Context: Induced mental fatigue negatively impacts sport performance and neurocognition. However, it is unclear how induced mental fatigue influences landing biomechanics. The purpose of this study was to examine the influence of mental fatigue on drop landing biomechanics in individuals with and without a concussion history. Design: Crossover design. Methods: Forty-eight (24 per group) recreationally active individuals were matched on age (±3 y), sex, and body mass index (±1 kg/m2). All participants completed an experimental (30-min Stroop task) and control (30-min reading magazines) intervention on separate days separated by a minimum of 24 hours. Drop landings were performed before and after both interventions. Outcomes included peak vertical ground reaction force (vGRF), vertical loading rate (VLR), knee flexion angle, knee abduction angle, external knee flexion moment, external knee abduction moment, and initial ground contact knee flexion and knee abduction angles. Separate 2 (group) × 2 (intervention) between-within analyses of covariance compared drop landing outcomes. Each group’s average pre-Stroop and premagazine outcomes were covariates. Results: There was a significant interaction for vGRF (P = .033, ) and VLR (P = .0497, ). The vGRF simple effects were not statistically significantly (P range = .052–.325). However, individuals with a concussion history displayed a medium effect size for greater vGRF post-Stroop compared with their own postmagazine vGRF (mean difference (95% confidence interval [95% CI] = 0.163 (–0.002 to 0.327) bodyweight (BW), p =.052, . In contrast, the control group displayed a small effect size (mean difference [95% CI] = 0.095 [–0.069 to 0.259] BW, p =.251, ). Individuals with a concussion history displayed greater VLR post-Stroop compared with controls (mean difference [95% CI], 26.29 [6.19 to 46.40] BW/s, P = .012, ) and their own postmagazine values (mean difference [95% CI] = 32.61 [7.80 to 57.42] BW/s, p =.011, ). Conclusion: Mental fatigue leads to greater VLR for individuals with a concussion history. Athletic competition and activities of daily living can increase mental fatigue. Training programs may seek to teach mental fatigue reducing strategies to athletes with a concussion history.

Individuals with obesity have impaired gait and muscle function that may contribute to reduced mobility and increased fall risk.(1) what is the difference in spatiotemporal gait parameters and joint kinetics between individuals with and without obesity; (2) what is the association between spatiotemporal gait parameters, joint kinetics, and quadriceps function?Forty-eight young adults with obesity (BMI = 33.0 ± 4.1 kg/mIndividuals with obesity walked with longer stance (p = 0.01), longer double-limb support (p 0.001), wider steps (p 0.001), lower cadence (p = 0.03), and a greater absolute (p 0.001) but lesser normalized total support moment (p = 0.03) compared with adults without obesity. In those with obesity, greater PT was associated with less double limb support (p = 0.011) and smaller double support to stance ratio (p = 0.006); greater early RTD was associated with less double limb support (r = -0.455, p = 0.0021), less stance time (r = -0.384, p = 0.008), and a smaller double support to stance ratio (r = -0.371, p = 0.011). In those without obesity, a larger total support moment was associated with longer step length (r = 0.512, p 0.001), lesser cadence (r = -0.497, p 0.001), and smaller GSR (-0.460, p = 0.001).Individuals with obesity walk with altered spatiotemporal gait parameters and joint kinetics that may compromise stability. Extended periods of support may be a strategy used by individuals with obesity to increase stability during gait and accomodate insufficient quadriceps function.

Research has identified an increased risk of lower extremity injury postconcussion, which may be due to aberrant biomechanics during dynamic tasks. The purpose of this study was to compare the drop landing biomechanics between individuals with and without a concussion history. Twenty-five individuals with and 25 without a concussion history were matched on age (±3 y), sex, and body mass index (±1 kg/m2). Three-dimensional landing biomechanics were recorded to obtain dependent variables (peak vertical ground reaction force, loading rate, knee flexion angle and external moment, knee abduction angle and external moment, and knee flexion and abduction angle at ground contact). A 1-way multivariate analysis of variance compared outcomes between groups. There was no difference in drop landing biomechanics between individuals with and without a concussion history (F10,39 = 0.460, P = .877, Wilk Λ = .918). There was an effect of time since concussion on knee flexion characteristics. Time since most recent concussion explained a significant amount of variation in both peak (ΔR2 = .177, β = −0.305, ΔP = .046) and initial ground contact (ΔR2 = .292, β = −0.204, ΔP = .008) knee flexion angle after covarying for sex and body mass index. Therefore, time since concussion should be considered when evaluating biomechanical patterns.

It is unclear if the Functional Movement Screen (FMS) scoring criteria identify kinematics that have been associated with lower extremity injury risk. The purpose was to compare lower extremity kinematics of the overhead deep squat (OHDS) during the FMS between individuals who were grouped on FMS scoring. Forty-five adults who were free of injury and without knowledge of the FMS or its scoring criteria (males = 19, females = 26; height = 1.68 0.08 m; mass = 70.7 7 13.0 kg). Three-dimensional lower extremity kinematics during an OHDS were measured using a motion capture system. One-way MANOVA was used to compare kinematic outcomes (peak hip flexion angle, hip adduction angle, knee flexion angle, knee abduction angle, knee internal rotation angle, and ankle dorsiflexion angle) between FMS groups. Those who scored a 3 had greater peak hip flexion angle (F2,42 = 8.75; p = 0.001), knee flexion angle (F2,42 = 13.53; p = 0.001), knee internal rotation angle (F2,42 = 12.91; p = 0.001), and dorsiflexion angle (F2,42 = 9.00; p = 0.001) compared to those who scored a 2 or a 1. However, no differences were found in any outcome between those who scored a 2 and those who scored a 1, or in frontal plane hip or knee kinematics. FMS scoring for the OHDS identified differences in squat depth, which was characterized by larger peak hip, knee, and dorsi- flexion angles in those who scored a 3 compared with those who scored 2 or 1. However, no differences were found between those who scored a 2 or 1, and caution is recommended when interpreting these scores. Despite a different FMS score, few differences were observed in frontal or transverse plane hip and knee kinematics, and other tasks may be needed to assess frontal plane kinematics.

Body mass index (BMI) and history of anterior cruciate ligament reconstruction (ACLR) independently influence gait biomechanics and knee osteoarthritis risk, but the interaction between these factors is unclear. The purpose of this study was to compare gait biomechanics between individuals with and without ACLR, and with and without overweight/obesity. We examined 104 individuals divided into four groups: with and without ACLR, and with low or high BMI (n = 26 per group). Three-dimensional gait biomechanics were evaluated at preferred speed. The peak vertical ground reaction force, knee flexion angle and excursion, external knee flexion moment, and external knee adduction moment were extracted for analysis. Gait features were compared between groups using 2 (with and without overweight/obesity) × 2 (with and without ACLR) analysis of variance. Primary findings indicated that those with ACLR and high BMI had a larger external knee adduction moment compared with those with low BMI and with (p = 0.004) and without ACLR (p = 0.005), and compared with those without ACLR and high BMI (p = 0.001). The main effects of ACLR and BMI group were found for the knee flexion moment, and those with ACLR and with high BMI had lower knee flexion moments compared with those without ACLR (p = 0.031) and with low BMI (p = 0.021), respectively. Data suggest that individuals with ACLR and high BMI may benefit from additional intervention targeting the knee adduction moment. Moreover, lower external knee flexion moments in those with high BMI and ACLR were consistent, but high BMI did not exacerbate deficits in the knee flexion moment in those with ACLR. Choe, KH, Coburn, JW, Costa, PB, and Pamukoff, DN. Hip and knee kinetics during a back-squat and deadlift. J Strength Cond Res 35(5): 1364-1371, 2021-The back-squat and deadlift are performed to improve hip and knee extensor function. The purpose of this study was to compare lower extremity joint kinetics (peak net joint moments [NJMs] and positive joint work [PJW]) between the back-squat and deadlift. Twenty-eight resistance-trained subjects (17 men: 23.7 ± 4.3 years, 1.76 ± 0.09 m, 78.11 ± 10.91 kg; 11 women: 23.0 ± 1.9 years, 1.66 ± 0.06 m, 65.36 ± 7.84 kg) were recruited. One repetition maximum (1RM) testing and biomechanical analyses occurred on separate days. Three-dimensional biomechanics of the back-squat and deadlift were recorded at 70 and 85% 1RM for each exercise. The deadlift demonstrated larger hip extensor NJM than the back-squat {3.59 (95% confidence interval [CI]: 3.30-3.88) vs. 2.98 (95% CI: 2.72-3.23) Nm·kg-1, d = 0.81, p < 0.001}. However, the back-squat had a larger knee extensor NJM compared with the deadlift (2.14 [95% CI: 1.88-2.40] vs. 1.18 [95% CI: 0.99-1.37] Nm·kg-1, d = 1.44 p < 0.001). More knee PJW was performed during the back-squat compared with the deadlift (1.85 [95% CI: 1.60-2.09] vs. 0.46 [95% CI: 0.35-0.58] J·kg-1, d = 2.10, p < 0.001). However, there was more hip PJW during the deadlift compared with the back-squat (3.22 [95% CI: 2.97-3.47] vs. 2.37 [95% CI: 2.21-2.54] J·kg-1, d = 1.30, p < 0.001). Larger hip extensor NJM and PJW during the deadlift suggest that individuals targeting their hip extensors may yield greater benefit from the deadlift compared with the back-squat. However, larger knee extensor NJM and PJW during the back-squat suggest that individuals targeting their knee extensor muscles may benefit from incorporating the back-squat compared with the deadlift.

The foot progression angle (FPA) influences knee loading during gait, but its determinants are unclear. The purpose of this study was to compare FPA between males and females and also examine the association between lower extremity kinematics during gait, hip strength, and the FPA. 25 males and 25 females completed 5 gait trials while FPA and frontal and transverse plane hip and knee angles were calculated from the dominant limb during the foot flat portion of stance. Hip extensor/flexor, abductor/adductor, and internal/external rotator strength were evaluated using maximum voluntary isometric contractions. One-way MANOVAs compared gait and strength outcomes. Stepwise regression assessed the association between FPA, and MVIC and kinematics after accounting for speed in males and females. There was no difference in FPA between sexes (p 0.05), but females had greater frontal and transverse plane hip angles compared with males (all p 0.05). Greater hip abduction (p = 0.02) strength was associated with greater FPA, but only in males. In males, greater hip abductor strength may contribute to a more neutral position of the foot during gait, which could help maintain an equal knee loading distribution. Our results suggest that there are sex specific control strategies to achieve a similar FPA during gait.

Recreational running can benefit knee cartilage, but the relationship between competitive running and knee cartilage is unclear. We compared femoral cartilage between collegiate runners, recreational runners, and controls; and evaluated the association between running amount, running kinetics and femoral cartilage characteristics. Thirty collegiate runners, 30 recreational runners, and 30 controls completed ultrasound imaging of the femoral cartilage and running gait analysis. Outcomes included cartilage thickness, and echo-intensity from the medial and lateral femoral condyles; and the peak external knee flexion (KFM) and knee adduction moments. Cartilage outcomes were compared via one-way MANOVA. The associations between running kinetics, running amount, and femoral cartilage characteristics were assessed via linear regression models adjusted for sex. No differences were found in cartilage outcomes between groups (p = 0.067). Among recreational runners, a larger peak KFM was associated with lower medial femoral cartilage echo-intensity (ΔR2 = 0.176, Δp = 0.014). In collegiate runners, a greater self-reported running amount was associated with higher medial femoral cartilage (ΔR2 = 0.117, Δp = 0.046) and lateral cartilage (ΔR2 = 0.121, Δp = 0.042) echo-intensity. Cartilage did not differ between groups, but the association between running kinetics, running amount, and knee cartilage may vary between collegiate and recreational runners.

Individuals with Prader-Willi Syndrome (PWS) have reduced mobility, which may be due to altered gait biomechanics. This study compared lower extremity intersegmental coordination and joint kinetics in adults with and without PWS. Walking biomechanics were evaluated in 10 adults with PWS and 10 controls without and 10 with obesity. The foot-shank and shank-thigh coordination was evaluated using modified vector coding and compared between groups using Kruskal-Wallis and Mann-Whitney U tests. The total support moment was summed from the ankle, knee, and hip extensor moments; and relative joint contributions were expressed as a percentage and compared between groups using one-way MANOVA. The group with PWS had greater exclusive shank segment rotation during later stance compared with controls with (p 0.001) and without obesity (p 0.001). The group with PWS also had a smaller absolute total support moment than controls with obesity during early and late stance (both p 0.001), and lower normalized total support moment compared to controls without obesity during early stance (p = 0.019) and compared to controls with obesity during late stance (p = 0.004). Extensor moment contributions was similar between groups during early and late stance (all p 0.05). Findings suggest a flat-footed gait pattern in PWS during late stance, which may negatively influence propulsion and speed. Moreover, those with PWS had lower total support moments than controls during early and late stance, but similar relative extensor contributions when walking at self-selected speeds. As such, improving overall torque generation in the lower extremity may be useful to improve stability and mobility during gait in PWS.

PURPOSE Obesity influences gait and muscle function, which may contribute to knee osteoarthritis. This study aimed 1) to compare gait biomechanics and quadriceps function between individuals with and without obesity and 2) to examine the association between quadriceps function and gait biomechanics. METHODS Forty-eight individuals with and 48 without obesity participated and were matched on age and sex. Gait biomechanics at standardized and self-selected speeds were used to assess peak vertical ground reaction force (vGRF), vertical loading rate (vLR), internal knee extension moment (KEM), peak knee flexion angle (KFA), knee flexion excursion (KFE), and knee joint stiffness. Quadriceps function was assessed using peak isometric strength (peak torque), early (RTD100) and late (RTD200) rate of torque development (RTD), and vastus lateralis cross-sectional area (CSA) and echo intensity (EI). RESULTS When normalized to fat-free mass, individuals with obesity had lower RTD100 (P = 0.04) and RTD200 (P = 0.02) but higher vastus lateralis CSA (P < 0.01) and EI (P < 0.01) compared with normal weight controls. The group-speed interaction was significant for normalized vGRF (P < 0.01), normalized vLR (P = 0.02), normalized KEM (P = 0.03), and normalized knee joint stiffness (P = 0.02). Post hoc analyses indicate a smaller normalized vGRF and normalized KEM, and lower knee joint stiffness in individuals with obesity compared with normal weight controls at self-selected speed. There were main effects of speed for all kinematic and kinetic variables, and body mass index group for all absolute kinetic variables as well as normalized vGRF (all P < 0.001). A lower vastus lateralis EI (P = 0.04) and greater RTD100 (P < 0.01) were associated with a larger KEM in individuals with obesity. CONCLUSION Individuals with obesity have quadriceps dysfunction that is weakly associated with KEM during walking. Exercise interventions that improve quadriceps function may improve walking mechanics.

PURPOSE Prader-Willi Syndrome (PWS) is a form of congenital obesity characterized by excessive body fat, hypotonia, muscle weakness, and physical/cognitive disability. However, the sources of muscle dysfunction and their contribution to mobility are unclear. The purposes of this study were to 1) compare plantar flexor function between adults with and without PWS; and 2) to examine the relationship between plantar flexor function and gait speed in adults with PWS. METHODS Participants included 10 adults with PWS, 10 adults without PWS and with obesity, and 10 adults without PWS and without obesity (matched on age and sex). Plantar flexor function was assessed using isokinetic dynamometry (peak torque [PT], early/late rate of torque development [RTD]), Hoffman reflex (H/M ratio), ultrasound imaging (cross-sectional area [CSA], echo intensity, pennation angle, and fascicle length), and peak propulsive force and plantar flexor moment during gait. Outcomes were compared between groups using one-way MANOVA. Associations between plantar flexor outcomes and gait speed were assessed using Pearson correlation in the PWS group. RESULTS Adults with PWS had lower absolute and normalized early RTD, and lower H/M ratio than controls with and without obesity; lower absolute PT and late RTD than controls with obesity (all P < 0.05). Cross-sectional area, propulsive force, and plantarflexor moment were lower, and echo intensity was higher, in adults with PWS compared with controls without obesity (all P < 0.05). Greater absolute PT (r = 0.64), absolute early RTD (r = 0.62), absolute late RTD (r = 0.64), gastrocnemii CSA (r = 0.55), and propulsive force (r = 0.58) were associated with faster gait speed (all P < 0.05). CONCLUSIONS Adults with PWS have impaired plantar flexor function likely attributable to reduced neuromuscular function and altered muscle morphology, which are associated with slower gait speeds.

Obesity and female sex are independent risk factors for knee osteoarthritis and also influence gait mechanics. However, the interaction between obesity and sex on gait mechanics is unclear, which may have implications for tailored gait modification strategies.The purpose of this study was to examine the influence of obesity and sex on sagittal and frontal plane knee mechanics during gait in young adults.Forty-eight individuals with (BMI = 33.03 ± 0.59; sex:50 % female; age:21.9 ± 2.6 years) and 48 without obesity (BMI:21.59 ± 0.25; sex:50 % female; age:22.9 ± 3.57 years) matched on age and sex completed over-ground gait assessments at a self-selected speed. Two (BMI) by two (sex) analysis of variance was used to compare knee biomechanics during the first half of stance in the sagittal (knee flexion moment [KFM] and excursion [KFE]) and frontal plane (first peak knee adduction moment [KAM], knee varus velocity [KVV]).We observed a BMI by sex interaction for normalized KFM (P = 0.03). Females had smaller normalized KFM compared to males (P = 0.03), but only in individuals without obesity. Males without obesity had larger normalized KFM compared to males with obesity (P = 0.01), while females did not differ between BMI groups. We observed main effects of sex and BMI group, where females exhibited greater normalized KAM (P 0.01) and KVV (P 0.01) compared to males, and individuals with obesity walked with greater KVV compared to those without obesity (P 0.01). All absolute joint moments were greater in individuals with obesity (all P0.01) and males had greater absolute KFM compared to females (P 0.01).We observed sex differences in gait mechanics, however, KFM differences between males and females were only evident in individuals without obesity. Further, females and individuals with obesity had a larger KAM and KVV, which may contribute to larger medial compartment joint loading.

ContextUltrasound imaging provides a cost-effective method of measuring quadriceps morphology, which may be related to self-reported function after anterior cruciate ligament reconstruction (ACLR).ObjectiveTo compare quadriceps morphology and strength between limbs in individuals with ACLR and matched control limbs and determine their associations with self-reported function.DesignCross-sectional study.SettingResearch laboratory.Patients or Other ParticipantsForty-two individuals with ACLR (females = 66%; age = 21.8 ± 2.6 years; time since ACLR = 50.5 ± 29.4 months) and 37 controls (females = 73%; age = 21.7 ± 1.2 years).Main Outcome Measure(s)Quadriceps peak torque (PT) and rate of torque development were assessed bilaterally. Ultrasonography was used to measure the cross-sectional area (CSA) and echo intensity (EI) of the rectus femoris, vastus lateralis (VL), and vastus medialis. Self-reported function was assessed via the International Knee Documentation Committee (IKDC) score and Knee Injury and Osteoarthritis Outcome Score (KOOS) subscales. Paired-samples t tests were calculated to compare involved and uninvolved limbs. Independent t tests were conducted to compare groups (α = .05). Linear regression was performed to analyze associations between quadriceps function and self-reported function after accounting for time since ACLR, activity level, and sex, and models for EI added subcutaneous fat as a covariate.ResultsIsometric PT did not differ between limbs or groups. Involved limbs had a lower rate of torque development compared with the control (P = .01) but not the uninvolved limbs (P = .08). Vastus lateralis CSA was smaller in the involved than in the uninvolved (P < .01) but not the control limbs (P = .10). Larger VL CSA (ΔR2 = 0.103) and lower VL EI (ΔR2 = 0.076) were associated with a higher IKDC score (P < .05). Larger VL CSA was associated with greater KOOS Symptoms (ΔR2 = 0.09, P = .043) and Sport and Recreation (ΔR2 = 0.125, P = .014) scores. Lower VL EI was associated with higher KOOS Symptoms (ΔR2 = 0.104, P = .03) and Quality of Life (ΔR2 = 0.113, P = .01) scores. Quadriceps PT and rate of torque development were not associated with IKDC or KOOS subscale scores.ConclusionsQuadriceps morphology was associated with self-reported function in individuals with ACLR and may provide unique assessments of quadriceps function.

Background Individuals with anterior cruciate ligament reconstruction (ACLR) are at greater risk for knee osteoarthritis, which may be in part due to altered gait biomechanics. Articular cartilage thickness is typically imaged using magnetic resonance imaging, which is costly and lacks portability. Ultrasonography may provide an alternative imaging method for articular cartilage. It is unclear if ultrasonographic measurements of cartilage thickness are associated with gait biomechanics in individuals with ACLR. Research Question To evaluate the association between sagittal and frontal plane knee mechanics during gait and resting femoral cartilage thickness from ultrasonography. Methods Twenty-five females with ACLR (age = 21.7 ± 2.6 years, time since ACLR = 60.6 ± 24.8 months) completed assessments of walking biomechanics and resting femoral cartilage thickness. Linear regression examined the association between gait biomechanics and cartilage thickness at the medial (MC) and lateral (LC) femoral condyles, and intercondylar notch (IC) after accounting for time since ACLR, meniscal injury, and gait speed. Results In the ACLR limb, larger vertical ground reaction force (ΔR2 = 0.21, pΔ = 0.03), knee flexion angle (ΔR2 = 0.15, pΔ = 0.05), knee flexion excursion (KFE) (ΔR2 = 0.16, pΔ = 0.04), and knee flexion impulse (KFI) (ΔR2 = 0.23, pΔ = 0.02) were associated with thicker MC cartilage. A larger knee adduction angle (ΔR2 = 0.20, pΔ = 0.03) and knee adduction moment (KAM) (ΔR2 = 0.20, pΔ = 0.03) were associated with thinner MC thickness. Larger KFE (ΔR2 = 0.20, pΔ = 0.03) was associated with thicker LC cartilage. Gait biomechanics were not associated with IC cartilage thickness. After accounting for co-variates, the combination of KFI and KAM was predictive of MC thickness (ΔR2 = 0.37, pΔ = 0.01; Total R2 = 0.52, p = 0.02). Meniscal injury, KAM, and KFI were significant predictors in the model. In the contralateral limb, KFE was associated with thicker MC cartilage (ΔR2 = 0.16, pΔ = 0.05). Significance Sagittal and frontal plane knee mechanics during gait are uniquely associated with ultrasonographic measurements of femoral cartilage thickness in individuals with ACLR. Furthermore, concomitant medial meniscal injury was associated with thinner MC cartilage.

Pamukoff, DN, Pietrosimone, B, Ryan, ED, Lee, DR, Brown, LE, and Blackburn, JT. Whole body vibration improves early rate of torque development in individuals with anterior cruciate ligament reconstruction. J Strength Cond Res 31(11): 2992-3000, 2017-The purpose of this study was to compare the effect of whole-body vibration (WBV) and local muscle vibration (LMV) on early and late quadriceps rate of torque development (RTD), and electromechanical delay (EMD) in individuals with anterior cruciate ligament reconstruction (ACLR). Twenty individuals with ACLR were recruited for this study. Participants performed isometric squats while being exposed to WBV, LMV, or no vibration (control) in a randomized order during separate visits. Early and late quadriceps RTD and EMD were assessed during a maximal voluntary isometric knee extension before and immediately after WBV, LMV, or control. There was a significant condition by time interaction for early RTD (p = 0.045) but not for late RTD (p = 0.11) or EMD of the vastus medialis (p = 0.15), vastus lateralis (p = 0.17), or rectus femoris (p = 0.39). Post hoc analyses indicated a significant increase in early RTD after WBV (+5.59 N·m·s·kg; 95% confidence interval, 1.47-12.72; p = 0.007). No differences were observed in the LMV or control conditions, and no difference was observed between conditions at posttest. The ability to rapidly produce knee extension torque is essential to physical function, and WBV may be appropriate to aid in the restoration of RTD after ACLR.

Collegiate cross country runners are at risk for running related injuries (RRI) due to high training volume and the potential for aberrant lower extremity biomechanics. However, there is a need for prospective research to determine biomechanical risk factors for RRI. The purpose of this study was to prospectively compare ankle, knee, and hip kinematics and kinetics and ground reaction force characteristics between injured and non-injured cross country runners over a 14-week season. Biomechanical running analyses were conducted on 31 collegiate-cross country runners using a 3-dimensional motion capture system and force plate prior to the start of the season. Twelve runners were injured and 19 remained healthy during the course of the season. Peak external knee adduction moment (KAM), a surrogate for frontal plane knee loading, and peak ankle eversion velocity were greater in runners who sustained an injury compared to those who did not, and no differences were noted in ground reaction force characteristics, or hip kinematics and kinetics. Reducing the KAM and ankle eversion velocity may be an important aspect of preventing RRI.

This study compared femoral cartilage characteristics between age- and sex-matched individuals with (n = 48, age = 22.8 ± 3.5 years; body mass index [BMI] = 33.1 ± 4.1 kg/m2 ) and without obesity (n = 48 age = 22.0 ± 2.6 years; BMI = 21.7 ± 1.7 kg/m2 ) and evaluated the associations between body composition, quadriceps function, and gait kinetics with femoral cartilage characteristics. Medial and lateral femoral cartilage thickness, medial:lateral thickness ratio and medial and lateral cartilage echo intensity were measured using ultrasound imaging. Body composition was assessed using air displacement plethysmography. Quadriceps function was assessed via maximal isometric knee extension. Three-dimensional gait biomechanics were recorded to extract peak external knee flexion and adduction moments, and peak loading rate of the vertical ground reaction force. Cartilage outcomes were compared between groups using one-way multivariate analysis of variance. We examined the association between footfall pattern and characteristics of lower limb muscle function and compared lower limb muscle function between forefoot and rearfoot runners. Fifteen rearfoot and 16 forefoot runners were evaluated using ultrasonography of the gastrocnemii and tibialis anterior while strike index and heel strike angle quantified footfall pattern. Higher strike index was associated with lower medial gastrocnemius echo intensity (p = 0.05), lower lateral gastrocnemius echo intensity (p = 0.04), smaller tibialis anterior pennation angle (p = 0.05), and longer lateral gastrocnemius fascicle length (p = 0.04). Larger heel strike angle was associated with smaller medial gastrocnemius cross-sectional area (p = 0.04), shorter lateral gastrocnemius fascicle length (p0.01), and lower plantar flexion moment (p0.01). Larger plantar flexion moment was associated with lesser medial gastrocnemius echo intensity (p = 0.04), lesser lateral gastrocnemius echo intensity (p = 0.03), and greater lateral gastrocnemius fascicle length (p = 0.02). A smaller plantar flexion moment, larger heel strike angle, lower tibialis anterior echo intensity, larger tibialis anterior pennation angle, and smaller lateral gastrocnemius pennation angle were observed in rearfoot compared to forefoot runners (p0.05). Lower limb muscle architecture is associated with footfall pattern and ankle mechanics during running.

Anterior cruciate ligament injury and reconstruction (ACLR) dramatically increase the risk of knee osteoarthritis, but the contributing factors, and therefore the targets for intervention, are poorly understood. Differences in loading characteristics between the ACLR and contralateral limbs during routine activities such as walking may elucidate the mechanical pathogenesis of post-traumatic knee osteoarthritis. Twenty-nine females with ACLR (age=21.7±3.1 years; time since ACL injury=48±41 months) performed walking gait at a self-selected speed from which the overall peak vertical ground reaction force (vGRF) in the first 50% of the stance phase and its linear (slope of the vGRF-time curve) and instantaneous (first time-derivative) loading rates were calculated. The magnitude of the vGRF peak immediately following heelstrike and its linear and instantaneous loading rates were also identified. Subjects were further classified as "Impulsive Loaders" or "Normal Loaders" based on whether the transient vGRF peak immediately following heelstrike was objectively classified as a heelstrike transient in the majority of trials. The vGRF magnitude immediately following heelstrike and instantaneous loading rates (both overall and immediately following heelstrike) were greater in the ACLR limb. Additionally, vGRF linear and instantaneous loading rates were greater in subjects classified as Impulsive Loaders. As higher loading rates are associated with greater cartilage degradation in animal models, these data suggest that the greater loading rates in the ACLR limb may play an important role in development of post-traumatic knee osteoarthritis. Additionally, the heelstrike transient appears to be an objective indicator of impulsive loading.

Chronic quadriceps dysfunction has been implicated as a contributor to knee osteoarthritis (OA) development after anterior cruciate ligament reconstruction (ACLR). This dysfunction potentially leads to impulsive/high-rate loading during gait, thus accelerating cartilage degradation. The purpose of this study was to examine relationships between several indices of quadriceps function and gait biomechanics linked to knee OA development in individuals with ACLR.Gait biomechanics and quadriceps function were assessed in 39 individuals with ACLR. Indices of quadriceps function included isometric peak torque and rate of torque development (RTD), isokinetic peak torque and power, and the central activation ratio. Gait biomechanics included the peak vertical ground reaction force and loading rate, and the heel strike transient (HST) magnitude and loading rate.Isometric peak torque was not associated with any of the gait biomechanical variables. However, greater RTD was associated with lesser peak vertical ground reaction force linear (r = -0.490, P = 0.003) and instantaneous (r = -0.352, P = 0.031) loading rates, as well as a lesser HST magnitude (r = -0.312, P = 0.049) and instantaneous loading rate (r = -0.355, P = 0.029). Greater central activation ratio was associated with greater HST instantaneous (r = 0.311, P = 0.050) and linear (r = 0.328, P = 0.033) loading rates. Isokinetic peak torque and power were not associated with any of the biomechanical variables.Poor quadriceps function, especially RTD, is associated with gait kinetics linked to cartilage degradation in individuals with ACLR. These results highlight the likely role of chronic quadriceps dysfunction in OA development after ACLR and the need to emphasize improving quadriceps function as a primary rehabilitation goal.

Introduction: The purpose of this study was to compare the effect of whole body vibration (WBV) and local muscle vibration (LMV) on quadriceps function. Methods: Sixty adults were randomized to WBV, LMV, or control groups. Quadriceps function [Hoffmann (H)-reflex, active motor threshold (AMT), motor evoked potential (MEP) and electromyographic amplitude, peak torque (PT), rate of torque development (RTD), and central activation ratio (CAR)] was assessed before and immediately after and 10 and 20 minutes after interventions. Results: WBV improved PT, CAR, AMT, EMG, and MEP amplitude, and EMG amplitude and CAR were greater than control after application. LMV improved EMG amplitude and AMT, and EMG amplitude was greater than control after application. AMT remained lower 10 and 20 minutes after WBV and LMV. No differences were noted between LMV and WBV. Vibration did not influence H-reflex or RTD. Impulsive, or high rate, loading contributes to cartilage degradation and is commonly identified via the heelstrike transient (HST) in the vertical ground reaction force (vGRF) during gait. Investigation of the HST may improve our understanding of knee osteoarthritis mechanical pathogenesis. However, the most appropriate method for objectively identifying the HST is unclear. Twenty-eight healthy subjects walked at a self-selected pace while vGRF data were captured. The efficacies of three HST identification methods (Radin, Hunt, and Modified Hunt) were evaluated using vGRF data lowpass filtered at three frequencies (raw/unfiltered, 75Hz, and 50Hz). Both the HST identification method and lowpass filter frequency influenced whether a HST was identified and whether a subject was classified as an "impulsive loader" (i.e. HST identified in 3 of 5 trials). The methods identified different phenomena in the vGRF, with the Radin and Modified Hunt methods identifying the HST 11-16ms following ground contact and the Hunt method identifying the HST 83-122ms following ground contact. Lowpass filtering the vGRF at 75Hz and implementing the Radin method was the most effective approach for identifying the HST. Future longitudinal observations are necessary to determine if specific HST criteria are indicative of knee osteoarthritis development and progression.

Context:Hamstring musculotendinous stiffness (MTS) is associated with lower-extremity injury risk (ie, hamstring strain, anterior cruciate ligament injury) and is commonly assessed using the damped oscillatory technique. However, despite a preponderance of studies that measure MTS reliably in laboratory settings, there are no valid clinical measurement tools. A valid clinical measurement technique is needed to assess MTS and permit identification of individuals at heightened risk of injury and track rehabilitation progress.Objective:To determine the validity and reliability of the Myotonometer for measuring active hamstring MTS.Design:Descriptive laboratory study.Setting:LaboratoryParticipants:33 healthy participants (15 men, age 21.33 ± 2.94 y, height 172.03 ± 16.36 cm, mass 74.21 ± 16.36 kg).Main Outcome Measures:Hamstring MTS was assessed using the damped oscillatory technique and the Myotonometer. Intraclass correlations were used to determine the intrasession, intersession, and interrater reliability of the Myotonometer. Criterion validity was assessed via Pearson product–moment correlation between MTS measures obtained from the Myotonometer and from the damped oscillatory technique.Results:The Myotonometer demonstrated good intrasession (ICC3,1 = .807) and interrater reliability (ICC2,k = .830) and moderate intersession reliability (ICC2,k = .693). However, it did not provide a valid measurement of MTS compared with the damped oscillatory technique (r = .346, P = .061).Conclusions:The Myotonometer does not provide a valid measure of active hamstring MTS. Although the Myotonometer does not measure active MTS, it possesses good reliability and portability and could be used clinically to measure tissue compliance, muscle tone, or spasticity associated with multiple musculoskeletal disorders. Future research should focus on portable and clinically applicable tools to measure active hamstring MTS in efforts to prevent and monitor injuries.

Background Obesity is a risk factor for knee osteoarthritis. Altered gait biomechanics are common in obese individuals, and may contribute to the development of knee osteoarthritis. Research has focused on older obese adults with knee osteoarthritis, and it is unclear if young obese individuals display similar aberrant biomechanics. The purpose of this study was to compare gait biomechanics between normal-weight and obese young adults. Methods 15 normal-weight (body mass index=21.5 (1.1)) and 15 obese (body mass index=33.5 (3.7)) young adults were recruited and categorized by body mass index. Lower extremity kinematics and kinetics were collected while participants walked at standardized (1m/s) and self-selected speeds. Analysis of variance (group by condition) was used to compare peak vertical ground reaction force, vertical loading rate, peak internal knee extension moment, peak internal knee abduction moment, peak knee flexion angle, and knee flexion excursion between groups. Findings Gait biomechanics did not differ between groups during walking at a self-selected speed. When walking at a standardized speed, obese subjects displayed greater instantaneous vertical loading rates (46.2 vs. 35.0N/s, P P =0.04). Instantaneous vertical loading rate was greater during walking at a self-selected speed compared to a standardized speed in the obese ( P =0.007) and normal weight groups ( P =0.001). Interpretation As greater loading rates are related to cartilage degeneration, these results suggest that obesity may contribute to knee osteoarthritis. Prospective studies are needed to identify the influence of higher loading rates on knee osteoarthritis.

Following anterior cruciate ligament reconstruction (ACLR), individuals have quadriceps muscle impairments that influence gait mechanics and may contribute to an elevated risk of knee osteoarthritis.To compare running mechanics and quadriceps function between individuals who have undergone ACLR and those in a control group, and to evaluate the association between quadriceps function and running mechanics.In this controlled, cross-sectional laboratory study, 38 individuals who previously underwent primary unilateral ACLR (mean ± SD time since reconstruction, 48.0 ± 25.0 months) were matched to 38 control participants based on age, sex, and body mass index, and underwent assessments of quadriceps muscle performance and running biomechanics. Quadriceps muscle performance was assessed via isokinetic and isometric knee extension peak torque and rate of torque development (RTD) over 2 time frames: 0 to 100 milliseconds (RTD100) and 0 to 200 milliseconds (RTD200). Running evaluation included assessment of the knee flexion angle (KFA), knee extension moment (KEM), rate of knee extension moment (RKEM), vertical instantaneous loading rate, and vertical impact peak.On average, there was a smaller KFA (P = .016) in the involved limb compared to the uninvolved limb in the ACLR group. Compared to limbs in the control group, involved limbs in the ACLR group had lower RTD100 (P = .015), lower peak torque at 60°/s (P = .007), lower peak torque at 180°/s (P = .016), smaller KFA (P.001), lower KEM (P = .001), lower RKEM (P = .004), and higher vertical instantaneous loading rate (P = .016). Compared to limbs in the control group, uninvolved limbs in the ACLR group had lower RTD100 (P = .003), lower peak torque at 60°/s (P = .017), and smaller KFA (P = .01). For the involved limbs in the ACLR group, there was a low correlation between isokinetic peak torque at 180°/s and RKEM (r = 0.38, P = .01), and a negligible correlation between RTD100 and RKEM (r = 0.26, P.05). No differences were found in isometric strength for any comparison.Individuals who have undergone ACLR have bilateral alterations in running mechanics that are weakly associated with diminished quadriceps muscle performance. Muscle strength is a determinate of physical function and increasing muscle strength is an important clinical goal for patients with knee injury. This article discusses the emerging evidence regarding a novel rehabilitation strategy that uses disinhibitory modalities to increase neuromuscular activation in conjunction with traditional muscle strengthening for the purpose of maximizing strength gains following acute knee injury or surgery and in patients with knee osteoarthritis. The use of disinhibitory modalities and specific types of neuromuscular training for clinically maximizing strength are discussed.

Greater lower extremity joint stiffness may be related to the development of tibial stress fractures in runners. Musculotendinous stiffness is the largest contributor to joint stiffness, but it is unclear what factors contribute to musculotendinous stiffness. The purpose of this study was to compare plantar flexor musculotendinous stiffness, architecture, geometry, and Achilles tendon stiffness between male runners with and without a history of tibial stress fracture. Nineteen healthy runners (age = 21 ± 2.7 years; mass = 68.2 ± 9.3 kg; height = 177.3 ± 6.0 cm) and 19 runners with a history of tibial stress fracture (age = 21 ± 2.9 years; mass = 65.3 ± 6.0 kg; height = 177.2 ± 5.2 cm) were recruited from community running groups and the university’s varsity and club cross-country teams. Plantar flexor musculotendinous stiffness was estimated from the damped frequency of oscillatory motion about the ankle follow perturbation. Ultrasound imaging was used to measure architecture and geometry of the medial gastrocnemius. Dependent variables were compared between groups via one-way ANOVAs. Previously injured runners had greater plantar flexor musculotendinous stiffness (P < .001), greater Achilles tendon stiffness (P = .004), and lesser Achilles tendon elongation (P = .003) during maximal isometric contraction compared with healthy runners. No differences were found in muscle thickness, pennation angle, or fascicle length.

Objective To evaluate the effects of whole body vibration (WBV) and local muscle vibration (LMV) on quadriceps function after experimental knee effusion (ie, simulated pathology). Design Randomized controlled trial. Setting Research laboratory. Participants Healthy volunteers (N=43) were randomized to WBV (n=14), LMV (n=16), or control (n=13) groups. Interventions Saline was injected into the knee to induce quadriceps arthrogenic muscle inhibition (AMI). All groups then performed isometric squats while being exposed to WBV, LMV, or no vibration (control). Main Outcome Measures Quadriceps function was assessed at baseline, immediately after effusion, and immediately and 5 minutes after each intervention (WBV, LMV, control) via voluntary peak torque (VPT) and the central activation ratio (CAR) during maximal isometric knee extension on a multifunction dynamometer. Results The CAR improved in the WBV (11.4%, P =.021) and LMV (7.3%, P P =.021) in the WBV group and 23% ( P =.078) in the LMV group immediately postintervention, but it did not increase in the control group. The magnitudes of improvements in the CAR and VPT did not differ between the WBV and LMV groups. Conclusions Quadriceps AMI is a common complication following knee pathology that produces quadriceps dysfunction and increases the risk of posttraumatic osteoarthritis. Quadriceps strengthening after knee pathology is often ineffective because of AMI. WBV and LMV improve quadriceps function equivocally after simulated knee pathology, effectively minimizing quadriceps AMI. Therefore, these stimuli may be used to enhance quadriceps strengthening, therefore improving the efficacy of rehabilitation and reducing the risk of osteoarthritis.

Background Obesity is considered a risk factor for knee osteoarthritis (OA) in part due to its influence on gait biomechanics. The heel strike transient (HST) is a characteristic of the ground reaction force that is indicative of a high rate of loading, but has not been evaluated in obese adults. Objective To compare the incidence of HST in obese compared to normal weight adults. Methods 15 normal-weight (males = 7, age = 20.4 ± 2.1 years, body mass index = 21.6 ± 1.3 kg/m2) and 15 obese (males = 7, age = 21.2 ± 1.9 years, body mass index = 33.5 ± 4.3 kg/m2) young adults completed 10 walking trials at a standardized speed and 10 trials at a self-selected speed while ground reaction force data were sampled. HST incidence was evaluated using a dichotomous method previously identified in the literature, and compared between groups using χ2 analyses. Results The number of individuals classified as possessing the HST differed between the obese and normal-weight groups (8/15 vs. 3/15, p = 0.047). Evaluation of the standardized residuals indicated a significantly greater than expected incidence of the HST in the obese group when walking at a standardized speed. Conclusion Our findings indicate that a greater proportion of obese compared to normal weight participants displayed a HST. The HST may provide a dichotomous method for identifying individuals with aberrant gait biomechanics.

PURPOSE Individuals with anterior cruciate ligament reconstruction (ACLR) are at greater risk for knee osteoarthritis, partially because of chronic quadriceps dysfunction. Articular cartilage is commonly assessed using magnetic resonance imaging and radiography, but these methods are expensive and lack portability. Ultrasound imaging may provide a cost-effective and portable alternative for imaging the femoral cartilage. The purpose of this study was to compare ultrasonography of the femoral cartilage between the injured and uninjured limbs of individuals with unilateral ACLR, and to examine the association between quadriceps function and ultrasonographic measures of femoral cartilage. METHODS Bilateral femoral cartilage thickness and quadriceps function were assessed in 44 individuals with unilateral ACLR. Quadriceps function was assessed using peak isometric strength, and early (RTD100) and late (RTD200) rate of torque development. RESULTS Cartilage thickness at the medial femoral condyle (P < 0.001) and femoral cartilage cross-sectional area (P = 0.007) were smaller in the injured compared with the uninjured limb. After accounting for time since ACLR, quadriceps peak isometric strength was associated with cartilage thickness at the medial femoral condyle (r = 0.35, P = 0.02) and femoral cartilage cross-sectional area (r = 0.28, P = 0.04). RTD100 and RTD200 were not associated with femoral cartilage thickness or cross-sectional area. CONCLUSIONS Individuals with ACLR have thinner cartilage in their injured limb compared with uninjured limb, and cartilage thickness is associated with quadriceps function. These results indicate that ultrasonography may be useful for monitoring cartilage health and osteoarthritis progression after ACLR.

Context: Verbal and tactile feedback (VTF) during rehabilitation exercises can increase muscle activation, thus improving the therapeutic benefits. However, it is unclear which feedback method elicits the greatest electromyographic (EMG) amplitude. Objective: To determine if the addition of tactile to verbal feedback (VF) increases EMG amplitude of selected shoulder musculature during scapular plane elevation (Ys), shoulder horizontal abduction with external rotation (Ts), and scapular retraction with external rotation (Ws). Design: Repeated-measures cross-over design. Setting: Biomechanics laboratory. Participants: A total of 30 physically active adults volunteered for this study—age = 20.23 (1.25) years; height = 1.71 (0.073) m; and mass = 70.11 (15.14) kg. Interventions: Electromyography of the serratus anterior; upper, middle, and lower trapezii; and anterior and posterior deltoids was recorded during Ys, Ts, and Ws with VTF and VF alone during separate testing sessions. Participants completed baseline trials without feedback, then received VTF and VF across 2 counterbalanced sessions. Main Outcome Measures: Difference scores were calculated between prefeedback and postfeedback interventions, and the difference score between baseline measurements was used as a control. One-way analysis of variance of the difference scores was used to evaluate the influence of VTF and VF on EMG amplitude during Ys, Ts, and Ws. Results: There was a significant difference between conditions for EMG amplitude of the middle trapezius (F2,28 = 4.09, P = .02) and serratus anterior (F2,28 = 3.91, P = .03) during Ys, the middle trapezius (F2,28 = 7.82, P = .001) during Ws, and the upper (F2,28 = 3.61, P = .03) and middle trapezii (F2,28 = 5.81, P = .01) during Ts. Post hoc testing revealed that both feedback conditions elicited greater EMG amplitude compared with no feedback, but there were no significant differences between the feedback conditions. Conclusions: The addition of tactile feedback to VF does not increase EMG amplitude compared with VF alone. This study indicates that feedback, regardless of type, is more beneficial than providing no feedback, for increasing EMG amplitude.

Objective To examine the effect of whole-body vibration (WBV) on running biomechanics in individuals with anterior cruciate ligament reconstruction (ACLR). Design Single-blind randomized crossover trial. Setting Research laboratory. Participants Individuals (N=20) with unilateral ACLR (age [± SD]=22.3 [±3.3] years; mass=71.8 [±15.3] kg; time since ACLR=44.9 [±22.8] months; 15 females, 10 patellar tendon autograft, 7 hamstrings autograft, 3 allograft; International Knee Documentation Committee Score=83.5 [±9.3]). Main Outcome Measure Participants performed isometric squats while being exposed to WBV or no vibration (control). WBV and control conditions were delivered in a randomized order during separate visits separated by 1-week washout periods. Running biomechanics of the injured and uninjured limbs were evaluated before and immediately after each intervention. Dependent variables included peak vertical ground reaction force (GRF) and loading rate (LR), peak knee flexion angle and external moment, and knee flexion excursion during the stance phase of running. Results There was an increase in knee flexion excursion (+4.1°, 95% confidence interval [CI]: 0.65, 7.5°) and a trend toward a reduction in instantaneous LR after WBV in the injured limb (−4.03 BW/sec −1 , 95% CI −0.38, −7.69). No effect was observed on peak GRF, peak knee flexion angle, or peak external knee flexion moment, and no effect was observed in the uninjured limb. Conclusions Our findings indicate that a single session of WBV acutely increases knee flexion excursion. WBV could be useful to improve running characteristics in individuals with knee pathology.

Context: Individuals with anterior cruciate ligament reconstruction (ACLR) have quadriceps dysfunction that contributes to physical disability and posttraumatic knee osteoarthritis. Bilateral quadriceps dysfunction is common in individuals with ACLR, potentially biasing these evaluations.Objective: To compare quadriceps function between individuals with ACLR and uninjured control participants.Design: Cross-sectional study.Setting: Research laboratory.Patients or Other Participants: Twenty individuals with unilateral ACLR (age = 21.1 ± 1.7 years, mass = 68.3 ± 14.9 kg, time since ACLR = 50.7 ± 21.3 months; females = 14; Tegner Score = 7.1 ± 0.3; 16 patellar tendon autografts, 3 hamstrings autografts, 1 allograft) matched to 20 control participants (age = 21.2 ± 1.2 years, mass = 67.9 ± 11.3 kg; females = 14; Tegner Score = 7.1 ± 0.4) on age, sex, body mass index, and Tegner Activity Scale.Main Outcome Measure(s): Maximal voluntary isometric knee extension was performed on an isokinetic dynamometer. Peak torque (PT), rate of torque development (RTD), electromyographic (EMG) amplitude, central activation ratio (CAR), and hamstrings EMG amplitude were assessed during maximal voluntary isometric knee extension and compared between groups using independent-samples t tests. Relationships between hamstrings co-activation and quadriceps function were assessed using Pearson correlations.Results: Participants with anterior cruciate ligament reconstruction displayed lesser quadriceps PT (1.86 ± 0.74 versus 2.56 ± 0.37 Nm/kg, P = .001), RTD (39.4 ± 18.7 versus 52.9 ± 16.4 Nm/s/kg, P = .03), EMG amplitude (0.25 ± 0.12 versus 0.37 ± 0.26 mV, P = .04), and CAR (83.3% ± 11.1% versus 93.7% ± 3.2%, P = .002) and greater hamstrings co-activation (27.2% ± 12.8% versus 14.3% ± 3.7%, P < .001) compared with control participants. Correlations were found between hamstrings co-activation and PT (r = −0.39, P = .007), RTD (r = −0.30, P = .03), and EMG amplitude (r = −0.30, P = .03).Conclusions: Individuals with ACLR possessed deficits in PT, RTD, and CAR compared with control participants. Peak torque is the net result of all agonist and antagonist activity, and lesser PT in individuals with ACLR is partially attributable to greater hamstrings co-activation.

Background Greater hamstring musculotendinous stiffness is associated with lesser anterior cruciate ligament loading mechanisms during both controlled joint perturbations and dynamic tasks, suggesting a potential protective mechanism. Additionally, lesser hamstring stiffness has been reported in females, potentially contributing to their greater risk of anterior cruciate ligament injury. However, the factors which contribute to high vs. low stiffness are unclear. Muscle geometry and architecture influence force production and may, therefore, influence stiffness. The purpose of this investigation was to evaluate the contributions of geometric and architectural muscle characteristics to hamstring stiffness. Methods Thirty healthy individuals (15 males, 15 females) volunteered for participation. Biceps femoris long head cross-sectional area, pennation angle, fiber length, tendon stiffness, and posterior thigh fat thickness were assessed via ultrasound imaging, and strength was measured via isometric contraction. Stiffness was assessed via the damped oscillatory technique. Findings Following normalization to anthropometric factors, only strength ( r = 0.535) and posterior thigh fat thickness (Spearman ρ = − 0.305) were correlated with stiffness. Normalized tendon stiffness (0.06 vs. 0.10 N/m · kg − 1 ) and strength (7.1 vs. 10.0 N · kg − 1 ) were greater in males, while posterior thigh fat thickness (10.4 vs. 5.0 mm) was greater in females. Interpretation Greater posterior thigh fat thickness may influence stiffness by contributing to greater intramuscular fat and shank segment mass, and lesser muscle per unit mass in the thigh segment. These findings suggest that training designed to increase hamstring strength and decrease fat mass may be beneficial for anterior cruciate ligament injury prevention.
