The increasing prevalence of the highly antibody-evasive Omicron sublineages increases the risk of breakthrough infections and leaves high-risk and vulnerable immunocompromised individuals with no effective options for prophylactic or therapeutic antibody treatments. Here, we report a heavily mutated anti-RBD monoclonal antibody, Acovimab, directed against a site in the receptor-binding motif (RBM) region of the CoV2 receptor-binding domain (RBD), that possesses very broad and highly potent neutralizing activity against CoV2 variants, including many Omicron variants. This antibody is derived from the IGHV1-58*01 germline sequence and possesses a relatively high level of mutation (15.5% of the VH aa sequence), which is unusual for anti-RBD antibodies. Neutralizing activity was very potent (IC50s range of 1-9 ng/ml) for early Omicron subvariants that possess an unmutated F486 residue and is retained but less potent (IC50s of 200-650 ng/ml) for more resistant Omicron subvariants which contain the F486V mutation (BA4/5, BA4.6, and BQ1.1), but is lost for the later ultra-resistant variants that contain F486S (XBB) or F486P (XBB.1.5) mutations. Based on these specificities, it is predicted that Acovimab by itself should protect against CoV2 infections other than those caused by the XBB/XBB.1.5 family. Acovimab also shows strong synergy in neutralization when combined with Sotrovimab, which neutralizes all Omicron variants, including XBB.1.5. Plasma from subjects with hybrid immunity (induced by vaccination + infection) possessed low levels of XBB.1.5 RBM-targeting plasma-neutralizing antibodies, and these also neutralized synergistically when combined with Sotrovimab. These results suggest potentially novel immunotherapeutic options for treating most of the CoV2 variants responsible for current infections.

Immunocompromised cancer patients are at significant risk of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. A method to identify those patients at highest risk is needed so that prophylactic measures may be employed. Serum antibodies to SARS-CoV-2 spike protein are important markers of protection against COVID-19 disease. We evaluated total and neutralizing antibody levels pre and post third booster vaccine and compared responses among different cancer-bearing and healthy veterans. This as a prospective, single site, comparative cohort observational trial. The setting was the West Palm Beach VA Medical Center cancer center. All veterans received a third SARS-CoV-2 mRNA booster. The main outcomes were anti-SARS-CoV-2 spike IgG and neutralizing antibodies to wild-type, and B.1.617, BA1, BA2, and BA4/5 variants were measured. Disease type and therapy, COVID-19 infection, and anti-CD20 antibody treatments were documented. The third mRNA vaccine booster increased the mean blood anti-spike IgG five-fold. The second anti-spike level was equal or greater than the first in 129/140 veterans. All the groups except the myeloma group, had post-booster antibody levels significantly higher than pre-booster with 4-fold, 12-fold, 4-fold, 6-fold and 3.5-fold increases for the control, solid tumor, CLL, B cell lymphoma and all B cell malignancy cohorts. The myeloma set showed only a non-significant 1.7-fold increase. Recently anti-CD20 antibody-treated patients were shown to have approximately 200-fold less anti-S IgG production after vaccine booster than other patients. There was a 2.5-fold enhancement of wild-type virus mean neutralizing antibodies after a third mRNA booster and mean neutralization of Delta and Omicron variants increased 2.2, 6.5, 7.7, and 6.2-fold versus pre-boost levels. B cell malignancies failed to show increased post-booster neutralization. The third SARS CoV-2 booster increased total anti-spike IgG and neutralizing antibodies for most subjects. Veterans with B cell malignancies particularly myeloma and those receiving anti-CD20 monoclonal antibodies had the weakest humoral responses. Neutralizing antibody responses to Omicron variants were less than for wild-type virus. A subset of patients without humoral immunity post-booster should be considered for prophylactic antibody or close monitoring.

Therapeutic proteins, including monoclonal antibodies, are typically manufactured using clonally-derived, stable host cell lines, since consistent and predictable cell culture performance is highly desirable. However, selecting and preparing banks of stable clones takes considerable time, which inevitably extends overall development timelines for new therapeutics by delaying the start of subsequent activities, such as the scale-up of manufacturing processes. In the context of the COVID-19 pandemic, with its intense pressure for accelerated development strategies, we used a novel transposon-based Leap-In Transposase® system to rapidly generate high-titer stable pools and then used them directly for large scale-manufacturing of an anti-SARS-CoV2 monoclonal antibody under cGMP. We performed the safety testing of our non-clonal cell bank, then used it to produce material at a 200L-scale for pre-clinical safety studies and formulation development work, and thereafter at 2000L scale for supply of material for a Phase 1 clinical trial. Testing demonstrated the comparability of critical product qualities between the two scales and, more importantly, that our final clinical trial product met all pre-set product quality specifications. The above expediated approach provided clinically-ready material within 4.5 months, in comparison to 12-14 months for production of clinical trial material via the conventional approach.

IAVI’s NAC provides a multi-disciplinary platform to decipher the interaction of bNAbs with HIV Env and accelerate translation to development of vaccines capable of eliciting potent neutralizing abs. Recent successes highlight the value of a consortium to expedite progress. IAVI initiated Protocol G to identify neutralizing sera from HIV-1 infected patients. Using these samples, a cross-disciplinary team discovered abs with unprecedented potency against HIV. PG9 was isolated from a subject infected with HIV-1 clade A. Consortium members reasoned that Env sequences from the clade A infected donor could lead to immunogen discovery. Using bioinformatics, MRCA sequence was aligned with 99 subtype A gp160 sequences from the Los Alamos HIV database and BG505 was identified with the highest homology. Soluble BG505 retains features of native trimer. While native Env trimer is the target for bnAbs, its instability impedes efforts to develop recombinant Env. Because BG505 retains features of native trimer, it might be optimal for immunogen design. NAC partners report construction of a soluble trimer from BG505 that binds PG9 with high affinity and show it is uniquely stable. Formulations of gp120 and trimers are under analysis for immunogenicity. Challenges remain in development including analysis of structure, immunogenicity, ab responses elicited; and production for human testing. A consortium approach of multidisciplinary experience will be required to expedite advancement of vaccine concepts.

The limited success of vaccines targeting the MPER, a target of three broadly neutralizing (bNt) monoclonal antibodies (MAbs), we hypothesize, reflects the difficulty of mimicking the neutralization-competent structure (NCS) of the MPER. We determined the contribution of the amino-acid sequence and the transmembrane domain (TM), to the antigenicity of the MPER in the context of the plasma membrane. DNA constructs encoding various gp41 ectodomain fragments, and the TM of either the platelet-derived growth factor receptor (PGDFR), or that of gp41, were produced and transiently expressed in COS-7 cells. Constructs expressing the MPER tethered to the gp41 TM followed by a 27-residue cytoplasmic tail fragment, MPER-TM1, produced optimal binding of MAbs 2F5, 4E10 and Z13e1. A series of 24 single amino-acid substitutions in the MPER-TM1 revealed critical binding residues for the three MAbs; similar substitutions were previously shown to ablate Ab-mediated viral neutralization. Neutralization-incompetent 2F5 Fab and 4E10 IgG mutant Abs failed to bind MPER-TM1, yet retained the ability to bind to peptide epitopes, indicating the plasma-membrane expressed MPER-TM1 closely approaches the NCS of the MPER. Substitution of the TM of gp41 with that from the PGDFR reduced binding by MAb 4E10, but not MAbs 2F5 or Z13e1. Our studies reveal that the gp41 TM appears to play a pivotal role both in orienting the 4E10 epitope, and affecting exposure of the MPER epitopes for all three bNtMAbs.

Researchers in historical source materials in the United States depend, at present, mainly on two publications for summary information about historical records. The first of these, the National Union Catalog of Manuscript Collections1 (hereinafter NUCMC), is a national manuscript catalog which prints detailed information about an average of slightly over 2,000 collections per year. The second publication, A Guide to Archives and Manuscripts in the United States,2 compiled by the National Historical Publication Commission's director, Philip M. Hamer, and now dated and out of print, identifies the papers of nearly 8,000 individuals as well as archives and records of numerous institutions and organizations in about 1,300 repositories throughout the country. In addition to these two publications, the researcher is faced with an ever-increasing array of published repository catalogs and guides as well as regional and topical guides, all of varying quality, currency, and format,3 unknown or inaccessible, in many cases, to the average user. There is no one national guide to repositories nor is there one at the collection/record group level to which a researcher may turn for a reasonably complete and current listing of repositories and their records.4 The National Historical Publications and Records Commission5 feels that such a
