Abstract

Retinal diseases are characterized by progressive damage to retinal cells, leading to irreversible vision loss. Among these, glaucoma stands out as a multifactorial neurodegenerative disease involving elevated intraocular pressure, retinal ganglion cell apoptosis, and optic nerve damage, ultimately resulting in blindness in both humans and dogs. Stem cell-based therapies have emerged as a promising therapeutic option for such conditions due to their regenerative and neuroprotective potential. These therapies, particularly those based on mesenchymal stem cells, offer the potential to repair and protect retinal tissues through the bioactive molecules (growth factors, cytokines, chemokines) secreted, their secretome. However, research in this field, especially on the use of umbilical cord mesenchymal stem cells' secretome, remains sparse. Most clinical trials focus on human glaucomatous patients, leaving a significant gap in veterinary patients' application, especially in dogs, with additional research being needed to determine its usefulness in canine glaucoma treatment. Abstract

Objective To determine the incidence and risk factors of first cardiovascular event (CVE) in patients with chronic inflammatory rheumatic diseases (CIRD). Methods Analysis of data after 2.5 years of follow-up from the prospective study CARMA project, that includes patients with CIRD [rheumatoid arthritis (RA), ankylosing spondylitis (AS), and psoriatic arthritis (PsA)] and matched individuals without CIRD from 67 hospitals in Spain. CVE cumulative incidence per 1000 patients was calculated after 2.5 years from the start of the project. Weibull proportional hazard model was used to calculate hazard ratio (HR) and 95% confidence interval (95% CI) of the risk factors. Results 2595 (89.1%) patients completed the 2.5 years of follow-up visit. Cumulative incidence of CVE in patients with CIRD was 15.30 cases per 1000 patients (95% CI: 12.93-17.67), being higher in the subgroup with AS; 22.03 (95% CI: 11.01-33.04). Patients with AS (HR: 4.11; 95% CI: 1.07-15.79), those with older age (HR: 1.09; 95% CI: 1.05-1.13), systolic hypertension (HR: 1.02; 95% CI: 1.00-1.04) and long duration of the disease (HR: 1.07; 95% CI: 1.03-1.12) were at higher risk of first CVE during the 2.5 years of follow-up. In contrast, female gender was a protective factor (HR: 0.43; 95% CI: 0.18-1.00). Conclusion Among CIRD patients prospectively followed-up at rheumatology outpatient clinics, those with AS show higher risk of first CVE. Abstract

To evaluate the impact of comorbidities on physical function in patients with ankylosing spondylitis (AS) and psoriatic arthritis (PsA). This was a cross‐sectional analysis of the baseline visit from the Cardiovascular in Rheumatology study. Multivariate models with physical function as the dependent variable (Bath Ankylosing Spondylitis Functional Index and Health Assessment Questionnaire for AS and PsA, respectively) were performed. Independent variables were a proxy for the Charlson Comorbidity Index (CCIp; range 0–27), sociodemographic data, disease activity (erythrocyte sedimentation rate [ESR] and Bath Ankylosing Spondylitis Disease Activity Index [BASDAI] in AS; Disease Activity Score in 28 joints [DAS28] using the ESR in PsA), disease duration, radiographic damage, and treatments. Results were reported as beta coefficients, 95% confidence intervals (95% CIs), and Pvalues. We included 738 patients with AS and 721 with PsA; 21% of patients had >1 comorbidity. Comorbidity burden (CCIp) was independently associated with worse adjusted physical function in patients with PsA (β = 0.11). Also, female sex (β = 0.14), disease duration (β = 0.01), disease activity (DAS28‐ESR; β = 0.19), and the use of nonsteroidal antiinflammatory drugs (β = 0.09), glucocorticoids (β = 0.11), and biologics (β = 0.15) were associated with worse function in patients with PsA. A higher education level was associated with less disability (β = –0.14). In patients with AS, age (β = 0.03), disease activity (BASDAI; β = 0.81), radiographic damage (β = 0.61), and the use of biologics (β = 0.51) were independently associated with worse function on multivariate analyses, but CCIp was not. The presence of comorbidities in patients with PsA is independently associated with worse physical function. The detection and control of the comorbidities may yield an integral management of the disease.

Abstract

Articles in International Journals A case of congenital lacrimal cyst or dacryops of the lacrimal gland in an 8-month-old Neapolitan Mastiff dog is reported. The dog presented with a swelling dorsolateral to the left globe, which had been present since birth. In addition, hyperplasia and prolapse of the superficial gland of the left nictitating membrane, and bilateral macropalpebral fissure and ‘diamond eye’ conformation were apparent. On manual eversion of the upper eyelid, a subconjunctival mass was visible that was translucent and pink and affected the upper conjunctival fornix. B-mode ultrasonography revealed the presence of an echolucent thin-walled cystic structure measuring 15 by 12 mm and containing an echodense border and a distended tubular fluid-filled structure that extended posteriorly. A viscous and transparent fluid was aspirated from the lesion. Surgery was performed to excise the lesion, reposition the nictitans gland, and correct the morphology of the palpebral fissure. Histopathology confirmed the mass to be a cyst and distended duct of the lacrimal gland. Although tear secretion was compromised, resection of the cyst was curative.

© Springer-Verlag 2009, Background In order to understand the role of ocular blood flow in normal and pathological conditions, knowledge of the pharmacological control mechanisms involved in the ocular vascular bed is essential. The present study was designed to investigate the reactivity of the rabbit external ophthalmic artery and its collaterals to amlodipine, in order to answer two questions: (1) What are amlodipine effects upon perfusion pressure and spontaneous oscillations in the in situ perfused rabbit eyes? (2) Can intraarterial amlodipine counteract ET-1 induced vasoconstriction? Methods Rabbit external ophthalmic arteries (n=12) in a head-mounted preparation were cannulated and perfused with warmed tyrode. Vasomotor response curves to intraarterial injections of amlodipine 3 mg/ml followed by phenylephrine 250 μg (group A, n=6) and to amlodipine 3 mg/ml after an intraarterial injection of endothelin-1 (ET-1) 27 μg/ml (group B, n=6) were obtained. For statistical analysis, the paired t-test and Fourier analysis of frequency spectrums of spontaneous oscillations were used. Results Before any drug administration, spontaneous oscillations were observed in the 12 rabbit models. In group A, amlodipine elicited vasodilation and a decrease in frequency and amplitude of the oscillations. In group B, ET-1 induced an increase in vasoconstrictor tone and vasomotion became more evident.With amlodipine after ET-1, we obtained vasodilation and abolition of the vasospasm. Conclusions Our study has two main conclusions: (1) amlodipine, an L-type calcium channel blocker, caused intense vasodilation and decreased both frequency and amplitude of the spontaneous oscillations observed in the rabbit external ophthalmic artery and its collaterals, and (2) when we applied amlodipine in arteries previously contracted by the administration of ET-1, vascular resistance greatly decreased and spontaneous oscillations were abolished. Dissertação de Doutoramento em Ciências Veterinárias A circulação ocular compreende dois compartimentos: o retiniano possui receptores autonómicos e é autoregulado, enquanto que o coroideu possui inervação adrenérgica e existe controvérsia acerca da presença de autoregulação. Nesta tese criámos e optimizámos um modelo experimental de olho isolado de coelho para ajudar a clarificar os mecanismos de controlo locais da circulação ocular independentemente de outras interferências presentes num modelo in vivo. Surpreendentemente, antes da administração de qualquer fármaco, observámos respostas de vasomotricidade intrínseca em todos os modelos experimentais. Validámos o modelo utilizando fármacos adrenérgicos de efeitos conhecidos. A injecção de fenilefrina (α1-adrenérgico) desencadeou uma resposta vasoconstritora e diminuiu a frequência e aumentou a amplitude das oscilações enquanto que a prazozina (α1-bloqueador), a fentolamina (α1,2 -bloqueador) e o labetalol, (αβ-bloqueador) provocaram vasodilatação e diminuíram a intensidade das oscilações de vasomotricidade intrínseca. Analisámos as variações de pressão decorrentes da injecção intraarterial de outros neurotransmissores: a Endotelina-1, o L-NAME (antagonista do Óxido Nítrico) e o Neuropéptido Y induziram uma resposta vasoconstritora e aumento da frequência e amplitude das oscilações enquanto que a L-Arginina (precursor do Óxido Nítrico), o Péptido Vasointestinal e a amlodipina (bloqueador dos canais de cálcio) desencadearam os efeitos opostos. Estes resultados abrem novas perspectivas para a manipulação terapêutica das doenças oculares isquémicas como sejam a retinopatia diabética e o glaucoma ABSTRACT: Ocular circulation has two compartments: the retina has autonomic receptors and is also autoregulated, while the choroid has adrenergic innervation but there is still some controversy about the presence of autoregulation. In the current investigation we developed an experimental model of isolated rabbit eye to help clarifying the local mechanisms of control of ocular circulation, independently of other interferences present in an “in vivo” model. Surprisingly, before any drug administration, we observed intrinsic vasomotricity in every animal model. We validated the model using adrenergic drugs of known effects. The injection of phenylefrine (α1-adrenergic) elicited vasoconstriction and decreased the frequency but increased the amplitude of the oscillations while prazozin (α1-blocker), phentolamine (α1,2 -blocker) and labetalol, (αβ-blocker) evoked vasodilation and a decrease in both the frequency and amplitude of the oscillations of intrinsic vasomotricity. We analysed the perfusion pressure changes following the intraarterial injection of other neurotransmitters: Endotelin-1, L-NAME (Nitric Oxide antagonist) and Neuropeptide Y elicited vasoconstrition and increased the frequency and amplitude of the oscillations while L-Arginine (Nitric Oxide precursor), Vasointestinal Peptide and amlodipine (calcium channel blocker) evoked the opposite effects. La Licenciatura de Medicina explora ante el reto del Espacio Europeo de Educación Superior (EEES) y la estructuración de los nuevos planes de estudios, el desarrollo de programas curriculares innovadores y la aplicación de nuevas metodologías que sin romper de forma brusca con el sistema tradicional, cumpla con la nuevas y crecientes demandas de la sociedad actual. Este intento de modular la enseñanza para reemplazar los enfoques clásicos por otros integrados y conseguir el inicio de la convergencia hacia un sistema que permita la lectura común de los conocimientos adquiridos ha sido el objetivo desarrollado en nuestra experiencia utilizando una metodología activa para el aprendizaje. Los participantes en este proyecto desde el Área que les compete, la Anatomía, entienden que enseñanza integrada, puede solucionar los problemas de la educación ocasionados por el aumento de los conocimientos científicos y debe desarrollarse mediante programas de estudios basados en la síntesis interdisciplinaria y en la generalización de los conocimientos. Desde los objetivos a alcanzar por las diferentes disciplinas implicadas en el estudio de un caso clínico, hemos desarrollado una estrategia metodológica basada la construcción del conocimiento por parte de Grupos reducidos de alumnos que han sido tutorizados de forma continúa por Profesores de diferentes disciplinas. El resultado ha sido la integrando de conocimientos procedentes no sólo de las clásicas Áreas afines de Fisiología y Anatomía sino de aportaciones desde las áreas de Histológica, Genética y Bioquímica que han permitido lograr nuestro objetivo: desarrollar un sistema cooperativo y activo en el que el alumno se convierte en el centro del proceso de enseñanza-aprendizaje para si mismo y para sus compañeros. Abstract

El progreso en investigación científica de los países más avanzados está logrando recientemente un cambio en la sensibilidad social hacia los temas relativos a la cultura científica. Con la finalidad de favorecer y potenciar este acercamiento, España celebró el pasado mes de Noviembre de 2001 la primera Semana de la Ciencia y la Tecnología. De las 800 actividades que se celebraron a nivel nacional durante estos días, más de 80 estuvieron relacionadas con la Ecología y el Medio Ambiente.
