Objective: To evaluate the utility of panoramic radiographs in pre-prosthetic screening of edentulous arches. Material and Methods: Panoramic radiographs taken for three years were retrospectively analyzed. Observations from the radiographs shall be categorized and classified into either of the two categories, namely: 'findings with minimal impact on denture fabrication' and 'findings which affect denture fabrication and require further evaluation.' Anatomic variations, jaw pathologies, and residual ridge resorption patterns were assessed. Results: This study included the initial screening of 23,020 panoramic radiographs, out of which 505 (showing either one or both edentulous arches) were included for the study purpose. The age range of the subjects was from 21 to 94 years. 52.6% of the radiographs showed positive findings. More than half of the radiographs belonged to the males (52.5%). Hyperpneumatization of the maxillary sinus, crestal position of the mental foramen, and retained root fragments were the most common entities noted in the radiographs. Changes in the mental foramen were significantly higher in males than females (p=0.002). Conclusion: Observations from this study showed that panoramic radiographs have high utility for screening edentulous arches, and they should be used in routine clinical practice before denture fabrication.

Objectives: To digitally evaluate the trueness and fit of additively and subtractively manufactured fixed complete dentures in materials intended for definitive use., Methods: An edentulous maxillary model with implants at the left first molar, left canine, right canine, and right first molar site was digitized and a fixed complete denture was designed. This design was used to fabricate fixed dentures in an additively manufactured resin for definitive use (AM), a high-impact polymer composite (SM-CR), and a strength gradient zirconia (SM-ZR) (n = 10). Each fixed denture was digitized and the surface (overall, occlusal, except occlusal, and abutments), linear, and interimplant distance deviations were analyzed. The fit was assessed with the triple-scan protocol. Data were analyzed with Welch analysis of variance and Games-Howell tests (α = 0.05)., Results: SM-ZR led to lower overall deviations than AM, which had the highest occlusal and the lowest abutments deviations (P ≤ 0.007). SM-ZR had the lowest occlusal and SM-CR had the highest except occlusal deviations (P ≤ 0.002). AM mostly had higher linear and SM-CR mostly had higher interimplant distance deviations (P ≤ 0.043). AM led to the highest marginal gap at the left canine site, while SM-CR had the highest and SM-ZR had the lowest gaps at the right canine site (P ≤ 0.022)., Conclusions: SM-ZR dentures mostly had trueness and marginal fit similar to or better than the other groups. Background: Differences in the effects of cream-type denture adhesives and home liners remain unclear., Objective: This cross-sectional study aimed to clarify the differences in the oral-hygiene status and masticatory performance between participants using cream-type denture adhesives and those using home liners., Methods: Masticatory performance with and without cream-type denture adhesives and home liners was evaluated using colour-changeable chewing gum. Oral hygiene was assessed using the tongue coating index (TCI). Propensity-score matching was performed using oral moisture, presence of palatal or mandibular protuberances, type of residual ridge, denture type (complete or partial denture), and denture retention as background factors. Wilcoxon signed-rank test was used to examine the differences in masticatory performance with and without cream-type denture adhesives and home liners and differences in masticatory performance and TCI between the cream-type denture adhesive and home-liner groups., Results: This study included 38 and 40 cream-type adhesive and home-liner users, respectively, and 17 pairs were obtained after propensity-score matching. The Wilcoxon signed-rank tests showed that masticatory performance was significantly different with and without the use of home liners, and higher values were obtained with the use of home-liners. However, the cream-type denture adhesive and home-liner groups showed no significant differences in terms of the change in masticatory performance with the use of them. Furthermore, Wilcoxon signed-rank tests showed that TCI values were significantly high in the cream-type adhesive group., Conclusion: The findings in this study suggest that home liners improve masticatory performance and that participants regularly using cream-type denture adhesives have poorer oral hygiene., (© 2024 John Wiley & Sons Ltd.)

Purpose: To investigate the influence of different denture-bearing conditions on the masticatory function and patient-reported outcome measures (PROMs) of complete denture wearers., Methods: Sixty edentulous patients were selected and allocated into two groups according to the American College of Prosthodontics' (ACP) classification: non-atrophic (NAT) (Classes I and II) (n = 24) and atrophic (AT) (Classes III and IV) (n = 36). All patients received new complete dentures (CDs). The objective variables (masticatory performance and swallowing threshold) were assessed as well as the PROMs (oral health-related quality of life (OHIP-EDENT), patient satisfaction) and quality of the prosthesis, at baseline (using the old CD) and after 4 months new prostheses use. Data were analyzed by Mann-Whitney test followed by the Generalized Equations Estimation (GEE), linear regression and Chi-square test., Results: Higher masticatory performance was observed in the NAT group (p < .05) for both time points, baseline and after 4 months. However, compared to baseline, both groups showed significant masticatory improvement after 4 months (p < .05). Satisfaction and overall quality of life improved after 4 months with no difference between groups (p > .05). Regarding the quality of the CD, baseline results were significantly (p < .05) lower in the AT group, but after 4 months, no significant differences were found between groups and in intragroup analysis (p > .05)., Conclusions: The denture-bearing conditions seems to impact masticatory function, but the PROMs are barely affected., (© 2024 John Wiley & Sons Ltd.)

Purpose: This systematic review examined the effectiveness of soft denture relining (SDR) materials., Study Selection: A comprehensive search of MEDLINE, Cochrane Library, and ICHUSHI was conducted up to July 26, 2020. Target outcomes were patient satisfaction, oral health-related quality of life (OHRQOL), masticatory ability (MA), denture functional duration, residual ridge resorption (RRR), and microbial contamination. An organization specializing in literature searches performed the reference searches, and two reviewers independently selected the literature sources, extracted the data, and assessed the risk of bias. The reviewers resolved any disagreements concerning the assortment of literature sources through discussion. SDR included acrylic- and silicone-based materials, which were evaluated separately., Results: Reviewers selected 7, 5, 11, 1, 4, and 6 studies to assess patient satisfaction, OHRQOL, MA, functional duration, RRR, and microbial contamination, respectively. The results confirmed that SDR improved patient satisfaction, OHRQOL, MA, and RRR. However, the functional duration of SDR material is shorter than that of hard denture relining (HDR) or acrylic resin material. Furthermore, SDR material is more susceptible to microbial contamination in the long term. The risk of bias for the included studies tended to be high because of specific issues (difficulty in blinding SDR versus HDR)., Conclusions: For patients who wear complete dentures, SDR often provides beneficial outcomes such as pain reduction and recovery from MA. However, caution should be exercised regarding their use owing to insufficient functional duration and the possibility of microbial contamination during long-term use.

Purpose: To evaluate and compare patient satisfaction levels in edentulous patients treated with different configurations of implant-supported prostheses as well as previous prosthesis experiences before implant treatment., Materials and Methods: A study population of 142 patients was identified and separated into four treatment groups: group 1 comprised 43 patients treated with an implant-supported overdenture in the mandible and conventional complete denture in the maxilla; group 2 comprised 32 patients treated with implant-supported overdentures in the maxilla and mandible; group 3 comprised 26 patients treated with an implant-supported overdenture in the mandible and a fixed prosthesis in the maxilla; and group 4 comprised 41 patients treated with implant-supported maxillomandibular fixed restorations. Questionnaires asking about masticatory performance, pronunciation, comfort, and social ability were used to evaluate treatment outcomes., Results: The patients in group 4 were significantly more satisfied regarding masticatory performance than other treatment modalities. Visual analog scale (VAS) scores for comfort and social ability were similar in groups 3 and 4 and significantly higher than those in groups 1 and 2. The patients who used a conventional removable prosthesis before implant treatment were more satisfied with their implant-supported prosthesis regarding social ability (P &#61; .03)., Conclusions: The treatment design of an implant-supported fixed prosthesis in the maxilla and implant overdenture in the mandible provided a comparable level of satisfaction with maxillomandibular fixed prosthesis for edentulous patients. This combination served as a practical solution especially for patients with mandibular atrophy who would require extensive surgeries to support maxillomandibular fixed prostheses. In the maxilla, patient satisfaction with an implantsupported overdenture was not superior to that with a conventional prosthesis. Note that previous removable prosthesis experience may influence social comfort for patients.

During the complete denture restoration process, accurate impression making is a crucial step for achieving good denture retention. With the increasing popularity of intraoral scanning technology in fixed restoration, the use of intraoral scanning technology in complete denture restoration for edentulous jaw has also been developed. This article systematically reviews the research progress and application of intraoral scanning of edentulous soft tissue, focusing on difficulties in intraoral scanning of edentulous jaws, scanning accuracy, clinical application effects, as well as precautions involved. The aim is to provide references for clinical application.

This study investigated the integration of online learning into complete denture rehabilitation (CDR) training for dental interns, highlighting its impact on their education and readiness for clinical practice. Given that CDR poses significant challenges due to its technical complexity and profound influence on patient well-being, online learning has emerged as a strategic educational tool to enhance interns' knowledge and skills. This research included the administration of a comprehensive questionnaire to 63 dental interns to assess their backgrounds, experiences with online learning, and attitudes toward its application in CDR education. The results revealed strong engagement with online learning, with a majority valuing its flexibility, accessibility, and capacity to facilitate self-paced, individualized learning. Despite the enthusiasm for online modalities, the results identified notable gaps in interns' confidence in and preparedness for performing CDR, highlighting the need for targeted improvements in online curriculum development. By emphasizing the essential role of innovative teaching methods, including virtual reality (VR), this study underscores the need for a balanced educational approach that combines traditional and digital platforms. This strategy aims to prepare future dental professionals for the complexities of modern clinical environments, ensuring that they are well equipped to meet the diverse needs of the edentulous population., (© 2024. Centric relation (CR) was used in the complete dentures for edentulous jaws firstly. Then, CR was gradually applied in the clinical diagnosis and treatment of dentate jaws. Regarding the application of CR in dentate jaws, there are many doubts about its rationality. For instance, should CR be the core of diagnosis and treatment of temporomandibular disorders? Should CR be used as the jaw position of establishing occlusion in occlusal reconstruction? Should CR serve as the target jaw position in orthodontic treatment? Meanwhile, varieties of CR clinical applications are objective. According to the existing evidence, this paper summarizes the applicable conditions of several main clinical situations with little controversy. We preliminarily put forward the decision tree for the clinical application of CR, which can be used as a reference in clinical practice.

Purpose: This systematic review aims to compare clinical outcomes of digital dentures with conventional dentures., Materials and Methods: This study followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and was registered in Prospero. The formulated population, intervention, comparison, outcome (PICO) question was "What is the clinical outcome of digital versus conventional complete dentures (CDs) in edentulous patients?". The search strategy used three main electronic databases and an additional manual search was completed in August 2023 by following an established search strategy. Initial inclusion was based on titles and abstracts, followed by a detailed review of selected studies, and clinical studies that evaluated the clinical outcome of digital (milled or 3D-printed) versus conventional CDs were included. A qualitative analysis for clinical studies was used to assess the risk of bias. The certainty of the evidence was assessed according to the grading of recommendations, assessment, development, and evaluations (GRADE) system. In addition, a single-arm meta-analysis was performed to evaluate the retention between digital versus conventional CDs., Results: The initial search yielded a total of 947 articles, out of which 19 were selected for a comprehensive review, and six met the eligibility criteria to be included in this systematic review. The computer-aided design and computer-aided manufacturing (CAD-CAM) CDs showed increased retention, no relevant differences in oral health-related quality of life (OHRQoL), and shorter working time compared to conventional dentures. Two studies were eligible for meta-analysis; retention was significantly better among CAD-CAM fabricated dentures (standardized mean difference [SMD] 0.501) than conventional dentures. The heterogeneity between studies was high (95% CI: 0.049-0.952)., Conclusions: Clinically, both the milled and the 3D-printed CD fared better than conventional dentures in terms of retention, reduction in the number of appointments, improved patient comfort, and improved predictable maintenance of the denture. Objectives: Conventionally and digitally manufactured removable complete dentures with different dentition forms were examined for manufacturing accuracy (trueness, precision), fracture forces under torsional loading and subsequent repairability., Methods: A total of 90 mandibular prostheses were manufactured. Ten were made using the injection molding technique and finished with prefabricated teeth. 40 bases each, were manufactured subtractively and additively. Digitally the prosthesis' dental arch was divided either into two quadrants or three sextants, or kept as full arch. Afterwards, ten additive and subtractive bases were finished with prefabricated teeth and ten of each with milled quadrants, sextants and full arches. After manufacturing, all specimens were rescanned for accuracy comparisons using the Root Mean Square (RMS). Lastly, all specimens were tested to failure under torsional loading., Results: Conventionally manufactured dentures showed the greatest deviation in accuracy. The type of base manufacturing did not determine the fracture resistance of the prostheses. The dentition form had a significant influence. While prefabricated teeth (86.01 ± 19.76 N) and quadrants (77.89 ± 9.58 N) showed a low fracture resistance, sextants (139.12 ± 21.41 N) and full arches (141.05 ± 17.14 N) achieved the highest fracture forces. Subtractive bases with prefabricated teeth or quadrants were assessed to be repairable, digital dentures with full arch were assessed as not repairable., Significance: The presented testing set-up is suitable to determine the fracture behavior of dentures rather than of standards. Statement of Problem: The demand to streamline workflow while increasing patient comfort and satisfaction in the manufacturing of removable complete dentures (CDs) is growing, and computer-aided design and computer-aided manufacturing (CAD-CAM) technology has gained prominence. However, a systematic review and meta-analysis of the clinical performance and patient-related outcome measures (PROMs) of digitally fabricated complete dentures is lacking., Purpose: The purpose of this systematic review and meta-analysis was to determine whether digitally fabricated CDs improve the clinical performance and PROMs compared with conventionally fabricated CDs., Material and Methods: This systematic review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and was registered at the International Prospective Register of Systematic Reviews (CRD42023433909). This review aimed to address the question "Do digitally fabricated CDs improve clinical performance and PROMs compared with conventional CDs?" Two independent authors conducted electronic searches up to September 2023. Denture base retention and patient satisfaction were analyzed through the continuous outcome evaluated by mean difference and standard deviation, with 95% confidence intervals. The risk of bias in each study was assessed using the Risk of Bias In Non-randomized Studies of Interventions and the Risk of Bias based on the Cochrane risk of bias tool for randomized trials. The certainty of evidence was assessed using Grading of Recommendations, Assessment, Development and Evaluation., Results: A total of 1505 articles were found during the search, and 16 were included in this review. The main outcomes were grouped into clinical performance (retention, adaptation or fit, clinician's denture quality assessment, masticatory efficiency, and occlusal force) and PROMs (patient satisfaction, oral health related quality of life, final choice, and willingness-to-pay). In terms of denture base retention, between conventional CDs and printed CDs analyses, conventional CDs exhibited better retention (P=.02), but no significant difference was found between the conventional CDs and milled CDs (P=.20). In terms of patient satisfaction, no statistically significant difference was found between the manufacturing methods of digitally fabricated CDs (printed CD, P=.55; milled CD, P=.08)., Conclusions: Digitally fabricated CDs showed similar or better clinical performance than conventional CDs, and different types of digital manufacturing processes for complete dentures did not influence PROMs., (Copyright © 2024 Editorial Council for The Journal of Prosthetic Dentistry. Purpose: To evaluate the accuracy of tooth positions in printed complete dentures with different designs and teeth positioning techniques., Materials and Methods: Five different designs of complete dentures and teeth positioning techniques were evaluated in this in vitro study. In Group I, the denture bases and teeth were designed as a single piece. In groups II and III, the denture bases were designed separately, and the denture teeth were designed as separate teeth. In groups IV and V, the denture bases were designed separately, and the denture teeth were designed as a single piece. Teeth positioning keys were designed for groups III and V. The dentures of all groups were scanned, and the data were imported to the surface matching software to evaluate the accuracy of the teeth positions. Statistical analysis was done using One -way ANOVA of Variance. The significance level was set at p ≤ 0.05., Results: The results showed that the highest deviations of the positions of the canines (0.0781 ± 0.0154 mm) and the first molars (0.0611 ± 0.0055 mm) were found in Group II. On the other hand, Group I showed the least deviations of the positions of the canines (0.0287 ± 0.0054 mm) and molars (0.0354 ± 0.005 mm)., Conclusions: The most accurate tooth positions are obtained in monolithic printed complete dentures. Statement of Problem: Long-term data are sparse on the use of metal-acrylic resin implant-supported fixed complete dentures., Purpose: The purpose of this retrospective clinical study was to assess the prosthetic survival and complication rate in patients treated with metal-acrylic resin implant-supported fixed complete dentures (ma-IFCDs)., Materials and Methods: The patient record system at the Dental College of Georgia was searched from 2006 to 2020 to identify all patients treated with ma-IFCDs by the graduate prosthodontic and general practice residents during this time. Variables collected were date of delivery of the prosthesis, arch treated (maxilla, mandible), date of the last visit, status of the opposing arch (complete arch fixed implant-supported prosthesis, complete denture, removable implant-supported overdenture, teeth and removable partial denture, teeth and implants, teeth only), date of replacement, and reason for replacement. Reasons for replacement were grouped as fracture of the framework, implant loss, patient-related concerns, catastrophic acrylic resin fracture (judged to be beyond predictable repair by the clinician), and extensive occlusal wear (judged to have worn to the point of requiring replacement)., Results: A total of 84 arches in 55 patients who met the inclusion criteria were identified from the electronic health review. Of the 84 complete arch prostheses, 43 were maxillary and 41 were mandibular. The median follow-up time was 2.8 years (interquartile range [IQR] 0.95, 4.65). A total of 31 of the 84 arches were identified as having failed, requiring replacement. The most frequent reason for replacement was catastrophic fracture of the acrylic resin (61%), followed by excessive occlusal wear (19%). It was found that 13% of the failures were associated with patient-related concerns, and 7% with the loss of an implant. Survival rates were analyzed based on the opposing arch. The combined survival rate (Kaplan-Meier, log-normal modeling) for ma-IFCDs was 54% at 5 years (95% confidence interval [CI] 42%, 66%) and 32% at 10 years (95% CI 20%, 47%)., Conclusions: Mid- to long-term survival rates for ma-IFCDs are concerningly low. The findings suggest that the status of the opposing arch plays a role in the survival rate of ma-IFCDs. This study aims to assess the dimensional accuracy of complete denture bases fabricated from different CAD/CAM technologies and a conventional method, including milling (CNC), PolyJet (PJ), laser sintering (SLS), digital light processing (DLP), and injection molding (IM). It also examines the influence of the removal of technology-specific connectors or support structures when present. Denture base surfaces were digitized using a laboratory scanner, and virtual measurement points were calculated with tetrahedral reference geometries. Defined distances were measured in all spatial directions and compared to design data (p<0.05), revealing significant differences in sagittal (p=0.004), transversal (p<0.001), and vertical (p<0.001) dimensions. Connector removal had no significant impact for CNC but significantly affected DLP. All technologies yielded clinically acceptable results, with CNC milling demonstrating the best overall outcome.

Purpose: To review the literature on CAD/CAM removable complete and partial dentures regarding patient- and clinician-reported outcomes., Materials and Methods: We performed an electronic search of the Cochrane Central Register of controlled trials (CENTRAL), MEDLINE (Ovid), EMBASE, Scopus, and Web of Science databases (last update: March 2023). All clinical trials or observational studies investigating CAD/ CAM removable dentures (conventional or implant-retained) were included., Results: The search yielded 4,035 records and led to 58 included studies. Results suggest that CAD/CAM complete and partial dentures, when compared to the conventional ones, can save time and resources while being rated either noninferior or superior by patients and clinicians in most studies. However, consensus on which workflow offers fewer adjustments and postoperative sessions is still pending., Conclusions: The literature suggests that CAD/CAM complete and partial dentures can combine substantial time and cost savings, with patient and clinician experiences at least comparable to conventional prostheses. Given the low evidence level of existing studies, future well-designed randomized trials with large sample sizes are required to confirm those advantages.

Background: Decreased salivary flow can make the patients uncomfortable with their complete dentures and affects the retention of the dentures. Milling and 3D printing have become an alternative to conventional denture construction techniques. The goal of this study was to evaluate the effect of conventional and digital techniques of the complete denture construction on the denture surface wettability with distilled water and saliva substitute before and after thermocycling., Methods: A total of 30 specimens were utilized in the present study. Specimens were divided according to the construction techniques into 3 groups (n = 10 each). Group I: Heat-polymerized polymethylmethacrylate (PMMA) group, group II: Milled group, and group III: 3-dimensional (3D)-printed group. All the specimens were subjected to 2000 cycles of thermal aging in a thermocycler. The wettability of all specimens to water and saliva substitute was measured via a contact angle goniometer (Olympus TGHM, Rame-hart Inc, USA) before and after thermocycling. Descriptive statistical analysis, plots, and the Shapiro-Wilk test were used to verify normality for each variable. One-way ANOVA was used to compare the 3 study groups, while paired samples t-test was used to compare the differences within each group (P < .05)., Results: The smallest contact angle of drop of water to the denture base specimens before and after thermocycling were recorded in the milled group (53.0 ± 4.77 and 50.27 ± 2.30, respectively), followed by the heat polymerized PMMA group (85.65 ± 4.71 and 65.06 ± 2.27, respectively), and the 3D-printed group (91.34 ± 6.74 and 90.86 ± 8.57, respectively). While the smallest contact angle of drop of saliva substitute to denture base specimens was recorded in the milled group (56.82 ± 2.29 and 34.85 ± 7.51, respectively), followed by the 3D-printed group (72.87 ± 4.83 and 58.14 ± 9.58, respectively) and the heat polymerized PMMA group (83.62 ± 4.12 and 67.82 ± 4.93, respectively). There was statistically significant difference between the groups (P < .05). A significant decline in the average contact angle of drop of saliva has been reported in all groups after thermocycling. The contact angle values differed significantly between saliva substitute and distilled water in both 3D-printed and milled groups after thermocycling (P < .001)., Conclusions: The milled denture base material presented the best wettability to water and saliva substitute than the 3D-printed and the heat-polymerized PMMA materials. Edentulism (total tooth loss) can be managed with four possible options: no prosthetic replacement, conventional complete dentures, implant-retained overdentures (removable), or implant-supported bridgework (fixed). Selection of these choices is influenced by social, medical, anatomical, technical, and economic factors. The care team involved in the decision-making process (be it patients, clinicians and occasionally commissioners of services) should be aware of the different treatment options and their potential impact on the patient's quality of life (QoL). The cost effectiveness of this impact can also be evaluated. Knowledge of the life-long sequelae of edentulism will also help practitioners guide patients in their treatment planning decisions when they may be approaching an edentulous state, preparing them for the challenges that may lie ahead. This paper aims to address the impact that edentulism has on QoL and the treatment options, focusing on the patient perspective.

Background: While we attempt to restore occlusion in completely edentulous patients there is a consensus among clinicians to articulate the cast using average value and avoid performing facebow transfer. Various studies question the application of facebow transfer in the fabrication of removable complete denture fabrication., Aim: This study was therefore aimed to study the effect on condylar guidance with casts mounted by the facebow method, Bonwill's method, and radiographic method., Methods: A cross-sectional study was performed using casts of edentulous patients attending the Department of Prosthodontics. After establishing a tentative jaw relationship using wax occlusal rims in the edentulous participants, the casts were mounted by Bonwill's method and the facebow transfer method to a semi-adjustable articulator. Protrusive records of the subjects were used to program the articulator to obtain the condylar guidance values. Digital lateral cephalographs were made both in centric and protrusive jaw relationships. The mandibular condyles were then traced and overlapped to obtain radiographic values. The condylar guidance values obtained by Bonwill's method, facebow method and radiographic method were compared using Wilcoxon's signed rank test and Mann-Whitney U test. The SPSS Statistics for Windows (Version 21.0. Armonk, NY, USA:IBM Corp.), and, at a probability value of 0.05, the significance of every statistical test was predetermined., Results: Ten edentulous 45-70 year olds participated in the study. The mean discrepancy between radiography and facebow methods was 12.8° for the right side and 12.7° for the left (P < 0.005). The difference between radiography and Bonwill's methods was significant (P < 0.005), with a mean difference of 34.3° for the right side and 34.7° for the left side. The aim of this report was to digitize traditional denture relining using a digital duplication method, in addition to assessing the wear resistance of three-dimensional (3D) printed denture teeth. A complete denture was relined using light body impression. The denture with impression was scanned yielding a standard tessellation language file that was designed to print the denture base and teeth. The printed teeth were fitted into the sockets of the printed denture base and then bonded using auto-polymerized acrylic resins, followed by finishing and polishing. Dentures were inserted and fit and occlusion were adjusted as needed, and the patient was scheduled for follow-up appointments at one week, three months, and six months. At each follow-up visit, dentures were scanned using a 3Shape E3 desktop scanner and scans were superimposed. The occlusal wear was assessed in reference to the first scan after the denture insertion visit. The accuracy of the intaglio surface of dentures was within clinically acceptable limits. The clinical evaluation of inserted dentures in terms of retention, occlusion, esthetic, and patient satisfaction was encouraging. Using digital duplication, conventional dentures could be relined. Statement of Problem: Complete denture wearers present with less sensory function than dentate individuals because of the loss of periodontal receptors. However, the role of the palatal receptors on such function is still unclear., Purpose: The purpose of this systematic review and meta-analysis was to evaluate the influence of palatal coverage associated with complete dentures or palatal devices on oral stereognosis in edentulous or dentate adults., Material and Methods: Eight major literature databases and the nonpeer-reviewed literature were searched up to December of 2021. Randomized clinical trials and nonrandomized clinical trials comparing oral stereognosis in edentulous or dentate participants using conventional complete dentures or implant-supported prostheses or those using palatal devices were included. Bias was assessed with Cochrane tools (ROBINS-I and RoB 2.0). Meta-analyses were conducted to compare oral stereognosis before and after palatal coverage (α=.05). Certainty of the evidence was verified using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach., Results: Of 14 included studies, the risk of bias was considered low for 9 studies and moderate for 5 studies. Despite the very low certainty of evidence, meta-analyses showed no differences in oral stereognosis immediately after complete denture insertion (standardized mean difference [SMD]=0.302; 95% confidence interval [CI]: -0.124, 0.728; P=.165; I 2 =92.4%). Oral stereognosis scores improved after ≥1 month of complete denture use compared with initial evaluations without them. Qualitative analysis indicated that oral stereognosis was not influenced by palatal coverage shortly after complete denture installation and that oral stereognosis scores improved over time after prosthesis use., Conclusions: Oral stereognosis improved with the use of palate-covering prostheses over time. Objective: To compare the efficacy of test (ultrasonic cleaner combined with immersion in denture cleanser solution) and control (immersion in denture cleanser solution followed by conventional brushing) denture cleaning interventions in enhancing denture cleanliness, reducing denture stomatitis, and improving patient satisfaction., Methods: A prospective, single-blind, block-randomised, two-period crossover, superiority-controlled clinical trial was conducted of a 3-month intervention. The study design included a pre-intervention period (2 weeks), intervention period one (3 months), washout period (2 weeks), and intervention period two (3 months). A total of 56 community-dwelling elders were block-randomized into either sequence Test/Control or sequence Control/Test. The intervention, period, and carryover effects for the changes in the cleanliness of extensive partial and complete acrylic dentures, denture stomatitis, and changes in patient satisfaction were estimated using Generalized Estimating Equations models., Results: Percentage plaque area coverage, patient satisfaction, and denture stomatitis were significantly improved for both intervention and control arms after 3 months (P < 0.05). The intervention arm was found to significantly improve denture cleanliness (P < 0.001) and patient satisfaction (P = 0.002) more than the control arm. Denture-wearing habits and denture age were also significantly associated with the changes in denture plaque coverage (P < 0.05). However, the effect of the test intervention on denture stomatitis was not significantly different compared to the control arm (P = 0.284)., Conclusion: This study revealed that the test intervention group was significantly more effective than the control group in improving denture cleanliness and patient satisfaction among community-dwelling elders. ABSTRACT Numerous pathogens, including SARS-CoV-2, can remain viable on surfaces over days, which favors cross-contamination. Preventive measures are essential to prevent infections and control the spread of COVID-19. Fox ruler is an essential device in the clinical planning of complete dentures and the contact with the patient’s oral fluids during handling is unavoidable. Despite its importance, the conventional polycarbonate Fox ruler is easily damaged in the sterilization process. Therefore, the purpose of this article is to propose the use of a disposable Fox ruler. The confection is carried out in a simple way with wooden toothpicks joined by cyanoacrylate glue, following the same shape as the conventional one, but with the additional advantage of being made in an individual size according to the upper edge. The proposed disposable Fox ruler makes it possible to obtain the parallelism of the upper wax roller with the bipupillary line and with the Camper plane. Therefore, the disposable device described in this article was able to perform the same functions as the conventional one, in a practical way, without risks of cross contamination and with low cost.

Background/Aim. Patients’ expectations with regard to their complete dentures are usually high. It is quite challenging to meet the patient’s criteria for denture satisfaction. Moreover, it also becomes a challenging task when the patient’s expectation exceeds the patient’s satisfaction regardless of the denture quality. The aim of this study was to determine the correlation in the denture satisfaction levels between the dentist and complete denture wearers. Methods. The study included 134 completely edentulous participants, between 48 and 65 years of age. The data were recorded through a validated questionnaire having a Cronbach α validity of 85%. Sociodemographic data, patients’ priorities, dentists' and patients' satisfaction with the dentures were recorded and rated on a Likert scale (1 to 5). Spearman’s correlation was applied to measure the similarity between the dentist’s and patient’s satisfaction score. Results. The mean dentists' and patients' overall satisfaction scores were calculated as 3.58 ± 0.74 and 3.538 ± 0.521, respectively. Participants ≥ 60 years of age and females showed a higher correlation than younger patients and men, while the employed participants showed a stronger correlation between dentist’s and patient’s satisfaction, compared to the opposite. In the educated participants a positive, moderate correlation between dentist's and patient's satisfaction was shown. The correlation between dentist’s satisfaction with occlusion and patient’s satisfaction with comfort was moderately strong (p < 0.05); weakly positive correlation (p < 0.05) was found between dentist’s satisfaction with the quality of denture and extension and patient’s satisfaction with aesthetics, mastication, phonetics, and comfort. Conclusion. A positive correlation between the dentists' and patients' satisfaction regarding denture quality was shown in older-age patients, females, as well as employed and educated patients. Improving dentist-patient communication is the most useful strategy to improve patients’ satisfaction with their dentures.

Background: While many denture base materials are currently available on the market, little data exists regarding their dimensional stability after exposure to the oral environment. This study aimed to evaluate the effect of thermocycling on the trueness and precision of milled, 3-dimensional (3D)-printed, and conventional digitally fabricated complete denture bases (CDBs)., Methods: A completely edentulous maxillary stone model was scanned to generate a standard tessellation language (STL) file; this was imported into metal-milling-machine software (Redon Hybrid CAD-CAM metal milling machine, Redon, Turkey) to produce a metal model for fabricating 30 CDBs. These were divided into three groups (n = 10 in each) according to the construction technique: group 1, CAD-CAM milled CDBs; group 2, 3D-printed CDBs; and group 3, conventional compression molded CDBs. All CDBs were scanned after fabrication and evaluated before and after thermocycling using superimposition. The data were analyzed using a one-way ANOVA, Tukey's post hoc test, and a paired t-test., Results: The level of trueness between the CAD-CAM milled, 3D-printed, and compression molded CDBs showed significant differences before and after thermocycling (P < 0.05). Group 1 showed the highest degree of trueness before and after thermocycling, group 3 exhibited a higher degree of trueness than group 2 before thermocycling, and group 2 had a higher degree of trueness than group 3 after thermocycling. There was a significant difference in the precision for each CDB type before and after thermocycling (P < 0.05)., Conclusion: The trueness of the CAD-CAM milling system in complete denture (CD) fabrication is superior to that of the 3D printing and conventional compression molding systems before and after thermocycling. Thermocycling had a significant effect on the precision of all CDB types. Purpose: To evaluate the masticatory efficiency of dentate and rehabilitated complete denture patients in various facial forms., Materials and Methods: A total of 60 subjects were selected for the study; 30 subjects belonged to the dentate group (D Group) and 30 subjects belonged to the completely edentulous group rehabilitated with conventional complete dentures (CECD Group). All patients were divided into a subgroup (brachyfacial, mesofacial, and dolichofacial facial forms) using facial index measurement and cephalometric analysis. After the subjects were divided into groups, masticatory efficiency testing was performed with spectrophotometric analysis using diluted expectorate of chewed natural beetroot test food (10, 15, and 20 strokes). The mean absorbance value was considered as the mean masticatory efficiency from which the average masticatory efficiency percentage was determined using a masticatory efficiency percentage equation and considered as the masticatory efficiency of that particular subject., Results: Statistical analysis revealed that in the D Group, brachyfacial subjects exhibited the highest masticatory efficiency (70.9 ± 12.64%), followed by mesofacial (60.6% ± 10.66%) and dolichofacial (34.3% ± 10.81%) subjects. In the CECD groups, the results for brachyfacial were highest (52.7% ± 8.11%), followed by the mesofacial (42.3% ± 6.52%), and dolichofacial (29.3% ± 7.76%) subjects., Conclusions: Along with other determinant factors, facial form plays an influential role on masticatory efficiency. Brachyfacial subjects exhibited the highest masticatory efficiency, followed by mesofacial and dolichofacial subjects in both study groups.

Purpose: To evaluate the clinical effect of three impression methods, conventional, closed-mouth, and tissue conditioner, on complete denture fabrication., Methods: 60 subjects (edentulous with severely resorbed alveolar ridges - Atwood classification III or IV) who visited the Prosthodontic Department of Wuxi Stomatology Hospital, China, between January 2022 and June 2023, were selected for this study. The subjects were randomly divided into three groups of 20: a conventional impression group (CI group), a closed-mouth impression group (CM group), and a tissue conditioner group (TC group). Three months after denture restoration was completed, denture quality was assessed by clinicians in terms of marginal extension, retention, and stability. In addition, patients completed the oral health impact profile-edentulous (OHIP-EDENT) questionnaire to provide subjective satisfaction evaluations of the final denture restoration outcomes., Results: The comprehensive denture quality evaluation results showed that the TC group had the lowest score, which was significantly lower than that of the CM (P= 0.014) and CI (P< 0.001) groups. The average score of the CM group was also significantly lower than that of the CI group (P= 0.004), indicating that tissue conditioner restoration was the most effective method. The OHIP-EDENT scores gradually decreased across the groups from CI to CM to TC (P= 0.001), indicating patients' oral health was significantly improved using tissue conditioner., Clinical Significance: Tissue conditioner is a suitable dynamic functional impression method. This report describes a step-by-step staged digital protocol for the terminal dentition patient using a Provi-guide. This protocol will improve implant placement accuracy, shorten treatment time, and assure an implant-supported fixed complete denture immediately loaded after extraction of the remaining terminal dentition and complete edentulism. The novelty of the Provi-guide technique described is that it serves 2 objectives: (1) a nonlimiting surgical guide, and (2) a fixed interim prosthesis. Implants placed in the described digital stagged protocol can serve both the interim and the final prosthesis.

Inflammatory movable tissues in the maxillary anterior region are the result of excessive stimulation of the anterior alveolar ridge, as the maxillary denture habitually rotates anteriorly and upward during mastication. The pressure required for a functional impression makes it difficult to obtain the accurate anatomy of the inflamed mucosal tissues. An intraoral scanner can be used to record the movable tissues in an immobile state. However, for removable dentures, the oral scanner is limited by the difficulty in evaluating the denture borders or the posterior palatal seal area accurately. Statement of Problem: Clinical trials comparing outcomes associated with digital complete dentures (CDs) fabricated from intraoral scan data with those of CDs fabricated by using the conventional workflow are lacking., Purpose: The purpose of this randomized clinical trial was to evaluate the clinical performance of and patient satisfaction associated with digitally versus conventionally fabricated CDs., Material and Methods: Eight participants requiring CDs were enrolled in this study. Two sets of CDs were fabricated for each participant. One set was fabricated by using a digital workflow, which involved digital scanning with an intraoral scanner, whereas the other set was made by using the conventional workflow. The participants were given 1 set of CDs for 1 month and another set for the next month. The order of placing CDs was randomly selected for each participant. The internal adaptation, masticatory force, and masticatory efficiency of the CDs in each group were evaluated for objective analysis. Additionally, a questionnaire was provided to the participants, and the responses were evaluated for subjective satisfaction analysis. All parameters were analyzed by using t tests (α=.05)., Results: The internal adaptation did not statistically significantly differ between the conventional and digital CDs with regard to the maxillary arches (P=.406) and mandibular arches (P=.412). The average masticatory force (P=.051) and maximum masticatory force (P=.110) likewise did not statistically significantly differ between the 2 types of CDs. Masticatory efficiency, expressed via the mixing ability index, was statistically better for conventional CDs than the digital CDs (P=.009). No statistically significant differences were observed between the 2 types of CDs in terms of overall patient satisfaction as assessed by using the study questionnaire (P=.172 for maxillary CD and P=.161 for mandibular CD). However, the conventional CDs were statistically significantly better than the digital CDs with regard to subjective satisfaction with pronunciation ability (P=.006)., Conclusions: The digital CDs were inferior to the conventional CDs in terms of masticatory efficiency and pronunciation. However, internal adaptation and overall patient satisfaction were comparable between conventional and digital CDs. Background: This study aimed to compare the perceived masticatory ability (PrMA) in completely edentulous patients (EDPs) with thermoplastic conventional complete dentures (CDs) versus single implant-retained mandibular overdentures., Methods: The current study was conducted in the outpatient Prosthodontic Clinic, Faculty of Dental Medicine, Al-Azhar University, Cairo, Egypt. PrMA was evaluated in 45 completely edentulous patients (46% males, mean age 50.4 ± 4.7 years). Each patient received a thermoplastic PMMA complete denture (Polyan IC TM Bredent GmbH & Co.KG, Germany). The PrMA was evaluated at one-month and six-month intervals of denture use. An immediate loading single implant was placed into the mid-symphyseal for each patient, and the denture was adjusted. Subsequently, the PrMA was reevaluated after one month and six months. The data were collected and statistically analyzed using the SPSS @ V25 to assess the changes in PrMA., Results: The PrMA demonstrated improvement after six months of thermoplastic conventional denture use. However, this improvement was not statistically significant ( p  = 0.405). In addition, the PrMA showed a substantial increase following a single implant placement at one and six months ( p  < 0.001) of the overdenture use compared to the conventional denture. In the digital workflow of complete denture fabrication, one solution for producing computer-aided design and computer-aided manufacturing dentures has been to mill the denture teeth and base separately and then bond them together. The correct bonding of the denture teeth and base is important to reproduce the designed occlusion in the definitive prosthesis. A novel technique is described to assist in the accurate positioning of denture teeth on the denture base by constructing auxiliary positioning slots on the denture base and auxiliary positioning posts on the denture teeth. Objective: Evaluate the assimilation capacity and ease of handling of the Fox plane accessory by dentistry students, through a questionnaire about the experience in using this device compared to the conventional one. Its intention is to minimize possible interpretation errors and challenges that the traditional method determines., Methods: After approval by the Research Ethics Committee, registration at Sistema Nacional de Informações sobre Ética em Pesquisa/National Information System on Research Ethics (SISNEP) and signing of the free and informed consent form, 51 undergraduate students treating patients in need of complete dentures at Instituto de Ciência e Tecnologia/Institute of Science and Technology (ICT) Unesp in São José dos Campos completed a questionnaire to evaluate the technical ease of use and provider acceptance. The obtained data were submitted to statistical analysis, evaluating technical ease and acceptance by operators., Results: The results were analyzed using descriptive and inferential statistics using the Jamovi 2.2.5 software. From the responses, the frequency was obtained for each question in the questionnaire, then the weighted mean was calculated, average rating and percentage. Statement of Problem: The Baltic Denture System provides a digital way to fabricate complete dentures in 2 visits. Conventional dentures using injection or compression molding require additional visits and complex laboratory procedures. However, how the fabrication method affects clinical outcomes is unclear., Purpose: The purpose of this clinical, randomized, controlled, double-blinded crossover trial was to evaluate the impact of the fabrication method (digital versus conventional production) of complete dentures on clinical outcomes., Material and Methods: Sixteen participants received 2 pairs of new complete dentures, produced in a digital and a conventional workflow. Each complete denture was worn for an observation period of 3 months. The order of the dentures was randomized. The primary outcome was the clinical assessment of the dentures by a blinded examiner, including peripheral extension, cutout for buccal and labial frenula, denture extension, and denture thickness. Denture esthetics were evaluated by the midline, position of anterior teeth, buccal corridor, and smile arc, and occlusal relationships were evaluated by the vertical dimension, sagittal relation, the Camper plane, and occlusion. In addition, the retention of maxillary and mandibular dentures and phonetics was evaluated. Differences between the prostheses were statistically analyzed with the McNemar test (α=.05)., Results: The borders of the digital dentures were significantly more often overextended at the time of insertion (P=.021), reducing the retention of the digital dentures, especially the maxillary dentures (P=.016). The borders of the dentures could be corrected so that after 2 weeks and 3 months, no significant differences could be seen between digital dentures and conventional dentures., Conclusions: The fabrication method has a significant influence only on the dimension of the denture border. It was significantly more often overextended in digital dentures and impaired retention, especially of the maxillary dentures, at the time of insertion. Statement of Problem: Biofunctional Prosthetic System (BPS) complete dentures have been reported to maintain a harmonious relationship with the surrounding oral structures and provide improved adaptation compared with conventional complete dentures. However, an evaluation of salivary changes and its constituents between BPS and conventional dentures is lacking., Purpose: The purpose of this parallel randomized clinical trial was to evaluate the alpha amylase, cortisol, and pH levels in saliva in participants provided with conventional and BPS complete dentures., Material and Methods: Sixteen completely edentulous participants were recruited for providing with conventional and BPS dentures (n=8). The levels of alpha amylase, cortisol, and pH in their saliva were evaluated by using an enzyme-linked immunosorbent assay (ELISA), colorimetric, and pH meter before insertion and 1 week and 1 month after denture insertion. The data were analyzed with repeated measure ANOVA and the Student t test (α=.05)., Results: The mean ±standard deviation for pH was 6.94 ±0.22 in conventional complete dentures and 7.04 ±0.12 in BPS dentures; for alpha amylase, it was 206.75 ±14.64 and 210.88 ±15.72 IU/mL; and for cortisol, it was 0.29 ±0.05 and 31 ±0.04 nmol/L before insertion and 1 week and 1 month after denture insertion. From the repeated measures ANOVA, the mean pH and salivary cortisol were not statistically different (P>.05), but salivary amylase was statistically different (P<.05). Objective: Opiorphin is a pentapeptide secreted in saliva and has a strong analgesic effect. Salivary opiorphin has been shown to increase in orofacial pain and may act as a pain reliever in pain caused by denture-related ulcers. The current study aimed to evaluate the salivary opiorphin levels in traumatic ulcers caused by ill-fitting dentures and demonstrate whether there is any correlation between trauma-related pain levels and salivary opiorphin levels., Materials and Methods: Twenty-two individuals with new full dentures and a complaint of pain due to ill-fitting were included in this study. Patients were asked to rate their level of pain on a visual analog scale (VAS). Then, saliva specimens were collected at the first visit with the complaint of pain and 7 days following the denture adjustment., Results: The average saliva opiorphin level before and after denture adjustment were 19.29 ± 5.44 and 15.78 ± 3.95 ng/mL, respectively. A dependent (paired) t-test determined that the mean salivary opiorphin level differed statistically significantly before and after the adjustment of the dentures., Conclusions: The findings show that salivary opiorphin levels increase in pain associated with denture-related traumatic ulcers. Aim: A total occlusal convergence (TOC) angle (ϕ) of 6 degrees is a typical crown preparation requirement. It has been shown that this is difficult to achieve clinically. The present study therefore aims to compare the ability of students to judge different steepnesses, including a -1-degree undercut of prepared canines and molars, under clinical conditions with different analog tools., Materials and Methods: The complete dentures of a patient were duplicated without denture teeth 16, 23, 33, and 46. For each of these gaps, six crown stumps were milled with ϕ/2 = -1, 3, 6, 9, 12, and 15 degrees, each of which was insertable using mini-magnets. A total of 144 dental students, 48 students each from the 1st, 6th, or 9th semester, estimated these angles intraorally with various aiding tools: In addition to basic dental instruments, they used a parallelometer mirror, an analog clock dial with a 6-degree visualization, and a scale of tooth stumps with ϕ/2 between -1 and 15 degrees., Results: The widely demanded 3 degrees were hardly recognized, but assumed to be steeper or even undercut. In contrast, the -1-degree divergent stump walls were predominantly estimated as parallel-walled or slightly conical. With increasing taper, the stumps tended to be classified as steeper, ie, "better." The additional tools did not result in a general improvement of the estimation performance. Students from higher semesters did not achieve better results., Conclusion: The present authors question the objectivity of an exclusively visual assessment of crown stump taper. It appears that dental training should at least focus on avoiding undercuts as the minimal prerequisite for an accurate intraoral scanning procedure. Digital control of the preparation angle by an intraoral scan and immediate clinical implementation of these results could help to produce appropriate preparations.

Objective: To describe the clinical procedures of complete denture set fabrication in three appointments., Background: Simplified approaches have proven not to be inferior to conventional ones in terms of quality, patient satisfaction and masticatory ability., Materials and Methods: The patient was a 77-year-old edentulous adult with mobility impairment seeking dental rehabilitation in a small number of appointments due to commuting difficulties., Results: A set of complete dentures was delivered within three appointments. The second appointment was dedicated to set-up trial due to the patient's aesthetic demands., Conclusion: Under certain circumstances and after a thorough study of each case, dental clinicians may propose the fabrication of complete dentures in a three-appointment protocol incorporating a set-up trial session., (© 2023 The Authors. Patient satisfaction and quality of life are integral to assessing oral health care quality. For many Americans still using conventional complete dentures (CDs) or implant-retained mandibular overdentures (IODs), it remains essential to consider improving their oral health outcomes and quality of life. Due to inexperienced student dentists providing dental care to dental school patients, patient grievances are generally considered a problem. Patient feedback and satisfaction have proven valuable resources for monitoring and improving patient safety. While CDs and IODs are the 2 leading treatment options for edentulism, more comparative studies in the literature need to compare their outcomes in a school setting. The research question that guided this comparative analysis was, "Is patient satisfaction and quality of life affected by the type of prostheses and provider?" A validated questionnaire was mailed to 520 individuals selected from records of patients who had received treatment for edentulous mandible at a student prosthodontic clinic at the University of Kentucky College of Dentistry from 2014 to 2016 with at least 1 year of follow-up time. A validated questionnaire for edentulous patients based on the Oral Health Impact Profile (OHIP-19) was used. In addition, information on patients' oral health-related quality of life, including questions related to the edentulous patients' satisfaction with their dentures, was collected. The response rate was 33% (N = 171). The study's findings confirm previous findings, suggesting that IODs may significantly impact oral health-related quality of life. Data show that 76% of the IOD group reported improvement in experience when using the implants to retain the mandibular denture. However, there were no statistically significant differences in the OHIP scores between overall CD and IOD patient groups. Males with IODs had lower physical pain, limitations, and disability scores than males with CD. However, females with IODs reported more significant concerns associated with a social disability and handicap domains. Comparing users who had experiences with both treatment options, this study discerned essential characteristics that contribute to increased patient satisfaction with IODs and identified significance in outcomes by gender. These findings guide prosthodontic practitioners' patient care practices and identify a continuing need to discuss CD and IOD treatment protocols within dental school curricula.

Objectives and Background: To validate a novel screening test for cognitive and functional decline in older patients rehabilitated with complete removable dental prostheses (CRDPs)., Materials and Methods: Edentate old in-patients rehabilitated with CRDPs were included in this study. Participants were requested to remove their prostheses before their intraoral examinations. The prostheses were then presented in an inverted orientation. Participants had to correct the orientation of the prostheses and insert them in the appropriate jaws. The test was repeated after the intraoral exam. Appropriate statistical models were used (⍺ = .05) to associate the test results with the participants' mini-mental state examination (MMSE) score, functional independence measure (FIM), age and sex., Results: Among the 86 participants (mean-age: 85.4 ± 6.4 years; mean MMSE: 19.8 ± 5.5; mean FIM: 77.9 ± 20.8), 21 (24.4%) failed to correctly insert the prosthesis. The prosthesis presentation test (PPT) was associated with the FIM but not the MMSE. Regression models further confirmed an association with age (P = .043), but not sex. Additional analyses revealed the PPT test is associated with the FIM's cognitive sub-sets of memory, problem solving and social interaction., Conclusion: The PPT is a novel, simple and quick screening tool that can help detect functional difficulties in older people. It can easily be performed during an oral examination. Future studies are needed to determine whether the PPT can be used to detect deficits in executive function, as a complement to the MMSE and also as a first assessment of a patient's ability to manage dentures independently., (© 2023 The Authors. Background and Objective: Complete denture is a common restorative treatment in dental patients and the design of the core components (major connector and retentive mesh) of complete denture metal base (CDMB) is the basis of successful restoration. However, the automated design process of CDMB has become a challenging task primarily due to the complexity of manual interaction, low personalization, and low design accuracy., Methods: To solve the existing problems, we develop a computer-aided Segmentation Network-driven CDMB design framework, called CDMB-SegNet, to automatically generate personalized digital design boundaries for complete dentures of edentulous patients. Specifically, CDMB-SegNet consists of a novel upright-orientation adjustment module (UO-AM), a dental feature-driven segmentation network, and a specific boundary-optimization design module (BO-DM). UO-AM automatically identifies key points for locating spatial attitude of the three-dimensional dental model with arbitrary posture, while BO-DM can result in smoother and more personalized designs for complete denture. In addition, to achieve efficient and accurate feature extraction and segmentation of 3D edentulous models with irregular gingival tissues, the light-weight backbone network is also incorporated into CDMB-SegNet., Results: Experimental results on a large clinical dataset showed that CDMB-SegNet can achieve superior performance over the state-of-the-art methods. Quantitative evaluation (major connector/retentive mesh) showed improved Accuracy (98.54 ± 0.58 %/97.73 ± 0.92 %) and IoU (87.42 ± 5.48 %/70.42 ± 7.95 %), and reduced Maximum Symmetric Surface Distance (4.54 ± 2.06 mm/4.62 ± 1.68 mm), Average Symmetric Surface Distance (1.45 ± 0.63mm/1.28 ± 0.54 mm), Roughness Rate (6.17 ± 1.40 %/6.80 ± 1.23 %) and Vertices Number (23.22 ± 1.85/43.15 ± 2.72). Moreover, CDMB-SegNet shortened the overall design time to around 4 min, which is one tenth of the comparison methods., Conclusions: CDMB-SegNet is the first intelligent neural network for automatic CDMB design driven by oral big data and dental features. Purpose: Older adults with removable dentures experience high rates of both caries (when the natural dentition remains) and salivary gland dysfunction. While many commercial dental therapeutic agents target these two interrelated problems, none have been labeled for use on the fitting surface of removable dentures. Off-label use of MI Paste (GC America, Alsip, IL), a dental therapeutic containing casein-phosphopeptide-amorphous-calcium-phosphate (CPP-ACP), was investigated following its application to the fitting surface of complete denture(s) with subsequent effects on salivary conditions measured., Materials and Methods: Salivary flow rate and pH were recorded at baseline and 15 min following the application of 1 mL of CPP-ACP paste to the fitting surface of each participant's denture through whole saliva collection. To assess buffering capacity, equivalent volumes of 0.01 M lactic acid were added to the collected saliva samples, and pH reduction was measured. Comparisons of salivary parameters between baseline and post-CPP-ACP paste application and between subjects with and without self-reported xerostomia were conducted using a paired-sample t-test, Wilcoxon signed-rank test, or two-sample t-test as appropriate., Results: Of the 28 participants (mean age = 70.3 ± 13.7 years, 17 males), 11 reported xerostomia. CPP-ACP-paste application was associated with decreased pH reduction during acid challenge compared to baseline (0.95 ± 0.24 vs. 1.54 ± 0.53, p < 0.001), and a higher final pH following acid challenge (5.93 ± 0.34 vs. 5.40 ± 0.66, p < 0.001). While the flow rates observed at post-CPP-ACP paste application were greater than those at baseline, the difference was not statistically significant (0.67 ± 0.44 mL/min vs. 0.55 ± 0.34 mL/min, p = 0.053). No significant differences were found in any salivary parameters between participants with or without self-reports of xerostomia., Conclusions: The findings highlight potential positive effects on salivary conditions following the application of the CPP-ACP-containing product, MI Paste, to the fitting surface of a removable complete denture as a potential caries-risk-management tool when natural dentition remains. ABSTRACT Objective: To analyze the self-perception that older adults have about rehabilitation regarding their uni or bimaxillary dentures. Methods: A form was applied to investigate the sociodemographic, economic and medical-dental variables of the users, a questionnaire to assess the satisfaction level of patients with their dentures, another to investigate the technical quality of the dentures from the aesthetic-functional point of view, and the Oral Health Impact Profile (OHIP-14) to ascertain the impact of prosthetic rehabilitation on the quality of life of users. Results: The majority of the participants (65.2%) used complete bimaxillary dentures, reported dissatisfaction during chewing, while the lower dentures had unsatisfactory technical quality for the retention requirements (69.6%) and stability (69.6%). The older adults’ complaints were predominantly related to the “physical pain” and “physical disability” domains. There were significant associations between general satisfaction with the dentures in use, their general technical quality and the OHIP-14 domains. Conclusion: The patient complaints were related to difficulties in their ability to chew, speak and with the retention and comfort of the lower dentures. The technical quality of the dentures had compromised retention and stability from the aesthetic-functional point of view. Dentures considered unsatisfactory caused negative impacts on the quality of life of users, especially in comfort during meals which was related to difficulty chewing and swallowing. The association between the instruments used in the study provides important tools for the performance of dental surgeons, in addition to enabling more adequate planning in the oral health services offered to the population.

ABSTRACT The aim of this study was to describe a clinical protocol for fabrication of maxillary and mandibular dentures using CAD/CAM technology. Further, digital and conventional dentures fabricated for the same patient were compared. The comparison was based on patient’s satisfaction, comfort, retention, number of follow-up sessions, masticatory performance and quality of life. No laboratorial step was needed for CAD/CAM fabrication and less sessions were required up to denture insertion. Compared to the conventional dentures, digital dentures provided better satisfaction, comfort and retention and required less follow-up sessions. Both dentures presented similar results about masticatory performance and quality of life. As a conclusion, digital denture is a viable treatment alternative for edentulous patients. However, controlled and randomized clinical trials are required to reveal the real benefits of this treatment alternative compared to the conventional denture.
