Surface contaminants on customized implant abutments could trigger an inflammatory response in the peri-implant tissues. The aim of this randomized controlled study was to assess the radiographic bone changes around implants restored with customized, platform-switched abutments, with and without autoclave treatment, 12 months after definitive restoration. Dental implants were placed 1 mm subcrestally in 64 systemically healthy patients (mean age: 63.3 ± 10.0 years; 31 patients had a history of periodontitis) to replace single or multiple missing teeth. According to a randomization list, abutments were subjected to steam and autoclave sterilization (43 implants, test group) or steam cleaning alone (44 implants, control group). Periapical standardized radiographs were taken at implant placement, prosthetic abutment connection, and 12 months after definitive cement-retained restoration. All implants were clinically stable without any sign of infection at the 12-month follow-up. An average marginal bone loss of 0.25 ± 0.19 mm was found in the test group compared to 0.35 ± 0.23 mm in the control group (no statistically significant difference), while the percentage of bleeding sites was significantly higher in the control group (8.7% ± 13.1% vs 19.1% ± 19.8%; P = .035). Purpose: To evaluate the dimensional accuracy of implant impressions obtained using five different tray types and two techniques. Materials and Methods: A partially dentate maxillary Kennedy Class II model was created as a reference model through 3D printing. Then, implant analogs 4.3 mm in diameter were placed at the first premolar, first molar, and second molar sites. Five types of trays were used to create impressions: (1) metal stock trays, (2) plastic stock trays, (3) custom trays fabricated using liquid crystal display (LCD), (4) custom trays fabricated using fused deposition modeling (FDM), and (5) custom trays fabricated using urethane dimethacrylate (UDMA) resin. Open and closed tray techniques were also compared. In total, 150 impressions were obtained. The reference model and impressions were scanned using a laboratory scanner. Additionally, the positional and angular deviations of implants with different tray types and techniques were evaluated using the superimposition method. Results: There was no statistically significant difference (P > .05) between the impression accuracy with the different tray types and impression techniques. The angular deviations with plastic and UDMA trays were greater than those with metal, FDM, and LCD trays. Angular deviation at the second molar was greater when using closed plastic trays compared to open plastic trays. The highest and lowest positional deviations were observed at the first molar implant with an open plastic tray impression (mean ± SD of 62.46 ± 28.54 mm) and a closed LCD tray impression (36.59 ± 29.93 mm). The greatest angular deviation was observed with an open FDM tray impression at the first premolar implant (0.067 ± 0.024 degree), and the lowest angular deviation was observed with a closed metal stock tray impression at the second molar implant (0.039 ± 0.025 degree). Statistical differences were detected using the Mann-Whitney U test for paired groups and the Kruskal-Wallis test for groups with more than three comparisons (P > .05). Conclusions: Plastic and metal stock trays or conventional and 3D-printed custom trays can be used to obtain implant impressions for maxillary partially edentulous arches with similar dimensional accuracy. Purpose: To investigate how well an implant stability quotient (ISQ) represents resonance frequency. Materials and Methods: Benchtop experiments on standardized samples that replicated a mandibular premolar site were conducted to correlate an ISQ value and a resonance frequency to synthetic bone density and an incremental insertion torque; then, a frequency spectrum analysis was performed to check the validity of the resonance frequency analysis (RFA). Brånemark Mk III implants (4 × 11.5 mm; Nobel Biocare) were placed in Sawbones test models of five different densities (40, 30, 40/20, 20, and 15 PCF). An incremental insertion torque was recorded during implant placement. To perform stability measurements, the test models were partially clamped in a vise (unclamped volume: 10 × 20 × 34 mm). A MulTipeg (Integration Diagnostics) was attached to the implants, and a Penguin (Integration Diagnostics) RFA measured ISQ. Simultaneously, the MulTipeg motion was monitored via a laser Doppler vibrometer and processed by a spectrum analyzer to obtain the resonance frequency. Tightness of the clamp was adjusted to vary the resonance frequency. A statistical analysis produced a linear correlation coefficient (R) among the measured ISQ, resonance frequency, and incremental insertion torque. Results: The resonance frequency had high correlation to the incremental insertion torque (R = 0.978), confirming the validity of using RFA for this study. Measured ISQ data were scattered and had low correlation to the resonance frequency (R = 0.214) and the incremental insertion torque (R = 0.386). The spectrum analysis revealed the simultaneous presence of multiple resonance frequencies. Conclusions: For the designed benchtop tests, resonance frequency does indicate implant stability in view of Sawbones density and incremental insertion torque. However, ISQ measurements do not correlate to the resonance frequency and may not reflect the stability when multiple resonance frequencies are present simultaneously. [ABSTRACT FROM AUTHOR]

The article discusses studies which show that full-ceramic prosthetic restorations for single crowns (SCs) and fixed dental prostheses (FDPs) are high quality and have a satisfactory survival rate comparable to porcelain-fused-to-metal (PFM). These studies include the survival outcome and complications in a retrospective clinical study of complete-arch fixed implant-supported prostheses (CAFIPs), the prevalence of prosthetic complications, and clinical performance of implant-supported FDPs.

Purpose: To compare the effect of taper, length, angle, and number of vertical axial walls on the retentive strength of a cemented crown on a partially customized hybrid abutment in the esthetic zone. Materials and Methods: A total of 35 metal copings were used and divided into two groups. One group had 30 copings cemented to their corresponding 8-degree tapered abutment with lengths from 3 to 8 mm, increasing in 1-mm increments (5 copings per length). The other group had the remaining 5 copings and consisted of a standardized metal coping that matched a hexagonal abutment with 3-mm vertical axial walls and the sequential removal of 1, 2, and 3 contiguous vertical axial walls. Dislodgment tests were performed for all copings in both groups. Maximum retentive forces were measured in retentive strength (kgF), with a conversion factor of 9.807 N = 1 kgF. Results: At each tapered abutment length, the retentive strength increased proportionally and was significantly different, ranging from 31.67 ± 4.10 kgF to 67.68 ± 11.22 kgF (F [5,24] = 20.46, P < .001). An unmodified hexagonal abutment demonstrated the highest retentive strength (70.15 ± 12.97 kgF). The sequential removal of 1, 2, and 3 contiguous vertical axial walls of the hexagonal abutment resulted in retentive strength values of 59.89 ± 10.06 kgF, 57.01 ± 9.62 kgF, and 55.99 ± 9.35 kgF, respectively, with no significant difference (P > .05) in strength. Conclusions: A partially customized abutment with vertical axial walls on one side and a profile reduction on the opposite side can provide comparable retention to cemented copings at one-third the length and at one-sixth the surface area of an 8-mm abutment with an 8-degree taper. Purpose: To measure the surface temperature distribution after using a CO2 laser to heat titanium dental implants via different power settings, application intervals, and irradiation times. Materials and Methods: A total of 10 tissue-level titanium implants (Screw-Line Promote Plus, Camlog; 4.3 × 11 mm) were placed (EpoFix, Struers) and irradiated with a CO2 laser (Denta 2, Lutronic) with a wavelength of 10.6 µm at power levels of 4 watts (Group 1), 6 watts (Group 2), 8 watts (Group 3), and 10 watts (Group 4). A continuous beam mode (setting I) and noncontinuous beam modes with 5-second (setting II) and 10-second (setting III) pause intervals were used. For each setting, a total irradiation time of 50 seconds was used and repeated 10 times. The temperature was measured using external thermocouple (Testo) in contact with the implant surface at the implant shoulder, middle, and apex. A linear regression model was used to analyze the data (P = .05). Results: Setting I demonstrated the most rapid increase in implant surface temperature in all three test sites as well as the greatest total temperature at 50 seconds of irradiation time. The greater the pause interval (settings II and III) during the 50 seconds of irradiation, the lower the rate of temperature increase as well as the total temperature in all three test sites and with all power levels. The average temperature difference between the apex and shoulder site was significant for setting III for all groups, but not for any groups in settings I and II. Conclusions: Heating the internal aspect of an implant with a CO2 laser produces different temperature distribution profiles depending on the laser power level and the application interval. Purpose: To investigate the screw loosening and fracture resistance of different hybrid abutment crown restorations after thermomechanical aging. Materials and Methods: Restorations were produced from zirconia, lithium disilicate, and hybrid ceramics with a CAD/CAM system (n = 10). Restorations and titanium bases (Ti-bases) were cemented, and the abutment screws were torqued to 30 Ncm. They were retorqued after 10 minutes, and then the removal torque values (RTVs) were measured. After 250,000 and 500,000 cycles of thermomechanical aging were completed, the RTVs were measured again, and removal torque loss (RTL) ratios were calculated. The specimens were subjected to a fracture resistance test after 750,000 cycles of thermomechanical aging, and the load at failure was recorded as the fracture resistance (N). The results were statistically analyzed (a = .05). Results: Aging was effective on the RTVs and RTL ratios (P < .001). The highest RTV was observed before aging; however, the RTVs significantly decreased and RTL ratios significantly increased after aging (P < .05). The fracture resistance values significantly differed among the ceramic materials. All lithium disilicate and hybrid ceramic specimens showed restoration fracture and most of the zirconia specimens showed Ti-base-related failure. Conclusions: Hybrid abutment crowns made of zirconia, lithium disilicate, and hybrid ceramics can withstand the average occlusal forces in the molar region; however, the ceramic type may affect the failure type. Retightening of the abutment screws after 10 minutes following the first tightening and annually may be beneficial to prevent the adverse effects of screw loosening on the integrity of the implant-abutment connection. Background: Platelet-rich fibrin (PRF) is used to prepare "sticky bone" by combining it with bone graft material. The present study investigated the ability of different bone grafts to absorb growth factors from the PRF and release them over time. Materials and Methods: Human blood was collected from 10 healthy volunteers for liquid PRF preparation. Bovine bone, allograft (mineralized and demineralized), and synthetic bone were each mixed with the PRF to prepare a sticky bone. All sticky bone samples were incubated for up to 4 days. The absorption and release pattern kinetics of two selective growth factors within the PRF--platelet-derived growth factor (PDGF) and bone morphogenetic protein-2 (BMP-2)--were quantified with immunofluorescence staining and enzyme-linked immunoassay (ELISA) testing. Results: All bone graft materials adsorbed the examined growth factors from the PRF. ß-TCP showed the highest adsorption levels, followed by the xenograft, and the allografts showed the lowest adsorption levels. Furthermore, PDGF showed a fast-release pattern from the grafts, whereas BMP-2 was released at a later stage. Similar to the adsorption pattern, the ß-TCP and xenograft were better able to sustain the release of the PRF growth factors from the graft than the allografts. Conclusions: The adsorption of PDGF and BMP-2 differ between graft materials, with superior results for ß-TCP, followed by xenograft, then allograft materials. [ABSTRACT FROM AUTHOR]

Objectives: The objective was to evaluate the polymerization efficiency of different bulk-fill resin-based composites cured by monowave and polywave light-curing units, by assessment of the degree of conversion and Vickers microhardness at different depths. Method and materials: Two commercially available bulk-fill resin-based composites were used: Filtek One Bulk Fill Restorative (3M ESPE) and Tetric N-Ceram Bulk Fill (Ivoclar Vivadent). The light-curing units utilized were two LED light-curing units: a monowave LED light-curing unit (BlueLEX LD-105, Monitex) and a polywave LED light-curing unit (Twin Wave GT-2000, Monitex). For each test, 20 cylindrical specimens (4 mm diameter, 4 mm thickness) were prepared from each bulk-fill resin-based composite using a split Teflon mold. Ten specimens were light-cured by the monowave light-curing unit and the other ten were light-cured by the polywave light-curing unit according to the manufacturer's recommendations. Attenuated total reflectance-Fourier transform infrared spectroscopy (ATRFTIR) was used to assess the degree of conversion, and a Vickers microhardness tester was used to assess Vickers microhardness. Statistical analysis was performed using three-way ANOVA and Tukey post-hoc tests (P < .05). Results: The degree of conversion and Vickers microhardness in bulk-fill resin-based composites containing only camphorquinone as photoinitiator were similar when cured with either monowave or polywave light-curing units. However, bulk-fill resin-based composites containing a combination of photoinitiators exhibited significantly higher degree of conversion and Vickers microhardness when cured with a polywave light-curing unit. Although all groups showed statistically significant differences between the top and bottom surfaces regarding degree of conversion and Vickers microhardness, all of them showed bottom/top ratios > 80% regarding degree of conversion and Vickers microhardness. Conclusion: The polywave light-curing unit enhanced the polymerization efficiency of bulk-fill resin-based composites especially when the latter contained a combination of photoinitiators, but does not prevent the use of a monowave light-curing unit. This clinical case outlines a comprehensive digital workflow for a minimally invasive multidisciplinary treatment. The process utilizes one open-source software for digital wax-up and one low-cost software to address esthetic concerns related to teeth misalignment. The patient's function was stabilized with a digitally made occlusal splint. The application of the described digital workflow technique, incorporating open-source, lowcost, and closed software, played a pivotal role in attaining a straightforward and predictable outcome with minimally invasive treatment. Furthermore, the continual evolution of technology contributes to the growing precision of dental procedures. The presented digital workflow helped formulate a predictable treatment plan, replicate a diagnostic digital wax-up, and achieve precise teeth alignment. This study aimed to evaluate the periodontal responses of subgingival proximal margins elevated using different restorative materials. A total of 120 proximal cavities with dentin/cementum gingival margins were elevated using one of four materials (resin-modified glass ionomer, glass hybrid, flowable bulk-fill resin composite, or bioactive ionic resin) and completed with the same overlaying resin composite. At 2 weeks (baseline), 6 months, 1 year, and 2 years postrestoration, periodontal parameters and the radiographic distance between the restoration margin and bone crest were evaluated. Appropriate statistical analyses were performed. The values of all periodontal parameters increased with time, although only the increases in Plaque Index and probing depth within each material group were statistically significant. There were no statistically significant differences in any of the periodontal parameters between the different materials within the same evaluation period, nor between different time points or material groups in the radiographic distances. This canine in vivo study assessed the effect of recombinant human platelet-derived growth factor (rhPDGF) on the healing of periapical tissues following apical surgery. From a total of 96 premolar teeth, 64 teeth from six beagle dogs (2 years old) were classified as experimental and were randomly assigned to four experimental groups (16 teeth per group). After having the pulp extirpated, leaving teeth open to the oral cavity for 1 week, and sealing with an immediate restorative material for 8 weeks, nonsurgical endodontic treatment was performed. A split-mouth design was used, and intra-animal randomization of treatment sides was applied to the groups as follows: apical curettage + 1.5-mm root-end resection (Group 1); apicoectomy + mineral trioxide aggregate (MTA) root-end filling (Group 2); apicoectomy + MTA root-end filling + rhPDGF (Group 3); and apical curettage + rhPDGF (Group 4). The animals were sacrificed 24 months after apical surgery, and histologic and µCT analyses were performed for bone volume loss (BVL). Group 1 showed partial resolution of the periapical lesions without signs of tissue regeneration (BVL: 49.09 ± 10.97 mm3). Group 2 had minimal bone regeneration and showed cementum reformation in 9 teeth, with no direct attachment to the MTA (BVL: 35.34 ± 10.97 mm3). Group 3 showed regeneration of all damaged apical tissues without direct contact between the cementum and MTA (BLV: 4.51 ± 1.55 mm3). Group 4 showed regeneration of PDL, bone, and cementum and attachment of functional cementum fibers (BVL: 2.82 ± 2.3 mm3). Purpose: To analyze the effectiveness of coating of abutments with antimicrobial agents and their influence on the physicochemical and biologic properties of the coated materials. Materials and Methods: This work was registered in Open Science Framework (osf.io/6tkcp) and followed the PRISMA protocols. A search of two independent reviewers of articles published up to October 29, 2021, was performed in the Embase, PubMed, Science Direct, and Scopus databases. Results: The databases found a total of 1,474 references. After excluding the duplicates, 1,050 remained. After reading the titles and abstracts and applying the inclusion criteria, 13 articles remained and were read in full. A total of 8 articles were included in this systematic review. Different antimicrobial agents have been used to coat abutments, including graphene oxide, polydopamine, titanium and zirconium nitride, lactoferrin, tetracycline, silver, and doxycycline with varied release times. Titanium-coated silver showed a better antimicrobial agent release time of up to 28 days. Chemical analysis confirmed the presence of antimicrobials on the surface after coating. Different pathogenic microorganisms, such as Streptococcus sanguinis, Streptococcus oralis, and Staphylococcus aureus, were inhibited when in contact with the coated surface. Conclusions: This review showed that there is still no consensus on which is the better antimicrobial agent and which coated materials have the better performance. However, the association of surface coating of abutments with antimicrobials is feasible and can benefit many patients, which can support their clinical use to favor the healing process and prevent infections that can lead to treatment failure with dental implants. Objectives: To compare marginal gap width and depth with different cementation systems, excess removal, and after polishing. Method and materials: In total, 80 composite crowns were milled, divided into ten groups, and cemented on identical artificial teeth. Eight crowns per group were fixed with (i) zinc phosphate cement (ZnOPh), (ii) glass-ionomer cement (GIC), (iii) resin-reinforced glass-ionomer cement (GIC mod), (iv) dual-curing adhesive composite (Comp dual), or (v) dual-curing self-adhesive composite (Comp SE dual). Excess removal was performed with a scaler after brief light-cure (tack-cure), final light-cure, during rubber or gel phase or by wiping with foam pellet. Curing was completed in chemical, dark cure, or light-curing modus. The specimens were polished and stored in water (37°C). The margins were digitized using a 3D laser-scanning microscope (VK-X100 series, Keyence). The width and the depth of the marginal gap were measured at 10 points between the crown margin and the preparation margin. Results: The width after excess removal varied between 65.1 ± 15.7 µm (Comp dual, wipe, with polishing) and 208.6 ± 266.7 µm (Comp SE dual, dark cure, without polishing). The depth varied between 29.8 ± 22.2 µm (Comp dual, wipe, with polishing) and 89.5 ± 45.2 µm (Comp SE dual, dark cure, without polishing). The impact on gap width and depth was detected for fixation material, excess removal, and polishing. Conclusion: The gap depth and width depend on the luting material and the mode of access removal. Objectives: To evaluate the effect of sonic oscillation on penetration depth and marginal adaptation of resin-based fissure sealants. Method and materials: Thirty-six extracted human third molars were randomized into three groups: in the HF group, teeth were sealed with a filled-resin-based sealant (Helioseal F); in the Sonic-HF group, teeth were sealed with Helioseal F and subjected to sonic oscillation (Compothixo Device, 140 Hz vibration frequency and ± 150 µm amplitude) before curing; in the CO group, teeth were sealed with an unfilled-resin-based sealant (Clinpro). All teeth were thermocycled for 1,500 cycles at 5°C and 55°C, then sectioned buccolingually into four sections (mesial, two middle, and distal). Sections were evaluated under SEM for marginal adaptation and scored. For penetration depth, three independent measurements from each tooth were recorded under SEM, and data were subjected to statistical analysis. Results: The group subjected to sonic oscillation (Sonic HF) resulted in significantly superior penetration depth and marginal adaptation compared with the HF group, but with no significant difference compared with the CO group. Conclusions: Sonic activation of resin-based sealant increased penetration depth into fissures and marginal adaptation to the enamel wall without decreasing filler load or compromising sealant mechanical properties. Periodontal intrabony defects (IBDs) are commonly encountered in dental practice and are usually treated by open flap debridement (OFD) with or without bone grafts and/or guided tissue regeneration (GTR), platelet-rich fibrin (PRF), or other additives. A difficulty with these approaches is maintaining the space for bone growth at the determined location. In this report, autologous sticky bone (ASB) was used and compared to a simple mixture of PRF and bone graft (PRF-BG) using the identical graft material-bioactive glass morsels-to assess its regenerative potential in IBDs, as sticky bone is known to maintain the space, according to previous research. A total of 21 IBDs were treated by OFD, PRF-BG mixture, or ASB. Regenerative assessment was clinically and radiographically performed by CBCT at 1 year. All the treatment modalities (OFD, PRF-BG, and ASB) showed statistically significant improvements clinically and radiographically at 1 year in terms of probing pocket depth reduction (PPDR), clinical attachment level gain (CALG), CBCT defect fill (CBCT-DF), and CBCT defect resolution (CBCT-DR); P < .05. The ASB group depicted the most favorable results (P < .05) in terms of the parameters at 1 year followed by the PRF-BG group and then the OFD group. Treatment of periodontal IBDs with ASB resulted in significant improvements in clinical and CBCT parameters compared to baseline at 1 year, and intrasurgical graft handling was much better in the ASB group. This study investigated the effect of erbium, chromium: yttrium-scandium-gallium-garnet (Er,Cr:YSGG) laser conditioning on dentin bond strength and nanoleakage of different universal and self-etch adhesives. A total of 84 intact human third molar teeth were cut at the dentin level, and half of them were laser conditioned. The specimens were divided into three groups; two different universal resins and one self-etch adhesive resin were applied and composite resin restorations were created. For the microtensile bond strength (pTBS) test, 20 microspecimens were prepared from the laser and control group of each adhesive (n = 20) and tested with a universal device. For nanoleakage observation, 10 specimens were prepared from each group (n = 10) and stored in silver nitrate solution, and then the amount of nanoleakage was analyzed by field emission scanning electron microscopy (FESEM). Data were analyzed with two-way ANOVA, Tukey's honestly significant difference, and chi-square tests. The mean dentin bond strength of all adhesives in the laser groups was found to be statistically significantly lower than those of the control groups (P < .05). No difference was found between the mean bond strength of the adhesives in the laser and control groups (P > .05). Higher nanoleakage was observed in all adhesives in the laser groups compared to the control groups (P < .05). Irradiation of the dentin surface with Er,Cr:YSGG could adversely affect the piTBS and nanoleakage, likely by affecting the structure of the hybrid layer. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate the marginal leakage of three different types of bonding agents used with two posterior composites and a giomer, available commercially. Material and methods: Class II box-only cavities were prepared on 90 mandibular first molars with margins extending 1 mm beyond the cementoenamel junction. The samples were divided into nine groups based on three different bonding agents, two different composites, and a giomer. Cavities were restored per the manufacturer's description. Teeth were subjected to a thermocycling procedure (500Ã--, 5°C to 55°C) and dye penetration by immersing in a 2% methylene blue solution (Merck Specialties) for 24 hours. The marginal adaptation was evaluated as a continuous margin at the gingival level under a stereomicroscope. The results were analyzed using the Kruskal-Wallis and Mann-Whitney U tests. Results: The results of the groups with the total-etch technique showed no statistical difference between the nanohybrid Filtek Z250 XT (3M ESPE) and fine hybrid SwissTEC composite (Coltène\Whaledent AG). The groups with the self-etch technique showed no statistical difference when used with either of the two composites. The acid-etch technique showed better marginal adaptation than the self-etch technique. When the giomer (Beautifil II, Shofu) was used with the total-etch technique, it showed better adaptation than when used with the self-etch technique but overall showed more marginal leakage than the composites. Conclusion: The total-etch technique provided better marginal adaptation for the composites and giomer compared to the self-etch technique. Clinical significance: The demand for esthetic restorations continues to increase, and composite restorations are now considered the material of choice for Class II cavities. There are various concerns with composite restorations, one of them being good marginal adaptation to increase the life of the restoration. This study evaluated marginal leakage with the nanohybrid composite Filtek Z250 XT, fine hybrid composite SwissTEC, and a giomer when used with the total-etch technique and self-etch technique and reported satisfactory adaptation with the total-etch technique. This article evaluated tooth discoloration after the use of calcium silicate-based materials to examine the effect of internal bleaching on tooth discoloration. The specimens were randomly divided into two experimental groups (n = 45) and a control group (n = 6). Cavities were filled with ProRoot MTA (Dentsply Sirona) in Group 1 and Biodentine (Septodont) in Group 2. Color measurements were taken with a spectrophotometer before and after the application of materials at 1 week, 13 weeks, and 6 months. After 6 months, Groups 1 and 2 were divided into three subgroups according to internal bleaching techniques. All color change ratios and lightness differences were calculated using the CIE L*a*b* system. Data were analyzed using repeated ANOVA and Kruskal-Wallis test (P = .05). There were statistically significant differences between Groups 1 and 2 at all time intervals (P < .05). Group 1 showed more discoloration than Group 2 (P < .05). There were no significant differences between the bleaching agents (P > .05). Additionally, specimens in both groups became lighter than their initial color (P < .05). Teeth treated with ProRoot MTA exhibited darkening at 1 week that increased over time, whereas those treated with Biodentine maintained the lightness for 6 months. [ABSTRACT FROM AUTHOR]

Purpose: To examine the stresses caused by different All-on-4 surgical techniques—conventional, a combination of monocortical and bicortical, bicortical, and nasal floor elevation—on the implant and the surrounding bone using 3D finite element analysis (FEA). Materials and Methods: A 3D bone model of the atrophic maxilla was created based on CT imaging of the fully edentulous adult patient. All implants used in the models were 4 mm in diameter, and the length was 13 mm in the anterior and 15 mm in the posterior. Implants were applied to four different atrophic maxillary models with the All-on-4 technique: anterior and posterior monocortical implants in the first model, anterior monocortical and posterior bicortical in the second model, anterior and posterior bicortical in the third model, and anterior and posterior bicortical with nasal floor elevation in the fourth model. Eight linear analyses were performed by applying force from both vertical and 45-degree oblique directions to the four models prepared in our study. Results: When the cortical and cancellous bone around the anterior implants was examined, it was observed that the oblique and vertical loading conditions and the stresses around the implant were similar in all models. When the posterior implants were examined, model 1 (ie, anterior and posterior monocortical implants) showed the greatest oblique compression, vertical compression, and vertical tension forces. According to the Von Mises stress (VMS) analysis results for anterior and posterior implants, higher values were observed in model 1 compared to models 3 and 4 under oblique and vertical forces. It was observed that bicortical placement of the implants reduced the stresses on the bone and implant-abutment system but had no significant effect on the stress on the bar. Conclusions: According to the results of our study, in the All-on-4 technique, bicortical placement of the implants reduced the stresses on the bone and implant when the anatomical limitations allowed. The article discusses research published in the 2021 issue of the "International Journal of Environmental Research and Public Health" which evaluated the use of short dental implants compared to standard dental implants. The research reviewed studies on short implants and standard-length implants placed in augmented and nonaugmented edentulous sites, short implant survival, stability and marginal bone levels, and implants placed in the posterior maxilla and in medically compromised patients.

Purpose: To measure the influence of postpolymerization condition (dry and water-submerged) and time (2, 10, 20, and 40 minutes) on the accuracy of additively manufactured model material. Materials and Methods: A bar standard tessellation language (STL) file was used to manufacture all the resin specimens using a 3D printer. Two groups (n = 80 each) were created based on postpolymerization condition: dry (D group) and water-submerged (W group). Each group was then divided into four subgroups (D1 to D4 and W1 to W4; n = 20 each), which were each assigned a postpolymerizing time (2, 10, 20, and 40 minutes). The specimens' dimensions were measured using a low-force digital caliper. The volume was calculated as follows: V = l × w × h. Shapiro-Wilk test revealed that the data were not normally distributed. Data were analyzed using Kruskal-Wallis and pairwise Mann-Whitney U tests (a = .05). Results: Significant differences in length, width, height, and volume were found among the subgroups (P < .0018). In all groups, the width dimension (x-axis) presented less accuracy compared to height (z-axis) and length (y-axis) (P < .0018). The D2 and D4 subgroups obtained the closest dimensions to the virtual design, and there were no significant differences between these subgroups (P < .0018). The dry condition showed higher manufacturing accuracy than the water-submerged condition. In the water-submerged subgroups, the highest accuracy was obtained in the W2 and W4 subgroups (P < .0018). Conclusions: Postpolymerization condition and time influenced the accuracy of the material tested. The dry postpolymerization condition with times of 10 and 40 minutes obtained the highest accuracy. Testing the biocompatibility of commercially available dental materials is a major challenge in dental material science. In the present study, the biocompatibility of four commercially available dental materials Mineral Trioxide Aggregate, Biodentine, Harvard BioCal-CAP and Oxford ActiveCal PC was investigated. The biocompatibility analysis was performed on zebrafish embryos and larvae using standard toxicity tests such as survivability and hatching rates. Comparative toxicity analysis of toxicity was performed by measuring apoptosis using acridine orange dye and whole mount immunofluorescence methods on zebrafish larvae exposed to the dental materials at different dilutions. Toxicity analysis showed a significant decrease in survival and hatching rates with increasing concentration of exposed materials. The results of the apoptosis assay with acridine orange showed greater biocompatibility of Biodentine, Oxford ActiveCal PC, Harvard BioCal-CAP and Biodentine compared to MTA, which was concentration dependent. Introduction: Orthodontic clear aligners and retainers have numerous advantages that is making them ever increasingly popular. However, they might, similar to any other oral appliance, contribute to biofilm formation and finally dental caries or white spot lesions or gingival inflammations. The literature on biofilm formation on orthodontic clear appliances is very scarce and limited to a few microorganisms and materials. Therefore, this experimental study evaluated the biofilm formation on 5 thermoformed and 3D printed CAD/CAM orthodontic retainers in 3 intervals. Methods: In this in vitro study, 345 specimens (270 test discs and 45 negative controls) were created from fabricated retainers. Retainers included a 3D printed CAD/CAM material (Detax) and four thermoformed retainers [Erkodent (polyethylene terephthalate glycol [PETG]); EasyVac (polyethylene); DB (polyester based on terephthalic acid); and Clear Tech]. They were all 1 mm thick, and all completely fabricated, i.e., heated or printed. The discs were placed in 96-well plates. Microorganisms were cultured on 270 discs for 24 h (90 discs), 72 h (90 other discs), and 5 days or 120 h (90 other discs). Biofilm formation of the strains and negative controls was measured using the microtiter plate assay by ELISA reading. The microbes' ability to produce biofilm was categorized based on the comparison of average optical density (OD) of tests versus a cut-off point OD (ODc) calculated as the average of the OD of corresponding negative controls plus 3× its standard deviation: non-biofilm former [OD ≤ ODc], weak biofilm former [ODc < OD ≤ (2 × ODc)], moderate biofilm former [(2 × ODc) < OD ≤ (4 × ODc)], and strong biofilm former [(4 × ODc) < OD]. These were also converted to ranked scores between zero (no biofilm) and 3. The difference between ODs with control ODs were calculated. These were analyzed using 3-way ANOVA, 2-way ANOVA, and Tukey tests (α = 0.05, α = 0.008). Results: The 3-way ANOVA showed that the overall difference among the ΔODs of 5 retainers (all microorganisms and all intervals combined, n = 270) was not significant (F = 1.860, P = 0.119). Nevertheless, the difference among 3 intervals (F = 31.607, P = 0.0000) and the difference among the 6 microorganisms (F = 24.044, P = 0.0000) were significant. According to the Tukey test, the differences between the 1st interval with either of the other two intervals was significant (both P values = 0.000). There were significant differences between Candida albicans with all other organisms (all 5 P values = 0.0000). All other pairwise comparisons were insignificant (all 10 P values ≥ 0.1). After taking the averages of the 3 intervals, the order of the biofilm generation for different materials were as follows: Detax (average score: 1.56), Easyvac (1.67), Erkodent (1.78), Clear Tech (1.83), BD (2.28). Conclusions: As far as these 6 microorganisms are of concern, there might not be a significant overall difference among the clear retainer materials tested in this study. A significant overall increase was observed between the first and third days, which later did not significantly increase more until day 5. ABSTRACT Objective Clinical Considerations Conclusions Clinical Significance The present paper aims to demonstrate the incorporation of the double monolithic protocol (DMP) into a chairside digital workflow to reproduce the fluorescence properties of natural teeth with chairside monolithic restorations when exposed to different light sources.A female patient reporting dissatisfaction with her upper anterior teeth was rehabilitated using seven veneers and a three‐element bridge. The DMP was applied to the bridge, which consisted of a primary lithium disilicate framework to which leucite‐reinforced glass ceramic veneers were cemented. The fluorescence of the different substrates and ceramic restorations was evaluated throughout the rehabilitation process, under 365 and 405 nm light.The DMP allows chairside procedures to be optimized by achieving predictable, mechanically resistant, and esthetic restorations.The DMP is a new chairside solution for developing ceramic restorations with optimal esthetics and combined mechanical properties, eliminating the need for sintering or glazing procedures. Background: The effect of an immune-boosting beverage (SAM) containing Sambucus Nigra, an energy beverage (ENE), an in-office bleaching (BLE) agent with 25% hydrogen peroxide superior, glazing (GLA) or polishing (POL) methods, and professional dental prophylaxis (PDP) on the color of CAD-CAM restorative materials is unknown. Methods: In total 210 specimens were prepared, consisting of CAD-CAM feldspathic (FC), zirconia-reinforced lithium disilicate ceramic (ZLS) and hybrid ceramic (HC). The ceramic specimens were divided according to the polishing methods of glazing (GLA) and mechanical polishing (POL). All materials were divided into two groups: with and without BLE. A 25% hydrogen peroxide superior (HPS) gel was used for BLE. After the baseline (BAS) measurement, the specimens were immersed in 3 different beverages (distilled water (DIS), SAM, ENE). After 28 days, a fine-grained (RDA 7) prophylaxis paste was applied. Statistical analysis of ∆E00 color difference values was performed by 3-way ANOVA and Bonferroni test (α = 0.05). Results: The effect of all other actions except material-BLE-beverage on color for BAS-Day 28 was statistically different (p < 0.05). The effect of material, material-BLE, beverage on color for Day 28-PDP was statistically different (p < 0.05). After 28 days, the lowest color change was found in FC-GLA and HC immersed in DIS (p = 0.0001) and the highest in FC-POL immersed in ENE (p = 0.0002). PDP was efficient in color recovery in HC immersed to DIS, ENE and SAM (p = 0.0010). For FC, HC and ZLS, BLE caused a higher color change (p < 0.0001). Regardless of the material, the highest color change for BLE-beverage was found in BLE-treated specimens immersed in ENE (p = 0.0496) and the lowest color change was found in non-BLE-treated specimens immersed in SAM (p = 0.0074). Conclusions: In materials pre-exposed to 25% HPS, the effect of PDP on color recovery was lower than in unexposed materials. After 28 days, mechanical polishing produced higher color change in FC than glazing, however, in ZLS effects of glazing and mechanical polishing on color were similar. For material/polishing method, HC was the most effective. ENE caused higher color change than DIS and SAM. PDP was more effective than ENE in restoring color to DIS- and SAM-immersed specimens. Background: Infected dentinal tubules are a possible source of bacteria that are responsible for the failure of root canal treatment. Therefore, disinfection of dentinal tubules by increasing the penetration of the irrigation solution is important for success in retreatment cases. This study utilized confocal laser scanning microscopy (CLSM) to assess and compare the impact of XPR, ultrasonic irrigation (UI) and sonic activation (SA) on NaOCl penetration into dentinal tubules following endodontic retreatment. Methods: A total of forty mandibular premolars were enrolled in this investigation. Following root canal preparation up to ProTaper X3 file (30/0.07), root canals were obturated with gutta-percha and bioceramic root canal sealer with single cone technique. The root canal filling materials were removed using ProTaper nickel-titanium rotary retreatment files until the working length was reached. The retreatment procedure was finalized using the ProTaper Next X4 (40/0.06). The teeth were divided into four groups based on the irrigation activation technique: control (conventional needle irrigation), SA, UI and XPR. During the final irrigation procedure, Rhodamine B dye was introduced to 5% NaOCl for visualization via CLSM. Subsequent to image acquisition, the maximum penetration, penetration percentage, and penetration area were calculated. Data were statistically analyzed using the Kruskal-Wallis, Friedman, and Bonferroni Dunn multiple comparison tests through R software (p < 0.05). Results: In the middle third, UI yielded a significantly higher penetration percentage than the control group (p < 0.05). The UI and XPR groups showed increased penetration percentages in the coronal and middle thirds compared with the apical third (P < 0.05). Maximum penetration was notably reduced in the apical third than in comparison with the coronal and middle thirds in all groups (p < 0.05). In the control, SA and XP groups, the penetration area was ranked in descending order as coronal, middle and apical (p < 0.05). Conversely, in the ultrasonic group, the penetration area was significantly lower in the apical third than in the middle and coronal thirds (p < 0.05). Conclusions: UI enhanced the penetration percentage in the middle third of the root compared with that in the control group. Background: Apexification is a procedure that promotes apical closure by forming mineralized tissue in the apex region of a nonvital young permanent tooth. Calcium silicate-based cement like Mineral trioxide aggregate (MTA) and Biodentine are commonly employed as apical barriers to facilitate this process. Microleakage, defined as the leakage along the junction between the canal wall and filling material, is a crucial aspect to assess in MTA and Biodentine applications as apical barriers, as it directly impacts the prevention of bacterial seepage and maintenance of structural integrity. The current study aims to assess the microleakage of MTA and Biodentine when used as apical barriers in simulated young permanent teeth. Methods: From a total of 128 extracted teeth, 114 were selected for the study and randomly allocated into three groups: G1 (MTA), G2 (Biodentine), and G3 (Control), with 38 teeth per group. After excluding 5 teeth from each group due to issues such as canal calcification, breakage during sectioning, and procedural errors, 33 teeth were analyzed to ensure equal distribution. To simulate young permanent teeth, samples were instrumented using a person-reamer with a diameter of 1.7 mm. A 4 mm thick apical plug of MTA and Biodentine was placed in G1 and G2, respectively, while G3 was the control group. Apical microleakage in all experimental groups was assessed using a dye penetration method. Specimens were longitudinally sectioned and examined under a stereomicroscope with graded eyepiece. Results: The Kruskal-Wallis test revealed variations in mean apical microleakage among the groups: G1 recorded 0.67, G2–0.16, and G3–1.62, with G2 showing the lowest value and G3 group exhibiting the highest. Conclusions: Biodentine was found to excel in its ability to create a secure seal and function effectively as an apical barrier in simulated young permanent teeth. These results underscore its potential as a highly efficient material for dental applications, particularly in scenarios requiring reliable sealing and barrier formation in the root canal system of developing permanent teeth. An ideal flowable composite (FC) requires good mechanical properties to fulfill its role as a dental restorative material and ensure durability and clinical efficacy, with these properties being influenced by different monomer ratios. The study aimed to evaluate the mechanical properties of the newly developed FC on Vickers hardness, flexural strength, and flexural modulus at different monomer ratios. The newly developed FC employed nanohybrid silica from rice husk and zirconia as fillers, and the ratio of urethane dimethacrylate to triethylene glycol dimethacrylate (UDMA:TEGDMA) at 20:80, 30:70, 50:50, 60:40, 80:20 and 90:10 as monomers. Filtek Supreme Ultra Flowable Restorative, Revolution Formula 2, and G-aenial Universal Flo served as control groups. The data were statistically analyzed using oneway ANOVA with post-hoc Bonferroni test. Generally, the higher UDMA ratio in the newly developed FCs exhibited higher mechanical properties. All newly developed FCs demonstrated a significant decrease in Vickers hardness than control groups. The flexural strength values at ratios of 50:50, 60:40, 80:20, and 90:10 were comparable with control groups and passed the International Organization for Standardization 4049 requirement. Meanwhile, the flexural modulus of the newly developed FC at the ratio of 90:10 was comparable to Revolution Formula 2. In summary, the differences in monomer ratios affect the mechanical properties of the newly developed FCs. The improved mechanical properties at ratios of 80:20 and 90:10 could potentially substitute sustainable dental restorative materials, subjected to further investigations. [ABSTRACT FROM AUTHOR]

Background: To predict the long-term performance of restorative materials in the oral environment, it is important to evaluate their resistance to chemical and mechanical degradation and to know the toxic potential of the type and amount of ions eluted from the filling material. In this study, home bleaching was applied to dental materials with different contents and it was aimed to determine the type and amount of ions released from these materials. Methods: In this study, amalgam, posterior composite resin, anterior composite resin, bulk fill composite resin, indirect composite resin, hybrid ceramic and all-ceramic were used as restorative materials. 10 specimens of each material were prepared according to the manufacturer's instructions. Each material group was divided into two subgroups as the bleached group and the control group. After bleaching, all specimens were stored in 1 ml of 75% ethanol/water solution. Solutions were renewed after 1, 14 and 28 days. The type and amount of ions released from the materials were determined using Inductively Coupled Plasma-Mass Spectrometer (ICP-MS). Data were analyzed using the Friedman, Wilcoxon Signed Ranks, and Mann-Whitney U tests (α = 0.05). Results: It was determined that the amount of ions release from the restorative materials decreased over time (p < 0.05). According to the results of the Mann-Whitney U test, there was no difference between the bleaching and control groups in most of the restorative materials (p > 0.05). Background: Different materials have been used as wound dressings after vital pulp therapies. Some of them have limitations such as delayed setting, difficult administration, slight degree of cytotoxicity, crown discoloration and high cost. Therefore, to overcome these disadvantages, composite scaffolds have been used in regenerative dentistry. This study aims to construct and characterize the physicochemical behavior of a novel injectable alginate hydrogel loaded with different bioactive glass nanoparticles in various concentrations as a regenerative pulpotomy filling material. Methods: Alginate hydrogels were prepared by dissolving alginate powder in alcoholic distilled water containing mesoporous bioactive glass nanoparticles (MBG NPs) or boron-doped MBG NPs (BMBG NPs) at 10 and 20 wt% concentrations. The mixture was stirred and incubated overnight in a water bath at 50 0 C to ensure complete solubility. A sterile dual-syringe system was used to mix the alginate solution with 20 wt% calcium chloride solution, forming the hydrogel upon extrusion. Then, constructed hydrogel specimens from all groups were characterized by FTIR, SEM, water uptake percentage (WA%), bioactivity and ion release, and cytotoxicity. Statistical analysis was done using One-Way ANOVA test for comparisons between groups, followed by multiple pairwise comparisons using Bonferroni adjusted significance level (p < 0.05). Results: Alginate/BMBG loaded groups exhibited remarkable increase in porosity and pore size diameter [IIB1 (168), IIB2 (183) (µm)]. Similarly, WA% increased (~ 800%) which was statistically significant (p < 0.05). Alginate/BMBG loaded groups exhibited the strongest bioactive capability displaying prominent clusters of hydroxyapatite precipitates on hydrogel surfaces. Ca/P ratio of precipitates in IIA2 and IIB1 (1.6) were like Ca/P ratio for stoichiometric pure hydroxyapatite (1.67). MTT assay data revealed that the cell viability % of human gingival fibroblast cells have declined with increasing the concentration of both powders and hydrogel extracts in all groups after 24 and 48 h but still higher than the accepted cell viability % of (˃70%). Conclusions: The outstanding laboratory performance of the injectable alginate/BMBGNPs (20 wt%) composite hydrogel suggested it as promising candidate for pulpotomy filling material potentially enhancing dentin regeneration in clinical applications. Background: Decreased salivary flow can make the patients uncomfortable with their complete dentures and affects the retention of the dentures. Milling and 3D printing have become an alternative to conventional denture construction techniques. The goal of this study was to evaluate the effect of conventional and digital techniques of the complete denture construction on the denture surface wettability with distilled water and saliva substitute before and after thermocycling. Methods: A total of 30 specimens were utilized in the present study. Specimens were divided according to the construction techniques into 3 groups (n = 10 each). Group I: Heat-polymerized polymethylmethacrylate (PMMA) group, group II: Milled group, and group III: 3-dimensional (3D)-printed group. All the specimens were subjected to 2000 cycles of thermal aging in a thermocycler. The wettability of all specimens to water and saliva substitute was measured via a contact angle goniometer (Olympus TGHM, Rame-hart Inc, USA) before and after thermocycling. Descriptive statistical analysis, plots, and the Shapiro-Wilk test were used to verify normality for each variable. One-way ANOVA was used to compare the 3 study groups, while paired samples t-test was used to compare the differences within each group (P <.05). Results: The smallest contact angle of drop of water to the denture base specimens before and after thermocycling were recorded in the milled group (53.0 ± 4.77 and 50.27 ± 2.30, respectively), followed by the heat polymerized PMMA group (85.65 ± 4.71 and 65.06 ± 2.27, respectively), and the 3D-printed group (91.34 ± 6.74 and 90.86 ± 8.57, respectively). While the smallest contact angle of drop of saliva substitute to denture base specimens was recorded in the milled group (56.82 ± 2.29 and 34.85 ± 7.51, respectively), followed by the 3D-printed group (72.87 ± 4.83 and 58.14 ± 9.58, respectively) and the heat polymerized PMMA group (83.62 ± 4.12 and 67.82 ± 4.93, respectively). There was statistically significant difference between the groups (P <.05). A significant decline in the average contact angle of drop of saliva has been reported in all groups after thermocycling. The contact angle values differed significantly between saliva substitute and distilled water in both 3D-printed and milled groups after thermocycling (P <.001). Conclusions: The milled denture base material presented the best wettability to water and saliva substitute than the 3D-printed and the heat-polymerized PMMA materials. Background: Root canal retreatment is necessary when initial treatment fails. Nickel-titanium systems help remove filling materials; however, some methods can cause canal content extrusion, resulting in inflammation and delayed healing. This study aimed to examine the impact of different preparation techniques and endodontic file systems on apical debris volume to limit material dispersion to periradicular tissues. Methods: Ninety extracted maxillary incisors were evaluated. Simulated apical root resorption was created by removing the apical 2 mm of the root. The teeth were divided into two groups: one filled with a single cone and the other with cold lateral compaction (CLC). Each group was further divided into three subgroups (n = 15 each): Subgroup 1 used Reciproc R25 for removal and R50 for final preparation; Subgroup 2 used ProTaper Universal Retreatment (PTUR) files D1, D2, and D3 for removal and ProTaper Next X5 for finalization; and Subgroup 3 used VDW.Rotate Retreatment (VDW.RotateR) for removal and VDW.Rotate 50.04 for completion. Debris from retreatment was collected in pre-weighed tubes to determine the amount. The apical extrusion data underwent a two-way analysis of variance. Results: The Reciproc group had a mean extruded debris weight of 0.54 ± 0.24 mg, higher than the PTUR (0.28 ± 0.15 mg) and VDW.RotateR (0.39 ± 0.29 mg) groups (p < 0.05). The single-cone technique (SCT) resulted in a mean debris weight of 0.34 ± 0.23 mg, lower than the CLC technique, which had a mean of 0.46 ± 0.27 mg (p < 0.05). SCT had a shorter retreatment duration (111.12 ± 33.46 s) compared to CLC (176.26 ± 52.26 s) (p < 0.05). Conclusion: The Reciproc file system and the CLC obturation technique resulted in greater apical debris extrusion than the other methods. SCT was quicker than CLC. Background: This study aimed to compare the marginal adaptation of a single customized gutta percha cone with calcium silicate-based sealer versus mineral trioxide aggregate (MTA) and Biodentine apical plugs in simulated immature permanent teeth. Methods: Thirty-nine extracted human maxillary anterior teeth were selected, prepared to simulate immature permanent teeth with an apical diameter 1.1 mm, placed in moist foam and divided into three groups. Group 1: Obturation with a single customized gutta percha cone and calcium silicate sealer. Group 2: MTA apical plug. Group 3: Biodentine apical plug. After incubation, teeth were horizontally sectioned at 1 mm and 3 mm from the apex and marginal adaptation was evaluated using scanning electron microscope (SEM). Results: Biodentine showed the least mean gap size at both 1 and 3 mm from the apex with no statistically significant differences compared to MTA (p > 0.05). The single customized cone with calcium silicate based sealer showed the greatest mean gap size at both 1 and 3 mm from the apex with a statistically significant difference compared to the other groups (p < 0.001). Conclusion: Biodentine and MTA apical plugs provide a significantly better marginal adaptation to the dentinal walls than a single customized gutta percha cone with calcium silicate based sealer in simulated immature permanent teeth. Background: Implant-retained obturators for maxillectomy cases have several advantages over traditional obturators but prosthetic design for specific conditions after maxillary resection has several challenges and the appropriate implant placement configuration is essential for improving retention and the stability of the implant-retained obturator. Objectives: The present study aimed to assess the retention force of using linear and nonlinear implant placement configurations using ball and socket attachment in implant-retained obturators at the initial retention and after simulation of six months of use. Materials and methods: Two identical epoxy resin maxillary models of a completely edentulous unilateral maxillary defect (Brown's class IIb) were used for implant placement, in the first model three implants were arranged with linear placement configuration, and in the second model three implants were arranged in nonlinear placement configuration. For proper sample sizing, 26 models and obturator were used. Two equal groups of obturators (13 for each group) were constructed, each with a different implant placement configuration. Both groups used the same attachment design (a non-splinted ball attachment). Using a cyclic loading machine that served as a dental insertion and removal simulator, each study group was subjected to 500 tension-compression cycles simulating 6 months of use. Using the universal testing machine, each obturator was removed at a speed of 50 mm/min for the crosshead. peak load to dislodgement was measured at the initial retention and after the simulations of six months of use. Data were analyzed using independent and paired t-tests while percent change was analyzed using the Mann Whitney U test. Results: There were a statistically significant differences in retention between the nonlinear implant placement configuration for Brown's class IIb maxillectomy and the linear implant placement configuration at initial retention evaluation with p-value of < 0.0001 and after simulation of six months of usage with p-value of < 0.0001 Also, after simulation of 6 months of use group I lose − 24.87 (10.16) % of its retention while group II lose − 17.49 (7.78) %. Conclusions: Non-linear implant placement is more retentive at the initial retention and after simulation of six months of use than linear and loses less retention after usage. The E-Health Big Data Evidence Innovation Network (FeederNet) in Korea, based on the observational medical outcomes partnership (OMOP) common data model (CDM), had 72.3% participation from tertiary hospitals handling severe diseases as of October 2022. While this contributes to the activation of multi-institutional research, concerns about the comprehensiveness of device data persist due to the adoption of national health insurance corporation (NHIC) claim codes as device identifiers in the medical device field. This study critically evaluated the effectiveness and compatibility of NHIC claim codes and unique device identifier (UDI) within FeederNet to identify the optimal identifier for efficient Post-market surveillance (PMS). Specifically, this study addressed three main questions: (1) the number of UDIs classified as NHIC-covered items, (2) the number of UDIs included in each NHIC claim code, and (3) the number of NHIC claim codes each UDI covers. Among the 1,979,655 UDIs registered domestically, only 36.02% (712,983) were classified as covered by National Health Insurance. NHIC-covered medical devices were limited to categories (A) medical devices, (B) medical supplies, and (C) dental materials, excluding most software and in vitro diagnostics (IVD). Multiple UDIs could be registered under a single NHIC claim code, and a single UDI could be registered under multiple NHIC claim codes. Only 32.62% (13,756/42,171) of NHIC claim codes had registered UDIs, with an average of 53 UDIs per claim code. Of the UDIs listed as NHIC covered, 92.39% (659,046/713,341) had one claim code, while 7.25% (51,652) had multiple claim codes. Additionally, 2643 UDIs were listed as NHIC covered but had no registered claim codes. Due to this complex relationship, NHIC claim code-based PMS may pool safe and unsafe models or disperse problematic models across multiple claim codes, leading to a lower problem rate or insignificant differences between claim codes, thus reducing signal detection sensitivity compared to UDI-based PMS. In conclusion, NHIC claim code-based PMS has limitations in granularity and signal detection sensitivity, necessitating the adoption of UDI-based PMS to address these issues. The UDI system can enhance the accuracy of medical device identification and tracking, playing a crucial role in generating real-world evidence (RWE) by integrating data from various sources. Future research should explore specific strategies for integrating and utilizing UDI with NHIC claim codes, contributing to the implementation of a more reliable and comprehensive PMS in Korea's healthcare system. AbstractOral diseases are a prevalent global issue, causing significant health and economic burdens that greatly diminish the quality of life for affected individuals. Tissue engineering strategies employing scaffolds offer a promising approach to restoring the original structures and biological functions of dental and dentoalveolar tissues. Among the various building materials, polysaccharides, known for their inherent biocompatibility, have gained significant attention. Topical advances in tissue engineering have led to the fabrication of composite scaffolds with the greatest utilization of favorable properties of each material. This review aims to explore the application of polysaccharide-based composite scaffolds for regenerating dental and dentoalveolar tissues, including both hard and soft tissues, as well as tissue complexes. To begin, we will provide a brief overview to commonly used polysaccharides. Subsequently, we will introduce the commonly used forms of composite polysaccharide scaffolds together with fabricating, engineering, and optimizing methods for dental and dentoalveolar tissue regeneration. Next, we will examine the latest progress and the potential of polysaccharide composite scaffolds in dental and dentoalveolar tissue regeneration applications. To conclude, this review will reflect on the limitations and prospects associated with polysaccharide composite scaffolds for dental and dentoalveolar tissue regeneration. This research aimed to create a new Li metasilicate-based glass-ceramic for dental use, prioritizing improved chemical durability (CD) and machinability while preserving good glass-forming ability (GFA) and adequate mechanical properties. Characterization also involved examining fracture strength (S f) and fracture toughness (K IC). The experimental approach involved precise microstructural adjustments through designed compositional changes and controlled thermal treatments. Four compositions underwent scrutiny using mechanical tests, microscopy, differential scanning calorimetry, and X-ray diffraction to analyze the impact of lithium metasilicate (LS) and disilicate (LS2) crystals on these properties. Machinability was evaluated by weight loss measurements during controlled grinding experiments. The developed GCs exhibited interconnected acicular LS and LS2 crystals, which resulted in favorable S f ∼300 MPa and K IC ∼1.7 MPa m1/2 (SEVNB). Although the K IC was smaller than that of an experimental LS-LS2 GC (285 MPa and 3 MPa m1/2(double torsion)) used as a reference, the chemical durability (126 μg/cm2 for ML14F GC) improved by nearly 50 % compared to the reference (∼220 μg/cm2). This study introduced three new GCs with LS as the major phase (rather than the traditional LS2), showcasing commendable GFA, chemical durability and machinability, with satisfactory flexural strength and fracture toughness. Bioactive glass ceramics (BGC) play a pivotal role in bone tissue engineering, specifically addressing challenges associated with bone repair and regeneration. In this study, we developed silica-based 55S0P and 45S10P spherical BGC nanoparticles (NPs), without and with P 2 O 5 content, using a modified Stöber sol-gel process with base hydrolysis. The fabricated spherical NPs with pronounced textural features effectively contribute to enhanced surface roughness, a desirable parameter for improved biological activity in physiologically relevant solutions and tissue matrices. A comprehensive evaluation of the structural properties of these novel spherical BGC-NPs using diverse characterization techniques revealed distinct nano-textural features in the form of cracks and pores. The HR-TEM confirmed the nanoscale dimensions (170–190 nm) of particles with spherical surface morphology and also systematically analysed via DLS, FE-SEM studies. An augmented surface area with mesopores when phosphate added to the silica glass network was revealed by BET analysis. On the biological front, the study records the formation of an HCA layer on the surfaces of the BGC-NPs, the thickness of which increased with an increase in both phosphate content and immersion time (0, 3, 14 & 28 days) in the SBF solution. The zeta potential values attain a maximum from −22 mV to −23.6 mV for 55S0P and −21.5 mV to −26.4 mV for 45S10P BGC-NPs, and improved with increased immersion time, thereby favouring cellular adhesion for desired biological responses. In vitro, cell culture studies on MG-63 cells, after 24 and 72 h of incubation, demonstrated higher cell adhesion and proliferation, with pronounced biocompatibility. In addition, these BGC-NPs exhibited good hemocompatibility even at the highest test concentration (15 mg/ml) and demonstrated effective antibacterial activity against K. pneumoniae and E. coli. A combinatorial analysis of structural and biological properties revealed that the 45S10P BGC-NPs are more potent than 55S0P BGC-NPs for desired biological performances. In conclusion, the results suggest that the sol-gel fabricated BGC-NPs with enhanced nano-textural features are evidently suitable for applications in bone tissue engineering and regenerative medicine. Background and objectives: Mineral Trioxide Aggregate (MTA) is one of the main retrograde filling materials that is used today as a root end filling material and perforation repair material. This study was conducted with the aim of investigating the antibacterial and antifungal properties of four types of bio-ceramic materials, AGM MTA, Ortho MTA, Pro root MTA and Cem cement for oral and dental health. Methods: In this study, the antibacterial activity of four types of bio-ceramic materials against two bacterial strains of Enterococcus faecalis (ATTC 29212), Escherichia coli (ATTC 35318) and antifungal activity against Candida albicans (ATTC 10231) were investigated using the well diffusion method. Results: In the context of the relationship between the type of microorganism and the diameter of the growth inhibitory zone for each type of bio-ceramic material, there was no significant difference for Enterococcus faecalis, and a significant difference was observed for Escherichia coli and Candida albicans (p < 0.05). Conclusion: The results show that each of the bio-ceramic materials AGM, Pro root, Cem cement and Ortho have antibacterial and antifungal properties. AGM MTA bio-ceramic material on Candida albicans fungus and Ortho MTA bio-ceramic material had the most effect on Escherichia coli bacteria. Background: The accuracy of intraoral scanning is critical for computer-aided design/computer-aided manufacturing workflows in dentistry. However, data regarding the scanning accuracy of various adjacent restorative materials and intraoral scanners are lacking. This in vitro study aimed to evaluate the effect of adjacent restorative material type and CEREC's intraoral scanners on the accuracy of intraoral digital impressions for inlay cavities. Methods: The artificial tooth was prepared with an occlusal cavity depth of 2 mm, a proximal box width at the gingival floor of 1.5 mm, and an equi-gingival margin extended disto-occlusally at the transition line angle on both the lingual and buccal sides for an inlay restoration. The adjacent teeth were veneered with crowns made of gold and zirconia, and an artificial tooth (resin) was utilized as the control group. The inlay cavity and adjacent teeth (Gold, Zirconia, and resin) were scanned 10 times using Chairside Economical Restoration of Esthetic Ceramics (CEREC) Primescan (PS), Omnicam (OC), and Bluecam (BC). A reference scan was obtained using a laboratory scanner (3-shape E3). Scanning was performed according to the manufacturer's instructions, including powder application for the BC group. Standard tesselation language files were analyzed using a three-dimensional analysis software program. Experimental data were analyzed using a two-way analysis of variance and the Tukey's post-hoc comparison test. Results: The restorative materials of the adjacent teeth significantly affected the accuracy of the intraoral digital impressions (p <.05). The zirconia group exhibited the highest trueness deviation, followed by the resin and gold groups, with each demonstrating a statistically significant difference (p <.05). The resin group demonstrated the highest maximum positive deviation and deviation in precision. Gold exhibited the lowest average deviation value for trueness compared with those of the other adjacent restorative materials. Intraoral scanner type significantly influenced the trueness and precision of the scan data (p <.05). The average deviation of trueness according to the intraoral scanner type increased in the following order: BC > PS > OC. The average deviation in precision increased in the following order: PS>OC>BC (p <.05). Conclusion: The restorative materials of the adjacent tooth and the type of intraoral scanner affect the accuracy of the intraoral digital impression. The trueness of the digital images of the BC group, obtained by spraying the powder, was comparable to that of the PS group. Among the adjacent restorative materials, zirconia exhibited the lowest trueness. This study aims to optimize the photopolymerization process of a dental composite in order to increase the degree of conversion and reduce the release of unpolymerized monomers. In addition, it seeks to understand the kinetics of composite degradation under various oral environmental conditions. Optimization of the photopolymerization process is carried out using a Box-Behnken design, exploring factors such as irradiation time, intensity and distance. Infrared spectroscopy is used to evaluate photopolymerization parameters. Release of unpolymerized monomers is quantified using liquid chromatography at different pH levels, incubation media and time. Thermogravimetric analysis is used to study thermal degradation and establish a kinetic model. Optimized conditions for photopolymerization, determined as an irradiation time of 40 s, an intensity of 1500 mW cm−2, and a distance of 5 mm, lead to a degree of conversion of 61%, reducing the presence of unpolymerized monomers. Chromatographic analysis reveals a pH-dependent release of monomers, with acid saliva showing the highest release. Thermal analysis indicates variable activation energy values depending on ageing conditions, underlining the importance of optimal conversion for increased material strength. Background: This study looks at the clinical applications of nanotechnology in dentistry, with an emphasis on implantology, preventive care, orthodontics, restorative dentistry, and endodontics. Methods: Following PRISMA criteria and registered in PROSPERO (ID: CRD 564245), a PubMed, Scopus, and Web of Science search was conducted for studies from January 2014 to April 2024. The criteria were English-language research on nanotechnology in dental coatings, with a focus on clinical trials and observational studies. The electronic database search yielded 8881 publications. Following the screening process, 17 records were selected for qualitative analysis. Results: Nanotechnology has revolutionized dentistry. In orthodontics, nanoparticles improve antibacterial characteristics, durability, and biocompatibility, lowering bacterial colonization and plaque. In preventative care, Casein Phosphopeptide-Amorphous Calcium Phosphate (CPP-ACP) combined with stannous fluoride (SnF2) and nano-sized sodium trimetaphosphate (TMPnano) substantially remineralizes enamel. Nanostructured surfaces in dental implants, particularly those containing calcium, improve osseointegration and stability. Nanoparticles in restorative dentistry improve composite and adhesive strength, aesthetics, and longevity. Conclusions: Nanotechnology improves dental materials and equipment, resulting in better treatment outcomes and increased patient comfort. Its integration provides more effective treatments, which improves dental care and patient outcomes. Oral bacteria cause tooth caries and periodontal disease. Much research is being conducted to prevent both major oral diseases by rendering dental materials' antimicrobial potential. However, such antimicrobial materials are regarded as 'combination' products and face high hurdles for regulatory approval. We loaded inorganic montmorillonite with the antimicrobial agent cetylpyridinium chloride, referred to below as 'CPC-Mont'. CPC-Mont particles in a 1, 3 and 5 wt% concentration were added to the considered gold-standard self-etch adhesive Clearfil SE Bond 2 ('CSE2'; Kuraray Noritake) to render its antibacterial potential (CSE2 without CPC-Mont served as control). Besides measuring (immediate) bonding effectiveness and (aged) bond durability to dentin, the antibacterial activity against S. mutans and the polymerization-conversion rate was assessed. Immediate and aged bond strength was not affected by 1 and 3 wt% CPC-Mont addition, while 5 wt% CPC-Mont significantly lowered bond strength and bond durability. The higher the concentration of the antimicrobial material added, the stronger the antimicrobial activity. Polymerization conversion was not affected by the CPC-Mont addition in any of the three concentrations. Hence, adding 3 wt% CPC-Mont to the two-step self-etch adhesive rendered additional antimicrobial potential on top of its primary bonding function. [ABSTRACT FROM AUTHOR]

The current development of dental materials aims to improve their properties and expand their clinical application. New flowable bulk-fill composites have been released which, unlike what was previously common in this material category, are intended to be used alone and without a top layer, in various cavities. The study compares their kinetic of light transmission during monomer-to-polymer conversion on a laboratory-grade spectrometer, as well as their elastoplastic and aging behavior under simulated clinical conditions. Major differences in the kinetic of light transmission was observed, which is related to the degree of mismatch between the refractive indices of filler and polymer matrix during polymerization and/or the type of initiator used. Compared to the literature data, the kinetic of light transmission do not always correlate with the kinetic of functional group conversion, and therefore should not be used to assess polymerization quality or to determine an appropriate exposure time. Furthermore, the initial mechanical properties are directly related to the volumetric amount of filler, but degradation during aging must be considered as a multifactorial event. In addition to zinc oxide-based cements, resin-based materials are also available for temporary cementation. The aim of this in vitro study was to determine the influence of the different material compositions on temporary bonds. In nine test series (n = 30), temporary bis-acrylate single-tooth crowns were bonded onto prefabricated titanium abutments with nine different temporary luting materials. After simulating an initial (24 h, distilled water, 37 °C), a short-term (7 days, distilled water, 37 °C) and a long-term provisional restoration period (12h, distilled water, 37 °C; thermocycling: 5000 cycles) in subgroups (n = 10), the bond strength was examined using a combined tensile–shear test. Statistical analysis was performed by univariate analysis of variance or a non-parametric Kruskal–Wallis test, followed by post hoc tests. Of the three resin-based materials, two showed significantly higher bond strength values compared to all other materials (p < 0.001), regardless of the storage procedure. The resin-based materials were followed by eugenol-free and eugenol-containing zinc oxide materials. Significant intragroup differences were observed between the composite-based materials after all storage periods. This was only observed for some of the zinc oxide-based materials. The results show that under in vitro conditions, not only the composition of the temporary luting materials but also the different storage conditions have a significant influence on temporary bonds. This study aimed to investigate the mechanical behavior of resin composites and hybrid glass ionomer cement in class I adhesive dental restorations under loading and shrinkage conditions. Three CAD models of a mandibular first molar with class I cavities were created and restored with different techniques: a bi-layer of Equia Forte HT with Filtek One Bulk Fill Restorative composite (model A), a single layer of adhesive and Filtek One Bulk Fill Restorative (model B), and a single layer of Equia forte HT (model C). Each model was exported to computer-aided engineering software, and 3D finite element models were created. Models A and B exhibited a similar pattern of stress distribution along the enamel–restoration interface, with stress peaks of 12.5 MPa and 14 MPa observed in the enamel tissue. The sound tooth, B, and C models showed a similar trend along the interface between dentine and restoration. A stress peak of about 0.5 MPa was detected in the enamel of both the sound tooth and B models. Model C showed a reduced stress peak of about 1.2 MPa. A significant stress reduction in 4 mm deep class I cavities in lower molars was observed in models where non-shrinking dental filling materials, like the hybrid glass ionomer cement used in model C, were applied. Stress reduction was also achieved in model A, which employed a bi-layer technique with a shrinking polymeric filling material (bulk resin composite).
