Objectives: Myringoplasties are frequently performed in pediatric patients, but there is still controversy regarding the best surgical technique and factors influencing its success. The aim of this study is to characterize a population of pediatric patients undergoing myringoplasty and evaluate the following outcomes: perforation closure, audiometric results and factors affecting surgical success. Study Design: Descriptive retrospective study Material and Methods: We evaluated 56 patients (57 ears) with a mean age of 10.4±3.2 years (6-17 years) over a 5-year period (2016-2020) Results: Most perforations occurred after grommet insertion. Retroauricular approach and underlay temporal fascia graft placement technique was used in most cases, with a success rate of 74% after a mean follow-up of 30 months. Pure tone average improved after the procedure. Craniofacial malformations were associated with incomplete air-bone gap closure. Conclusions: Pediatric myringoplasty has a high success rate. Objetivos: As miringoplastias são cirurgias frequentemente realizadas em idade pediátrica, existindo controvérsia quanto à melhor técnica cirúrgica e fatores que influenciam o seu sucesso. O objetivo deste estudo é caracterizar uma população de doentes pediátricos submetidos a miringoplastia e avaliar como desfechos: o encerramento da perfuração, resultado audiométrico e existência de factores determinantes de sucesso cirúrgico. A técnica cirúrgica mais utilizada foi a abordagem retroauricular com colocação de fascia temporalis underlay obtendo-se uma taxa de sucesso de 74% após 30 meses de seguimento médio. O limiar tonal médio da via aérea melhorou significativamente após o procedimento. A presença de malformações craniofaciais relacionou-se com um menor encerramento do Rinne audiométrico. Conclusões: As miringoplastias em idade pediátrica têm uma elevada taxa de sucesso.

O cádmio é um metal pesado tóxico e carcinogénico, que representa uma ameaça séria para a saúde humana. A população geral pode encontrar-se exposta ao cádmio por diversas vias e o estudo INSEF-ExpoQuim visou caracterizar a exposição da população portuguesa adulta (28-39 anos ) a este agente químico através de um estudo transversal. Este estudo incluiu 295 indivíduos, os quais reponderam a um questionário (dados sociodemográficos, de estilos de vida e possíveis vias de exposição ) e facultaram uma amostra de urina para quantificação de cádmio por espectrometria de massa com plasma indutivo acoplado ( ICP- -MS ). Observou-se uma média geométrica de 0,092 μg Cd/g creatinina [ IC ( 95%): 0,084-0,101 ], apresentando as mulheres valores mais elevados ( 0,103 vs 0,079 ), bem como os fumadores ( 0,135 vs 0,081). O arsénio é um elemento naturalmente presente no meio ambiente e que também pode ser libertado em resultado de atividades antropogénicas, podendo encontrar-se na forma de compostos orgânicos ou inorgânicos. O arsénio é considerado pela Organização Mundial de Saúde como um dos dez químicos de maior preocupação para a saúde pública, uma vez que o arsénio e os compostos inorgânicos de arsénio estão classificados como cancerígenos pela Agência Internacional para a Investigação do Cancro. A exposição a arsénio inorgânico ocorre principalmente por via alimentar, através do consumo de água e de alimentos contaminados. Em Portugal são escassos os dados sobre a exposição a arsénio na população geral, pelo que o objetivo do presente estudo é a caracterização da exposição da população adulta portuguesa a arsénio, utilizando amostras de urina colhidas num estudo populacional nacional ( INSEF-ExpoQuim). O presente trabalho incluiu 171 participantes no estudo INSEF-ExpoQuim que forneceram uma amostra de urina para análise da concentração de arsénio total por espectrometria de massa com plasma indutivo acoplado ( ICP-MS ). Observou-se uma média geométrica de 47,1 μg As/L [ IC95%: 41,4-53,5 ] e 8,8% dos indivíduos apresentavam valores de arsénio considerados elevados. Não se observaram diferenças nos níveis de arsénio por sexo, grupo etário, grau de urbanização da área de residência, nível de escolaridade e situação face ao trabalho. Os resultados preliminares obtidos apontam para a necessidade de monitorização dos níveis de arsénio na população portuguesa. Arsenic is an element naturally present in the environment, but it can also be released as a result of anthropogenic activities. In the environment, arsenic can be found in the form of organic compounds and in the form of inorganic compounds. Arsenic is considered by the World Health Organization as one of the ten chemicals of greatest concern for public health, since arsenic and inorganic arsenic compounds are classified as carcinogenic by the International Agency for Research on Cancer. Exposure to inorganic arsenic occurs mainly through food, through consumption of contaminated water and food. In Portugal, data on arsenic exposure in the general population are scarce, so the aim of the present study was to characterize the exposure of the Portuguese adult population to arsenic, using urine samples collected in a national population study ( INSEF-ExpoQuim). The present work included 171 participants in the INSEF-ExpoQuim study who provided a urine sample for analysis of total arsenic concentration by inductively coupled plasma mass spectrometry (ICP-MS). A geometric mean of 47.1 μg As/L [ 95% CI: 41.4-53.5 ] was observed and 8.8% of the individuals had arsenic levels considered high. No differences were observed in arsenic levels by sex, age group, degree of urbanization of the area of residence, educational level and employment situation. The obtained preliminary results point to the need to monitor the levels of arsenic in the Portuguese population. A dieta vegetariana é um padrão alimentar que tem ganho popularidade na última década em todo o mundo, por motivos éticos, de saúde, religiosos e também ambientais. No entanto, a preocupação com a escolha desta dieta e a supressão de todas as necessidades nutricionais tem vindo a ganhar relevância. Assim, as carências nutricionais, nomeadamente de iodo, na população vegetariana, têm sido alvo de estudos no sentido de entender se a alimentação, por si só, consegue fornecer os valores diários recomendados para este elemento. Neste estudo, determinou-se o teor de iodo, por espectrometria de massa com plasma indutivo acoplado ( ICP-MS), em 37 alimentos mais consumidos pela população vegetariana. A seleção dos alimentos foi feita tendo por base o Inquérito Alimentar Nacional e de Atividade Física. Entre os alimentos analisados as algas Nori apresentam os valores mais elevados de iodo com cerca de 6511 ± 321 μg/100 g, podendo ser um alimento que se consumido na quantidade de cerca de 2,3 g/dia (equivalente a 1 folha) poderá suprir as necessidades nutricionais deste elemento. Também os lacticínios, nomeadamente os queijos (entre 53,4 ± 0,4 e 28,0 ± 0,9 μg/100 g) e os iogurtes (entre 17,2 ± 0,2 e 14,4 ± 0,1 μg/100 g), são alimentos com teores de iodo relevantes. Apenas com a alimentação, a população vegetariana e vegan poderá ter dificuldade em alcançar as necessidades diárias de iodo (150 μg/dia ), sendo assim necessário criar alternativas para aumentar os teores de iodo na alimentação. A fortificação das plantações e também dos alimentos, a utilização de sal iodado na confeção ou, se necessário, a suplementação desta população mais suscetível, são estratégias alternativas possíveis. É essencial perceber mais sobre o padrão de alimentação desta população a fim de perceber a melhor forma de mitigar este tipo de carência. The vegetarian diet is a dietary pattern that has gained popularity in the last decade around the world, due to ethical, health, religious and environmental reasons. However, the concern with the choice of this diet and the suppression of all nutritional needs has gained relevance. Nutritional deficiencies, namely iodine in this population, have been the subject of studies to understand whether diet by itself can provide the recommended daily values for this element. Thus, the iodine content was determined by inductively coupled plasma mass spectrometry ( ICP-MS ) in thirty-seven foods most consumed by the vegetarian population. The selection of foods was based on the National Food and Physical Activity Survey of Portugal. Among the analysed foods, Nori seaweeds have the highest levels of iodine with about 6511 ± 321 μg/100 g, which may be a way to meet the nutritional needs of this element, consuming about 2.3 g/day (equivalent to 1 leaf), followed by dairy products, namely cheeses (between 53.4 ± 0.4 and 28.0 ± 0.9 μg/100 g) and yoghur ts (between 17.2 ± 0.2 and 14.4 ± 0.1 μg /100 g). Just with food, the vegetarian and vegan population may have difficulty to achieve the daily iodine requirement (150 μg/day). Therefore, it is necessary to create alternatives to increase the levels of iodine in the diet, such as, for example, the fortification of crops and also of foods, the use of iodized salt in the preparation of food or, if necessary, through the supplementation of this more susceptible population. There is a wide range of therapeutic approaches to laryngotracheal stenosis (LTS). In this study, all the patients that underwent LTS surgical correction in the ENT department of IPO Lisboa between 2000 and 2020 were identified. Variable studied included patient demographics, etiology of the laryngotracheal pathology, LTS location, previous procedures, type of procedure, post-surgical results and complications and follow-up duration. From 53 patients that were included, mean age 50 years and identical gender distribution, 49,1% have had previous oro-tracheal intubation. First surgery was realized by an external approach in 77,4% of the patients. Tese de Doutoramento em Ciências Veterinárias na Especialidade de Ciências Biológicas e Biomédicas, área científica - Sanidade Animal ABSTRACT - MOB1 is a conserved protein that regulates cellular proliferation versus apoptosis, centrosome duplication and cellular differentiation in multicellular eukaryotes and also cytokinesis and division axis orientation in unicellular and multicellular eukaryotes. Toxoplasma gondii, an obligate intracellular parasite of veterinary and medical importance, presents one MOB1 protein. T. gondii interconverts between several cellular stages during its life cycle, namely between fast replicating tachyzoite and slow replicating bradyzoite stages during its asexual cycle, a key ability for its success as a parasite. Bradyzoites produce tissue cysts, establishing a chronic infection that enables recrudescence. Conversion is dependent on cell cycle regulation and involves cell differentiation and regulation of replication. This led us to select MOB1 as a strong candidate to be involved in the Toxoplasma replication process. We employed reverse genetics to assess the Mob1 function in T. gondii. In opposition to what was observed in other unicellular eukaryotes, as Tetrahymena and Trypanosoma, Mob1 knockout in T. gondii showed no cytokinesis impairment in its asexual cycle. Instead, we observed an increase in replication, a decrease in parasitophorous vacuole regularity and a significant loss in tachyzoite to bradyzoite conversion. Additionally, recombinant MOB1 accumulates in a midline between the daughter nuclei at the end of mitosis, suggesting MOB1 may be involved in this process. To elucidate how MOB1 acts in T. gondii, we employed a proximity biotinylation method and identified the MOB1 interactome. This analysis detected proteins related to several functional categories, indicating a multivalent role for MOB1 regulated by the ubiquitin proteasome system. We also verified that the Mob1 locus is transcribed from both genomic strands and gives rise to alternatively spliced variants. Our results indicate that MOB1 is tightly regulated along the cell cycle and along the life cycle of T. gondii, contributing to the control of replication and tachyzoite-bradyzoite differentiation. Historicamente, as proteínas MOB foram descritas como ativadoras de cinases que participam nas vias de sinalização mitotic exit network/ septation initiation network (MEN/SIN) que têm papéis centrais na regulação da citocinese, polaridade celular, proliferação celular e destino celular para controlar o crescimento e a regeneração dos órgãos. Toxoplasma gondii é um parasita intracelular obrigatório de grande importância veterinária e médica, afetando uma grande variedade de espécies animais e estando presente por todo o mundo. A sua distribuição e o facto de poder parasitar potencialmente todos os animais de sangue quente contribuíram para ser considerado o parasita de maior sucesso. A Metrologia da Alimentação e Nutrição é uma área recente da Metrologia que, apesar de global, foi impulsionada na União Europeia (UE) em virtude de cada país ter a sua autoridade alimentar e ser necessária uma política europeia que congregue estas vontades. Partindo desta premissa, e dada a singularidade do processo metrológico da alimentação e nutrição, foi criada a METROFOOD-RI – Infraestrutura para a Promoção da Metrologia em Alimentação e Nutrição financiada pela UE, através do programa-quadro de investigação e inovação da Horizonte 2020. Neste artigo é feita uma breve descrição desta infraestrutura, sendo apresentada uma panorâmica da sua estrutura funcional composta por três pilares: metrológico, alimentar e digital. São também descritos os diferentes níveis de conhecimento para as categorias de componentes nos alimentos como consumidos. No âmbito da prevenção do ambiente obesogénico é feita uma breve introdução à importância da rastreabilidade das medições em estudos de obesidade, exemplificando com a aplicação da Metrologia à exatidão das medições exigida aos profissionais de nutrição e alimentação. Estas medições podem estar envolvidas em sistemas de medições, quer antropométricos quer de avaliação nutricional, nomeadamente da quantidade dos alimentos, cruciais na estimativa do aporte nutricional ou da exposição a contaminantes. The Metrology of Food and Nutrition is a recent area of Metrology that, despite being global, was promoted in the European Union because each country has its food authority, and a European policy that brings these wills together is necessary. Based on this premise, and given the uniqueness of the metrological process of food and nutrition, the METROFOOD-RI infrastructure was created - Infrastructure for the Promotion of Metrology in Food and Nutrition financed by the European Union Horizon 2020 research and innovation programme. This article provides a brief description of this infrastructure and presents an overview of its functional structure composed of three pillars: metrological, food and digital. The different levels of knowledge for the categories of components in foods as consumed are also described. Within the scope of preventing the obesogenic environment, a brief introduction is made to the importance of traceability of measurements in obesity studies, exemplifying with the application of Metrology to the accuracy of measurements required of nutrition and food professionals. Objectives: To describe the results of subannullar tube placement in pediatric patients with pars tensa retraction. Study design: Observational transversal retrospective study. Materials and methods: Clinical records of patients submitted to subannullar tube placement between 2008 and 2019 were evaluated for medical history and clinical and audiological outcomes. Results: Fifteen patients (18 ears) were included, with an average age of 9.4 and an average follow-up of 47 months. The most common comorbidities were rhinitis (47%) and palatine cleft (29%). All patients presented pars tensa retraction and 56% had middle ear effusion. Some patients were simultaneously submitted to mastoidectomy with tympanoplasty (50%), tympanoplasty alone (39%) and/or adenoidectomy (33%). Tube extrusion occurred in 44% of ears, on average after 42 months. Tube obstruction (39%), otorrhea (17%) and residual perforation (17%) were the leading complications. Pars tensa retraction resolved in 94%. Air conduction pure tone average improved 8.2dB. Conclusions: Subannullar tubes appear to be a viable alternative for middle ear ventilation. Objetivos: Caracterizar população e resultados da colocação de tubos subanulares (TSA) em doentes pediátricos com retrações da pars tensa. Desenho do estudo: Estudo observacional transversal retrospetivo Material e métodos: Análise do processo clínico de doentes que colocaram TSA entre 2008 e 2019. Avaliaram-se antecedentes, resultados clínicos e audiométricos. Resultados: Incluíram-se 15 doentes (18 ouvidos), com idade média de 9,4 anos (DP=2,7). As comorbilidades mais frequentes foram rinite (47%) e fenda palatina (29%). O tempo médio de seguimento foi de 47 meses. Observou-se retração da pars tensa em todos e efusão em 56%. Simultaneamente, realizou-se mastoidectomia com timpanoplastia de reforço (50%), timpanoplastia de reforço (39%) e/ou adenoidectomia (33%). Das complicações destacam-se: obstrução (39%), otorreia (17%), perfuração timpânica (17%). Os tubos exteriorizaram-se em 44% dos casos, em média após 42 meses. A retração da pars tensa resolveu em 94%. O limiar tonal médio da via aérea melhorou 8,2dB. Conclusões: Os TSA são uma alternativa viável na ventilação de longa duração.

A cavala é uma das espécies de peixe mais abundantes em Portugal, sendo muito rica em diversas vitaminas e micronutrientes. Entre os micronutrientes existentes neste alimento destaca-se o selénio (Se), essencial para o ser humano, mas que se pode tornar tóxico quando ingerido acima da dose diária recomendada. Sendo parte integrante de um conjunto de proteínas com ação antioxidante (as selenoproteínas) um aporte adequado de Se conduz a um melhor desempenho do sistema imunitário, com repercussões positivas no tratamento e/ou prevenção do cancro ou no retardar do desenvolvimento de doenças degenerativas, como o Alzheimer. O organismo humano não sintetiza o selénio sendo a alimentação a fonte que mais contribui para o aporte diário deste elemento. Neste estudo, pretendeu- se estudar o teor de selénio, e de outros micronutrientes, em cavalas da costa portuguesa. Esta análise foi realizada mensalmente, entre março de 2019 e fevereiro de 2020, sendo o Se determinado por espectrometria de massa acoplada a plasma indutivo (ICP-MS), e os restantes micronutrientes (Zn, Fe, Mg, Cu, K, P) por espectrometria de emissão ótica por plasma acoplado indutivamente (ICP-OES). Concluiu-se que a ordem de concentração dos elementos estudados foi estabelecida como K> P> Na> Mg> Fe> Zn. Relativamente à concentração de Se, e por utilização do teste LSD, foram identificadas algumas diferenças significativas ao longo do ano, variando esta entre 451 e 606 μg/ml, excluindo o mês de maio que foi considerado um outlier. Contudo, este resultado poderá dever-se aos hábitos alimentares da cavala e época de desova, pelo que serão necessários mais anos de estudo, de modo, a se verificarem estas conclusões preliminares. Mackerel is one of Portugal's most abundant fish species, being very rich in several vitamins and micronutrients. Among the micronutrients in this food, selenium (Se) stands out, essential for humans, but can become toxic when ingested above the recommended daily dose. As an integral part of a set of proteins with antioxidants action (selenoproteins), an adequate intake of Se leads to a better performance of the immune system, with positive repercussions in the treatment and/or prevention of cancer or in delaying the development of degenerative diseases, such as Alzheimer's. The human organism does not synthesize selenium, so the source that most contributes to this element's daily supply is food. This study intended to evaluate the content of selenium and other micronutrients in mackerel from the Portuguese coast. This analysis was carried out monthly, between March 2019 and February 2020, with the Se being determined by mass spectrometry coupled to inductive plasma (ICP-MS), and the remaining micronutrients (Zn, Fe, Mg, Cu, K, P) by inductively coupled plasma optical emission spectrometry (ICP-OES). It was concluded that the order of concentration of the studied elements was established as K> P> Na> Mg> Fe> Zn. Regarding Se content, by the LSD test, some significant differences were identified throughout the year, varying between 451 and 606 μg / ml, excluding May, which was considered an outlier. However, this result may be due to the mackerel's eating habits and spawning season. More years have to be evaluated to verify these preliminary conclusions. Dissertação de Mestrado em Arquitetura com a especialização em Arquitetura apresentada na Faculdade de Arquitetura da Universidade de Lisboa para obtenção do grau de Mestre. A presente proposta espelha, na sua génese, uma inquietação referente à (inexistente) qualidade de vida das classes sociais mais baixas, vítimas da sociedade capitalista neo-liberal. Este sistema político e económico explora simultaneamente o ser humano e o ambiente natural, extraindo o que lhe convém para a obtenção do maior lucro. No bairro do 2ºTorrão, na Trafaria, Portugal, o elevado grau de habitabilidade precária e o sentimento de segregação sócio-espacial são constantes. Invoca-se o direito a habitar dignamente e comunitariamente os espaços doméstico e público. Nesse sentido, trabalhámos o re-alojamento da população do 2ºTorrão para a proximidade do actual bairro, consolidando a zona a oeste do núcleo histórico da Trafaria. Novas casas e um equipamento escolar são propostos – ambos instrumentos de integração social – re-afirmando a ideia de empoderamento colectivo. Conceitos de flexibilidade e habitação evolutiva são introduzidos, permitindo estratégias mais adequadas às necessidades e possibilidades da população. A importância das áreas naturais é enfatizada, com a introdução de novos jardins, pomares e hortas comunitárias. Re-pensando o impacto da arquitectura e construção no meio ambiente natural, apostámos numa estrutura leve de madeira (light wood framing) portuguesa. A madeira é proveniente de cultivo sustentável, consequentemente reduzindo a pegada ecológica. Em suma, a proposta versa sobre duas premissas: a social e a ecológica; entendendo que pertencemos todos à mesma casa, à mesma comunidade: o nosso planeta. ABSTRACT : The genesis of the current proposal concerns the poor quality of life in low-income communities, victims of the neo-liberalist, capitalist society. This political and economic system exploits both the people and the natural environment, extracting resources; with the sole aim of maximizing profit. In the neighborhood of 2ºTorrão, in Trafaria, Portugal, the poor housing conditions and the feeling of social-spatial segregation are common. We invoke the right for the residents to inhabit the domestic and public spaces communally and with dignity. The project centers on the resettlement of the residents of 2ºTorrão, proximal to their current location by consolidating the west zone of Trafaria’s historical center. New houses and a school are proposed – both instruments of social integration – reaffirming the idea of collective empowerment. Concepts of flexibility and incremental housing are introduced, providing suitable strategies and possibilities to the evolving local population. The importance of natural areas is emphasized, with the introduction of new gardens – both contemplative and edible. Rethinking the physical impact of architecture and construction on the natural environment, we selected a lightweight portuguese wood framing for the structures. The wood is sustainably harvested, thus minimizing the ecological footprint. In conclusion, our proposal addresses two aspects of life: the social and the ecological; understanding that ultimately, we all belong to the same house, the same community: our planet. N/A

Neuropsychiatric symptoms, notably as they relate to mood, are frequent in obese subjects. Inflammation is believed to contribute to this effect. Obesogenic dietary patterns are not only an essential risk factor for obesity, but they also harbor a pro-inflammatory potential that may set the stage for the development of neuropsychiatric symptoms. They could thus contribute to an increased neuropsychiatric vulnerability when subjects are submitted to conditions that can promote mood symptoms. Nevertheless, being vulnerable may not be sufficient, and some mechanistic pathways need to be involved/activated to precipitate vulnerable individuals into clinically relevant neuropsychiatric states. In this context, tryptophan (TRP), which is at the interface of diet, inflammation, and mood, presents as a good candidate. In support of this notion, inflammation has a strong influence on TRP metabolism, and there is mounting evidence for the role of inflammation-related alterations in TRP metabolism in the pathophysiology of depression in subjects with high levels of inflammation. This thesis aimed to evaluate the relationship between systemic inflammation, alterations in tryptophan metabolism, and neuropsychiatric symptoms in subjects with obesity and determine whether dietary habits with high obesogenic and inflammatory potential may prime healthy subjects to neuropsychiatric alterations. Chapter 1 aimed to characterize neuropsychiatric symptoms in obesity and determine the influence of inflammation and obesogenic dietary habits on this association. Results indicate that obesity is characterized by an increased prevalence of inter-related neuropsychiatric symptoms and low-grade systemic inflammation augmenting with adiposity. Moreover, they support the notion that obesity-related chronic low-grade inflammation contributes to neuropsychiatric symptoms and comorbidity. Finally, they reveal that obesogenic dietary habits promote the development of systemic inflammation and modulate stress-induced cognitive alterations before the instauration of obesity. Chapter 2 aimed to investigate pathways of TRP metabolism in obese subjects and non-obese subjects with obesogenic dietary habits and assess their association with systemic inflammation. Results indicate that the different pathways of TRP metabolism are altered in obesity, particularly in the form of high kynurenine (KYN)/TRP ratio and iv low serotonin and indoles levels, compared to non-obese controls. Alterations in the KYN and indole pathways of TRP metabolism were associated with obesity-related systemic inflammation. In contrast, TRP metabolism in the indole pathway appeared to be impacted before the onset of obesity, as the adherence to an obesogenic dietary pattern was sufficient to decrease levels of indole metabolites. Chapter 3 aimed to evaluate the relevance of alterations in both KYN and indole pathways of TRP metabolism in neuropsychiatric symptoms in obesity, focusing on subsyndromal depression. Results show that inflammation-related changes in TRP metabolism, notably characterized by an increased neurotoxic output of the KYN pathway and low indole-3-carboxaldehyde levels, are associated with subsyndromal depressive symptoms in obese subjects. These results open different avenues of research and provide support for the development of personalized prevention and treatment strategies of depression based on the clinical, dietary, and biological profiles of patients., L’obésité est associée à une prévalence élevée de symptômes neuropsychiatriques, notamment émotionnels et cognitifs. Des données récentes suggèrent l’implication de mécanismes immunitaires. L’obésité se caractérise en effet par un état d’inflammation chronique à bas bruit avec production soutenue de facteurs inflammatoires au niveau du tissu adipeux. L’obésité résulte de l’intrication de plusieurs facteurs, notamment alimentaires. Une alimentation obésogène, riche en gras, sucres et sel, favorise l'activation de processus immuno-inflammatoires, et peut accroître la vulnérabilité à des troubles neuropsychiatriques. Néanmoins, être vulnérable peut ne pas être suffisant. Il important que certains mécanismes se mettent en place pour précipiter des individus vulnérables dans un état pathologique ou significatif/manifeste d’un point de vue clinique. Dans ce contexte, des altérations du métabolisme du tryptophane (TRP) apparaissent constituer un mécanisme pertinent. À l'appui de cette notion, l'inflammation est associée à des perturbations du métabolisme du TRP, et un grand nombre de données de la littérature suggèrent une possible implication de ces altérations dans la physiopathologie de la dépression survenant dans des conditions inflammatoires. L’objectif principal de cette thèse est d’évaluer la relation entre inflammation chronique de bas grade, altérations du métabolisme du TRP, et symptômes neuropsychiatriques chez le sujet obèse et de déterminer si les habitudes alimentaires à fort potentiel obésogène et inflammatoire confèrent à des sujets sains une vulnérabilité neuropsychiatrique accrue. Ce travail s’articule en 3 chapitres correspondant à 3 sous-objectifs précis : 1) caractériser les symptômes neuropsychiatriques chez des sujets obèses et déterminer le rôle de l’inflammation et des régimes alimentaires obésogènes dans cette relation; 2) évaluer les relations entre l'inflammation systémique et métabolisme du TRP chez des sujets obèses et non-obèses mais présentant des habitudes obésogènes; 3) évaluer la pertinence de ces processus dans la symptomatologie dépressive des sujets obèses. ii Les résultats du chapitre 1 indiquent que l'obésité est caractérisée par une prévalence accrue de symptômes neuropsychiatriques (dépressifs, cognitifs, fatigue) interdépendants ainsi que par une inflammation chronique de bas niveau qui augmente graduellement avec les classes de l'IMC. Cette relation entre le degré d'adiposité, l'inflammation systémique et l'intensité des altérations neuropsychiatriques est linéaire et n’est pas modulée par les caractéristiques métaboliques de l’individu. Enfin, les résultats exposés dans le chapitre 1 indiquent que les régimes alimentaires obésogènes, avant l’instauration d’un état d'obésité, sont associés à une inflammation systémique à bas bruit et à une altération des performances cognitives lors de l'exposition à un stress psychologique aigu. Les résultats du chapitre 2 montrent une altération des différentes voies du métabolisme du TRP dans l'obésité. En particulier, les sujets obèses présentaient un rapport kynurénine (KYN)/TRP augmenté ainsi que des niveaux de sérotonine et d'indoles diminués, comparativement aux témoins non obèses. De façon intéressante, les altérations des voies de la KYN et des indoles étaient associées à l’inflammation systémique chez le sujet obèse. En revanche, seule la voie des indoles était altérée chez les sujets présentant habitudes alimentaires obésogènes, suggérant que l'adhésion à ce type de régime alimentaire obésogène est suffisante pour perturber cette voie. Les résultats exposés dans le chapitre 3 de cette thèse indiquent que l’activité d’IDO dans l’obésité est plus spécifiquement associée à l’activation de la branche neurotoxique de la voie de la KYN. De façon intéressante, l’activation de cette voie ainsi que des niveaux d’indole-3-carboxaldéhyde étaient significativement associés à la symptomatologie dépressive « sous-syndromique » des individus obèses.
