Background: A considerable body of evidence accumulated especially during the last decade, demonstrating that early nutrition and lifestyle have long-term effects on later health and disease (“developmental or metabolic programming”). Methods: Researchers involved in the European Union funded international EarlyNutrition research project consolidated the scientific evidence base and existing recommendations to formulate consensus recommendations on nutrition and lifestyle before and during pregnancy, during infancy and early childhood that take long-term health impact into account. Systematic reviews were performed on published dietary guidelines, standards and recommendations, with special attention to long-term health consequences. In addition, systematic reviews of published systematic reviews on nutritional interventions or exposures in pregnancy and in infants and young children aged up to 3 years that describe effects on subsequent overweight, obesity and body composition were performed. Experts developed consensus recommendations incorporating the wide-ranging expertise from additional 33 stakeholders. Findings: Most current recommendations for pregnant women, particularly obese women, and for young children do not take long-term health consequences of early nutrition into account, although the available evidence for relevant consequences of lifestyle, diet and growth patterns in early life on later health and disease risk is strong. Interpretation: We present updated recommendations for optimized nutrition before and during pregnancy, during lactation, infancy and toddlerhood, with special reference to later health outcomes. These recommendations are developed for affluent populations, such as women and children in Europe, and should contribute to the primary prevention of obesity and associated non-communicable diseases.

OBJECTIVE:The associations of maternal plasma n-3 and n-6 polyunsaturated fatty acid (PUFA) concentrations during pregnancy with infant subcutaneous fat were examined. METHODS:In a population-based prospective cohort study among 904 mothers and their infants, maternal plasma n-3 and n-6 PUFA concentrations were measured at midpregnancy. Body mass index, total subcutaneous fat, and central-to-total subcutaneous fat ratio were calculated at 1.5, 6, and 24 months. RESULTS:Maternal n-3 PUFA levels were not consistently associated with infant body mass index or total subcutaneous fat. Higher maternal total n-3 PUFA levels, and specifically eicosapentaenoic acid, docosapentaenoic acid, and docosahexaenoic acid, were associated with higher central-to-total subcutaneous fat ratio at 1.5 months, whereas higher maternal total n-3 PUFA levels were associated with lower central-to-total subcutaneous fat ratio at 6 months (all P values < 0.05). These associations were not present at 24 months. Maternal n-6 PUFA levels were not consistently associated with infant subcutaneous fat. BACKGROUND: Little is known about the effect of fatty acid (FA) concentrations in cord blood on long-term behavioral outcomes. OBJECTIVE: We assessed the effect of FAs in cord blood serum on children's behavioral difficulties at the age of 10 y. DESIGN: A longitudinal study of 416 children from the population-based Influences of Lifestyle-Related Factors on the Immune System and the Development of Allergies in Childhood (LISAplus) birth cohort from Munich was conducted. Individual glycerophospholipid FAs in blood were analyzed in venous cord blood. Data on children's behavior were collected with a parent-reported Strength and Difficulties Questionnaire at 10 y of age. Zero-inflated Poisson regression models were applied and adjusted for sex, parental income, smoking during pregnancy, and dietary intake of arachidonic acid (AA) and DHA at 10 y. RESULTS: A 1% increase in DHA in cord blood serum was found to decrease total difficulties by (exp)β(5)(adj) = 0.93 (SE = 0.02, P < 0.0001) and hyperactivity or inattention by (exp)β(adj) = 0.94 (SE = 0.03, P < 0.04). Higher long-chain (LC) PUFA concentrations in cord blood serum were associated with fewer emotional symptoms [(exp)β(adj) = 0.95, SE = 0.03, P = 0.01], and similarly higher AA concentrations were associated with fewer emotional symptoms [(exp)β(adj) = 0.94, SE = 0.03, P = 0.03]. CONCLUSION: Increased concentrations of DHA, LC-PUFAs, and AA in cord blood serum were associated with lower scores on a parent-completed behavioral screen. An appropriate FA supply to the developing fetus may be essential for optimal long-term behavioral outcomes in children.

The dietary intake of certain fatty acids might contribute to the development of atopic diseases like allergic rhinitis and asthma. We investigated the association between the ratio of n-6 and n-3 fatty acids in serum phospholipids and hay fever or allergic sensitisation in adults. Data from a population based cross-sectional study on respiratory health including measurement of fatty acids in serum phospholipids of 740 adults between 20 and 64 years of age were analysed. We could not find any significant association between n6/n3-ratio of fatty acids in serum phospholipids and hay fever or allergic sensitisation neither in the total population nor in the sub-population stratified by sex. Since no previous study on fatty acid intake confirmed the n6/n3-ratio hypothesis and this study did not find any association between the n6/n3-ratio in serum-phospholipids and hay fever or allergic sensitization, we conclude that the n6/n3- ratio hypothesis is not confirmed although biological plausible.
