1. 'Nothing That I Was Specifically Looking For': LGBTQ+ Youth and Intentional Sexual Health Information Seeking

Abstract: Lesbian, gay, bisexual, transgender, queer, questioning, and non-cisgender or non-heterosexual (LGBTQ+) youth (ages 15 to 25) in the United States often struggle to access relevant and comprehensive sexual health information. LGBTQ+ youth rely on the internet, particularly social media, as an information resource. We interviewed 17 LGBTQ+ youth about their online sexual health information seeking experiences to understand the specific role of social media in meeting their sexual health information needs. We present two main findings: 1. LGBTQ+ youth occupy existing online spaces unrelated to sexual health in explicit ways but they are "exposed to relevant sexual health topics" and inspired to undertake more "intentional online sexual health information seeking;" 2. Social media facilitates the sharing of "lived experiences" in the context of sexual health information seeking which makes it a desirable resource for LGBTQ+ youth. We discuss implications for these findings in the development of sexual health information resources inclusive of LGBTQ+ youth and their experiences.

2. Moderating Sex Ed: How Social Media Content Moderation Impacts Access to Comprehensive Sexual and Reproductive Health Information

Abstract: In the United States, the internet is a vital resource for LGBTQ+ youth to meet their sexual and reproductive health information needs, especially those who cannot receive necessary information from family, healthcare providers, and classrooms. In this dissertation, I present three papers that connect content moderation policies and their impacts with access to comprehensive sexual and reproductive health information. Participants in all three studies developed goals and expectations for social media, which became difficult to achieve in their respective information contexts. Through ingenuity, persistence, and algorithmic folk theorization, participants developed strategies to resist platform policies and attempt to achieve their goals. This dissertation's first paper is an interview study further exploring how LGBTQ+ youth seek sexual and reproductive health information online. LGBTQ+ youth participants occupied existing online spaces not explicitly related to health information but would naturally encounter relevant information online in these spaces. These information encounters led participants to shift their information seeking behaviors to intentional sexual health information seeking. Participants explained social media's importance in sexual health and reproductive health information seeking, although many expressed concerns about information credibility. In the second paper, I interviewed healthcare providers and sexual health educators who create sexual and reproductive health content on social media. I explore how experts created accurate health content that users like the LGBTQ+ youth participants might encounter online. Providers and educators overwhelmingly felt that content moderation policies restricted their abilities to share comprehensive and medically accurate sexual and reproductive health information. Participants developed, tested, and shared algorithmic folk theories to make sense of their experiences as content creators. Shadowbanning was a particularly troublesome content moderation experience for participants. For the final paper, I used qualitative surveys and interviews with social media users who experienced content moderation to further understand how they make sense of what they believe to be shadowbanning. Participants used strategies similar to the providers and educators for understanding their content removals and perceived changes in social media engagement and metrics. I propose algorithmic altruism as one such strategy where users collectively understood and resisted how they experienced platform moderation algorithms. Although shadowbanning is difficult to define and platforms typically deny its existence, participants believed it happened to them and used strategies to "prove" that it happened to them. This dissertation extends previous research on content moderation, affordances theories, and algorithmic folk theories. Sexual and reproductive health information on social media provides a unique context for considering how these three research areas coalesce. Social media's imagined affordances influence what participants think they can achieve online. I argue that participants use resistance strategies such as algorithmic folk theories to still use the social media affordances they imagine are possible, especially when experiencing unclear content moderation practices. Opaque content moderation policies shift the burden to users making sense of their online experiences and evading shadowbanning or content removals. In this sexual and reproductive health context, this shift means that providers and educators struggle to share comprehensive and accurate information with potential viewers like LGBTQ+ young people. 3. Community-Engaged Participatory Methods to Address Lesbian, Gay, Bisexual, Transgender, Queer, and Questioning Young People’s Health Information Needs With a Resource Website: Participatory Design and Development Study

Abstract: BackgroundLesbian, gay, bisexual, transgender, queer, and questioning (LGBTQ+) young people (aged 15 to 25 years) face unique health challenges and often lack resources to adequately address their health information needs related to gender and sexuality. Beyond information access issues, LGBTQ+ young people may need information resources to be designed and organized differently compared with their cisgender and heterosexual peers and, because of identity exploration, may have different information needs related to gender and sexuality than older people. ObjectiveThe objective of our study was to work with a community partner to develop an inclusive and comprehensive new website to address LGBTQ+ young people’s health information needs. To design this resource website using a community-engaged approach, our objective required working with and incorporating content and design recommendations from young LGBTQ+ participants. MethodsWe conducted interviews (n=17) and participatory design sessions (n=11; total individual participants: n=25) with LGBTQ+ young people to understand their health information needs and elicit design recommendations for the new website. We involved our community partner in all aspects of the research and design process. ResultsWe present participants’ desired resources, health topics, and technical website features that can facilitate information seeking for LGBTQ+ young people exploring their sexuality and gender and looking for health resources. We describe how filters can allow people to find information related to intersecting marginalized identities and how dark mode can be a privacy measure to avoid unwanted identity disclosure. We reflect on our design process and situate the website development in previous critical reflections on participatory research with marginalized communities. We suggest recommendations for future LGBTQ+ health websites based on our research and design experiences and final website design, which can enable LGBTQ+ young people to access information, find the right information, and navigate identity disclosure concerns. These design recommendations include filters, a reduced number of links, conscientious choice of graphics, dark mode, and resources tailored to intersecting identities. ConclusionsMeaningful collaboration with community partners throughout the design process is vital for developing technological resources that meet community needs. We argue for community partner leadership rather than just involvement in community-based research endeavors at the intersection of human-computer interaction and health.

4. Red Code Management in a Pediatric Emergency Department: A Retrospective Study

Abstract: The “red code” (RC) represents the highest level of emergency in the emergency department (ED). This study retrospectively analyzed RCs in the Regina Margherita Children’s Hospital ED, a regional referral center in north Italy, between 1 July 2020 and 30 June 2023. The aim was to describe RC characteristics and to identify significant correlations between presenting complaints and clinical management. The study includes 934 RCs (0.9% of overall ED admissions); 64% were assigned based on the Pediatric Assessment Triangle alteration. Most patients, 86.5%, followed the medical pathway, while 13.5% were surgical cases. Admission complaints were respiratory (46.9%), neuropsychiatric (26.7%), traumatic (11.8%), cardiologic (9.3%), metabolic (3.8%), and surgical (1.5%). Seventy-six percent of patients received vascular access, and intraosseous access was obtained in 2.2% of them. In one-third of RCs, an urgent critical care evaluation was necessary, and 19% of cases required admission to the intensive care unit. The overall mortality rate was 3.4% (0.4% in ED setting). The study identified six distinct diagnostic pathways, each associated with specific characteristics in clinical presentation, management, therapeutic interventions, and outcomes. Our findings underscore the need for a systematic approach in pediatric emergency settings, supported by international and national guidelines but also by clearly defined diagnostic pathways, aiming to enhance the quality of care and patient outcomes.

5. First diagnosis of multisystem inflammatory syndrome in children (MIS-C): an analysis of PoCUS findings in the ED

Abstract: Abstract Children with multisystem inflammatory syndrome (MIS-C) tend to develop a clinical condition of fluid overload due both to contractile cardiac pump deficit and to endotheliitis with subsequent capillary leak syndrome. In this context, the ability of point-of-care ultrasound (PoCUS) to simultaneously explore multiple systems and detect polyserositis could promote adequate therapeutic management of fluid balance. We describe the PoCUS findings in a case-series of MIS-C patients admitted to the Emergency Department. At admission 10/11 patients showed satisfactory clinical condition without signs and symptoms suggestive for cardiovascular impairment/shock, but PoCUS showed pathological findings in 11/11 (100%). In particular, according to Rapid Ultrasound in SHock (RUSH) protocol, cardiac hypokinesis was detected in 5/11 (45%) and inferior vena cava dilatation in 3/11 (27%). Peritoneal fluid was reported in 6/11 cases (54%). Lung ultrasound (LUS) evaluation revealed an interstitial syndrome in 11/11 (100%), mainly localized in posterior basal lung segments. We suggest PoCUS as a useful tool in the first evaluation of children with suspected MIS-C for the initial therapeutic management and the following monitoring of possible cardiovascular deterioration.

Abstract: OBJECTIVES:. To describe the prevalence, associated risk factors, and outcomes of serious neurologic manifestations (encephalopathy, stroke, seizure, and meningitis/encephalitis) among patients hospitalized with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. DESIGN:. Prospective observational study. SETTING:. One hundred seventy-nine hospitals in 24 countries within the Society of Critical Care Medicine Discovery Viral Infection and Respiratory Illness Universal Study COVID-19 Registry. PATIENTS:. Hospitalized adults with laboratory-confirmed SARS-CoV-2 infection. INTERVENTIONS:. None. RESULTS:. Of 16,225 patients enrolled in the registry with hospital discharge status available, 2,092 (12.9%) developed serious neurologic manifestations including 1,656 (10.2%) with encephalopathy at admission, 331 (2.0%) with stroke, 243 (1.5%) with seizure, and 73 (0.5%) with meningitis/encephalitis at admission or during hospitalization. Patients with serious neurologic manifestations of COVID-19 were older with median (interquartile range) age 72 years (61.0–81.0 yr) versus 61 years (48.0–72.0 yr) and had higher prevalence of chronic medical conditions, including vascular risk factors. Adjusting for age, sex, and time since the onset of the pandemic, serious neurologic manifestations were associated with more severe disease (odds ratio [OR], 1.49; p < 0.001) as defined by the World Health Organization ordinal disease severity scale for COVID-19 infection. Patients with neurologic manifestations were more likely to be admitted to the ICU (OR, 1.45; p < 0.001) and require critical care interventions (extracorporeal membrane oxygenation: OR, 1.78; p = 0.009 and renal replacement therapy: OR, 1.99; p < 0.001). Hospital, ICU, and 28-day mortality for patients with neurologic manifestations was higher (OR, 1.51, 1.37, and 1.58; p < 0.001), and patients had fewer ICU-free, hospital-free, and ventilator-free days (estimated difference in days, –0.84, –1.34, and –0.84; p < 0.001). CONCLUSIONS:. Encephalopathy at admission is common in hospitalized patients with SARS-CoV-2 infection and is associated with worse outcomes. While serious neurologic manifestations including stroke, seizure, and meningitis/encephalitis were less common, all were associated with increased ICU support utilization, more severe disease, and worse outcomes.

Abstract: IMPORTANCE:. At the start of the coronavirus disease 2019 pandemic, medications repurposed for management of coronavirus disease 2019 were used in the absence of clinical trial evidence. OBJECTIVES:. To describe the variation and evolution in use of repurposed medications for coronavirus disease 2019. DESIGN, SETTING, AND PARTICIPANTS:. Observational cohort study of adults hospitalized with coronavirus disease 2019 between February 15, 2020, and April 12, 2021, across 76 United States and international hospitals within the Society of Critical Care Medicine’s Discovery Viral Infection and Respiratory Illness Universal Study coronavirus disease 2019 registry. MAIN OUTCOMES AND MEASURES:. Hospital variation was quantified using multivariable adjusted random effects logistic regression models and unsupervised clustering. Repurposed medications included antivirals, corticosteroids, hydroxychloroquine, immunomodulators, and therapeutic dose anticoagulants. RESULTS:. Among 7,069 adults hospitalized with coronavirus disease 2019, 1,979 (28%) received antivirals, 2,876 (41%) received corticosteroids, 1,779 (25%) received hydroxychloroquine, 620 (9%) received immunomodulators, and 2,154 (31%) received therapeutic dose anticoagulants. Contribution of hospital site to risk-adjusted variation was 46% for antivirals, 30% for corticosteroids, 48% for hydroxychloroquine, 46% for immunomodulators, and 52% for therapeutic dose anticoagulants. Compared with the early pandemic, the later pandemic practice phenotypes converged with increased use of antivirals (odds ratio, 3.14; 95% CI, 2.40–4.10) and corticosteroids (odds ratio, 5.43; 95% CI, 4.23–6.97), with decreased use of hydroxychloroquine (odds ratio, 0.02; 95% CI, 0.01–0.04) and immunomodulators (odds ratio, 0.49; 95% CI, 0.34–0.70). There was no clinically significant change in the use of therapeutic dose anticoagulants (odds ratio, 1.01; 95% CI, 1.01–1.02). There were no differences in risk-adjusted mortality between hospitals with high rates of repurposed medication use compared with hospitals with low rates of use. CONCLUSIONS AND RELEVANCE:. Hospital variation in the use of repurposed medications varied widely across hospitals early in the pandemic and later converged with the emergence of randomized clinical trials. Platforms developed for rapid activation and enrollment in clinical trials of repurposed medications are needed prior to the next pandemic to expedite effective, evidence-based practice.

Abstract: IMPORTANCE:. Even with its proclivity for older age, coronavirus disease 2019 has been shown to affect all age groups. However, there remains a lack of research focused primarily on the young adult population. OBJECTIVES:. To describe the epidemiology and outcomes of coronavirus disease 2019 and identify the risk factors associated with critical illness and mortality in hospitalized young adults. DESIGN, SETTINGS, AND PARTICIPANTS:. A retrospective cohort study of the Society of Critical Care Medicine’s Viral Infection and Respiratory Illness Universal Study registry. Patients 18–40 years old, hospitalized from coronavirus disease 2019 from March 2020 to April 2021, were included in the analysis. MAIN OUTCOMES AND MEASURES:. Critical illness was defined as a composite of mortality and 21 predefined interventions and complications. Multivariable logistic regression was used to assess associations with critical illness and mortality. RESULTS:. Data from 4,005 patients (152 centers, 19 countries, 18.6% non-U.S. patients) were analyzed. The median age was 32 years (interquartile range, 27–37 yr); 51% were female, 29.4% Hispanic, and 42.9% had obesity. Most patients (63.2%) had comorbidities, the most common being hypertension (14.5%) and diabetes (13.7%). Hospital and ICU mortality were 3.2% (129/4,005) and 8.3% (109/1,313), respectively. Critical illness occurred in 25% (n = 996), and 34.3% (n = 1,376) were admitted to the ICU. Older age (p = 0.03), male sex (adjusted odds ratio, 1.83 [95% CI, 1.2–2.6]), and obesity (adjusted odds ratio, 1.6 [95% CI, 1.1–2.4]) were associated with hospital mortality. In addition to the above factors, the presence of any comorbidity was associated with critical illness from coronavirus disease 2019. Multiple sensitivity analyses, including analysis with U.S. patients only and patients admitted to high-volume sites, showed similar risk factors. CONCLUSIONS:. Among hospitalized young adults, obese males with comorbidities are at higher risk of developing critical illness or dying from coronavirus disease 2019.

Abstract: The UNESCO World Heritage Site of Petra is characterized by a spectacular geo-archaeological landscape that lead to its inscription as World Heritage Site in 1985. Petra is also a fragile site facing a wide diversity of risks, ranging from those posed by environmental factors as well as those attributed to tourism. In recent years, hazardous natural phenomena were registered as increasingly impacting the site, and most specifically the 'Siq', a 1.2 km naturally formed gorge serving as the only tourist entrance to the archaeological park, posing a major threat to cultural heritage and visitors. These recent events have prompted UNESCO Amman Office, in cooperation with the national authorities, to develop a strategy towards prevention and mitigation of instability phenomena at the 'Siq' and, thus, further contribute to the management and conservation of the site through the implementation of the multi-year Italian funded "Siq Stability" project Actions have been primarily focusing on the analysis of the stability conditions of the 'Siq' slopes, the installation of an integrated monitoring system and the definition and implementation of mitigation measures against rock instability. This paper reports a detailed description of the integrated monitoring system installed paying particular attention on the wireless monitoring devices and the EASA applications, which proved to be some of the most successful systems implemented in the framework of the project. Some preliminary results regarding the data retrieved, policies applied and actions taken to ensure long-term sustainability and capacity development of the national authorities are also reported.

Abstract: Esophageal perforation is a welldefined and severe clinical condition. The associated mortality rates range between 5% and 40% and are worsened by delayed diagnosis. Rapid diagnosis and therapy provide the best chance for survival; however, a delay in diagnosis is common, resulting in substantial morbidity and mortality. There are several aetiologies of esophagus perforation. Most esophageal ruptures are secondary to medical instrumentation. Other causes are Boerhaave syndrome, toxic ingestions and radiation, foreign body ingestion, penetrating trauma, and, rarely, blunt chest trauma. We reported the clinical management and the diagnostic work-up case of esophagus perforation due to the foreign body ingestion.

Abstract: Feeding gastrostomy is used worldwide for adults and children with feeding impairment to obtain long-term enteral nutrition. Percutaneous endoscopic gastrostomy insertion is considered the gold standard, but after the first months requires gastrostomy tube replacement with a low-profile button. The replacement is known as an easy procedure, but several minor and major complications may occur during and after the manoeuvre. We describe intraperitoneal bumper migration in a 3-year-old boy, a rare complication following gastrostomy tube replacement, and we discuss the recent literature regarding similar cases.

16. Large scale debris-flow hazard assessment: a geotechnical approach and GIS modelling

Abstract: A deterministic distributed model has been developed for large-scale debris-flow hazard analysis in the basin of River Vezza (Tuscany Region – Italy). This area (51.6 km 2 ) was affected by over 250 landslides. These were classified as debris/earth flow mainly involving the metamorphic geological formations outcropping in the area, triggered by the pluviometric event of 19 June 1996. In the last decades landslide hazard and risk analysis have been favoured by the development of GIS techniques permitting the generalisation, synthesis and modelling of stability conditions on a large scale investigation (>1:10 000). In this work, the main results derived by the application of a geotechnical model coupled with a hydrological model for the assessment of debris flows hazard analysis, are reported. This analysis has been developed starting by the following steps: landslide inventory map derived by aerial photo interpretation, direct field survey, generation of a database and digital maps, elaboration of a DTM and derived themes (i.e. slope angle map), definition of a superficial soil thickness map, geotechnical soil characterisation through implementation of a backanalysis on test slopes, laboratory test analysis, inference of the influence of precipitation, for distinct return times, on ponding time and pore pressure generation, implementation of a slope stability model (infinite slope model) and generalisation of the safety factor for estimated rainfall events with different return times. Such an approach has allowed the identification of potential source areas of debris flow triggering. This is used to detected precipitation events with estimated return time of 10, 50, 75 and 100 years. The model shows a dramatic decrease of safety conditions for the simulation when is related to a 75 years return time rainfall event. It corresponds to an estimated cumulated daily intensity of 280–330 mm. This value can be considered the hydrological triggering threshold for the whole Vezza basin.

Abstract: Abstract Background Adherential pathology is the most common cause of small bowel obstruction. Laparoscopy in small bowel obstruction does not have a clear role yet; surely it doesn't always represent only a therapeutic act, but it is always a diagnostic act, which doesn't interfere with abdominal wall integrity. Methods We performed a review without any language restrictions considering international literature indexed from 1980 to 2007 in Medline, Embase and Cochrane Library. We analyzed the reference lists of the key manuscripts. We also added a review based on international non-indexed sources. Results The feasibility of diagnostic laparoscopy is high (60–100%), while that of therapeutic laparoscopy is low (40–88%). The frequency of laparotomic conversions is variable ranging from 0 to 52%, depending on patient selection and surgical skill. The first cause of laparotomic conversion is a difficult exposition and treatment of band adhesions. The incidence of laparotomic conversions is major in patients with anterior peritoneal band adhesions. Other main causes for laparotomic conversion are the presence of bowel necrosis and accidental enterotomies. The predictive factors for successful laparoscopic adhesiolysis are: number of previous laparotomies ≤ 2, non-median previous laparotomy, appendectomy as previous surgical treatment causing adherences, unique band adhesion as phatogenetic mechanism of small bowel obstruction, early laparoscopic management within 24 hours from the onset of symptoms, no signs of peritonitis on physical examination, experience of the surgeon. Conclusion Laparoscopic adhesiolysis in small bowel obstruction is feasible but can be convenient only if performed by skilled surgeons in selected patients. The laparoscopic adhesiolysis for small bowel obstruction is satisfactorily carried out when early indicated in patients with a low number of laparotomies resulting in a short hospital stay and a lower postoperative morbidity. Although a higher small bowel obstruction recurrence remains the major postoperative risk of the laparoscopic management of these patients.

Since the hyper-inflammatory syndrome develops after the acute viral event and despite the fact that nasopharyngeal and rectal swab turn out to be negative, at least IgG detected in the blood of these patients are quite high.[5] In conclusion, routine serological test for SARS-CoV-2 should be performed in infants with unusual abdominal pain and elevation of inflammatory markers and who had no diagnosis of appendicitis, since early detection and treatment of SARS-CoV-2 hyper-inflammatory syndrome is vital for prompt management. Abstract: Lalibela is located in the northern-central part of Ethiopia, approx. 600 km north of Addis Ababa in Northern Wollo, one of the most structural food deficit areas of the Amhara Region. The town, which has about 12000 inhabitants, is situated at an altitude of 2500 m (Fig. 26.1). In its center, a unique complex of 11 rock-hewn Christian Orthodox churches is loacted. The churches were cut out of the living rock some 800 years ago during the kingdom of King Lalibela (1167–1207) of the Zagwe dynasty. One of these churches, Biet Aba Libanos, is a monolithic church anchored to the rock from which it was carved. Two major damaging phenomena affect the church: weathering of volcanic tuff in the lower part of the edifice and sliding of the façade and lateral walls, as consequence of a prone discontinuity. A first destruction of the façade was already occurred in the past, as consequence of an old planar sliding, still in coincidence of the same joint. Presently, the walls prone to slide are the structures constructed to replace the original rock that collapsed during the slide as well as some of the original rock hewn lateral walls. Kinematical analysis and numerical modeling implemented, clearly evidence the hazardous conditions of the rock hewn church of Biet Aba Libanos and the need of a prompt and proper intervention. Abstract: The village of Craco (Basilicata, Italy), is being affected by severe landslide phenomena mainly due to the geological and geomorphological setting of the area. The village has been interested by a progressive abandon of the population after the occurrence in the time of landslides and earthquakes that caused the disruption of large portions of the urban settlement. Several landslide typologies can be recognized in the area: rock-falls in the upper part of the hill, rotational and translational earth slides, earth-flows, rock lateral spreading. The main purpose of the paper is to reconstruct the evolution of the geological and morphological dynamics acting on the southern slope of Craco, where the largest landslides occurred in the past. The analysis of deep landslide phenomena has been carried out with a multi-temporal geomorphological approach using analogical and digital photogrammetry on 4 different set of aerial photos (1954–1999). The analysis has stressed a progressive retrogression of the crown area in the upper portions of the hill that caused, in the recent past, the degradation or, in some cases, the complete disruption of part of the historical village, where most of cultural heritage was located. The reconstruction of the landslide evolution acting in this area can be very useful as model to transfer in other areas characterized by similar geological and morphological setting that may result in large landslide phenomena. Abstract: The geology of Machu Picchu area is characterized by granitoid bodies that had been emplaced in the axial zones of the main rift system. Deformation of the granite, caused by cooling and tectonic phases, originated 4 main joint sets, regularly spaced (few decimeters to meters). Several slope instability phenomena have been identified and classified according to mechanism, material involved and state of activity. They are mainly related to rock falls, debris flows, rock slides and debris slides. Origin of phenomena is kinematically controlled by the structural setting and relationship with slope face (rock falls, rock slide and debris slides); the accumulated materials are the source for debris flows. Geomorphological evidences of deeper deformations are currently under investigation. A low environmental impact monitoring system has been established on the area with the purpose to minimize equipments usage and, in the mean time, to collect reliable data on surface deformations. The monitoring network is based on a GPS, multitemporal laser scanner survey, Ground based Radar interferometry (GB-SAR) and Satellite Interferometric Synthetic Aperture Radar (InSAR). The preliminary results are partially confirming the field evidences of slope deformation but, in the mean time, they require a longer period of observations since the sliding processes are relatively slow. Mean S-Se concentrations were 1.163 (SD 0.170) μmol/liter, 1.156 (0.163) μmol/liter, and 1.171 (0.179) μmol/liter in the total group, and female and male subjects, respectively. The difference by sex was statistically significant (F= 6.97, P= 0.0083). In male subjects S-Se levels were inversely associated to age (simple correlation coefficient, r= −0.2135, P< 0.001), alcohol consumption (Alcohol, r= −0.0688, P< 0.01), smoking habit (Smoke, r= −0.0663, P< 0.01), body mass index (BMI, r= −0.0596, P< 0.02), and lognormal transformation of triglycerides (Ln-Trig, r= −0.0767, P< 0.005). In multiple regression analysis Age, Smoke, Ln-Trig, and Alcohol remained significantly and inversely related to S-Se; a significant direct association of S-Se to high-density lipoprotein cholesterol (HDL) and non-HDL cholesterol (non-HDL) was also pointed out. In female subjects, S-Se was directly related to HDL (r= 0.1436, P< 0.001) and non-HDL (r= 0.0967, P< 0.001). In multiple regression analysis S-Se was directly related to both HDL and non-HDL and an inverse significant association of S-Se to Age and Alcohol was evidenced. In multivariate regression models analyzing systolic (SEP), diastolic (DBP), and mean blood pressure (MBP) as dependent variables, S-Se was a weak significant positive predictor in male but not in female subjects.

Abstract: We describe a 3‐year‐old boy with complete agenesis of corpus callosum, developmental delay/mental retardation, anterior diaphragmatic hernia, Morgagni type, severe hypermetropia, and facial dysmorphism suggesting the diagnosis of Donnai–Barrow syndrome. Subtelomeric FISH analysis revealed a paternally‐derived t(9;16) (q34.3;q24.3) translocation with partial 9q monosomy and partial 16q trisomy. As some facial features resemble the 9q emerging phenotype, we suggest the hypothesis that some patients with Donnai–Barrow syndrome might be abscribed to 9q terminal deletion. © 2006 Wiley‐Liss, Inc.

46. Emergency treatment of complicated incisional hernias: a case study.

Abstract: Background: The emergency treatment of incisional hernias is infrequent but it can be complicated with strangulation or obstruction and in some cases the surgical approach may also include an intestinal resection with the possibility of peritoneal contamination. Our study aims at reporting our experience in the emergency treatment of complicated incisional hernias., Methods: Since January 1999 till July 2008, 89 patients (55 males and 34 females) were treated for complicated incisional hernias in emergency. The patients were divided in two groups: Group I consisting of 33 patients that were treated with prosthesis apposition and Group II, consisting of 56 patients that were treated by performing a direct abdominal wall muscles suture., Results: All the patients underwent a 6-month follow up; we noticed 9 recurrences (9/56, 16%) in the patients treated with direct abdominal wall muscles suture and 1 recurrence (1/33, 3%) in the group of patients treated with the prosthesis apposition., Conclusions: According to our experience, the emergency treatment of complicated incisional hernias through prosthesis apposition is always feasible and ensures less post-operative complications (16% vs 21,2%) and recurrences (3% vs 16%) compared to the patients treated with direct muscular suture.

47. Virtual colonoscopy in stenosing colorectal cancer.

Abstract: Background: Between 5 and 10% of the patients undergoing a colonoscopy cannot have a complete procedure mainly due to stenosing neoplastic lesion of rectum or distal colon. Nevertheless the elective surgical treatment concerning the stenosis is to be performed after the pre-operative assessment of the colonic segments upstream the cancer. The aim of this study is to illustrate our experience with the Computed Tomographic Colonography (CTC) for the pre-operative assessment of the entire colon in the patients with stenosing colorectal cancers., Methods: From January 2005 till March 2009, we observed and treated surgically 43 patients with stenosing colorectal neoplastic lesions. All patients did not tolerate the pre-operative colonoscopy. For this reason they underwent a pre-operative CTC in order to have a complete assessment of the entire colon. All patients underwent a follow-up colonoscopy 3 months after the surgical treatment. The CTC results were compared with both macroscopic examination of the specimen and the follow-up coloscopy., Results: The pre-operative CTC showed four synchronous lesions in four patients (9.3% of the cases). The macroscopic examination of the specimen revealed three small sessile polyps (3-4 mm in diameter) missed in the pre-operative assessment near the stenosing colorectal cancer. The follow-up colonoscopy showed four additional sessile polyps with a diameter between 3-11 mm in three patients. Our experience shows that CTC has a sensitivity of 83,7%., Conclusion: In patients with stenosing colonic lesions, CTC allows to assess the entire colon pre-operatively avoiding the need of an intraoperative colonoscopy. More synchronous lesions are detected and treated at the time of the elective surgery for the stenosing cancer avoiding further surgery later on.
