Introduction: On 10thFebruary 2017, Uganda Ministry of Health was notified of a suspected cholera outbreak in Nebbi district. The district experienced numerous cholera outbreaks with the latest in 2016. We investigated to determine the scope, mode of transmission, and exposures. Methods: We defined a suspected case as sudden onset of acute watery diarrhoea in a resident (>5 years) from Parombo or Panyimur subcounties in Nebbi district, during 1 January-9 March 2017. A confirmed case was a suspected case with culture-confirmed Vibrio cholerae from stool. We conducted descriptive epidemiology of case-persons to inform hypothesis generation and a case-control study involving 67 case-persons and 134 control-persons to test the hypothesis. Results: We identified 222 suspected case-persons; samples from two yielded Vibrio cholerae O139. Three case-persons died (CFR=1.4%). The epidemic curve indicated a point-source outbreak. Among 67 cases, 40 (60%) drank river water, compared with 56 (42%) of 134 controls (OR 2.2, 95% CI: 1.2- 4.1). Visual assessment revealed that river water had high turbidity and we observed mass open defecation. Conclusion: This outbreak affected two sub-counties and was associated with drinking contaminated river water. We recommended treating drinking water by the community members and health education on drinking water safety and proper waste disposal in the communities. And for long term, the district water department should increase the number and functionality of boreholes or piped water in the communities.

Introduction. Rift Valley fever (RVF) is a mosquito-borne viral zoonosis. The Uganda Ministry of Health received alerts of suspected viral haemorrhagic fever in humans from Kiruhura, Buikwe, Kiboga, and Mityana districts. Laboratory results from Uganda Virus Research Institute indicated that human cases were positive for Rift Valley fever virus (RVFV) by polymerase chain reaction. We investigated to determine the scope of outbreaks, identify exposure factors, and recommend evidence-based control and prevention measures. Methods. A suspected case was defined as a person with acute fever onset, negative malaria test result, and at least two of the following symptoms: headache, muscle or joint pain, bleeding, and any gastroenteritis symptom (nausea, vomiting, abdominal pain, diarrhoea) in a resident of Kiruhura, Buikwe, Mityana, and Kiboga districts from 1st October 2017 to 30th January 2018. A confirmed case was defined as a suspected case with laboratory confirmation by either detection of RVF nucleic acid by reverse-transcriptase polymerase chain reaction (RT-PCR) or demonstration of serum IgM or IgG antibodies by ELISA. Community case finding was conducted in all affected districts. In-depth interviews were conducted with human cases that were infected with RVF who included herdsmen and slaughterers/meat handlers to identify exposure factors for RVF infection. A total of 24 human and 362 animal blood samples were tested. Animal blood samples were purposively collected from farms that had reported stormy abortions in livestock and unexplained death of animals after a short illness (107 cattle, 83 goats, and 43 sheep). Convenient sampling for the wildlife (10 zebras, 1 topi, and 1 impala) was conducted to investigate infection in animals from Kiruhura, Buikwe, Mityana, and Kiboga districts. Human blood was tested for anti-RVFV IgM and IgG and animal blood for anti-RVFV IgG. Environmental assessments were conducted during the outbreaks in all the affected districts. Results. Sporadic RVF outbreaks occurred from mid-October 2017 to mid-January 2018 affecting humans, domestic animals, and wildlife. Human cases were reported from Kiruhura, Buikwe, Kiboga, and Mityana districts. Of the 24 human blood samples tested, anti-RVFV IgG was detected in 7 (29%) human samples; 1 human sample had detectable IgM only, and 6 had both IgM and IgG. Three of the seven confirmed human cases died among humans. Results from testing animal blood samples obtained from Kiruhura district indicated that 44% (64/146) cattle, 46% (35/76) goats, and 45% (9/20) sheep tested positive for RVF. Among wildlife, (1/10) zebras, (1/1) topi, and (1/1) impala tested positive for RVFV by serological tests. One blood sample from sheep in Kiboga district tested RVFV positive. All the human cases were exposed through contact or consumption of meat from infected animals. Conclusion. RVF outbreaks occurred in humans and animals in Kiruhura, Buikwe, Mityana, and Kiboga districts. Human cases were potentially infected through contact with infected animals and their products.

Introduction: Gender-based violence (GBV) in Uganda is widespread. The 2012 Uganda Bureau of Statistics report indicated that 60% of Ugandan women experienced GBV, which is approximately twice the global rate. There is paucity of information on epidemiology of injuries due to GBV in Uganda. We studied trends and distribution of injuries due to GBV in Uganda. Methods: We conducted a retrospective descriptive analysis using routinely-collected surveillance data from the Uganda Health Management Information System over five years (2012–2016). We calculated prevalence and annual injury rates due to GBV per 10,000 population and compared them across different population segments and time periods. We used logistic regression to determine trends. Results: In 2016, GBV resulted in 49,842 hospital visits in Uganda. Injury rates due to GBV were 13.5 per 10,000 population for the year 2012, and 13.6 per 10,000 population for 2016. Injury rates declined slightly from 2012 to 2016 (OR: 0.995, 95% CI: 0.991–0.997). In 2016, the odds of injuries due to GBV were 1.36 times in females compared to males (OR: 1.36, 95% CI: 1.33–1.38). Northern and Eastern regions (17.4 and 16.9 per 10,000, respectively) had higher GBV-associated injury burden than Central and Western regions (11.0 and 10.1 per 10,000 population, respectively). Conclusion: The incidence of injuries due to GBV in Uganda has not changed meaningfully from 2012 to 2016. We recommend development of interventions for GBV prevention targeting both males and females. There is a need for targeted efforts in the Eastern and Northern region.

Background The Uganda National Malaria Control Programme recognizes the importance of minimizing the effect of malaria among pregnant women. Accordingly, strategies including intermittent preventive treatment of malaria in pregnancy using sulfadoxine-pyrimethamine (IPTp-SP) have been scaled up. Uptake of IPTp-SP among pregnant women in Uganda, aged 15–49 years who had had a live birth 2 years preceding the 2016 Uganda Demographic and Health Survey (UDHS) was determined and factors associated with the uptake of optimal IPTp-SP doses were identified. Methods This was a secondary analysis of the UDHS 2016 dataset. The outcome variable was uptake of IPTp-SP doses among women 15–49 years old who had had a live birth 2 years preceding the survey. Independent variables were residence type, age, marital status, education, wealth status, region of residence, parity, number of antenatal care (ANC) attendance, timing to first ANC visit, and exposure to messages through radio. Logistic regression was used to identify factors associated with the uptake of optimal IPTp-SP doses. Results Uptake of three or more doses of IPTp-SP was 18%. The likelihood of taking optimal doses of IPTp-SP was increased among those who had attained a secondary-level education (aOR: 1.5, 95% CI 1.04–2.15), those who attended ANC ≥ 4 times (aOR: 1.34, 95% CI 1.12–1.60), and those exposed to radio messages (aOR: 1.23, 95% CI 1.02–1.48). Among those in the age category > 34 years (aOR: 0.70, 95% CI 0.53–0.92), and those who attended first ANC in the third trimester of pregnancy (aOR: 0.58, 95% CI 0.38–0.87) the odds of uptake were decreased. Conclusions Education status, exposure to radio messages about health and frequency of ANC attendance were associated with increased uptake while timing of first ANC attendance and being > 34 years were associated with decreased uptake. The findings suggest a need to strengthen behaviour change communication among women of child-bearing age in order to improve uptake of IPTp-SP during pregnancy.

Background: Malaria ranks among the leading global public health challenges. Resistance to insecticides used in vector control by malaria vectors threatens the effectiveness of this intervention. We analyzed data from routine susceptibility tests conducted in sentinel sites in five Ugandan districts to determine the status and pattern of insecticide resistance among Anopheles gambiae mosquito vectors, and to assess the trend of mortality rates of the vector. Methods: We conducted a cross-sectional study using secondary data from sentinel sites in Apac, Kanungu, Hoima, Tororo, and Wakiso Districts of Uganda. Chemicals from different classes of insecticides were subjected to susceptibility tests, which included both the World Health Organization (WHO) standard test kit and the Centers for Disease Control and Prevention (CDC) bottle bioassay tests. Resistance was defined according to the standard WHO criteria of insecticide resistance. The Fisher’s Exact Test was used to determine the difference in mortality rates across years in the study period.Results: A total of eight chemicals were used in the selected districts over the years of study. Out of the 5 districts, we found that the average mortality rate of the mosquito vector by the carbamates was over 98% in 3 districts. In Tororo and Wakiso Districts, the average was about 90%. Mortality of mosquitoes by pyrethroids used averaged less than 80% in all five districts. The organochlorines averaged less than 50% in four out of five districts. In Apac District, organochlorines averaged a mortality rate of 94%. The organophosphates averaged 100% mortality in all sentinel sites. There was no specific pattern in mortality of Anopheles gambiae by insecticides.Conclusion: There was widespread resistance to pyrethroids and organochlorines, with patchy resistance observed against carbamates. Only organophosphates showed potency in all sentinel sites. This threatens gains made in malaria control, and renews calls for continued insecticide resistance monitoring.

Background Measles is a highly infectious viral disease. In August 2017, Lyantonde District, Uganda reported a measles outbreak to Uganda Ministry of Health. We investigated the outbreak to assess the scope, factors facilitating transmission, and recommend control measures. Methods We defined a probable case as sudden onset of fever and generalized rash in a resident of Lyantonde, Lwengo, or Rakai Districts from 1 June-30 September 2017, plus ≥1 of the following: coryza, conjunctivitis, or cough. A confirmed case was a probable case with serum positivity of measles-specific IgM. We conducted a neighborhood- and age-matched case-control study to identified exposure factors, and used conditional logistic regression to analyze the data. We estimated vaccine effectiveness and vaccination coverage. Results We identified 81 cases (75 probable, 6 confirmed); 4 patients (4.9%) died. In the case-control study, 47% of case-patients and 2.3% of controls were hospitalized at Lyantonde Hospital pediatric department for non-measles conditions 7–21 days before case-patient’s onset (ORadj = 34, 95%CI: 5.1–225). Estimated vaccine effectiveness was 95% (95%CI: 75–99%) and vaccination coverage was 76% (95%CI: 68–82%). During the outbreak, an “isolation” ward was established inside the general pediatric ward where there was mixing of both measles and non-measles patients. Conclusions This outbreak was amplified by nosocomial transmission and facilitated by low vaccination coverage. We recommended moving the isolation ward outside of the building, supplemental vaccination, and vaccinating pediatric patients during measles outbreaks.

Background Anthrax is a vaccine-preventable disease that can be transmitted to humans from infected animals. During May–June 2017, three persons with suspected cutaneous anthrax were reported in Arua District, Uganda; one died. All had recently handled carcasses of livestock that died suddenly. During July, a bull in the same community died suddenly. A skin lesion from the deceased person and a blood sample from the bull tested positive by PCR for Bacillus anthracis. The aim of this investigation was to establish the scope of the problem, identify exposures associated with illness, and recommend evidence-based control measures.Methods We defined a probable case as acute onset of a papulo-vesicular skin lesion subsequently forming an eschar in a resident of Arua District during January 2015–August 2017. A confirmed case was a probable case with a skin sample testing positive by polymerase chain reaction (PCR) for B. anthracis . We identified cases by medical record review and active community search. In a case-control study, we compared exposures between case-patients and frequency- and village-matched asymptomatic controls. We interviewed key animal health staff to learn about livestock deaths.Results We identified 68 case-patients (67 probable, one confirmed), and two deaths. Cases occurred throughout the three-year period, peaking during dry seasons. All cases occurred following sudden livestock deaths in the villages. Case-patients came from two neighboring sub-counties: Rigbo (attack rate (AR)=20.1/10,000 population) and Rhino Camp (AR=2.1/10,000). Males (AR=17/100,000) were more affected than females (AR=0.52/100,000). Persons aged 30-39 years (AR=26/100,000 population) were most affected. Among all cases and 136 controls, skinning (OR M-H =5.0, 95%CI: 2.3–11), butchering (OR M-H =22, 95%CI: 5.5–89), and carrying the carcass of livestock that died suddenly (OR M-H =6.9, 95%CI: 3.0–16) were associated with illness.Conclusions Exposure to carcasses of animals that died suddenly was a risk factor for cutaneous anthrax in Arua District during 2015-2017. We recommended public education about signs and symptoms of anthrax, prompt treatment of illness following exposure to animals that died suddenly, and safe disposal of carcasses of animals that die suddenly. In addition, the anthrax burden in livestock should be investigated to inform the cost-effectiveness of livestock vaccination.

Conclusions Exposure to carcasses of animals that died suddenly was a likely risk factor for cutaneous anthrax in Arua District during 2015–2017. The recommendations are investigation of anthrax burden in livestock, prevention of animal infections through vaccinations, safe disposal of the carcasses, public education on risk factors for infection and prompt treatment of illness following exposure to animals that died suddenly., Author summary We are honored to submit an original research article entitled “Cutaneous anthrax associated with handling carcasses of animals that died suddenly of unknown cause: Arua District, Uganda, January 2015–August 2017”. Our paper is based on 68 cutaneous anthrax case-patients and cases occurred throughout the three-year period, peaking during dry seasons in Arua District of Uganda. All cases occurred following sudden livestock deaths in the villages. Our findings are based on outbreak investigation which gives a better representation of the actual situation. Our article looks at the scope of the anthrax outbreak, exposures associated with illness, and evidence-based control measures. In this manuscript, we show that exposure to carcasses of animals that died suddenly was a risk factor for cutaneous anthrax in Arua District during 2015–2017. We believe the content and objectives of our work provide knowledge on epidemiology of cutaneous anthrax.

Background. Malaria is a leading cause of morbidity and mortality in Uganda. In April 2018, malaria cases surged in Nwoya District, Northern Uganda, exceeding expected limits and thereby requiring epidemic response. We investigated this outbreak to estimate its magnitude, identify exposure factors for transmission, and recommend evidence-based control measures. Methods. We defined a malaria case as onset of fever in a resident of Anaka subcounty, Koch Goma subcounty, and Nwoya Town Council, Nwoya District, with a positive rapid diagnostic test or microscopy for malaria from 1 February to 25 May 2018. We reviewed medical records in all health facilities of affected subcounties to find cases. In a case-control study, we compared exposure factors between case-persons and asymptomatic controls matched by age and village. We also conducted entomological assessments on vector density and behavior. Results. We identified 3,879 case-persons (attack rate [AR] = 6.5%) and two deaths (case-fatality rate = 5.2/10,000). Females (AR = 8.1%) were more affected than males (AR = 4.7%) (p<0.0001). Of all age groups, 5–18 years (AR = 8.4%) were most affected. Heavy rain started in early March 2018, and a propagated outbreak followed in the first week of April 2018. In the case-control study, 55% (59/107) of case-persons and 18% (19/107) of controls had stagnant water around households for several days following rainfall (ORM-H = 5.6, 95% CI = 3.0–11); 25% (27/107) of case-persons and 51% (55/107) of controls wore full extremity covering clothes during evening hours (ORM-H = 0.30, 95% CI = 0.20–0.60); 71% (76/107) of case-persons and 85% (91/107) of controls slept under a long-lasting insecticide-treated net (LLIN) 14 days before symptom onset (ORM-H = 0.43, 95% CI = 0.22–0.85); 37% (40/107) of case-persons and 52% (56/107) of controls had access to at least one LLIN per 2 household members (ORM-H = 0.54, 95% CI = 0.30–0.97). Entomological assessment indicated active breeding sites in the entire study area; Anopheles gambiae sensu lato species were the predominant vector. Conclusion. Increased vector-breeding sites after heavy rainfall and inadequate malaria preventive measures were found to have contributed to this outbreak. We recommended increasing coverage for LLINs and larviciding breeding sites in the area.

Background In August 2017, the Uganda Ministry of Health was notified of increased cases of multidrug-resistant tuberculosis (MDR-TB) in Arua District, Uganda during 2017. We investigated to identify the scope of the increase and risk factors for infection, evaluate health facilities’ capacity to manage MDR-TB, and recommend evidence-based control measures. Methods We defined an MDR-TB case-patient as a TB patient attending Arua Regional Referral Hospital (ARRH) during 2013–2017 with a sputum sample yielding Mycobacterium tuberculosis resistant to at least rifampicin and isoniazid, confirmed by an approved drug susceptibility test. We reviewed clinical records from ARRH and compared the number of MDR-TB cases during January–August 2017 with the same months in 2013–2016. To identify risk factors specific for MDR-TB among cases with secondary infection, we conducted a case-control study using persons with drug-susceptible TB matched by sub-county of residence as controls. We observed infection prevention and control practices in health facilities and community, and assessed health facilities’ capacity to manage TB. Results We identified 33 patients with MDR-TB, of whom 30 were secondary TB infection cases. The number of cases during January–August 2017 was 10, compared with 3–4 cases in January–August from 2013 to 2016 (p = 0.02). Men were more affected than women (6.5 vs 1.6/100,000, p 15 months from symptom onset (aOR = 11, 95% CI: 1.5–87) were associated with MDR-TB. All ten facilities assessed reported stockouts of TB commodities. All 15 ambulatory MDR-TB patients we observed were not wearing masks given to them to minimize community infection. The MDR-TB ward at ARRH capacity was 4 patients but there were 11 patients. Conclusion The number of cases during January–August in 2017 was significantly higher than during the same months in 2013–2016. Poor adherence to TB drugs and delayed treatment initiation were associated with MDR-TB infection. We recommended strengthening directly-observed treatment strategy, increasing access to treatment services, and increasing the number of beds in the MDR-TB ward at ARRH.
