A dermatology symposium held in December 1989 in San Francisco focused on the changing nature of AIDS; skin disorders associated with AIDS; prospects for treatment and vaccine; and protection of dermatologists from human immunodeficiency virus (HIV) infection. The AIDS epidemic affects many people in the most productive stage of their lives, and has placed an immense financial burden on the health care system. AIDS has also become a risk for health care personnel, who must comply with universal precautions against blood-borne disease to prevent accidental infection in the workplace. At the same time, health care personnel must provide compassionate care to HIV-infected patients. Dermatologists should be aware of the early signs of HIV infection, particularly skin disorders, and take precautions against infection. Early detection of HIV infection and effective treatment with antiviral agents, such as zidovudine, may prevent the development of symptoms and prolong the survival of HIV-infected patients. In the future, HIV disease may become a chronic rather than fatal illness that is treated to improve long-term survival as in other chronic diseases, such as diabetes or heart disease. A symposium was held in April 1990 to discuss the treatment of dermatologic hypersensitivity disorders, or disease conditions resulting from increased sensitivity of the skin. The new, low-sedating H1 antihistamines, which include terfenadine, astemizole, cetirizine, and loratadine, are both effective and have fewer sedative effects than the traditional H1 antihistamines. The new H1 antihistamine cetirizine has been used to treat two forms urticaria, or hives: dermatographia, a form of urticaria resulting from physical allergy, and cold-induced urticaria. One study showed that cetirizine was effective in decreasing the size of wheals, which are circular, transient swellings on the skin surface, and accumulation of eosinophils, a type of white blood cell, associated with urticaria induced by pressure. In another study, cetirizine was shown to prevent the accumulation of inflammatory cells after exposure to skin allergens, or substances that provoke an allergic response, and certain drugs. Treatment of atopic dermatitis, an inherited skin disorder characterized by inflammation and severe itching, was discussed at the meeting as well. Recent data suggest that interleukins and cytokine antagonists, which are substances involved in the regulation of growth and other cellular processes, may be useful in treating atopic dermatitis. Interleukins are released from lymphocytes, a type of immune cell, which are involved in various immune or natural defense responses. The contribution of eosinophils in the inflammation of the skin was examined. Degranulation, or breakdown of storage compartments called granules within the cells, may result in the release of toxic granule proteins, contributing to the disease processes associated with allergic skin disease.
