Background: Interventional cardiology procedures are constantly increasing in management of structure heart disease in developing countries. Percutaneous catheterization plays an important role in diagnosis and treatment of pediatric and adult with CHD included Trans catheter device, balloon angioplasty and valvoplasty, atrial septostomy. Aim of study: 1. To recognize the Libyan Experience in Congenital Heart Intervention in Eastern region of Libya. 2. To evaluate outcome of the non-surgical intervention in treatment of structure heart disease. Methods: Retrospective study was done include 192 of children and adult undergoing Percutaneous diagnostic and intervention catheterization at NHC in a period between 2011 and 2020. Patients were analysis according to type of catheterization either diagnostic or interventional and type of congenital heart disease. Result: Revealed that Diagnostic were 68 patients (35.4%) and 124 interventional patients (64.5%). (47.9%) were male and (52%) were female ,42 patients (21.8%) underwent ASDs, PFO Transcatheter closure, 37 patients (19.2%) were Transcatheter closure of PDA, 27 patients (14%) were pulmonary balloon valvuloplasty, (11) patients (5.7%) were coronary angiography, (5) patients (2.6%) were atrial septostomy and (2) patients (1%) ballowen angio to COA stent. Complication in 2 patients (1%), failure in 2 patients (1%) and no death.Conclusion: - Transcatheter intervention is an alternative to surgical intervention to repair certain types of congenital heart disease (CHD) with less morbidity and mortality than the surgical method of treatment.Key words: -NHC: National Heart Center NHC, CHD: Congenital Heart Disease, ASD: Atrial Septal Defect, PFO: Patent Foramen Ovale. Several lines of evidence support the notion that Bone Morphogenetic Proteins (BMPs) and gonadotropic hormones are major regulators of ovarian follicle development by inducing gonadotropin-mediated steroid hormone production and regulating ovarian follicle cell proliferation. Research from our laboratory and elsewhere have demonstrated that BMPs play important roles during the early stages of folliculogenesis. Despite the extensive research to reveal the mechanism of interaction between BMPs and gonadotropic hormones in the process of folliculogenesis, the exact mechanism of such an interaction is not adequately understood. Previously, we developed a unique mouse model characterized by a short-term attenuation of the BMP signaling system using passive immunization against bone morphogenetic protein 4 (BMP-4) and bone morphogenetic protein receptor 1B (BMPR1B). This model unequivocally demonstrated that the attenuation of BMP action was an effective method of enhancing fertility reserve and promoting follicle and ovulation rates in female mice. In addition, this study pointed to the possibility of bi-directional mutual regulation between BMPs, Follicle-Stimulating Hormone (FSH), and Luteinizing Hormone (LH). To gain further insight into this mechanism we used this mouse model to examine the protein expression and mRNA level of BMPR1B, Follicle-Stimulating Hormone Receptor (FSHR) and Luteinizing Hormone/Choriogonadotropin Receptor (LHCGR) in multiple stages of follicle development in female mice. Immunofluorescent analysis of female mice treated with anti-BMPR1B antibodies showed a significant upregulation of BMPR1B, FSHR, and LHCGR in the ovarian granulosa cells during the main stages of follicle development, whereas treatment with anti-BMP4 antibodies showed no effect. The scientific review presents the significance of some amino acids in the process of dispersing of bacterial biofilms. To write the article, information was searched using Scopus, Web of Science, MedLine, PubMed, Google Scholar, EMBASE, Global Health, the Cochrane Library, CyberLeninka. The influence of amino acids as nutrients on the late stages of biofilm life is considered, the effect of the D-enantiomers of amino acids on tryptophan and its derivatives on the biofilm dispersion is presented. It is emphasized that D-amino acids, namely, D-tyrosine, D-methionine, D-leucine, D-tryptophan, prevent the adhesion of amy­loid fibers, which keep the aggregation of the biofilms. D-amino acids inhibit the expression of genes involved in the production of components of the biofilm matrix, and thus realize their antibiotic effect. The ability of tryptophan to prevent the development and dispersion of biofilms, in particular those associated with the bacteria Pseudomonas aeruginosa, is highlighted. It is indicated that the role of anthranilates is to disrupt the structure of the biofilm and to stimulate the separation of sites from biofilms formed by the bacteria Pseudomonas aeruginosa. Anthranilates can help interrupt the chronicity of the infectious process. It is assumed that the use of amino acids and their derivatives, the creation of new chemical compounds that mimic the structure of amino acid molecules or alter the activity of the synthesis and degradation of certain amino acids will allow purposefully to control the life cycle of biofilms of certain types of pathogenic bacteria, which will contribute to the recovery of infectious recurrent and chronic diseases. Отражено влияние аминокислот как нутриентов на поздние стадии жизни биопленки, представлено влияние D-энантиомеров аминокислот, триптофана и его дериватов на диспергирование биопленки. Припущено, що використання амінокислот і їх похідних, створення нових хімічних сполук, які імітують структури молекул амінокислот або змінюють активність синтезу і деградації певних амінокислот, дозволить цілеспрямовано управляти циклом життя біоплівок деяких видів патогенних бактерій, що буде сприяти виліковуванню інфекційних рецидивуючих і хронічних захворювань. Background. The definition of epilepsy and its complications includes comorbidity. However, many questions about comorbid disorders in children with epilepsy at the time of onset of disease and prior to antiepileptic therapy remain unsolved. The purpose was to establish the frequency and identify comorbid disorders in children of different age groups at the stage of disease manifestation prior to the start of antiepileptic therapy. Materials and ­methods. Children from birth to 17 years old after the first unprovoked seizure (n = 556) were examined and included in a prospective study. Two groups were formed: 109 children with a single seizure and 447 patients with epilepsy. The frequency of comorbid disorders at the time of the first seizure was analyzed, as well as in age groups taking into account the age at the manifestation of epilepsy: up to 1 year, 1–3, 4–5, 6–13, 14–17 years. Results. The frequency of comorbid disorders in the onset of epilepsy is significantly higher than in single seizure: 68.0 and 13.8 %, respectively, pFisher < 0.001. The presence of comorbid pathology at the first seizure increases the risk of developing epilepsy by 13 times (odds ratio = 13.322; 95% confidence interval [7.459; 23.795]). A high comorbidity was detected in children under the age of 1 year — 80.8 %. Almost every third child has mental disorders at the onset of epilepsy (30.6 %) compared to a group of single seizure (0.9 %), pFisher < 0.001. In the group of up to 1 year (n = 193), already at the first epileptic seizure, more than half of the children had mental and speech/pre­verbal developmental delay — 53.4 %. Conclusions. A high frequency of comorbid disorders was detected in children with epilepsy in the onset of disease before the initiation of antiepileptic therapy (68.0 %). Malignancy-induced hypercalcemia is a very rare condition in children whereas it is more common among adult patients with malignancy. The mechanisms of malignancy-induced hypercalcemia include the over-secretion of parathyroid hormone-related protein (PTHrP), osteolytic metastases and the over-production of 1,25-dihydroxyvitamin D (calcitriol). Although hypercalcemia due to PTHrP secretion has been published before, overproduction of calcitriol has not been reported yet in pediatric squamous cell skin carcinoma cases. Herein, we report calcitriol-mediated severe hypercalcemia in a 5-year-old boy with squamous cell skin carcinoma arising in the background of xeroderma pigmentosum (XP) which responded well to zoledronate treatment. Children with cerebral palsy (CP) often present with upper extremity (UE) function impairments that hamper physical and functional abilities necessary for daily activities of daily living (ADL) and participation at play and leisure activities in various environments. The traditional repetitive task practices (RTP) have proven helpful when therapists provide one-on-one therapy for long duration and with high repetitions. Unfortunately, compliance with such time-intensive therapy interventions is low. We have developed a low-cost computer game-assisted rehabilitation platform (GRP) based on RTP principles to provide fine and gross motor UE exercises for children with CP. The aim of this thesis was to determine the reliability and validity of the Computer games-assisted Upper Extremity (CUE) assessment tool and feasibility of conducting a full scale, multicenter randomized controlled trial (RCT) to evaluate the treatment effect of the RTP-based computer game-assisted rehabilitation platform (GRP) in improving upper limb function in 4 to 10-year-old children with CP. For understanding the feasibility, a mixed method exploratory randomized controlled trial was conducted. For assessing the test-retest reliability and convergent validity of the CUE assessment tool, thirty-five children with CP (age 4 to 10 years) were recruited. Assessments were performed two weeks apart. During the first assessment session, participants were assessed using the Quality of Upper Extremity Skills Test (QUEST) and Peabody Developmental Motor Score version-2 (PDMS-2) along with the CUE assessment tool. Data analysis showed moderate to high Intra Class Coefficient (ICC) scores with low to moderate Minimal Detectable Change (MDC) values, with excellent test-retest reliability. Background: Prematurity is a leading cause of death in Indian scenario. There are lack of studies done on early physiotherapy interventions in neonates. Early care interventions in premature newborns show positive effect. Growth retardation and demineralisation of bones in newborn are thought to be caused by prolong immobilisation or lack of physical activities. In this study we are going to give combination of massage, passive movements, facilitatory techniques, joint compression and kangaroo mother care, which has not been done before. Indeed this made us to study the effectiveness of early physiotherapy intervention on weight gain in low birth weight neonates. Objectives: To determine the effectiveness of early physiotherapy intervention on weight gain in moderate premature neonates (32–37 weeks), very premature (28–32 weeks) and extremely premature neonates (less than 28 weeks). Methods: Individuals fulfilling the inclusive criteria were selected. Written consent was taken from parents and care taker of neonates. Total 20 low birth weight neonates were taken. They were differentiated into moderate premature, very premature and extremely premature neonates. The weight was measured and breast feeding frequency was noted before treatment. Then the neonates were given massage, passive movements, proprioception stimulation by individual joint compression, oral stimulation and kangaroo mother care. This was given for 25-30 minutes each session for two times per day for 7 days. After that again weight and breast feeding frequency was checked to see progression. Later evaluation and interpretation was done using statistical analysis. Results: The early physiotherapy intervention is effective in low birth weight baby for improving weight in pre mature neonates. Conclusion: On the basis of result it can be concluded that the early physiotherapy is effective in improving weight, breast feeding frequency in low birth weight neonates. The article presents an analysis of the recent scientific findings on vitamin D status in patients with different etiopathogene­tic varieties of nontraumatic transient loss of consciousness. The results of recent researches suggest that vitamin D may play an indirect and, in some cases, direct role in the pathogenesis of nontraumatic transient loss of consciousness. Data on the cause-effect relationship between a low vitamin D status and syncope are mixed, controversial and ambiguous, which is largely due to the use of different doses of vitamin D, its initial concentration, duration of therapy, genetic diffe­rences in the vitamin D receptors, different age groups, physical parameters of the surveyed, medications, peculiarities of nutrition with special supplements, differences in physical activity, peculiarities of the climate and season, and others. Until now there is no convincing evidence of the benefits of using vitamin D in the treatment and prevention of syncope. Most studies are observational and relate mainly to adult populations. Therefore, randomized controlled studies focused on children may be a promising field of research., В статье представлен анализ новейших научных достижений по изучению проблематики витамин-D-статуса у пациентов с различными этиопатогенетическими вариантами преходящих потерь сознания нетравматического генеза. Результаты последних научных исследований позволяют предположить, что витамин D может оказывать опосредованное, а в некоторых случаях и прямое действие в патогенезе транзиторных потерь сознания. Данные о причинно-следственной связи между низким статусом витамина D и синкопальными состояниями являются смешанными, противоречивыми и неоднозначными, что во многом связано с использованием различных доз витамина, его исходной концентрацией, продолжительностью терапии, генетическими различиями рецептора витамина D, разным возрастом, физическими параметрами исследуемых, приемом лекарств, особенностями питания с использованием специальных добавок, различиями в физической активности, особенностями климата и временем года и др. До сих пор нет убедительных данных о пользе применения витамина D в лечении и профилактике обмороков. Большинство исследований имеют наблюдательный характер и касаются в основном взрослой популяции, поэтому перспективным направлением развития науки в этой области является проведение рандомизированных контролируемых исследований с акцентуацией на детское население., У статті поданий аналіз новітніх наукових досягнень із вивчення проблематики вітамін-D-статусу в пацієнтів із різними етіопатогенетичними варіантами транзиторних втрат свідомості нетравматичного генезу. Результати останніх наукових досліджень дають змогу припустити, що вітамін D може справляти опосередковану, а в деяких випадках і пряму дію в патогенезі транзиторних втрат свідомості. Дані щодо причинно-наслідкового зв’язку між низьким статусом вітаміну D і синкопальними станами є змішаними, суперечливими й неоднозначними, що великою мірою пов’язано із використанням різних доз вітаміну, його вихідною концентрацією, тривалістю терапії, генетичними відмінностями рецептора вітаміну D, різним віком, фізичними параметрами обстежуваних, прийомом ліків, особливостями харчування з використанням спеціальних добавок, відмінностями у фізичній активності, особливостями клімату, порою року тощо. До сьогодні немає переконливих даних про користь застосування вітаміну D у лікуванні та профілактиці синкопе. Більшість досліджень мають спостережний характер та стосуються в основному дорослої популяції, а тому перспективним напрямком розвитку науки в цій галузі є проведення рандомізованих контрольованих досліджень з акцентуацією на дитяче населення.

This review addresses the current strategies of inducing tolerance development in infant and childhood cow's milk protein allergy (CMPA). The change in prevention strategies for CMPA has been emphasized based on the lack of evidence to support the efficacy of food allergen avoidance in infancy and the concept of the dual-allergen-exposure hypothesis, which suggests that allergen exposure through the skin leads to sensitization, whereas early oral consumption of allergenic food protein induces oral tolerance. The new approach is based on the likelihood of early introduction of allergenic foods to the infant's diet to reduce the development of food allergies through oral tolerance induction. The latest treatment guidelines recommend the continuation of breast feeding and the elimination of cow's milk and products from the maternal diet in exclusively breast-fed infants with CMPA, the use of an extensively hydrolyzed infant formula (eHF) with proven efficacy in CMPA as the first elimination diet in formula-fed infants with CMPA and the use of amino acid-based formula (AAF) in severe cases, such as anaphylaxis, enteropathy, eosinophilic esophagitis, and food protein-induced enterocolitis syndrome (FPIES), as well as cases of multiple system involvement, multiple food allergies, and intolerance to extensively hydrolyzed formula (eHF). Comparative, functional, developmental, and some morphological studies on animal anatomy require accurate visualization of three-dimensional structures. Nowadays, several widely applicable methods exist for non-destructive whole-mount imaging of animal tissues. The purpose of this study was to optimize specimen preparation and develop a method for quantitative analysis of the total pulmonary vasculature in fetal rats. Tissues were harvested at E21 and fetuses fixed overnight in 4% paraformaldehyde/phosphate buffered saline. They were treated with 25% Lugol solution for 72 hours to ensure perfusion. Four different methods were used for fetal specimen preparation; isolated lung, upper torso, direct right ventricle contrast injection, and whole body with partial thoracic skin excision. The microCT scan was performed, and pulmonary vasculature was segmented. Vessels were analyzed for diameter, length, and branching. Of the four preparation methods, only whole body with partial thoracic skin excision resulted in adequate reconstruction of the pulmonary vasculature. In silico generated 3D images gathered by micro CT showed pulmonary vasculature distributed throughout the lung, which was representative of the shape and structure of the lungs. The mean number of vessels segmented in the pulmonary tree was 900 +/- 24 with a mean diameter of 134.13 mu m (range 40.72-265.69 mu m). While up to the 30th generation of vessels could be segmented, both for arteries and veins, the majority of branching was between the 21st and 30th generations. Passive diffusion of contrast material enables quantitative analysis of the fetal pulmonary vasculature. Hypoglycemia is one of the most important and most common metabolic problems of the newborn because it poses a risk of neurological injury, if it is prolonged and recurs. Therefore, newborns who carry a risk of hypoglycemia should be fed immediately after delivery and the blood glucose level should be measured with intervals of 2-3 hours from the 30th minute alter feeding. The threshold value for hypoglycemia is 40 mg/dL for the first 24 hours in symptomatic babies. In asymptomatic babies, this value is considered 25 mg/dL for 0-4 hours, 35 mg/dl for 4-24 hours, 50 mg/dL alter 24 hours and 60 mg/dL after 48 hours. Screening should be performed with bed-side test sticks. When values near the limit value are obtained, confirmation with laboratory method should be done and treatment should be initiated, if necessary. The level targeted with treatment is considered 50 mg/dL in the postnatal first 48 hours before feeding, 60 mg/dL after 48 hours in babies with high risk and above 70 mg/dL in babies with permanent hypoglycemia. In cases in which the blood glucose level is below the threshold value and can not be increased by feeding, a glucose infusion of 6-8 mg/kg/min should be initiated. If symptoms accompany, a mini bolus of 10% dextrose (2 ml/kg/min) should accompany. Incements (2 mg/kg/min) should be performed, if the target level can not be achieved and decrements (2 ml/kg/min) should be performed, if nutrition and stabilization is provided. The infusion should be discontinued, if the infusion rate decreases to 3-5 mg/kg/min. If necessary, blood samples should be obtained during hypoglycemia in terms of differential diagnosis and the investigation should be performed following a 6-hour fasting period in babies fed enterally and at any time when the plasma glucose is <50 mg/dL in babies receiving parenteral infusion. Background/aims: maturity onset diabetes of the young (MODY) is a rare condition often misdiagnosed as type 1 diabetes (T1D). The purposes of this study were: to identify any patients followed in a large Turkish cohort as T1D, with an atypical natural history, who may in fact have MODY, and to define the criteria which would indicate patients with likely MODY as early as possible after presentation to allow prompt genetic testing. Methods: Urinary C-peptide/creatinine ratio (UCPCR) was studied in 152 patients having a diagnosis of T1D for at least 3 years. Those with a UCPCR ≥0.2 nmol/mmol were selected for genetic analysis of the Glucokinase (GCK), Hepatocyte nuclear factor 1a (HNF1A), Hepatocyte nuclear factor 4a (HNF4A), and Hepatocyte nuclear factor 1b (HNF1B) genes. This UCPCR cut-off was used because of the reported high sensitivity and specificity. Cases were also evaluated using a MODY probability calculator. Results: twenty-three patients from 152 participants (15.1%) had a UCPCR indicating persistent insulin reserve. The mean age ± SD of the patients was 13.6 ± 3.6 years (range 8.30-21.6). Of these 23, two (8.7%) were found to have a mutation, one with HNF4A and one with HNF1B mutation. No mutations were detected in the GCK or HNF1A genes. Conclusion: in Turkish children with a diagnosis of T1D but who have persistent insulin reserve 3 years after diagnosis, up to 9% may have a genetic mutation indicating a diagnosis of MODY., NA

Hyperglycemia has become an important risk factor for mortality and morbidity in the neonatal period, especially with increased survival rates of very low birth weight neonates. Hyperglycemia in the neonatal period develops as a result of various mechanisms including iatrogenic causes, inability to supress hepatic glucose production, insulin resistance or glucose intolerance, specifically in preterm neonates. Initiation of parenteral or enteral feeding in the early period in preterm babies increases insulin production and sensitivity. The plasma glucose is targeted to be kept between 70 and 150 mg/dL in the newborn baby. While a blood glucose value above 150 mg/dL is defined as hyperglycemia, blood glucose values measured with an interval of 4 hours of >180-200 mg/dL and +2 glucosuria require treatment. Although glucose infusion rate is reduced in treatment, use of insulin is recommended, if two blood glucose values measured with an interval of 4 hours are >250 mg/dL and glucosuria is present in two separate urine samples. / Yenidoğan döneminde özellikle çok düşük doğum ağırlıklı bebeklerin yaşamda kalmaya başlamaları ile birlikte, hiperglisemi morbidite ve mortalite için önemli bir risk etmeni olmuştur. Jaundice is one of the most common problems in the newborn. It is generally accepted as a physiologic condition; most cases are benign and transient. However, in a small portion of jaundiced newborn infants, serum bilirubin concentrations increase to a level at which irreversible brain damage can occur. The timely diagnosis and management of severe hyperbilirubinemia is essential to prevent acute bilirubin encephalopathy and kernicterus. Kernicterus still occurs although it is almost always preventable. The focus of this guideline is to reduce the incidence of severe hyperbilirubinemia and bilirubin encephalopathy. Therefore, a system-based approach using the recommendations of this guideline should be implemented in all birthing facilities and continued in ambulatory care of the newborn infants. / Sarılık yenidoğan bebeklerde sık görüllen bulgulardan biridir. Background: The aim of this case report is to discuss diagnostic workup and clinical management of cytomegalovirus reactivation in a critically ill immunocompetent pediatric patient. Case presentation: A 2-year-old white boy who had no medical history presented with respiratory distress and fever. His Pediatric Risk of Mortality and Pediatric Logistic Organ Dysfunction scores were 20 and 11, respectively. Our preliminary diagnosis was multiple organ dysfunction secondary to sepsis. Antibiotic treatment was started; he was intubated and artificially ventilated. Norepinephrine infusion was started. Hemophagocytic lymphohistiocytosis was diagnosed because our patient had elevated levels of serum ferritin, bicytopenia, splenomegaly, fever (> 38.5 °C), and hemophagocytosis shown in a bone marrow sample. Therapeutic plasma exchange and intravenously administered high-dose corticosteroid for hemophagocytic lymphohistiocytosis and continuous renal replacement treatment for acute renal failure were initiated. Following 5-day high-dose corticosteroid administration, therapeutic plasma exchange, and continuous renal replacement treatment, his clinical status and kidney and liver functions improved, and vasoactive requirement and ferritin levels decreased. He was extubated on the seventh day. On the tenth day of hospitalization he had a seizure and was diagnosed as having septic encephalopathy. His immune functions were found to be normal. Although his medical condition improved continuously, he had left spontaneous pneumothorax on the 21st day of admission as a complication of necrotizing pneumonia. Since pneumothorax persisted, left upper lobectomy surgery was performed on the 30th day of hospitalization. In the pathological examination of the excised lung tissue, features of cytomegalovirus infection were observed. Ganciclovir treatment was started. Serological tests indicated that our patient had cytomegalovirus reactivation. Antiviral treatment was stopped after 17 days, w, NA

В мире зарегистрировано около 500 видов гельминтов, паразитирующих у человека, причем распространенность гельминто-протозойных инфекций достаточно высока.Паразитозы способствуют более частому возникновению соматических и обострению хронических заболеваний, оказывая многоплановое воздействие на организм хозяина, в том числе на его иммунную систему. Аллергическое воспаление при паразитозах развивается по классическому пути и призвано создать условия, приводящие к гибели и/или элиминации паразита. С учетом этого целью работы стало изучение аллергического анамнеза и показателей иммунитета у детей с аскаридозом и лямблиозом до и после проведения антигельминтной терапии препаратом Вормил.Нами было отобрано 134 ребенка, все дети были разделены на три группы. 1-ю группу составили 28 человек с кишечной формой аскаридоза, 2-ю группу — 25 детей с манифестными формами лямблиоза. В контрольную группу вошел 81 ребенок без паразитарной инвазии.В результате проведенных исследований установлено, что клинические признаки аллергического воспаления встречаются в 1,9 раза чаще у детей с аскаридозом и с манифестным лямблиозом в сравнении с детьми без паразитоносительства. У детей с аскаридозом и лямблиозом уровень иммуноглобулина Е до начала терапии был повышен. Аскаридоз и лямблиоз у обследованных детей сопровождались дисбалансом показателей Т- и В-лимфоцитов, снижением уровня Т-популяционного состава и В-клеток.Применение препарата Вормил способствовало нормализации иммунологических показателей, что можно считать результатом ликвидации сенсибилизирующего влияния глистных и паразитарных инвазий., У світі зареєстровано близько 500 видів гельмінтів, що паразитують у людини, причому поширеність гельмінто-протозойних інфекцій досить висока.Паразитози сприяють більш частому виникненню соматичних і загостренню хронічних захворювань, справляючи багатоплановий вплив на організм господаря, у тому числі на його імунну систему. Allergic inflammation in parasitosis develops by the classical way and is aimed at creating conditions leading to death and/or elimination of the parasite.With this in mind, the objective of the work was to study allergic background and immunological parameters in children with ascariasis and giardiasis before and after anthelmintic therapy using Vormil.We have selected 134 children, all the children were divided into three groups. Group 1 comprised 28 people with intestinal ascariasis, group 2 — 25 children with symptomatic giardiasis. The control group included 81 children without parasitic infestation.The studies found that the clinical signs of allergic inflammation occur 1.9 times more frequently in children with ascariasis and symptomatic giardiasis as compared to children without parasitosis. In children with ascariasis and giardiasis IgE level before treatment was increased. Ascariasis and giardiasis in surveyed children were accompanied by an imbalance in indicators of T- and B-lymphocytes, reduced level of T- and B-cells.Use of Vormil contributed to the normalization of immunological parameters, which can be considered the result of the elimination of the sensitizing influence of helminthic and parasitic infestations.

The article presents the analysis of the literature and the authors’ own research data on diagnostics, long-term consequences of Langerhans cells histiocytosis (LCH), as well as clinical signs, treatment and long­term follow-up for patients with childhood­-onset of disease.LCH — granulomatous disease, which is based on the abnormal proliferation and accumulation in organs and tissues pathological myeloid cells, which are phenotypically similar to Langerhans cells. LCH is an intriguing rare disease and can be characterized as a myeloid tumor having inflammatory properties. Local and systemic inflammatory effects lead to granuloma formation and damage of tissue in various organs. There are different clinical variations of LCH: from one bone lesion (single system LCH; SS-­LCH),) to involving of several organs (multisystem LCH, MS­-LCH). Over the last decades mortality of MS-­LCH patients has been dramatically reduced and with adequate therapy survival rates now make up 85—90 %. Nevertheless, in the remote period majority survivors have long-­term consequences of the disease which affect their quality of life. Availability of long­term consequences LCH is primarily caused by the localization of the pathological process and the degree of their severity ­ by the tissue destruction and scarring. The study of clinical characteristics of LCH and its long­-term effects allows to conclude that the organ and hormonal disorders (e. g. central diabetes insipidus) are often already determined during primary diagnosis and can be considered as a manifestation of the underlying disease. Other pathological processes (e. g. neurodegenerative damage of the central nervous system) may develop gradually and clinically manifest through the months, years and even decades. All this makes it difficult to conduct long­term routine monitoring to determine late consequences of the disease., В статье представлены анализ литературы и собственные исследования авторов по вопросам диагностики отдаленных последствий гистиоцитоза из клеток Лангерганса (Langerhans cell histiocytosis — LCH), а также клинической картины, лечения и долгосрочного мониторинга больных, перенесших заболевание в детском возрасте. LCH — гранулематозное заболевание, в основе которого лежит аномальная пролиферация и накопление в органах и тканях патологических миелоидных клеток, которые фенотипически схожи с клетками Лангерганса. LCH относится к редким заболеваниям и его можно характеризовать как миелоидную опухоль, обладающую воспалительными свойствами. Местные и системные воспалительные эффекты приводят к формированию гранулемы и повреждению тканей в различных органах. Клинический спектр LCH колеблется от одного костного поражения (моносистемное поражение; SS-LCH) до поражения нескольких органов (многосистемный LCH; МS-LCH). За последние десятилетия резко сократилась смертность от MS-LCH: при использовании адекватной терапии выживаемость пациентов составляет до 85—90 %. Тем не менее в отдаленный период значительная часть выживших лиц имеют отдаленные последствия заболевания, которые влияют на качество их жизни. Наличие отдаленных последствий LCH обусловлено в первую очередь локализацией патологического процесса, а степень их тяжести — деструкцией ткани и образованием рубцов. Наявність віддалених наслідків LCH зумовлена передовсім локалізацією патологічного процесу, а ступінь їх тяжкості — деструкцією тканини і утворенням рубців. Вивчення клінічної картини LCH і його віддалених наслідків дає змогу зробити висновок про те, що органні й гормональні порушення (наприклад, центральний нецукровий діабет) часто визначаються вже під час встановлення діагнозу і їх можна розглядати як вияв основного захворювання. Інші патологічні процеси (наприклад, нейродегенеративне ушкод­­ження центральної нервової системи) розвиваються поступово і клінічно маніфестують через місяці, роки й навіть десятиліття. Усе це створює складнощі у проведенні довгострокового рутинного моніторингу для визначення пізніх ефектів захворювання.

Background: Abdominal tuberculosis, the sixth commonest extrapulmanory tuberculosis, is easily misdiagnosed due to nonspecific gastrointestinal symptoms. The diagnosis is highly dependent on clinician's suspicion of the disease; therefore to increase awareness we retrospectively reviewed clinical features of eight children in a four year-period. Methods: The medical records of children with abdominal TB were retrospectively reviewed. The diagnosis of abdominal TB was defined as Mycobacterium tuberculosis infection of gastrointestinal tract along with peritoneal or solid organ involvement. Results: There were six girls and two boys with a mean age of 13.6 +/- 2.8 years (range, 7 - 16 years). Abdominal pain and weight loss were common complaints in all patients (100%) at presentation; fever was present in four (50%) patients and abdominal distension in two (25%) patients. Mean duration of symptoms before diagnosis was 2.5 +/- 1 months. Laparotomy performed in six patients, and the peritoneum was the most common infection site (in 5 patients). Chest X-rays of 7 patients indicated lung involvement. The most common abdominal computed tomography and ultrasonography finding was ascites. Bowel wall thickening was recorded in 2 patients and ileal thickening in 1 patient. Inguinal lymphadenopathy and multiple mesenteric lymphadenitis were present as single cases. Conclusions: The abdominal tuberculosis should be suspected in children with ongoing abdominal pain, fever, and abdominal distension, laparoscopy or laparotomy could be useful in the differential diagnosis and utilizing imaging techniques, invasive methods with clinical suspicion may prevent delay of the diagnosis., NA

Background: Thyroid function tests in neonates have been challenging to interpret because their levels are affected by several neonatal and delivery-related factors. The aim of the study was to evaluate reference values of thyroxine (T4) and thyrotropin (TSH) levels in different gestational age groups and to demonstrate the affect of perinatal factors on thyroid hormones. Methods: Medical records of 7616 neonates whose gestational age ranges between 34 and 42 weeks were analyzed retrospectively. Gender, mode of delivery, gestational age, postnatal age and birth weight were noted together with TSH and T4 levels. Results: Gestational age (r = 0.14, p < 0.001) and birth weight (r = 0.12, p < 0.001) had positive correlation with T4 levels, whereas they had no effect on TSH levels. Males had higher TSH and lower T4 levels (p = 0.001 for both) compared with females. T4 levels of babies born via vaginal delivery were lower than the ones born via cesarean section (p = 0.01). Multivariable analysis yielded gestational age as the only factor affecting T4 levels (p < 0.001). T4 and TSH levels based on 2.5-97.5 percentile cutoffs according to gestational age were presented. Background: This case report presents differential lung ventilation in an infant. The aim is to define an alternative technique for performing differential lung ventilation in children. To the best of our knowledge, this is the first report of this kind. Case presentation: A 4.2-kg, 2.5-month-old Asian boy was referred to our facility with refractory hypoxemia and hypercarbia due to asymmetric lung disease with atelectasis of the left lung and hyperinflation of the right lung. He was unresponsive to conventional ventilator strategies; different ventilator settings were required. To perform differential lung ventilation, two separate single-lumen endotracheal tubes were inserted into the main bronchus of each lung by tracheotomy; the tracheal tubes were attached to discrete ventilators. The left lung was ventilated with a lung salvage strategy using high-frequency oscillatory ventilation, and the right lung was ventilated with a lung-protective strategy using pressure-regulated volume control mode. Differential lung ventilation was performed successfully with this technique without complications. Conclusions: Differential lung ventilation may be a lifesaving procedure in select patients who have asymmetric lung disease. Inserting two single-lumen endotracheal tubes via tracheotomy for differential lung ventilation can be an effective and safe alternative method., NA

The article presents a case of non-endemic, rare disease in the Crimea — the visceral form of leishmaniasis in 11-month-old child, which clinically occurs with prolonged fever, hepatolienal syndrome with manifestations of hepatitis, anemia, thrombocytopenia. Establishing final definitive diagnosis was preceded by diagnoses: acute intestinal infection, salmonellosis, hepatitis, fever of unknown origin, cytomegalovirus infection. The final diagnosis — visceral leishmaniasis was made in Israel by the results of liver biopsy with the discovery of Leishman — Donovan bodies. The child was treated with AmBisome (liposomal amphotericin B). After the treatment the child with his recovery returned to Ukraine. It is suggested that the above incident was a manifestation of the course of multicomponent non-cyclic infectious process., В статье представлен случай неэндемичного, редкого в Крыму заболевания — висцеральной формы лейшманиоза у ребенка 11 месяцев, клинически протекавшего с длительной лихорадкой, гепатолиенальным синдромом с проявлениями гепатита, анемией, тромбоцитопенией. Установлению окончательного диагноза предшествовали диагнозы: острая кишечная инфекция, сальмонеллез, гепатит, лихорадка неясного генеза, цитомегаловирусная инфекция. Окончательный диагноз — висцеральный лейшманиоз установлен в Израиле по результатам биопсии печени с обнаружением телец Лейшмана — Донована. Ребенок был пролечен препаратом AmBisome (липосомальный амфотерицин В). После проведенного лечения ребенок с выздоровлением вернулся в Украину. Высказано предположение, что описанный случай стал проявлением течения многокомпонентного нециклического инфекционного процесса., У статті наведено випадок неендемічного, рідкісного в Криму захворювання — вісцеральної форми лейшманіозу в дитини 11 місяців, що клінічно перебігав із тривалою лихоманкою, гепатолієнальним синдромом з проявами гепатиту, анемією, тромбоцитопенією. Встановленню остаточного діагнозу передували діагнози: гостра кишкова інфекція, сальмонельоз, гепатит, лихоманка неясного генезу, цитомегаловірусна інфекція. Остаточний діагноз — вісцеральний лейшманіоз установлено в Ізраїлі за результатами біопсії печінки з виявленням тілець Лейшмана — Донована. Дитина була пролікована препаратом AmBisome (ліпосомальний амфотерицин В) згідно з протоколом. Після проведеного лікування дитина з одужанням повернулася в Україну. Зроблено припущення, що описаний випадок є проявом перебігу багатокомпонентного нециклічного інфекційного процесу.

The defi nition and classifi cation of osteogenesis imperfecta (OI) have been transformed since last two decades. In the past, it has been described as a type I collagenopathy due to identifi ed mutations in COL1A1 and COL1A2 genes encoding collagen type I which led to all OI cases. At least 10 genes I, nvolved in the process of type I collagen biosynthesis have been described until now. COL1A1/2 genes are responsible for almost 90 % of OI prevalence. However, there is a number of newly discovered genes which encode proteins involved in posttranslational overmodifi cation of procollagen, whereas mutations in these genes lead to approximately 10 % types of OI inherited recessively. This review provides an update on the traditional Sillence classifi cation and also recent developments in understanding of underlying mechanisms in OI. The paper discusses currently published modifi cation of the OI classifi cation (new types of the disease: V–VIII) and a new approach to the diagnostic methods focusing mainly on phenotypic manifestation, independent of the genetic or molecular background. We point out the importance of clinical symptoms and severity of the disorder both of which appear the most useful criteria for management of OI patients in everyday practice., За последние 2 десятилетия понятие несовершенного остеогенеза (НО) и его классификация претерпели существенные изменения. Ранее его рассматривали как колагенопатию I типа в связи с выявленными мутациями генов COL1A1 и COL1A2, кодирующих цепи коллагена I типа. Данные нарушения и является причиной развития различных видов НО. До сих пор описано не менее 10 генов, вовлеченных в биосинтез коллагена I типа. Гены COL1A1 / 2 ответственные практически 90% случаев НО. Однако недавно было открыто еще несколько генов, кодирующих протеины, вовлеченные в процесс посттрансляционной модификации проколлагена. Мутации именно этих генов обусловливают развитие остальных 10% случаев НО, что наследуется по рецессивному типу. В этом обзоре предлагается пересмотр традиционной классификации Силенса и рассматриваются современные подходы к пониманию механизмов, лежащих в основе развития НО. В статье обсуждаются недавно опубликованы изменения в классификации НО (выделенные новые типы заболевания - V-VIII) и новый подход к диагностическим методам, основанные в первую очередь на фенотипических признаках, а не на генетических и молекулярных данных. Нами было подчеркнуто, что клинические симптомы и тяжесть заболевания определяют тактику ведения пациентов с НО и важные в повседневной практике., За останні 2 десятиліття поняття недосконалого остеогенезу (НО) і його класифікація зазнали істотних змін. Раніше його розглядали як колагенопатію I типу у зв’язку з виявленими мутаціями генів COL1A1 і COL1A2, що кодують ланцюги колагену I типу. Дані порушення і є причиною розвитку різних видів НО. До сьогодні описано принаймні 10 генів, залучених у біосинтез колагену I типу. Гени COL1A1/2 відповідальні практично за 90 % випадків НО. Однак нещодавно було відкрито ще декілька генів, що кодують протеїни, залучені в процес посттрансляційної модифікації проколагену. Мутації саме цих генів обумовлюють розвиток інших 10 % випадків НО, що успадковується за рецесивним типом. У цьому огляді пропонується перегляд традиційної класифікації Сіленса й розглядаються сучасні підходи до розуміння механізмів, що лежать в основі розвитку НО. У статті обговорюються нещодавно опубліковані зміни в класифікації НО (виділені нові типи захворювання — V–VIII) і новий підхід до діагностичних методів, що засновані в першу чергу на фенотипових ознаках, а не на генетичних і молекулярних даних. Нами було підкреслено, що клінічні симптоми й тяжкість захворювання визначають тактику ведення пацієнтів із НО і важливі у повсякденній практиці.
