Background Abdominal wall hernia is a common surgical condition. Patients may present in an emergency with bowel obstruction, incarceration or strangulation. Small bowel obstruction (SBO) is a serious surgical condition associated with significant morbidity. The aim of this study was to describe current management and outcomes of patients with obstructed hernia in the UK as identified in the National Audit of Small Bowel Obstruction (NASBO). Methods NASBO collated data on adults treated for SBO at 131 UK hospitals between January and March 2017. Those with obstruction due to abdominal wall hernia were included in this study. Demographics, co‐morbidity, imaging, operative treatment, and in‐hospital outcomes were recorded. Modelling for factors associated with mortality and complications was undertaken using Cox proportional hazards and multivariable regression modelling. Results NASBO included 2341 patients, of whom 415 (17·7 per cent) had SBO due to hernia. Surgery was performed in 312 (75·2 per cent) of the 415 patients; small bowel resection was required in 198 (63·5 per cent) of these operations. Non‐operative management was reported in 35 (54 per cent) of 65 patients with a parastomal hernia and in 34 (32·1 per cent) of 106 patients with an incisional hernia. The in‐hospital mortality rate was 9·4 per cent (39 of 415), and was highest in patients with a groin hernia (11·1 per cent, 17 of 153). Complications were common, including lower respiratory tract infection in 16·3 per cent of patients with a groin hernia. Increased age was associated with an increased risk of death (hazard ratio 1·05, 95 per cent c.i. 1·01 to 1·10; P = 0·009) and complications (odds ratio 1·05, 95 per cent c.i. 1·02 to 1·09; P = 0·001). Conclusion NASBO has highlighted poor outcomes for patients with SBO due to hernia, highlighting the need for quality improvement initiatives in this group., This study shows that small bowel obstruction due to abdominal wall hernia is common, typically involves a co‐morbid group of patients and has poor outcomes, including high mortality rates. - Abstract
Background: \ud Small bowel obstruction is a common surgical emergency, and is associated with high levels of morbidity and mortality across the world. The literature provides little information on the conservatively managed group. The aim of this study was to describe the burden of small bowel obstruction in the UK.\ud \ud Methods: \ud This prospective cohort study was conducted in 131 acute hospitals in the UK between January and April 2017, delivered by trainee research collaboratives. Adult patients with a diagnosis of mechanical small bowel obstruction were included. The primary outcome was in‐hospital mortality. Secondary outcomes included complications, unplanned intensive care admission and readmission within 30 days of discharge. Practice measures, including use of radiological investigations, water soluble contrast, operative and nutritional interventions, were collected.\ud \ud Results: \ud Of 2341 patients identified, 693 (29·6 per cent) underwent immediate surgery (within 24 h of admission), 500 (21·4 per cent) had delayed surgery after initial conservative management, and 1148 (49·0 per cent) were managed non‐operatively. The mortality rate was 6·6 per cent (6·4 per cent for non‐operative management, 6·8 per cent for immediate surgery, 6·8 per cent for delayed surgery; P = 0·911). The major complication rate was 14·4 per cent overall, affecting 19·0 per cent in the immediate surgery, 23·6 per cent in the delayed surgery and 7·7 per cent in the non‐operative management groups (P < 0·001). Cox regression found hernia or malignant aetiology and malnutrition to be associated with higher rates of death. Malignant aetiology, operative intervention, acute kidney injury and malnutrition were associated with increased risk of major complication.\ud \ud Conclusion: \ud Small bowel obstruction represents a significant healthcare burden. Patient‐level factors such as timing of surgery, acute kidney injury and nutritional status are factors that might be modified to improve outcomes.

- Abstract
Aim To assess the safety and tolerability of adjuvant subcutaneous trastuzumab (Herceptin® SC, H SC), delivered from an H SC Vial via hand-held syringe (Cohort A) or single-use injection device (Cohort B), with or without chemotherapy, for human epidermal growth factor receptor 2 (HER2)-positive stage I to IIIC early breast cancer (EBC) in the phase III SafeHer study (NCT01566721). Methods Patients received 600 mg fixed-dose H SC every 3 weeks for 18 cycles. The chemotherapy partner was at the investigators' discretion (H SC monotherapy was limited to ≤10% of the population). Data from the first H SC dose until 28 days (plus a 5-day window) after the last dose are presented. Results are descriptive. Results In the overall population, 2282/2573 patients (88.7%) experienced adverse events (AEs). Of the above, 128 (5.0%) patients experienced AEs leading to study drug discontinuation; 596 (23.2%) experienced grade ≥ 3 AEs and 326 (12.7%) experienced serious AEs. Grade ≥ 3 cardiac disorders were reported in 24 patients (0.9%), including congestive heart failure in eight (0.3%). As expected, the AE rates varied according to the timing of chemotherapy in both cohorts, with higher rates in concurrent versus sequential chemotherapy subgroups. In the concurrent chemotherapy subgroup, AEs were more common during the actual period of concurrent chemotherapy compared with the period when patients did not receive concurrent chemotherapy. Conclusion SafeHer confirms the safety and tolerability of the H SC 600 mg fixed dose for 1 year (every 3 weeks for 18 cycles) as adjuvant therapy with concurrent or sequential chemotherapy for HER2-positive EBC. These primary analysis results are consistent with the known safety profile for intravenous H and H SC.

- Abstract
Ultrafast laser inscription has been used to produce channel waveguides in Ge22As20Se58 glass (GASIR-1, Umicore N.V). The mode field diameter and waveguide losses at 2.94 mm were measured along with the waveguide dispersion in the 1 to 4.5 mm range, which is used to estimate the zero-dispersion wavelength. Z-scan measurements of bulk samples have also been performed to determine the nonlinear refractive index. Finally, mid-IR supercontinuum generation has been shown when pumping the waveguides with femtosecond pulses centered at 4.6 mm. Supercontinuum spanning approximately 4 mm from 2.5 to 6.5 mm was measured which, to the best of the authors' knowledge, represents the broadest and the deepest IR supercontinuum from an ultrafast laser inscribed waveguide to date. This work, combined with the long wavelength transmission of GASIR-1 up to 15 mm, paves the way for realizing further ultrafast laser inscribed waveguide devices in GASIR-1 for mid-IR integrated optics applications.

- Abstract
As the notion of circular economy gains momentum, intelligent assets increasingly form the backbone of sustainable ecosystems. Although these assets can supply the necessary knowledge for fueling the value drivers of a circular economy, the generated value will be significantly amplified by allowing third parties to manage them and profit from better asset utilisation. However, for an everchanging networked environment consisting of numerous assets, ownership needs to be dynamic, granular and adaptable in order to maximise gains. Blockchain-based mechanisms can effectively serve this need by enabling transfer of asset ownership directly between parties participating in the circular economy while introducing trust, efficiency and automation in asset exchange contracts.

- Abstract
In this paper, we describe the construction of a semantically annotated corpus of clinical texts for use in the development and evaluation of systems for automatically extracting clinically significant information from the textual component of patient records. The paper details the sampling of textual material from a collection of 20,000 cancer patient records, the development of a semantic annotation scheme, the annotation methodology, the distribution of annotations in the final corpus, and the use of the corpus for development of an adaptive information extraction system. The resulting corpus is the most richly semantically annotated resource for clinical text processing built to date, whose value has been demonstrated through its use in developing an effective information extraction system. The detailed presentation of our corpus construction and annotation methodology will be of value to others seeking to build high-quality semantically annotated corpora in biomedical domains.

- Abstract
HLA-DRB1*07:01 allele carriage was characterised as a risk biomarker for lapatinib-induced liver injury in a large global study evaluating lapatinib, alone and in combination with trastuzumab and taxanes, as adjuvant therapy for advanced breast cancer (adjuvant lapatinib and/or trastuzumab treatment optimisation). HLA-DRB1*07:01 carriage was associated with serum alanine aminotransferase (ALT) elevations in lapatinib-treated patients (odds ratio 6.5, P=3 × 10−26, n=4482) and the risk and severity of ALT elevation for lapatinib-treated patients was higher in homozygous than heterozygous HLA-DRB1*07:01 genotype carriers. A higher ALT case incidence plus weaker HLA association observed during concurrent administration of lapatinib and taxane suggested a subset of liver injury in this combination group that was HLA-DRB1*07:01 independent. Furthermore, the incidence of ALT elevation demonstrated an expected correlation with geographic HLA-DRB1*07:01 carriage frequency. Robust ALT elevation risk estimates for HLA-DRB1*07:01 may support causality discrimination and safety risk management during the use of lapatinib combination therapy for the treatment of metastatic breast cancer.

27. A comparative evaluation of modern English corpus grammatical annotation schemes

- Abstract
Many English Corpus Linguistics projects reported in ICAME Journal and elsewhere involve grammatical analysis or tagging of English texts (eg Atwell 1983, Leech et al 1983, Booth 1985, Owen 1987, Souter 1989a, O’Donoghue 1991, Belmore 1991, Kytö and Voutilainen 1995, Aarts 1996, Qiao and Huang 1998). Each new project has to review existing tagging schemes, and decide which to adopt and/or adapt. The AMALGAM project can help in this decision, by providing descriptions and analyses of a range of tagging schemes, and an internet-based service for researchers to try out the range of tagging schemes on their own data. The project AMALGAM (Automatic Mapping Among Lexico-Grammatical Annotation Models) explored a range of Part-of-Speech tagsets and phrase structure parsing schemes used in modern English corpus-based research. The PoS-tagging schemes include: Brown (Greene and Rubin 1981), LOB (Atwell 1982, Johansson et al 1986), Parts (man 1986), SEC (Taylor and Knowles 1988), POW (Souter 1989b), UPenn (Santorini 1990), LLC (Eeg-Olofsson 1991), ICE (Greenbaum 1993), and BNC (Garside 1996). The parsing schemes include some which have been used for hand annotation of corpora or manual post-editing of automatic parsers, and others which are unedited output of a parsing program. Project deliverables include: – a detailed description of each PoS-tagging scheme, at a comparable level of detail. This includes a list of PoS-tags with descriptions and example uses from the source Corpus. The description of the use of PoS-tags is also illustrated in a multi-tagged corpus: a set of sample texts PoS-tagged in parallel with each PoS-tagset (and proofread by experts), for comparative studies – an analysis of the different lexical tokenization rules used in the source Corpora, to arrive at a ‘Corpus-neutral’ tokenization scheme (and consequent adjustments to the PoS-tagsets in our study to accept modified tokenization) – an implementation of each PoS-tagset in conjunction with our standardised tokenizer, as a family of PoS-taggers, one for each PoS-tagset – a method for ‘PoS-tagset conversion’, taking a text tagged according to one PoS-tagset and outputting the text annotated with another PoS-tagset – a sample of texts parsed according to a range a parsing schemes: a Multi-Treebank resource for comparative studies – an Internet service allowing researchers worldwide free access to the above resources, including a simple email-based method for PoS-tagging any English text with any or all PoS-tagset(s).

Non-Revenue Water (NRW) is one of the major issues water utilities are facing today, especially in areas with severe water scarcity conditions. The WATERLOSS-DSS, a user friendly decision support system (DSS), was developed to help water operators decide which NRW reduction measure(s) to apply in their water networks. The DSS evaluates the water network performance level and finally proposes a list of prioritized NRW reduction measures. This DSS was checked for Nicosia’s (capital of Cyprus) urban water network. Three problematic District Metered Areas (DMAs) were chosen. The DSS provided a list of prioritized NRW reduction measures targeting at various NRW causes for each DMA. Nicosia’s water utility officials decided to apply some of the short-listed measures in those DMAs, resulting in significant NRW reduction and additional benefits. The annual water savings came up to 1 million m3, equal to 4.8% of the water entering the network. Economic benefits of more than 700,000 € are noted. Additional benefits are important energy savings, reduced overtime, and better knowledge of the network. These were incredibly important for Nicosia, as Cyprus is facing severe water scarcity conditions. [ABSTRACT FROM PUBLISHER]

According to the World Health Organisation diabetic retinopathy (DR) is a high priority eye disease. This paper investigates a method for creating disease models for DR using the ontologies BioTopLite2 and SNOMED CT and different biomedical resources: 1) consultation notes from anonymised electronic health records; 2) the clinical practice guideline for DR by the American Academy of Ophthalmology; 3) the BMJ Best Practice for DR; and 4) neural language models from Deep Learning (CBOW and Skip-gram) using a 14M PubMed dataset. As SNOMED CT does not contain disease models, the novelty of this study is twofold: a) evaluation of the utility of CBOW and Skip-gram for obtaining DR disease models from the biomedical literature; and b) the proposed method for building ontology-based disease models exploiting SNOMED CT reference sets. In our method, we first propose a representation of SNOMED CT reference sets for DR in OWL by extracting upper modules from SNOMED CT. Secondly, we use content ontology design patterns with BioTopLite2 and SNOMED CT that act as templates to semantically represent clinical content in OWL. Reflecting upon the sustainability challenges cities will be facing in the near future and the recent technological developments allowing cities to become "smart", we introduce IDEAL-CITIES; a framework aiming to provide an architecture for cyber-physical systems to deliver a datadriven Circular Economy model in a city context. In the IDEALCITIES ecosystem, the city's finite resources as well as citizens will form the pool of intelligent assets in order to contribute to high utilization through crowdsourcing and real-time decision making and planning. We describe two use cases as a vehicle to demonstrate how a smart city can serve the Circular Economy paradigm

Aim To assess the safety and tolerability of adjuvant subcutaneous trastuzumab (Herceptin ® SC, H SC), delivered from an H SC Vial via hand-held syringe (Cohort A) or single-use injection device (Cohort B), with or without chemotherapy, for human epidermal growth factor receptor 2 (HER2)-positive stage I to IIIC early breast cancer (EBC) in the phase III SafeHer study (NCT01566721). Methods Patients received 600 mg fixed-dose H SC every 3 weeks for 18 cycles. The chemotherapy partner was at the investigators' discretion (H SC monotherapy was limited to ≤10% of the population). Data from the first H SC dose until 28 days (plus a 5-day window) after the last dose are presented. Results are descriptive. Results In the overall population, 2282/2573 patients (88.7%) experienced adverse events (AEs). Of the above, 128 (5.0%) patients experienced AEs leading to study drug discontinuation; 596 (23.2%) experienced grade ≥ 3 AEs and 326 (12.7%) experienced serious AEs. Grade ≥ 3 cardiac disorders were reported in 24 patients (0.9%), including congestive heart failure in eight (0.3%). As expected, the AE rates varied according to the timing of chemotherapy in both cohorts, with higher rates in concurrent versus sequential chemotherapy subgroups. In the concurrent chemotherapy subgroup, AEs were more common during the actual period of concurrent chemotherapy compared with the period when patients did not receive concurrent chemotherapy. Conclusion SafeHer confirms the safety and tolerability of the H SC 600 mg fixed dose for 1 year (every 3 weeks for 18 cycles) as adjuvant therapy with concurrent or sequential chemotherapy for HER2-positive EBC. These primary analysis results are consistent with the known safety profile for intravenous H and H SC.

Material and energy recovery within circular economy offer sustainable solutions to this problem by extending the life cycle of textiles through repurposing, recycling, and upcycling. These initiatives not only reduce waste but also contribute to the reduction of the demand for virgin materials (i.e. cotton, wool), ultimately benefiting the environment and society. The circular economy approach, which aims to recreate environmental, economic, and societal value, is based on three key principles: waste reduction, material circulation, and ecological restoration. Given these difficulties, circularity incorporates the material recovery approach, which is focused on the conversion of waste into secondary raw resources. The goal of this notion is to extract more value from resources by prolonging final disposal as long as feasible. When a textile has outlived its functional life, material recovery is critical for returning the included materials or energy into the manufacturing cycle. The aim of this paper is to examine the material and energy recovery options of main raw materials used in the fashion industry while highlighting the need of close observation of the relation between circularity and material recovery, including the investigation of barriers to the transition towards a truly circular fashion industry. The final results refer to the main barriers of circular economy transition within the industry and a framework is proposed. - Abstract
Introduction: In the South African Breast Cancer and HIV Outcomes (SABCHO) study, we previously found that breast cancer patients living with HIV and treated with neoadjuvant chemotherapy achieve lower rates of complete pathologic response than patients without HIV. We now assess the impact of comorbid HIV on receipt of timely and complete neoadjuvant and adjuvant chemotherapy., Materials and Methods: Since June 2015, the SABCHO study has collected data on women diagnosed with breast cancer at 6 South African hospitals. We selected a sample of participants with stages I-III cancer who received ≥2 doses of neoadjuvant or adjuvant chemotherapy. Data on chemotherapies prescribed and received, filgrastim receipt, and laboratory values measured during treatment were captured from patients' medical records. We calculated the mean relative dose intensity (RDI) for all prescribed chemotherapies. We tested for association between full regimen RDI and HIV status, using linear regression to control for demographic and clinical covariates, and for association of HIV with laboratory abnormalities., Results: The 166 participants living with HIV and 159 without HIV did not differ in median chemotherapy RDI: 0.89 (interquartile range (IQR) 0.77-0.95) among those living with HIV and 0.87 (IQR 0.77-0.94) among women without HIV. Patients living with HIV experienced more grade 3+ anemia and leukopenia than those without HIV (anemia: 10.8% vs. 1.9%, P = .001; leukopenia: 8.4% vs. 1.9%, P = .008) and were more likely to receive filgrastim (24.7% vs. 10.7%, P = .001)., Conclusions: HIV status did not impact neoadjuvant or adjuvant chemotherapy RDI, although patients with breast cancer living with HIV experienced more myelotoxicity during treatment., (© The Author(s) 2023.
