Background Odontogenic keratocysts (OKC) are cystic lesions appearing in the jaws, usually asymptomatic with a progressive growth into the bone. Many of them are diagnosed by a routine radiological examination. Material and methods This study reports a 12-year-old girl that presented an asymptomatic large radiolucent unilocular lesion associated to the crown of 3.8 that caused displacement of the molar and the inferior alveolar canal. Differential diagnosis included OKC, unicystic ameloblastoma, ameloblastic fibroma, dentigerous cyst and orthokeratinized odontogenic cyst. Two surgical interventions were performed; first, a marsupialization, and 10 months after, the third molar extraction plus cyst enucleation, mucosa excision and the application of Carnoy's solution. Results The anatomopathological exam confirmed diagnosis of OKC. There was no evidence of recurrence after 2 years of follow-up. Conclusions Marsupialization followed by surgical enucleation with mucosa excision and Carnoy's solution can help manage treatment of OKC, a lesion characterized by an aggressive behavior. We read with great interest the comments written by Dr. Shetty and colleagues, and firstly we would like to thank them for their interest in our article. The removal of the mandibular third molars is the most common surgical procedure in dentistry. It is also considered a standardized surgical operation and, indeed, a suitable model for investigating the safety and efficacy of anesthetic solutions.

Background A literature review was made to determine when third molar (3M) extraction is recommended in symptomatic patients and when it is not recommended. Material and methods A Medline (PubMed) and EMBASE search was made for articles related to indications for the extraction of 3Ms, published in the last 10 years and up until September 2018. Results The electronic search yielded 175 articles. After eliminating duplicates, a total of 173 articles were subjected to review of the title and abstract. Only 19 studies were finally included in the systematic review. There was a well documented increase in morbidity associated to impacted 3Ms (non-restorable caries, fracture, infection, periodontal disease, repeated pericoronitis, cysts and tumors), and in the presence of disease, extraction was considered to be indicated. The extraction of 3Ms with signs and/or symptoms of periodontal disease improved periodontal health at the distal surface of the second molar. Postoperative quality of life of patients with symptomatic 3Ms and with disease improved after surgical extraction. Conclusions Extraction is indicated in the presence of disease associated to an impacted 3M, whether symptomatic or not. In contrast, extraction is not indicated in the absence of infection or other associated disease conditions. This study investigated the possibility of re-treating a calcium silicate-based sealer (CSBS), compared to an epoxy-resin sealer (RBS), using rotary instrumentation at different times from obturation (1 month/1 year). Thirty-six human mandibular premolars, extracted as a result of orthodontic or periodontal problems, were instrumented and randomly divided into three groups of 12: BR and BR*, which were filled with CSBS and re-treated after one month and one year of storage, respectively, and AH, which was filled with RBS and re-treated after one month. The same re-treatment protocol was used for all teeth, and the times required for the procedure was recorded. The re-treated specimens were longitudinally sectioned and examined at the stereomicroscope (SM) at 20× magnification. Image J Software was used to process the microphotographs. The percentage of residual filling materials in the root canal and the apical third, the ability to reach working length WL and patency, and the time taken to complete the re-treatment were recorded and analyzed by ANOVA and post hoc Bonferroni test (p = 0.05). Scanning electron microscopy (SEM) and coupled energy-dispersive spectroscopy (EDS) were applied to representative samples to evaluate canal cleanliness and chemical elements. Patency and WL were re-established in all of the teeth. Residual filling materials were retained in all specimens of the three groups. The mean percentage of residual materials was significantly different between BR and BR* (p-value = 0.048), with BR* showing the highest values. The mean time to complete re-treatment was significantly lower for AH, followed by BR (p = 0.0001) and BR* (p = 0.0078). Conclusions: After both medium and long storage periods, the CSBS can be concluded to have been successfully removed from canals with simple anatomy.

Background: The authors aimed to assess whether 4% articaine is a safe and effective local anesthetic (LA) for mandibular third-molar extractions. Types of studies reviewed: The authors searched MEDLINE (PubMed), Cochrane Library, Scopus, and Web of Science databases to identify randomized clinical trials that fulfilled the eligibility criteria. Risk of bias was evaluated using the Cochrane risk-of-bias assessment tool. The authors performed a meta-analysis of safety and efficacy variables comparing 4% articaine with different LAs. Results: The authors assessed 482 articles but only 14 randomized clinical trials met the inclusion criteria for review. No statistically significant differences were found among the selected LAs regarding safety. Four percent articaine required fewer reinjections than 2% lidocaine and had a shorter onset time than 2% lidocaine, 0.5% bupivacaine, and 4% lidocaine. Four percent articaine had a longer anesthesia effect than 2% lidocaine and 2% mepivacaine, but a shorter anesthesia effect than 0.5% bupivacaine. Practical implications: Use of 4% articaine for mandibular third-molar extraction is a safe choice that requires fewer reinjections and has a shorter onset time than other aminoamide-type LAs.

Background. A solitary bone cyst or simple bone cyst is a nonneoplastic osseous lesion, with no epithelial lining, also considered as a pseudocyst. These lesions, with an intact bony wall and fluid-filled, are frequently discovered by chance in radiological studies. The etiopathogenesis has not been studied in depth, and the management remains controversial. Case Presentation. We present a clinical case of a 15-year-old boy who underwent an orthopantomography to assess the development and position of the third molars during a routine postorthodontic check-up. By chance, the X-ray identified an asymptomatic radiolucent image in the left jaw, measuring 12.0 mm×17.8 mm and compatible with a solitary bone cyst involving teeth 35 and 36. We describe our technique for performing minimally invasive decompression of the lesion using a microperforated catheter. We describe the entire course of the follow-up, both clinical and radiological, until complete cure. Conclusions. This straightforward continuous decompression technique poses no problems for the patient, has a low risk of sequelae, and is clearly cost-effective. In view of the highly satisfactory evolution, whenever possible, we favor this minimally invasive technique for the treatment of solitary bone cysts in the jaw.

Background Little is known about the effect of non-nutritive sucking habits (pacifier and digital sucking) in the prevalence of molar Class in mixed dentition. The aim of this study was determinate the relation between non-nutritive sucking habits, and Angle´s molar Class, in the horizontal plane, and it´s relation with gender. A convenience sample of 326 children with ages between 6 and 12 years was selected from three schools of Oporto. Material and methods To collect the epidemiologic data, was used a method recommended by the WHO. An indirect questionnaire about the medical history, dental habits, was used. It was adapted from Sanchez-Molins and validated by Clinical Dental III of Integrated Dental University Institute Health Sciences, Gandra, Portugal. Results In this study, 326 infants were examined in order to determine the prevalence of non-nutritive sucking habits. Only 45 observed children did not mentioned any kind of non-nutritive sucking habit; the remaining 281 children mentioned at least one potential bad habit. Children with non-nutritive sucking habits show a higher molar Class II percentage in females, while molar Class III is more frequent among males compared with children with no sucking habits. Conclusions Children with non-nutritive sucking habits, presented a higher-Class II prevalence with statistically significance. It was detected a direct relationship between Angle´s molar Class and gender. Key words:Finger sucking, pacifier sucking, Angle Class malocclusion.

Background Dentigerous cyst appears surrounding the crown of an included tooth. On the radiographic exam, a radiolucent rounded well-defined lesion can be observed. Material and methods This study reports a clinical case of a 34-years old man with a pericoronal radiolucent lesion associated to an impacted lower third molar with the presence of radiopaque material inside. The radiological differential diagnosis was calcifying odontogenic tumor, adenomatoid odontogenic tumor and dentigerous cyst. The impacted third molar was removed and the lesion was sent for the histopathological exam. Results The histopathological diagnosis was dentigerous cyst with capsular calcifications. Specifically, parietal calcifications on its connective wall and a piece of cemento-osseous tissue inside. Conclusions Non-neoplastic lesions such as dentigerous cysts could develop radiopacities inside the radiolucent pericoronal area. Objectives: The understanding and adherence to postoperative care instructions are factors that influence the recuperation process after any surgical procedure. The aim of this study was to determine the percentage of patients who strictly follow the postoperative instructions after the extraction of an impacted lower third molar in relation to sociocultural level, preoperative anxiety scores and how postoperative information is provided to the patient. Study Design: Patients were randomly assigned to one of three different test groups according to how the postoperative instructions were presented: verbal, written and a group that received additional information. Before surgery, patients were required to complete the Corah Dental Anxiety Scale and personal information (age, gender and educational level) was also collected. P 0.05). Quitting smoking or drinking of alcoholic/carbonated beverages were the main influential factors for the lack of adherence to postoperative care instructions during the week after surgery. Conclusions: Presentation of postoperative instructions, preoperative anxiety scores and sociocultural level do not appear to be key factors that promote the adherence to postoperative instructions.

Background The objective of this study was to evaluate the association between the presence of mandibular third molars and the occurrence of carious lesions in the distal aspect of the mandibular second molar. Material and methods A retrospective cohort study comprising 327 lower third molars extracted in the Oral Surgery and Implantology Master's Degree program of the School of Dentistry of the University of Barcelona (Barcelona, Spain) was carried out. A descriptive and bivariate analysis was made. The diagnosis of caries in the second molar and the position of the mandibular third molar were evaluated through panoramic radiographies. Results The sample included 203 patients, 94 males (46.3%) and 109 females (53.7%), with a mean age of 26,8 years and 327 lower third molars. The prevalence of second molar distal caries was 25.4% (95% CI= 20.6% to 30.2%). This pathology was significantly more frequent when the third molar was in a horizontal position (27.7%), when the contact point was at (45,8%) or below (47.0%) the cementoenamel junction (CEJ), and when the distal CEJ of the mandibular second molar and the mesial CEJ of the third molar was 7 to 12 mm apart. Conclusions Horizontal lower third molars with contact points at or below the CEJ are more likely to produce distal caries in the mandibular second molars. Due to the high prevalence of this pathology (20.6% to 30.2%), a prophylactic removal of lower third molars with the above-mentioned features might be advisable. [spa] La literatura científica permite estimar que se extrae entre un 18 y un 40 % de terceros molares (3M) asintomáticos. La justificación se basa en la necesidad de minimizar el riesgo futuro de patología asociada al diente y reducir la morbilidad quirúrgica asociada a la edad. Aunque existen algunas publicaciones que comparan las indicaciones de extracción entre diversos clínicos, hay muy poca información sobre cuáles son los factores, tanto del paciente como del dentista, que influyen en la decisión de extraer o no un 3M asintomático. El objetivo del presente estudio es valorar el grado de dificultad de la extracción del 3M y determinar la proporción de las principales causas de indicación o de no indicación de la extracción en función del clínico y de las características clínico-radiológicas del paciente y del 3M. De este modo se podrá actuar sobre las creencias de los odontólogos y la aplicación de Guías de Práctica Clínica será más eficaz. Se envió por correo electrónico a todos los dentistas portugueses y españoles una solicitud para rellenar una encuesta con 15 casos clínicos que iban documentados con ortopantomografías e información clínica del paciente y de los 3M inferiores. Para cada uno el clínico tenía que valorar el grado de dificultad de la extracción, si la recomendaba o no y el motivo. Los resultados fueron analisados estadísticamente con el software IBM SPSS® 22.0 software (IBM Corp., New York, USA). Fueron incluidos en el estudio 381 clínicos. La mayoría de los profesionales tenían más de 13 años de experiencia clínica. En España el número de postgraduados en Cirugía Bucal así como de clínicos con dedicación diaria a esta área fue significativamente mayor. Un promedio del 42% de los encuestados optaron por recomendar la extracción de 3M asintomáticos. La indicación de extracción fue significativamente mayor en dentistas portugueses. La experiencia clínica se correlacionó negativamente con la dificultad percibida de la extracción. Se percibió como significativamente más difícil la extracción de 3M en pacientes varones y menores de 26 años. Asimismo, los clínicos han antecipado como más difíciles los 3M con mayor recubrimiento de tejidos, con posiciones distintas de la vertical, en inclusiones más profundas o con menor espacio mesio-distal entre el 2M y la rama ascendente de la mandíbula. La curvatura radicular o la proximidad al nervio dentario inferior también producieron un aumento de la percepción de la dificultad de la cirugía de los 3M. Las principales indicaciones para recomendar la extracción de los 3M asintomáticos, por orden de frecuencia fueron: el riesgo de periconaritis, el riesgo de caries en la cara distal del 2M, la falta de función del diente y el riesgo de reabsorción y de lesión del 2M permanente. En 3M mesioangulados, horizontales, profundos o con poco espacio mesio-distal la posible afectación del 2M fue la principal preocupación de los encuestados. Con el aumento de profundidad de inclusión aumentó la preocupación por la reabsorción del 2M y la posible lesión periodontal del 2M. La caries en el 2M y la pericoronaritis fueron las principales motivaciones para la extracción de 3M asintomáticos en pacientes mayores de 26 años de edad, dientes parcialmente erupcionados y con espacio mesio-distal suficiente para poder erupcionar. Los principales motivos para contraindicar la extracción de 3M inferiores asintomáticos fueron la ausencia de un motivo para la extracción y la posible lesión del nervio dentario inferior, principalmente en pacientes de menos de 26 años y en inclusiones más profundas., [eng] Reports in the scientific literature indicate that around 18% to 40% of 3Ms are extracted without any pathological sign. The decision is based on the need to minimise the future risk of tooth pathology and to reduce age-related surgical morbidity. Although some publications compare extraction indications between clinicians, there is very little information about the factors (related either to the patient or the dentist) that influence the decision to extract an asymptomatic 3M. The objectives of the present study were to look into the indications for extracting asymptomatic lower 3Ms and to link such indications to the clinical and radiological properties of the 3M and to the clinicians' training and professional experience. A survey consisting of 15 cases of asymptomatic lower third molars was emailed to Portuguese and Spanish dentists. The clinicians were asked to assess the level of difficulty of the extractions and to make a reasoned recommendation based on the panoramic radiographs, gender and age of the patients. The results were analysed using IBM SPSS® 22.0 software (IBM Corp., New York, USA). A total of 381 clinicians filled in the questionnaires. Most of the professionals had over 13 years of clinical experience. The number of Spanish clinicians with postgraduate degrees in Oral Surgery was significantly higher. On average, 42% of respondents recommended extraction of asymptomatic third molars. The indication for extraction was significantly higher among Portuguese dentists. The estimated difficulty of 3M surgery was negatively correlated with experience: the most recent graduates rated the cases as more difficult. 2M caries and prevention of pericoronaritis were the main reason given for the extraction in patients older than 26 years, 3M partially erupted and with limited mesio-distal space to erupt, whereas that for not extracting was the absence of a clear indication and the risk of injuring the inferior alveolar nerve.

Objectives: The aim of this study is to compare the analgesic and anti-inflammatory effects of the local postoperative administration of a single 12-mg dose of betamethasone after the surgical removal of impacted lower third molars. Study Design: A split-mouth, triple-blind, randomized, placebo-controlled clinical trial of 25 patients requiring the surgical removal of symmetrical lower third molars was performed. In the experimental side, a 12-mg dose of betamethasone was administered submucosally after the surgical procedure, while in the control side a placebo (sterile saline solution) was injected in the same area. To assess postoperative pain, visual analogue scales and the consumption of rescue analgesic were used. The facial swelling and trismus were evaluated by measuring facial reference distances and maximum mouth opening. Results: There were no significant differences between the two study groups regarding postoperative pain, facial swelling and trismus. Conclusions: The injection of a single dose of betamethasone does not seem to reduce pain, facial swelling and trismus after impacted lower third molar removal when compared to placebo. [spa] OBJETIVOS: El uso de membranas de colágeno reabsorbibles (MBM) en el tratamiento de defectos intraóseos y bolsas periodontales profundas en la cara distal del segundo molar inferior (2MI) después de la extracción quirúrgica de un tercer molar inferior impactado (3MI) ha mostrado resultados contradictorios. Este estudio evaluó los efectos de la colocación de MBM en la curación de un defecto óseo distal al 2MI después de la extracción quirúrgica de los 3MI incluidos en posición horizontal o mesioangular. PACIENTES Y MÉTODOS: Se llevó a cabo un ensayo controlado aleatorio de grupos paralelos con 2 grupos independientes de 30 pacientes que requieren la extracción quirúrgica de un 3Mi. Después de la extracción, los pacientes recibieron una MBM (Bio-Gide, Geistlich Pharma AG, Wolhusen, Suiza) o sólo sutura. En el control inicial y durante la monitorización postoperatoria al 1, 3 y 6 meses, se midió la profundidad de sindaje distovestibular, distal y distolingual, así como el nivel de inserción vestibular del 2MI. RESULTADOS: La edad (grupo control, 33,8 ± 6,9 años; grupo regeneración tisular guiada, 35.6 ± 6.3 años; p = 0,322) y el número de mujeres (grupo de control, 15 de 29; grupo regeneración tisular guiada, 14 de 27, p =. 992) fueron similares en los 2 grupos. Las profundidades de sondaje distovestibular, distal y distolingual y el relleno del defecto infraóseo mostraron una mayor mejoría a los 6 meses después de la extracción quirúrgica en el grupo MBM ( P, [eng] PURPOSE: The use of resorbable collagen membranes (RMs) in the treatment of intraosseous defects and deep periodontal pockets on the distal side of a lower second molar (L2M) after surgical extraction of an impacted lower third molar (L3M) has shown contradictory results. This study evaluated the effects of RM placement on the healing of a bone defect distal to an L2M after surgical extraction of a horizontal or mesioangular impacted L3M. PATIENTS AND METHODS: A parallel-group randomized controlled trial with 2 independent groups of 30 patients requiring surgical extraction of an L3M was carried out. After extraction, patients received an RM (Bio-Gide, Geistlich Pharma AG, Wolhusen, Switzerland) or only suture. At the initial checkup and during postoperative monitoring at 1, 3, and 6 months, the distal vestibular, distal, and distolingual probing depths and distal vestibular attachment level of the L2M were measured. RESULTS: Age (control group, 33.8 ` 6.9 yr; guided tissue regeneration group, 35.6 ` 6.3 yr; P = .322) and the number of women (control group, 15 of 29; guided tissue regeneration group, 14 of 27; P = .992) were similar in the 2 groups. The distal vestibular, distal, and distolingual probing depths of the L2M, distal vestibular attachment level, distance from the cementoenamel junction, and distance from the alveolar crest to the base of the defect showed greater improvement 6 months after surgical extraction in the RM group (P < .05). CONCLUSIONS: The use of RMs after surgical extraction of mesioangular or horizontally impacted L3Ms stimulates bone regeneration, improving the attachment level and bone fill distal to the L2M. Likewise, it decreases the distal probing depth and results in faster recovery. RM placement after surgical extraction of an impacted L3M is recommended because it prevents periodontal defects after L3M surgery.

Background The aim of this study was to evaluate the clinical and microbiological impact of an antibacterial suture (Monocryl® Plus) in the surgical removal of I3M. Material and Methods A “split-mouth”, prospective pilot clinical study was designed involving 20 patients programmed for the surgical removal of I3M. Each side was randomly sutured with Monocryl® Plus or silk suture and removed for microbiological study 72 hours and 7 days after surgery. Presence of SSI, wound bleeding and the degree of discomfort associated with each type of suture material (scored by means of a visual analog scale) were evaluated. The level of contamination of each material was observed under the scanning electron microscope. Results Wound bleeding upon suture removing was slightly greater after 72 hours and 7 days with black silk suture, though the differences were not statistically significant (p=0.752 and p=0.113, respectively). Patient discomfort was very similar with both types of suture material (p=0.861). Only one case of SSI was recorded with black silk suture after 72 hours. Treball Final de Grau d'Odontologia, Facultat d'Odontologia, Universitat de Barcelona, Any: 2015, Director: Eduardo Valmaseda Castellón, Objetivo: El Objetivo principal del estudio fue evaluar la calidad de vida (CdV) y el grado de satisfacción de los pacientes que se sometieron a extracciones de terceros molares superiores bajo anestesia local. El segundo objetivo fue describir la evolución del dolor descrita por los pacientes mediante la escala analógica visual durante los siete días postoperatorios y correlacionarlo con los factores pre- e intraoperatorios. Diseño del estudio: Estudio longitudinal prospectivo. Treinta y siete pacientes recibieron el cuestionario sobre aislamiento social, aislamiento laboral, habilidad para comer, modificaciones en la dieta, habilidad para hablar, dificultades para dormir, apariencia física, dolor e incomodidad al retirar la sutura y satisfacción con el tratamiento pasados 7 días desde la extracción. Mediante la escala analógica visual (VAS) de 100mm de longitud, los pacientes marcaron cada día, durante siete días, cuál fue su dolor promedio. Resultados: Treinta pacientes rellenaron correctamente los cuestionarios. Las puntuaciones de dolor reportadas después de 7 días mostraron una reducción progresiva y lineal del dolor. Durante el segundo y tercer día fue cuando hubo una reducción significativamente mayor del dolor. Se observó una asociación positiva entre técnica quirúrgica, grado de erupción, patología previa en el molar, complicaciones durante la intervención y mayores niveles de dolor postoperatorio. La inflamación fue el cambio más importante reportado por los pacientes. Conclusión: La extracción del tercer molar superior afectó significativamente la calidad de vida y de relación de los pacientes sobre todo durante los 2 primeros días.

Apart from reflecting modern human dental variation, differences in dental size among populations provide a means for studying continuous evolutionary processes and their mechanisms. Dental wear, on the other hand, has been widely used to infer dietary adaptations and variability among or within diverse ancient human populations. Few such studies have focused on modern foragers and farmers, however, and diverse methods have been used. This research aimed to apply a single, standardized, and systematic quantitative procedure to measure dental size and dentin exposure in order to analyze differences among several hunter-gatherer and agricultural populations from various environments and geographic origins. In particular, we focused on sexual dimorphism and intergroup differences in the upper and lower first molars. Results indicated no sexual dimorphism in molar size and wear within the studied populations. Despite the great ethnographic variation in subsistence strategies among these populations, our findings suggest that differences in sexual division of labor do not affect dietary wear patterns. Background Coronectomy is the surgical removal of the crown of the tooth deliberately leaving part of its roots. This is done with the hope of eliminating the pathology caused, and since the roots are still intact, the integrity of the inferior alveolar nerve is preserved. Objectives The aim is to carry out a systematic review in order to be able to provide results and conclusions with the greatest scientific evidence possible. Material and Methods A literature review is carried out through the following search engines: Pubmed MEDLINE, Scielo, Cochrane library and EMI. The level of evidence criteria from the Agency for Healthcare Research and Quality was applied, and the clinical trials’ level of quality was analyzed by means of the JADAD criteria. Results The following articles were obtained which represents a total of 17: 1 systematic review, 2 randomized clinical trials and 2 non-randomized clinical trials, 3 cohort studies, 2 retrospective studies, 3 case studies and 4 literature reviews. Conclusions Coronectomy is an adequate preventative technique in protecting the inferior alveolar nerve, which is an alternative to the conventional extraction of third molars, which unlike the former technique, presents a high risk of injury to the inferior alveolar nerve. However, there is a need for new clinical studies, with a greater number of samples and with a longer follow-up period in order to detect potential adverse effects of the retained roots. Deviations from expected perfect symmetry in biological forms can occur, and organisms develop several kinds of asymmetries. Among others, there are fluctuating asymmetry (FA) and directional asymmetry (DA). Thirty-two complete skulls were obtained from horses belonging to the"Cavall Pirinenc Català" breed and the mesiodistal and the palatinovestibular lengths of the right and left upper molar series were analyzed in order to study FA and DA. Neither FA nor DA were detected in the studied sample. The results of this study will provide researchers with new morphological information about natural teeth wearing (based on grazing) and may also be used as a reference for designing experimental studies, especially on mandibular catch-up growth, and as an aid to zooarchaeologists in evaluating results from buried animal studies for comparison with living equine populations.

Objectives: To compare the efficacy and complication rates of two different techniques for inferior alveolar nerve blocks (IANB). Study Design: A randomized, triple-blind clinical trial comprising 109 patients who required lower third molar removal was performed. In the control group, all patients received an IANB using the conventional Halsted technique, whereas in the experimental group, a modified technique using a more inferior injection point was performed. Results: A total of 100 patients were randomized. The modified technique group showed a significantly higher onset time in the lower lip and chin area, and was frequently associated to a lingual electric discharge sensation. Three failures were recorded, 2 of them in the experimental group. No relevant local or systemic complications were registered. Conclusions: Both IANB techniques used in this trial are suitable for lower third molar removal. However, performing an inferior alveolar nerve block in a more inferior position (modified technique) extends the onset time, does not seem to reduce the risk of intravascular injections and might increase the risk of lingual nerve injuries. La hipomineralización incisivo molar es una alteración del desarrollo dentario que involucra a los primeros molares permanentes pudiendo estar comprometidos, también, los incisivos permanentes. La prevalencia de HIM varía en la literatura entre 2,5 a 40 % en la población infantil mundial. El objetivo de este estudio fue determinar la prevalencia de la HIM en una población de 772 niños catalanes entre 6,9 y 14,11 años de edad. Se estableció el diagnóstico de acuerdo a los criterios de la Academia Europea de Odontopediatría y la prevalencia encontrada fue del 7,94 % siendo comparable a la obtenida en estudios realizados a nivel mundial.

La hipomineralización incisivo molar (HIM) es una alteración cualitativa del esmalte de etiología no determinada que genera opacidades circunscritas de color variable. En el presente artículo se ha llevado a cabo una revisión bibliográfica de una alternativa de tratamiento de molares, severamente afectados de HIM con pronóstico incierto. Se plantea la exodoncia en consenso entre odontopediatra y ortodoncista con la finalidad de manejar integralmente la maloclusión existente.

Objectives: The purpose of this study was to determine the incidence and clinical symptoms associated with sharp mandibular bone irregularities (SMBI) after lower third molar extraction and to identify possible risk factors for this complication. Study Design: A mixed study design was used. A retrospective cohort study of 1432 lower third molar extractions was done to determine the incidence of SMBI and a retrospective case-control study was done to determine potential demographic and etiologic factors by comparing those patients with postoperative SMBI with controls. Results: Twelve SMBI were found (0.84%). Age was the most important risk factor for this complication. The operated side and the presence of an associated radiolucent image were also significantly related to the development of mandibular bone irregularities. The depth of impaction of the tooth might also be an important factor since erupted or nearly erupted third molars were more frequent in the SMBI group. Conclusions: SMBI are a rare postoperative complication after lower third molar removal. Older patients having left side lower third molars removed are more likely to develop this problem. Objective: To compare the anesthetic action of 0.5% bupivacaine in relation to 4% articaine, both with 1:200,000 epinephrine, in the surgical removal of lower third molars. As a secondary objective hemodynamic changes using both anesthetics were analyzed. Study Design: Triple-blind crossover randomized clinical trial. Eighteen patients underwent bilateral removal of impacted lower third molars using 0.5% bupivacaine or 4% articaine in two different appointments. Preoperative, intraoperative and postoperative variables were recorded. Differences were assessed with McNemar tests and repeated measures ANOVA tests. Introduction: The overeruption of upper molars due to the premature loss of antagonist teeth can be treated with the help of miniscrews. The aim of this study was to evaluate the movement of a typodont molar according to the biomechanical approach used with miniscrews. Study design: The study was conducted with four plaster models filled with typodont wax. In each model we used one absolute anchorage on the palatal side and another on the buccal side in different positions, thus generating four different biomechanical systems. A force of 150 g was applied to each side of the resin tooth. Periapical radiographs were taken preintrusion and immediately after completion of the intrusion. Photographs were taken in both the sagittal and occlusal planes every 3 min. The radiographic films and photographs were measured and compared. Results: A vertical movement of the molar was observed in all the models, with system 4 showing the greatest movement. Rotation in the occlusal plane only occurred in system 2, while in system 1 there was a change in the axial axis of 37 degrees. Conclusions: The anchorage site and the combination of forces applied may determine the resulting tooth movement.

Introduction. Third molar extraction is the most frequent procedure in oral surgery. The present study evaluates the indication of third molar extraction as established by the primary care dentist (PCD) and the oral surgeon, and compares the justification for extraction with the principal reason for patient consultation. Patients and method. A descriptive study was made of 319 patients subjected to surgical removal of a third molar in the context of the Master of Oral Surgery and Implantology (Barcelona University Dental School, Barcelona, Spain) between July 2004 and March 2005. The following parameters were evaluated: sex, age, molar, type of impaction, position according to the classifications of Pell and Gregory and of Winter, and the reasons justifying extraction. Results. The lower third molars were the most commonly extracted molars (73.7%). A total of 69.6% of the teeth were covered by soft tissues only. Fifty-six percent of the lower molars corresponded to Pell and Gregory Class IIB, while 42.1% were in the vertical position. The most common reason for patient reference to our Service of Oral Surgery on the part of the PCD was prophylactic removal (51.0% versus 46.1% in the case of the oral surgeon). Discussion and conclusions. Our results show prophylaxis to be the principal indication of third molar extraction, followed by orthodontic reasons. Regarding third molars with associated clinical symptoms or signs, infectious disease ? including pericoronitis ? was the pathology most often observed by the oral surgeon, followed by caries. This order of frequency was seen to invert in the case of third molars referred for extraction by the PCD. A vertical position predominated among the third molars with associated pathology.

Introducción: la extracción de los terceros molares es el procedimiento más frecuente en Cirugía Bucal. El objetivo de este estudio fue valorar la indicación de la extracción de los terceros molares por parte del odontólogo de atención primaria (OAP) y del cirujano bucal así como comparar dichas justificaciones con el motivo principal de consulta que tenía el paciente. Pacientes y Método: se realizó un estudio descriptivo simple de 319 pacientes a los que se había efectuado la extracción quirúrgica de un tercer molar en el Máster de Cirugía Bucal e Implantología Bucofacial de la Universidad de Barcelona entre julio de 2004 y marzo de 2005. Se valoraron los siguientes parámetros: sexo, edad, cordal, tipo de inclusión, posición según las clasificaciones de Pell y Gregory y de Winter así como los motivos que justificaron su exodoncia. Resultados: los terceros molares inferiores fueron extraídos en mayor proporción (73,7%). Un 69,6% de los dientes estaban cubiertos únicamente por tejidos blandos. La clase IIB de Pell y Gregory representó el 56% de los cordales inferiores, mientras que la posición vertical supuso un 42,1% de los casos. El motivo más frecuente de remisión a nuestro Servicio de Cirugía Bucal por parte del OAP fue la extracción profiláctica (51,0%) de manera muy similar a la que indicó el cirujano bucal (46,1%). Discusión y Conclusiones: la extracción de los terceros molares por motivos profilácticos fue la que se indicó con mayor frecuencia, seguida por la indicación ortodóncica. Con respecto a los terceros molares con signos patológicos o síntomas clínicos asociados, la patología infecciosa incluyendo la pericoronaritis fue la más frecuentemente observada por el cirujano bucal, seguida de la caries. Este orden de frecuencia se invirtió para los terceros molares remitidos para extracción por el OAP. La posición vertical predominó entre los terceros molares con patología asociada.

Patients and method. Different variables were recorded, including patient age, sex, molar angulation, surgical technique and radiological sinus proximity, to determine the relation between third molar extraction and the incidence of OSC. Results. Only 5.1% (95% CI: 2.2-7.3%) of the upper molar surgical extractions produced OSC, the risk of which was found to be similar in all age groups and increased with the depth of third molar inclusion, the complexity of the surgical technique and the performance of an ostectomy.

El objetivo de este estudio fue identificar la incidencia de las CBS tras la extracción de 389 cordales superiores realizadas durante el año 2003 en el Máster de Cirugía Bucal e Implantología Bucofacial de la Universidad de Barcelona. Material y método. Se registraron diversas variables con el fin de determinar la relación de la extracción del tercer molar con la incidencia de las CBS: la edad y el sexo del paciente, la angulación del cordal, la técnica quirúrgica y la sospecha radiológica de proximidad con el seno maxilar. Resultados. Únicamente el 5.1% (IC 95%: 2.2-7.3%) de las extracciones quirúrgicas de los cordales superiores provocaron una CBS. El riesgo de producir una CBS fue similar en todos los grupos de edad, y aumentó con la profundidad de inclusión del tercer molar, la complejidad de la técnica quirúrgica y al efectuar ostectomía.

Los defectos de estructura del esmalte pueden ser de origen hereditario o bien de origen ambiental, sean de causa sistémica o local. Estudios recientes muestran la prevalencia de un tipo de alteraciones del esmalte en los primeros molares e incisivos permanentes con características peculiares, y cuya etiología no está completamente esclarecida. El objetivo de esta revisión es actualizar los conocimientos acerca de los defectos por hipomineralización de incisivos y primeros molares permanentes, así como, conocer los posibles factores etiológicos, la prevalencia, las características clínicas y las posibilidades de tratamiento que enmarcan la situación clínica de hipomineralización incisivo molar.

Objetivo: En este estudio evaluamos la eficacia analgésica del diclofenaco sódico en comparación con el ibuprofeno, después de la extracción quirúrgica de un tercer molar inferior incluido. -Diseño de estudio: Los pacientes que participaron en el estudio fueron distribuidos de forma aleatoria en dos grupos. Uno fue el grupo ibuprofeno y el otro el grupo diclofenaco. La intervención practicada fue la extracción quirúrgica de un tercer molar inferior cuya dificultad fue determinada en función del grado de inclusión en todos los pacientes. Las variables registradas fueron la intensidad del dolor y la necesidad de medicación de rescate durante un periodo de una semana. Los registros se realizaron una vez al día a la misma hora y registrados en un cuaderno por parte del paciente. -Resultados: Un total de 81 pacientes (87.1%) fueron incluidos en el estudio. Los valores fueron similares en las primeras 48 horas postoperatorias, pero a partir del tercer día existió una tendencia del grupo diclofenaco a mostrar valores de dolor superiores, aunque sin llegar a existir diferencias estadísticamente significativas (p>0.05). Este estudio pretende observar la erupción de la dentición permanente en niños y niñas residentes en una población de la provincia de Barcelona. La muestra se estudió en cuanto al sexo, la edad y los primeros y segundos molares erupcionados, encontrando que la primera pieza en aparecer es el primer molar a partir de los 5,33 años de edad, y de la misma forma, el segundo molar permanente es el que cierra todo el proceso eruptivo. Se adjuntan tablas para complementar la secuencia eruptiva.

La introducción de los ultrasonidos para la realización de cajas apicales en cirugía periapical ha simplificado la técnica quirúrgica y facilitado la retroobturación con amalgama de plata. Esta técnica es especialmente útil en los molares cuya accesibilidad a los ápices radiculares es más dificultosa. En este trabajo presentamos una serie de 31 pacientes en los que se realizó cirugía periapical en un total de 31 molares mandibulares. No se presentaron complicaciones intraoperatorias. Las únicas complicaciones postoperatorias relevantes fueron la parestesia transitoria del nervio mentoniano en dos casos, y una fístula intraoral en otro. Tras un año mínimo de seguimiento, solamente tuvimos tres fracasos, siendo la curación clínica completa en el 90,4%. Curación radiográfica completa obtuvimos en el 54,8%, incompleta o parcial en el 32,3% y no curación en el 12,9%.

El objetivo de este estudio fue comparar la frecuencia cardíaca y la saturación de oxígeno bajo el efecto de tres anestésicos locales diferentes, con la misma concentración de epinefrina, durante la extracción de un tercer molar inferior en pacientes sanos. Los anestésicos locales utilizados fueron articaína al 4%, lidocaína al 2% y mepivacaína al 2%, los tres con una concentración de epinefrina de 1:100.000. Se diseñó un estudio de doble ciego, paralelo y no aleatorizado. Los pacientes fueron divididos en tres grupos de quince, uno para cada anestésico. En cada paciente se monitorizó la frecuencia cardíaca y la saturación de oxígeno antes de inyectar el anestésico, justo después, un minuto después de la utilización del motor para efectuar la ostectomía, al inicio de la fuerza para realizar la exodoncia, al finalizar la sutura y a la semana, al retirar los puntos de sutura, valor usado, este último, como basal. También se registró la presión arterial y se calculó la presión arterial media, el producto presión-frecuencia y el cociente presión-frecuencia. Sólo la variable frecuencia cardíaca presentó diferencias significativas a lo largo de las intervenciones, independientemente del tipo de anestésico local utilizado. Supernumerary molars are not uncommon and may be found nearly anywhere in the dental alveolar arches. A series of 36 patients that presented with 53 supernumerary molars are reviewed. They occurred more frequently in the maxilla (86.8%), had little or no clinical significance, tended to be impacted, and were not associated with the impactation of the third molar. En la cirugía periapical de molares inferiores, si existe una cortical vestibular gruesa es necesario realizar una amplia ostectomía, eliminando considerable cantidad de hueso. Decribimos, a propósito de cuatro casos una técnica quirúrgica alternativa en cirugía periapical de molares mandibulares. Consiste en realizar una osteotomía en lugar de ostectomía de la cortical ósea vestibular. El método propuesto permite tener a través de una 'ventana ósea' un campo quirúrgico adecuado y además permite conservar el hueso en bloque y reposicionarlo en el mismo lugar donde se retiró. La modificación del método descrito en el presente trabajo es una opción a considerar en la cirugía periapical de los molares inferiores, cuando exista una cortical vestibular gruesa y suficiente.

El propósito de este estudio es valorar el dolor, la inflamación, el trismo y el consumo de analgésicos durante los 7 días del postoperatorio, tras la extracción quirúrgica de terceros molares inferiores impactados, en relación con la dificultad de la intervención. Incluimos 100 pacientes sanos que fueron sometidos a la extracción quirúrgica de terceros molares inferiores impactados. Valoramos el dolor postquirúrgico usando una escala analógica-visual, la inflamación mediante una escala subjetiva, el trismo como disminución de la apertura interincisiva y el consumo de analgésicos. El tiempo de ostectomía, la práctica de odontosección y la duración de la intervención, influyeron en el postoperatorio aumentado el dolor, la inflamación y el trismo. El tiempo de ostectomía y la duración de la intervención presentaron una relación estadísticamente significativa con el trismo, y la odontosección con el dolor y el trismo. El estudio trata de ser una herramienta para ayudar a prever los efectos secundarios del postoperatorio, en función de la dificultad del acto quirúrgico.

Presentamos un estudio de las complicaciones surgidas en la extracción quirúrgica de 190 terceros molares inferiores incluidos. No se produjeron complicaciones intraoperatorias. Encontramos en 25 casos complicaciones postoperatorias (13%), las más frecuentes fueron edema persistente en 17 casos (9%) y alveolitis seca en 4 casos (2%). Otras complicaciones recogidas fueron 2 pacientes con parestesia del nervio dentario inferior, 1 con parestesia del nervio lingual y 1 caso de hemorragia postoperatoria. Todas las complicaciones remitieron favorablemente en el postoperatorio sin ningún tipo de secuela.

La extracción del tercer molar inferior incluido provoca efectos secundarios como dolor, inflamación y trismo. Presentamos un estudio cuyo objetivo fue relacionar la edad, el sexo, el tabaco y la higiene oral con el postoperatorio de 100 de terceros molares inferiores incluidos. La edad, el sexo y el consumo de tabaco no influyeron significativamente en el postoperatorio. Sólo hallamos una correlación estadísticamente significativa entre la higiene oral y el dolor a las 6 horas de la exodoncia.

En el presente trabajo se ha realizado una revisión bibliográfica de la relación existente entre la erupción de los terceros molares y la posibilidad de que aparezca un apiñamiento anterior (normalmente inferior). El apiñamiento es una malposición dental muy frecuente en la población, por lo que si realmente se puede relacionar con la erupción del tercer molar, el tratamiento precoz en dichos casos evitaría la aparición de esta malposición dental. Conoceremos las posturas de los diferentes autores al respecto, los distintos tratamientos que se pueden realizar y otros aspectos relacionados con el tema.

Estudiamos 17 quistes paradentales asociados a terceros molares mandibulares impactados, revisando sus aspectos clínico-patológicos más importantes. Once pacientes fueron mujeres y 6 hombres, con una edad media de 23,6 años. El tamaño de los quistes osciló entre los 4 y los 15 mm y se presentaron unidos a la corona de lo molares a nivel de la unión amelocementaria, especialmente a su porción distal ( 3,3% ele los casos). Su histología, similar a otros quistes odontogénicos inflamatorios, mostró un epitelio escamoso e estratificado, marcadamente hiperplásico, asociado a un infiltrado inflamatorio crónico.

Se presenta el caso de una mujer de 23 años de edad que presentaba los molares inferiores permanentes segundo y tercero impactados verticalmente en la rama ascendente de la mandíbula. Las superficies oclusales de ambos dientes se encontraban en contacto. Tras un diagnóstico preciso, que incluyó una tomografía computarizada, se decidió extraer ambos dientes. El conducto del nervio dentario inferior, que en las radiografías se consideró que se encontraba bajo riesgo, no sufrió ningún daño, gracias a una técnica quirúrgica cuidadosa.

La infraoclusión de los molares temporales es un fenómeno relativamente frecuente que afecta a la población infantil desde los tres años en adelante, aunque puede prolongarse hasta la edad adulta. El presente artículo realiza una revisión de la literatura más reciente sobre dicha entidad y presenta algunos casos.

Se describe un caso que afecta al primer molar permanente inferior derecho, con tumefacción, fluctuación y fístula , con ápices no cerrados. El tratamiento de elección hubiese sido una técnica clásica de apicoformación con hidróxido de calcio. Pero por motivos de colaboración, se realizó una pulpotomía con hidróxido de calcio. Tras los controles radiológicos a los 3 meses, al año y al año y medio se comprobó que continuaba el desarrollo radicular y finalizaba el cierre apical.

La correcta identificación anatómica del conducto dentario inferior, por el que circula el nervio dentario inferior, con respecto al tercer molar es esencial cuando es preciso practicar la exéresis de los cordales inferiores incluidos, puesto que la proximidad de ambas estructuras condiciona la posibilidad de lesionar dicho nervio. En este artículo, se revisa el estudio radiológico del conducto dentario inferior y se comentan las diferentes técnicas de diagnóstico por la imagen (ortopantomografía, radiografías periapicales y oclusales y tomografía axial computadorizada), así como sus diversas indicaciones y los signos radiológicos útiles para determinar la situación y trayecto del conducto dentario inferior con respecto a los ápices.

Se han estudiado 65 pacientes sometidos a intervención quirúrgica periapical en 72 molares ( 120 raíces y 155 conductos radiculares). Se analizó una serie de características preoperatorias (edad, antecedentes patológicos, patologías asociadas, distribución de dientes afectados, tamaño de la lesión periapical, tipo de endodoncia ortógrafa y calidad radiológica de la endodoncia previa), peroperatorias (tipo de incisión, existencia de comunicación oroantral, afectación cortical, tamaño de la resección apical, obturación retrógrada y complicaciones) y postoperatorias (estudio histológico, complicaciones y curación radiológica completa, incompleta, incierta y fracaso). En el seguimiento a un año, el porcentaje de curación completa fue del 77,8; el de curación incompleta fue dell3,9%; el de curación incierta, 6,9% y el de fracaso 1,4%. Durante la última visita de seguimiento, el porcentaje de curación completa fue del 86,1 %; de curación incompleta, 4,1% y de fracaso, 9,7%. Hubo un 18,5% de complicaciones intraoperatorias y un 6,2% de complicaciones postoperatorias. El análisis histológico de las lesiones demostró un 69,2% de quistes periapicales y un 30,7% de granulomas periapicales.

En la actualidad sabernos que, para conseguir la eliminación de los gérmenes del interior de los conductos radiculares, el medio fundamental lo constituye la preparación biomecánica y no el uso de medicaciones, las cuales desempeñan en todo caso un papel coadyuvante...

Objetivo: El propósito de este estudio fue analizar la incidencia de complicaciones después de la extracción quirúrgica de los terceros molares en pacientes de 12 a 18 años de edad. Pacientes y método: Se realizó un estudio retrospectivo de 390 extracciones quirúgicas de terceros molares superiores e inferiores en 173 pacientes en edades comprendidas entre los 12 y 18 años de edad, intervenidos bajo anestesia locorregional en el año 2000 en el Máster de Cirugía Bucal e Implantología Bucofacial de la Universidad de Barcelona. Los pacientes fueron divididos en tres grupos (A: 12-14 años, B:15-16 años, C: 17-18 años). Se registró la edad y el sexo del paciente, el motivo de la extracción, el grado de desarrollo dentario, la posición, la angulación y la inclusión del tercer molar. Finalmente se valoró la asociación entre dichas variables y la aparición de complicaciones postoperatorias. Resultados: La mayoría de los pacientes intervenidos fueron del sexo femenino (66,9%), y el grupo de edad en la que se realizó el mayor número de extracciones (62,8%) fue en el grupo C (17-18 años). El principal motivo de extracción fue la indicación por parte del ortodoncista (40,5%), seguido por la indicación profiláctica (39,5%). La presencia de clínica fue el motivo de extracción en un 20% de los casos. Se observó una incidencia del 15,6% de complicaciones postoperatorias tras la extracción de 390 terceros molares. En el grupo A el riesgo de complicaciones fue de 17,4%, en el grupo B de 19% y en el grupo C de 13,7%. Todas las complicaciones fueron reversibles y de corta duración como el dolor y la inflamación persistente, la infección, el trismo y la equimosis. Se presentó un caso de parestesia del nervio dentario inferior y un caso de parestesia del nervio lingual que remitieron a los 2 meses y al mes respectivamente. Conclusiones: este estudio demuestra que no hay diferencias significativas en las complicaciones observadas entre los tres grupos de edad, y por lo general, éstas son leves y reversibles. Se observó una mayor tendencia de complicaciones en las mujeres y el porcentaje de complicaciones aumenta cuanto menor es el espacio disponible para la erupción. Objective: An analysis is made of the incidence of complications following third molar surgical extraction in patients between 12 and 18 years of age. Patients and method: A retrospective study was conducted of 390 surgical extractions of upper and lower third molars in 173 patients operated upon under locoregional anesthesia during the year 2000 in the Master in Oral Surgery and Implantology of Barcelona University Dental School (Spain). The reason for extraction, the degree of dental development, and third molar position, angle and impaction were recorded. Finally, the association of these variables to the appearance of postoperative complications was analyzed. The main reason for extraction was orthodontist indication (40,5%), closely followed by prophylaxis (39,5%). The existence of clinical manifestations was an indication for extraction in 20% of cases. The postoperative complications rate after the extraction of the global 390 molars was 15,6%. In Groups A, B and C the complication risks were 17,4%, 19% and 13,7%, respectively. All complications were reversible and of short duration. One case each of inferior alveolar nerve paresthesia and lingual nerve paresthesia was recorded, which subsided after one and two months, respectively. Conclusions: There were no significant differences in complications between the three age groups. An increased tendency towards complications was observed in females, and the percentage of postoperative problems increased.
