reached the age of 2 years between November 2012 and June 2019. Singletons born after COS were randomly selected for follow-up until young adulthood. PARTICIPANTS/MATERIALS SETTING METHODS: Anthropometric parameters and health status data were prospectively collected at birth, 4 months and 2 years in cohorts of singletons followed since birth. Results were adjusted for neonatal (birthweight z-score, birth order), treatment (day of transfer, number of embryos transferred, mode of transfer) and parental (maternal smoking, age, body mass index (BMI), anti-Müllerian hormone level, PCOS phenotype, gestational diabetes, hypertensive disorder and paternal BMI) characteristics. MAIN RESULTS AND THE ROLE OF CHANCE: Overall, no differences were found for bodyweight, height and head circumference z-score between IVM and COS children at birth, at 4 months or at 2 years (all P > 0.05). In addition, z-scores of waist and mid-upper arm circumference at 2 years were comparable in IVM and COS children. Adjustment for covariates did not change the conclusion. Surgical intervention rate as well as the hospital admission rate were comparable between the IVM and COS group (all P > 0.05). The proportion of children born to mothers with metabolically unfavorable PCOS phenotypes (A and C) was comparable in the two groups (52.1% in IVM and 45.9% in COS). Mothers giving birth to a child conceived using IVM were younger than mothers in the COS group but their BMI was comparable. LIMITATIONS REASONS FOR CAUTION: Although our study describes the largest cohort to date of singletons born after IVM applied in mothers with well-defined PCOS phenotypes, the current sample size only allowed us to detect moderate differences in anthropometry. Also, follow-up of children born after IVM for indications other than PCOS, for example fertililty preservation after cancer diagnosis, is highly recommended. WIDER IMPLICATIONS OF THE FINDINGS: We did not observe adverse effects of IVM on growth parameters in offspring ~2 years of age compared to COS, but future studies should focus on cardiovascular and metabolic outcomes in these children and adolescents given their mother's PCOS condition. Background Customized foot orthoses (FOs) are designed based on foot posture and function, while the interaction between these metrics and FO deformation remains unknown due to technical problems. Our aim was to predict FO deformation under dynamic loading using an artificial intelligence (AI) approach, and to report the deformation of two FOs of different stiffness during walking. Methods Each FO was fixed on a plate, and six triad reflective markers were fitted on its contour, and 55 markers on its plantar surface. Manual loadings with known magnitude and application point were applied to deform “sport” and “regular” (stiffer) FOs in all regions (training session). Then, 13 healthy male subjects walked with the same FOs inside shoes, where the triad markers were visible by means of shoe holes (walking session). The marker trajectories were recorded using optoelectronic system. A neural network was trained to find the dependency between the orientation of triads on FO contour and the position of markers on its plantar surface. After tuning hyperparameters and evaluating the performance of the model, marker positions on FOs surfaces were predicted during walking for each subject. Objective Foot orthoses (FOs) are popular treatment to alleviate several abnormalities of lower extremity. FO designs might alter lower extremity biomechanics differently, but the association is not yet known. This review aimed to evaluate how different FO designs, namely FO with medial posting, lateral posting, arch support, or arch & heel support, change lower limb kinematics and kinetics during walking. Literature survey Electronic database search were conducted from inception to March 2019, and 25 papers passed the inclusion criteria. Two independent reviewers checked the quality using a modified Downs and Black checklist (73.7 ± 5.5%) and a biomechanical quality checklist (71.4 ± 17.1%). Effect sizes for differences between with- and without- FO walking were calculated, and meta-analysis was performed whenever at least two studies reported the same variable. Results Medial posting reduced peak ankle eversion moment. Lateral posting brought about higher peak ankle dorsiflexion and peak ankle eversion for kinematics, as well as higher peak ankle abduction moment, lower peak knee adduction moment, and higher peak mediolateral ground reaction force (GRF) for kinetics. FOs with either arch support or arch & heel support tended to decrease vertical ground reaction force, but it was not significant. Conclusion The findings of this review reveal that medial or lateral posting work efficiently to change foot and knee kinematics and kinetics. However, the impact force is just slightly decreased by arch-supported and heel supported FOs. Due to the small number of available studies, and heterogeneity in meta-analysis findings, further research with more standardized biomechanical approach are required.

International audience; Within the framework of biocontrol development, three natural substances produced by Bacillus subtilis, called lipopeptides, have been studied: fengycin (F), surfactin (S) and mycosubtilin (M). Their antifungal properties were tested in vitro, in liquid medium, on two strains of Venturia inaequalis, ascomycete fungi causing apple scab. These two strains were respectively sensitive and less sensitive to tebuconazole, an active substance of the triazole family. These three molecules were tested on their own, in binary (FS, FM, SM) and ternary mixtures (FSM). The antifungal activities of lipopeptides were estimated by calculating an IC50, compared to tebuconazole chemical substance. In tests involving the sensitive strain, all lipopeptide modalities exhibited antifungal activity. However, modalities involving fengycin and its mixtures exhibited the best antifungal activities; the activity of fengycin alone being very similar to that of tebuconazole. Interestingly, regarding the strain with reduced sensitivity to tebuconazole, surfactin and fengycin alone were not efficient while mycosubtilin and the different mixtures showed interesting antifungal activities. Specifically, the antifungal activity of FS and FSM mixture were equivalent to that of tebuconazole. For both fungal strains, microscopic observations revealed important morphological modifications in the presence of fengycin and in a less important proportion in the presence of surfactin but not in the presence of mycosubtilin. Overall, this study highlights the diversity in mode of action of lipopeptides on apple scab strains.

Objective To investigate test-retest reliability of a hip strength assessment system (GroinBar). Design Test-retest reliability study. Setting Laboratory. Participants Twenty asymptomatic varsity soccer players. Main outcome measures Maximal isometric hip strength (adduction, abduction, internal and external rotation, flexion and extension) was assessed using the GroinBar. Intraclass correlation coefficient (ICC) and relative standard error of measurement (SEM) were calculated to evaluate reliability of peak (ICC3,1) (highest peak within 3 trials) and average peak (ICC3,3) (average of 3 trials) force and rate of force development (RFD). Hotelling's T2, were also used to compare bilateral and reciprocal ratios between dominant and non-dominant leg. Results ICC for both peak force and RFD values revealed moderate to good reliability (0.53–0.88 and 0.61–0.84, respectively), whereas reliability was good to excellent regarding their average values (0.77–0.95 and 0.81–0.92, respectively). SEM of average peak force and RFD values (4.1–9.4% and 8.2–13.9%, respectively) were lower than that of peak force and RFD values (5.7–13.0% and 10.7–19.1%, respectively). No significant difference was found in bilateral and reciprocal force ratios between dominant and non-dominant leg. Conclusions The GroinBar is a reliable tool to assess hip muscle function in athletic populations and could be used for player screening and follow-up.
