Introducción: la ética profesional permite calificar los actos humanos desde una perspectiva afirmativa o negativa, donde los valores éticos y sociales permiten relacionarse eficazmente en el entorno profesional. Objetivo: identificar el nivel de satisfacción de los pacientes desde la perspectiva de los valores éticos de la relación odontólogo-paciente de los consultorios “Centro Odontológico Integral” del Dr. Guillermo Méndez y “Design Dental” del Dr. Freddy Santillán, Santo Domingo de los Tsáchilas, Ecuador. Método: se realizó un estudio descriptivo transversal, mediante la aplicación de una encuesta a 44 pacientes de dichos consultorios donde se analizaron las variables: satisfacción en la relación interpersonal, satisfacción sobre calidad de atención, escala de valoración sobre la atención odontológica y tiempo ideal de espera en consulta, luego de recibir su tratamiento odontológico. Resultados: el 89 % de los encuestados percibió una relación con el odontólogo amigable y favorable. En la satisfacción con la calidad de la atención un 41 % consideró que sí ha creado un vínculo interpersonal con su odontólogo, un 59 % no lo cree así. En cuanto a la evaluación de la atención odontológica el 64 % le otorgó el máximo de 10 puntos. El 25 % de los pacientes concordó en que el tiempo ideal de espera en consulta debía ser de 20 minutos y otro porcentaje, alto, de 48 %, consideró que el ideal fuera de 30 minutos. Conclusiones: más de la mitad de los pacientes indican que se sienten satisfechos con el tratamiento que les brindó el odontólogo.

espanolLas lesiones pulpares y periodontales son las mas prevalentes en la cavidad bucal y, tal vez por ello, son las causas mas frecuentes de consulta odontologica en nuestro medio y en el mundo. Estas patologias tienen principal-mente, etiologia bacteriana y una flora polimicrobiana. Las estructuras anatomicas e histologicas de los tejidos periodontales y pulpares presentan interrelaciones, que deben considerarse en la etiopatologia de sus procesos. El diagnostico de las lesiones endoperiodontales no siempre se puede definir claramente, lo cual crea vacios que dificultan el diagnostico de las enfermedades pulpares y periodontales. El odontologo debe estar preparado y en-trenado para que le sea posible definir el diagnostico diferencial de las dos enfermedades y brindar un tratamiento eficiente. Se reporta un caso clinico, donde el manejo del examen clinico, asi como de la anatomia endoperiodontal, son decisivos para llegar al diagnostico correcto y el tratamiento oportuno y adecuado. EnglishPulp and periodontal lesions are the most prevalent in the oral cavity, and are the most frequent causes of dental consultation in our environment and in the world. These pathologies are mainly of bacterial etiology, with a polymicrobial flora. The anatomical and histological structures of the periodontal and pulp tissues present interrelationships that must be taken into account in the etiopathology of these processes. The diagnosis of endoperiodontal lesions is not always clearly defined, which constitutes certain gaps for the diagnosis. The dentist must be trained to perform the differential diagnosis of these two diseases and provide efficient treatment in our patients. A clinical case is reported, where the management of the clinical examination as well as the endoperiodontal anatomy is important to reach the correct diagnosis and its subsequent treatment.

Background Poor oral health in pregnancy can be associated with poor pregnancy outcome, however, dental consultation among pregnant women appears to be low. Methods This was a questionnaire-based study of 413 women who attended the antenatal clinic of University of Nigeria Teaching Hospital (UNTH), Ituku/Ozalla, Enugu. The information obtained was analyzed using SPSS version 22. A p-value of less than 0.05 was considered statistically significant. Results Only 36 (8.7%) of the respondents had dental consultations in index pregnancy for complaints such as tooth ache and decay (66.7%) and pain as well as swelling of the gum (33.3%). The most common reason given for not visiting a dentist during the index pregnancy was the visit not being relevant to their pregnancy outcome (69.2%). After counseling them, only 249 (60.3%) agreed to have dental consultation during subsequent pregnancies. The relationship between visiting the dentist and place of residence ( Conclusion The prevalence of dental consultation during pregnancy is very low. Pregnant women should be encouraged to have routine dental consultation with oral health counseling and check-up incorporated as part of routine antenatal care.

Background: The goal of “Health for All” initiated by WHO envisions securing the health and well-being of people across the globe. SARS-COV-2 coronavirus discovered in December 2019 is said to cause severe pneumonia characterized by fever, cough, shortness of breath and was declared as a global pandemic by WHO. In India the second wave of coronavirus has caused lot of problems with hospitals and medical facilities overwhelmed. WHO and CDC has recommended several biosafety measures and guidelines due to high risk of contamination during dental care. Covid-19 pandemic risk and lockdown measures to curtail the transmission has led to suspension of dental healthcare to the public in many places. Teleconsultation/ Telehealth refers to interactions that happen between a clinician and a patient for the purpose of providing diagnostic or therapeutic advice through electronic means. Teledentistry a subunit of telehealth can be used in routine dental practise to avoid unnecessary exposure of the healthy subjects to the suspects and to decrease the possibility of transmission in dental office. Methodology: A quantitative dichotomous cross-sectional questionnaire survey will be conducted among the rural population. The study population will be chosen randomly among rural population. Only adults will be considered in the survey. Researcher made Questions will be given to the participants in hard copy. The questions will include the Demographic data, Gender, accessibility of dental set up and dentists and awareness of dental tele-consultations. Data obtained will be utilised to assess the accessibility of dental treatment for the public and mode of dental tele consultation preference among rural population. Conclusion: COVID-19 pandemic has caused a lot of concerns regarding the safety during dental procedures. Since the risk of transmission is high due to the very nature of the procedure, assessing the awareness in rural population is crucial. Teledentistry can play a key role in bridging the dental issues and the accessibility of dentists. Proper protocol about its usage with training about it to the dental professionals would be advised. Further steps are required to create awareness about it in the rural population.

Maintaining good oral health is important because oral diseases are related to systemic diseases, and community pharmacies play a key role in maintaining the health of local residents. This study aimed to examine the effects of oral health check-ups and information provision at community pharmacies on oral health-associated behaviors as well as patient satisfaction. We conducted oral health check-ups and provided information about oral health self-care to 84 patients at a community pharmacy, and then asked them to complete a questionnaire survey. One month later, we sent them a follow-up questionnaire and received responses from 66.7% (56/84) of the participants. The large majority were satisfied with the salivary test (95.2%) and the information (96.4%) we provided. Most of the participants (89.3%) indicated that they wanted to use the oral health check-up service again in the future. Compared with baseline, the ratio of participants restricting their intake of sugar-rich foods and drinks significantly increased 1 month later (p = 0.021). About 60% of those who had not undergone a regular dental examination at baseline reported newly visiting or planning to visit a dental clinic. The results revealed high satisfaction with the oral health check-up and information about oral self-care they received at the community pharmacy. The results suggested that oral health check-ups had the potential to change both oral self-care habits and dental consultation behavior. Our findings indicate that community pharmacies can contribute to the maintenance and promotion of oral health by providing oral health check-ups to local residents.

Diabetes mellitus and periodontal disease are among the most frequently occurring conditions that have a substantial effect on the global health economy. The literature regarding medical professionals' knowledge of the bidirectional link between diabetes mellitus and periodontal disease has not been analyzed systematically. The review aimed to investigate the knowledge and understanding of physicians and specialists regarding the two-way relationship between diabetes mellitus and periodontal disease and their approach to referring their patients for a dental consultation. An electronic search of PubMed and Google Scholar databases was conducted to review the studies that assessed knowledge and understanding of medical professionals regarding the relationship between diabetes mellitus and periodontal disease. Data from 13 included studies involved 4,027 participants: 3,256 primary care physicians and 771 medical specialists. Just over 50% of the medical professionals had an understanding of oral health and/or periodontal disease. Over one-third of medical professionals were ignorant of the relationship between oral health and diabetes mellitus. Only 30% reported ever referring their patients for an oral health assessment. Another key finding of the investigation was the absence of interprofessional collaborative care between medical and dental professionals while managing patients with diabetes mellitus. Medical professionals with an integrated knowledge of elementary oral health education and training could play a central role in the timely diagnosis and management of periodontal disease in patients living with diabetes mellitus.

Aims: The aim of the study was to determine the distribution of medical conditions among dental patients in our local environment. Settings and Design: This is a descriptive cross‑sectional study conducted at the Oral Diagnosis (OD) Clinic of Obafemi Awolowo University Teaching Hospital Complex (OAUTHC), Ile‑Ife, Osun State. Materials and Methods: Participants were selected using a simple random method from the pool of dental patients who presented at the OD Clinic of OAUTHC during the study period. Information was obtained from each participant using a structured questionnaire organized into sections. Section 1 collected information on participants’ biodata such as age, sex, and weight. Section 2 recorded data on the history of the presence of relevant oral and systemic symptoms, while section 3 collected information on findings from intraoral and extraoral examination. Statistical Analysis Used: The results were analyzed using STATA 13 statistical software. Results: Of the 1503 patients who visited dental hospital during the study period, 176 (11.7%) had medical problems. The mean age of patients with medical problems was 48.5 ± 21.0 years. The mean age was higher in men compared to women (P = 0.013). More than one‑fifth (21.6%) of the patients were first diagnosed in the dental clinic. Hypertension was the most frequently seen medical condition, closely followed by peptic ulcer and diabetes mellitus. Patients whose medical problems were first diagnosed following dental consultation were significantly older than those with known medical conditions. Conclusions: Prevalence of medical conditions in dental patients is high. Hypertension is the most frequently associated systemic conditions among dental patients. Medical conditions were common among older age group (>50 years), males, and those with gum problems.

Introducción: el consumo de cocaína y cannabis durante el embarazo constituye un problema en ascenso, de importancia para la Salud Pública mundial. Las mujeres consumidoras pueden presentar un amplio rango de complicaciones durante el embarazo y perinatales, pero los conocimientos sobre su evolución posterior son escasos. Objetivos: describir las trayectorias sanitarias durante 4 años de mujeres consumidoras de cocaína y/o cannabis durante el embarazo, y compararlas con mujeres no consumidoras. Métodos: estudio de cohorte retrospectivo con grupo de comparación doble. Las mujeres consumidoras fueron detectadas en el Servicio de Neonatología de un Hospital público con prueba en orina posparto positivo para cocaína y/o cannabis, entre 2009 y 2013. A consultation model for dentistry is presented, with communication skills in a didactic and structural format, applicable for most types of visits, patients and problems. A characteristic of the dental visit is its division into dialogue phases and clinical phases, which makes verbal exchange especially challenging. The original Four Habits Model (4H) has been adapted from medicine to the specific structure and content of a dental visit, and a modified model is proposed. The dental model consists of a structural core framework relevant for all dental consultations, and additional extensions applicable to commonly occurring issues. Facilitate Perceived Control has been added to the model, due to its crucial importance in dental visits, and we suggest naming the model "The Four + One Habits Model for Dental Visits (4 + 1HD)." The proposed model should have clinical relevance for improving patient care and patient experience, as well as easing the busy everyday life for dentists in a variety of clinical settings. It is also likely to be a flexible framework for communication skills training at all levels of dental education. More research is needed to validate and test the model in various clinical and educational settings.

The inclusion of the maxillary incisors, although rare, poses even more difficulty to the practitioner who diagnoses it. This difficulty is increased in front of an angulated incisor in a labial position with the “policeman's sign”, the prognosis of orthodontic therapy being reserved.This article aims to describe a clinical case of retention of two maxillary central incisors with radicular angulation and in a vestibular position in an 11-year-old adolescent, referred in the Rabat dental consultation and treatment center. clinically, we can note the absence of the two maxillary central incisors on the dental arch, confirmed by the X-ray with the presence of obstacles on the path of the incisors which are in a very high position with angulated roots. The challenge was to put on the arch of the teeth and especially the aesthetic challenge. The satisfactory result obtained is only the result of the motivation of the patient as well as the dexterity of the practitioner. Despite the current treatment, we are comforted in our choice to treat. it is obvious for us that the only absolute contraindication to traction in the case of an impacted incisor remains dilaceration

Currently, patients destined for total edentulous no longer accept to show their infirmity by remaining edentulous for a longer or shorter period of time. When the anterior area is affected by dental extractions, the problem increases and the immediate prosthetic restoration is required. It aims, on the one hand, to meet the esthetic and functional requirements and, on the other hand, to ensure the biological and psychological integration of the prosthesis. We propose, through a clinical case treated in the prosthetic department of the Dental Consultation and Treatment Center of Casablanca, to present the different clinical steps as well as the difficulties to be observed during the prosthetic rehabilitation.

The biological assessment is a complementary examination, which makes it possible to confirm or to cancel the presence of disorder suspected during the clinical examination. It has an important role and is a necessary step towards to better care. The aim of this study is to determine the biological reports prescribed and consulted by the pedodontic service and to evaluate the practitioners’ knowledge of biological reports as well as their training needs. The descriptive coss-sectional epidemiological survey was conducted on 35 practitioners. Our population is made up of all residents, specialists and teachers at the Pedodontics Department in Dental Consultation and Treatment Centre in Casablanca. The survey was conducted over a two-month period, from November 2018 to December 2018. The data collection was carried out through a questionnaire. 100% of practitioners prescribed a biological assessment for children, 62,9% of the practitioners surveyed have not received any continuing training in biological assessment, 100% of the practitioners need training to develop their knowledge about biological assessment. In the light of the results, it appears that knowledge of the principle of these tests is therefore essential to prescribe them in a relevant manner and to be able to interpret the results in collaboration with the attending physician. This allows better planning of potentially risky oral health care.

Rickets is a disease that affects the growth of a child or individual because of defect in bone mineralization. There is utmost requirement of vitamin D for the proper bone formation and mineralization and any defect in its production and metabolism will lead to development of rickets. Though there is abundance of sunlight, vitamin D deficiency now emerges as a major health problem in the South East Asian population including India. A timely dental consultation and management is critical for prevention of significant oral findings in the affected individuals. Multiple dental abscesses in absence of caries or trauma is one of the most significant dental findings in Rickets. The goal of early intervention in the treatment of rickets is to minimize the late complications of rickets in affected children. The aim of this case report is to discuss about various oral findings of 17 years old boy suffering from rickets and to raise awareness on dental characteristics of this disorder.

Abstract: Introduction: Cone-beam computed tomography is a frequently used diagnostical method in the head and neck region. The thickening of the maxillary sinus mucosa is often observed in these images. Aim: Determining the prevalence of teeth and roots that can be identified as cause of maxillary odontogenic sinusitis, and recording the average observed mucosa thickening on these images. Method: The scans that give the base of the study have been performed by the Department of Dentistry of Medical University of Pécs between 2015–2016. The size of the voxels had been varied between 0.25 and 0.4 mm and the size of the image had been set up to 15×12 cm. The acquisition time had been set up to 27 seconds. Results: 170 cases of the 260 revised records fulfilled the initial criterion conditions. The average mucosa thickness was 8.8 mm. During the present study, the upper first molars palatal and the second molars mesiobuccal roots were mainly associated with maxillary odontogenic sinusitis. Conclusion: Whether in the case of chronic maxillary sinusitis that is not or only temporarily responsive to conventional therapy, may it be justifiable to provide dental consultation and cone-beam computed tomography to exclude the dental origin of sinusitis. The number of people living with HIV is gradually increasing and many of them desire the dental consultation same as ordinarily people without HIV. Therefore, we must prepare the medical system they can easily refer to dental office as soon as possible we can. In this report, we took a questionnaire on AIDS and/or HIV infection from 1,092 candidates living in Kitakantou and Koushinetsu district. The male to female ratio was set to 1:1, with age between 20 and 79 (mean 48.7 ± 15.4) years. They consist of 152 candidates in each prefecture, and no significant differences of age distribution were found among the prefectures. The young generation of the candidate had an experience of having a lecture about AIDS/HIV in their school days: thus, the validity of their knowledge regarding AIDS/HIV was found to be significantly much more abundant than that of other generations. Conversely, there are many candidates in each generation who don't know the HIV infection route or the disease natural course enough. More than 50% of the people, especially over 6th decade, answered dental practices are one of HIV infection routes, but most candidates in 3rd decade or students were negative for this answer. It is not an important factor that is prepared to take a patient with HIV in or not for approximately 80% of candidates to select their dental office. Therefore, general imaging of AIDS/HIV and the behavior of the dental consultant may have a slight disparity. AIDS/HIV-related misconceptions may lead to negative behavior toward HIV infected patients, and provision of the necessary education can lead to not only the right knowledge but also beneficial behavior modification., エイズ/HIV感染者は徐々に増加しているが，多くは健常者と同じように歯科治療を受けることを望んでいる。よって，エイズ/HIV感染者が歯科医院を受診できる医療体制整備が求められている。そこで今回，北関東甲信越に住む1,092人を対象としたエイズ/HIVに関するアンケートを行った。回答者の男女比は1:1に設定し，年齢は20-79歳(平均48.7±15.4歳)であった。対象とした7県別ではそれぞれ152人ずつであり，各県ごとの男女比や年齢層の分布に有意差はみられなかった。若年齢層ではエイズ/HIVに関する授業を受けた経験がある人が多く，知識においても有意に正答率が高かった。一方HIVの感染経路や感染後の経過に関しては，いずれの年齢層においても正しく知らない人が多かった。歯科治療を介してHIV感染しうると考えている人は全体の半数以上にみられ，とくに50歳以上にその傾向が強かったが，20歳代や学生では感染しないとする人が多かった。また自身が受診する歯科医院を選ぶ基準としてHIV感染者の診察の有無は約8割の人が「わからない，意識しない」を選んでおり，一般的なエイズ/HIVに対するイメージと歯科受診という行動との間には若干の隔たりがあると思われた。誤った知識はエイズ/HIV感染者への否定的な態度につながりかねないが，今後は知識だけでなく行動変容を生じうる教育が必要であると考えられる。

Background Denture use for recovery of major tooth loss can reduce the need for care, and thus, it is important for dentists to identify the types of patients who use removable dentures. Objective To identify factors associated with removable denture use in independent older Japanese people. Methods This cross-sectional study investigated the oral condition; demographics; lifestyle; dental visit behaviour; and higher-level functional capacity, as assessed using the Tokyo Metropolitan Institute of Gerontology index of competence (TMIG index), of independent older people aged 65-74 years. Participants were divided into four groups based on sex and condition of the posterior occluding pairs (POPs). Multivariate-adjusted logistic regression analyses were performed for each group, with denture use or non-use as the dependent variable and the other items as independent variables. Results Denture use or non-use was significantly associated with only dental consultation behaviour (regular dental visits and time since the last dental visit) in all groups. Subsequent analyses excluding dental consultation behaviour demonstrated that denture use or non-use was significantly associated with educational background in female participants with decreased POPs, with the type of household in male participants with lost POPs and with the TMIG index (social role) in female participants with lost POPs. Conclusions Denture use was associated with dental consultation behaviour. We revealed factors related to denture use or non-use in females with decreased POPs and both males and females with lost POPs.

Objective: To measure the heart rate (HR) and the behavior of children and teenagers with Down Syndrome (DS) during the dental appointment. Material and Methods: Two groups (n = 52), of both genders, aged 2-14 years, matched by age group were formed: study group (SG) - individuals with DS and control group (CG) - normotypical school children. The participants were submitted to clinical examination and prophylaxis. An oximeter was used to measure the HR at five moments of the dental consultation: before entering the practice room (T0), when sitting in the dental chair (T1), during the clinical examination (T2), during prophylaxis (T3) and immediately after prophylaxis (T4). Behavior, classified according to the Frankl Scale, was observed at T3. Mann Whitney, Kruskal-Wallis, Dunn and Pearson's Chi-square tests were used to analyze and compare variables (significance level at 5%) . Background In the dental clinic impacted teeth are frequent findings, especially upper and lower third molars, leading to their exodontia. Among surgical techniques piezosurgery is advantageous for delicate structures in the oral cavity. Extracted teeth, usually discarded, have been revalued as biological material, providing living tissues with possible applications in regenerative dentistry. The aim was to compare cross-section methods of upper included third molars by ultrasonic piezoelectric technique to obtain dental pulp, with diamond-coated tip (DT) against titanium nitride-coated tip (TN), according to the pulp tissue cell viability and the section surface characteristics. Material and methods Patients attending dental consultation were evaluated. Upper third molars (n= 24) were avulsed from 15 patients with exodontia indication, age 18-26 years old, who agreed to participate of the study. Third molars were cross-sectioned at amelocemental junction level with piezoelectric device using DT or TN inserts. Pulps were mechanic and enzymatically treated, and tissue viability determined by Trypan Blue test. Sectioned teeth were visualized using Scanning Electron Microscope (SEM). Ethical principles of biomedical research were respected; all patients gave their informed consent. Results Viability of pulp tissue was 84.71% not associated to sex (p= 0.141) nor to teeth position, upper right third molar or upper left third molar (p= 0.580). According to the insert used, pulp tissue viability was 85.21% with TN, similar to 84.00% with DT (p= 0.611). By SEM, cut performed by TN insert showed smooth and uniform surfaces, while DT insert surfaces were irregular, porous, with fissures. Conclusions Piezosurgery applied to cross-section upper third molars with both types of inserts showed differences in the cut surfaces but similar effectiveness regarding preservation of pulp tissue with high viability, thus, they could be allocated for further cellular developments. Objective To investigate the association between dental consultation and oral health status among male Japanese employees. Methods The participants were 3351 male employees who received a workplace oral health examination conducted at the ages of 35, 40, 45, 50, 55, and 59 years before retirement in conjunction with an annual health checkup. Data on dental expenditures were collected from health insurance claims. The number of dental visits and dental care expenses, alone or in combination, were used as indices of the dental consultation status for the analyses. The effects of dental consultation status on oral health status (number of total teeth, number of decayed teeth, and periodontal status) were analyzed using multivariate multinomial logistic regression analyses adjusted for confounders. Results Multivariate analyses revealed that the odds ratio (OR) for 20‐27 teeth (losing 1‐8 teeth) was significantly higher (OR 1.4, 95% confidence interval (CI) 1.1‐1.7) in those who had a high number of dental visits and high dental care expenses than in those who did not have a dental visit. By contrast, the ORs for ≤19 teeth (losing ≥9 teeth), having ≥3 decayed teeth, or having a periodontal pocket ≥6 mm were significantly lower (OR 0.2, 95% CI 0.1‐0.6; OR 0.5, 95% CI 0.3‐0.6; OR 0.7, 95% CI 0.5‐1.0, respectively) in those who had fewer dental visits and lower dental care expenses. Conclusions These results imply that the dental consultation status is associated with oral health status among male employees.

Resumo Este estudo teve por objetivo medir a prevalência e identificar fatores associados à não utilização de assistência odontológica entre gestantes residentes no município de Rio Grande, RS. Em 2013, entrevistadores previamente treinados aplicaram questionário único, padronizado em até 48 horas após o parto à todas puérperas residentes neste município. Utilizou-se teste do qui-quadrado para comparar proporções e, da análise multivariável, regressão de Poisson com ajuste robusto da variância para a obtenção da razão de prevalências. Dentre 2.653 puérperas incluídas neste estudo, 60,1% (IC95%: 58,2% – 61,9%) não utilizaram qualquer tipo de serviço odontológico durante a gestação. Após ajuste, a probabilidade de não uso destes serviços foi significativamente maior entre gestantes de menor idade, renda e escolaridade, que viviam com maior número de pessoas no domicílio, que realizaram um menor número de consultas de pré-natal, que fizeram pré-natal no serviço público, e que não foram atendidas pela Estratégia Saúde da Família (ESF). Incentivar gestantes com este perfil epidemiológico a procurar por serviços odontológicos durante as consultas de pré-natal e ampliar a oferta da ESF são medidas com grande potencial de elevar a cobertura para este tipo de serviço na localidade estudada.

The research project is funded by the Scottish Government-Childsmile Programme. Background There is a paucity of research concerning paediatric dental consultations in primary care. This is potentially due to the difficulty of measuring the communication behaviours in the complex triadic consultations. The present study aims to describe the development and refinement of a coding scheme to record the triadic communication between dental professionals, child patients and parents. Methods The PaeD-TrICS was developed from video observation of triadic communications and refined through an iterative process. Its practical applicability was assessed via implementation of the scheme on specialised behavioural coding software. Reliability was calculated using Cohen’s Kappa. Results The PaeD-TrICS contains 45 codes. Forty-four dental professional-child-parent communications were successfully coded through administering the scheme on The Observer XT 10.5 system. Cohen’s Kappa was 0.83 (inter-coder) and 0.90 (intra-coder). “Parental verbal facilitation” (mean = 1.68/min) was the most frequent behaviour. Dental professionals’ “dentally engaging talk” (mean = 1.24/min), “praise” (mean = 1.10/min) and “instruction” (mean = 0.62/min) were frequently seen. Children’s common behaviours included “speech other” (mean = 0.66/min) and non-verbal behaviour i.e. “non-verbal agreement” and verbal behaviour “speech yes” (mean = 0.26/min). Conclusions The PaeD-TrICS is developed to capture the communication behaviour of the triadic consultations in a preventive dental setting. It demonstrates satisfactory intra- and inter-coder reliability and has been successfully used in paediatric dental consultations. Publisher PDF

Adopting a patient-centred approach when engaging with patients has been linked to positive outcomes in medicine. However, in the field of dentistry, patient-centred care (PCC) has been understudied. The aim of this study was to explore dentists' opinions about the usefulness of a theoretical model of PCC and its applicability to a dental setting. Twenty semi-structured interviews were conducted with dentists working at a major university hospital in south London. The interviews were audiotaped and analysed using thematic analysis. The analysis showed that dentists found the PCC model to be useful for practical application but mostly for 'other' dentists. They viewed the lowest and highest levels of PCC that the model advocates as 'out-dated' and 'ideal', respectively. Dentists felt that giving patients full choice and control over their treatment was not applicable to everyday clinical practice. They expressed that a tool developed on the basis of this hierarchy of PCC could be useful for self-assessment purposes, although with some reservations. In conclusion, dentists seemed to embrace the practice of PCC and felt that their provision of PCC was at an acceptable standard. They believed that a moderate level of information and choice was most suitable for patients.

The aim of this work was to perform a cephalometric analysis of the craniofacial parameters and natural head posture of mouth-breathers compared with control subjects, and to study the relationship between nose-breathing and the vertical dimension.The headfilms taken at start of treatment of 53 cases (28 girls, 25 boys) aged 9 to 30 who consulted the dento-facial orthopedic department of the Ibn Rochd Casablanca dental consultation and treatment center were studied.Among the mouth-breathers, we noted mandibular retrusion (SNB) in association with posterior rotation and more pronounced tilt of the mandibular plane (PP-MP) compared with the controls (P0.05), a disproportionate increase in anterior face height and a lessening of posterior face height (hyperdivergence). These increases in anterior face height are often associated with retrognathism (and open bite).Our study has shown that there are cephalometric differences between mouth-breathers and nose-breathers. There are several studies in the literature with results that support ours. Collaboration between the pediatric dentist, the orthodontist and the ENT specialist is important so as to establish an early diagnosis of mouth-breathing in children and initiate appropriate treatment to recreate the best conditions for harmonious development.

Background: In current dental consultations, collaborations and treatment plans may not always be based on the best practice of shared decision making, but rather rest on the traditional paternalistic style of decision making. Aim: The planned organisational development project aims to improve shared decision making by introducing the ‘talk’ model into dental consultation. Rationale: The paternalistic style of decision making can lead to dissatisfaction, compromised consent, and poor patient-centred care. The literature identified shared decision making as central to patient engagement, effective communication and quality of care. Change Process Plan: The Health Service Executive Change Model was utilized as a framework for the systematic planning and future implementation of the planned project. The four stages included are: initiation, planning, implementation, and mainstreaming. This change model is coupled with a consideration of risk management, a supported financial case, a communication plan and a project management plan, so as to ensure a thorough foundation for successful change. Evaluation: Project evaluation included a mixed method approach of informal interviews, questionnaires, and observations. Kirkpatrick’s model of evaluation was employed to evaluate educational aspects of the project. Conclusion: Leading the planning for an organisational development project in healthcare is complex and challenging. The proposed implementation and evaluation of this planned project will enhance the dentist-patient relationship. For the project plan to be successful, it is important to understand the organisation’s vision, culture, and its stakeholders. Finally reflecting and learning strengths and limitations, both at an organisational and individual level are crucial.

Introduction : This study analyzed dental consultation requests to the division of oral and maxillofacial surgery in a Brazilian tertiary hospital. Methods : A cross-sectional study with data collected retrospectively from inpatients’ electronic medical records containing dental consultation requests made between January 2013 and December 2017. Results : 327 consultation requests were analyzed. Mean (SD) patient age was 38.71 (24.4) years; 164 (50.2%) were male and 267 (81.7%) were Caucasian. Regarding systemic conditions, 34 (10.4%) were classified as ASA I, 86 (26.3%) as ASA II, 182 (55.7%) as ASA III, and 25 (7.6%) as ASA IV. Dental consultations were mostly requested by the internal medicine team (n = 42, 12.8%). The most common reason for consultation was septic teeth (n = 131, 40.1%). Complementary tests were required in 188 (57.5%) cases. Surgical intervention was required in 82 (25.0%), with tooth extraction as the most prevalent procedure (20.2%). The most demanding service was inpatient care, with 276 (84.4%) requests. Cases were resolved in 249 (76.1%). Conclusions : The division of oral and maxillofacial surgery in our hospital deals with a great amount of consultations, contributing with surgical procedures to the adequacy of patients’ oral health with a high effectiveness rate. Our data illustrate the contribution of dentists in a hospital setting, assisting the medical team in providing comprehensive care for inpatients. The purpose of this series of case reports was to evaluate the anxiety generated in pediatric patients submitted to two mechanical techniques of cavity preparation: the conventional rotary instruments (i.e. high and low speed) using diamond tips and ultrasonic abrasion using CVD™ system tips. To this end, 04 patients aged 06-09 years had at least two deciduous molars with occlusal caries were selected for this study. The teeth were randomly divided into two experimental groups CVD (CVD system with cavity preparation) and CRI group (cavity preparation with rotary instruments). The preparation procedures were performed under local anesthesia and rubber dam and cavities restored with composite resin. At the beginning and end of each visit, a closed questionnaire which aimed to assess the child's anxiety during the visit in which each preparation technique was used was applied. The results were submitted to descriptive statistical analysis. According to the results it was found that dental consultation and local anesthesia were of concern in all patients. The two preparation techniques resulted in similar patterns of anxiety both before and after use. Within the limitations of this study, it can be concluded that all the factors involved in dental appointment contribute to make it a cause for concern among pediatric patients. Objetivo: o objetivo desta série de relatos de caso foi avaliar a ansiedade em pacientes pediátricos submetidos a duas técnicas mecânicas de preparo cavitário: instrumentos rotatórios convencionais (alta e baixa velocidade) com ponta de diamante e abrasão ultra-sônica com sistema CVD ™. dicas. Material e Métodos: quatro pacientes, com idades entre 06 e 09 anos, que tinham pelo menos dois molares decíduos com superfície oclusal de cárie foram selecionados para este estudo. Os dentes foram divididos aleatoriamente em dois grupos experimentais: CVD (sistema CVD com preparo cavitário) e grupo CRI (preparo cavitário com instrumentos rotatórios). Os procedimentos de preparo foram realizados sob anestesia local e o dique de borracha e as cavidades foram restauradas com resina composta. No início e no final de cada visita, aplicou-se um questionário fechado que objetivou avaliar a ansiedade da criança durante a visita, na qual cada técnica de preparo foi utilizada. Os resultados foram submetidos à análise estatística descritiva. Resultados: comportamento e ansiedade foram avaliados em 04 dentes de cada grupo (dois de cada criança). A ansiedade moderada / grave foi observada em todos os pacientes, independentemente da técnica utilizada para a remoção da cárie. Independentemente da intervenção testada na maioria dos casos, as crianças tratadas ficaram ansiosas antes das consultas. Conclusão: considerando as limitações deste estudo preliminar, pode-se observar que o tratamento odontológico e seus eventos relacionados, independentemente das técnicas mecânicas de preparo cavitário utilizadas, podem estar relacionados à ansiedade do paciente pediátrico.

Currently, music is a part the list of nonpharmacological alternative therapies. Literature is reviewed to see the effects of music therapy on human health in general, and oral health in particular. The main effect is the reduction of anxiety and stress in patients who, taking different conventional treatments, received music as therapy. The benefits on children are described, prior to dental consultation and in orthodontic treatments. Positive results were also registered on cancer patients undergoing chemotherapy or palliative treatments, children with Specific Language Disorders and people who experience anxiety, depression and somatization.

BACKGROUND Dental injury as a result of oroendotracheal intubation during general anesthesia is very common. We report our experiences of using mouthguard to prevent dental injury during intubation based on our protocol. METHODS This retrospective study enrolled patients referred for preanesthetic evaluation, those patients with a history of any of the dental treatments to their anterior teeth listed on our fabrication protocol from January 1, 2009 to June 30, 2010. RESULTS No cases of dental trauma during oroendotracheal intubation were reported among the 202 patients who used a protective device. 66% of the patients had risk factors for hard tissue damage aged 10-40 years. At the ages of 40-70 years, the incidence of risk group for periodontal damage was higher. CONCLUSIONS Preanesthetic consultation was effective for preventing dental injury, so preanesthetic questionnaire and proper dental consultation would be helpful.

International audience; For every patient, nowadays, dentists use a software to generate the dental scheme. The dental scheme is basically a diagram representing the whole dentition of the patient. On this diagram, each tooth is represented along with the various operations the patient underwent. The dental scheme for instance shows whether some teeth are missing, or if any treatment was ever performed on the dental roots, it also represents the dental fillings, removable prosthesis, dental crowns or tooth bridges. Filling up the dental scheme may be tedious for dentists, as for every new patient, they would have to carefully make an inventory of every dental care the patient underwent. In this work, we intend to study the feasibility of automatically generating the dental scheme from radiographs. Indeed, we aim to propose an image processing method that would automatically detect missing teeth, as well as any dental care in the dentition, this may save a significant amount of time during the dental consultation. In a first step, our method extracts the relevant portion of the scanner image, i.e. we automatically crop the dentition and thus remove the jaws and chin. The bending of the jaw (dentition curvature) is also estimated, and allows to distinguish the upper and lower jaws. A local minimum/maximum computation coupled with the Hough transform, and a fit with Gaussian Mixture Models helps us to segment the teeth despite strong luminance irregularities due to the imaged spine.

Oral diseases are common in school children of underprivileged region of developing countries. The community-based oral health promotion approach, using schools as a platform, is a way to tackle the oral health problems of community with limited human and financial resources. A base-line study was conducted in a school of Jyamrung so as to evaluate oral health related knowledge, attitude and practices among schoolchildren, based on which an oral health promotion program will be developed for the school. A descriptive study was conducted among all the students of the school. Oral heath related information of the students was collected using DMFT (Decayed Missing Filled Teeth) index and a questionnaire. A total of 88 children with age ranging from 6 to 16 years took part in the study. A total of 61% of the children had dental caries. Mean DMFT of the schoolchildren was 1.78 (SD 2.04). The Filled component score was 0. Children with the experience of tooth pain the previous year was 36.5% for 6-11 years of age, and 58.3% for 12-16 years of age. None of them had received any form of treatment following the pain. Only 4 children have dental consultation. Almost 92% of them have never received oral health education in their school. A comprehensive oral health care program is required to address the present oral health needs of the schoolchildren.
