Root canal irrigants are difficult to diffuse deep into the dentinal tubules for root canal disinfection. The purpose of this study was to assess the ability of different diffusing enhancers to deliver chlorhexidine (CHX) into dentinal tubules. The diffusing property of five diffusing enhancers (acetone, DMSO, Triton X-100, JFC-E and azone) into dentinal tubules was firstly assessed using the Rhodamine B. The ability of the diffusing enhancers to deliver CHX into dentinal tubules was then evaluated both qualitatively and quantitatively. A deep dentinal tubule Enterococcus faecalis infection model was further established to test the bactericidal effect of CHX delivered by different diffusing enhancers. Finally, the in vitro cytotoxicity of these diffusing enhancers was tested using the CCK-8 method. Azone, Triton X-100 and JFC-E exhibited the largest maximum diffusing depths at all root canal parts (P < 0.05). Azone group also showed the highest percentage of diffusing depths for all root canal parts, followed by Triton X-100 (P < 0.05). The percentage of dead bacteria in dentinal tubules close to cementum layer was significantly higher in the CHX + azone group than in the other groups (P < 0.05), followed by CHX + Triton X-100 and CHX + JFC-E groups. The concentration of CHX diffusing onto the exterior surface of root was significantly higher in CHX + azone group than in the other groups (P < 0.05). All the diffusing enhancers showed relatively low cytotoxicity. Azone showed the highest diffusing ability with low cytotoxicity and might be employed as a carrier agent for either intracanal irrigants or medications to achieve more thorough root canal disinfection. Aim: To investigate the dynamic recovery of biofilms within dentinal tubules after primary irrigation with different protocols, and to evaluate the efficacy of various re‐irrigation protocols on recovered biofilm, considering factors such as smear layer, nutrient conditions, and primary irrigants. Methodology: A total of 416 mono or multi‐species biofilms samples were prepared from human teeth and incubated for 3 weeks. After inducing a smear layer on half of the samples, all specimens were irrigated with one of the following irrigant sequences: (1) 6% NaOCl +17% EDTA, (2) 6% NaOCl +8.5% EDTA, (3) 6% NaOCl and (8.5% EDTA +10 μg/mL DJK‐5 antimicrobial peptide), or (4) sterile water. Thirty‐two samples were used to assess immediate effect, whilst the rest were re‐incubated to assess biofilms recovery. Nutrient conditions were defined based on whether culture media were changed (nutrient‐rich) or not (nutrient‐poor) during re‐incubation. After 16 weeks, recovered biofilms underwent re‐irrigation using four additional protocols, with or without DJK‐5 peptide, based on primary irrigants. Confocal laser scanning microscopy was employed to evaluate immediate irrigant effects, biofilms recovery intervals (1, 3, 5, 8, 12, and 16 weeks after primary irrigation), and re‐irrigation effects at the 16‐week. Statistical analysis included one‐way anova and two‐way mixed anova tests. Results: The DJK‐5 peptide irrigation protocols demonstrated the highest killing rates during primary irrigation and resulted in a longer biofilms recovery time of 16 weeks compared to non‐peptide protocols (p <.001). Both primary irrigation type and smear layer presence significantly influenced biofilms recovery (p <.001). In the absence of smear layer, re‐irrigation efficacy didn't significantly differ from primary irrigation, regardless of primary irrigation type or nutrient conditions. However, with a smear layer present, re‐irrigation led to significantly higher proportion of dead bacteria compared to primary irrigation (p <.05). Inclusion of the DJK‐5 peptide into the re‐irrigation protocol displayed superior killing rate compared to other protocols (p <.001). Conclusions: Biofilms exhibited susceptibility to both peptide and non‐peptide protocols during re‐irrigation, irrespective of nutrient conditions or primary irrigation protocols. Aim: The aim of this study is to investigate the use of 15% carbamide peroxide in combination with fluoro calcium phosphosilicate desensitizing dentifrice during bleaching to evaluate postoperative dentinal tubule occlusion. Materials and Methods: Premolars which were extracted and bleached using 15% carbamide peroxide with fluoro calcium phosphosilicate dentifrice were used in this investigation, and the shade change was assessed. They were subsequently divided into sections and examined with scanning electron microscope. The quantity of open tubules, fully obstructed as well as moderately obstructed tubules, was recorded for each specimen. The statistical analysis of shade changes was conducted using the paired t-test. The tubule numbers were statistically evaluated after being assigned a mean and standard deviation. Results: The mean values between test and control groups in completely blocked tubules are 81.370 ± 1.989 and 42.586 ± 11.316, respectively. In partially blocked tubules, the mean values were found to be 13.219 ± 1.747 and 32.264 ± 11.554, respectively. In open tubules, the mean values are 5.398 ± 1.260 and 25.149 ± 8.781. The mean values of shade comparison for test group and control group are 1.95 ± 2.78 and 3.40 ± 2.83, respectively. Conclusions: According to the findings of the study, dentinal tubules were blocked when fluoro calcium phosphosilicate dentifrice was administered with 15% carbamide peroxide, but the bleaching process was unaffected. The biology of dentin occupies a central place in the theory of tooth integrity as an organ, and knowledge about the microstructure of dentin changes with the development of new research methods, such as scanning electron microscopy. The structures that form dentin, and the odontoblast processes, are an extremely complex biological structure due to cellular polarization. The structure of the dentin of crowns from 30 intact teeth extracted for orthodontic reasons has been studied. Dentin was examined on longitudinal chips using scanning electron microscopy, determining the average number of dentinal tubules per unit area of dentin, the length and diameter of the dentinal canaliculi along the dentinal tubule. Scanning electron microscopy of demineralized teeth showed that numerous fine fibrillar structures were found in the predentin, connecting the odontoblast processes (dentinal canaliculi) and the walls of the dentinal tubules. The odontoblast processes were always in close contact with the microfibrillar network and were located within the dentinal tubule. It was found that the dentinal canaliculi have their own anchoring microfibrillar system, which allows each dentinal canaliculus to be held in a central position. The microfibrils are attached to the wall of the dentinal tubule and form a continuous mesh structure among the dentin tissue and are directly fixed on one side to the wall of the dentinal tubule, and on the other side to the wall of the dentinal canaliculus, which is a derivative of the odontoblast process. In different areas of the dentin, the length, number, and diameter of the microfibrils have a wide variative range. Microstructures were numerous in different regions of the dentinal tubule. Their number decreased with distance from the level of the outer dentin. In the inner third of the dentinal tubule, microfibrillar structures are observed that form a dense network of different types, thicknesses and diameters. The most common direction of the microfibrils was from the surface of the odontoblast process to the wall of the dentinal tubule, and in other cases these microfibrils were attached to both opposite inner surfaces of the dentinal tubule. The base of the microfibril is attached to the wall of the dentinal tubule, as if forming a continuous structure among the dentin tissue and the surface of the odontoblast process. It is shown that the microfibrillar structures may be a previously unknown framework system that ensures the stabilization of odontoblast processes inside the dentinal tubule. In the scientific classification of dentin ultrastructure, it is advisable to use the term "dentinal tubule" and "dentinal canaliculus", since this structure is a derivative of the odontoblast process.

BACKGROUND: Nano hydroxyapatite is an excellent bioactive material, which has a certain protective effect on demineralized enamel and dentin, but the effect and mechanism of its solution on remineralization of demineralized dentin are not clear. OBJECTIVE: To explore the feasibility of nano hydroxyapatite in the prevention and treatment of dentin sensitivity and dentin caries. METHODS: Abandoned human bicuspid specimens were collected, and 72 dentin slice samples were prepared from isolated bicuspids. The samples were randomly divided into 6 groups, with 12 samples per group: After demineralization, each group was immersed in 10% nano hydroxyapatite solution (group A), 6% bioactive glass solution (group B), 10% nano hydroxyapatite and 6% bioactive glass composite solution (group C), 0.05% sodium fluoride (group D), artificial saliva (group E), and deionized water (group F). After 14 days of treatment, working surfaces of samples were observed by scanning electron microscope. The surface composition was detected with energy spectrum analyzer. Dentin Rhodamine B staining was detected under a confocal laser microscope. RESULTS AND CONCLUSION: (1) Scanning electron microscopy: The dentinal tubules orifice in group A was completely sealed, and the surface of the dentin was uniformly covered by a layer of tightly connected granular material. The dentinal tubules in group C were partially sealed. A small amount of scattered particles were found on the dentin surface and in the dentinal tubules of group B, group D and group E, and the dentinal tubules were open. In group F, there was no particulate matter deposition on the dentin surface or in the dentinal tubules, and the dentinal tubules were open. (2) X-ray energy dispersive spectrocopy results: The Ca/P ratio of group A was 1.61, which was closest to the theoretical value of healthy dentin; the Ca/P ratio was higher in group A than that in the groups B, E, and F (P < 0.05). There was no significant difference in Ca/P ratio between group A and group C (P > 0.05). (3) Laser confocal microscopy test results showed that the fluorescence intensity and fluorescence area of group A were the smallest, and the mean fluorescence intensity was smaller in group A than that in other five groups (P < 0.05). The mean fluorescence intensity of group C was lower than that of groups B, D, E and F (P < 0.05). (4) It is indicated that nano hydroxyapatite has a good remineralization effect on demineralized dentin. The combination of nano hydroxyapatite and bioactive glass can also deposit on the dentin surface, but there is no synergistic effect. Dentinal hypersensitivity (DH) is a common clinical condition usually associated with exposed dentinal surfaces. The aim of this study was to study the effect of sodium fluoride varnish, Gluma, and Er,Cr:YSGG laser, in the dentin hypersensitivity treatment. One hundred sixty-five teeth with dentin hypersensitivity in 55 patients were involved in this study. Teeth are divided into five groups based on the received treatment (n = 33): G group: Gluma; F group: sodium fluoride varnish (5%); L group: Er,Cr:YSGG laser (wavelength 2780 nm, frequency 20 Hz, power 0.25 W, energy density 44.3 J/cm2, and pulse width of 150 µs at distance of 1 mm for 30 s) which was followed by Er,Cr:YSGG laser; GL group: Gluma + laser; VL group: both sodium fluoride varnish and Gluma, which are common treatments for hypersensitivity, were selected as control groups. The treatment was performed in one session, and the sensitivity to air spray conditioning was recorded after the treatment, at 15 min, 1 week, 1 month, and 6 months as the VAS. Statistical analysis was performed using SPSS Ver. 21 software. One-way ANOVA was used to compare the VAS between all treatment groups at each time-point. One-way repeated measurements ANOVA (RM-ANOVA) and two-way-repeated measurements ANOVA (RM-ANOVA) were used to compare the hypersensitivity of each group and sensitivity of all treatment groups, respectively. Tukey post hoc test was used to compare the groups pairwise. The hypersensitivity between different groups at before and 15 min after the treatment was not significantly different (P = 0.063). The hypersensitivity of all studied groups was decreased after the treatment. The Er,Cr:YSGG laser, alone or in combination with Gluma, in 1 week, 1 month, and 6 month follow-ups, had significantly reduced the hypersensitivity instead of sodium fluoride varnish. All treatments significantly reduced the dentin hypersensitivity up to 6 months. Er,Cr:YSGG laser alone or in combination with Gluma was more effective than sodium fluoride varnish; however, it was not significantly different from other treatments. The aim of this study was to synthesize methylene blue‐incorporated quartz particles (MB@QP) and to investigate its anti‐bactericidal properties. Methylene blue was incorporated inside QP and characterized for morphology and chemical structure using scanning electron microscope (SEM) and Fourier transformed infrared spectroscopy (FTIR). Specimens were randomly divided into experimental and control groups (n = 9/groups). The dentin specimen infected with Enterococcus faecalis was treated using different treatment modalities: Control groups: treatment with 5.25% of sodium hypochlorite (NaOCl) for 60 s; MB: treatment with 1 ml MB solution and incubated for 60 s; MB–PDT (photodynamic therapy): treatment with 1 ml MB solution followed by irradiation using diode laser for 60 s; MB‐QP‐PDT: specimens treated with MB@QP and irradiated by the diode laser for 60s, and Er,Cr:YSGG laser alone. MB@PDT therapy showed the highest efficacy in reducing the survival rate of E. faecalis (0.49%) in comparison to control NaOCl (0.78%) and Er,Cr:YSGG laser treatment (2.17%). Encapsulating MB into QP followed by the PDT significantly improved the bactericidal capacity and significantly reduced the bacterial survival rate to 0.11% (p <.05) compared to other groups. The combination of MB incorporated into QP and PDT could be an alternative treatment modality to conventional disinfection method for eliminating bacteria from the tooth dentin. Background: The present study aims to assess area of open dentinal tubules by use of diode laser and electrode alone and in combination with hydroxyapatite powder and strontium chloride powder by using the scanning electron microscope (SEM). Materials and Methods: A double-blinded study was carried out with 30 extracted teeth with no carious lesion or restored teeth were selected and grouped randomly as Group A - diode laser, Group B - electrode application, Group C - hydroxyapatite powder plus diode laser, Group D - hydroxyapatite powder plus electrode application, Group E - strontium chloride powder plus diode laser, and Group F - strontium chloride powder plus electrode application. All the samples were made by preparing a notch of 2 mm measured with UNC-15 probe at cementoenamel junction using a diamond cylindrical bur at high speed. The teeth were then washed with distilled water and stored in solution containing 2.5% glutaraldehyde solution in 0.1 M sodium phosphate buffer for 24 h. The samples were treated as mentioned in above said groups and were viewed under SEM the degree of occlusion of the dentinal tubules were quantified using an image analyzer. Results: Highest number of open dentinal tubules was found with Group B, that is, electrode application, whereas Group C, that is, hydroxyapatite powder plus diode laser showed highest occluded tubules. One-way ANOVA showed statistically significant difference across all the groups (P = 0.000); further, intergroup comparison with Tukey's test showed better tubular occlusion with Group C. Conclusion: The SEM results showed highest tubular occlusion with hydroxyapatite powder plus diode laser and least with electrode.

Aim: This in vitro study aims to assess and compare the effectiveness of different irrigation activation techniques in removing the smear layer from the root canal dentin using Scanning electron microscope (SEM) analysis., Materials and Methods: A total of 60 extracted single-rooted premolar with straight canal and mature apex were used for this study. After the selection of teeth, all the samples were decoronated followed by biomechanical preparation. The sample after preparation was irrigated with sodium hypochlorite and randomly divided into three groups with 20 sample in each group ( n = 20), (Group 1) control, (Group 2) ultrasonic, and (Group 3) laser. The purpose of this study was to investigate whether tooth-brushing with a microcurrent was effective in inducing dentinal tubule occlusion. The specific aims of the study were (1) to evaluate the effectiveness of tooth-brushing with a microcurrent on dentinal tubule occlusion by using scanning electron microscopy (SEM); and (2) to compare the dentinal fluid flow rate after tooth-brushing with a microcurrent by using a sub-nanoliter-scaled fluid flow measuring device (NFMD). All experimental groups showed partially occluded dentinal tubules and crystal-like structures at a specific microcurrent intensity indicated that tooth-brushing with a microcurrent could efficiently occlude dentinal tubules. The decrease in dentinal fluid flow rate in the tooth-brushing with microcurrents group indicated that dentinal tubules were occluded and the flow of dentinal fluid had decreased. Purpose: Dentin hypersensivity (DH) is one of the most common complications occurring after periodontal treatment. Various methods such as Nd:YAG and Er:YAG have been used to treat DH. Previous studies support the effectiveness of these lasers, but their effect on the tooth surface largely depends on certain parameters of the tooth surface. This study attempts to evaluate some changes of the dentin surface after Nd:YAG and Er:YAG laser irradiation by using SEM. Materials and Methods: Fifteen freshly extracted mandibular molars were selected, and 4 specimens with known dimensions (1 × 2 × 2 mm) from the buccal surface and below CEJ of all the teeth were prepared to obtain a total of 60 specimens. These specimens were then divided into 4 groups. Group 1 (control) was not subjected to laser irradiation. Group 2 was subjected to Nd:YAG laser irradiation (0.5 W, 50 mJ, 10 Hz, 60 s). Group 3 was irradiated by Nd:YAG laser (1 W, 100 mJ, 10 Hz, 60 s). Group 4 was irradiated by Er:YAG laser (0.3 W, 100 mJ, 3 Hz, 60 s). After preparation and gold coating of specimens, they were observed under SEM. Finally, the number and diameter of dentinal tubules, craters, and microcracks were determined in different groups. Finally, the data were analyzed using ANOVA. Results: The number of open tubules showed significant differences between the Nd:YAG (1 W) group and other groups (p < 0.05), but no significant differences were observed between the Nd:YAG (0.5 W), Er:YAG (0.3 W), and control groups. Tubule diameter showed significant differences across all groups (p < 0.001), except between the Nd:YAG (1 W) and Er:YAG groups. Craters and carbonization were only observed in some samples of the Er:YAG group. No microcracks were detected in the study groups. Conclusion: The results of this study show that Nd:YAG and Er:YAG laser irradiation can cause thermal effects such as decreased dentinal tubule diameter or occlusion. Considerable knowledge has accumulated over the years on the structure and function of the denial pulp and dentin. Some of this knowledge has important clinical implications. This review, which is the first of seven articles, will be limited lo those parts of the normal structure and physiology of the pulp and dentin that have been shown lo result in, or are likely lead to, tissue reactions associated with the clinical treatment of these tissues. Although certain normal structures will be highlighted in some detail, a basic knowledge of pulpal and dentinal development and structure is a prerequisite for an understanding of this text. CaCO3@mesoporous silica reacted with phosphoric acid (denoted as CCMS-HP) enables the growth of calcium phosphate crystals in dentinal tubules. This study tested whether CCMS-HP could be used to form a biomimetic barrier on the exposed dentin for prevention of dentin sensitivity without impairing the bonding of Single Bond Universal (SBU) self-etch adhesive to the dentin. Methods: Twenty-four dentin disks were prepared and divided into three groups: (1) SBU group (n = 8), in which SBU self-etch adhesive was bonded to the dentin disk directly; (2) CCMS-HP group (n = 8), in which CCMS-HP was applied onto the dentin surface; and (3) CCMS-HP/SBU group (n = 8), in which the dentin surface was first treated with CCMS-HP and then boned by SBU. The permeation depth of crystals into the dentinal tubules was examined and measured with a scanning electron microscope. The shear bonding strength of SBU and CCMS-HP/SBU to dentin was also measured. Results: The mean crystal permeation depth was 35.8 ± 6.9 μm for the CCMS-HP/SBU group and 33.6 ± 12.2 μm for the CCMS-HP group; no significant difference was found between the two groups. Moreover, the mean shear bonding strength was 22.7 ± 6.7 MPa for the CCMS-HP/SBU group and 23.3 ± 7.0 MPa for the SBU group. There was also no significant difference between the two groups. Conclusion: CCMS-HP can be used to form a biomimetic barrier for prevention of dentin sensitivity because it neither impedes the bonding of SBU to dentin nor impairs the shear bonding strength between the SBU and dentin.

Dentin hypersensitivity is one of the most common clinical conditions usually associated with exposed dentinal tubules. The purpose of this study was to identify the potential of a graphene oxide quantum dot coating for mesoporous bioactive glass nanoparticles as a new material for the treatment of dentin hypersensitivity by investigating its mineralization activity and dentinal tubules sealing. Mesoporous bioactive glass nanoparticle was fabricated by modified sol-gel synthesis. X-ray diffraction was performed to characterize the synthesized nanoparticle Fourier transform infra-red spectroscopy investigated the functionalized surfaces. The distribution of the specific surface area and the pore size was measure by Pore size analysis. The morphology of sample was observed by Field Emission Scanning Electron Microscope (FESEM) and Field Emission Transmission Electron Microscope (FETEM). After disk-shaped specimens of mesoporous bioactive glass nanoparticles and graphene oxide quantum dot coated mesoporous bioactive glass nanoparticles (n = 3) were soaked in the simulated body fluid for 0, 1, 5, 10,and 30 days, the amount of ions released was observed to confirm the ionic elution for mineralization. Sensitive tooth model discs (n = 20) were applied with two samples and evaluated the dentinal tubule sealing ability. The spherical mesoporous bioactive glass nanoparticles and graphene oxide quantum dot coated mesoporous bioactive glass nanoparticles with a diameter of about 500 nm were identified through FESEM and FETEM. The ion release capacity of both samples appeared to be very similar. The amount of ion released and in vitro mineralization tests confirmed that graphene oxide quantum dot coating of mesoporous bioactive glass nanoparticles did not inhibit the release of calcium, silicon and phosphate ions, but rather that graphene oxide quantum dot promoted hydroxyapatite formation. In the FESEM image of the sensitive tooth disc surface, it was observed that graphene oxide quantum dot coated mesoporous bioactive glass nanoparticles sealed tightly the dentinal tubules. The graphene oxide quantum dot coating of mesoporous bioactive glass nanoparticles not only showed the excellent dentinal sealing ability but also rapidly promoted mineralization while minimizing the size increase by coating the mesoporous bioactive glass nanoparticles.
