Introduction Various research sites are pursuing 14 T MRI systems. However, both local SAR and RF transmit field inhomogeneity will increase. The aim of this simulation study is to investigate the trade-offs between peak local SAR and flip angle uniformity for five transmit coil array designs at 14 T in comparison to 7 T. Methods Investigated coil array designs are: 8 dipole antennas (8D), 16 dipole antennas (16D), 8 loop coils (8D), 16 loop coils (16L), 8 dipoles/8 loop coils (8D8L) and for reference 8 dipoles at 7 T. Both RF shimming and kT-points were investigated by plotting L-curves of peak SAR levels vs flip angle homogeneity. Results For RF shimming, the 16L array performs best. For kT-points, superior flip angle homogeneity is achieved at the expense of more power deposition, and the dipole arrays outperform the loop coil arrays. Discussion and conclusion For most arrays and regular imaging, the constraint on head SAR is reached before constraints on peak local SAR are violated. Furthermore, the different drive vectors in kT-points alleviate strong peaks in local SAR. Flip angle inhomogeneity can be alleviated by kT-points at the expense of larger power deposition. For kT-points, the dipole arrays seem to outperform loop coil arrays.

PURPOSE: To characterize the acceleration capabilities of a silent head insert gradient axis that operates at the inaudible frequency of 20 kHz and a maximum gradient amplitude of 40 mT/m without inducing peripheral nerve stimulation. METHODS: The silent gradient axis' acquisitions feature an oscillating gradient in the phase-encoding direction that is played out on top of a cartesian readout, similarly as done in Wave-CAIPI. The additional spatial encoding fills k-space in readout lanes allowing for the acquisition of fewer phase-encoding steps without increasing aliasing artifacts. Fully sampled 2D gradient echo datasets were acquired both with and without the silent readout. All scans were retrospectively undersampled (acceleration factors R = 1 to 12) to compare conventional SENSE acceleration and acceleration using the silent gradient. The silent gradient amplitude and the readout bandwidth were varied to investigate the effect on artifacts and g-factor. RESULTS: The silent readout reduced the g-factor for all acceleration factors when compared to SENSE acceleration. Increasing the silent gradient amplitude from 31.5 mT/m to 40 mT/m at an acceleration factor of 10 yielded a reduction in the average g-factor (gavg ) from 1.3 ± 0.14 (gmax = 1.9) to 1.1 ± 0.09 (gmax = 1.6). Furthermore, reducing the number of cycles increased the readout bandwidth and the g-factor that reached gavg = 1.5 ± 0.16 for a readout bandwidth of 651 Hz/pixel and an acceleration factor of R = 8. CONCLUSION: A silent gradient axis enables high acceleration factors up to R = 10 while maintaining a g-factor close to unity (gavg = 1.1 and gmax = 1.6) and can be acquired with clinically relevant readout bandwidths.

Objective Sodium concentration is responsible for (at least part of) the stiffness of articular cartilage due to the osmotic pressure it generates. Therefore, we hypothesized that we could use sodium MRI to approximate the stiffness of cartilage to assess early cartilage degeneration. Methods Four human tibial plateaus were retrieved from patients undergoing total knee replacement (TKR), and their cartilage stiffness mapped with indentation testing, after which samples were scanned in a 7 T MRI to determine sodium concentration. The relation of biomechanical parameters to MRI sodium and glycosaminoglycan (GAG) concentration was explored by a linear mixed model. Results Weak correlations of GAG concentration with apparent peak modulus (p = 0.0057) and apparent equilibrium modulus (p = 0.0181) were observed and lack of correlation of GAG concentration versus MRI sodium concentration was observed. MRI sodium concentration was not correlated with apparent peak modulus, though a moderate correlation of MRI sodium concentration with permeability was shown (p = 0.0014). Discussion and conclusion Although there was correlation between GAG concentration and cartilage stiffness, this was not similar with sodium concentration as measured by MRI. Thus, if the correlation between MRI sodium imaging and GAG concentration could be resolved, this strategy for assessing cartilage functional quality still holds promise.

Single-voxel MRS (SV MRS) requires robust volume localization as well as optimized crusher and phase-cycling schemes to reduce artifacts arising from signal outside the volume of interest. However, due to local magnetic field gradients (BWe investigated all possible coherence pathways associated with imperfect localization in a semi-localized by adiabatic selective refocusing (semi-LASER) sequence for potential rephasing of signals arising from unwanted coherence pathways by a local magnetic field gradient. We searched for locations in the BOur analysis revealed that potential sources of spurious signals were scattered over multiple locations throughout the brain. This was confirmed by 3D-CSI data. T2* mapping is promising for the evaluation of articular cartilage collagen. In this work, a groove model in a large animal is used as a model for posttraumatic arthritis. We hypothesized that T2* mapping could be employed to differentiate between healthy and (subtly) damaged cartilage. Eight carpal joints were obtained from four adult Shetland ponies that had been included in the groove study. In this model, grooves were surgically created on the proximal articular surface of the intermediate carpal bone (radiocarpal joint) and the radial facet of the third carpal bone (middle carpal joint) by either coarse disruption or sharp incision. After 9 months, T2* mapping of the entire carpal joint was carried out on a 7.0‐T whole‐body magnetic resonance imaging (MRI) scanner by means of a gradient echo multi‐echo sequence. Afterwards, assessment of collagen orientation was carried out based on Picrosirius Red‐stained histological sections, visualized by polarized light microscopy (PLM). The average T2* relaxation time in grooved samples was lower than in contralateral control sites. Opposite to the grooved areas, the “kissing sites” had a higher average T2* relaxation time than the grooved sites. PLM showed mild changes in orientation of the collagen fibers, particularly around blunt grooves. This work shows that T2* relaxation times are different in healthy cartilage vs (early) damaged cartilage, as induced by the equine groove model. Additionally, the average T2* relaxation times are different in kissing lesions vs the grooved sites., The median T2* relaxation time was lower in the sample group of grooved cartilage than in contralateral control sites. The sample group of kissing sites had a higher median T2* relaxation time as compared to the opposite, grooved sites. This work shows that T2* relaxation times are different in healthy cartilage vs (early) damaged cartilage, as induced by the equine groove model.

Purpose To improve imaging performance for body MRI with a local transmit array at 10.5T, the geometry of a dipole antenna was optimized to achieve lower peak specific absorption rate (SAR) levels and a more uniform transmit profile. Methods Electromagnetic simulations on a phantom were used to evaluate the SAR and B 1 + -performance of different dipole antenna geometries. The best performing antenna (the snake antenna) was simulated on human models in a 12-channel array configuration for safety assessment and for comparison to a previous antenna design. This 12-channel array was constructed after which electromagnetic simulations were validated by B 1 + -maps and temperature measurements. After obtaining approval by the Food and Drug Administration to scan with the snake antenna array, in vivo imaging was performed on 2 volunteers. Results Simulation results on a phantom indicate a lower SAR and a higher transmit efficiency for the snake antenna compared to the fractionated dipole array. Similar results are found on a human body model: when comparing the trade-off between uniformity and peak SAR, the snake antenna performs better for all imaging targets. Simulations and measurements are in good agreement. Preliminary imaging result were acquired in 2 volunteers with the 12-channel snake antenna array. Conclusion By optimizing the geometry of a dipole antenna, peak SAR levels were lowered while achieving a more uniform transmit field as demonstrated in simulations on a phantom and a human body model. The array was constructed, validated, and successfully used to image 2 individuals at 10.5T.

Patient-derived cancer cells cultured in vitro are a cornerstone of cancer metabolism research. More recently, the introduction of organoids has provided the research community with a more versatile model system. Physiological structure and organization of the cell source tissue are maintained in organoids, representing a closer link to in vivo tumor models. High-resolution magic angle spinning magnetic resonance spectroscopy (HR MAS MRS) is a commonly applied analytical approach for metabolic profiling of intact tissue, but its use has not been reported for organoids. The aim of the current work was to compare the performance of HR MAS MRS and extraction-based nuclear magnetic resonance (NMR) in metabolic profiling of wild-type and tumor progression organoids (TPOs) from human colon cancer, and further to investigate how the sequentially increased genetic alterations of the TPOs affect the metabolic profile. Sixteen metabolites were reliably identified and quantified both in spectra based on NMR of extracts and HR MAS MRS of intact organoids. The metabolite concentrations from the two approaches were highly correlated (r = 0.94), and both approaches were able to capture the systematic changes in metabolic features introduced by the genetic alterations characteristic of colorectal cancer progression (e.g., increased levels of lactate and decreased levels of myo-inositol and phosphocholine with an increasing number of mutations). The current work highlights that HR MAS MRS is a well-suited method for metabolic profiling of intact organoids, with the additional benefit that the nondestructive nature of HR MAS enables subsequent recovery of the organoids for further analyses based on nucleic acids or proteins.

PURPOSE Deuterium metabolic imaging could potentially be used to investigate metabolism in skeletal muscle noninvasively. However, skeletal muscle is a tissue with a high degree of spatial organization. In this study, we investigated the effect of incomplete motional averaging on the naturally abundant deuterated water signal in 7 Tesla deuterium spectra of the lower leg muscles and the dependence on the angle between the muscle fibers and the main magnetic field B0 , as determined by DTI. METHODS Natural abundance deuterium MRSI measurements of the right lower leg muscles were performed at 7 Tesla. Three subjects were scanned in a supine position, with the right leg parallel with the B0 field. One subject was scanned twice; during the second scan, the subject was laying on his right side and the right knee was bent such that the angle between the right lower leg and B0 was approximately 45°. DTI was performed in the same subjects in the same positions at 3 Tesla to determine muscle fiber angles. RESULTS We observed splittings in the natural abundance deuterated water signal. The size of the splittings varied between different muscles in the lower leg but were mostly similar among subjects for each muscle. The splittings depended on the orientation of the muscle fibers with respect to the main magnetic field B0 . CONCLUSION Partial molecular alignment in skeletal muscle leads to residual deuteron quadrupolar couplings in deuterated water, the size of which depends on the angle between the muscle fibers and B0 .

PURPOSE: A novel silent imaging method is proposed that combines a gradient insert oscillating at the inaudible frequency 20 kHz with slew rate-limited gradient waveforms to form a silent gradient axis that enable quiet and fast imaging. METHODS: The gradient insert consisted of a plug-and-play (45 kg) single axis z-gradient, which operated as an additional fourth gradient axis. This insert was made resonant using capacitors and combined with an audio amplifier to allow for operation at 20 kHz. The gradient field was characterized using field measurements and the physiological effects of operating a gradient field at 20 kHz were explored using peripheral nerve stimulation experiments, tissue heating simulations and sound measurements. The imaging sequence consisted of a modified gradient-echo sequence which fills k-space in readout lanes with a width proportional to the oscillating gradient amplitude. The feasibility of the method was demonstrated in-vivo using 2D and 3D gradient echo (GRE) sequences which were reconstructed using a conjugate-gradient SENSE reconstruction. RESULTS: Field measurements yielded a maximum gradient amplitude and slew rate of 40.8 mT/m and 5178T/m/s at 20 kHz. Physiological effects such as peripheral nerve stimulation and tissue heating were found not to be limiting at this amplitude and slew rate. For a 3D GRE sequence, a maximum sound level of 85 db(A) was measured during scanning. Imaging experiments using the silent gradient axis produced artifact free images while also featuring a 5.3-fold shorter scan time than a fully sampled acquisition. CONCLUSION: A silent gradient axis provides a novel pathway to fast and quiet brain imaging.

Purpose The coax dipole antenna, a flexible antenna for body imaging at 7T is presented. Similar to the high impedance coil, this coaxial cable antenna is fed on the central conductor and through gaps in the shield, the current passes to the outside of the antenna to generate B1 field. This could achieve more favorable current distributions and better adaptation to the body curvature. Methods Finite difference time domain (FDTD) simulations are performed to optimize the positions of the gaps in the shield for a flat current profile. Lumped inductors are added to each end to reduce losses. The performance of a single antenna is compared to a fractionated dipole using B1 maps and MR thermometry. Finally, an array of eight coax dipoles is evaluated in simulations and used for in-vivo scanning. Results An optimal configuration is found with gaps located at 10 cm from the center and inductor values of 28 nH. In comparison to the fractionated dipole antenna, in single antenna phantom measurements the coax dipole achieves similar B1 amplitude with 18% lower peak temperature. In simulations, the eight-channel array of coax dipoles improved B1 homogeneity by 18%, along with small improvements in transmit efficiency and specific absorption rate (SAR). MRI measurements on three volunteers show more consistent performance for the coax dipoles. Conclusion The coax dipole is a novel antenna design with a flattened current distribution resulting in beneficial properties. Also, the flexible design of the coax dipoles allows better adaptation to the body curvature and can potentially be used for a wide range of imaging targets.

The human cerebellum is involved in a wide array of functions, ranging from motor control to cognitive control, and as such is of great neuroscientific interest. However, its function is underexplored in vivo, due to its small size, its dense structure and its placement at the bottom of the brain, where transmit and receive fields are suboptimal. In this study, we combined two dense coil arrays of 16 small surface receive elements each with a transmit array of three antenna elements to improve BOLD sensitivity in the human cerebellum at 7 T. Our results showed improved B 1 + and SNR close to the surface as well as g‐factor gains compared with a commercial coil designed for whole‐head imaging. This resulted in improved signal stability and large gains in the spatial extent of the activation close to the surface (, Human cerebellar fMRI is hindered by B 1 + cancellation in ultra‐high fields and the need for high resolution. Using a dedicated 3Tx array improved the transmit field close to the cerebellar surface compared with a commercial 8Tx coil. The signal‐to‐noise and parallel imaging performance were further increased by the usage of a high‐density surface receive array. The combination of improved transmit and receive fields resulted in increased BOLD sensitivity close to the surface of the cerebellum.

Purpose To investigate inter‐subject variability of B 1 +, SAR and temperature rise in a database of human models using a local transmit array for 7 T cardiac imaging. Methods Dixon images were acquired of 14 subjects and segmented in dielectric models with an eight‐channel local transmit array positioned around the torso for cardiac imaging. EM simulations were done to calculate SAR distributions. Based on the SAR distributions, temperature simulations were performed for exposure times of 6 min and 30 min. Peak local SAR and temperature rise levels were calculated for different RF shim settings. A statistical analysis of the resulting peak local SAR and temperature rise levels was performed to arrive at safe power limits. Results For RF shim vectors with random phase and uniformly distributed power, a safe average power limit of 35.7 W was determined (first level controlled mode). When RF amplitude and phase shimming was performed on the heart, a safe average power limit of 35.0 W was found. According to Pennes' model, our numerical study suggests a very low probability of exceeding the absolute local temperature limit of 40 °C for a total exposure time of 6 min and a peak local SAR of 20 W/kg. For a 30 min exposure time at 20 W/kg, it was shown that the absolute temperature limit can be exceeded in the case where perfusion does not change with temperature. Conclusion Safe power constraints were found for 7 T cardiac imaging with an eight‐channel local transmit array, while considering the inter‐subject variability of B 1 +, SAR and temperature rise., 14 subject specific dielectric body models were constructed based on dixon MRI scans. SAR and thermal simulations were performed in all these for a 7T body array of 8 fractionated dipole antennas to find distributions of peak SAR and temperature. Based on our model database, safe power limits were found for 7T cardiac imaging, while considering inter‐subject variation of SAR, temperature and B1+.

Purpose: DWI is a promising modality in breast MRI, but its clinical acceptance is slow. Analysis of DWI is hampered by geometric distortion artifacts, which are caused by off-resonant spins in combination with the low phase-encoding bandwidth of the EPI sequence used. Existing correction methods assume smooth off-resonance fields, which we show to be invalid in the human breast, where high discontinuities arise at tissue interfaces. Methods: We developed a distortion correction method that incorporates high-resolution off-resonance maps to better solve for severe distortions at tissue interfaces. The method was evaluated quantitatively both ex vivo in a porcine tissue phantom and in vivo in 5 healthy volunteers. The added value of high-resolution off-resonance maps was tested using a Wilcoxon signed rank test comparing the quantitative results obtained with a low-resolution off-resonance map with those obtained with a high-resolution map. Results: Distortion correction using low-resolution off-resonance maps corrected most of the distortions, as expected. Still, all quantitative comparison metrics showed increased conformity between the corrected EPI images and a high-bandwidth reference scan for both the ex vivo and in vivo experiments. All metrics showed a significant improvement when a high-resolution off-resonance map was used (P < 0.05), in particular at tissue boundaries. Conclusion: The use of off-resonance maps of a resolution higher than EPI scans significantly improves upon existing distortion correction techniques, specifically by superior correction at glandular tissue boundaries.

Objective To determine T-1 and T-2 relaxation times of healthy pancreas parenchyma at 7 T using a multi-transmit system. Materials and methods Twenty-six healthy subjects were scanned with a 7 T MR system using eight parallel transceiver antennas, each with two additional receive loops. A Look-Locker sequence was used to obtain images for T-1 determination, while T-2 was obtained from spin-echo images and magnetic resonance spectroscopy measurements with different echo times. T-1 and T-2 times were calculated using a mono-exponential fit of the average magnitude signal from a region of interest in the pancreas and were tested for correlation with age. Results The age range of the included subjects was 21-72 years. Average T-1 and T-2 relaxation times in healthy pancreas were 896 +/- 149 ms, and 26.7 +/- 5.3 ms, respectively. No correlation with age was found. Conclusion T-1 and T-2 relaxation times of the healthy pancreas were reported for 7 T, which can be used for image acquisition optimization. No significant correlations were found between age and T-1 or T-2 relaxation times of the pancreas. Considering their low standard deviation and no observable age dependence, these values may be used as a baseline to study potentially pancreatic tissue affected by disease.

Proton Magnetic Resonance Spectroscopy ((1)H MRS) provides non-invasive, quantitative metabolite profiles of tissue and has been shown to aid the clinical management of several brain diseases. Whilst most modern clinical MR scanners support MRS capabilities, routine use is largely restricted to specialized centers with good access to MR research support. Widespread adoption has been slow for several reasons, and technical challenges towards obtaining reliable good-quality results have been identified as a contributing factor. Considerable progress has been made by the research community to address many of these challenges, and in this paper a consensus is presented on deficiencies in widely available MRS methodology and validated improvements that are currently in routine use at several clinical research institutions. In particular, the localization error for the popular point resolved spectroscopy (PRESS) localization sequence was found to be unacceptably high at 3T, and the use of the semi-adiabatic localization by adiabatic selective refocusing (semi-LASER) sequence is a recommended solution. The incorporation of simulated metabolite basis-sets into analysis routines is recommended for reliably capturing the full spectral detail available from short echo time acquisitions. In addition, the importance of achieving a highly homogenous static magnetic field (B(0)) in the acquisition region is emphasized, and the limitations of current methods and hardware are discussed. Most recommendations require only software improvements, greatly enhancing the capabilities of clinical MRS on existing hardware. We anticipate the implementation of these recommendations will strengthen current clinical applications and advance progress towards developing and validating new MRS biomarkers for clinical use.

Patients with psychotic disorders often show prominent cognitive impairment. Glutamate seems to play a prominent role, but knowledge on its role in deep gray matter regions is limited and previous studies have yielded heterogeneous results. The aim was to evaluate glutamate levels within deep gray matter structures in patients with a psychotic disorder in relation to cognitive functioning, using advanced spectroscopic acquisition, reconstruction and post-processing techniques. A 7 tesla MRI scanner combined with a unique lipid suppression coil and subject specific water signal suppression pulses were used to acquire high-resolution magnetic resonance spectroscopic imaging data. Anatomical scans were used to perform tissue fraction correction and registration to a standard brain for group comparison in specifically delineated brain regions. The brief assessment of cognition in schizophrenia was used to evaluate cognitive status. Average glutamate levels across deep gray matter structures (i.e. caudate, pallidum, putamen, and thalamus) in patients with a psychotic disorder (n=16, 4 females) were lower compared to healthy controls (n=23, 7 females). Stratified analyses showed lower glutamate levels in the caudate and putamen but not in the pallidum or thalamus. Average glutamate levels across deep gray matter structures were positively correlated with cognition, particularly to psychomotor speed. We find reduced glutamate levels across deep brain structures such as the caudate and putamen in patients with a psychotic disorder that are linked to psychomotor speed. Our results underscore the potential role of detailed in vivo glutamate assessments to understand cognitive deficits in patients with psychotic disorders.

The goal of this study was to introduce and evaluate the performance of a lightweight, high‐performance, single‐axis (z‐axis) gradient insert design primarily intended for high‐resolution functional magnetic resonance imaging, and aimed at providing both ease of use and a boost in spatiotemporal resolution. The optimal winding positions of the coil were obtained using a genetic algorithm with a cost function that balanced gradient performance (minimum 0.30 mT/m/A) and field linearity (≥16 cm linear region). These parameters were verified using field distribution measurements by B0‐mapping. The correction of geometrical distortions was performed using theoretical field distribution of the coil. Simulations and measurements were performed to investigate the echo planar imaging echo‐spacing reduction due to the improved gradient performance. The resulting coil featured a 16‐cm linear region, a weight of 45 kg, an installation time of 15 min, and a maximum gradient strength and slew rate of 200 mT/m and 1300 T/m/s, respectively, when paired with a commercially available gradient amplifier (940 V/630 A). The field distribution measurements matched the theoretically expected field. By utilizing the theoretical field distribution, geometrical distortions were corrected to within 6% of the whole‐body gradient reference image in the target region. Compared with a whole‐body gradient set, a maximum reduction in echo‐spacing of a factor of 2.3 was found, translating to a 344 μs echo‐spacing, for a field of view of 192 mm, a receiver bandwidth of 920 kHz and a gradient amplitude of 112 mT/m. We present a lightweight, single‐axis gradient insert design that can provide high gradient performance and an increase in spatiotemporal resolution with correctable geometrical distortions while also offering a short installation time of less than 15 min and minimal system modifications., A single‐axis gradient insert, designed to be plug‐and‐play while providing a significant boost in gradient performance (gradient strength = 200 mT/m and slew rate = 1300 T/m/s), was presented. This coil was characterized in terms of linearity, image distortions and reduction in echo‐spacing for echo planar imaging. Distortions were found to be correctable and echo‐spacing could be reduced by a factor of 2.3 compared with a whole‐body gradient set (40 mT/m and 200 T/m/s).

Ultrahigh field magnetic resonance imaging facilitates high spatiotemporal resolution that benefits from increasing the number of receiver elements. Because high‐density receiver arrays have a relatively small element size compared with the transmitter, a side effect is that such setups cause low flux coupling between the transmitter and receiver. Moreover, when transmitters are designed in a multitransmit configuration, their relative size is much smaller than the sample, reducing coupling to the sample and thereby potentially also the coupling to the receivers. Transmitters are traditionally detuned during reception. In this study, we investigate, for a 32‐channel receiver head array at 7 T, if transmitter detuning of a quadrature birdcage or of an eight‐channel transmit coil can be omitted without substantially sacrificing signal‐to‐noise ratio (SNR). The transmit elements are operated once with and once without detuning and, in the latter, the received signals are either merged with the array or excluded for image reconstruction. For each of the three measurements, SNR and 1/g‐factor maps are investigated. The tuning of the quadrature and eight‐channel transmit coils during signal reception introduced a 10.1% and 6.5% penalty in SNR, respectively, relative to the SNR received with detuned transmitters. When also incorporating the signal of the transmit coils, the SNR was regained to 98.5% or 101.4% for the quadrature and eight‐channel coil, respectively, relative to the detuned transmitters, while the 1/g‐factor maps improved slightly. For the 32‐channel receive coil used the SNR penalty can become negligible when omitting detuning of the transmit coils. This not only simplifies transmit coil designs, potentially increasing their efficiency, but also enables the transmitters to be used as receivers in parallel to the receiver array, thus increasing parallel imaging performance., Traditionally, transmitter coils are detuned from receivers during acquisition to exclude their noise contribution. However, with high‐density arrays that have smaller elements, this noise contribution can become negligible and can even be overcompensated by acquiring the signal from the tuned transmit elements as well. We demonstrate the effect of a tuned quadrature birdcage and an eight‐channel transmit coil on a 32‐channel receiver array at 7 T in terms of signal‐to‐noise ratio and 1/g‐factor performance when loaded with a human head.

Introduction Magnetic resonance spectroscopic imaging (MRSI) has the potential to add a layer of understanding of the neurobiological mechanisms underlying brain diseases, disease progression, and treatment efficacy. Limitations related to metabolite fitting of low signal‐to‐noise ratios data, signal variations due to partial‐volume effects, acquisition and extracranial lipid artifacts, along with clinically relevant aspects such as scan time constraints, are among the challenges associated with in vivo MRSI. Methods The aim of this work was to address some of these factors and to develop an acquisition, reconstruction, and postprocessing pipeline to derive lipid‐suppressed metabolite values of central brain structures based on free‐induction decay measurements made using a 7 T MR scanner. Anatomical images were used to perform high‐resolution (1 mm3) partial‐volume correction to account for gray matter, white matter (WM), and cerebral‐spinal fluid signal contributions. Implementation of automatic quality control thresholds and normalization of metabolic maps from 23 subjects to the Montreal Neurological Institute (MNI) standard atlas facilitated the creation of high‐resolution average metabolite maps of several clinically relevant metabolites in central brain regions, while accounting for macromolecular distributions. Partial‐volume correction improved the delineation of deep brain nuclei. We report average metabolite values including glutamate + glutamine (Glx), glycerophosphocholine, choline and phosphocholine (tCho), (phospo)creatine, myo‐inositol and glycine (mI‐Gly), glutathione, N‐acetyl‐aspartyl glutamate(and glutamine), and N‐acetyl‐aspartate in the basal ganglia, central WM (thalamic radiation, corpus callosum) as well as insular cortex and intracalcarine sulcus. Conclusion MNI‐registered average metabolite maps facilitate group‐based analysis, thus offering the possibility to mitigate uncertainty in variable MRSI data., We use a 7 T MRI scanner along with a combination of advanced acquisition, reconstruction, and postprocessing techniques to derive metabolic distribution in deep brain structures.

Purpose: Inhomogeneous excitation at ultrahigh field strengths (7T and above) compromises the reliability of quantified dynamic contrast-enhanced breast MRI. This can hamper the introduction of ultrahigh field MRI into the clinic. Compensation for this non-uniformity effect can consist of both hardware improvements and post-acquisition corrections. This paper investigated the correctable radiofrequency transmit ((Formula presented.)) range post-acquisition in both simulations and patient data for 7T MRI. Methods: Simulations were conducted to determine the minimum (Formula presented.) level at which corrections were still beneficial because of noise amplification. Two correction strategies leading to differences in noise amplification were tested. The effect of the corrections on a 7T patient data set (N = 38) with a wide range of (Formula presented.) levels was investigated in terms of time-intensity curve types as well as washin, washout and peak enhancement values. Results: In simulations assuming a common amount of T 1 saturation, the lowest (Formula presented.) level at which the SNR of the corrected images was at least that of the original precontrast image was 43% of the nominal angle. After correction, time-intensity curve types changed in 24% of included patients, and the distribution of curve types corresponded better to the distribution found in literature. Additionally, the overlap between the distributions of washin, washout, and peak enhancement values for grade 1 and grade 2 tumors was slightly reduced. Conclusion: Although the correctable range varies with the amount of T 1 saturation, post-acquisition correction for inhomogeneous excitation was feasible down to (Formula presented.) levels of 43% of the nominal angle in vivo.

Purpose This study evaluates the performance of 2 processing methods, that is, principal component analysis‐based denoising and Wiener deconvolution, to enhance the quality of phosphorus 3D chemical shift imaging data. Methods Principal component analysis‐based denoising increases the SNR while maintaining spectral information. Wiener deconvolution reduces the FWHM of the voxel point spread function, which is increased by Hamming filtering or Hamming‐weighted acquisition. The proposed methods are evaluated using simulated and in vivo 3D phosphorus chemical shift imaging data by 1) visual inspection of the spatial signal distribution; 2) SNR calculation of the PCr peak; and 3) fitting of metabolite basis functions. Results With the optimal order of processing steps, we show that the effective SNR of in vivo phosphorus 3D chemical shift imaging data can be increased. In simulations, we show we can preserve phosphorus‐containing metabolite peaks that had an SNR < 1 before denoising. Furthermore, using Wiener deconvolution, we were able to reduce the FWHM of the voxel point spread function with only partially reintroducing Gibb‐ringing artifacts while maintaining the SNR. After data processing, fitting of the phosphorus‐containing metabolite signals improved. Conclusion In this study, we have shown that principal component analysis‐based denoising in combination with regularized Wiener deconvolution allows increasing the effective spectral SNR of in vivo phosphorus 3D chemical shift imaging data, with reduction of the FWHM of the voxel point spread function. Processing increased the effective SNR by at least threefold compared to Hamming weighted acquired data and minimized voxel bleeding. With these methods, fitting of metabolite amplitudes became more robust with decreased fitting residuals.

Deuterium metabolic imaging (DMI) is a novel MR-based method to spatially map metabolism of deuterated substrates such as [6,6'-2H2]-glucose in vivo. Compared with traditional 13C-MR-based metabolic studies, the MR sensitivity of DMI is high due to the larger 2H magnetic moment and favorable T1 and T2 relaxation times. Here, the magnetic field dependence of DMI sensitivity and transmit efficiency is studied on phantoms and rat brain postmortem at 4, 9.4 and 11.7 T. The sensitivity and spectral resolution on human brain in vivo are investigated at 4 and 7 T before and after an oral dose of [6,6'-2H2]-glucose. For small animal surface coils (Ø 30 mm), the experimentally measured sensitivity and transmit efficiency scale with the magnetic field to a power of +1.75 and −0.30, respectively. These are in excellent agreement with theoretical predictions made from the principle of reciprocity for a coil noise-dominant regime. For larger human surface coils (Ø 80 mm), the sensitivity scales as a +1.65 power. The spectral resolution increases linearly due to near-constant linewidths. With optimal multireceiver arrays the acquisition of DMI at a nominal 1 mL spatial resolution is feasible at 7 T.

To be able to examine dynamic and detailed brain functions, the spatial and temporal resolution of 7 T MRI needs to improve. In this study, it was investigated whether submillimeter multishot 3D EPI fMRI scans, acquired with high‐density receive arrays, can benefit from a 2D CAIPIRINHA sampling pattern, in terms of noise amplification (g‐factor), temporal SNR and fMRI sensitivity. High‐density receive arrays were combined with a shot‐selective 2D CAIPIRINHA implementation for multishot 3D EPI sequences at 7 T. In this implementation, in contrast to conventional inclusion of extra kz gradient blips, specific EPI shots are left out to create a CAIPIRINHA shift and reduction of scan time. First, the implementation of the CAIPIRINHA sequence was evaluated with a standard receive setup by acquiring submillimeter whole brain T2*‐weighted anatomy images. Second, the CAIPIRINHA sequence was combined with high‐density receive arrays to push the temporal resolution of submillimeter 3D EPI fMRI scans of the visual cortex. Results show that the shot‐selective 2D CAIPIRINHA sequence enables a reduction in scan time for 0.5 mm isotropic 3D EPI T2*‐weighted anatomy scans by a factor of 4 compared with earlier reports. The use of the 2D CAIPIRINHA implementation in combination with high‐density receive arrays, enhances the image quality of submillimeter 3D EPI scans of the visual cortex at high acceleration as compared to conventional SENSE. Both the g‐factor and temporal SNR improved, resulting in a method that is more sensitive to the fMRI signal. Using this method, it is possible to acquire submillimeter single volume 3D EPI scans of the visual cortex in a subsecond timeframe. Overall, high‐density receive arrays in combination with shot‐selective 2D CAIPIRINHA for 3D EPI scans prove to be valuable for reducing the scan time of submillimeter MRI acquisitions., This study investigated whether submillimeter 3D EPI fMRI scans acquired with high‐density receive arrays can still benefit from a CAIPI sampling pattern, in terms of scan time per volume and (temporal) SNR. Background: In MRI, the signal-to-noise ratio (SNR) theoretically increases with B0 field strength. However, because of attenuation of the radiofrequency (RF) fields at 7T, it is not certain if this SNR gain can be realized for prostate imaging. Purpose/Hypothesis: To investigate the SNR gain in prostate imaging at 7T as compared with 3T. It is expected that SNR will improve for prostate imaging at 7T compared with 3T. Study Type: Prospective. Subjects: Four healthy volunteers and one prostate cancer patient. Field Strength/Sequence: All subjects were scanned at 3T and at 7T using optimal coil setups for both field strengths. For all volunteers, proton density-weighted images were acquired for SNR analysis and actual flip angle imaging (AFI) (Formula presented.) maps were acquired for correction of measured SNR values. In the patient, a T2-weighted (T2w) image was acquired at 3T and at 7T. Assessment: SNR was calculated in the prostate region for all volunteers. SNR was normalized for flip angle, receiver bandwidth, and voxel volume. SNR was also calculated for different sensitivity encoding (SENSE) acceleration factors. Statistical Testing: SNR values are represented as the arithmetic mean of SNR values in the prostate. Estimated SNR in the T2w image is calculated as the arithmetic mean of the signal intensity (SI) divided by the standard deviation of the SI in a specified zone. Tumor-to-tissue contrast is calculated as (SItumor+SIzone)/(SItumor-SIzone). Results: An increase in SNR ranging from 1.7-fold to 2.8-fold was measured in the prostate at 7T in comparison to 3T for four volunteers. At 7T, it is possible to achieve a 4-fold SENSE acceleration in the left-right direction with similar SNR to a nonaccelerated 3T image. T2w imaging was done at 3T and 7T in one patient, where improved tumor-to-tissue contrast was demonstrated at 7T. Data Conclusion: SNR improves for prostate imaging at 7T as compared with 3T. Phosphorus (31P) MRSI provides opportunities to monitor potential biomarkers. However, current applications of 31P MRS are generally restricted to relatively small volumes as small coils are used. Conventional surface coils require high energy adiabatic RF pulses to achieve flip angle homogeneity, leading to high specific absorption rates (SARs), and occupy space within the MRI bore. A birdcage coil behind the bore cover can potentially reduce the SAR constraints massively by use of conventional amplitude modulated pulses without sacrificing patient space. Here, we demonstrate that the integrated 31P birdcage coil setup with a high power RF amplifier at 7 T allows for low flip angle excitations with short repetition time (T R) for fast 3D chemical shift imaging (CSI) and 3D T 1‐weighted CSI as well as high flip angle multi‐refocusing pulses, enabling multi‐echo CSI that can measure metabolite T 2, over a large field of view in the body. B 1 + calibration showed a variation of only 30% in maximum B 1 in four volunteers. High signal‐to‐noise ratio (SNR) MRSI was obtained in the gluteal muscle using two fast in vivo 3D spectroscopic imaging protocols, with low and high flip angles, and with multi‐echo MRSI without exceeding SAR levels. In addition, full liver MRSI was achieved within SAR constraints. The integrated 31P body coil allowed for fast spectroscopic imaging and successful implementation of the multi‐echo method in the body at 7 T. Moreover, no additional enclosing hardware was needed for 31P excitation, paving the way to include larger subjects and more space for receiver arrays. The increase in possible number of RF excitations per scan time, due to the improved B 1 + homogeneity and low SAR, allows SNR to be exchanged for spatial resolution in CSI and/or T 1 weighting by simply manipulating T R and/or flip angle to detect and quantify ratios from different molecular species., An integrated custom‐built whole‐body birdcage coil enabled 31P MRSI in large volumes without sacrificing space. The B1+‐field uniformity, comparable to 3 T proton MR birdcage coils, allowed using conventional amplitude modulated pulses. This massively reduced SAR constrains compared to adiabatic pulses. The increase in RF pulses per unit of time enabled fast and multi‐echo MRSI in the body at 7 T. Local liver spectra were acquired. Presented setup creates opportunities for new body‐focused 31P/X‐nuclei MRS methodologies.

Currently, it is difficult to predict effective therapy response to molecular therapies for the treatment of lung cancer based solely on anatomical images. 31P MR spectroscopic imaging could provide as a non‐invasive method to monitor potential biomarkers for early therapy evaluation, a necessity to improve personalized care and reduce cost. However, surface coils limit the imaging volume in conventional 31P MRSI. High‐energetic adiabatic RF pulses are required to achieve flip angle homogeneity but lead to high SAR. Birdcage coils permit use of conventional amplitude modulated pulses, even over large FOV, potentially decreasing overall SAR massively. Here, we investigate the feasibility of 3D 31P MRSI at 7 T in lung carcinoma patients using an integrated 31P birdcage body coil in combination with either a dual‐coil or a 16‐channel receiver. Simulations showed a maximum decrease in SNR per unit of time of 8% for flip angle deviations in short TR low flip‐angle excitation 3D CSI. The minimal SNR loss allowed for fast 3D CSI without time‐consuming calibration steps (>10:00 min.). 31P spectra from four lung carcinoma patients were acquired within 29:00 minutes and with high SNR using both receivers. The latter allowed discrimination of individual phosphodiesters, inorganic phosphate, phosphocreatine and ATP. The receiver array allowed for an increased FOV compared to the dual‐coil receiver. 3D 31P‐CSI were acquired successfully in four lung carcinoma patients using the integrated 31P body coil at ultra‐high field. The increased spectral resolution at 7 T allowed differentiation of multiple 31P metabolites related to phospholipid and energy metabolism. Simulations provide motivation to exclude 31P B1 calibrations, potentially decreasing total scan duration. Employing large receiver arrays improves the field of view allowing for full organ coverage. 31P MRSI is feasible in lung carcinoma patients and has potential as a non‐invasive method for monitoring personalized therapy response in lung tumors., The feasibility of 31P MRSI was demonstrated in four lung carcinoma patients using a 31P body coil in combination with a two‐ or 16‐channel receiver at 7 T. A maximum decrease of 8% SNR/unit of time from simulations motivated excluding B1 calibrations in fast 3D CSI. With improved MRI, 31P MRSI potentially provides a non‐invasive method to monitor biomarkers for early therapy evaluation, aiming for improved personalized care and reduced cost in lung tumor therapy.

Summary: Energy metabolism of the human visual cortex was investigated by performing 31P functional MRS. Introduction: The human brain is known to be the main glucose demanding organ of the human body and neuronal activity can increase this energy demand. In this study we investigate whether alterations in pH during activation of the brain can be observed with MRS, focusing on the mitochondrial inorganic phosphate (Pi) pool as potential marker of energy demand. Methods: Six participants were scanned with 16 consecutive 31P-MRSI scans, which were divided in 4 blocks of 8:36 minutes of either rest or visual stimulation. Since the signals from the mitochondrial compartments of Pi are low, multiple approaches to achieve high SNR 31P measurements were combined. This included: a close fitting 31P RF coil, a 7 T-field strength, Ernst angle acquisitions and a stimulus with a large visual angle allowing large spectroscopy volumes containing activated tissue. Results: The targeted resonance downfield of the main Pi peak could be distinguished, indicating the high SNR of the 31P spectra. The peak downfield of the main Pi peak is believed to be connected to mitochondrial performance. In addition, a BOLD effect in the PCr signal was observed as a signal increase of 2–3% during visual stimulation as compared to rest. When averaging data over multiple volunteers, a small subtle shift of about 0.1 ppm of the downfield Pi peak towards the main Pi peak could be observed in the first 4 minutes of visual stimulation, but no longer in the 4 to 8 minute scan window. Indications of a subtle shift during visual stimulation were found, but this effect remains small and should be further validated. Conclusion: Overall, the downfield peak of Pi could be observed, revealing opportunities and considerations to measure specific acidity (pH) effects in the human visual cortex.

Radiofrequency (RF) coils are an essential MRI hardware component. They directly impact the spatial and temporal resolution, sensitivity, and uniformity in MRI. Advances in RF hardware have resulted in a variety of designs optimized for specific clinical applications. RF coils are the “antennas” of the MRI system and have two functions: first, to excite the magnetization by broadcasting the RF power (Tx‐Coil) and second to receive the signal from the excited spins (Rx‐Coil). Transmit RF Coils emit magnetic field pulses ( B1+) to rotate the net magnetization away from its alignment with the main magnetic field (B0), resulting in a transverse precessing magnetization. Due to the precession around the static main magnetic field, the magnetic flux in the receive RF Coil ( B1−) changes, which generates a current I. This signal is “picked‐up” by an antenna and preamplified, usually mixed down to a lower frequency, digitized, and processed by a computer to finally reconstruct an image or a spectrum. Transmit and receive functionality can be combined in one RF Coil (Tx/Rx Coils). This review looks at the fundamental principles of an MRI RF coil from the perspective of clinicians and MR technicians and summarizes the current advances and developments in technology. The brain can be considered a network, existing of multiple interconnected areas with various functions. MRI provides opportunities to map the large-scale network organization of the brain. We tap into the neurobiochemical dimension of these networks, as neuronal functioning and signal trafficking across distributed brain regions relies on the release and presence of neurotransmitters. Using high-field MR spectroscopic imaging at 7.0 T, we obtained a non-invasive snapshot of the spatial distribution of the neurotransmitters GABA and glutamate, and investigated interregional associations of these neurotransmitters. We demonstrate that interregional correlations of glutamate and GABA concentrations can be conceptualized as networks. Furthermore, patients with epilepsy display an increased number of glutamate and GABA connections and increased average strength of the GABA network. The increased glutamate and GABA connectivity in epilepsy might indicate a disrupted neurotransmitter balance. In addition to epilepsy, the ‘neurotransmitter networks’ concept might also provide new insights for other neurological diseases., Highlights • We introduce the concept of “neurotransmitter networks”. • Ultrahigh field spectroscopic images of the brain were obtained. • Interregional correlations of Glutamate and GABA are conceptualized as networks. • Increased glutamate and GABA connectivity are shown in patients with epilepsy. • Neurotransmitter networks might provide new insights for neurological diseases.

Purpose 13 C magnetic resonance spectroscopy (MRS) in combination with infusion of 13 C-labeled substrates has led to unique insights into human brain metabolism and neurotransmitter cycling. However, the low sensitivity of direct 13 C MRS and high radiofrequency power requirements has limited 13 C MRS studies to predominantly data acquisition in large volumes of the occipital cortex. The purpose of this study is to develop an MRS technique for localized detection of 13 C-labeling of glutamate and glutamine in the human frontal lobe. Methods We used an indirect (1 H-[13 C]), proton-observed, carbon-edited MRS sequence (selPOCE) for detection of 13 C-labeled metabolites in relatively small volumes located in the frontal lobe at 4 T. The SelPOCE method allows for selective and separate detection of glutamate and glutamine resonances, which significantly overlap at magnetic field strengths used for clinical MRI. Results Phantom data illustrate how selPOCE can be tuned to selectively detect 13 C labeling in different metabolites. Three-dimensional specific absorption rate simulations of radiofrequency power deposition show that the selPOCE method operates comfortably within the global and local Food and Drug Administration specific absorption rate guidelines. In vivo selPOCE data are presented, which were acquired from a 45-mL volume in the frontal lobe of healthy subjects. The in vivo data show the time-dependent 13 C-labeling of glutamate and glutamine during intravenous infusion of [1-13 C]-glucose. Metrics describing spectral fitting quality of the glutamate and glutamine resonances are reported. Conclusions The SelPOCE sequence allows the detection of 13 C-labeling in glutamate and glutamine from a relatively small volume in the human frontal lobe at low radiofrequency power requirements. Patients with psychotic disorders often show prominent cognitive impairment. Glutamate seems to play a prominent role, but knowledge on its role in deep gray matter regions is limited and previous studies have yielded heterogeneous results. The aim was to evaluate glutamate levels within deep gray matter structures in patients with a psychotic disorder in relation to cognitive functioning, using advanced spectroscopic acquisition, reconstruction and post-processing techniques. A 7 tesla MRI scanner combined with a unique lipid suppression coil and subject specific water signal suppression pulses were used to acquire high-resolution magnetic resonance spectroscopic imaging data. Anatomical scans were used to perform tissue fraction correction and registration to a standard brain for group comparison in specifically delineated brain regions. The brief assessment of cognition in schizophrenia was used to evaluate cognitive status. Average glutamate levels across deep gray matter structures (i.e. caudate, pallidum, putamen, and thalamus) in patients with a psychotic disorder (n=16, 4 females) were lower compared to healthy controls (n=23, 7 females). Stratified analyses showed lower glutamate levels in the caudate and putamen but not in the pallidum or thalamus. Average glutamate levels across deep gray matter structures were positively correlated with cognition, particularly to psychomotor speed. We find reduced glutamate levels across deep brain structures such as the caudate and putamen in patients with a psychotic disorder that are linked to psychomotor speed. Our results underscore the potential role of detailed in vivo glutamate assessments to understand cognitive deficits in patients with psychotic disorders.

Purpose To develop short water suppression sequences for 7 T magnetic resonance spectroscopic imaging, with mitigation of subject-specific transmit RF field ( B1+) inhomogeneity. Methods Patient-tailored spiral in-out spectral-spatial saturation pulses were designed for a three-pulse WET water suppression sequence. The pulses' identical spatial subpulses were designed using patient-specific B1+ maps and a spiral in-out excitation k-space trajectory. The subpulse train was weighted by a spectral envelope that was root-flipped to minimize peak RF demand. The pulses were validated in in vivo experiments that acquired high resolution magnetic resonance spectroscopic imaging data, using a crusher coil for fast lipid suppression. Residual water signals and MR spectra were compared between the proposed tailored sequence and a conventional WET sequence. Results Replacing conventional spectrally-selective pulses with tailored spiral in-out spectral-spatial pulses reduced mean water residual from 5.88 to 2.52% (57% improvement). Pulse design time was less then 0.4 s. The pulses' specific absorption rate were compatible with magnetic resonance spectroscopic imaging TRs under 300 ms, which enabled spectra of fine in plane spatial resolution (5 mm) with good quality to be measured in 7.5 min. Conclusion Tailored spiral in-out spectral-spatial water suppression enables efficient high resolution magnetic resonance spectroscopic imaging in the brain. Purpose Robust fat suppression is essential in bilateral breast MRI at 7 Tesla. The lack of good fat suppression can result in errors when calculating the enhancement curve from dynamic contrast-enhanced acquisitions. In this work we propose discretized frequency shimming to improve the quality of fat suppression by exploiting the capabilities of a parallel transmit setup. Methods With discretized frequency shimming, different resonance frequencies are used for the different transmitters of the radiofrequency coil. This can be particularly useful for frequency-selective radiofrequency pulses, such as water excitation. The potential of this method has been investigated by simulations on seven acquired B0 maps. Prospective experiments on phantoms and four healthy volunteers were performed to illustrate the performance of discretized frequency shimming. Results Simulations show a decrease in off-resonance excitation for all subjects when discretized frequency shimming is used. This decrease is highly subject-dependent, but can result in up to 30% better fat suppression, as observed in phantom and volunteer experiments. Conclusions Discretized frequency shimming improves fat suppression in bilateral breast MRI at 7 Tesla. Background The quality of carotid wall MRI can benefit substantially from a dedicated RF coil that is tailored towards the human neck geometry and optimized for image signal-to-noise ratio (SNR), parallel imaging performance and RF penetration depth and coverage. In last decades, several of such dedicated carotid coils were introduced. However, a comparison of the more successful designs is still lacking. Objective To perform a head-to-head comparison over four dedicated MR carotid surface coils with 4, 6, 8 and 30 coil elements, respectively. Material and methods Ten volunteers were scanned on a 3T scanner. For each subject, multiple black-blood carotid vessel wall images were measured using the four coils with different parallel imaging settings. The performance of the coils was evaluated and compared in terms of image coverage, penetration depth and noise correlations between elements. Vessel wall of a common carotid section was delineated manually. Subsequently, images were assessed based on vessel wall morphology and image quality parameters. The morphological parameters consisted of the vessel wall area, thickness, and normalized wall index (wall area/total vessel area). Image quality parameters consisted of vessel wall SNR, wall-lumen contrast-to-noise ratio (CNR), the vessel g-factor, and CNR index ((wall–lumen signal) / (wall+lumen signal)). Repeated measures analysis of variance (rmANOVA) was applied for each parameter for the averaged 10 slices for all volunteers to assess effect of coil and SENSE factor. If the rmANOVA was significant, post-hoc comparisons were conducted. Results No significant coil effect were found for vessel wall morphological parameters. SENSE acceleration affected some morphological parameters for 6- and 8-channel coils, but had no effect on the 30-channel coil. The 30-channel coil achieved high acceleration factors (10x) with significantly lower vessel g-factor values (ps 0.01), but lower vessel wall SNR and CNR values (ps 0.01). Conclusion All four coils were capable of high-quality carotid MRI. The 30-channel coil is recommended when rapid image acquisition acceleration is required for 3D measurements, whereas 6- and 8-channel coils demonstrated the highest SNR performance.
