ABSTRACT: While food risks are increasing in many countries, consumers in Turkey are also worried about the risks of agricultural products. Uncontrolled agricultural production in Turkey both threatens consumer health and prevents export earnings. Agricultural products that cannot be exported in Turkey are easily marketed in the country. Low-income level in Turkey limits the consumption of certified safe food. This study particularly focused on consumers whose demands direct the production of safe food. This study determined the food safety perception of consumers, their demands for safe agricultural products with good agricultural logo, and their willingness to pay extra. Please no new line Data were obtained through face-to-face interviews with 422 individuals over the age of 18 residing in the city center of Tokat in Turkey. This study provided information on the necessity of good agriculture for consumer health, agriculture sustainability , sustainability of agricultural exports and willingness to pay for safe products with the GAP logo in Turkey. In Turkey, consumers’ awareness of food safety is low, only 20.85% of the participants know the concept of food safety and only 52% recognize the good agriculture logo. Twenty-seven per cent of the participants stated that they would pay 10% higher for products with good agriculture logos while 23% of them would pay 20% higher. According to ordered probit analysis, it was determined that there was a relationship between knowing the good agriculture logo, knowing the logo of organic agriculture, adopting innovations, marital status, and willingness to pay extra for safe food.

In this study, we obtain approximate solution for singularly perturbed problem of differential equation having two integral boundary conditions. With this purpose, we propose a new finite difference scheme. First, we construct this exponentially difference scheme on a uniform mesh using the finite difference method. We use the quasilinearization method and the interpolating quadrature formulas to establish the numerical scheme. Then, as a result of the error analysis, we show that the method under study is convergent in the first order. Consequently, theoretical findings are supported by numerical results obtained with an example. Approximate solutions curves are compared on the chart to provide concrete indication. The maximum errors and convergence rates obtained are given on the table for different varepsilon and N values.

Bu araştırmanın amacı, sınıf öğretmenlerinin yazma güçlüğüne ilişkin görüşlerini belirlemektir. Araştırmada nitel araştırma yöntemlerinden olgubilim kullanılmıştır. Araştırmanın çalışma grubunu ilkokullarda görev yapan 33 sınıf öğretmeni oluşturmaktadır. Araştırma verileri sınıf öğretmenlerinden, elektronik ortamda, oluşturulan açık uçlu sorularla toplanmıştır. Öğretmenlerden elde edilen veriler betimsel analiz tekniği ile incelenmiştir. Bu kapsamda elde edilen veriler araştırmacı tarafından kodlanmış ve temalara ayrılarak, sınıf eğitimi alanında doktora yapan bir araştırmacının ve bir sınıf öğretmeninin görüşüne sunulmuştur. Güvenirlik Miles ve Huberman tarafından geliştirilen formülle hesaplanmıştır. Değerlendiriciler arası güvenirlik 0.83 olarak belirlenmiştir. Araştırma sonuçlarına göre öğretmenlerin yazma güçlüğünün tanımı ve içeriği hakkında yeterli bilgiye sahip olmadıkları, yazma güçlüğü gösteren öğrencilerin benzer harflerde sorun yaşadıkları, yazma güçlüğünün nedenlerinin farklılık gösterdiği belirlenmiştir. Ayrıca sınıf içi ve sınıf dışı çalışmaların yazının daha çok mekanik yönüne yönelik olduğu, yazma güçlüğü gösteren öğrencilerin aileleri ve akranları ile olan iletişimlerinin genellikle iyi düzeyde olduğu belirlenmiştir. Son olarak öğretmenler pandemi sürecinin yazı çalışmalarını olumsuz yönde etkilediğini ve bu konuda desteğe ihtiyaç duyduklarını belirtmişlerdir.

Aort koarktasyonu dikkatli bir fizik muayene ve ekokardiyografi ile bebeklik ve çocukluk çağında kolayca tanı konulabilen doğumsal kalp hastalıklarından biridir. Erken tanı alamayan hastalar koarktasyonunun şiddetine bağlı olarak bebeklik, çocukluk ve erişkin dönemde çeşitli komplikasyonlar ile başvururlar. Aort koarktasyonu olan vaklarda ağır komplikasyonlar gelişebileceği için aort koarktasyonu teşhis edilir edilmez vakaların yaşına ve koarktasyonun durumuna göre stent implantasyonu, balon dilatasyonu veya cerrahi onarım yapılması hayati önem taşımaktadır. Biz burada ateş ve karın ağrısı ile müracaat eden ve yapılan ekokardiyografide aort koarktasyonu saptanan 6 yaşında bir hastayı sunduk.

Purpose: The aim of this study was to compare the effects of hyperbaric oxygen therapy (HBOT) induced hyper-oxygenation and normobaric oxygen therapy (NBOT) on myocardial repolarisation parameters in children with carbon monoxide (CO) poisoning. Materials and Methods: This prospective study included 77 girls and boys aged 0–18 years who were diagnosed and treated for CO poisoning. There were no changes in the routine clinical evaluation and treatment practices of patients. Patients who received NBOT (n=40) and HBOT (n=37) were divided into two groups. These groups were compared in terms of their demographic characteristics, carboxyhaemoglobin, lactate, troponin levels and myocardial repolarisation parameters of electrocardiography (ECG) (Tp-e interval, Tp-e dispersion, QTc and Tp-e/QTc ratio). Results: There were no significant intergroup differences in terms of carboxyhaemoglobin, lactate and troponin levels at the time of admission; admission and post-treatment Tp-e, Tp-e dispersion, corrected QTc and TPe/QTc ratio and post-treatment change rates of each ECG parameter. Conclusion: There was no intergroup difference in terms of repolarisation parameters of ECG in children with CO poisoning. The possible reason for this may be myocardial reperfusion damage due to hyper-oxygenation associated with HBOT therapy.

Sürdürülebilir kalkınma yerel dinamiklerin harekete geçirilmesi ile toplumun sosyal, kültürel ve ekonomik olarak iyileşmesini amaçlar. Bu ancak, çevresel değerlerin korunması ve ekonomik kalkınma arasındaki uzlaşma ile gerçekleşebilir. Bu çalışma, Cittaslow ve SlowFood hareketinin, bir yerel kalkınma modeli olabileceği fikrini tartışmak amacı ile hazırlanmış olup Niksar ilçesini kapsamaktadır. Çalışmada Niksar’ın Cittaslow’a ait 72 kriter açısından değerlendirilmesi yapılmıştır. Değerlendirmelere sonucunda; 72 kriterin 22 tanesini sağlamayıp, 50 âdetinde ise plan ve proje bulunmaktadır. Niksar İlçesi’nin üyelik potansiyelinin pozitif olduğu tespit edilmiştir. İkinci olarak Niksar’da yaşayan kişilerin Cittaslow algılarını ve desteklemeye dair görüşlerini belirlemek için 88 kişiyle anket yapılmıştır. Verilere faktör analizi, anova ve t testi uygulanmıştır. Analiz sonucunda Niksar’ın Cittaslow unvanına sahip olmasını isteyenlerin ve Cittaslow kavramını bilme oranının yüksek olduğunu, Niksar’da yerel kalkınmanın turizm ve yerel değerlerin tanıtımı ile sağlanacağı fikri belirlenmiştir.

Tıbbi ve aromatik bitkiler (TAB), tüketicilerin doğal beslenme ve yaşama olan ilgisinin artmasıyla birlikte son yirmi yılda ön plana çıkmıştır. Türkiye, bulunduğu coğrafi konum, sahip olduğu iklim ve bitki çeşitliliği sayesinde TAB açısından önemli bir yere sahiptir. Oldukça geniş bir varyete gösteren TAB’ın; lezzet, koku, tat verici, antimikrobiyal, antioksidan ve terapötik gibi etkileri nedeniyle çeşitli formlarda (bütün, öğütülmüş, toz, ekstrakt, ekstre, hidrosol veya uçucu yağlar) kullanımları mevcuttur. Bileşimlerindeki sekonder metabolitlerden dolayı tıp ve eczacılık alanlarının yanı sıra fonksiyonel gıda bileşeni ve takviye edici gıda olarak kullanımları da önem taşımaktadır. Son yıllarda dünya ölçeğinde artarak yaşanan küreselleşme, kentleşme, değişen yaşam koşulları ve beslenme alışkanlarının farklılaşması ile birlikte ortaya çıkan yetersiz ve dengesiz beslenme sonucu tüketicilerin takviye edici gıdalara yönelimini artırmıştır. Takviye edici gıdalar pazarında TAB’ın kullanımına daha fazla yer verilmesiyle bu alanda gerekli düzenlemelere gidilmiştir. Bu derlemede, TAB ile bunların takviye edici gıda üretiminde kullanımı üzerine bazı bilgilere yer verilmiştir.

The study aimed to investigate the suitability of some food wastes for use in the production of herbal tea and to reveal their properties in terms of bioactive component content, some physical properties and sensory quality. Three new blends consisting of banana, pomegranate, mandarin, eggplant and red onion peels, walnut shell, cherry stalk and corn tassel were infused at different temperatures (70 and 100 °C) and time (3, 4 and 5 min). Corn tassel tea, which has a high phenolic component content (677.7 mg GAE/L), received less sensory acceptance due to its bitter aftertaste. Therefore, walnut shell tea containing moderate phenolic content was the most preferred tea blend. Significant effects of different infusion temperatures and times on the color indices of teas were also demonstrated. The sensory properties of samples infused for 3 min were more acceptable. The highest correlation coefficients were calculated between total phenolic compounds and subjective odor parameters. Utilization of mandarin, pomegranate, banana, eggplant and red onion peels, walnut shell, corn tassel and cherry stalk in formulating a functional and an alternative food product can be attractive to consumers and industrial producers due to their affordability and high bioactivity.

Bu araştırmada ilköğretim birinci sınıf öğrencilerinin dik temel yazı okunaklılığını ve hızını incelemek amacıyla betimsel tarama yöntemi kullanılmıştır. Çalışma 2019-2020 eğitim öğretim yılının birinci döneminde Burdur’da bir devlet okulunda eğitim gören 13’ü kız, 12’si erkek olmak üzere 25 birinci sınıf öğrencisine uygulanmıştır. Veri toplama aracı olarak “Dikte Çalışması Takip Formu” ve “İlkokullar İçin Hikâye Yazmaya Yönelik Bütüncül Puanlama Yönergesi” birinci sınıflara uyarlanarak kullanılmıştır. Öğrencilerin yazıları üç aralıklı boş satırlardan oluşan kâğıtlara alınmıştır. Öğrencilere dikte ve bakarak yazma çalışması için 3 dakika, serbest yazma için 20 dakika süre verilmiştir. Araştırmadan elde edilen bulgulara göre çalışmaya katılan birinci sınıf öğrencilerinin çoğunlukla eksik harf ve yanlış harf yazma hataları yaptıkları görülmüştür. Öğrenciler bakarak yazma çalışmasında dikte ve serbest yazmaya göre daha hızlı yazmaktadırlar. Öğrencilerin serbest yazmada metne uygun başlık koyabilme becerileri yeterli düzeydeyken olay örgüsü, yer, zaman, kişiler ve ana fikir belirtme becerileri yetersiz düzeydedir.

Amaç: Bu çalışmanın amacı akut romatizmal ateşle ilişkili internet tabanlı hasta eğitim materyallerinin okunabilirliğini değerlendirmektir.Gereç ve Yöntem: Herbir anahtar kelime (“akut romatizmal ateş”, “akut eklem romatizması”, “romatizmal ateş”, “akut romatizmal kardit '',“ romatizmal kardit '') için Google arama motorunda (www.google.com) 50 site olmak üzere toplam 250 web sitesi değerlendirildi. On cümleden daha az bilgi içeren siteler, sohbet, forum ve ticari blog siteleri çalışma dışı bırakıldı. Ortalama kelime sayısı, ortalama hece sayısı ve ortalama hece sayısı 4 ve üzerinde olan kelimeler hesaplanmıştır. Ortalama okunabilirlik seviyesi Ateşman ve Bezirci-Yılmaz okunabilirlik formülleri kullanılarak analiz edilmiştir.Bulgular: Hariç tutma ölçütleri uygulandıktan sonra 250 web sitesinin toplam 50'si araştırma için uygun bulundu. İncelenen sitelerden 33'ü sağlık uzmanları ve 17'si sağlık personeli olmayan kişiler tarafından oluşturulmuştur. Ateşman formülüne göre web sitelerinin okunabilirliği orta derecede zor, Bezirci-Yılmaz formülüne göre lisans düzeyinde insanlar tarafından anlaşılabilir bulunmuştur.Sonuç: Web sitelerindeki akut romatizmal ateş alanıyla ilgili hasta eğitimi metinlerinin okunabilirlik seviyesinin akademik okuryazarlık düzeyinde olduğu belirlenmiş ve bu bilgilerin Türkiye'deki ortalama eğitim düzeyi ile anlaşılması güç olacağı düşünülmektedir.

Research background. Utilization of wheat germ and wheat germ oil is limited due to high enzymatic activity and the presence of unsaturated fatty acids, which require stabilization techniques to overcome this problem. Experimental approach. In this study, the effects of stabilization methods (dry convective oven heating at 90 and 160 °C, microwave radiation at 180 and 360 W, and autoclave steaming) on both wheat germ and its oil were evaluated. Results and conclusions. Steaming caused the most dramatic changes in lipoxygenase activity, free fatty acid content, DPPH radical scavenging activity, and mass fractions of tocopherols and tocotrienols. Lower peroxide values were measured in the oil samples treated with convectional heating (160 °C) and steaming at temperatures above 100 °C. However, p-anisidine values of samples treated at higher temperatures were considerably greater than those of samples stabilized at lower temperatures. Oven heating at 160 °C was also one of the most effective treatments, after steaming, for the inactivation of lipoxygenase. Steaming significantly reduced mass fraction of total tocopherols, which was directly associated with the greater loss of β-tocopherol content. On the contrary, γ- and δ-tocopherol and tocotrienol homologues were abundant with higher amounts in steamed samples. α-Tocopherol and γ-tocotrienol were the most resistant isomers to stabilization processes. Novelty and scientific contribution. This study shows that the high temperature oven heating method, which is widely used in the industry for thermal stabilization of wheat germ, does not provide an advantage in oxidative stability compared to steaming and microwave applications. Steaming delayed oxidation in the germ, while further inhibiting lipoxygenase activity. Moreover, tocotrienols were more conservable. In industrial application, low-power microwave (180 instead of 360 W) and oven heating at lower temperature (90 instead of 160 °C) would be preferable.

The effects of different boron (B) dosages (0, 2 and 12 mg B/kg) were determined on four soybean (Glycine max (L.) Merr.) cultivars, namely 13935, Türksoy, ME 3399 and Deficiency. B contents of the dried plant samples, dry weight, total oil, biomass, seed yield (g/pot), seed protein contents and seed fatty acid compositions were estimated. The seed protein content and shoot dry weight of soybean cultivars increased and decreased with B supply, respectively. The seed oil of cv. Türksoy had the highest ratio of stearic and oleic acids under 2 mg B/kg treatment. The highest total oil content under 12 mg B/kg treatment was observed in cv. Deficiency with 8% higher total oil content. The ratio of saturated fatty acids to unsaturated fatty acids decreased in cvs. 13935 and ME 3399, and increased in cvs. Türksoy and Deficiency at B treatments. Seeds oil of cvs. 13935 and ME 3399 showed the highest α-linolenic acid levels under 2 mg B/kg and 12 mg B/kg soil treatment, respectively. The study revealed that high concentrations of boron had a diminishing effect on seed yield (except cv. Türksoy), increasing effect on protein content and variable effect on saturated and unsaturated fatty acid compositions. This specifies the involvement of boron in the formation of seed protein and fatty acids in soybean. However, detailed research is required to understand the mechanisms behind the process.

Dill (Anethum graveolens L.) and parsley (Petroselinum crispum Mill.) were dried by using sun and oven (50oC) drying methods. Oven drying decreased the drying time approximately 50% when compared to the sun drying method for both herbs. K, Ca, P, Mg and Na were the most abundant elements in dill and parsley samples. Oven-dried samples had higher mineral values than that of the sun-dried samples. Page, Modified Page, Midilli and Küçük and Diffusion approach were shown to give a good fit to the sun drying of parsley. The Midilli and Küçük and Verma models have shown a better fit to the experimental sun drying data of dill. Wang-Singh and Midilli and Küçük models gave the highest r2 values for oven drying of parsley and dill, respectively.

Ultrasound pretreatment with different times (0, 4, 8, 10 min) on olive paste previous malaxation of olive oil extraction along with depitting and water application procedures were studied. The effects of these procedures on oil yield, quality parameters and oxidative stability have been studied. Therefore, the olive oil yield increase with increasing time of ultrasound treatment. On the other hand, the application of ultrasound did not adversely affect the quality characteristics, as well as the antioxidant activity, when comparing with untreated paste. Furthermore, the oxidative stability data, we can conclude that ultrasound treatment can affect the olive oil oxidative stability. This study could provide useful information for industry to produce olive oil with high yield and quality.

The aim of this paper is to present finite difference method for numerical solution of singularly perturbed linear differential equation with nonlocal boundary condition. Initially, the nature of the solution of the presented problem for the numerical solution is discussed. Subsequently, the difference scheme is established on Bakhvalov-Shishkin mesh. Uniform convergence in the second-order is proven with respect to the ε− perturbation parameter in the discrete maximum norm. Finally, an example is provided to demonstrate the success of the presented numerical method. Thus, it is shown that indicated numerical results support theoretical results.

Carpenter syndrome (Acrocephalopolysyndactyly type 2, OMIM 201000) is a rarely seen autosomal recessive disorder. In addition to abnormalities such as acrocephaly, craniosynostosis, facial asymmetry, polydactyly and syndactyly, obesity, hypogonadism, mental retardation, and corneal opacity, it may frequently be accompanied by congenital heart diseases such as ventricular septal defect, patent ductus arteriosus and pulmonary stenosis. Double outlet right ventricle is a defect in which both major arteries originate in the morphological right ventricle. To the best of our knowledge, this is the first report in the literature of double outlet right ventricle disease in combination with Carpenter syndrome.

Wheatgrass juice is mainly derived from the common wheat Triticum aestivum L. The present study focused on the analysis of the potential of different perennial turfgrass species in grass juice production by determining certain compositional characteristics. The effects of fertilisers on the plants and the cutting time on some chemical constituents and antioxidant potential of grass juices were addressed. The juices from the different species of grasses, such as T. durum, T. aestivum, Lolium perenne L., Festuca arundinacea Schreb. were obtained by pressing. DPPH (1, 1diphenyl-2-picrylhydrazyl) radical scavenging activity, elemental composition, total chlorophylls, total carotenes, tocopherols, vitamin C, total phenols, viscosity and the colour profiles of the grass juice samples were analysed. Water-soluble dry matter, protein, total phenols, flavonoids and vitamin E (especially Durum cultivar) content were found to be higher in the juices of Triticum species as compared to the other grasses. Turfgrass species demonstrated higher concentrations of oil (in grass), vitamin C (unfertilised samples), chlorophyll (except L. perenne local) and major elements. The pressing of turfgrass was an easy process. Such perennial grass varieties have advantages over the Triticum species in terms of having higher concentrations of vitamin C (unfertilised samples) and major elements. The results of sensory analysis suggested that L. perenne (cultivar) is the most promising cultivar in terms of obtaining pressed grass juice.

Olanzapine is an atypical antipsychotic drug, commonly used in the management of psychotic symptoms in patients with schizoprenia and bipolar affective disorder. Deep venous thrombosis is a manifestation of venous thromboembolism. It is very well known that use of antipsychotic drugs increase the risk of thrombosis in a patient with schizophrenia. It has been reported in many studies the effects of olanzapine on thrombosis but there isn’t any report about the effect of olanzapine on international normalized ratio (INR). in the medical literature. In this paper, a patient with schizophrenia and also family history of deep venous thrombosis who emerged deep venous thrombosis after being started on olanzapine treatment and effect of olanzapine treatment on INR has been reported.

BACKGROUND AND OBJECTIVES: It has been demonstrated that smoking increases pain perception; however the effect of smoking on perception of pain during venous cannulation is not known. The purpose of this study is to determine whether or not smoking has an effect on pain perception due to peripheral venous cannulation. METHODS: 220 patients scheduled to have elective surgery were enrolled in the study and were divided into two groups (Group S and C, n = 110 for each) according to their smoking habits. Numerical rating scale was introduced to the patients and then peripheral venous cannulation at the dorsum of the hand was made with a 20 G intracath. Pain perception of the patients was scored by subsequent numerical rating scale questioning. RESULTS: The demographic characteristics of the groups were identical. Numerical rating scale scores in Group S and C were 3.31 ± 1.56 and 1.65 ± 1.23, respectively (p < 0.001). CONCLUSION: Pain perception due to peripheral venous cannulation is higher in smokers. Future studies on pain treatment should consider the smoking habits of patients.

Amaç: Bu çalışmada, çocuk kardiyoloji kliniğimizde üç yıl boyunca pulmoner balon valvüloplasti işlemi yapılan çocuklarla ilgili klinik deneyimlerimizin paylaşılması ve sonuçlarının değerlendirilmesi amaçlandı . Gereç ve yöntem: Bu retrospektif çalışmaya, Kasım 2010- Kasım 2013 tarihleri arasında pulmoner balon valvüloplasti işlemi uygulanmış olan çocuklar dahil edildi. Hastalardan sendromik olan yoktu. Pulmoner balon valvüloplasti işlemi, hastalar kateterizasyon laboratuarında genel anestezi altında entübe edilerek gerçekleştirildi. Bulgular: Çalışmaya yedisi kız, altısı erkek 13 hasta dahil edildi. Hastaların ortalama yaşı 2.9±3.74 yıl, yaş aralığı dört günlük-10 yaş arasında idi. Pulmoner darlık 10 hastada valvüler tipte olmasına karşılık, üç hastada valvüler ve infundibuler darlık birlikteydi. İşlemde monofoil kateterler kullanıldı, kateterlerin ortalama çapı 12.2 mm 6-20 mm idi. Balon/anülüs çap oranı ortalaması 1.21 1.1-1.33 olarak hesaplandı. Kalp kateterizasyonu sırasında pulmoner kapak üzerinde ölçülen basınç gradiyenti, işlem öncesinde ortalama 75 mmHg 40-120 mmHg , işlem sonrasında ise ortalama 36.4 mmHg 0-95 mmHg olarak ölçüldü. Hastalara ortalama üç kere dilatasyon yapıldı. Hastalardan dördünde darlık gradiyentinde anlamlı bir düşüş olmadığı için hastalar operasyona verildi, bir hastaya ise takiplerinde rezidü darlık geliştiğinden dolayı ikinci kere balon valvüloplasti işlemi yapıldı. Dört hastada % 30 hafif pulmoner yetmezlik tespit edildi, başka majör veya minör bir komplikasyon gelişmedi. Sonuç: Pulmoner balon valvüloplasti işlemi, pulmoner kapak darlıklarında, her yaştaki hastada ilk tedavi seçeneğidir. Güvenli ve etkili bir işlemdir. Balon/anülüs çap oranı 1.2-1.4 arasında olursa; başarı şansı daha yüksek ve komplikasyon gelişme ihtimali daha düşüktür

Renal tübüler asidoz (RTA), böbrek tübüllerindeki doğuştan ya da kazanılmış bozukluk nedeniyle, diyet ve metabolizmadan normal asit üretimi varlığında, normal plazma bikarbonat konsantrasyonun sağlanamadığı bir klinik sendromdur. Hidrojen iyon sekresyonunda veya bikarbonat geri emilimindeki bozukluğa bağlı olarak metabolik asidoz meydana gelir. RTA normal anyon açığı, hiperkloremik metabolik asidoz, bikarbonatüri ve azalmış asit atılımı ile karakterizedir. Distal RTA ise asit atılımında bozukluğa bağlı olarak primer veya sekonder sebeplerle oluşabilir. Distal RTA’lı hastalar asemptomatik olabilir veya kusma, büyüme geriliği, işitme kaybı, hemolitik anemi, renal taş, nefrokalsinozis, rikets ve osteomalazi ile başvurabilir. Erişkinlerde tekrarlayan böbrek taşı ve eklem ağrısı, kas ağrısı ve kemik ağrısı gibi kas iskelet şikayetleri görülmektedir. Bu yazıda, inatçı kusma ve gelişme geriliği ile başvuran distal RTA’lı 2,5 aylık bir olgu sunulmuştur.

JUSTIFICATIVA E OBJETIVOS: Iniciar a anestesia peridural com anestésicos locais de longa duração consome uma quantidade significativa de tempo, o que pode ser problemático em centros de anestesia obstétrica muito movimentados. Aventamos a hipótese de que uma combinação de articaína e ropivacaína proporcionaria início mais rápido e mesmo uma recuperação precoce das características do bloqueio sensório-motor. MÉTODOS: Sessenta parturientes a termo agendadas para cesariana eletiva foram randomicamente alocadas em três grupos para receber 20 mL de articaína a 2% (Grupo A), 10 mL de articaína a 2% + 10 mL de ropivacaína a 0,75% (Grupo AR) ou 20 mL de ropivacaína a 0,75% (Grupo R) via cateter peridural. O tempo de início do bloqueio sensorial até T10-T6 e o nível máximo de bloqueio, o tempo para a regressão de dois segmentos do nível máximo de bloqueio sensorial e o tempo de início e duração do bloqueio motor foram todos registrados. A necessidade de analgésicos adicionais, intra- e pós-operatoriamente, também foi registrada. RESULTADOS: Os dados demográficos foram semelhantes. Os tempos de início do bloqueio sensorial até os níveis T10 e T6 foram significativamente menores nos grupos A e AR, em comparação com o Grupo R (p < 0,05). Os tempos de início do bloqueio motor foram semelhantes em todos os grupos, mas um bloqueio motor mais profundo foi observado no Grupo R (p < 0,05). O tempo para a regressão de dois segmentos e a duração do bloqueio motor foram significativamente menores nos grupos A e AR, em comparação com o Grupo R (p < 0,05). A necessidade de analgésico adicional no período intraoperatório foi maior no Grupo A do que nos outros dois grupos (p < 0,05). CONCLUSÃO: A combinação de 2% articaína e 0,75% ropivacaína para anestesia peridural em cesariana deve ser preferida à administração peridural de ropivacaína a 0,75% sozinha.

Amaç: Bu retrospektrif çalışma nöral tüp defekti olan çocuklardaki doğuştan kalp hastalığı sıklığını belirlemek için yapılmıştır. Yöntem: Bu çalışmada, Selçuk Üniversitesi Tıp Fakültesi Etik Kurulu’ndan onay alındıktan sonra nöral tüp defekti tanısı olan vakaların ekokardiyografi verileri üç yıl için retrospektif olarak incelendi. Bulgular: Çalışma, yaşları ortalama 21.7±23.5 ay olan 82 hastayı içermektedir. Nöral tüp defekti olan çocuklardaki doğuştan kalp hastalığı sıklığı %17.1 idi. En sık görülen nöral tüp defekti meningomyelosel %73 iken, en sık görülen doğuştan kalp hastalığı ise atriyal septal defekt idi %6.3 . Sonuç: Yazarlar nöral tüp defektinde doğuştan kalp kusurlarının rapor edildiğinden daha sık olduğunu bildirmektedirler ve tarama ekokardiyografi gerekmektedir. Bu nedenle, bu durum özellikle hastalardaki küçük veya büyük cerrahi prosedürlerde akılda tutulmalıdır

Background and objectives: : Initiation of epidural anesthesia with long-lasting local anesthetics consumes a significant amount of time, which could be problematic in busy obstetric anesthesia suites. We have hypothesized that a combination of articaine and ropivacaine provides faster onset and even an early recovery of sensory-motor block characteristics. Methods: : Sixty term parturients scheduled to have elective cesarean section were randomly allocated into three groups to receive either 20 mL 2% articaine (Group A), 10 mL 2% articaine + 10 mL 0.75% ropivacaine (Group AR) or 20 mL 0.75% ropivacaine (Group R) via lumbar epidural catheter. The onset time of sensory block to T10, T6 and maximum sensory block level, time to two segments regression from maximum sensory block level, onset time and duration of motor block were all recorded. Intraoperative and postoperative additional analgesic requirements were also recorded. Results: : Demographic data were similar. The onset times of sensorial block to T10 and T6 were significantly shorter in Groups A and AR in comparison with Group R (p < 0.05). The onset times of motor block were similar in all groups, but a more intense motor block was observed in Group R (p < 0.05). Two segments regression time and motor block durations were significantly shorter in Groups A and AR in comparison with Group R (p < 0.05). Intraoperative supplementary analgesic requirements were higher in Group A than in the other two groups (p < 0.05). Conclusion: : A combination of 2% articaine and 0.75% ropivacaine for epidural anesthesia in a cesarean section should be preferred over epidural 0.75% ropivacaine alone. Objective: Cognitive deterioration has negative effects on the duration of a patient’s hospital stay. Pre-recognition of patients’ cogitive characterictics affects treatment success. We want to put forward the cognitive profiles of inpatients.Material and Methods: One hundred-six hospitalized patients over 50 years of age were assessed between 01-15 February 2009. Patients, not coooperative because of loss of consciousness and those who stayed in hospital because of primary cognitive disorder, have been excluded from study. At first evaluation, patients having cognitive deterioration have been re-evalued at a dementia clinic and the patients’cognitive profile identified. Groups with or without cognitive deterioration were compared for demographic properties. Results: Fifty-seven (%53.7) of 106 patients took 24, under 24 score from MMSE. 30 patients (28.3%) of 106 had mild cognitive deficiency. 8 patients (7.5%) had primary dementia syndrome. When groups with and without cognitive deterioration were examined, there was no difference in gender and family history (p=0.48, p=0.58). Amaç: Kliniğimizde iki yıl boyunca transkateter yolla atriyal septal defekt kapatılması işlemi yapılan hastalarla ilgili klinik deneyimlerimizin paylaşılması ve sonuçlarının değerlendirilmesi amaçlandı. Gereç ve yöntemler: Çalışmaya, sekundum atriyal septal defekt nedeniyle transkateter yolla kapatma işlemi uygulanan, ortalama yaşı 8 olan 5 yaş 3 ay-16 yaş 24 hasta 16 kız, 8 erkek , retrospektif olarak dahil edildi. Bulgular: Kapatma işlemi hastaların 22’sinde % 92 başarılı bir şekilde gerçekleştirildi ve takiplerde cihaz erezyonu, aritmi, tromboz veya rezidüel şant gibi herhangi bir komplikasyon gelişmedi. Bir hastada işlem sırasında cihaz embolizasyonu oldu, cerrahiye verilen hastada cihaz çıkartıldı ve defekt başarılı bir şekilde kapatıldı. İki hastada % 8 işlem başarısız oldu. Atriyal septal defekt kapatma cihazı olarak Amplatzer septal okluder kullanıldı. Kapatma işlemi yapılan hastalara altı ay süre ile antiagregan dozda asetilsalisilik asit tedavisi verildi. Sonuç: Sekundum atriyal septal defektlerin perkütan yolla tıkayıcı cihazlarla kapatılması etkili, güvenli ve başarı oranı yüksek, komplikasyon gelişme ihtimali düşük bir tedavi şeklidir. Son yıllarda cerrahi tedavinin yerini almıştır

The effect of oregano the (Origanum vulgare L.) oil at the levels of 0.05, 0.1 and 0.3% and oregano hydrosol on the physicochemical, microbiological and sensory characteristics of the green olive (Edremit variety) fermentation was investigated. The initial pH of the oregano oil added samples were higher than the other treatments, which were above 5.8. The highest final acidity was observed in the hydrosol+brine combination (0.53%). The initial LAB population on the first day of the fermentation was high in the diluted hydrosol (8.89 log cfu ml-1) and control (8.47 log cfu ml-1) samples. But a significant difference was not observed between the LAB counts of the treatments on the 40th day of fermentation. The control and brine+oregano hydrosol samples had the highest sensory scores.
