To fill this gap, we performed a multicentric analysis of the real-world treatment outcomes with the propensity score matching in a cohort of Italian patients with unresectable HCC who were treated with either sorafenib or lenvatinib.Aims and Methods: To evaluate the effectiveness of sorafenib and lenvatinib as primary treatment of advanced HCC in clinical practice we performed a multicentric analysis of the treatment outcomes of 288 such patients recruited in 11 centers in Italy. A propensity score was used to mitigate confounding due to referral biases in the assessment of mortality and progression-free survival.Results: Over a follow-up period of 11 months the Cox regression model showed 48% reduction of death risk for patients treated with lenvatinib (95% CI: 0.34– 0.81; p = 0.0034), compared with those treated with sorafenib. The median PFS was 9.0 and 4.9 months for lenvatinib and sorafenib arm, respectively. Patients treated with lenvatinib showed a higher percentage of response rate (29.4% vs 2.8%; p < 0.00001) compared with patients treated with sorafenib. Sorafenib was shown to be correlated with more HFSR, diarrhea and fatigue, while lenvatinib with more hypertension and fatigue.Conclusion: Our study highlighted for the first time the efficacy and safety of lenvatinib in an Italian cohort of patients.Keywords: hepatocarcinoma, sorafenib, lenvatinib

2. A comparison principle based on couplings of partial integro-differential operators

Abstract

This paper is concerned with a comparison principle for viscosity solutions to Hamilton-Jacobi (HJ), -Bellman (HJB), and -Isaacs (HJI) equations for general classes of partial integro-differential operators. Our approach innovates in three ways: (1) We reinterpret the classical doubling-of-variables method in the context of second-order equations by casting the Ishii-Crandall Lemma into a test function framework. This adaptation allows us to effectively handle non-local integral operators, such as those associated with L\'evy processes. (2) We translate the key estimate on the difference of Hamiltonians in terms of an adaptation of the probabilistic notion of couplings, providing a unified approach that applies to differential, difference, and integral operators. (3) We strengthen the sup-norm contractivity resulting from the comparison principle to one that encodes continuity in the strict topology. We apply our theory to a variety of examples, in particular, to second-order differential operators and, more generally, generators of spatially inhomogeneous L\'evy processes.

3. Existence of viscosity solutions for Hamilton-Jacobi equations on Riemannian Manifolds via Lyapunov control

Abstract

We give a new perspective on the existence of viscosity solutions for a stationary and a time-dependent first order Hamilton-Jacobi equation. Following recent comparison principles, we work in a framework in which we consider a subsolution and a supersolution for two equations in terms of two Hamiltonians that can be seen as an upper and lower bound of our original Hamiltonian respectively. The upper and lower bound are formulated in terms of a relaxed Lyapunov function which allows us to restrict part of the analysis to compact sets and to work with almost optimizers of the considered control problems. For this reason, we can relax assumptions on the control problem: most notably, we do not need completeness of set of controlled paths. Moreover, our strategy avoids a-priori analysis on the regularity of the candidate solutions. We then explore the context of Hamilton-Jacobi-Bellman and Hamilton-Jacobi-Isaacs equations to verify our assumptions in a convex and non-convex setting respectively.

4. Dynamical properties of critical exponent functions

Abstract

In the last years the attention towards topological dynamical properties of highly discontinuous maps has increased significantly. In [D.Corona, A. Della Corte. The critical exponent functions. Comptes Rendus Math\'ematique, 360(G4), 315-332, 2022], a class of densely discontinuous interval maps, called "critical exponent maps", was introduced. These maps are defined through the word-combinatorics concept of critical exponent applied to the binary expansion of reals and show highly chaotically properties as well as some challenging problems. In this paper we identify an error in the proof of Theorem 7 in [D.Corona, A. Della Corte. The critical exponent functions. Comptes Rendus Math\'ematique, 360(G4), 315-332, 2022], a purely combinatorial result which in fact does not hold. We show that most of the results in [D.Corona, A. Della Corte. The critical exponent functions. Comptes Rendus Math\'ematique, 360(G4), 315-332, 2022], obtained there through Theorem 7, can be recovered. Moreover, we propose as a conjecture a weaker form of Theorem 7.

Abstract

Despite considerable progress in developing artificial intelligence (AI) algorithms for prostate cancer detection from whole slide images, the clinical applicability of these models remains limited due to variability in pathological annotations and existing dataset limitations. This article proposes a novel approach to overcome these challenges by leveraging a Bayesian framework to seamlessly integrate new data, and present results as a panel of annotations. The framework is demonstrated by integrating a Bayesian prior with one trained AI model to generate a distribution of Gleason patterns for each pixel of an image. It is shown that using this distribution of Gleason patterns rather than a ground-truth label can improve model applicability, mitigate errors, and highlight areas of interest for pathologists. Additionally, we present a high-quality, hand-curated dataset of prostate histopathological images annotated at the gland level by trained pre-medical students and verified by an expert pathologist. We highlight the potential of this adaptive and uncertainty-aware framework for developing clinically deployable AI tools that can support pathologists in accurate prostate cancer grading, improve diagnostic accuracy, and create positive patient outcomes., Comment: 5 pages, 3 figures, one table. Abstract

We prove comparison principle for viscosity solutions of a Hamilton-Jacobi-Bellman equation in a strong coupling regime considering a stationary and a time-dependent version of the equation. We consider a Hamiltonian that has a representation as the supremum of a difference of two functions: an internal Hamiltonian depending on a control variable and a function interpreted as a cost of applying the controls. Our major innovation lies in the use of a cost function that can be discontinuous, unbounded and depending on momenta, enabling us to address previously unexplored scenarios such as cases arising from the theory of large deviations and homogenisation. For completeness, we also state the existence of viscosity solutions and we verify the assumptions for an example arising from biochemistry.

Abstract

Abstract Background The contemporary management of mass casualty incidents (MCIs) relies on the effective application of predetermined, dedicated response plans based on current best evidence. Currently, there is limited evidence regarding the factors influencing the accuracy of first responders (FRs) in applying the START protocol and the associated prehospital times during the response to MCIs. The objective of this study was to investigate factors affecting FRs’ accuracy in performing prehospital triage in a series of simulated mass casualty exercises. Secondly, we assessed factors affecting triage-to-scene exit time in the same series of exercises. Methods This retrospective study focused on simulated casualties in a series of simulated MCIs Full Scale Exercises. START triage was the triage method of choice. For each Full-Scale Exercise (FSEx), collected data included exercise and casualty-related information, simulated casualty vital parameters, simulated casualty anatomic lesions, scenario management times, and responder experience. Results Among the 1090 casualties included in the primary analysis, 912 (83.6%) were correctly triaged, 137 (12.6%) were overtriaged, and 41 (3.7%) were undertriaged. 15. A cohort-based Partial Internal Model for demographic risk

Abstract

We investigate the quantification of demographic risk in a framework consistent with the market-consistent valuation imposed by Solvency II. We provide compact formulas for evaluating inflows and outflows of a portfolio of insurance policies based on a cohort approach. In this context, we maintain the highest level of generality in order to consider both traditional policies and equity-linked policies: therefore, we propose a market-consistent valuation of the liabilities. In the second step we evaluate the Solvency Capital Requirement of the idiosyncratic risk, linked to accidental mortality, and the systematic risk one, also known as trend risk, proposing a formal closed formula for the former and an algorithm for the latter. We show that accidental volatility depends on the intrinsic characteristics of the policies of the cohort (Sums-at-Risk), on the age of the policyholders and on the variability of the sums insured; trend risk depends both on accidental volatility and on the longevity forecasting model used.

Abstract

In this note, our aim is to show that families of smooth hypersurfaces of $\mathbb R^{n+1}$ which are all $C^1$--close enough to a fixed compact, embedded one, have uniformly bounded constants in some relevant inequalities for mathematical analysis, like Sobolev, Gagliardo-Nirenberg and ``geometric'' Calder\'on-Zygmund inequalities. This technical result is quite useful, in particular, in the study of the geometric flows of hypersurfaces.

Abstract

The NASA Double Asteroid Redirection Test (DART) mission performed a kinetic impact on asteroid Dimorphos, the satellite of the binary asteroid (65803) Didymos, at 23:14 UTC on September 26, 2022 as a planetary defense test. DART was the first hypervelocity impact experiment on an asteroid at size and velocity scales relevant to planetary defense, intended to validate kinetic impact as a means of asteroid deflection. Here we report the first determination of the momentum transferred to an asteroid by kinetic impact. Based on the change in the binary orbit period, we find an instantaneous reduction in Dimorphos's along-track orbital velocity component of 2.70 +/- 0.10 mm/s, indicating enhanced momentum transfer due to recoil from ejecta streams produced by the impact. For a Dimorphos bulk density range of 1,500 to 3,300 kg/m$^3$, we find that the expected value of the momentum enhancement factor, $\beta$, ranges between 2.2 and 4.9, depending on the mass of Dimorphos. If Dimorphos and Didymos are assumed to have equal densities of 2,400 kg/m$^3$, $\beta$= 3.61 +0.19/-0.25 (1 $\sigma$). These $\beta$ values indicate that significantly more momentum was transferred to Dimorphos from the escaping impact ejecta than was incident with DART. Abstract

Urinary tract infections (UTIs) are the most common bacterial infections in children, occurring both in children with normal urinary tracts and in ones with urinary tract abnormalities. Children with UTIs can present relevant clinical symptoms and risk long-term consequences. Current recommended preventive measures include chemoprophylaxis and dietary supplements such as cranberry, probiotics and vitamins A and E. Although chemoprophylaxis still represents the gold standard, it raises concerns about antimicrobial resistance. N-acetylcysteine (NAC), a precursor of the antioxidant glutathione, has been proven both to inhibit biofilm formation and to destroy developed biofilms. In adults, NAC has been demonstrated to prevent UTIs and to improve the effect of antibiotics, but so far it has not been analyzed as an antimicrobial option for pediatric UTIs. In this work, we aim to discuss the current applications of NAC in adult urology and its future possible evolutions in pediatric urology.

Abstract

Abstract Images collected during NASA’s Double Asteroid Redirection Test (DART) mission provide the first resolved views of the Didymos binary asteroid system. These images reveal that the primary asteroid, Didymos, is flattened and has plausible undulations along its equatorial perimeter. At high elevations, its surface is rough and contains large boulders and craters; at low elevations its surface is smooth and possesses fewer large boulders and craters. Didymos’ moon, Dimorphos, possesses an intimate mixture of boulders, several asteroid-wide lineaments, and a handful of craters. The surfaces of both asteroids include boulders that are large relative to their host body, suggesting that both asteroids are rubble piles. Abstract

Abstract Spacecraft observations revealed that rocks on carbonaceous asteroids, which constitute the most numerous class by composition, can develop millimeter-to-meter-scale fractures due to thermal stresses. However, signatures of this process on the second-most populous group of asteroids, the S-complex, have been poorly constrained. Here, we report observations of boulders’ fractures on Dimorphos, which is the moonlet of the S-complex asteroid (65803) Didymos, the target of NASA’s Double Asteroid Redirection Test (DART) planetary defense mission. We show that the size-frequency distribution and orientation of the mapped fractures are consistent with formation through thermal fatigue. The fractures’ preferential orientation supports that these have originated in situ on Dimorphos boulders and not on Didymos boulders later transferred to Dimorphos. Based on our model of the fracture propagation, we propose that thermal fatigue on rocks exposed on the surface of S-type asteroids can form shallow, horizontally propagating fractures in much shorter timescales (100 kyr) than in the direction normal to the boulder surface (order of Myrs). The presence of boulder fields affected by thermal fracturing on near-Earth asteroid surfaces may contribute to an enhancement in the ejected mass and momentum from kinetic impactors when deflecting asteroids.

Abstract

Abstract Asteroids smaller than 10 km are thought to be rubble piles formed from the reaccumulation of fragments produced in the catastrophic disruption of parent bodies. Ground-based observations reveal that some of these asteroids are today binary systems, in which a smaller secondary orbits a larger primary asteroid. However, how these asteroids became binary systems remains unclear. Here, we report the analysis of boulders on the surface of the stony asteroid (65803) Didymos and its moonlet, Dimorphos, from data collected by the NASA DART mission. The size-frequency distribution of boulders larger than 5 m on Dimorphos and larger than 22.8 m on Didymos confirms that both asteroids are piles of fragments produced in the catastrophic disruption of their progenitors. Dimorphos boulders smaller than 5 m have size best-fit by a Weibull distribution, which we attribute to a multi-phase fragmentation process either occurring during coalescence or during surface evolution. The density per km2 of Dimorphos boulders ≥1 m is 2.3x with respect to the one obtained for (101955) Bennu, while it is 3.0x with respect to (162173) Ryugu. Such values increase once Dimorphos boulders ≥5 m are compared with Bennu (3.5x), Ryugu (3.9x) and (25143) Itokawa (5.1x). This is of interest in the context of asteroid studies because it means that contrarily to the single bodies visited so far, binary systems might be affected by subsequential fragmentation processes that largely increase their block density per km2. Direct comparison between the surface distribution and shapes of the boulders on Didymos and Dimorphos suggest that the latter inherited its material from the former. This finding supports the hypothesis that some asteroid binary systems form through the spin up and mass shedding of a fraction of the primary asteroid.

The European Space Agency's Rosetta mission to comet 67P/Churyumov-Gerasimenko has offered scientists the opportunity to study a comet in unprecedented detail. Four instruments of the Rosetta orbiter, namely, the Micro-Imaging Dust Analysis System (MIDAS), the Grain Impact Analyzer and Dust Accumulator (GIADA), the COmetary Secondary Ion Mass Analyser (COSIMA), and the Rosetta Orbiter Spectrometer for Ion and Neutral Analysis (ROSINA) have provided information on cometary dust particles. Cross-instrument comparisons are crucial to characterize cometary dust particles beyond the capabilities of individual sensors, as they are sensitive to different dust components. We present the first comparison between detections of the ROSINA COmet Pressure Sensor (COPS) and GIADA. These two instruments are complementary as the former is sensitive solely to volatiles of icy particles, while the latter measured the dust particle as a whole, including refractories and condensed (semi)volatiles. Our goal is to correlate the particles detected by COPS and GIADA and to assess whether they belong to a common group. We statistically analyzed the in situ data of COPS and GIADA by calculating Pearson correlation coefficients. Among the several types of particles detected by GIADA, we find that COPS particles are significantly correlated solely with GIADA fluffy agglomerates (Pearson correlation coefficient of 0.55 and p-value of $4.6\cdot 10^{-3}$). This suggests that fluffy particles are composed of both refractories and volatiles. COPS volatile volumes, which may be represented by equivalent spheres with a diameter in the range between 0.06 $\mu$m and 0.8 $\mu$m, are similar to the sizes of the fractal particle's subunits identified by MIDAS (i.e., 0.05-0.18 $\mu$m)., Comment: 6 pages, 3 figures, accepted for publication in A&A

Abstract

We consider the context of molecular motors modelled by a diffusion process driven by the gradient of a weakly periodic potential that depends on an internal degree of freedom. The switch of the internal state, that can freely be interpreted as a molecular switch, is modelled as a Markov jump process that depends on the location of the motor. Rescaling space and time, the limit of the trajectory of the diffusion process homogenizes over the periodic potential as well as over the internal degree of freedom. Around the homogenized limit, we prove the large deviation principle of trajectories with a method developed by Feng and Kurtz based on the analysis of an associated Hamilton--Jacobi--Bellman equation with an Hamiltonian that here, as an innovative fact, depends on both position and momenta.

Abstract

To report a rare case of a right interstitial pregnancy spontaneously occurring in a patient who had previously undergone homolateral salpingo-oophorectomy, and to propose possible explanations for the mechanisms involved in the genesis of this rare scenario. A 32-year-old G3P1 female presented to our emergency room with symptoms related to a suspected ectopic interstitial pregnancy managed in another hospital using a conservative pharmacological approach. After discussing the risks, firstly she underwent a transvaginal ultrasound examination, then a diagnostic hysteroscopy to clarify the unclear ultrasound finding, followed by a successful robot-assisted laparoscopic cornual resection. Hysteroscopy demonstrated an empty uterine cavity, confirming the suspect of pregnancy localization into the interstitial portion of the tubal stump. Through the robot-assisted laparoscopic approach, all the trophoblastic tissue was removed without causing significant damage to the surrounding myometrium and preserving the patient’s fertility. No post-operative complications were recorded. The robotic approach successfully allowed the cornual resection, with minimal blood loss and optimal suturing of the uterine defect. Although our knowledge is still limited, it is possible that the pregnancy nested in the tubal residue after being properly fertilized into the intact tube. However, it cannot be ruled out that there have been remodeling phenomena of the tubal residue so that it has acquired the ability to intercept the oocyte.

Abstract

Process towers, whose purpose includes executing operations of mass transfer such as stripping, distillation, or liquid-liquid extraction, are usually constructed as vertical steel cylindrical pressurised shells, supported on a cylindrical skirt. Seismic risk analysts in an industrial setting, especially in NaTech context, are often faced with the issue that structural vulnerability models are not always available for the various industrial facilities. Process towers, although widespread, are one of those cases. The objective of the study presented in the paper is to simulate the behaviour of such towers when subjected to earthquake-induced ground shaking, with the goal of numerically deriving seismic fragility models. To this end, as a case study, two towers were considered with the same height and internal diameter, but different operating pressures, which determines the thickness of the pressure vessel and, consequently, the configuration of the anchorage system. For each of the two models, multi-stripe dynamic analysis was conducted, using a selection of hazard-consistent ground motion records, on joystick models. The results of the dynamic analyses are presented and their use for the derivation of fragility functions for certain damage states is discussed. This modelling strategy can contribute to streamline the fragility evaluation of such structures, given the variety of geometries which appear in industrial plants.

Abstract

Summary: Background: The inflammatory response in patients with traumatic brain injury (TBI) offers opportunities for stratification and intervention. Previous unselected approaches to immunomodulation in patients with TBI have not improved patient outcomes. Methods: Serum and plasma samples from two prospective, multi-centre observational studies of patients with TBI were used to discover (Collaborative European NeuroTrauma Effectiveness Research [CENTER-TBI], Europe) and validate (Transforming Research and Clinical Knowledge in Traumatic Brain Injury [TRACK-TBI] Pilot, USA) individual variations in the immune response using a multiplex panel of 30 inflammatory mediators. Mediators that were associated with unfavourable outcomes (Glasgow outcome score-extended [GOS-E] ≤ 4) were used for hierarchical clustering to identify patients with similar signatures. Findings: Two clusters were identified in both the discovery and validation cohorts, termed early-inflammatory and pauci-inflammatory. The early-inflammatory phenotype had higher concentrations of interleukin-6 (IL-6), IL-15, and monocyte chemoattractant protein 1 (MCP1). Patients with the early-inflammatory phenotype were older and more likely to have an unfavourable GOS-E at 6 months. There were no differences in the baseline injury severity scores between patients in each phenotype. A combined IL-15 and MCP1 signature identified patients with the early-inflammatory phenotype in both cohorts. Inflammatory processes mediated outcomes in older patients with moderate-severe TBI. Interpretation: Our findings offer a precision medicine approach for future clinical trials of immunomodulation in patients with TBI, by using inflammatory signatures to stratify patients. Abstract

Summary: Background: Traumatic brain injury is conventionally categorised as mild, moderate, or severe on the Glasgow Coma Scale (GCS). Recently developed biomarkers can provide more objective and nuanced measures of the extent of brain injury. Methods: Exposure–response relationships were investigated in 2479 patients aged ≥16 enrolled in the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) prospective observational cohort study. Neurofilament protein-light (NFL), ubiquitin carboxy-terminal hydrolase L1 (UCH-L1), and glial fibrillary acidic protein (GFAP) were assayed from serum sampled in the first 24 h; concentrations were divided into quintiles within GCS severity groups. Relationships with the Glasgow Outcome Scale-Extended were examined using modified Poisson regression including age, sex, major extracranial injury, time to sample, and log biomarker concentration as covariates. Findings: Within severity groups there were associations between biomarkers and outcomes after adjustment for covariates: GCS 13–15 and negative CT imaging (relative risks [RRs] from 1.28 to 3.72), GCS 13–15 and positive CT (1.21–2.81), GCS 9–12 (1.16–2.02), GCS 3–8 (1.09–1.94). RRs were associated with clinically important differences in expectations of prognosis. In patients with GCS 3 (RRs 1.51–1.80) percentages of unfavourable outcome were 37–51% in the lowest quintiles of biomarker levels and reached 90–94% in the highest quintiles. Similarly, for GCS 15 (RRs 1.83–3.79), the percentages were 2–4% and 19–28% in the lowest and highest biomarker quintiles, respectively. Interpretation: Conventional TBI severity classification is inadequate and underestimates heterogeneity of brain injury and associated outcomes. The adoption of circulating biomarkers can add to clinical assessment of injury severity. Abstract

Orchestration systems are becoming a key component to automatically manage distributed computing resources in many fields with criticality requirements like Industry 4.0 (I4.0). However, they are mainly linked to OS-level virtualization, which is known to suffer from reduced isolation. In this paper, we propose RunPHI with the aim of integrating partitioning hypervisors, as a solution for assuring strong isolation, with OS-level orchestration systems.
