Purpose: Long-lasting COVID-19 symptoms are closely associated with psychiatric sequelae. However, little is known about whether it is possible to predict a protracted course early in the disease. In this study, we aimed to understand how post-COVID-19 psychiatric symptomatology evolves over time and the variables that affect these symptoms. Materials and Methods: Anxiety, depression, acute stress, and sleep disorders among patients with COVID- 19 were prospectively evaluated during hospitalization and six months after discharge. Adult patients recently admitted to non-intensive care units with COVID-19 were eligible. Their psychiatric status was assessed using the Hospital Anxiety and Depression Scale, National Stressful Events Survey Acute Stress Disorder Short Scale, Pittsburgh Sleep Quality Index. The patients' clinical data were gathered from hospital records. Six months after they were discharged, the same questionnaire and a checklist to assess ongoing physical symptoms were distributed to the patients via WhatsApp. Results: 143 participants were enrolled, 47 of whom completed the study. The results showed that patients' depression, anxiety, and insomnia significantly decreased during the follow-up. However, there was an increase in the number of patients who reported acute stress symptoms. The highest C-reactive protein level during acute infection (OR=1.09) predicted depression during the follow-up. Experiencing a higher number of infectious symptoms during hospitalization predicted a higher number of protracted symptoms six months later (OR=1.5). Conclusion: This indicates that the severity of systemic inflammation during acute COVID-19 infection may predispose patients to persistent depression. Patients with a higher number of symptoms during acute infection may be at risk of developing long-term COVID-19. [ABSTRACT FROM AUTHOR]

Lapper'in şarkı söyleyerek öğretmeye çalıştığı yöntem kabul görmemiştir. Ulshöfer'in Alman Dışişleri'ne gönderdiği raporlarda öğrenciler, ders kitapları ve Almanca derslerinin yöntemi hakkında bilgiler verilmiştir. Bu çalışmada Yüksek Ziraat Ensitüsü'nün kuruluş yıllarında görev yapmış olan Lapper ve Ulshöfer'in çalışmalarına dair bulunan arşiv kaynaklarına dayanarak Almanca dersleri incelenmiştir. The High Agricultural Institute (YZE) started education in Ankara in 1933 with German professors. German teachers such as Neumann, Lapper, Matschenz, Marchand, Ulshöfer and Göbel took part in the Foreign Languages Seminar established within the Institute. The method Lapper tried to teach by singing was not accepted. In the reports Ulshöfer sent to the German Foreign Office, information was given about the students, textbooks and the method of German lessons. In this study, German lessons were examined based on the archival sources found on the work of Lapper and Ulshöfer, who had served in the founding years of the Higher Institute of Agriculture.

It shows importance to approach COVID-19 pandemic with a psychiatricc persepctive. To identify the effects of virus on central nervous system, would provide advantage to all physicians. nCoV-2019, which carries many potential of pathophysiological mechanisms, including ACE-2 protein on neurons. can cause primary and secondary neuropsychiatric effects. Abundant studies were made on teh neuropsychiatric effects of nCoV-19, as well as other viruses form Coronaviridae family. It can cause, provoke or exacerbate mass depression, anxiety and stress. As certain COVID-19 medications bear significant psychiatric side-effects, certain psychiatric drugs can also interact with COVID-19 medication. Both the psychiatric and neuropsychiatric effects of Covid infection, and diagnosis and treatment arrangements of patients who have the infection are important. In this review, we aimed to review the relationship between Covid-19 and psychiatry according to the literature., COVID-19 pandemisine psikiyatrik bir perspektiften yaklaşılması, klinisyen için önem arzeder. Virüsün merkezi sinir sistemi üzerindeki etkilerinin tanınması, tüm hekimler için bir avantaj sağlayacaktır. Nöronlardaki ACE-2 proteini dahil olmak üzere pek çok potansiyel patofizyolojik mekanizması olan nCoV-2019, hem primer hem de sekonder olarak nöropsikiyatrik yan etkilere yol açabilir. Hem nCoV-2019 hem de aynı Coronaviridae ailesinden virüslerin nöropsikiyatrik etkileri üzerine yapılmış literatürde pek çok çalışma mevcuttur. Kitlesel boyutta sekonder anksiyete, depresyon, stres ortaya çıkarabilir ya da bu semptomları tetikleyebilir veya alevlendirebilir. Belli COVID-19 ilaçlarının psikiyatrik yan etkileri olabileceği gibi psikiyatride kullanılan belli ilaçlar, COVID-19 ilaçlarıyla etkileşime girebilir. Hem Covid enfeksiyonunun psikiyatrik ve nöropsikiyatrik etkileri, hem de enfeksiyona yakalanan hastalarda tanı ve tedavi düzenlemesi önem arzetmektedir. Bu derlemede Covid-19 ve psikiyatri ilişkisini literatür verileri doğrultusunda anlatmayı amaçladık.

Asıl adı Tâceddîn-zâde Mustafa el-Ankaravî olan Şeyh Tâceddîn-i Velî, 17. yüzyılda Ankara'da yaşamış ve Ankara'nın Hamamönü mevkiinde yaptırdığı cami ve dergâh ile Celvetiyye Tarikatı'nın Ankara'daki teşekkülüne ve gelişimine öncülük etmiş bir mutasavvıftır. Aslen kendisi de Ankaralı olan Aziz Mahmud Hüdâyî ve onun halifelerinden biri olan Fenâyî Cennet Mehmed Efendi'ye intisap eden Tâceddîn-i Velî'nin hayatı hakkında çok fazla bilgi olmamasına rağmen günümüzde Ankara halkı onu, Hamamönü semtinde bulunan Tâceddîn Camii, Dergâhı ve Mehmet Akif Ersoy'un Millî Mücadele yıllarında bu dergâhta bir müddet kalmış ve burada `İstiklal Marşı`nı yazmış olması vesilesiyle tanımış, sevmiş ve bu sevgi dolayısıyla kendisine Tâceddîn-i Velî, Tâceddîn Sultan gibi isimleri uygun görmüştür. İlk olarak tarihçi yazar Enver Behnan Şapolyo, 1958'de Ankara Belediye Dergisi'nde ve iki yıl sonra da Türk Büyükleri kitabında yazdığı tanıtım yazılarında, kendisine hediye edilen bir yazma eser içerisinde şeyhe ait birçok değerli şiirin bulunduğunu söyleyip şiirlerden bazı örnekleri paylaşarak şeyhin edebi yönüne dikkat çekmiştir. Şapolyo'nun sözünü ettiği yazma eserin çalışmamızda faydalanmış olduğumuz Milli Kütüphane 06 Mil Yz A 10182 numarada kayıtlı Külliyât adlı eser olması ihtimali yüksektir. Şeyh Tâceddîn-i Velî, tasavvufi şiirlerinde `Tâceddînoğlu` mahlasını kullanmıştır. Bu çalışmada Ankaralı Şeyh Tâceddîn-i Velî'nin Milli Kütüphane'de bulunan Külliyat adlı eserinden yola çıkarak onun hayatı, tarikatı, edebi şahsiyeti hakkında bazı tespitlerde bulunulmuş; ayrıca Külliyat'taki İlahiyyat-ı Tâceddînzâde bölümü ile Koyunoğlu Müzesi Kütüphanesi 13889 arşiv numarasıyla kayıtlı yazmanın 147b-180b varakları arasındaki Divan-ı İlahiyat eserinde bulunan şeyhe ait tasavvufi şiirlerin transkripsiyonlu metni verilerek, metin tasavvufi açıdan incelenmiş; ayrıca metnin bağlamlı dizini ve işlevsel sözlüğü yapılmıştır. Sheikh Tâceddîn-i Velî, whose real name was Tâceddîn-zâde Mustafa al-Ankaravî, lived in Ankara in the 17th century and pioneered the formation and development of the Celvetiyye Sect in Ankara with the mosque and lodge built in Ankara's Hamamönü locality. Although there's not much information about the life of Tâceddîn-i Veli, who was a disciple of Aziz Mahmud Hüdayi -also from Ankara- and his khalif Fenâyî Cennet Mehmed Efendi, was named Tâceddîn-i Velî and Tâceddîn Sultan by the people of Ankara because of his Tâceddîn Mosque and the Dervish Lodge and also because Mehmet Akif Ersoy has stayed and wrote `İstiklal Marşı` there at the times of War of Indepence. Firstly, the historian writer Enver Behnan Şapolyo, in his introductory writings he wrote in Ankara Belediye Dergisi in 1958 and two years later in the Türk Büyükleri Kitabı, said that there were many valuable poems belonging to the sheikh in a manuscript, and he shared some examples of the sheikh's literary works, bringing attention to their literal value. It is highly probable that the manuscript referred to by Şapolyo is Külliyât, registered at the National Library 06 Mil Yz A 10182, which we have benefited from in our study. Şeyh Tâceddîn-i Velî used the pseudonym `Tâceddînoğlu` in his sufi poems. In this study, some determinations will be made about his life, sect and literary personality based on the book of Külliyat in the National Library of Şeyh Tâceddîn-i Velî of Ankara. In addition, the transcript of the mystical poems belonging to the sheikh in the Divan-ı İlahiyat which is between the divisions 147b-180b of the manuscript registered with the archive number 13889 of the İlahiyât-ıTâceddînzâde section of the Külliyat and the Koyunoğlu Museum Library will be shown, and the text will be contextual and functional. 265

24. Şizofreni etyopatogenezi ve intihar riskinde Sitomegalovirüs: Vaka-kontrol çalışması.

- Abstract
Amaç: Bu vaka-kontrol çalışmasında, Sitomegalovirüs (CMV) infeksiyonu ile şizofreni veya intihar riski arasındaki olası ilişkinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Vaka-kontrol çalışması olarak dizayn edilen bu araştırmaya 134 şizofreni tanılı hasta ve 140 sağlıklı kontrol olmak üzere toplam 274 olgu dahil edilmiştir. Çalışmaya alınan olguların sosyodemografik özellikleri, klinik verileri ile intihar risk puanları yüzyüze görüşmelerde kaydedilmiş, serum örneklerindeki CMV IgG seroprevalansı belirlenmiştir. Bulgular: Çalışmaya dahil edilen olguların yaş ortalamasının 30.82±0.59 yıl; %50.7'sinin kadın, %49.3'ünün erkek; %5.1'nin okur -- yazar olmadığı, %26.3'ünün ilköğretim, %42.3'ünün ortaöğretim ve %26.3'ünün ise lisans mezunu; İDÖ (İntihar Davranış Ölçeği) puan ortalamasının ise 6.27±3.12 olduğu belirlenmiştir. Çalışmada CMV IgG seropozitiflik oranı %64.6 (177/274) olarak belirlenmiştir. CMV IgG seropozitiflik oranlarının sağlıklı kontrol grubunda %48.6 (68/140), şizofreni grubunda ise %81.3 (109/134) olduğu ve şizofreni ile CMV seropozitifliği arasındaki ilişkinin istatistiksel olarak anlamlı olduğu belirlenmiştir (p<0.001). CMV IgG seropozitifliği intihar riskine sahip olmayan ve olan katılımcılarda sırası ile %61.5 ve %70.7 olarak belirlenmiş ve CMV IgG seropozitifliği ile intihar riski arasındaki ilişki istatistiksel olarak anlamlı bulunamamıştır (p=0.137). Sonuç: Latent CMV infeksiyonunun, dopamin ve diğer nörotransmitter düzeylerine olan potansiyel etkileri ile şizofreni etyopatogenezinde rol oynayabileceği düşünülmüştür. Ancak, şizofreni ve CMV ile ilgili daha ileri araştırmalara ihtiyaç bulunduğu ve böylece şizofreni tedavisinde iyileştirme sağlanabileceği düşünülmektedir. [ABSTRACT FROM AUTHOR]

Introduction: The mechanism of ubiquitination-related abnormalities causing neural development problems is still unclear. We examined the association between autism and serum transactive response DNAbinding protein-43 (TDP-43) and ubiquitin c-terminal hydrolase-L1 (UCH-L1) levels, both of which are members of the ubiquitinproteosome system. Methods: We measured serum levels of TDP-43 and UCH-L1 in 24 children with autism and 24 healthy children. Childhood Autism Rating Scale (CARS) was used to assess symptom severity at admission. Neurocognitive deficits detected in bipolar patients have a negative influence on clinical course and severity. We reviewed studies on neurocognitive deficits in euthymic bipolar subjects and their possible neuroanatomical correlates. We also examined studies suggesting deficits seen in euthymic patients may be endophenotypes of the disorder. Results of the studies in euthymic patients and their healthy relatives indicate that have cognitive dysfunctions especially of verbal memory, learning and executive domains, which were found to be related to prefrontal cortex and temporo-limbic circuitry. Longitudinal studies investigating heritability of cognitive impairment in bipolar disorder and its relation with brain connectivity and epigenetic mechanisms are needed. Keywords: Bipolar disorder, neurocognitive deficit, endophenotype, Nörobilişsel defisitlerin bipolar bozukluğun seyri ve ciddiyeti üzerinde belirle-yici olduğu bilinmektedir. Bu derlemede ötimik bipolar hastalarda nörobilişsel defisit varlığını, bu defisitlerle bipolar hastalarda saptanan nöroanatomik lezyonların ilişkisi ve bu hastalarda saptanan bazı defisitlerin bir endofenotip olabileceği ile ilgili çalışmaları gözden geçirilmiştir. Çalışmalar ötimik hastalarda ve sağlıklı aile bireylerinde prefrontal korteks, temporo-limbik döngü ile ilişkili bulunan sözel bellek, öğrenme ve yürütücü fonksiyon alanlarında defisit gösterdiklerini teyit etmiştir. Ötimik hastalarda saptanan bilişsel defisitlerin genetik temellerini, bipolar hastalarda nöroanatomik tutu-lumla olan ilişkisini ve epigenetik mekanizmaların hastalık sürecine olası etkilerini araştırmaya yönelik çalışmalara ihtiyaç vardır.

31. The views of medical students about psychiatry clerkship education

Objectives: Psychiatry education in medical schools seems to be given little attention and has not been fully integrated into curriculum. In this study our purpose was to get feedback about all phases of psychiatry clerkship from medical students, who completed psychiatry clerkship. Methods: A 31 item questionnaire investigating the views of medical students about psychiatry clerkship and a socio-demographic survey were given to the medical students, who completed psychiatry clerkship and signed informed consent prior to study procedures. Results: A total of 208 students agreed and signed consent to participate in the study. The number of female and male students were 114 & 94, respectively and mean age was 22.7±1.02. Most of the students (89%) expected from the lecturers “to teach how to manage aggressive patients.” Seventy six percent of students specified the requirement of consultation liasion education because “psychiatric symptoms could be seen in general medical conditions.” Seventy percent of students reported that they wanted to watch and discuss standart patient video records. Fifty three percent of the students emphasized that “Psychiatry examination was only assessing knowledge, but not the skills.” Seventy eight percent of students reported that “they wished to follow up the patients along a resident’’ and 64% “with an attending.” Also, 54% of students supported to participate in a research project or seminar related to psychiatry (e.g. neuroimmunology, neuroanatomy, psychoendocrinology) in previous years. Conclusion: The efforts to increase interest and improve outcome and effectiveness in psychiatry clerkship education have become more important. This is the first study on this topic in our country. To improve outcome and effectiveness in psychiatry clerkship education, the educators should find the balance between the views of medical students and experienced faculty members about the topics to be taught during the rotation., Amaç: Tıp fakültelerinde psikiyatri eğitimine az önem verilmiş ve müfredata çok iyi entegre edilmemiş gibidir. Bu çalışmada psikiyatri stajını bitirmiş öğrencilerin stajın hemen bütün safhaları hakkında düşüncelerini geri bildirimle bize aktarmalarını amaçladık. Yöntem: Bu çalışmaya fakültemizde okuyan, psikiyatri stajını bitiren ve çalışmaya katılmayı kabul eden tıp öğrencilerin bilgilendirilmiş onamı alındıktan sonra, sosyo-demografik özellik anketinin ardından, ekibimizin hazırladığı 31 sorudan oluşan, psikiyatri stajına öğrencilerin bakışını araştıran bir anket verildi. Bulgular: Çalışmaya 208 öğrenci katılmayı kabul etmiştir. Bu öğrencilerin 114’ü kadın iken 94’ü erkekti ve ortalama yaşları 22.7±1.02 idi. Öğrenciler, en yüksek oranda eğitimcilerden saldırgan hasta karşısında neler yapabileceğinin öğretilmesini beklemekteydi (%89). Yüzde 76 öğrenci psikiyatrik belirtilerin genel tıbbi durumlarda da görülebileceğine inandığı için eğitimde konsültasyon ve liyezona ağırlık verilmesi gerektiğini belirtmişti. Yüzde 70 öğrenci standart hasta video kayıtlarını izlemek ve tartışmak istediklerini bildirmişti. Yüzde 53 öğrenci psikiyatri sınavının yalnızca bilgiyi ölçtüğüne ve beceriyi ölçmediğine vurgu yapmıştı. Öğrencilerin %78’i araştırma görevlisi ile ve %64’ü bir hocayla hasta takip etmek istediğini söylemişti. Ayrıca, %54 öğrenci daha önceki sınıflarda iken psikiyatri ile ilgili (nöroimmünoloji, nöroanatomi, psikoendokrinoloji gibi) seminer/araştırma projelerine katılmayı doğru bulmuştu. Sonuç: Psikiyatri stajyer eğitiminde ilgiyi ve verimi artırma çabaları giderek önem kazanmaktadır. Bu çalışma ülkemizde bu amaçla yapılan ilk çalışma olması nedeniyle önemlidir. Ancak eğiticiler eğitimde verim almak için öğrencilerin görüşünü göz önünde bulundururuken, deneyimli bir klinisyen olarak da öğretilmesi gerekenler arasında dengeyi gözetmelidir.

Objective: Although schizotypy is generally described as a condition predisposing to schizophrenia, it can also be referred to as the nonspecific psychosis proneness in general. When we look at the dimensional evaluation, it may be suggested that the size of schizotypic dimension can be extended to other pathologies such as bipolar and obsessive compulsive disorder which do not end with the schizophrenia spectrum. Therefore, in this study, we aimed to evaluate the schizotypal features in a group of BP patients and to compare BP patients and their siblings in terms of schizotypal features. Material and Methods: Thirty-one bipolar patient and 31 healthy siblings who did not have any psychiatric diagnosis were included in the study. BP patients and healthy siblings were administered the Schizotypal Personality Questionnaire. Results: Total schizotype scores of the patients were significantly higher than healthy siblings (p<0.001). In the comparison of scale scores between patients and siblings, it was observed that six of nine subscales of patient group were significantly higher than the scores of healthy siblings. According to three dimenisonal evaluation, cognitive perceptual and interpersonal schizotypy scores were found to be significantly higher in the patients (p<0.001). Besides, negative and positive schizotype scores were also higher in patient group (p<0.001). It was observed that the scale scores of the patients and their siblings were correlated. Conclusion: Our study showed that the total and subscores of Schizotypal Personality Questionnaire for BP patients were higher than their siblings, and that this result was correlated with the siblings. 38. Latent toxoplasma gondii infection associated with suicidality in mood disorder patients.

Objective: T. gondii infection has an impact on human behavior and etiological association with some psychiatric conditions such as schizophrenia and bipolar disorder. Herein, we would like to present preliminary outcomes of an observational study of healthy controls and suicide attempted mood disorder patients. Methods: Case-control study approach was preferred. Ethical clearance was sought with B.30.2.AYD.0.00.00-480.2/217 protocol number. Eligible a total of 147 BD and 161 MD patients as a case group, and 310 volunteers, without any history of psychiatric diagnosis and suicide attempt, as a control group was included. The data of mood disorder patients diagnosed with SCID-5 were collected from the Department of Psychiatry, Istanbul University-Cerrahpaşa. To detect the prevalence of the latent T.gondii infection, T.gondii IgG and DNA were assessed by ELISA and real-time PCR. All relevant data were evaluated by statistical methods. Results: There were no statistical differences between patients and healthy individuals regarding age and gender. Overall, T. gondii seropositivity rates were calculated as 57.1% in BD, 29.2% in MD, 64.8% in suicide attempters, and 21.3% in healthy individuals. Number of patients attempted to suicide, irrelevant the diagnosis, was significantly linked with T. gondii positivity detected by ELISA (OR= 12.44; 95% CI= 6.85-22.57; p< 0.001) and PCR (OR= 2.32; 95% CI= 1.26-4.28; p= 0.006). To conduct the binary logistic regression, the presence of T. gondii status was adjusted with sociodemographic data. Stratified analyses of the T. gondii seropositivity was a predictor in BD (OR= 3.56; 95% CI= 2.19-5.80; p< 0.001) and suicide attempters (OR= 17.17; 95% CI= 8.12-36.28; p< 0.001). Conclusion: We speculate that T. gondii has a relationship between suicidal behavior on the mood disorders and etiology of BP. - Abstract
Clozapine is a uniquely efficacious atypical antipsychotic for treatment-resistant schizophrenia, as it may offer unique benefits both in acute psychotic episode and long-term progonosis. Because of its lesser tendency to induce extra-pyramidal side-effects and tardive dyskinesia, clozapine could sometimes be prescribed as a second-choice drug in patients for whom previous antipsychotic therapy was not tolerated due to serious motor side-effects. Myocarditis is a rare but life-threatening complication of clozapine use. High index of suspicion and prompt cessation of clozapine could be life-saving. Here, we present a case of a sixty-two-year-old woman with schizophrenia who was started on clozapine during an acute inpatient psychiatric admission. Within three weeks of starting clozapine the patient developed fever, pleuritic chest pain and lethargy. After cessation of clozapine and with supportive medical care the patient became fully recovered. Throughout the following case, and a brief overview of the literature, we have tried to emphasise the difficult and important clinical features in the diagnosis of clozapine-induced myocarditis.tt. Klingsor Syndrome has been defined as a self-mutilative behaviour to genital organs, which is rare but can be accompanied by serious complications, and is often accompanied by psychotic symptoms. In the literature, risk factors of Klingsor syndrome are found as having commanding type auditory hallucinations, religious delusions, personality disorders, history of suicidal attempt, thoughts of guilt related to sexual conflicts, depressive features, and severe childhood deprivation. In this paper, our aim is presenting the clozapine response of a patient with schizophrenia, who had recurrent suicidal attempts, and performed genital self mutilation due to commanding type hallucinations. In this patient with severe self-mutilative behavior, and a history of multiple suicidal attempts, it was observed that parasuicidal thoughts regressed, self-mutilative behavior, and thoughts did not recur during the 28-week follow-up period. Recognition of risky patients, and taking precautions for this, treatment of psychotic symptoms and performing anatomical repair in terms of surgery are important for prognosis of these type of cases. - Abstract
The etiology of gender dysphoria (GD) is not fully known. In recent years, there are studies suggesting that the development of GD is related to some biological mechanisms. The hypothesis conveying that there is a biological background in the development of GD emphasize the importance of exposure to sex hormones in prenatal period. According to this hypothesis, while prenatal exposure to sex hormones causes development of male identity, non-exposure results with female identity. This hypothesis is supported with determination of the relationship between prenatal exposure to testosterone and masculine behaviors, in complete androgen insensitivity syndrome mostly development of female identity, and presence of near values of 2nd and 4th finger ratio in individuals with GD and the individuals with the sex they want to pass. In this article, one of the twins is male and the other one is female-to- male GD case was discussed in terms of the roles of prenatal exposure to sex hormones in the etiology of GD. - Abstract
The use of atypical antipsychotics in patients with schizophrenia is tightly related to the metabolic abnormalities like weight gain, diabetes, dyslipidemia and/or hypertension. Beside of having unique receptor binding features, their heterogeneous receptor binding properties ara also involved in the occurence of the metabolic side effects caused by atypical antipsychotics. It is striking that atypical antipschotics related metabolic side effects have the potential to influence each other in a negative way. Metabolic abnormalities related to atypical antipschotics do not only lead to increase in mortality, but also worsen clinical course and functionality of patients with schizophrenia. Thus, patients treated with atypical antipsychotics should be evaluated routinely with respect to their metabolic states. In this article, metabolic side effects of atypical antipsychotics and their mechanisms were investigated on pharmacological basis. [ABSTRACT FROM AUTHOR]

Neurocognitive deficits detected in bipolar patients have a negative influence on clinical course and severity. We reviewed studies on neurocognitive deficits in euthymic bipolar subjects and their possible neuroanatomical correlates. We also examined studies suggesting deficits seen in euthymic patients may be endophenotypes of the disorder. Results of the studies in euthymic patients and their healthy relatives indicate that have cognitive dysfunctions especially of verbal memory, learning and executive domains, which were found to be related to prefrontal cortex and temporo-limbic circuitry. Longitudinal studies investigating heritability of cognitive impairment in bipolar disorder and its relation with brain connectivity and epigenetic mechanisms are needed. [ABSTRACT FROM AUTHOR]

Giriş: Şizofrenide depresif belirtiler oldukça yaygındır. Bu belirtilerin, daha çok ilaç kullanımı, tedavi uyumsuzluğu, hastanede daha fazla yatış, işlevsellikte daha fazla bozulma ve yaşam kalitesinde kötüleşme ile ilişkili olduğuna dair kanıtlar bulunmaktadır. Şizofrenide depresyon gelişimini açıklamak üzere çeşitli ilişkili psikolojik faktörleri araştıran çalışmaların sayısı azdır ve bu çalışmalarda genellikle az sayıda faktör üzerinde durulmuştur. Amaç: Bu çalışmada, şizofrenili hastalarda bazı psikolojik faktörlerin depresyon açısından öngördürücülüğünü araştırmak amaçlanmıştır. Bu amaçla depresyon puanları yüksek ve düşük hastalar; pozitif ve negatif belirtiler, anksiyete, stresle başa çıkma tarzları, sosyal destek ve kontrol odağı türü açısından karşılaştırılmıştır. Yöntem: Şizofreni hastalarına Calgary Şizofrenide Depresyon Ölçeği ile belirlenen depresyonu olan (n=29) ve olmayan (n=31) iki grup Sosyodemografik Veri Formu, Pozitif ve Negatif Sendrom, Beck Anksiyete, Rotter'in İç-Dış Kontrol Odağı, Çok Boyutlu Algılanan Sosyal Destek ve Stresle Başa Çıkma Tarzları Ölçekleri uygulanarak karşılaştırılmıştır. Bulgular: Depresyonu olan ve olmayan iki grup arasında sosyodemografik ve klinik özelliklerle sosyal destek puanları ve stresle başa çıkma tarzları açısından farklılık saptanmamıştır. Depresyonu olan ve olmayan gruplar arasında PANSS pozitif, negatif, genel psikopatoloji alt ölçekleri ve PANSS toplam puanları ile anksiyete puanları ve kontrol odağı inancı açısından istatistiksel olarak anlamlı farklılık bulunmuştur. Sonuç: Bu çalışma, şizofrenili hastalarda negatif, pozitif ve genel psikopatolojik belirti düzeylerinin yüksekliği, dış kontrol odağı ve yüksek anksiyete puanlarının depresyon için öngördürücü olabileceğini göstermiştir. Benzer ya da farklı psikolojik faktörleri daha geniş hasta gruplarında inceleyecek çalışmalar, etkili olabilecek faktörleri erken dönemde saptamak ve müdahale etmek yoluyla morbiditeyi ve hatta neden olduğu yüksek intihar riski nedeniyle mortaliteyi azaltmak imkanı sağlayabilir. Introduction: Depressive symptoms are very common in schizophrenia. There are evidences that these symptoms are associated with more drug use, treatment incompliance, more hospitalization, greater impairment of functionality, and deterioration in quality of life. The number of studies investigating related psychological factors is scarce and these studies usually focus on a small number of factors. Objective: In this study, we aimed to investigate the predictability of some psychological factors on depression in patients with schizophrenia. For this purpose, patients with high and low depression scores were compared to positive and negative symptoms, anxiety, stress coping styles, social support and locus of control. Method: The two groups, with (n=29) and without (n=31) depression, determined by the Calgary Depression Scale in Schizophrenia were compared. Results: There were no differences between the two groups, with and without depression, in terms of sociodemographic and clinical characteristics, social support scores, and coping styles. Statistically significant differences were found between the groups, with and without depression, in terms of PANSS positive, negative, general psychopathology subscales, PANSS total scores, anxiety scores and locus of control. Conclusion: This study showed that high levels of negative, positive and general psychopathological symptoms, external locus of control, and high anxiety scores may be predictive of depression in patients with schizophrenia. Studies that examine similar or different psychological factors in larger patient groups may provide the opportunity to reduce morbidity and mortality due to the high risk of suicide caused by early detecting and interfering with the factors that may be effective. 103
