Ruxolitinib cream was approved in the US in September 2021 for the treatment of mild-to-moderate AD. This analysis describes treatment patterns before and after initiation of ruxolitinib cream among patients with AD.Patients and Methods: This retrospective, observational study used medical and pharmacy claims data from the Healthcare Integrated Research Database (HIRD®) and included adults and adolescents (aged ≥ 12 years) with an AD diagnosis, a first claim for ruxolitinib cream (index date) between October 2021 and July 2022, and continuous enrollment in a commercial or managed Medicare plan for 6 months before and after the index date. Analyses were also conducted in a subset of patients with a history of more advanced AD therapy (ie, systemic therapies, phototherapy, or ultrahigh-potent topical corticosteroids). Data from 6 months before ruxolitinib cream initiation (baseline period) and after initiation (follow-up period) were summarized using descriptive statistics.Results: Of 1,581 patients in the overall AD cohort, 749 had a history of more advanced AD therapy. During the follow-up period, 43.8% of patients did not receive any other AD treatment. Compared with baseline, fewer patients received topical corticosteroids (52.3% vs 30.4%), topical calcineurin inhibitors (13.9% vs 6.6%), and topical phosphodiesterase-4 inhibitors (4.4% vs 2.3%) during the follow-up period; slightly greater reductions were observed among the subset with a history of more advanced AD therapies. Oral corticosteroid use decreased from 20.9% to 15.5% overall and from 44.1% to 20.7% in the subset with more advanced baseline therapy. Among patients receiving biologics at baseline, 17.4% did not receive these treatments during the follow-up period.Conclusion: These 6-month follow-up data suggest that initiating ruxolitinib cream for AD may reduce the overall need for other topical treatments, oral corticosteroids, and biologics.Plain Language Summary: Ruxolitinib cream is a recently approved medicine used to treat atopic dermatitis (AD), a disease that causes itchy and dry skin. It is a topical Janus kinase inhibitor and not a steroid. We wanted to explore how the use of ruxolitinib cream affected the use of other medicines used to treat AD. Underserved populations are not well represented in clinical trials of immune checkpoint inhibitors (ICI).Methods: Demographics were tabulated for 194 patients treated with ICI at the Montefiore-Einstein Comprehensive Cancer Center (MECCC) between 2017 and 2022. Categorical variables were analyzed by Chi-squared test, and survival was analyzed using Kaplan–Meier (KM) curves.Results: MECCC patients were 40.7% Hispanic and 20.6% Black, compared with 3% and 2%, respectively, in the landmark IMbrave 150 study. Median overall survival (mOS) on ICI was 9.0 months, 25.0 months for the 100 (51.5%) favorable-prognosis Child Pugh A (CPA) patients included in HCC clinical trials. Disease control rate (DCR) was 58.5% among 123 evaluable patients per mRECIST 1.1. Baseline liver function, as defined by CP and the Model for End-Stage Liver Disease-Sodium (MELD-Na), correlated with survival (p < 0.001). Hepatitis C Virus (HCV) and alcoholism were over-represented relative to National Cancer Institute (NCI) data (56.2% vs 4.7% and 38.7% vs 8.2%, respectively). HCV treatment correlated with prolonged survival in infected patients (p = 0.0017). AFP decline correlated with response (p = 0.001). Hispanic patients lived longer when clinical variables were controlled for (mOS 52 vs 23 months; p = 0.011).Conclusion: In an underserved HCC population, ICI yielded a DCR of 58.5% and low rates of severe toxicity. 9. Characterization and Biological Activities of Silver Nanoparticles Synthesized using Grewia tiliifolia Vahl Leaf Extract

Background: An eco-friendly approach for the synthesis of noble metal nanoparticles employing various plant extracts has become of great interest in the field of nanotechnology. In the present study, the efficacy of Grewia tiliifolia leaf extract in reducing 1 mM silver nitrate to silver nanoparticles (AgNPs) has been reported for the first time. We also investigated the anticancer, antituberculosis and antioxidant activity. Methods: Characterization of biosynthesized AgNPs using G. tiliifolia leaf extract was evaluated by different techniques. Efficacy of biosynthesized AgNPs using G. tiliifolia leaf extract was tested for cytotoxicity against A549 Lung cancer cell lines by MTT Assay and against the infectious agent Mycobacterium tuberculosis using MABA assay. Further antioxidant activity was evaluated by DPPH radical scavenging assay. Results: The biosynthesis of AgNPs was evident by a color change of the reaction mixture from dark yellow to reddish-brown. Biofabricated AgNPs were further confirmed by characteristic surface plasmon absorption peak at 409 nm by UV-vis analysis. FTIR data reveals the presence of phytochemicals involved in bioreduction and biocapping of AgNPs, XRD analysis depicted the crystallographic nature of AgNPs. Further, size, charge, and polydispersity nature were studied using DLS (40.2 nm with polydispersity index 0.361) and Zeta potential (-35.8 mV). The morphology of AgNPs was determined by TEM analysis with a size ranging from 11-34 nm. The plant-derived AgNPs exhibited a cytotoxic effect on the lung cancer cell line with an IC50 value of 23.45 µg/ml and were also found to be effective against M. tuberculosis with a MIC of 6.25 µg/ml in comparison to the leaf extract (MIC 50 µg/ml). Antioxidant activity observed by AgNPs was moderate with IC50 value of 49.60 µg/ml. Conclusion: The findings indicate that the AgNPs synthesized from leaf extract of G. tiliifolia are eco-friendly, cost-effective, non-toxic and can be effective natural anticancer, antituberculosis and antioxidant agents.

13. Prevalence of extended-spectrum beta-lactamase producing gram negative isolates in Enterobacteriaceae group of bacteria in clinical samples from a tertiary care hospital

Introduction: Extended spectrum β-lactamases (ESBLs) are enzymes that intervene resistance to extended-spectrum (third generation) cephalosporins (e.g., ceftazidime, cefotaxime, and ceftriaxone) and but do not affect carbapenems (e.g., meropenem or imipenem). Though the number of ESBLs producing organism has been increasing day by day, the detection methods and treatment option for them are extremely limited. Objective: Objective of the study was to investigate the rate of ESBLs production and their antibiotic susceptibility pattern. Materials and method: A total 200 Gram negative isolates from various clinical samples received in microbiology laboratory, Sir Takhtsinhji General Hospital, Bhavnagar were studied and Antibiotic susceptibility test was done for commonly used antibiotics. A hospital-based study was conducted in microbiology laboratory, Sir Takhtsinhji General Hospital, Bhavnagar from February 2012 to August 2012. A total of 200 Gram negative isolates from various clinical samples were collected and identified using the conventional biochemical tests following the Clinical and Laboratory Standard Institute (CLSI) guidelines. Antimicrobial susceptibility testing (AST) was performed using the standardized Kirby-Bauer disk diffusion method. Results: Among the total isolates 89(44.5%) were ESBLs producer, and the rate of ESBLs positivity was 39.8% for E. coli (33 out of 83), 10% for Proteus mirabilis (1 out of 10), 51.4% for Klebsiella spp (55 out of 107). ESBLs producing organisms were resistant to most of the antibiotics but 100% were sensitive to imipenem, meropenem, and cefoperazone + sulbactam. Conclusion: Screening for ESBLs production requires to be carried out regularly in all clinical diagnostic laboratories to direct clinicians in appropriate selection of antibiotics.

Alopecias, both non-scarring and scarring, frequently occur in the context of LE and can assume several different patterns. Furthermore, alopecia occurring with LE may be considered LE-specific if LE-specific features are present on histology; otherwise, alopecia is considered non-LE-specific. Non-scarring alopecia is highly specific to systemic LE (SLE), and therefore has been regarded as a criterion for the diagnosis of SLE. Variants of cutaneous LE (CLE), including acute, subacute, and chronic forms, are also capable of causing hair loss, and chronic CLE is an important cause of primary cicatricial alopecia. Other types of hair loss not specific to LE, including telogen effluvium, alopecia areata, and anagen effluvium, may also occur in a patient with lupus. Lupus alopecia may be difficult to treat, particularly in cases that have progressed to scarring. The article summarizes the types of lupus alopecia and recent insight regarding their management. Data regarding the management of lupus alopecia are sparse and limited to case reports, and therefore, many studies including in this review report the efficacy of treatments on CLE as a broader entity. In general, for patients with non-scarring alopecia in SLE, management is aimed at controlling SLE activity with subsequent hair regrowth. Topical medications can be used to expedite recovery. Prompt treatment is crucial in the case of chronic CLE due to potential for scarring and irreversible damage. First-line therapies for CLE include topical corticosteroids and oral antimalarials, with or without oral corticosteroids as bridging therapy. Second and third-line systemic treatments for CLE include methotrexate, retinoids, dapsone, mycophenolate mofetil, and mycophenolate acid. Additional topical and systemic medications as well as physical modalities used for the treatment of lupus alopecia and CLE are discussed herein.Keywords: discoid, hair loss, cicatricial, scarring, non-scarring

Plexiform neurofibromas (PNs) occur in 20– 50% of NF1 cases, causing multiple morbidities and conferring a risk of malignancy. NF1 with PN is poorly characterized in the literature with regard to treatment patterns, healthcare resource utilization, and costs in the real world.Methods: This was a retrospective analysis of data from a commercial claims database in the US between October 2014 and March 2018. Persons with at least 1 diagnostic code for both NF1 and PN, aged ≤ 18 years on the index date, and continuously enrolled for ≥ 12 months before the index date were included. The index date was defined as the date of the first diagnosis of NF1 or PN during the study period, whichever occurred later. Healthcare resource utilization during follow-up included outpatient, inpatient, emergency room (ER), and pharmacy encounters; corresponding costs were calculated as the mean per patient per year (PPPY) in 2018 US dollars. Treatments were classified as PN surgery, pain medication, chemotherapy, radiotherapy, and targeted therapies. All analyses were descriptive.Results: A total of 301 patients were included. In the follow-up period, nearly all patients (99.7%) had outpatient visits, while 81.1% had pharmacy visits, 25.2% had ER visits, and 13.0% had inpatient visits. Mean ± SD [median] total healthcare costs PPPY were $38,292 ± $80,556 [$16,037]. During follow-up, 44.2% of patients used pain medications, 23.9% received chemotherapy, 5.0% underwent surgery for PN, 1.3% received radiotherapy, and 1.0% received targeted therapies.Conclusion: Commercially insured pediatric patients diagnosed with NF1 and PN were treated primarily with supportive care, highlighting a substantial unmet need in the United States.Keywords: neurofibromatosis type 1, plexiform neurofibromas, treatment patterns, healthcare resource use, treatment costs

The Canadian Patented Medicine Review Board (PMPRB), through new regulations, will apply HTA assessment results from the Canadian Agency for Drugs and Technologies in Health (CADTH) and Institut national d’excellence en santé et en services sociaux (INESSS) when setting the maximum price that can be charged for Category I patented medicines (treatments with an annual cost exceeding 150% of GDP per capita of Canada or with expected annual market size >$50M). Through these regulations, PMPRB has also established a willingness-to-pay threshold of CAD$200,000 or CAD$150,000 per quality adjusted life year (QALY) for medications with a prevalence of no more than 1 in 2000 across all approved indications. We reviewed the orphan drug submissions made to CADTH’s Common Drug Review (CDR) January 2015–May 2020 to understand how the methodology of assessing cost-effectiveness of orphan drugs has guided pricing in Canada. A total of 35 orphan drug submissions were assessed by CDR in this period, none of which met the willingness-to-pay threshold of CAD$50,000 per QALY. Only one drug met the CAD$200,000 per QALY for Therapeutic Criteria Level I, and two drugs met CAD$150,000 per QALY for other Therapeutic Criteria Levels proposed by PMPRB. Price reductions of 32– 99% were recommended for treatments that were approved in order to be listed for reimbursement. This review showed that the new PMPRB regulations could be creating challenges for manufacturers of rare disease treatments to meet Canadian pricing regulations. These regulations may jeopardize the launch of new medicines and limit opportunities to add to the development of real-world evidence of orphan drugs, which can be used in reimbursement approaches such as pay-for-performance.Keywords: orphan drugs, rare diseases, CADTH, QALY, HTA

As research moves toward improved diagnosis and therapy of aHUS, it will be important to better understand its epidemiology. Our objective was to conduct a systematic literature review to assess the incidence and prevalence estimates of aHUS globally. A comprehensive literature search was conducted in Embase and MEDLINE. Additionally, practice guidelines, databases of national/international organizations, and regulatory agencies were searched. From 2960 publications identified via MEDLINE and Embase, 105 publications were eligible for full-text screening, and a total of eight full-text articles met eligibility criteria for inclusion. Regional epidemiologic estimates were obtained for Europe and Oceania. Country-specific data were available for France, Norway, Australia, and Italy. Four of the identified studies reported on the prevalence of aHUS, prevalence in the age group of 20 years or younger was ranging from 2.2 to 9.4 per million population, while the only study that reported prevalence in all ages showed a prevalence of 4.9 per million population. Six studies reported on the incidence of aHUS, annual incidence in the age group of 20 years or younger was ranging from 0.26 to 0.75 per million population, and for all ages, annual incidence was ranging from 0.23 to 1.9 per million population. To our knowledge, this is the first systematic review conducted to provide a comprehensive overview of global incidence and prevalence estimates of aHUS. In general, incidence estimates were similar across all the studies; however, prevalence data were found to be more variable. Study limitations were related to inconsistencies in the definitions of aHUS between studies and also a dearth of epidemiological studies assessing incidence and prevalence of aHUS outside of Europe.Keywords: incidence, prevalence, aHUS, rare, orphan, kidney

- Abstract
Wilms tumor, the most common pediatric kidney cancer, resembles embryonic renal progenitors. Currently, there are no ways to therapeutically target Wilms tumor driver mutations, such as in the microRNA processing gene DROSHA. In this study, we used a "multiomics" approach to define the effects of DROSHA mutation in Wilms tumor. We categorized Wilms tumor mutations into four mutational subclasses with unique transcriptional effects: microRNA processing, MYCN activation, chromatin remodeling, and kidney developmental factors. In particular, we find that DROSHA mutations are correlated with de-repressing microRNA target genes that regulate differentiation and proliferation and a self-renewing, mesenchymal state. We model these findings by inhibiting DROSHA expression in a Wilms tumor cell line, which led to upregulation of the cell cycle regulator cyclin D2 (CCND2). Furthermore, we observed that DROSHA mutations in Wilms tumor and DROSHA silencing in vitro were associated with a mesenchymal state with aberrations in redox metabolism. Accordingly, we demonstrate that Wilms tumor cells lacking microRNAs are sensitized to ferroptotic cell death through inhibition of glutathione peroxidase 4, the enzyme that detoxifies lipid peroxides. Implications: This study reveals genotype-transcriptome relationships in Wilms tumor and points to ferroptosis as a potentially therapeutic vulnerability in one subset of Wilms tumor.

It has often been suggested that supernova remnants (SNRs) can trigger star formation. To investigate the relationship between SNRs and star formation, we have examined the known sample of 45 SNRs in the Large Magellanic Cloud to search for associated young stellar objects (YSOs) and molecular clouds. We find seven SNRs associated with both YSOs and molecular clouds, three SNRs associated with YSOs but not molecular clouds, and eight SNRs near molecular clouds but not associated with YSOs. Among the 10 SNRs associated with YSOs, the association between the YSOs and SNRs can be either rejected or cannot be convincingly established for eight cases. Only two SNRs have YSOs closely aligned along their rims; however, the time elapsed since the SNR began to interact with the YSOs' natal clouds is much shorter than the contraction timescales of the YSOs, and thus we do not see any evidence of SNR-triggered star formation in the LMC. The 15 SNRs that are near molecular clouds may trigger star formation in the future when the SNR shocks have slowed down to <45 km/s. Venous and Lymphatic Medicine (VLM) is a specialty that has benefitted from contributions from specialists from several medical disciplines. Optimally, the societies, boards, and residency review committees representing these disciplines would uniformly recognize the scope of VLM to develop education and assessment standards to allow training and identification of qualified practitioners. In order to inform the standard setting bodies and other stakeholders of the current scope of VLM, a task force of VLM experts from cardiology, dermatology, emergency medicine, general surgery, interventional radiology, vascular medicine, and vascular surgery was formed to revise a 2014 consensus document defining the core content of the specialty of VLM. We have used the NRAO Very Large Array (VLA) in conjunction with the Very Long Baseline Array (VLBA) Pie Town antenna as a real-time interferometer system to measure the size of the extragalactic source J1801-231 as a function of frequency from 1285 to 4885 MHz. These observations were made in an attempt to determine the effect interstellar scattering has on the observed sizes of OH (1720 MHz) masers in the nearby (d=2.5 kpc) supernova remnant W28. The observations clearly show that J1801-231 displays angular broadening due to turbulence in the Galaxy's interstellar medium. The minimum distance of the nearby (two arcminutes from J1801-231) pulsar PSR B1758-23 is constrained to be 9.4 +/- 2.4 kpc. This value is based on both the measured size of 220 mas for J1801-231 at 1715 MHz and the temporal broadening of the pulsar. A single thin scattering screen along the line of sight to the W28 OH(1720 MHz) masers must be at 4.7 +/- 1.2 kpc for this minimum pulsar distance. The screen may be placed closer to the Earth, but for reasonable values of the pulsar distance (i.e., the pulsar is within the Galaxy), this choice leads to a negligible scattering contribution to the sizes of the masers. Thus the OH(1720 MHz) masers, at a distance of 2.5 +/- 0.7 kpc, are unaffected by interstellar scattering, and the measured maser sizes must be intrinsic. 34. The Sizes of 1720 MHz OH Masers: VLBA and MERLIN Observations of the Supernova Remnants W44 and W28

We have used the NRAO Very Long Baseline Array (VLBA) to image OH(1720 MHz) masers in the supernova remnants W28 and W44 at a resolution of 40 mas. We also used MERLIN to observe the same OH(1720 MHz) masers in W44 at a resolution of 290 x 165 mas. All the masers are resolved by these VLBA and MERLIN observations. The measured sizes range from 50 to 180 mas and yield brightness temperature estimates from 0.3--20 x 10**8 K. We investigate whether these measured angular sizes are intrinsic and hence originate as a result of the physical conditions in the supernova remnant shock, or whether they are scatter broadened sizes produced by the turbulent ionized gas along the line of sight. While the current data on the temporal and angular broadening of pulsars, masers and extragalactic soures toward W44 and W28 can be understood in terms of scattering, we cannot rule out that these large sizes are intrinsic. Recent theoretical modeling by Lockett et al. suggests that the physical parameters in the shocked region are indicative of densities and OH abundances which lead to estimates of sizes as large as what we measure. If the sizes and structure are intrinsic, then the OH(1720 MHz) masrs may be more like the OH(1612 MHz) masers in circumstellar shells than OH masers associated with HII regions. Botulinum neurotoxin type A (BoNT-A) is effective in treating CD but little is known about its associated cost-effectiveness.Objective: To evaluate the cost-effectiveness of abobotulinumtoxinA for treating CD from the UK payer perspective.Methods: A Markov model was developed to evaluate the cost-effectiveness of abobotulinumtoxinA versus best supportive care (BSC) in CD, with a lifetime horizon and health states for response, nonresponse, secondary nonresponse, and BSC in patients with CD (mean age: 53 years; 37% male). Clinical improvement measured using Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS) was mapped to utility using data from a randomized trial of abobotulinumtoxinA. Health care resource use, costs, and other inputs were from the British National Formulary, Personal Social Services Research Unit, published literature, or expert opinion. Costs and outcomes were discounted at 3.5% per annum.Results: In the base case, the incremental lifetime quality-adjusted life-years (QALYs) gained from abobotulinumtoxinA arm versus BSC was 0.253 per patient, whereas the incremental cost was £7,160, leading to an incremental cost-effectiveness ratio (ICER) of £30,468 per QALY. One-way sensitivity analyses showed that these results were sensitive to the proportion of responders to abobotulinumtoxinA at first injection, duration between injections, the number of reinjections allowed among primary nonresponders, and any difference in baseline TWSTRS value between the BSC and abobotulinumtoxinA arms. Probabilistic sensitivity analysis showed that abobotulinumtoxinA was cost-effective 46% and 49% of times at thresholds of £20,000 and £30,000 per QALY, respectively. Scenarios are considered including vial-sharing, productivity losses, secondary response/nonresponse at subsequent injections, 5-year time horizon, and alternative reinjection intervals for BoNT-As produced ICERs ranging from cost-saving to £40,777 per QALY, versus BSC.Conclusion: AbobotulinumtoxinA was found to be cost-effective in treating adults with CD, at acceptable willingness-to-pay thresholds in the UK. - Abstract
Gender equity is the process of being fair to women and men. To ensure fairness, strategies and measures must often be available to compensate for women’s historical and social disadvantages that prevent women and men from otherwise operating on a level playing field. Equity leads to equality. Gender equality requires equal enjoyment by women and men of socially-valued goods, opportunities, resources and rewards. Where gender inequality exists, it is generally women who are excluded or disadvantaged in relation to decision-making and access to economic and social resources. Therefore a critical aspect of promoting gender equality is the empowerment of women, with a focus on identifying and redressing power imbalances and giving women more autonomy to manage their own lives. Gender equality does not mean that men and women become the same; only that access to opportunities and life changes is neither dependent on, nor constrained by, their sex. Achieving gender equality requires women’s empowerment to ensure that decision- making at private and public levels and access to resources is no longer weighted in men’s favor, so that both women and men can fully participate as equal partners in productive and reproductive life. Gender Inequality is deep penetrated in Indian society. Although it is a global phenomenon, but in the patriarchal Indian society,- it has spread its tentacles widely. Discrimination against women is found even before her birth in the form of female feticide and when she is born female infanticide has been found in practice.

- Abstract
The COVID-19 pandemic (and its aftermath) highlights a critical need to communicate health information effectively to the global public. Given that subtle differences in information framing can have meaningful effects on behavior, behavioral science research highlights a pressing question: Is it more effective to frame COVID-19 health messages in terms of potential losses (e.g., “If you do not practice these steps, you can endanger yourself and others”) or potential gains (e.g., “If you practice these steps, you can protect yourself and others”)? Collecting data in 48 languages from 15,929 participants in 84 countries, we experimentally tested the effects of message framing on COVID-19-related judgments, intentions, and feelings. Loss- (vs. gain-) framed messages increased self-reported anxiety among participants cross-nationally with little-to-no impact on policy attitudes, behavioral intentions, or information seeking relevant to pandemic risks. These results were consistent across 84 countries, three variations of the message framing wording, and 560 data processing and analytic choices. Thus, results provide an empirical answer to a global communication question and highlight the emotional toll of loss-framed messages. Critically, this work demonstrates the importance of considering unintended affective consequences when evaluating nudge-style interventions.

- Abstract
What are the symmetries of a dataset? Whereas the symmetries of an individual data element can be characterized by its invariance under various transformations, the symmetries of an ensemble of data elements are ambiguous due to Jacobian factors introduced while changing coordinates. In this paper, we provide a rigorous statistical definition of the symmetries of a dataset, which involves inertial reference densities, in analogy to inertial frames in classical mechanics. We then propose SymmetryGAN as a novel and powerful approach to automatically discover symmetries using a deep learning method based on generative adversarial networks (GANs). When applied to Gaussian examples, SymmetryGAN shows excellent empirical performance, in agreement with expectations from the analytic loss landscape. SymmetryGAN is then applied to simulated dijet events from the Large Hadron Collider (LHC) to demonstrate the potential utility of this method in high energy collider physics applications. Going beyond symmetry discovery, we consider procedures to infer the underlying symmetry group from empirical data.
