Abstract

The rare patients who are able to spontaneously control HIV replication in the absence of therapy show signs of a particularly efficient cellular immune response. To identify the molecular determinants that underlie this response, we characterized the T cell receptor (TCR) repertoire directed at Gag293, the most immunoprevalent CD4 epitope in the HIV-1 capsid. HIV controllers from the ANRS CODEX cohort showed a highly skewed TCR repertoire that was characterized by a predominance of TRAV24 and TRBV2 variable genes, shared CDR3 motifs, and a high frequency of public clonotypes. The most prevalent public clonotypes generated TCRs with affinities at the higher end of values reported for naturally occurring TCRs. The high-affinity Gag293-specific TCRs were cross-restricted by up to 5 distinct HLA-DR alleles, accounting for the expression of these TCRs in HIV controllers of diverse genetic backgrounds. Transfer of these TCRs to healthy donor [CD4.sup.+] T cells conferred high antigen sensitivity and polyfunctionality, thus recapitulating key features of the controller CD4 response. Transfer of a high-affinity Gag293-specific TCR also redirected [CD8.sup.+] T cells to target HIV-1 capsid via nonconventional MHC II restriction. Together, these findings indicate that TCR clonotypes with superior functions are associated with HIV control. Abstract

La loi relative à la bioéthique est sans conteste particulière. Par son objet, car le développement des biotechnologies appliquées à l’humain engage les sciences « dures », mais convoque également les sciences humaines et sociales dans une approche tant individuelle que populationnelle. La bioéthique invite chacun à réfléchir à l’équilibre entre les possibilités d’applications de la science, en constante évolution, et le respect de nos valeurs intangibles, au premier rang desquelles la sauvegarde de la dignité humaine. Particulière aussi par sa procédure préparatoire : les États généraux de la bioéthique en 2018 ont permis une vaste consultation des Français destinée à recueillir l’opinion de la cité sur les différents thèmes abordés par la loi. Cet ouvrage, au travers de 10 questions, analyse les principales dispositions de la loi du 2 août 2021, en s’attardant sur la raison d’être des changements attendus par certains, redoutés par d’autres. Il offre ainsi une vue synthétique et analytique du nouveau cadre légal relatif à la bioéthique qui concerne les professionnels de santé, juristes, associations de patients et, au-delà, impacte la vie des citoyens.

Abstract

HIV infection results in [CD4.sup.+] T cell deficiency, but efficient combination antiretroviral therapy (c-ART) restores T cells and decreases morbidity and mortality. However, immune restoration by c-ART remains variable, and prolonged T cell deficiency remains in a substantial proportion of patients. In a prospective open-label phase I/IIa trial, we evaluated the safety and efficacy of administration of the T cell regulator IL-7. The trial included 13 c-ART--treated HIV-infected patients whose [CD4.sup.+] cell counts were between 100 and 400 cells/[mu]l and plasma HIV RNA levels were less than 50 copies/ml. Patients received a total of 8 subcutaneous injections of 2 different doses of recombinant human IL-7 (rhIL-7; 3 or 10 [micro]g/kg) 3 times per week over a 16-day period. rhIL-7 was well tolerated and induced a sustained increase of naive and central memory [CD4.sup.+] and [CD8.sup.+] T cells. In the highest dose group, 4 patients experienced transient increases in viral replication. However, functional assays showed that the expanded T cells responded to HIV antigen by producing IFN-[gamma] and/or IL-2. In conclusion, in lymphopenic HIV-infected patients, rhIL-7 therapy induced substantial functional and quantitative changes in T cells for 48 weeks. The additional 48-week optional treatment extension of the T-20 versus Optimized Regimen Only (TORO) studies evaluated long-term safety and efficacy of enfuvirtide (ENF) through week 96 in patients receiving ENF plus optimized background (OB) and patients switching to ENF plus OB from OB alone. Patient randomization was 2:1 to ENF plus OB (n = 663) and OB (n = 334), of which 89.7% and 89.8% were male, 89.3% and 88.6% were Caucasian, and median age was 41 and 42 years, respectively. HIV risk factors were comparable between the ENF plus OB and OB groups with the major factors being 65.2% versus 66.2% homosexual contact, 17.8% versus 19.8% heterosexual contact, 4.1% versus 4.8% bisexual contact, respectively, and 6.9% injection drug use in both groups. OB patients were allowed to switch to ENF plus OB at virologic failure before week 48 and required to switch at week 48 to continue in the study (n = 230). Efficacy and safety assessments were conducted for each group. At week 96, 55% of ENF plus OB subjects completed the study and 26.5% achieved a viral load of less than 400 copies per milliliter (17.5% achieved less than 50 copies per milliliter). Viral load and CD4 mean change from baseline was -2.1 and -1.1 [log.sub.10] HIV-1-RNA copies per milliliter and +166 and +116 CD4 cells/[mm.sup.3] for ENF plus OB and switch patients, respectively. No new ENF-related safety issues emerged in weeks 48-96. Injection site reactions led to discontinuation in 7% and 10% of ENF plus OB and switch patients, respectively. In conclusion, these data demonstrate durable efficacy and safety of ENF over 96 weeks and that early use of ENF in combination with other agents for the treatment of antiretroviral-experienced HIV-infected subjects is beneficial.

Studies were conducted to evaluate the expression of the IL-7R alpha chain and to determine the responses of CD4 and CD8 T lymphocytes to IL-7 in three groups of chronically HIV-infected patients. Results reveal that the effects of HAART were less pronounced in patients with undetectable VL but low CD4 counts.

Abstract

The analysis of pooled safety data from the pivotal phase three T-20 Versus Optimized Background Regimen Only (TORO) trials indicates that enfuvirtide is a well-tolerated antiretroviral agent with a maintained safety profile over 48 weeks in treatment-experienced HIV-1-infected adults. It is found that the rates of other adverse agents favored enfuvirtide or were generally comparable between treatments.

The casual pathways by which age and the CCR5-Delta32, CCR2-64I, and SDF-1 3'A alleles influence progression to AIDS is investigated. It is concluded that the age and the CCR5-Delta32 deletion and CCR2-64I mutation influence AIDS progression by affecting CD4 and HIV-1 RNA and the SDF-1 3'A allele increases the AIDS risk, but this effect is countered by its effect on CD4 and HIV-1 RNA level.

Abstract

Recommended antiretroviral regimens include a nucleoside reverse transcriptase inhibitor (NRTI) component and this class of antiretrovirals has problems such as class cross-resistance and mitochondrial toxicity. The lopinavir/ritonavir-efavirenz combination is associated with a high rate of virologic response and should be compared with more classic NRTI-containing regimens in randomized and controlled clinical trials.
