Background: Differences in the effects of cream-type denture adhesives and home liners remain unclear., Objective: This cross-sectional study aimed to clarify the differences in the oral-hygiene status and masticatory performance between participants using cream-type denture adhesives and those using home liners., Methods: Masticatory performance with and without cream-type denture adhesives and home liners was evaluated using colour-changeable chewing gum. Oral hygiene was assessed using the tongue coating index (TCI). Propensity-score matching was performed using oral moisture, presence of palatal or mandibular protuberances, type of residual ridge, denture type (complete or partial denture), and denture retention as background factors. Wilcoxon signed-rank test was used to examine the differences in masticatory performance with and without cream-type denture adhesives and home liners and differences in masticatory performance and TCI between the cream-type denture adhesive and home-liner groups., Results: This study included 38 and 40 cream-type adhesive and home-liner users, respectively, and 17 pairs were obtained after propensity-score matching. The Wilcoxon signed-rank tests showed that masticatory performance was significantly different with and without the use of home liners, and higher values were obtained with the use of home-liners. However, the cream-type denture adhesive and home-liner groups showed no significant differences in terms of the change in masticatory performance with the use of them. Furthermore, Wilcoxon signed-rank tests showed that TCI values were significantly high in the cream-type adhesive group., Conclusion: The findings in this study suggest that home liners improve masticatory performance and that participants regularly using cream-type denture adhesives have poorer oral hygiene., (© 2024 John Wiley & Sons Ltd.)

Introduction: The primary challenge associated with implant overdentures lies in the occurrence of denture fractures around the attachments. Hence, it is recommended to enhance flexural strength through reinforcement frameworks. This study aimed to assess and compare the prosthetic maintenance of mandibular implant overdentures reinforced with Co-Cr and PEKK frameworks., Methods: Twenty-four participants with completely edentulous ridges were selected, and two implants were placed at the mandibular canine areas. After osseointegration period, ball attachments were installed. Participants were randomly assigned into two groups: Group I received a mandibular implant overdenture reinforced with a Co-Cr framework, while Group II received a mandibular implant overdenture reinforced with a PEKK framework. Prosthetic maintenance evaluations were conducted in both groups twelve months post-denture insertion. Categorical data were analyzed, and results were presented as frequency and percentage values., Results: Group II exhibited a significantly higher percentage of cases with screw looseness, denture relining, and tooth separation compared to Group I. Although Group II cases showed a non-significant increase in the percentage of insert wear and retention loss., Conclusion: Within the limitations of this study, the findings suggest that Co-Cr, in contrast to PEKK frameworks, offers a more reliable reinforcement of the implant-retained overdentures., (Copyright© 2024 Dennis Barber Ltd.)

Most edentulous patients prefer fixed restoration or detachable prostheses over conventional complete dentures. The case report aims to address and discuss the clinical and laboratory steps of implant-supported, bar-retained maxillary overdenture and mandibular implant-supported, metal-acrylic resin, screw-retained, fixed complete denture. Impression techniques and laboratory steps are discussed and demonstrated to minimize the postfabrication or insertion complication. It lets you achieve the passive fit of the prosthesis that maintains the osseointegration by reducing stress on implants and the surrounding bone.

Objectives: This systematic review compares the impact of ball and locator attachments on marginal bone loss in implant-retained overdentures in completely edentulous patients., Method and Materials: Following PRISMA guidelines, health science librarians completed literature searches from inception to 17 March 2023 in seven databases. There were 15,686 items exported to EndNote from Embase.com, CINAHL (EBSCO), Cochrane Library, Ovid MEDLINE-ALL, PubMed, Scopus, and Web of Science. Hand-searching added four more articles. After deduplication, 6,756 items were screened for eligibility. Twenty-nine studies were assessed by full text, of which ten studies, involving 424 subjects, were included in the review. Risk of bias assessment was conducted using the Cochrane risk-of-bias tool and the Newcastle-Ottawa scale. A meta-analysis was performed to synthesize and analyze the collective data from the selected studies., Results: The included studies used diverse methodologies, implant systems, and loading protocols. Most studies reported no significant difference in marginal bone loss between ball and locator attachments. The meta-analysis revealed high heterogeneity., Conclusion: The results of this systematic review suggest that ball and locator attachments exhibit similar performance in terms of marginal bone loss in implant-retained overdentures. However, the limited number, risk of bias, and heterogeneity of studies highlight the need for standardized research designs and larger sample sizes in future investigations to draw more definitive conclusions.

A technique for the virtual designing of retentive attachment inserts for bar-retained 2-implant mandibular overdentures is described. The bar attachment is scanned intraorally, the retentive insert designed with the MESHMIXER software program, and the file 3D printed in resin, which is replicated in polyetheretherketone (PEEK). Purpose: To evaluate and compare the initial retention force of three resilient unsplinted attachment systems for implant-retained mandibular overdentures: two cylindrical attachment systems (Locator R-Tx® and Novaloc®), and one ball attachment system (Ball System)., Materials and Methods: For each attachment system, initial retention is measured as the average of the maximal dislodging forces during 10 insertion-removal cycles. For the Ball System, three activation degrees of the matrix are included versus four and six color-coded retention devices for the Locator R-Tx® and the Novaloc®, respectively, to represent the complete regular retention devices panel. For each retention device or activation degree, eight samples are tested., Results: The initial retention range is similar between the Ball System (7.7 ± 3.4 N - 19.9 ± 4.6 N) and the Novaloc® (2.0 ± 0.5 N - 18.9 ± 1.4 N) and broader for the Locator R-Tx® (3.3 ± 5.0 N - 60.2 ± 6.0 N). In each attachment system, the initial retention of each retention device is significantly different from the others, except for the two most retentive Novaloc® ones. Retention devices were also classified according to their initial retention (low, medium, and maximum). In each retention group, the Novaloc® and the Ball System provided similar retention values lower than the Locator R-Tx®., Conclusion: Most of the retention devices tested provided an initial retention force of over 5 N for all three attachment systems. Purpose: To test the retention of two different overdenture attachment matrices and straight abutments when implants are placed at 0-, 15-, and 30-degree diverging angulations as well as the retention of 15-degree-angled abutments to correct the overall angulation to 0-degrees., Materials and Methods: Matching aluminum blocks were machined to incorporate two dental implants at 0-degree, 15-degree, and 30-degree relative angulations and overdenture attachments to simulate a two-implant overdenture. At 0-degree, 15-degree, and 30-degree implant angulation, straight abutments were studied. At 30-degree implant angulation, an additional group was compared utilizing 15-degree angulated abutments that corrected the overall implant angulation to 0-degrees. A custom-designed testing apparatus that allowed automated insertion and removal of the simulated overdenture was designed, with three independent testing stations, each consisting of one simulated arch and one simulated overdenture base. The baseline and residual retention forces after 30,000 dislodging cycles of the simulated overdenture were measured. One-way ANOVA was used to compare retention differences among different color patrices within the 0-, 15-, and 30-degree implant angulation groups followed by Tukey's multiple comparison test. Two sample t-tests were used to compare 0-degree versus 15-degree implant groups with straight abutments and 30-degree implant groups with straight abutments versus 30-degree implant groups with angulated abutments., Results: Regardless of implant angulation or abutment correction, the change in retention exhibited by the Novaloc system after testing was not statistically significant for all patrice types (p > 0.05); however, the change in retention exhibited by the Locator system was statistically significant for the tested group (p = 0.0272). In both the Novaloc and Locator systems, the baseline and final retention values provided by the different patrices were significantly different except for the white and green Novaloc patrices in the 15-degree divergent implant group which did not meet the specified level of significance (p = 0.0776)., Conclusion: Within the limitations of this study, implant angulations upto 15 degrees do not affect differential change in retention of Novaloc patrices. There is no difference between Novaloc white inserts (light retention value) and green inserts (strong retention values) when implants diverge upto 15 degrees. When Novaloc straight abutments were placed on implants diverging by 30 degrees, blue extra-strong retention inserts outperformed yellow medium retention inserts by maintaining a higher retention value after 30,000 cycles. When utilizing Novaloc 15-degree angulated abutments that correct the overall implant angulation to zero degrees, the red light retentive patrice provides steady retention. Finally, the Locator-green patrice system provides greater retention than the comparable Novaloc-blue patrice combination; however, it also loses more retention after 30,000 cycles., (© 2023 by the American College of Prosthodontists.)

Purpose: To compare retention and stability of Locator and bar attachments for implant-retained maxillary overdentures., Materials and Methods: Four implants were inserted into a maxillary acrylic resin model in canines and second premolar areas. Experimental overdentures were connected to the implants with bar (group I) or Locator (group II) attachments. Locators were divided into 3 subgroups according the degree of retention of the patrix nylon insert: Locator blue (group IIa), Locator pink (group IIb), and Locator transparent (group IIc). Retention (vertical dislodging) and stability (lateral, anterior, posterior dislodging) forces (N) were measured at the start of the experiment (initial retention) and after 540 cycles of denture insertion and removal (final retention)., Results: The highest initial and final stability was recorded with group IIc, followed by group IIb and group IIa, and the lowest retention and stability was noted with group I. For all groups, the highest final retention and stability forces were noted with vertical dislodging, followed by posterior dislodging, anterior dislodging, and lateral dislodging. The highest loss of retention and stability was recorded with group I, followed by group IIc, group IIb, and group IIa., Conclusion: Locator attachments are recommended to retain maxillary overdentures over Dolder bar attachments, as Locator attachments were associated with high retention and stability after wear simulation with minimal retention loss., (© 2017 by the American College of Prosthodontists.)

This clinical report describes a modification of the conventional direct retainer design, which incorporates advantages of the dual path partial denture, but with a greater ease of design and fabrication. Prosthetic rehabilitation of a completely edentulous patient should never be restricted to the replacement of missing teeth. The ultimate aim of complete denture treatment should be restoration of the full range of oral functions and esthetics. Slumped cheeks are always a concern for esthetically demanding complete denture patients. This article describes a simple, scientific, cost-effective technique to improve facial esthetics in a completely edentulous patient with the help of a cheek plumper. The technique used here implements the concept of neutral zone to precisely determine the amount of space available for the cheek plumper. The simple friction lock attachments that retained the cheek plumpers on the prosthesis were also fabricated after determining the space available in the appropriate areas. Purpose: This in vitro study aimed to evaluate and compare the effect of different bar designs on the retention forces of implant-retained maxillary overdentures., Materials and Methods: A maxillary edentulous acrylic resin model without alveolar undercut was fabricated. Four implant analogs were placed in the canine and second premolar regions. Overdentures were made and attached to the analogs with Dolder bar, Hader bar, and milled bar attachments. A universal testing machine was used to measure axial (resistance to vertical displacement) and nonaxial (resistance to anterior, posterior, and lateral displacement) retention forces (in Newtons). Measurements were made at the start of the experiment (initial retention) and after 540 cycles of denture insertion and removal to simulate 6 months of clinical function (final retention)., Results: Hader bar had the highest retention after insertions and removals, and Dolder bar had the lowest retention. Vertical displacement showed the highest retention for Dolder and milled bars, and posterior displacement showed the highest retention for Hader bar. Lateral displacement recorded the lowest retention for all bar designs. No significant difference in axial retention loss was observed between different bar designs. Dolder bar showed the highest nonaxial retention loss, and milled bar showed the lowest retention loss., Conclusion: Hader bar is recommended to retain maxillary implant overdentures, as it was associated with higher axial and nonaxial retention compared with Dolder and milled bars after 6 months of simulated denture wear.

An overdenture on implants is the treatment of choice for patients with complaints about their conventional complete dentures. While a lot is known regarding the performance of implant-supported mandibular overdentures, much less is known regarding the performance of implant-retained maxillary overdentures. In this doctoral dissertation, various aspects of overdentures on 4 dental implants in the maxilla are discussed. From a systematic review of the literature, it appeared that an overdenture (maxillary or mandibular) on implants improved chewing efficiency, increased maximum bite force and increased patient satisfaction. Implants placed with a maximum dehiscence of two thirds of the buccal implant surface also appeared to function well after 5 years. An overdenture with a bar attachment system was associated with less marginal bone loss, a better subjective chewing efficiency and greater patient satisfaction than an overdenture retained by locators. The differences were, however, small and from a cost-effectiveness point of view, the use of overdentures retained by locators would not be a bad choice.

Objective: Clasps of removable partial dentures (RPDs) often suffer from plastic deformation and failure by fatigue; a common complication of RPDs. A new technology for processing metal frameworks for dental prostheses based on laser-sintering, which allows for precise fabrication of clasp geometry, has been recently developed. This study sought to propose a novel method for designing circumferential clasps for laser-sintered RPDs to avoid plastic deformation or fatigue failure., Methods: An analytical model for designing clasps with semicircular cross-sections was derived based on mechanics. The Euler-Bernoulli elastic curved beam theory and Castigliano's energy method were used to relate the stress and undercut with the clasp length, cross-sectional radius, alloy properties, tooth type, and retention force. Finite element analysis (FEA) was conducted on a case study and the resultant tensile stress and undercut were compared with the analytical model predictions. Pull-out experiments were conducted on laser-sintered cobalt-chromium (Co-Cr) dental prostheses to validate the analytical model results., Results: The proposed circumferential clasp design model yields results in good agreement with FEA and experiments. The results indicate that Co-Cr circumferential clasps in molars that are 13mm long engaging undercuts of 0.25mm should have a cross-section radius of 1.2mm to provide a retention of 10N and to avoid plastic deformation or fatigue failure. However, shorter circumferential clasps such as those in premolars present high stresses and cannot avoid plastic deformation or fatigue failure., Significance: Laser-sintered Co-Cr circumferential clasps in molars are safe, whereas they are susceptible to failure in premolars., (Copyright © 2018 The Academy of Dental Materials. Purpose: The aim of this study was to evaluate the clinical and radiographic peri-implant tissues of bar, Locator, and resilient telescopic attachments for two-implant stabilized overdentures in subjects with mandibular atrophied ridges., Materials and Methods: Ninety edentulous individuals with mandibular ridge atrophy were randomly assigned into three equal groups and received two implants in the canine areas. Mandibular overdentures were constructed and attached to implants with Dolder bar attachments (BOD), resilient telescopes (TOD), and Locators (LOD). Plaque scores, gingival scores, pocket depths, implant stability, width of keratinized mucosa, vertical bone loss, and horizontal bone loss were evaluated at the time of prosthesis delivery and 6 months and 12 months after delivery., Results: With the exception of pocket depth and implant stability, all parameters showed a significant increase from prosthesis delivery to 6 months. BOD recorded the highest plaque scores, gingival scores, and pocket depths followed by LOD, and TOD recorded the lowest values. No significant difference in implant stability and keratinized mucosa was observed between groups. TOD and BOD recorded the highest vertical and horizontal bone losses, respectively. LOD recorded the lowest vertical and horizontal bone losses. There was no difference in implant survival rate between groups., Conclusion: Bar, resilient telescopic, and Locator attachments can be used successfully for two-implant stabilized overdentures in subjects with mandibular atrophied ridges after a 1-year follow-up period. Telescopic attachments were associated with improved clinical peri-implant soft tissues compared with other attachments. However, Locator attachments may be advantageous in terms of peri-implant bone preservation.

Splinted and unsplinted overdenture attachment systems have unique advantages and disadvantages. The aim of the present systematic review was to determine the influence of splinted and unsplinted overdenture attachment systems on the marginal bone loss, prosthetic complications and implant survival rate. PubMed/MEDLINE, Scopus and Cochrane databases were searched for articles published up to October 2017, using the following search terms: "overdenture AND attachment OR overdenture AND bar OR overdenture splinted." The PICO question "Do splinted overdenture attachment systems promote better clinical results in comparison to unsplinted systems?" was evaluated. Eligible studies included randomized controlled clinical trials, prospective studies with at least 10 participants and a minimum follow-up of 6 months, and studies published in English that compared splinted and unsplinted attachment systems within the same study. The 95% confidence interval (CI) was considered for all outcomes analysed. After completion of the different steps in the article selection process, nine articles were included in the qualitative and quantitative analyses. A total of 984 implants were placed in 380 patients (mean age: 62.8 years). The meta-analysis demonstrated no statistically significant differences between splinted and unsplinted attachment systems with regard to marginal bone loss (P = .39; MD: -0.11; 95% CI: -0.37 to 0.14), complications (P = .31; RR: 1.26; CI: 0.80-1.99) and implant survival rate (P = .14; RR: 0.37% CI: 0.10-1.36). Purpose: To evaluate by 3-dimensional (3-D) finite element analysis the stress generated on overdenture-retaining bar system with different cross sections (round, ovoid, or Hader) and misfit levels (50, 100, and 200 μm) manufactured with different metallic alloys (Au type IV, Ag-Pd, Ti cp, and Co-Cr)., Materials and Methods: Three-dimensional finite element models were modeled using specific 3-D software (SolidWorks) and imported into mechanical simulation software (ANSYS). A displacement simulating the settlement of the screw was performed until the infrastructure was settled properly on the prosthetic platform., Results: Higher misfit levels caused higher stress on the overdenture-retaining bar system components, where (1) Hader bar presented the highest values (bar = 730.71; screw = 59.66, and periimplant bone tissue = 42.96, in megapascal) and (2) round bars made with type IV Au alloy presented lower stress values on the bar framework (193.99 MPa) and screw (10.27 MPa)., Conclusions: Higher vertical misfit increased the stress values on the overdenture-retaining bar system; complex cross-sectional designs increased stress values on the bar framework, prosthetic screw, and periimplant bone tissue; stiffer alloys caused higher stress values on all the studied conditions.

Purpose: To evaluate and compare retentive properties of O-ring and Locator attachments for implant-retained maxillary overdentures., Materials and Methods: Four implant analogs were inserted in canine and second premolar areas of an acrylic edentulous maxillary model. A metal-reinforced experimental acrylic overdenture was constructed and connected to the analogs using either O-ring (group I) or Locator (group II) attachments. Locators were divided into 3 subgroups according the degree of retention of the patrix nylon insert: Locator extra-light retention (group IIa), Locator light retention (group IIb), and Locator medium retention (group IIc). Vertical and oblique (lateral, anterior, and posterior) dislodging forces were measured at the beginning of the study (initial retention) and after 540 cycles of denture insertion and removal (final retention)., Results: For vertical, lateral, and anterior dislodging, group IIc recorded the highest initial and final retention, and group I recorded the lowest retention. For posterior dislodging, group I recorded the highest retention, and group IIa recorded the lowest retention. For group II, vertical dislodging recorded the highest initial and final retention, and lateral dislodging recorded the lowest retention. For group I, posterior dislodging recorded the highest initial and final retention, and lateral dislodging recorded the lowest retention. For all dislodging forces (except posterior dislodging), the highest retention loss was recorded in group I, and the lowest retention loss was recorded in group IIa., Conclusion: Locator medium attachment was associated with favorable retention during axial (vertical) and nonaxial (anterior and lateral) dislodging compared to other types of Locator inserts and O-ring attachments after a simulated 6-month period of overdenture use., (© 2016 by the American College of Prosthodontists.)

Aim: In this study, we investigated the retention of two attachment types, Straumann ball (SB) and Straumann Locator ® (SL) attachments, on different implant angulations and identified the most appropriate treatment type or attachment system for each angulation., Materials and Methods: The attachments placed on angulation of 0°-, 10°-, and 20° implants were subjected to 1440 vertical insertion-separation cycles. The retention values of the attachments after 0, 720, and 1440 cycles were measured using the Instron machine. In addition, scanning electron microscopy images of the attachments and abutments were obtained before and after the insertion-separation process., Results: There was a significant difference between the SB and SL attachments that were placed on 0° after 1440 cycles (P < 0.05) and between 20° SB and 20° SL attachments after 720 and 1440 cycles (P < 0.05) in terms of retention. No significant difference was observed between 20° SB and 20° SL attachments at 0 cycles (P > 0.05)., Conclusions: In implants with a 20° angle, retention of stud attachments decreased more than ball attachments after use. Thus, the total angle between the implants should not be <20° if long-term retention is desired when using stud attachments. For implants with angles >20°, 6-month patient controls are required to control retention of attachments by considering factors in the mouth with the two tested attachment systems., Competing Interests: There are no conflicts of interest

Background: Implant-supported overdenture is one of the most predictable treatment options used in complete edentulism. However, differences have been reported between bar and ball attachments used to retain overdentures in terms of patient satisfaction and prosthesis retention., Purpose: The purpose of this study is to compare the effectiveness of bar and ball attachments for conventionally loaded implant-supported overdentures in completely edentulous patients to improve patient satisfaction and prosthesis retention., Materials and Methods: We conducted the review according to the Cochrane methods and following MECIR standards. We searched Cochrane Oral Health Group Trial register, Cochrane Central Register of Controlled Trials, MEDLINE, and the WHO ICTRP (March 31, 2017). Two review authors assessed trials for inclusion and risk of bias, extracted data, and checked for accuracy. We have expressed results as risk ratio or mean differences, together with their 95% confidence intervals., Results: We included 10 trials (465 participants). After 5 y, one trial reported higher patient satisfaction when bar attachment was used (MD 1.30, 95% CI 0.20-2.40), and reported no difference between both systems in prosthesis retention (MD -0.90, 95% CI -1.90 to 0.10). Two trials reported no implant failures after 1 and 5 y in both attachments. Downgrading of evidence was based on the unclear risk of bias of included studies and the wide CI crossing the line of no effect., Conclusions: There is insufficient evidence to support bar or ball attachment to be used with implant-supported overdentures in completely edentulous patients to improve patient satisfaction and prosthesis retention (PROSPERO 2014:CRD42014014594)., (© 2017 Wiley Periodicals, Inc.)

This study aimed to evaluate the peri-implant strain around mesially inclined implants used to retain mandibular overdentures with Locator resilient attachments. Four mandibular edentulous acrylic resin models received two implants in the canine areas with 0°, 5°, 10°, and 20° mesial inclinations. Overdentures were connected to the implants using Locator attachments. Pink nylon inserts (light retention) were used for all implant inclinations, and red inserts were used for 20° inclination (20°red). Four strain gages were bonded on the mesial (M), distal (D), buccal (B), and lingual (L) surfaces of each implant. Peri-implant strains were measured during bilateral and unilateral loading. The 20° inclination showed the highest strain, followed by 10° and 5°, and both 0° and 20°red presented with the lowest strain. Site D was associated with the highest strain, followed by M, B, and L, which showed the lowest strain values. Unilateral loading and the loading side presented with significantly higher strain values than bilateral loading and the nonloading side, respectively. Hence, in this study, strains around the two-implant-retained overdentures with Locator attachments increased with increases in mesial implant angulation, except when red male inserts were used.

Objectives: To assess the effect of implant angulation on the retention of two different attachment systems for implant-supported overdentures after a simulated fatigue period of 5.5 years., Material and Methods: Two test set-ups were constructed. A two-implant mandibular implant-supported overdenture design was simulated using acrylic resin blocks to support implant replicas (Model 1 and Model 2). The replicas were set up in the canine regions (22-mm inter-implant distance). In Model 1, the implants were parallel (0° to the vertical axis) and in Model 2 they were divergent (20° to the vertical axis). The Locator and the ball attachment systems were tested at both set-ups. A simulated fatigue period of 5.5 years that equated to 10,000 cycles of insertion and removal of the denture was used. Retention values for each attachment system at each angulation set-up were measured at baseline, every 500 cycles until 4000 cycles and then every 1000 cycles., Results: After simulated fatigue, the retention for both systems at both set-ups reduced. The Locator system reduced from 108.9 to 20.2 N in the parallel set-up and from 82.3 to 17.3 N in the divergent set-up. For the ball system, the retention reduced from 56.2 to 46 N when parallel and from 45.7 to 40.7 N when divergent., Conclusion: Both attachment systems showed a significant reduction in retention after simulated fatigue at both parallel and divergent set-ups. The change in implant angulation caused a significant reduction in retention for the ball attachment system only, although this was still higher at the end of testing than the Locator attachment system. The Locators also had a significantly faster rate of loss of retention at both set-ups. The objective of this research was to clinically compare peri-implant tissue health of bar-clips vs silicone-resilient liners used with bilateral posterior bars for retaining 4 implant-supported mandibular overdentures. Thirty completely edentulous male patients (mean age, 65 years) were randomly assigned into 2 equal groups. Each patient received 4 implants in the canine and first molar regions of the mandible using a flapless surgical technique. Mandibular overdentures were immediately connected to the implants with bilateral prefabricated instant adjusting bars. According to the method of retention to the bar, 1 group was retained with clips (GI), whereas the other group was retained with a silicone-resilient soft liner (GII). Peri-implant tissue health was evaluated clinically in terms of plaque scores (MPI), bleeding scores (MBI), probing depth (PD), and implant stability (IS). MPI, MBI, and PD were measured at mesial, distal, buccal, and lingual surfaces of each implant. Evaluations were performed 2 weeks (T0), 6 months (T6), and 12 months (T12) after overdenture insertion. Implants of GI with clips demonstrated significant increase in plaque, bleeding, and PD scores compared with those of GII with silicone-resilient liner at all observation times. Implants in GI demonstrated a significant decrease in implant stability compared with those of GII at T6 and T12 anteriorly and at T12 posteriorly. Resilient liners are considered better than bar-clips when used with bilateral posterior bars for retaining implant-supported mandibular overdentures in terms of peri-implant soft tissue health. Bilateral posterior ready-made bars cannot be proposed as a promising design for supporting implant-assisted mandibular overdentures.

Frameworks made of carbon fibre-reinforced composites (CFRC) seem to be a viable alternative to traditional metal frameworks in implant prosthodontics. CFRC provide stiffness, rigidity and optimal biocompatibility. The aim of the present prospective study was to compare carbon fibre frameworks versus metal frameworks used to rigidly splint implants in full-arch immediate loading rehabilitations. Forty-two patients (test group) were rehabilitated with full-arch immediate loading rehabilitations of the upper jaw (total: 170 implants) following the Columbus Bridge Protocol with four to six implants with distal tilted implants. All patients were treated with resin screw-retained full-arch prostheses endowed with carbon fibre frameworks. The mean follow-up was 22 months (range: 18-24). Differences in the absolute change of bone resorption over time between the two implant sides (mesial and distal) were assessed performing a Mann-Whitney U-test. The outcomes were statistically compared with those of patients rehabilitated following the same protocol but using metal frameworks (control group: 34 patients with 163 implants - data reported in Tealdo, Menini, Bevilacqua, Pera, Pesce, Signori, Pera, Int J Prosthodont, 27, 2014, 207). Ten implants failed in the control group (6·1%); none failed in the test group (P = 0·002). A statistically significant difference in the absolute change of bone resorption around the implants was found between the two groups (P = 0·004), with greater mean peri-implant bone resorption in the control group (1 mm) compared to the test group (0·8 mm). Purpose: Implant-retained overdentures are the first choice of rehabilitation for edentulous mandibles. Bone morphology and anatomical landmarks may be influenced by the location and angulation of implants and distances between the implants. The purpose of this study was to investigate stress distribution characteristics and to compare stress levels of three different attachment designs of three-implant-retained mandibular overdentures with three different interimplant distances., Materials and Methods: Three photoelastic mandibular models with three implants were fabricated using an edentulous mandible cast with moderate residual ridge resorption. The center implants were embedded parallel to the midline, and the distal implants were aligned at a 20-degree angulation corresponding to the center implants. Distances between the center and distal implants were set at 11, 18, and 25 mm at the photoelastic models. Bar, bar-ball, and Locator attachment-retained overdentures were prepared for the models. Vertical loads were applied to the overdentures, and stress levels and distribution were evaluated by a circular polariscope., Results: The greatest observed stress level was moderate for the tested overdenture designs. The Locator attachment system showed the lowest stress level for the 11-mm and 25-mm photoelastic models. The bar attachment design transmitted less stress compared with the other tested designs for the 18-mm photoelastic model., Conclusion: Stresses were observed on the loaded side of the photoelastic models. The lowest stress was found with the Locator and bar attachments for the 11-mm photoelastic model, which transmitted little or no discernible stress around the implants.

This article reports a case of oral rehabilitation with two-implant-retained mandibular overdenture using bar/clip and ball attachment bilaterally on the distal side. A patient aged 69 years, fully edentulous, presented herself to private practice complaining about the instability of mandibular denture. Several possibilities of rehabilitation with advantages and disadvantages were explained to the patient, based in the clinical and radiograph examinations, and patient desire, in which she opted by conventional maxillary complete denture and two-implant-retained mandibular overdenture. Two external hexagon implants of 3.75 × 10 mm were placed in anterior area of mandible. The fabrication of the metal framework with bar/clip and ball attachment bilaterally on the distal side was planned based on prior arrangement of the artificial teeth. Internal adaptation and occlusal adjustments were performed in the day of insertion of finalized denture and instructions about maintenance and sanitation. The patient showed satisfaction with the final result of her treatment, and no complications were observed during this period. The association of bar/clip with ball system may be viable as option of retention for mandibular implant-retained complete overdenture; however, further randomized controlled trials are necessary to obtain detailed knowledge about the topic.

Introduction: Every patient needs a comprehensive treatment planning. Dentists must consider the advantages and disadvantages of the available implant prosthetic options and match them to patient's expectations. Hybrid denture prosthesis is one, i.e., fabricated and retained by screw threaded into implant abutments, most of the time on four implants. Sometimes due to failure of an implant, the prosthesis needs to be replaced with a newer casting to accommodate the existing implants. Clinical consideration: This article presents a novel technique, wherein the existing framework of the fixed prosthesis can be used to convert the fixed prosthesis to removable prosthesis on the existing implants, without recasting., Clinical Significance: The implant is economical and easily convertible from a fixed-fixed to a fixed-removable prosthesis. Hence, there is reduced morbidity, reduced cost, and an increased psychological comfort for the patient.

Purpose: This study aimed to compare the difference in mechanical behavior between two types of female-part clips of the Stress-Free Implant Bar (SFI-Bar) system during simulation of insertion-removal cycles., Materials and Methods: A total of 10 samples simulating SFI-Bar-attachment-retained implant overdentures were fabricated and randomly divided into two groups (n = 5). One group used E-clips (Elitor alloy) as the female part of the SFI-Bar, and the other used T-clips (all titanium grade IV with red nylon inserts). A total of 14,000 insertion-removal cycles were carried out on each sample. Retentive forces from each cycle were recorded for analysis., Results: Significant differences were found between the two groups (P < .05)., Conclusion: The retentive force of E-clips increased as the number of dislodging cycles increased, suggesting that some adjustment may be needed to lower this part's retentive force. T-clips with changeable nylon inserts were deformed after about 4,200 insertion-removal cycles, which interfered with insertion. This indicated that T-clips may need replacement after 2 to 3 years of clinical use.

Purpose: A total of 24 zirconia-based single tooth-retained restorations inserted in 18 patients were followed up for 15 to 61 months (mean: 35 months)., Materials and Methods: Recall consisted of dental and hygiene status as well as quality assessment and check for occlusal relationship. One patient was lost to follow-up. In the incuded patients, 15 central incisors, 3 canines, and 5 lateral incisors replaced 16 lateral incisors and 7 central incisors, mostly in the maxilla (n = 17)., Results: One retainer debonded, two pontics experienced incisal chip-offs, and one patient showed an orthodontic relapse. The success rate was estimated at 82.4%, with 100% survival at 36 months., Conclusion: Technical protocol and occlusal protection are crucial.

Objective: This in vitro study evaluated the influence of implant angulations on the retentive behavior of two overdenture attachments during cyclic dislodging., Methods: Models simulating a two-implant overdenture situation were fabricated. They were divided into five groups based on their simulated implant angulations (Groups: 1 = 0°; 2 = 20°; 3 = 30°; 4 = 40°; and 5 = 60°; n = 90). Each group was further divided into two subgroups based on its attachment system (control attachment: LOCATOR(®) ; test attachment: SFI-Anchor(®) ) except for group 5 which had no LOCATOR(®) group. All models underwent 10,000 insertion-removal cycles in a wet environment. Mean retentive forces were recorded. ANOVA and linear regression models were used for statistical analyses, and the level of significance was at P < 0.05., Results: The ANOVA model revealed an effect of dislodging cycles for both attachments (P = 0.0070). The linear regression model with repeated measures revealed a significant effect of angulation within the LOCATOR(®) groups (0° vs. 20°: P < 0.0001; 0° vs. 30°: P < 0.0001; 0° vs. 40°: P < 0.0001), but was insignificant within the SFI-Anchor(®) groups (0° vs. 20°: P = 0.544; 0° vs. 30°: P = 0.134; 0° vs. 40°: P = 0.254; 0° vs. 60°: P = 0.979). It further revealed a significant increase in the retentive force between the LOCATOR(®) and the SFI-Anchor(®) (20°: P = 0.041; 30°: P < 0.0001; 40°: P < 0.0001), although there was no significant difference between the attachments at 0° (P = 0.623)., Conclusions: This study confirms that the retentive behavior of SFI-Anchor(®) is not influenced by implant axial inclination even at angulations of up to 60°. This study aimed to evaluate peri-implant tissue health of immediate loaded two implants retaining a mandibular overdenture with either magnetic or locator attachment. Thirty two completely edentulous patients (20 males/12 females) were randomly assigned into two groups. Each patient received two implants in the canine area of the mandible using flapless surgical technique. Mandibular overdentures were immediately connected to the implants with either magnetic (group I, GI) or locator (group II, GII) attachments. Peri-implant tissue health was evaluated clinically in terms of plaque scores (PI), bleeding scores (BI), probing depth (PD), implant stability (ISQ) and interleukin-1-β (IL-1b) concentrations in peri-implant sulcular fluid. PI, BI and PD were measured at mesial, distal, buccal and lingual surfaces of each implant. Radiographic evaluation was performed in terms of vertical (VBL) and horizontal (HBLO) alveolar bone loss. Evaluations were performed 2 weeks (T0), 6 months (T1) and 12 months (T2) after overdenture insertion. Plague scores, PD, IL-1b, VBL and HBLO increased significantly with time. ISQ decreased significantly with time. BI showed no significant differences between observation times. GI recorded significant higher PI, ISQ and IL-1b at T2 compared to GII. GII recorded significant higher VBL than GI at T2 only. For HBLO, no significant differences between groups were noted. VBL and HBLO showed a significant positive correlation with PD. In contemporary implant and prosthodontic practice, high levels of surgical and biologic success rates have been achieved. However, mechanical and technical complications continue to exist. Of these, one of the biggest challenges to the clinician is the conservative retrieval of fractured screw fragments from within the implants and/or abutments. While removal is desirable, the fragments cannot always be removed, and alternative solutions for the management of such clinical situations must be considered. This clinical report describes the replacement of 2 fractured, irretrievable, Locator abutment screws of a mandibular implant-retained overdenture with a cast dowel and laser welded Locator attachments. Fabrication of implant-supported maxillary or mandibular overdentures can seem to be difficult procedures. Many things could go wrong and/or unnoticed until the fabrication has been completed. Implants must be correctly surgically placed in viable bone at the proper angulation and spacing within an arch. The type of attachment must be considered and future treatment of the appliance should be simple and efficient. The appliance must function not only initially but also for many years to come. The author has found the use of the GPS attachment to be an ideal tool to achieve the goals of retention and stability. Careful planning is the most important part of this process, and understanding the benefits and risks of creating overdentures should be well understood by the dentists. By sequentially planning and treating these types of cases, the patient is able to function reasonably during the stages of implant healing. The final prosthesis is created and remaining teeth that held the transitional appliance in place are remove on the day of final seating. This is an excellent simplified retentive system option for those patients who are anxious about losing their teeth, even those teeth that are diseased and ugly.

Statement of Problem: Fracture of maxillary implant overdentures (IODs) is a frequent complication. However, no studies have examined the effects of clip materials on the stress applied to maxillary IODs., Purpose: The purpose of this study was to investigate the effects of clip materials for bar attachments on the stress applied to maxillary IODs without palatal coverage., Material and Methods: This study included finite element analysis and a model study. An edentulous maxillary model with 4 implants and an experimental overdenture with bar attachments was fabricated. Strain gauges were attached on the denture surface adjacent to the end of each clip. Four clip conditions (n=5) were prepared: 3 plastic clips, 1 plastic clip, 3 metal clips, and 1 metal clip. A vertical load of 50 N was applied on the IOD. The maximum principle strain (MPS) was statistically analyzed by 2-way ANOVA and the post hoc Tukey HSD tests with clip materials and clip numbers as factors (α=.05)., Results: The greatest stress was observed on the resin surface around the end of the clip on the loaded side. The MPSs between both clip materials (P=.048) and numbers (P<.001) differed significantly on the palatal side of the end of the center clip on the loaded side: the MPSs of the metal clips were significantly larger than those of the plastic clips, and the MPS of 1 clip was significantly larger than for 3 clips., Conclusions: The stress around plastic clips tended to be lower than that around metal clips., (Copyright © 2016 Editorial Council for the Journal of Prosthetic Dentistry. Retrospective data of 32 patients supplied with implant-supported and Locator-attached overdentures were screened for prosthetic complications and maintenance requirements, which were recorded and statistically analyzed. Mean observation time was 4.78 ± 1.72) years. Loss of retention was the most frequently observed event (n = 22). Damage and exchange of the insert holders (n = 4) and loosening of locator attachments (n = 2) and fracture of the insert holder (n = 2) were uncommon events; no loss of locator attachments was observed. Loss of retention in Locator-attached overdentures is frequent; correlating patient-specific parameters with prosthetic complications is necessary to define recommendations for the use of Locator attachments.

Patients: This report describes the case of a 48-year-old female patient who initially received a 4-unit fixed-fixed (FF) resin-bonded fixed dental prosthesis (RBFDP) for replacement of mandibular left second premolar and first molar. Twenty-one months later, debond of the RBFDP was found at one of retainers. A new fixed-movable RBFDPs with a modified non-rigid connector as well as increased resistance form features was placed and successfully retained at the 16-year clinical review., Discussion: RBFDPs are a conservative tooth replacement option that requires minimum tooth preparation and retention by a resin cement to etched enamel. However debonding is the most frequently seen complication and has been attributed to the relative movement between abutment teeth during function that stress the bonding interface. This case report highlights the use of modified non-rigid connector which allows relative movement between abutment teeth and therefore a reduced stress on the bonding interface., Conclusion: It is proposed that the use of modified non-rigid connectors that allow independent movement between the abutment teeth during function was responsible for the long term clinical success of fixed-movable RBFDPs., (Copyright © 2015 Japan Prosthodontic Society. Purpose: The aim of this study was to evaluate the oral health-related quality of life (OHRQoL) of patients treated with three different types of mandibular implant overdentures (IOD) after at least 3 years in function., Materials and Methods: A total of 122 patients treated with mini implant (50), locator (56), or bar (16) mandibular IODs participated. Modified plaque (PI) and gingival (GI) indices were also assessed., Results: Older patients and patients with mini mandibular IODs were the most satisfied with their OHRQoL. The bar overdenture presented the worst PI and GI results., Conclusion: The mini mandibular overdenture patients were assessed with a better OHRQoL than those with bar and locator IODs.

A 37 year old male presented with the complaint of a loose and bulky acrylic obturator prosthesis. He had previously tried to using a different acrylic obturator prostheses as well as both cobalt chromium and titanium framework obturators. The most successful previous prosthesis was a titanium based obturator which had performed well prior to a fractured clasp. Accordingly, following an exploration of the available surgical and prosthodontic treatment options, a further tooth borne partial maxillary obturator was provided successfully. The case highlights the relative merits and limitations of the use of cast titanium as a denture base material in partial denture and obturator construction.

Large defects of the human face often cause esthetic as well as functional disorders. We present a new technique for reconstruction of the mandible with prosthodontic rehabilitation in a single surgery, using the implant-supported, bar-retained overdenture as an external fixator. A 58-year-old patient presented with a near total defect of the mandible after cancer resection. For rehabilitation, the mandibular condyles were virtually positioned in the centre of the fossae, and four dental implants were planned. The position of the fibula segments as well as their angulation and lengths were adapted to the implant position. To transfer this plan into surgery, a combined cutting/implant drilling guide was computer-aided printed. To provide the correct angulation of the fibula segments, a CAD/CAM dental arch-bar was made from titanium, fulfilling three functions: to bear the provisional prosthesis; to stabilize the molded fibula as an external fixator; and to position the complete fibula with the prosthesis in a correct relation to the upper jaw and occlusion, as indicated by the prosthesis. Purpose: The aim of this study was to evaluate the axial tightening force applied by conventional and diamondlike carbon (DLC)-coated screws and to verify, through three-dimensional finite element analysis (FEA), the stress distribution caused by different framework materials and prosthetic screws in overdenture frameworks with different misfit levels., Materials and Methods: The axial tightening force applied by the screw was evaluated by means of a titanium matrix connected to a load cell. Conventional titanium or DLC-coated screws were tightened with a digital torque wrench, and the load values were recorded. The values were applied in an FEA to a bar-clip attachment system connected to two 4.0 × 11-mm external-hexagon titanium implants placed in an anterior edentulous arch. DLC-coated and conventional screws were modeled with their respective axial forces obtained on the experimental evaluation for three bar framework materials (titanium, nickel-chromium, and cobalt-chromium) and three levels of misfit (100, 150, and 200 μm). Von Mises stresses for prosthetic components and maximum principal stress and microstrains (maximum principal strains) for bone tissue were measured., Results: The mean force applied by the conventional screw was 25.55 N (± 1.78); the prosthetic screw coated with a DLC layer applied a mean force of 31.44 N (± 2.11), a statistically significant difference. In the FEA, the DLC screw led to higher stresses on the framework; however, the prosthetic screw suffered lower stress. No influence of screw type was seen in the bone tissue., Conclusion: Titanium frameworks reduced the stress transmitted to the bone tissue and the bar framework but had no influence on the screws. Higher misfit values resulted in an increased stress/strain in bone tissue and bar framework, which was not the case for retention screws.

Crown-root ratio (CRR) is commonly recorded when planning prosthodontic procedures. However, there is a lack of longitudinal clinical data evaluating the association between CRR and tooth survival. The aim of this longitudinal practice-based study was to assess the impact of CRR on the survival of abutment teeth for removable partial dentures (RPDs). Data were collected from 147 patients provided with RPDs at a dental hospital in Japan. In total, 236 clasp-retained RPDs and 856 abutment teeth were analyzed. Survival of abutment teeth was assessed using Kaplan-Meier methods and Cox's proportional hazard (PH) regression. The Cox PH regression was used to assess the prognostic significance of initial CRR value with adjustments for clinically relevant factors, including age, sex, frequency of periodontal maintenance programs, occlusal support area, type of abutment tooth, status of endodontic treatment, and probing pocket depth. Abutment teeth were divided into 1 of 5 risk groups according to CRR: A (≤0.75), B (0.76-1.00), C (1.01-1.25), D (1.26-1.50) and E (≥1.51). The 7-year survival rate was 89.1% for group A, 85.9% for group B, 86.5% for group C, 76.9% for group D, and 46.7% for group E. The survival curves of groups A, B, and C were illustrated to be quite similar and favorable. The multivariable analysis treating CRR as a continuous variable allowed estimation of the hazard ratio at any specific CRR value. When CRR = 0.80 was set as a reference, the estimated hazard ratio was 0.58 for CRR = 0.50 (95% confidence interval [CI], 0.36-0.91), 1.13 for CRR = 1.00 (95% CI, 0.93-1.37), 1.35 for CRR = 1.25 (95% CI, 1.02-1.80), 1.53 for CRR = 1.50 (95% CI, 1.15-2.08), or 1.95 for CRR = 2.00 (95% CI, 1.44-2.65). Purpose: Many different attachment systems (eg, bars, studs, magnets, telescopic copings) have been used to retain overdentures. The current study aimed to investigate the load transfer characteristics and to compare the stress levels of four attachment designs for mandibular overdentures retained by one central implant and two inclined distal implants., Materials and Methods: Photoelastic mandibular models fabricated with three screw-type implants (Tapered Screw-Vent, 3.75 × 13 mm) were placed in the parasymphyseal area. The center implant was vertically oriented to the midline, and the other implants were embedded in the canine areas with a 20-degree angulation relative to the center implant. Four overdentures with different attachment designs (bar, bar/ball, bar/distally placed Rk-1s, and Locators) were studied in the context of this model. Vertical loads (100 N) were applied to the central fossa of the right first molar area of each overdenture. Stress levels that developed in the denture-bearing areas and around the implants were observed photoelastically and evaluated visually., Results: The studied attachment designs showed low and moderate stress levels. The greatest stress was found with the bar/ball design, while the lowest stress levels were observed with the Locator attachment design., Conclusion: Stresses were concentrated on the loaded side for each design. All tested designs experienced moderate stress around the posterior edentulous area. None of the designs experienced more than moderate stress. The lowest stress was noted with the Locator attachments, which transmitted little discernible stress around the implants.

Purpose: To evaluate whether short (5.0 to 8.5 mm) dental implants could be a suitable alternative to longer (at least 11.5 mm-long) implants placed in atrophic maxillae augmented with autogenous bone for supporting dental prostheses., Materials and Methods: Twenty-eight patients with fully edentulous atrophic maxillae, whom had 5 to 9 mm of residual crestal bone height which was at least 5 mm thick, measured using computerised tomography (CT) scans, were randomised into two groups either to receive 4 to 8 short (5.0 to 8.5 mm) implants (15 patients) or autogenous bone from the iliac crest to allow the placement of at least 11.5 mm-long implants (13 patients). Bone blocks and the windows at the maxillary sinuses were covered with rigid resorbable barriers. Grafts were left to heal for 4 months before placing implants which were submerged. After 4 months, provisional reinforced acrylic prostheses or bar retained overdentures were delivered. Provisional prostheses were replaced, after 4 months, by definitive screw-retained metal-resin cross-arch restorations. Outcome measures were prosthesis and implant failures, any complications, peri-implant marginal bone level changes and patient satisfaction. Patients were followed up to 1 year after loading., Results: All patients were rehabilitated with implant-supported prostheses but two patients dropped out from the augmented group. One bilateral sinus lift procedure failed for infection, although short implants could be placed. One implant failed in the augmented group versus two short implants in two patients (Fishers exact test P = 1.00; difference in proportions = 0.06; 95% CI -0.28 to 0.17). All failures occurred before loading. Significantly more complications occurred in augmented patients: eight complications occurred in 5 augmented patients (all of them complained of pain 1 month after bone harvesting from the iliac crest) versus no complications in the short implant (Fisher's exact test P = 0.013; difference in proportions = 0.38; 95% CI 0.11 to 0.65). Both groups presented a significant peri-implant marginal bone loss at 1 year after loading (P < 0.0001); -1.05 (0.20) mm for short implants and -1.01 (0.16) mm for the augmented group, respectively, with no statistically significant differences between the two groups (mean difference -0.04 mm; 95% CI -0.22 to 0.14; P = 0.59). All patients were fully satisfied with the treatment and would have it again., Conclusions: This pilot study suggests that short implants may be a suitable, cheaper and faster alternative to longer implants placed in bone augmented with autogenous bone for rehabilitating edentulous atrophic maxillae, however, these preliminary results need to be confirmed by larger trials with follow-ups of at least 5 years.
