Abstract

Conclusion: Contralateral temporal lobe activation decreases with aging, regardless of hearing status, with elderly individuals showing reduced right ear advantage.Background: Aging and hearing loss possibly lead to presbycusis speech discrimination decline.Objectives: To evaluate presbycusis patients’ auditory cortex activation under verbal stimulation.Method: Thirty-six patients were enrolled: 10 presbycusis patients (mean age = 64 years, range = 60–70), 10 in the healthy aged group (mean age = 66 years, range = 60–70), and 16 young healthy volunteers (mean age = 25 years, range = 23–28). These three groups underwent simultaneous 1 kHz and 90 dB single-syllable word stimuli and (blood-oxygen-level-dependent functional magnetic resonance imaging) BOLD fMRI examinations.Results: The main activation regions were superior temporal and middle temporal gyrus. For all aged subjects, the right region of interest (ROI) activation volume was decreased compared with the young group. With left ear stimulation, bilateral ROI activation intensity held. With right ear stimulation, the aged group’s activation intensity was higher. Using monaural stimulation in the young group, contralateral temporal lobe activation volume and intensity were higher vs ipsilateral, while they were lower in the aged and presbycusis groups. 44. Artemether Exhibits Amoebicidal Activity against Acanthamoeba castellaniithrough Inhibition of the Serine Biosynthesis Pathway

Abstract

ABSTRACTAcanthamoebasp. parasites are the causative agents of Acanthamoebakeratitis, fatal granulomatous amoebic encephalitis, and cutaneous infections. However, there are currently no effective drugs for these organisms. Here, we evaluated the activity of the antimalarial agent artemether against Acanthamoeba castellaniitrophozoites and identified potential targets of this agent through a proteomic approach. Artemether exhibited in vitroamoebicidal activity in a time- and dose-dependent manner and induced ultrastructural modification and cell apoptosis. The iTRAQ quantitative proteomic analysis identified 707 proteins that were differentially expressed after artemether treatment. We focused on phosphoglycerate dehydrogenase and phosphoserine aminotransferase in the serine biosynthesis pathway because of their importance to the growth and proliferation of protozoan and cancer cells. The expression of these proteins in Acanthamoebawas validated using quantitative real-time PCR and Western blotting after artemether treatment. The changes in the expression levels of phosphoserine aminotransferase were consistent with those of phosphoglycerate dehydrogenase. Therefore, the downregulation of phosphoserine aminotransferase may be due to the downregulation of phosphoglycerate dehydrogenase. Furthermore, exogenous serine might antagonize the activity of artemether against Acanthamoebatrophozoites. These results indicate that the serine biosynthesis pathway is important to amoeba survival and that targeting these enzymes would improve the treatment of Acanthamoebainfections. Artemether may be used as a phosphoglycerate dehydrogenase inhibitor to control or block Acanthamoebainfections.

Abstract

Abstract: Acanthamoeba spp. can be parasitic in certain situations and are responsible for serious human infections, including Acanthamoeba keratitis, granulomatous amoebic encephalitis, and cutaneous acanthamoebiasis. We analyzed the fatty acid composition of Acanthamoeba castellanii trophozoites and tested the inhibitory activity of the main fatty acids, oleic acid and arachidonic acid, in vitro. Oleic acid markedly inhibited the growth of A. castellanii, with trophozoite viability of 57.4% at a concentration of 200 μM. Caspase‐3 staining and annexin V assays showed that apoptotic death occurred in A. castellanii trophozoites. Quantitative PCR and dot blot analysis showed increased levels of metacaspase and interleukin‐1β converting enzyme, which is also an indication of apoptosis. In contrast, arachidonic acid showed negligible inhibition of growth of A. castellanii trophozoites. Stimulated expression of Atg3, Atg8 and LC3A/B genes and monodansylcadaverine labeling suggested that oleic acid induces apoptosis by triggering autophagy of trophozoites. [ABSTRACT FROM AUTHOR]

46. Evaluating and Improving the Utilization of Automated Dispensing Cabinets (ADCs) in a Pediatric Hospital

Abstract

The objectives of this study were: 1) To analyze the utilization of ADCs and to identify the utilization problems for improvement. 2) To develop a simulation model to evaluate and improve the utilization of ADCs. Method A retrospective data analysis approach was applied. One-year’s data (10/01/2015 to 09/30/2016) from EPIC and Pyxis were analyzed. EPIC administration and EPIC dispensing data in were matched to determine where the administered medications were dispensed from. Pyxis activity reports and Pyxis stockout reports were analyzed to determine the Pyxis utilizations in different nursing units, including: frequency of stockout, average stockout duration, vend to fill ratio, Inventory turnover rate, number and cost of expired items, and frequency of overrides. Pearson correlation tests were performed to determine the correlation between different measurements. EPIC administration and EPIC dispensing data were compared determine the data and nursing units to be used to develop the model. Three months of EPIC dispensing data of Pyxis in the two units were used to develop the model, output variables were compared with the measurements calculated from Pyxis activity report and stockout report to validate the model. Four proposed improvement inventory strategies were tested using the model to determine how these strategies will impact the utilizations of Pyxis.Results Currently the Pyxis dispensed 46.20% of all administration transactions in all inpatient nursing units, most controlled substances were dispensed from onsite Pyxis (84.68%) and most non-controlled substances (60.79%) were dispensed from Central Pharmacy. By adjusting the inventory levels, we can increase the dispense from Pyxis to 51.00% and 92.52% if not restrictions considered. The overall stockout frequency was 5,099, average vend to fill ratio was 13.91 and average inventory turnover rate were 16,66 (by dollar) and 24.38(by item). The total quantity of expired medications in Pyxis were 12,515 and total cost was $23,711. Different measurements were correlated with each other. Comparison of dispensing and administration data showed differences between two datasets, EPIC dispensing data from two nursing units (N1 and N11) were selected to develop and validate the model. The model output had very close predictions compared with the measurements calculated from reports: For N1 unit, stockout frequency from model (M) was 15 comparing with 20 from Pyxis report (P); vend to fill ratio were 19.61(M) and 19.45(P); inventory turnover rate (ITO) were 11.03(M) and 11.34(P). For N11 unit, stockout frequency were 25(M) and 38(P), vend to fill ratio were 14.45(M) and 16.06(P); and ITO were 10.11(M) and 11.78(P). The four proposed inventory strategies showed different impact on the different Pyxis utilization measurements.Conclusion A systematic analysis of Pyxis utilization in a pediatric hospital was conducted and results showed there is space for improvement based on current inventory strategies. A simulation model to evaluate Pyxis utilization was developed and validated. Four different inventory strategies showed that one strategy can have both positive impact on some utilization measurements and negative impact on other measurements. This model provided the tool for pharmacy management to predict the outcome of their proposed inventory strategies.
