2. Development of a Blood-based Multiple-marker Screening Test for Hypertrophic Cardiomyopathy.

Abstract

HCM is the most commonly inherited heart disease, impacting ∼ 700,000 adults in the U.S., yet up to 85% of those may remain undiagnosed. The slow off-rate modifed aptamer (SOMAmer®) SOMAscan® assay can enable assessment of thousands of protein markers in a single blood sample. The primary objective of this study was to develop a proteomic model to differentiate between individuals who have been diagnosed with HCM versus non-HCM controls (healthy or mildly symptomatic individuals without HCM diagnosis). Plasma samples from 8 cohorts were analyzed using SomaScan Assay v4.0/v4.1 to measure approximately 5,000 unique human protein analytes. A total of 764 HCM cases and 4,266 controls were split into subsets of 70%/15%/15% for training, verification, and validation. The outcome measure was a classification endpoint denoting HCM diagnosis (based on LV wall thickness ≥15 mm with appropriate ICD diagnosis code or confirmation that diagnosis met ACCF/AHA guidelines) ("Case") or not ("Control") at the time of blood draw. The diagnosis endpoint was modeled as a binary classification measure. The pre-specified passing criteria were set at a sensitivity ≥ 0.75 and a specificity ≥ 0.85. Robust feature selection led to a 12-plasma protein elastic net logistic regression model which identified HCM cases with validation performance metrics (95% CI) of sensitivity 0.878 (0.823, 0.933), specificity 0.923 (0.902, 0.943) and AUC 0.959 (0.941, 0.974). Plasma proteomic profiling on SomaScan has identified a 12-protein signature that may have utility as a screening test to assist in the diagnosis of HCM. [ABSTRACT FROM AUTHOR]

Abstract

We report genetic maps for diploid (D) and tetraploid (AtDt) Gossypium genomes composed of sequence-tagged sites (STS) that foster structural, functional, and evolutionary genomic studies. The maps include, respectively, 2584 loci at 1.72-cM (~600 kb) intervals based on 2007 probes (AtDt) and 763 loci at 1.96-cM (~500 kb) intervals detected by 662 probes (D). Both diploid and tetraploid cottons exhibit negative crossover interference; i.e., double recombinants are unexpectedly abundant. We found no major structural changes between Dt and D chromosomes, but confirmed two reciprocal translocations between At chromosomes and several inversions. Concentrations of probes in corresponding regions of the various genomes may represent centromeres, while genome-specific concentrations may represent heterochromatin. Locus duplication patterns reveal all 13 expected homeologous chromosome sets and lend new support to the possibility that a more ancient polyploidization event may have predated the A-D divergence of 6-11 million years ago. Identification of SSRs within 312 RFLP sequences plus direct mapping of 124 SSRs and exploration for CAPS and SNPs illustrate the 'portability' of these STS loci across populations and detection systems useful for marker-assisted improvement of the world's leading fiber crop. These data provide new insights into polyploid evolution and represent a foundation for assembly of a finished sequence of the cotton genome.

Abstract

A backcross-self population from a cross between Gossypium hirsutum and G. barbadense was used to dissect the molecular basis of genetic variation governing two parameters reflecting lint fiber fineness and to compare the precision of these two measurements. By applying a detailed restriction fragment length polymorphism (RFLP) map to 3,662 BC3F2 plants from 24 independently derived BC3 families, we were able to detect 32 and nine quantitative trait loci (QTLs) for fiber fineness and micronaire (MIC), respectively. The discovery of larger numbers of QTLs in this study than previously found in other studies based on F2 populations grown in favorable environments reflects the ability of the backcross-self design to resolve smaller QTL effects. Although the two measurements differed dramatically in the number of QTLs detected, seven of the nine MIC QTLs were also associated with fiber fineness. This supports other data in suggesting that fiber fineness more accurately reflects the underlying physical properties of cotton fibers and, consequently, is a preferable trait for selection. “Negative transgression,” with the majority of BC3F2 families showing average phenotypes that were poorer than that of the inferior parent, suggests that many of the new gene combinations formed by interspecific hybridization are maladaptive and may contribute to the lack of progress in utilizing G. barbadense in conventional breeding programs to improve upland cotton.

Abstract

Linkage disequilibrium (LD) mapping provides a powerful method for fine-structure localization of rare disease genes, but has not yet been widely applied to common disease1. We sought to design a systematic approach for LD mapping and apply it to the localization of a gene (IBD5) conferring susceptibility to Crohn disease. The key issues are: (i) to detect a significant LD signal (ii) to rigorously bound the critical region and (iii) to identify the causal genetic variant within this region. We previously mapped the IBD5 locus to a large region spanning 18 cM of chromosome 5q31 (P<10−4). Using dense genetic maps of microsatellite markers and single-nucleotide polymorphisms (SNPs) across the entire region, we found strong evidence of LD. We bound the region to a common haplotype spanning 250 kb that shows strong association with the disease (P<2×10−7) and contains the cytokine gene cluster. This finding provides overwhelming evidence that a specific common haplotype of the cytokine region in 5q31 confers susceptibility to Crohn disease. However, genetic evidence alone is not sufficient to identify the causal mutation within this region, as strong LD across the region results in multiple SNPs having equivalent genetic evidence—each consistent with the expected properties of the IBD5 locus. These results have important implications for Crohn disease in particular and LD mapping in general.

Abstract

Peroxisome proliferator-activated receptor (PPAR) agonists can cause peripheral edema in susceptible individuals. To investigate the mechanistic basis underlying this adverse event, we performed a candidate gene analysis of patients enrolled in clinical trials of muraglitazar, an investigational PPAR dual agonist, and developed a cell culture-based gene expression assay and nonhuman primate model of edema to study the edemagenic properties of PPAR agonists.

Background:Improved risk stratification strategies are desired to enhance clinical care and trial design in heart failure with reduced ejection fraction (HFrEF).Objective:To comprehensively determine plasma proteins associated with adverse outcomes in patients with HFrEF.Methods:The Penn HF study (PHFS) was a referral cohort from three US HF specialty centers consisting of mostly HFrEF patients (n = 2248, HFrEF n = 1615). A proteomic profile was generated for each baseline PHFS plasma sample with the SomaLogic V4 SOMAscan? assay, based on 5207 aptamers to 4988 unique human protein targets. We tested associations between each protein aptamer level and time to the combined endpoint of death, placement of a ventricular assist device (VAD), or death using Cox proportional hazards in the context of relevant clinical covariates. We adjusted for violation of the proportional hazards assumption and applied Bonferroni adjustment for multiple testing.Results:Over a median follow-up of 4.3 years, there were 626 patients who experienced the composite outcomes. A proteomic profile was generated for each PHFS plasma sample with the SomaLogic V4 SOMAscan? assay, based on 5207 aptamers to 4988 unique human protein targets. We constructed a regularized Cox model using elastic nets to assess the relationship between proteins, select clinical covariates (age, sex and race), and the combined endpoint of death, placement of a ventricular assist device (VAD), or death.Results:Over a median follow-up of 4.3 years, there were 626 patients who experienced the composite outcomes. The regularized Cox model defined an optimal subset of proteins which can be treated as a composite marker, consisting of 37 protein aptamers (with no selected clinical covariates). We computed a linear predictor that combines all of these covariates into a single score to stratify patient survival. In a comparison, this composite marker score outperformed any individual protein to predict death. Our composite score also outperformed the MAGGIC score (composite score: C-index = 0.769, SE = 0.010; MAGGIC: C-index = 0.738, SE = 0.011; MAGGIC computed without COPD and 18 month diagnosis scores).Conclusions:Our study identified an optimal subset of predictive circulating plasma proteins as well as a composite marker capable of enhancing risk stratification in HFrEF.
